Introduzione: La fragilità dell’anziano è fisica e psichica e su di entrambe vi è influenza degli eventi stressanti della vita. Le paure che insorgono possono essere determinate da stereotipi che spesso egli tende ad accettare e che lo portano a ritenersi incapace fino ad un lento declino intellettuale. Questa ricerca ha l'obiettivo di approfondire le conoscenze sulle paure dell’anziano ricoverato e sperimentare la “presenza dell’infermiere” come possibile intervento. Materiali e Metodi: All'interno di un Reparto “Cure Intermedie” è stata svolta una ricerca qualitativa con osservazione partecipante (studio osservazionale descrittivo tipo “serie di casi”). I dati raccolti sono stati analizzati attraverso il metodo l’analisi del contenuto. Risultati: Dalle osservazioni di 13 pazienti (9 donne e 4 uomini; età media 71 anni), sono emersi principalmente contenuti positivi come “Gioia (47)”, “Attesa (35)” e “Offerta-ricordo (33)”. Il contenuto "Paura (16)” diversamente dalla bibliografia consultata è risultato presente ma non preponderante. Discussione: Dall’analisi approfondita del materiale pare emergere che gli aspetti negativi comprendenti la “Paura” sono tra loro molto collegati ma il grosso nucleo di sofferenza viene destabilizzato dagli aspetti affettivi (speranza, aspettativa, amore) che arriva ad una prospettiva positiva (gioia, piacere, gratitudine). “Ascolto” e “disponibilità” sono stati i principali interventi attuati durante l’osservazione e coerentemente a quanto descritto in letteratura hanno permesso di registrare un impatto positivo sui pazienti. Un approfondimento è stato dedicato al concetto di “Offrire-dono” inteso come atto di riconoscimento attivo da parte del paziente verso l’operatore, con le sue implicazioni simboliche e psicologiche. Nell’analisi del materiale sono anche stati presi in considerazione aspetti legati alla psicologia positiva e allo “human caring”, come l’importanza e l’effetto del sorriso, della presenza fisica, dell’empatia nella condivisione e i risvolti nell’ambito di cura. In base agli indicatori della Diagnosi Infermieristica di Paura di Lynda Juall Carpenito-Moyet, la valutazione iniziale durante la fase di accertamento degli stati di paura del paziente, permette un'assistenza basata anche sulla pratica dell'ascolto, competenza fondamentale dell'infermiere. Nei pazienti valutati si è assistito ad una riduzione dello stato di paura nelle osservazioni successive e nelle stesse, ipotizzando un effetto efficace della “presenza”.

Il beneficio protesico e il ruolo delle abilità cognitive, dell’età e del deficit uditivo nel paziente anziano.Questo studio analizza il beneficio protesico in un campione di soggetti anziani (n = 102, età media 81,1 anni), indagando il ruolo svolto dall’età, dal deficit uditivo e dalle abilità cognitive. Il beneficio protesico è stato valutato sia come miglioramento delle caratteristiche audiometriche con protesi (in termini di soglie tonali e soglie di percezione del parlato in quiete e nel rumore) sia come diminuzione della percezione della disabilità e dell’handicap dovuti alla ipoacusia. Sono state valutate diverse abilità cognitive, tra cui la memoria a breve termine, le funzioni esecutive e l’attenzione. I dati raccolti sono stati analizzati tramite un’analisi multivariata, seguita da un’analisi di correlazione e regressione per evidenziare possibili fattori latenti tra le variabili in gioco e valutare come l’età, il deficit uditivo e le abilità cognitive influenzavano il beneficio protesico. L’analisi ha rivelato un miglioramento significativo del profilo audiometrico con protesi e una diminuzione della disabilità e dell’handicap percepiti. L’analisi multivariata, insieme con quella di correlazione e di regressione ha evidenziato che i soggetti con migliori abilità cognitive e minor deficit uditivo erano significativamente caratterizzati da un più evidente miglioramento del profilo audiometrico con protesi. Inoltre, è stata rilevata una relazione significativa tra la disabilità e l’handicap percepiti, il profilo audiometrico con protesi e il grado di ipoacusia: i soggetti con miglior profilo audiometrico con protesi e minor deficit uditivo erano significativamente caratterizzati da una più evidente diminuzione dell’handicap e della disabilità percepiti. Lo studio non ha invece evidenziato alcuna relazione diretta tra le abilità cognitive e il beneficio protesico riguardante la percezione dell’handicap e della disabilità.This study describes the benefits of hearing aid (HA) use in a sample of elderly experienced HA users (n = 102, mean age 81.1 years), investigating the role of the age, hearing impairment and cognitive function. The benefit was assessed in aided condition by measuring audiometric outcomes (aided thresholds and speech reception in quiet and in noise) and self-assessed outcomes addressing the disability and the handicap domains of auditory dysfunction. Several cognitive abilities were assessed, including short-term memory, working memory and executive functions. To discover potential latent factors and assess which factors significantly influenced the benefit of HA use, age, hearing impairment, cognitive function, audiometric and self-assessed outcomes were examined with multivariate analysis, followed by correlation and regression analysis. The analysis revealed a significant improvement in aided audiometric outcomes and a decrease in the perceived disability and handicap in the sample population. Multivariate, correlation and regression analyses showed that better aided audiometric outcomes were significantly associated with hearing impairment of lower degree and higher cognitive abilities. Moreover, self-assessed outcomes were significantly associated with audiometric outcomes and hearing impairment: subjects with better audiometric outcomes and lower hearing impairment tend to perceive more benefit in using their HAs. Cognition was only indirectly associated with self-assessed outcomes through its direct correlation with aided audiometric outcomes, meaning that the perceived subjective benefit with HA was not directly associated with better cognitive abilities.

