Die Fallserie des Schlaflabors der Vestischen Kinder- und Jugendklinik Datteln umfasste in dem Zeitraum 1995–2015 64 Patienten mit Narkolepsie (32 Jungen, 32 Madchen, Lebensalter [Median] 12,5 Jahre). Wahrend in dem Zeitraum von 1995 bis 2010 (15 Jahre) bei 5 Patienten die Diagnose Narkolepsie gestellt worden war, kam es von 2011 bis 2015 (5 Jahre) zu einem Anstieg auf 59 Narkolepsiepatienten. Die Analyse des Beschwerdebeginns zeigte den Anstieg bereits ab 2008. Das Intervall zwischen Beschwerdebeginn und Diagnosestellung betrug 9,5 Monate (Median). Alle Patienten wiesen das Hauptsymptom Hypersomnie auf, davon 53 % eine Kataplexie, 3,1 % eine Schlaflahmung und 1,6 % hypnagoge Halluzinationen. In 65,6 % der Falle erfolgte die klinische Manifestation „plotzlich“, in 34,4 % „schleichend“. Eine positive Familienanamnese wurde bei 15,6 % der Betroffenen gefunden. In 9,4 % der Falle war eine H1N1-Impfung (Pandemrix®) vorausgegangen. Neben den klinisch-anamnestischen Daten der Fallserie werden die Ergebnisse der Polysomnographie (PSG), des multiplen Schlaflatenztests (MSLT) und der Labordiagnostik („Human-leukocyte-antigen“[HLA]Marker, Hypocretinkonzentration im Liquor cerebrospinalis) dargestellt. Die Fallserie zeigt das Problem der unvollstandigen Auspragung der typischen Narkolepsiesymptome und -befunde im Kindes- und Jugendalter. Daher sollte die PSG-Diagnostik bei „unklarer Hypersomnie“ wiederholt werden, wenn die klinische Symptomatik weiterbesteht.

Summary Introduction Although sleep disturbances in disabled children are of central clinical importance, there is little research on that topic. There are no data available on frequency, severity or aetiology of sleep disturbances and related symptoms in this specific patient group. Objective To review the current state of research and outline future research objectives. Methods We searched international scientific databases for relevant publications from 1980-2009. From all papers qualifying for further analysis we retrieved systematic information on sample characteristics, sleep assessment tools and their test quality criteria, and core findings. Results 61 publications including 4392 patients were categorized as "mixed" (reporting on heterogeneous diagnoses), or "specified" papers (specific diagnoses) based on international classification of diseases (ICD) 10 classification. To assess sleep disturbances, most authors relied on subjective instruments with poor psychometric quality. Mean prevalence of sleep disturbances was 67% (76%,"mixed" group; 65%, "specified" group). In children suffering severe global cerebral injury, the prevalence of sleep disturbances was even higher (>90%). The most frequent symptoms were insomnia and sleep-related respiratory disorders. Some of these symptoms were closely associated with specific medical syndromes. Conclusion There is an urgent need for sleep disturbance assessment tools evaluated for the patient group of interest. By use of validated assessment tools, patient factors, which may be crucial in causing sleep disturbances, may be investigated and appropriate treatment strategies may be developed.

Schlafstorungen und schlafassoziierte Storungen stellen im Kindesalter ein haufiges Problem dar. Die Zuweisung an ein padiatrisches Schlaflabor wie auch die Diagnostik und Einleitung von Therapiemasnahmen werden uneinheitlich gehandhabt. In diesem Konsensuspapier werden daher Empfehlungen formuliert, wann padiatrische Patienten einem Kinderschlaflabor zugewiesen werden sollten und welche Voruntersuchungen notwendig sind. Des Weiteren wird erlautert, fur wie viele Nachte der Patient untersucht und welche Parameter ermittelt werden sollten.

