A significant increase of mRNA expression of thymic stromal lymphopoietin (TSLP) has been reported in the bronchial mast cells (MCs) of asthmatic subjects; however, the mechanism underlying the upregulation of TSLP mRNA and protein remains unknown. FcepsilonRI-mediated activation of human MCs upregulated TSLP mRNA expression by 5.2+/-2.9-fold, while activation of the MCs using lipopolysaccharide and polyriboinosinic:polyribocytidylic acid failed to upregulate TSLP. Stimulation of MCs with interleukin (IL)-4 alone did not affect the TSLP mRNA expression, while pre-incubation of MCs with IL-4 for 48 h significantly enhanced the FcepsilonRI-mediated TSLP mRNA expression (by 53.7+/-15.9-fold; p0.05) and the amount of TSLP in the cell pellets increased significantly from 23.4+/-4.3 pg mL(-1) to 121.5+/-3.7 pg mL(-1) (p0.0001). However, the released TSLP was rapidly degraded by proteases that were released by MCs. We identified the population of cells expressing TSLP in the lungs of 16 asthmatic and 11 control subjects by immunohistochemistry. The percentage of TSLP-positive MCs in the total population of MCs was significantly increased in asthmatic airways (p0.0001). Thus, MCs are able to store TSLP intracellularly and to produce TSLP following aggregation of FcepsilonRI in the presence of IL-4.

Malignant peripheral nerve sheath tumors(MPNSTs) of the acoustic nerve are very rare. Only seven cases of MPNST arising from the acoustic nerve have been reported. The authors present a case of MPNST with divergent cartilage and melanotic differentiation of the acoustic nerve. The patient was a 69 year old man admitted to our neurosurgical service in January 1999 complaining of left facial nerve palsy and hearing difficulty of his left ear. The initial CT showed a tumor at the left cerebellopontine angle region. On MRI the tumor was depicted as low intensity on T 1-weighted image and high intensity on T 2-weighted image, the mass was heterogeneously enhanced after administration of Gd-DTPA. The partial removal of the tumor was performed in January 1999. He was discharged February 1999. But he was admitted again because of progressive cerebellar ataxia. MRI showed the rapid regrowth of the residual tumor. In March 1999, complete removal of the tumor was performed. Histopathological analysis revealed a malignant spindle cell neoplasm with divergent cartilage and melanotic differentiation. We review the relevant literature concerning MPNST of the acoustic nerve and discuss the clinical features of malignant eighth cranial nerve tumor.

We reported an autopsy case of cerebral infarction with primary lung cancer. The patient was a 50-year-old man. Despite having been treated with warfarin potassium and ticlopidine hydrochloride, he relapsed cerebral infarction. His laboratory data on admission showed that lupus anticoagulant was positive, together with a high value of beta-thromboglobulin, thrombin-antithrombin III complex, markers of platelet and coagulation activation, CEA and CA 19-9. The autopsy finding revealed a primary papillary adenocarcinoma in the right lower lung, multiple cerebral infarction, renal infarction, pulmonary infarction and splenic infarction. The atherosclerotic changes were mild in the whole tissues and findings of vasculitis were not observed. Recurrence of cerebral infarction was effectively suppressed with the addition of steroid therapy to antithrombotic therapy. This case was considered as catastrophic antiphospholipid syndrome. It is necessary to differentiate antiphospholipid syndrome in case of the abnormal coagulation and fibrinolytic factors with recurrent cerebral infarction. Moreover, systemic examinations are important, because malignant tumor may exist on the background of the case.

Temporary vessel occlusion (TO) for aneurysmal clipping is an effective technique to facilitate dissection between aneurysm and parent vessels, and to place a permanent clip at the aneurysmal neck precisely. However, several unsolved problems remain regarding the overall safety and risk resulting from this technique. The authors examined a series of patients in whom mannitol 500 ml, tocopherol acetate 500 mg, and phenytoin 500 mg were administered intravenously as ischemic protection during TO for the aneurysmal clipping. The study comprises a nonconcurrent retrospective analysis of 144 consecutive aneurysm clippings performed with the aid of intentional TO at the Hiroshima Prefectural Hospital from 1985 to 1995. To identify technical and patient-specific risk factors for perioperative stroke, factors studied included duration, location of the temporary clip application, number of occlusive episodes, patient sex, age, and preoperative neurological status, timing of operation, as well as postoperative, temporary or permanent, neurological deficits (ND) due to TO were used. Overall frequency of postoperative ND due to TO manifested clinically and radiologically were 9.0% and 9.7%, respectively. In both univariate and multivariate analysis there were no significant factors relevant to the ND. However, duration of the temporary occlusion time over 20 minutes was the factor most influential on the ND due to TO. Duration of the temporary occlusion time was shown to have no link with outcome. Based on our findings the authors conclude that temporary vessel occlusion within 20 minutes with anti-ischemic drugs is a relatively safe adjunct to aneurysmal surgery.

