The functional brain connectome is highly dynamic over time. However, how brain connectome dynamics evolves during the third trimester of pregnancy and is associated with later cognitive growth remains unknown. Here, we use resting-state functional Magnetic Resonance Imaging (MRI) data from 39 newborns aged 32 to 42 postmenstrual weeks to investigate the maturation process of connectome dynamics and its role in predicting neurocognitive outcomes at 2 years of age. Neonatal brain dynamics is assessed using a multilayer network model. Network dynamics decreases globally but increases in both modularity and diversity with development. Regionally, module switching decreases with development primarily in the lateral precentral gyrus, medial temporal lobe, and subcortical areas, with a higher growth rate in primary regions than in association regions. Support vector regression reveals that neonatal connectome dynamics is predictive of individual cognitive and language abilities at 2  years of age. Objective: To investigate if magnetic resonance imaging (MRI) is an accurate predictor for death or moderate-severe disability at 18-22 months of age among infants with neonatal encephalopathy in a trial of cooling initiated at 6-24 hours., Study Design: Subgroup analysis of infants ≥36 weeks of gestation with moderate-severe neonatal encephalopathy randomized at 6-24 postnatal hours to hypothermia or usual care in a multicenter trial of late hypothermia. MRI scans were performed per each center's practice and interpreted by 2 central readers using the Eunice Kennedy Shriver National Institute of Child Health and Human Development injury score (6 levels, normal to hemispheric devastation). Neurodevelopmental outcomes were assessed at 18-22 months of age., Results: Of 168 enrollees, 128 had an interpretable MRI and were seen in follow-up (n = 119) or died (n = 9). MRI findings were predominantly acute injury and did not differ by cooling treatment. At 18-22 months, death or severe disability occurred in 20.3%. No infant had moderate disability. Agreement between central readers was moderate (weighted kappa 0.56, 95% CI 0.45-0.67). The adjusted odds of death or severe disability increased 3.7-fold (95% CI 1.8-7.9) for each increment of injury score. The area under the curve for severe MRI patterns to predict death or severe disability was 0.77 and the positive and negative predictive values were 36% and 100%, respectively., Conclusions: MRI injury scores were associated with neurodevelopmental outcome at 18-22 months among infants in the Late Hypothermia Trial. Background: Magnetic resonance imaging (MRI) scores have been well validated in moderate/severe hypoxic-ischemic encephalopathy (HIE). Infants with mild HIE can have different patterns of injury, yet different scores have not been compared in this group of infants. Our objective was to compare the ability of three = MRI scores to detect abnormalities in infants with mild HIE, and infants with moderate/severe HIE were included for comparison., Methods: This is a single-center prospective cohort study of infants ≥36 weeks' gestation with HIE born at a level III neonatal intensive care unit from 2017 to 2019. All infants with HIE underwent an MRI, but only infants with moderate/severe HIE underwent therapeutic hypothermia. At least two experienced MRI readers who were unaware of all clinical variables independently assigned three scores (Barkovich, NICHD NRN, and Weeke)., Results: A total of 42 newborns with varying HIE severity underwent MRI on day five of life. In the overall cohort, abnormalities were reported in three (7%) infants using the Barkovich, in 10 (24%) using the NICHD NRN, and in 24 (57%) using the Weeke score. Agreement was excellent for each score: Barkovich score (k = 1.0), NICHD NRN (k = 0.92), and Weeke score (k = 0.9)., Conclusions: Subtle injury due to mild HIE was detected with the highest frequency using the Weeke score, while inter-rater reliability was excellent for all three scores. These findings suggest that infants with mild HIE and subtle MRI abnormalities may benefit from detailed scoring systems, which is important for studies investigating the benefit of hypothermia in mild HIE., (Copyright © 2020 Elsevier Inc. Background: Magnetic resonance imaging is a surrogate biomarker for major neurodevelopmental