The prevalence of obstructive sleep apnea syndrome (OSAS) is clearly increased in adults with polycystic ovarian syndrome (PCOS), whereas OSAS does not seem to be frequent in adolescents with PCOS, pointing towards the fact that some patients with PCOS develop OSAS in the further course of the disease. We therefore aimed to analyze the changes of polysomnographic variables in obese adolescents with PCOS in a longitudinal analysis.Fifteen adolescents with PCOS (age 15.3 years ± 1.2, BMI 32.9 kg/m(2) ± 6.4, SDS-BMI 2.5 ± 0.8) underwent overnight 12-channel polysomnography at baseline and after a mean duration of 28 ± 6 months (age 17.8 years ± 1.1, BMI 32.7 kg/m(2) ± 7.0, SDS-BMI 2.1 ± 0.9). After performing the initial polysomnography, we treated hyperandrogenemia and insulin resistance in the study group. We determined parameters of body weight/body composition, parameters of glucose metabolism, and serum androgens in all patients at baseline and follow-up. At follow-up, we compared the polysomnographic variables of the study group to those of healthy female adults.The polysomnographic variables, the parameters of body weight/body composition, and the parameters of glucose metabolism in the study group did not change significantly during the observation period. The serum levels of total testosterone and sex hormone binding globulin increased significantly, whereas free androgen index decreased significantly. At follow-up, the polysomnographic variables of the study group did not differ from those of healthy female adults.OSAS does not seem to develop in adolescents with PCOS being treated for hyperandrogenism and insulin resistance. The pathogenesis of OSAS in PCOS needs to be examined in larger controlled studies.

We aimed to determine the differences in polysomnographic variables between obese adolescents with polycystic ovarian syndrome (PCOS) with and without metabolic syndrome, as the prevalence of obstructive sleep apnea syndrome (OSAS) is increased in adults with PCOS, OSAS has been regarded as a manifestation of the metabolic syndrome, and the prevalence of metabolic syndrome is increased in patients with PCOS.Fourteen obese adolescents with PCOS and metabolic syndrome [15.7 years ± 1.9, body mass index (BMI) 36.2 kg/m(2) ± 6.2], 14 obese adolescents with PCOS without metabolic syndrome (15.7 years ± 1.1, BMI 33.8 kg/m(2) ± 6.2), 19 healthy, obese adolescents without PCOS or metabolic syndrome (15.3 years ± 1.0, BMI 34.4 kg/m(2) ± 6.5), and 14 healthy, normal-weight adolescents (15.4 years ± 0.7, BMI 21.1 kg/m(2) ± 2.2) underwent polysomnography to compare transcutaneous arterial oxygen saturation (Sat O(2)), apnea index (AI), hypopnea index (HI), apnea-hypopnea index (AHI), the absolute number of obstructive apneas (NOA), percentage sleep stages 3 and 4 of non REM-sleep (stages 3 and 4), percentage of rapid eye movement (REM) sleep (%REM), sleep-onset latency, and sleep efficiency.We found no differences among the four groups concerning AI, HI, AHI, NOA, and stages 3 and 4. Significant differences among the groups were found regarding Sat O(2) (P = 0.04), %REM (P = 0.03), sleep-onset latency (P = 0.002), and sleep efficiency (P = 0.01).Weight status, PCOS, and metabolic syndrome do not seem to have significant effects on respiratory polysomnographic variables in adolescent girls with PCOS, suggesting that the pathomechanisms leading to OSAS in patients with PCOS develop in the later course of the disease.

We aimed to determine the impact of insulin resistance and hyperandrogenemia on polysomnographic variables in obese adolescents with polycystic ovarian syndrome (PCOS), as studies in adults with PCOS suggest that parameters of glucose metabolism and serum androgens are related to respiratory polysomnographic variables (RPV), and the symptoms of PCOS usually begin around menarche.We divided our study group of obese adolescents with PCOS according to HOMA-index and in a second analysis according to free androgen index (FAI). Study group A consisted of 14 girls with HOMA-index4, study group B of 17 girls with HOMA-index4. Study group C consisted of 19 girls with FAI10, and study group D of 18 girls with FAI10. The control group for both analyses consisted of 19 healthy obese adolescents without PCOS. All girls underwent overnight 12-channel polysomnography.In both analyses, we found no differences between the groups concerning the RPV. Study group B demonstrated a significantly lower percentage of REM-sleep than the control group (p = 0.02). Study group D demonstrated a significantly lower percentage sleep stages 3 and 4 of non-REM-sleep than study group C and the controls (p = 0.008). Study group D demonstrated significantly lower sleep efficiency than the controls (p = 0.03).Insulin resistance and hyperandrogenemia do not seem to have a significant impact on RPV in obese adolescents with PCOS. Differences in sleep architecture found between patients with PCOS and controls, however, are possibly influenced by insulin resistance and/or serum androgens.