Compression of the crus cerebri against the free edge of the tentorium contralateral to a supratentorial mass, the so-called Kernohan's notch, can be a cause of false localizing sign. Kernohan's notch has been thoroughly studied clinically and pathologically, but not radiographically. The authors describe a case of left chronic subdural hematoma, which resulted in left hemiparesis caused by Kernohan's notch. Injury to the contralateral cerebral peduncle was clearly shown by magnetic resonance imaging (MRI) performed in the postoperative period. A 43-year-old man was transferred to our hospital in deep coma with dilated pupils, unreactive to light. Computed tomography (CT) scans obtained on admission revealed a left chronic subdural hematoma and a midline shift to the right. After drainage and irrigation of the left chronic subdural hematoma through a single burr hole, his clinical condition improved gradually. But 1 month after the operation, mild left hemiparesis still persisted. MRI T2-weighted images demonstrated an abnormally increased signal area in the right cerebral peduncle. T1-weighted coronal images showed the anatomical relationship between the hypointense lesion in the right cerebral peduncle and tentorial edge. Three-dimensional-MRI (3D-MRI) clearly demonstrated the surface image of Kernohan's notch. We emphasize the utility of 3D-MRI for detecting evidence of brain stem injury, such as Kerno han's notch.

We describe a case of intracranial hypotension syndrome due to overdrainage of cerebrospinal fluid presented with hearing loss after ventriculoperitoneal shunting procedure. A 69-year-old man suffering from subarachnoid hemorrhage presented with an angiogram showing two aneurysms, one of the right internal carotid and one of the middle cerebral artery. Neck clipping was performed. One month later, he developed normal pressure hydrocephalus (NPH), which was treated by ventriculoperitoneal (NPH), which was treated by ventriculoperitoneal shunting procedure using low pressure Pudenz system. Trias of NPH were improved by insertion of shunt system. However, he complained of hearing loss which was worsened by upright position and improved by lying down. Such kinds of phenomenon were demonstrated by audiogram showing that the transitory decrease of hearing and electrocochleography showing the elongation of N1 latency at upright position. These data suggested that his hearing loss was caused by inner ear or auditory nerve lesion. After the shunt system was replaced into the antisiphon device, his hearing disturbance improved. Axial computed tomography of bone window at the level of orbitomeatal line demonstrated widely perilymphatic duct on both sides. This finding suggested that the fluctuation of intracranial pressure was easily transmitted into the cochlear through the widened perilymphatic duct, resulting in hearing disturbance.

The purpose of this study was to investigate the indication of direct early surgery for elderly cases aged over 70 years with ruptured intracranial aneurysms. The result of early surgical treatment in 39 elderly cases was analyzed and was compared with that of conservative treatment in 39 elderly patients. According to the multiple logistic analysis, surgery was only significant factor influenced the outcome, however, HuntKosnik grade (HKG), activity of daily life (ADL) before subarachnoid hemorrhage (SAH) and past history were related to the outcome. The outcome in the direct early surgical group statistically correlated (p0.05) with preoperative HKG, although the outcome in the conservative group did not correlate with HKG. The outcome in conservative group was miserable because of rerupture or poor preoperative neurological grade. The indication of direct early surgery for elderly cases with ruptured intracranial aneurysms should be considered as follows, HKG 1 to 3, ADL before SAH 1, no or minor past history and age under 84 since most elderly patient was 84 in our series. Direct early surgery in elderly patients seems to improve the outcome under these indications.

A 68-year-old male presented with choroid plexus carcinoma in the left lateral ventricle manifesting as dysarthria and gait disturbance. Magnetic resonance imaging showed a homogeneously enhanced mass in the trigone of the left lateral ventricle. Selective left posterior cerebral arteriography showed the tumor was fed by the left medial posterior choroidal artery. Detailed examinations found no evidence of an extraneural primary focus. He underwent partial removal of the tumor followed by local Lineac irradiation (50 Gy). After irradiation, the serum level of carcinoembryonic antigen decreased and the size of the residual tumor was reduced.