SommarioUn aumento del numero di pazienti acromegalici anziani è atteso nei prossimi anni grazie ai miglioramenti apportati in campo diagnostico e terapeutico. La diagnosi di acromegalia nell’anziano può risultare insidiosa a causa di quadri clinici sfumati che possono sovrapporsi alle manifestazioni tipiche dell’invecchiamento. Non esistono criteri diagnostici specifici per l’anziano. La chirurgia si è dimostrata efficace e relativamente sicura anche nel paziente anziano che, tuttavia, presenta una buona risposta agli analoghi della somatostatina.

A supplemented very-low-protein diet (sVLPD) seems to be safe when postponing dialysis therapy.Prospective multicenter randomized controlled study designed to assess the noninferiority of diet versus dialysis in 1-year mortality assessed by using intention-to-treat and per-protocol analysis.Italian uremic patients without diabetes older than 70 years with glomerular filtration rate of 5 to 7 mL/min (0.08 to 0.12 mL/s).Randomization to an sVLPD (diet group) or dialysis. The sVLPD is a vegan diet (35 kcal; proteins, 0.3 g/kg body weight daily) supplemented with keto-analogues, amino acids, and vitamins. Background: The levels of B-type natriuretic peptide (BNP), a marker of heart failure, are higher in women and anaemic subjects, and tend to be lower in obese people. These relationships are still largely unexplained and it is unclear whether they also apply to the N-terminal portion of BNP precursor (NT-proBNP). Aims: This cross-sectional study was performed to assess general and abdominal obesity, sex and other variables as possible extra-cardiac determinants of NT-proBNP. Methods and results: A random sample of 713 subjects aged 65–74 years resident of Pianoro (Northern Italy) underwent assessment of NT-proBNP, several haemato-chemical variables, body mass index (BMI), body fat estimation (through skinfold measurement), waist circumference, intra-abdominal thickness and possible presence of hepatic steatosis (by ultrasound examination). Un numero sempre maggiore di pazienti anziani inizia la dialisi. Tuttavia, malgrado i potenziali vantaggi della DP rispetto all'HD, tale tecnica è sottoutilizzata, specialmente nell'anziano. È, di recente, comparso un dibattito sulla rivista Seminars in Dialysis, relativamente al fatto di considerare o meno la DP come terapia sostitutiva di prima scelta per il paziente anziano.

Objectives: One of the objectives of the VINTAGE study was to collect the initiatives carried out in Europe and review the published grey literature about this topic. Methods: Email-based survey addressed to researchers, professionals and policymakers, and internet search of grey literature. Results: Three hundred nine contacts were finally made, and 21 of the 36 collected initiatives were considered as useful in preventing the harmful use of alco-hol amongst the elderly. Out of the about 2900 references identified 96 were classified as relevant. Conclusions: Despite a growing interest, alcohol use in the elderly is not yet perceived as a major issue for prevention. The type of hemodialysis vascular access (fistula, graft, catheter) employed plays an important role in the results of dialysis treatment. Moreover, different complications can affect the vascular access and interfere with the morbidity and mortality of patients. The ideal vascular access is the Cimino Brescia fistula. Graft and catheter methods should be considered as “second choice” because they present a higher incidence of complications, mainly due to thrombosis and infections. Finally, in elderly patients the vascular bed is frequently damaged and this may make it difficult to create a Cimino Brescia fistula. In a 5-year period, 140 elderly patients (>65 years) and 63 “young” patients (< 65 years) started dialysis treatment in our facility. In the elderly group, a native fistula was created in 88% of cases, whereas in the younger patients the percentage was 94% (p: NS). The grafts were, respectively, 11% in elderly and 6% in young patients. Only in one case, in one elderly patient, was a permanent catheter the first vascular access. We also report survival rate of the first vascular access, the incidence of thrombosis, and the need for creating another type of access. We suggest that a native fistula can be easily created in elderly patients and a “second choice” access should be limited to a small proportion of patients.