Summary The aim of this study was to compare polysomnographic variables of obese adolescents with polycystic ovarian syndrome (PCOS) to those of healthy controls and to analyse whether polysomnographic variables correlate to parameters of body weight/body composition, to serum androgens and to parameters of glucose metabolism. Thirty-one obese adolescents with PCOS (15.0 years ± 1.0, body mass index 32.7 kg per m2 ± 6.2) and 19 healthy obese adolescents without PCOS (15.2 years ± 1.1, body mass index 32.4 kg per m2 ± 4.0) underwent polysomnography to compare apnoea index, hypopnoea index, apnoea–hypopnoea index, the absolute number of obstructive apnoeas, percentage sleep Stages 1, 2, 3 and 4 of non-rapid eye movement (NREM) sleep, percentage of REM sleep, TIB, total sleep time (TST), sleep-onset latency, total wake time (TWT), wakefulness after sleep onset (WASO) and sleep efficiency. Furthermore, we correlated polysomnographic variables to parameters of body weight/body composition, to serum androgens and to parameters of glucose metabolism. We found no differences between the two groups concerning the respiratory indices, percentage sleep Stages 2, 3 and 4 of NREM sleep, TIB and sleep-onset latency. The girls with PCOS differed significantly from the controls regarding TST, WASO, TWT, sleep efficiency, percentage Stage 1 of NREM sleep and percentage of REM sleep. We found a weak significant correlation between insulin resistance and apnoea index and between insulin resistance and apnoea–hypopnoea index. Concerning the respiratory variables, adolescents with PCOS do not seem to differ from healthy controls; however, there seem to be differences concerning sleep architecture.

In den letzten Jahren hat das Restless-Legs-Syndrom als “Volkskrankheit” bei Erwachsenen — insbesondere bei den uber 65-Jahrigen — enorme Bedeutung erlangt. Auch in der Padiatrie ist es aktuell, nicht zuletzt, weil Kinderarzte zunehmend mit besorgten Eltern konfrontiert werden, die wissen wollen, ob hinter den “unruhigen Beinen” ihres Kindes ein Restless-Legs-Syndrom stecken konnte.

The prevalence of obstructive sleep apnea syndrome (OSAS) is clearly increased in adults with polycystic ovarian syndrome (PCOS). The symptoms of PCOS usually begin around menarche. However, data concerning polynotsomnographic variables in adolescents with PCOS are limited. As obesity is a well-known risk factor for OSAS, we aimed to analyze differences in polysomnographic variables between obese and extremely obese adolescents with PCOS and healthy, normal-weight, obese, and extremely obese controls. Methods: Sixteen obese and 17 extremely obese adolescents with PCOS, 18 normal-weight, 17 obese, and 13 extremely obese controls underwent polysomnonotgraphy to compare mean transcutaneous arterial oxygen saturation (Sat O2), apnea-index (AI), hypopnea- index (HI), apnea-hypopnea index (AHI), the absolute number of obstructive apneas (NOA), percentage sleep stages 3 and 4 of non REM-sleep (stages 3 & 4), percentage of REM-sleep (% REM), sleep-onset latency, and sleep efficiency. Results: We found no significant differences between the groups concerning AI, HI, AHI, NOA, and stages 3 & 4. Significant differnotences between the groups were found regarding Sat O2, % REM, sleep-onset latency, and sleep efficiency. Conclusions: Concerning the respiratory variables, adolescents with PCOS do not seem to differ from healthy controls regardless of weight status, but there seem to be differences in sleep architecture.

Folgendes sollte bei Kindern mit Achondroplasie geklart werden: Haufigkeit anamnestisch erfassbarer Symptome (tages- und schlafbezogene Symptomatik) von schlafbezogenen Atmungsstorungen (SBAS), Beziehung zu erhohter Sterblichkeit, pathophysiologische Mechanismen sowie polysomnographische Formen und Schweregrade von SBAS. Es wurde eine systematische Literaturrecherche durchgefuhrt. Ausgewertet wurden 16 Publikationen aus den Jahren 1984 bis 2009: 2 Fallberichte, 8 klinische Fallserien, 1 Fragebodenstudie mit Referenzkollektiv, 5 physiologische, neuroradiologische bzw. molekulargenetische Studien, davon 3 mit Referenzkollektiven. Populationsbezogene epidemiologische Studien lagen nicht vor. Mehrere mit Achondroplasie assoziierte Faktoren (Mittelgesichtshypoplasie, muskulare Hypotonie, anatomische Veranderungen des Thorax und des kraniozervikalen Ubergangs) bedingen, dass diese Kinder als Gruppe ein erhohtes Risiko fur SBAS und chronische respiratorische Insuffizienz aufweisen. Im 1. Lebensjahr besteht ein erhohtes Risiko fur plotzlichen Tod, z. T. bedingt durch eine Hirnstammkompression. Im Laufe der Kindheit nimmt das Risiko der Kompression ab, und es kommt zu einer Verbesserung der Lungenfunktion. Trotz vieler Publikationen zu den Themen „SBAS bei Achondroplasie“, „prognostische Bedeutung von SBAS“ und „Pathophysiologie der SBAS bei Achondroplasie“, ist die Datenbasis limitiert. Die Resultate beziehen sich uberwiegend auf kleine Fallserien und sind durch Selektionsbias bzw. Recall Bias verzerrt. Aufgrund des hohen Risikos fur Atmungskomplikationen bei Achondroplasie erscheint es sinnvoll, alle Kinder in den ersten Lebensjahren polysomnographisch zu untersuchen.