The authors report a case of dural arteriovenous malformation (dAVM) in the anterior fossa that was associated with an occlusion of the unilateral middle cerebral artery with Moyamoya phenomenon. The patient was a 79-year-old man referred to our department with an episode of consciousness disturbance and headache. Computed tomography scan showed intracerebral hematoma in the left frontal lobe, combined with left frontal subdural hematoma and subarachnoid hemorrhage. Angiography revealed dAVM in the left anterior fossa, fed by the bilateral ethmoidal and the bilateral middle meningeal arteries, and draining into the superior and inferior sinuses through the pial veins with vascular sacs. While angiography revealed an occlusion of the right middle cerebral artery (M1 portion) good collaterals via leptomeningeal anastomosis had developed. T1-weighted magnetic resonance (MR) images showed an intracerebral hematoma as a low intensity area, and flow-void areas were indicative of vascular sacs. Time-of-flight MR angiography also clearly showed dAVM in the left anterior fossa. Co-existence of an occlusion of an intracranial major artery and dAVM in this location has not been reported previously and it was speculated that this association had occurred by chance.

Supratentorial hemangioblastoma is encountered very rarely. About 80 cases in all have been reported. The authors present a case of supratentorial hemangioblastoma in the thalamus with gradually enlarging cysts. The patient was a 45-year-old man with complaints of left hemiparesis and headache. Computed tomographic scans of the brain showed a cystic mass with mural nodule in the right thalamus. Left vertebral angiography demonstrated a vascular tumor fed by a thalamogeniculate artery. A right temporo-parietal craniotomy was performed and the tumor was totally resected through the transcortical approach. The histological diagnosis was hemangioblastoma. Post-operative course was good except for a minor complication of the left lower quadrant homonymous hemianopsia and left hemiparesis which had disappeared at the time of discharge.

A case of deep-seated brain abscess that ruptured twice into the ventricle and resulted in death is presented. A 45-year-old man had experienced pyrexia and headache for 3 days before admission. On admission he was somnolent (GCS: 13) but there were no abnormal neurological findings except nuchal rigidity and Kernig's sign. Computed tomography (CT) scan showed a ring enhanced mass near the left caudate head and dilated ventricles. In comparison with CT performed at the former hospital it was diagnosed that a rupture into the ventricle of the brain abscess had occurred. Ventricular drainage was performed at once and white purulent cerebrospinal fluid was obtained. Thereafter, he was treated with some antibiotics and his conditions seemed to stabilize for a while. Serial CT images demonstrated that the size of the abscess seemed to be enlarging. Just when we planned to undertake stereotactic aspiration, the second ventricular rupture occurred and he died. According to this case, it is suggested that once a deep-seated brain abscess near the ventricular system is suspected, it should be aspirated by means of CT-guided stereotactic surgery immediately.

Two cases of traumatic intracerebral pneumocephalus, a rare complication of head trauma, are presented. Case 1: A 14-year-old boy had a strong concussion in his forehead due to a motorbike accident. Slightly obtunded on admission showing GCS 10, he became conscious in several days. Head CT performed after 17 days showed a round air image in the right frontal lobe which kept increasing in size thereafter. Bilateral frontal craniotomy was performed 31 days after the injury. A craniodural defect with a herniated brain was found in the superior wall of the posterior ethmoid sinus and repaired. Case 2: A 55-year-old man received a left forehead concussion when his motorbike ran into a car from behind. Although he had been conscious ever since admission, head CT after 15 days showed a round air image in the left frontal lobe. MRI demonstrated the air to be located in the cerebral parenchyma distinctly and the brain to have herniated into the frontal sinus. As the air showed a tendency to increase in volume and mild psychic and memory disturbances appeared, bilateral frontal craniotomy was performed 34 days after the injury. A craniodural defect with a herniated brain was detected in the posterior wall of the frontal sinus and repaired. These two patients showed a small amount of cerebrospinal fluid (CSF) rhinorrhea before the operation. Following the surgical repair, no recurrence of pneumocephalus and CSF rhinorrhea has been seen in either case. Intracerebral pneumocephalus secondary to closed head trauma was thought to have been due to herniation of contused brain into a craniodural defect.(ABSTRACT TRUNCATED AT 250 WORDS)

A case of a 13-year-old boy with a primary, intrasellar malignant germ cell tumor is reported. An immunohistochemical study revealed that the serum HCG and AFP levels were highly elevated and that the tumor contained elements of seminoma, choriocarcinoma, a yolk sac tumor, and an embryonal carcinoma. After operation, combined chemotherapy (cisplatin, vinblastine and bleomycin, i.e. PVB therapy) and irradiation were performed in hope of achieving the radiosensitizing effect of cisplatin. As a result, partial remission was obtained. Although PVB therapy is considered effective, combined radio-chemotherapy, as was done in this case, is thought to be better method for treating an intracranial, malignant germ cell tumor.