disabilities in survivors of perinatal hypoxic-ischemic encephalopathy because injury to the basal ganglia/thalami is highly predictive of major neuromotor and cognitive problems. Major disabilities and the appearance of neonatal magnetic resonance imaging are improved with therapeutic hypothermia. We evaluated neurodevelopmental outcomes when conventional magnetic resonance imaging showed minimal or no brain injury., Methods: Institutional review board-approved series of 62 infants (≥36 weeks; ≥1800 g; 34 boys/28 girls) cooled for hypoxic-ischemic encephalopathy between 2005 and 2011 who underwent neonatal magnetic resonance imaging and Bayley Scales of Infant and Toddler Development-III at 22 ± 7 months of age. Magnetic resonance imaging at 5-14 (mean 8) days was scored as normal (score = 0), showing focal gray or white matter injury only (score = 1), or basal ganglia/thalamic and/or watershed lesions with or without more extensive hemispheric injury (score = 2). Sensitivity, specificity, and positive and negative predictive values for magnetic resonance scores 0 and 1 and statistical interaction between magnetic resonance imaging score and age at magnetic resonance imaging were determined., Results: Magnetic resonance score = 0 was seen in 35/62 patients; 26/35 (74%) were typically developing, seven (20%) had moderate and two (6%) had severe delay. Magnetic resonance score = 1 was seen in 17/62 (27%) patients; 5/17 (29%) were normal, 11/17 (65%) had moderate delay, and 1/17 (6%) had severe neurodevelopmental delay. Of the 52 patients with magnetic resonance scores of 0 and 1, 40% were abnormal. The negative predictive value of a normal magnetic resonance imaging was 74%. For score 1, sensitivity was 95% (confidence interval 63%-83%), specificity 84% (confidence interval 70%-90%), positive predictive value 84% (confidence interval 71%-93%), and negative predictive value 74% (confidence interval 62%-82%)., Conclusions: Caution is warranted when prognosticating about neurodevelopmental status in early childhood after hypoxic ischemic encephalopathy with cooling, and longer follow-up studies are needed to determine the prognostic significance of a neonatal magnetic resonance imaging showing no or minor degrees of brain injury., (Copyright © 2014 Elsevier Inc. The cerebral metabolic rate of oxygen (CMRO2) is the rate of oxygen consumption by the brain, and is thought to be a direct index of energy homeostasis and brain health. However, in vivo measurement of CMRO2 is challenging, in particular for the neonatal population, in whom conventional radiotracer methods are not applicable because of safety concerns. In this study, we propose a method to quantify global CMRO2 in neonates based on arteriovenous differences in oxygen content, and employ separate measurements of oxygenation and cerebral blood flow (CBF) parameters. Specifically, arterial and venous oxygenation levels were determined with pulse oximetry and the novel T2 relaxation under spin tagging (TRUST) MRI, respectively. Global CBF was measured with phase contrast (PC) flow velocity MRI. The proposed method was implemented on a standard 3-T MRI scanner without the need for any exogenous tracers, and the total scan duration was less than 5 min. We demonstrated the feasibility of this method in 12 healthy neonates within an age range of 35-42 gestational weeks. CMRO2 values were successfully obtained from 10 neonates. It was found that the average CMRO2 in this age range was 38.3 ± 17.7 µmol/100 g/min and was positively correlated with age (p = 0.007; slope, 5.2 µmol/100 g/min per week), although the highest CMRO2 value in this age range was still less than half of the adult level. Test-retest studies showed a coefficient of variation of 5.8 ± 2.2% between repeated CMRO2 measurements. In addition, given the highly variable blood flow velocity within this age range, it is recommended that the TRUST labeling thickness and position should be determined on a subject-by-subject basis, and an automatic algorithm was developed for this purpose. Although this method provides a global CMRO2 measure only, the clinical significance of an energy consumption marker and the convenience of this technique may make it a useful tool in the functional assessment of the neonatal population., (Copyright © 2014 John Wiley & Sons, Ltd.)