The purpose of this study was to determine the differences in polysomnographic variables between obese adolescents with polycystic ovarian syndrome (PCOS) and healthy, normal-weight and obese controls, as the prevalence of obstructive sleep apnea syndrome (OSAS) is increased in adults with PCOS.Twenty-two obese adolescents with PCOS (mean age 15.2 +/- 1.3 years, mean BMI 31.7 +/- 6.2 kg/m(2)), 18 healthy, normal-weight adolescents (mean age 15.0 +/- 0.9 years, mean BMI 20.6 +/- 2.3 kg/m(2)), and 11 healthy, obese adolescents (mean age 15.0 +/- 1.0 years, mean BMI 34.8 +/- 8.7 kg/m(2)) underwent polysomnography to compare mean transcutaneous arterial oxygen saturation (Sat O(2)), apnea index (AI), hypopnea index (HI), apnea-hypopnea index (AHI), the absolute number of obstructive apneas (NOA), percentage sleep stages 3 and 4 of non-REM sleep (stages 3 and 4), percentage of REM sleep (%REM), sleep-onset latency, and sleep efficiency.We found no differences between the three groups concerning Sat O(2), AI, HI, AHI, NOA, and stages 3 and 4. The girls with PCOS differed from normal-weight and obese controls regarding sleep-onset latency and sleep efficiency and from the normal-weight controls regarding %REM.OSAS does not seem to be more prevalent in adolescents with PCOS. Concerning the respiratory variables, adolescents with PCOS do not seem to differ from healthy controls; however, there seem to be differences concerning sleep architecture.

BACKGROUND: The appearance of autoimmunity in tumor patients seems to be associated with a better prognosis. The aim of this retrospective study was to detect autoimmunity under adjuvant treatment with pegylated interferon-alpha-2b and to determine a correlation between autoimmune induction and survival. PATIENTS AND METHODS: To detect autoimmunity clinical and serological parameters were examined in 25 patients with R0-resected histological proven stage Ib-IV melanoma before and every three months during an 18-month adjuvant treatment with 2 μg/kg pegylated interferon-alpha-2b given once a week. RESULTS: Under treatment 16 of 25 patients (64 %) developed autoantibodies. There was no significant difference regarding survival data in patients with or without induction of autoantibodies (31,6 versus 31,9 months). However, it is remarkable that all 6 patients developing a clinically significant autoimmune thyroiditis and/or specific antinuclear antibodies were still alive 36 months after treatment beginning, with two of these patients being in complete remission from stage IV melanoma, compared to a mean survival time of 26,73 months for the remaining patient group with physiological irrelevant positive antibodies (n = 10/16). CONCLUSIONS: In our cohort, the number of patients who developed autoimmunity under pegylated interferon-alpha-2b is significantly higher as published data of non-pegylated interferon-alpha. Nevertheless, the significance of specific autoimmune induction as a prognostic indicator for a disease controlling antitumor immune response still needs to be confirmed.