A case of a 13-year-old boy with malignant mixed germ cell tumor of parasellar origin was reported. After partial removal of the tumor, combined chemotherapy (using cisplatin, vinblastine and bleomycin: PVB therapy) and irradiation were performed, and the tumor was reduced to under one tenth. In spite of PVB maintenance chemotherapy, it became refractory later. The patient was then treated with both CDDP 20 mg/m2 daily for 5 days and etoposide 100 mg/m2 for 1, 3, 5 days. After this one course, the tumor was diminished. Oral etoposide was administered at our outpatient department, and the boy was free of disease for six months.

This study was carried out to evaluate the effects of carmofur (Mifurol), a masked compound of 5-fluorouracil (5-FU), in preventing postoperative recurrence of urogenital carcinoma (especially in bladder cancer). Carmofur was given orally at the dose of 300 approximately 600 mg/day to 134 patients (90 males and 44 females) after the resection. Out of 134 patients, 121 patients were evaluable and the rate of their recurrence was 10.7%. The rate of cumulative nonrecurrence for 65 patients it was 96.0, 73.0 and 60.8% in 1, 2 and 3 years, respectively. The group of curative resection demonstrated a higher cumulative nonrecurrence rate than that of noncurative or palliative resection group. In considering the resection methods, transurethral resection of bladder tumor group demonstrated a higher rate than partial or total cystectomy. The average concentration of 5-FU in serum was 0.65 microgram/ml at 120 minutes after oral administration of 200 mg of carmofur. Moreover, the concentration of 5-FU (15 approximately 35 micrograms/ml) in urine was higher than that of carmofur. The rate of side effects that appeared was 19.6%. The side effects were hot feeling (12.2%), pollakisuria (4.3%), anorexia (4.3%), an antabuse-like reaction by drinking (3.8%) and so on. These results were similar to those already reported.

A case of dissecting aneurysm of the cervical internal carotid artery due to rare mechanism by non-penetrating injury is described. A 45-year-old right-handed man had complaints of the right lateral neck pain during exercise of KENDO wearing a tight headneck protector. Following sudden dysarthria and left hemiplegia, he developed loss of consciousness and generalized convulsion. Five hours after admission, he became alert and had no neurological deficits. Four days after these episodes, he loss visual acuity of the right eye, and a few days later he showed left hemiplegia, hypotension, hypersomnia and right-sides Horner's syndrome. Right retrograde brachial angiography revealed so-called "string sign" in the right extracranial internal carotid artery and delayed circulation in the right cerebral hemisphere. He was diagnosed as having traumatic dissecting aneurysm due to blunt (rubbing) injury. He was treated with STA-MCA anastomosis 3 weeks after the accident. Usually, carotid dissecting aneurysm due to blunt injury is produced by hyperextension and contralateral rotation of the neck or direct blow to the neck, but our case shows a possible mechanism of rubbing injury such as simple anteroposterior flexion under tight neck fixation.

A case of renal tumor with secondary erythrocytosis and acute occlusion of the left common iliac artery is reported. The patient was a 73-year-old male who complained of right upper abdominal pain. Laboratory investigation at admission revealed erythrocytosis. Radiological examinations including excretory urography, angiography and computed tomographic (CT) scan showed that the hypervascular tumor was related to the right kidney with pulmonary metastases. Transabdominal echography revealed a tumor thrombus in the inferior vena cava. The tumor was not surgically removed, and IFN-alpha (HLBI) at a dose of 3 x 10(6) units was administered intramuscularly for 6 consecutive days every week and UFT at a dose of 3 capsules was given by oral route daily. About two months later, acute occlusion of the left common iliac artery with necrosis of the left toes occurred, and the left lower extremity was amputated. The high haematocrit resulted in an increase of blood viscosity which caused acute arterial occlusive disease. The association between renal tumor and erythrocytosis is discussed.