Summary: A case of cerebellopontine angle and prepontine cistern subependymoma in a 15-year-old adolescent boy is presented with a review of the literature. Apparent diffusion coefficient values for subependymoma are reported. Differential considerations for the unusual location of this rare tumor are discussed.

Lissencephaly is a rare brain malformation characterized histologically by arrested neuronal migration such that the brain resembles that of a fetus before 23-24 weeks gestation. We studied a neonate with lissencephaly by using diffusion tensor imaging and suggest the dysplastic densely cellular layer IV is visible as a band of anisotropic diffusion. Fiber tracking showed lack of connectivity between the cortex and deep white matter and an abnormal limbic system.

Purpose: To prospectively compare two-dimensional (2D) time-of-flight cerebral magnetic resonance (MR) venography with gadolinium-enhanced three-dimensional (3D) gradient-echo cerebral MR venography in children., Materials and Methods: This investigation had investigational review board approval and was Health Insurance Portability and Accountability Act compliant; parental informed consent was obtained. Thirty-seven patients (20 boys, 17 girls) who ranged in age from 4 days to 15 years underwent 2D and 3D MR venography. Two pediatric neuroradiologists compared the visibility of the superior sagittal, straight, transverse, and sigmoid sinuses and the internal jugular veins on images obtained with the two sequences., Results: In 17 (46%) of the 37 patients, the sequences were equivalent in terms of their depiction of venous anatomy. In 19 (51%) of the 37 patients, 3D MR venography was superior to 2D MR venography. Suboptimal enhancement of veins occurred in one (3%) patient at 3D MR venography. Venous anomalies suggested at 2D MR venography but not present at 3D MR venography included flow gaps in the nondominant transverse sinuses of four patients, unilateral transverse sinus atresia in eight, and a narrowed superior sagittal sinus in two. Two-dimensional MR venography results failed to reveal a persistent falcine sinus associated with straight sinus atresia in one patient and suggested transverse sinus thrombosis in two patients in whom 3D MR venography results were normal. Additionally, the extent of dural thrombosis was overestimated at 2D MR venography in one patient. As compared with 3D MR venography, 2D MR venography failed to reveal sigmoid sinus stenosis in one patient and poorly depicted posterior fossa dural sinus anatomy in two patients with dural arteriovenous fistula., Conclusion: Three-dimensional MR venography is often superior to 2D MR venography in the delineation of major cerebral venous structures in children. Most of the artifactual loss of vascular signal seen with the use of 2D MR venography occurred in nondominant transverse sinuses., (Copyright RSNA, 2005.)

Chiari II malformations and holoprosencephaly have been considered to be brain malformations that differ with respect to teratogenic insult, embryologic mechanism, and morphology. We herein describe coexistent Chiari II malformation and holoprosencephaly that occurred in a viable infant. A review of the literature regarding Chiari II malformations and holoprosencephaly suggests that a disturbance to the mesenchyme in early embryologic life may be the cause of both malformations.

Purpose: We compared visibility of residual juvenile cerebellar pilocytic astrocytomas (JPAs) on early postoperative and follow-up MR studies to determine whether early postoperative MR imaging has a valid role as a baseline study., Methods: We reviewed the MR images of 21 consecutive children who had undergone resection of cerebellar JPA. The diagnosis of residual tumor was made on the basis of nodular enhancement that corresponded to enhancing tumor on the preoperative MR studies and/or nonenhancing nodular T2 signal that corresponded to nonenhancing tumor. Because no patient received chemotherapy or radiation therapy, abnormal T2 signal or enhancement on the early postoperative study that resolved on the follow-up study was presumed to be due to peritumoral edema and/or surgical manipulation. Nodular T2 signal and/or enhancement in the tumor bed not seen on the initial postoperative MR study but present on the subsequent MR study and unchanged on serial follow-up MR studies was presumed to represent residual tumor rather than tumor that had recurred., Results: Compared with follow-up studies, the initial postoperative MR images were true-positive for residual tumor in six patients, false-positive in five, equivocal for residual tumor in four, true-negative in five, and false-negative in one. Residual tumor did not consistently enhance, and peritumoral edema and changes resulting from surgical manipulation tended to mask or simulate residual tumor., Conclusion: Early postoperative MR imaging is not accurate in differentiating residual JPA from postoperative changes, and the role of early postoperative MR imaging as a baseline study for comparison with further studies is questionable.

Objective: To evaluate the safety and efficacy of orally administered perflubron for bowel recognition on MR imaging in a pediatric population., Materials and Methods: A multicenter trial evaluated 39 pediatric subjects before and after ingestion of perflubron with T1-, proton-density, and T2-weighted sequences through the abdomen and/or pelvis. Post-contrast images were compared with pre-contrast images. Safety was evaluated through assessment of adverse events, clinical laboratory parameters, and vital signs., Results: With regard to efficacy analysis, improvement in the percent of bowel darkened was observed for 85 % of the subjects on T1-weighted images and for 95 % of the subjects on proton-density and T2-weighted images. For images of the abdominal region, the percent of bowel darkened was improved for 90-92 % of the subjects across pulse sequences. Improvement rates for the images of the pelvic region ranged from 71 % to 100 %. For at least 75 % of the subjects, proton-density and T2-weighted images of the body and tail of the pancreas, left lobe of the liver, mesenteric fat, and pathological tissue were improved relative to predosing images. Twenty-three percent of the subjects experienced some adverse effects, most of which were minor and related to the digestive system. Clinical laboratory and vital sign evaluations revealed no trends associated with the administration of perflubron., Conclusion: Perflubron is a relatively safe and effective gastrointestinal MR contrast agent in the pediatric population.