Die vorgelegte Darstellung soll eine These stutzen, nach der sich die polysomnographisch objektivierbaren Atmungsphanomene des gesunden, ausgetragenen Sauglings parallel zur Reifung von Schlafarchitektur und Atmungsregulation entwickeln. Es wird dargestellt,wie im Laufe des ersten Lebensjahres unregelmasiges Atmen als Aquivalent aktiven Schlafes und paradoxe Atmung abnehmen, wahrend regelmasiges Atmen und koordinierte Atmung – die Entsprechungen von ruhigem Schlaf – in gleichem Mase zunehmen. Mit zunehmender Reifung des Organismus schwindet das Atemmuster der periodischen Atmung, wie auch Haufigkeit und Auspragung der verschiedenartigen Atempausen zuruckgehen. Qualitative und quantitative Abweichungen von als altersnormal erfasstem Atmungsverhalten finden sich im ersten Lebensjahr in signifikantem Ausmas bei nicht gesunden und bei unreif geborenen Kindern sowie bei Sauglingen, die zu epidemiologischen Risikogruppen des plotzlichen Sauglingstodes gehoren. So wurde das Atmungsverhalten von Sauglingen im Schlaf insbesondere von Fruhgeburtlichkeit, von Rauchen und Drogenkonsum der Mutter wahrend der Schwangerschaft und von Luftwegsinfekten zum Zeitpunkt der Untersuchung beeinflusst. Auch prospektiv untersuchte spatere SID-Opfer wiesen hinsichtlich ihrer Atmung Besonderheiten auf.

Hypersomnolence is the leading symptom of non-restorative sleep. Complaints of the child’s restless, irritative or aggressive behaviour may be prominent, masking the problem of sleepiness. Assessing the significance of daytime sleepiness in pediatric patients may be difficult with respect to the child’s age, ranging from the 1-day-old newborn infant to the 18-years-old adolescent. Physiological developmental changes of the individual degree of wakefulness have to be distinguished from pathological sleepiness in patients with hypersomnic and insomnic states. Thus, the Pediatric Group of the German Sleep Society (DGSM) has elaborated an algorithm providing a rational pathway with respect to the International Classification of Sleep Disorders (ICSD) as well as an economic use of resources.

Respiratory instability as well as tissue damage by free radicals (oxidative stress) have been hypothesized to play a role in cases of sudden and unexpected infant death in the first year of life. The ratio of the oxidized/reduced form of redox compounds in the circulation could be used as a marker of oxidative stress. Therefore, the sleep apnoea rate and redox status of coenzyme Q10 (CoQ10) (percentage of the oxidized form in total CoQ10) were measured in a population of clinically healthy infants in their first year of life in order to study whether a physiological parameter of respiratory instability is related to a biochemical parameter of oxidative stress. Between May and December 1999, 323 infants in the first year of life were referred to a paediatric sleep laboratory. Sleep apnoea rate, periodic breathing and parameters of oxygenation (SaO2 and TcPO2) were calculated based on polysomnographic recordings. The CoQ10 redox status was calculated based on high-pressure liquid chromatographic (HPLC) analysis. Statistical analysis showed an age-dependent decrease in apnoea rate (r=−0.38) and CoQ10 redox status (r=−0.40). An increased CoQ10 redox status (median: 16.6%; range: 7.3–29.7%) was found in infants with high apnoea rates above the 90th percentile of a reference group in comparison with infants with apnoea rates below the 90th percentile of a reference group (median: 10.4%; range: 5.1–20.4%; P=0.031). These findings may indicate that high apnoea rates are accompanied by increased formation of free radicals in clinically healthy infants in the first year of life.

Objective To derive reference curves of polysomnographic parameters for clinically healthy infants and children in the first and second year of life.