This is a report of two cases of the hypereosinophilic syndrome with an unusual apical filling defect suggestive of an intraventricular mass. Case 1 (45-year-old man): This patient had marked eosinophilia, congestive heart failure, and murmurs of aortic and mitral regurgitation. A subcutaneous cervical mass revealed histologic features of angiolymphoid hyperplasia with eosinophilia (so-called Kimura's disease). Left ventriculography confirmed mitral regurgitation with extensive obliteration of the left ventricular cavity. It was suspected that an extensive ventricular mural thrombus was superimposed on a thickened endocardium. Right ventricular endomyocardial biopsy showed focal regions of fibrosis in the myocardium, and infiltration of eosinophils were identified. Case 2 (70-year-old man): This patient had marked eosinophilia and congestive heart failure. Left and right angiography showed apical filling defects suggestive of intraventricular mass. Right ventricular endomyocardial biopsy showed focal areas of fibrosis, and infiltration of eosinophils were observed. Diagnosis of Löffler's endocarditis was made. It is suggested that the spectrum of ventricular abnormalities in these patients results from endomyocardial fibrosis of varying severity, and that eosinophilia may play a leading role or may associate with the genesis of the disease in these two cases.

The effectiveness and tolerance of the new semisynthetic acylureidopenicillin 6-[(R)-2-[3-methylsulfonyl-2-oxo-imidazolidine-1-carboxamido]-2-phenyl-acetamido]-penicillanic acid sodium salt (mezlocillin, Baypen) were investigated in 10 patients with bone and soft tissue infections of the mouth and jaw. The antibiotic was given in doses of 2 to 10 g/d. Indications for treatment were osteomyelitis of the jaw and/or phlegmon of the floor of the mouth and abscesses of the soft palate. 9 out of 10 infections could be cured. Due to the short treatment period one case was not assessable. In 6 patients 9 causative aerobic and anaerobic organisms were isolated which were eliminated. The clinical and laboratory control examinations did not reveal any adverse reactions.

Tracheo-innominate artery fistula is one of the severest complications associated with tracheostomy. Early recognition and prompt aggressive therapy are necessary. An 18-year-old male patient who had this complication was surgically managed with success. The pathogenesis and treatment of tracheo-innominate artery fistula are discussed.

Seven patients with renal calculi were evaluated by ultrasonography and intravenous pyelography (IVP). After surgical treatment, the diagnostic value of these examinations was considered. Correct diagnosis of renal calculus size and number had been established in 4 of the 7 patients by ultrasonography, and 5 of the 7 patients by IVP. In some cases, ultrasonography gave a more accurate diagnosis of renal stone than drip intravenous pyelography.

Estramustine phosphate disodium (Estracyt) was used in the treatment of 40 patients with prostatic carcinoma. Of these 20 patients 18 were treated with Estracyt as primary treatment and 22 had been treated with diethyl stilbestrol dephosphate and/or bilateral orchiectomy for more than 4 months before the Estracyt treatment. The drug was given orally in a dose of 560 mg/day in 2 divided oral doses. The clinical evaluation was done after 3 months treatment. The response in subjective symptoms and objective signs were documented and evaluated according to 5 criteria. In this study, Estracyt showed 80% improvement of dysuria, 60% of nykturia, 35% of pain and 55% of general condition. In objective signs, it showed 52.5% improvement of size of the prostate, 42.5% of consistency and 70% of residual urine. It would be emphasized that Estracyt had almost equal efficacy in both the primary treatment group and secondary treatment group. As side effects of this drug, gynecomastia, gastro-intestinal disturbance, angina pectoris like chest pain were observed.

Amikacin sulfate (AMK) was used against urinary tract infections (UTI) as local administration such as bladder lavage, renal pelvic lavage and vesical instillation. Forty four patients with UTI were treated by this method, 32 patients having Foley catheter indwelling in the bladder and 12 patients having drainage catheter indwelling in the renal pelvis. The overall clinical effect under UTI judgment was 3 excellent cases, 24 good, 11 fair and 6 poor, with an efficiency of 61.4%. A total of 100 bacteria, 47 gram positive, 33 gram negative, 6 anaerobes and 14 fungi, were found in the urine and bacteriological effect was 68.8% and 57.1% in single and combined bacterial infection, respectively. The serum concentration of AMK was measured in 9 patients by radioimmunoassay, and the maximum concentration was 0.38 micrograms/ml which is a low absorption rate. There were no adverse reactions during or after treatment with AMK.