An 11-month-old infant presented with cranial nerve palsy and ataxia. MR revealed a large, enhancing pontine mass and small, nonenhancing parafalcial lesions; no organisms were seen in cerebrospinal fluid. After empiric treatment for brain stem glioma, the patient died. Autopsy revealed meningoencephalitis caused by leptomyxid amebae.

Despite its usefulness in adults with cerebral gliomas, indications for thallium-201 single-photon emission computed tomography (SPECT) in pediatric brain tumor patients are not well defined. We prospectively compared thallium SPECT with gadolinium-enhanced MR (Gd-MR) to determine if thallium SPECT provides clinically useful information that cannot be derived from Gd-MR. We studied 24 pediatric brain tumor patients, 7 at presentation and 17 during therapy. MR imaging included T2 and pre- and postgadolinium T1 images. Thallium SPECT was done within 48 h of MR imaging; thallium indices were calculated for 12 of 14 lesions which showed thallium uptake. Surgery and/or clinical follow-up are available in all patients. The tumors included pilocytic astrocytoma (7), medulloblastoma (5), brainstem glioma or glioblastoma (4), germinoma (3), optic glioma (2), mixed glioma (1), primitive neuroectodermal tumor (1), and choroid plexus carcinoma (1). Among the primary tumors, compared to MR, thallium SPECT was false-negative for tumor in 1 patient and true-positive in 6 patients. Among the patients studied while on therapy, compared to MR, thallium SPECT was true-negative for tumor in 7, true-positive in 5, false-negative in 3, and false-positive in 2. In both groups of patients, thallium SPECT underestimated tumor burden as nonenhancing regions of the tumors were not thallium-avid. Thallium indices did not correlate with histologic grade, biologic aggressiveness, or tumor type. We were unable to establish indications for the use of thallium SPECT in this setting as there was little clinically useful information derived from thallium SPECT that was not provided by Gd-MR.

Purpose: To define the role of MR in evaluating term neonates with seizures the most common clinical manifestation of cerebral injury in neonates., Methods: Fifteen term infants with seizures underwent MR imaging. The presence and pattern of MR findings were compared with clinical markers of perinatal distress, cause of cerebral injury, and short-term neurologic outcome., Results: Seizures were caused by hypoxic-ischemic encephalopathy in three patients, bacterial meningitis in three, and prenatal cocaine exposure in one. Nine patients had no identifiable risk factors. By MR, five patients had focal ischemic injury of the cerebral hemispheres and/or basal ganglia and brain stem. Six patients had diffuse cerebral edema: of these, five had basal ganglia edema; one had brain stem edema. One patient had superior sagittal sinus thrombosis with venous infarcts. Three patients had normal MR studies. There was no correlation between markers of perinatal distress, risk factors for seizures, and presence or pattern of MR findings. There was some correlation between MR findings of diffuse cerebral injury and neurologic outcome, and between MR findings of basal ganglia and brain stem abnormalities and neurologic outcome; these findings correlated with spasticity and hemiplegia at 6 to 24 months follow-up., Conclusion: The presence or pattern of MR findings does not appear to correlate with with clinical signs of perinatal distress or presumed causes of perinatal cerebral injury. Further investigation is needed to identify prospectively neonates with seizures who are at risk for significant neurologic morbidity.

Thirty-two children with medulloblastoma were evaluated postoperatively with conventional and gadolinium-enhanced MR imaging. Eleven patients had abnormal cranial MR studies; nine of these had recurrent tumor. In six patients recurrent tumor enhanced with Gd, while in the other three patients recurrent tumor did not enhance. The remaining two patients had areas of abnormal Gd enhancement that were caused by radiation-induced breakdown of the blood-brain barrier rather than by recurrent tumor. This study shows that not all recurrent medulloblastoma enhances and that the absence of Gd enhancement does not necessarily indicate the absence of recurrent tumor.