Bei Kindern mit obstruktivem Schlaf-Apnoe-Syndrom besteht haufig eine Hyperplasie der Adenoide und Tonsillen, so das sich ein Ansatzpunkt fur eine chirurgische Therapie ergibt. Wir berichten uber unsere Erfahrungen mit der pra- und postoperativen polysomnographischen Diagnostik in einem Zeitraum von 10 Jahren. Von 1989 bis 1998 wurden im Schlaflabor der Vestischen Kinderklinik Datteln, einem regionalen Zentrum fur schlafbezogene Storungen des Kindesalters, polysomnographische Schlafuntersuchungen bei 226 Kindern mit klinisch-anamnestischen Hinweisen auf OSAS durchgefuhrt. 62 (27%) Patienten erfullten die Kriterien fur die Aufnahme in die retrospektive Analyse (Polygraphie vor Therapie mit Bestatigung der Diagnose OSAS, chirurgische Therapie, postoperative polygraphische Kontrolle). Die Fallserie umfaste 38 Jungen und 24 Madchen im Alter von 0,3 bis 14,3 Jahren (Median 4,5 J.). Schnarchen und Mundatmung (100 %), Atempausen im Schlaf (81%), gestorter Schlaf (77%) und die subjektive Angabe “schweres Atmen” (53%) waren haufige anamnestische Symptome, die in der Regel uber einen langen Zeitraum bestanden. Die praoperative padiatrische und HNO-arztliche Diagnostik ergab bei 15 (24%) Patienten hyperplastische Adenoide, bei 17 (28%) hyperplastische Tonsillen und bei 30 (48%) eine Hyperplasie der Adenoide und Tonsillen. Bei 19 der 62 Patienten (31%) bestanden komplexe Krankheitsbilder. Es wurden bei 17 Patienten (28%) eine Adenotomie, bei 17 (28%) eine Tonsillektomie, bei 28 (45%) eine Adenotonsillektomie durchgefuhrt. Zusatzlich wurden bei 9 Patienten weitere Operationsverfahren durchgefuhrt. Die Polysomnographie zeigte postoperativ in der Gruppe eine signifikante Verbesserung der kardiorespiratorischen Parameter. Bei Kindern mit OSAS in Verbindung mit einer Hyperplasie des lymphatischen Rachenrings stellen Adenotomie und Tonsillektomie effektive Therapieverfahren dar. Die Indikationsstellung setzt eine sorgfaltige padiatrische und HNO-arztliche Diagnostik voraus, die die exakte Beschreibung des anatomischen und funktionellen Status zum Ziel hat. Der Schweregrad der Funktionsstorungen kann mit Hilfe der Polysomnographie quantifiziert werden.

Bei aufmerksamkeitsgestorten und hyperaktiven Kindern ergeben sich anamnestisch hauf ge Klagen uber Schlafstorungen dieser Patienten. Dies gab den Anlas, den Schlaf von Patienten mit Aufmerksamkeitsdefizit-Hyperaktivitat-Syndrom (Attention Deficit Hypractivity Disorder, ADHD) zu untersuchen. Von Oktober 1996 bis Oktober 1998 wurden polygraphische Schlafuntersuchungen an 22 Patienten mit ADHD (20 Jungen, 2 Madchen, Lobensalter 8±2 Jahre) und 11 Vergleichskindern ohne ADHD (8 Jungen, 3 Madchen, Lobensalter 7,2±3,5 Jahre) durchgefuhrt. Bei 6 ADHD-Patienten wurde eine medikamentos Therapie mit Methylphenidat durchgefuhrt. Bei den Kindern der Vergleichsgruppe wurdie Polysomnographie zur Abklarung von Parasomnien durchgefuhrt. Die polygraphisch Schlafuntersuchung umfaste die Registrierung kardiorespiratorischer und neurophysiologischer Parameter. Auf der Grundlage von EEG, EOG und EMG wurde visuell die Klassilzierung der Schlafstadien unter Berucksichtigung der Richtlinien von Rechtschaffen un-Kales durchgefuhrt. Bei Kindern mit ADHD, die keine medikamentose Therapie erhielte wurden verschiedene Muster des gestorten Schlafes gefunden: Einschlaf- und Durchschla probleme sowie gestorte Schlafzyklen. Als Gruppe wiesen sie im Vergleich zu gesunde Gleichaltrigen eine verminderte Schlafzeit und eine verminderte Schlafeffizienz auf. Die Unterschiede zu Vergleichskindern konnten bei ADHD-Patienten, die mit Methylphenid behandelt wurden, nicht nachgewiesen werden. Bei Kindern mit ADHD gibt es Hinwei auf eine Neurotransmitterstorung im Bereich des „limitierten Kapazitatskontrollsystem des Gehirns. Diese im Wachen manifeste Storung kann durch zentral wirksame indirek Sympatomimetika beeinflust werden. Fur die Schlafregulation ist das Aktivitatsverhaltraminerger und cholinerger Neuronenpopulationen im Hirnstamm von Bedeutung. Dies Aktivitatsverhaltnis konnte bei Kindern mit ADHD verandert sein und zu verschieden Mustern des gestorten Schlafs bei diesen Kindern fuhren. Veranderungen objektiver Schlaparameter im Sinne einer verminderten Schlafqualitat sind mit dem ADHD assoziiert, jedoch liegt kein einheitliches Muster des gestorten Schlafes vor. Unter der medikamentos Therapie mit zentral wirksamen Stimulantien zur Beeinflussung der Aufmerksamkeitssirung kommt es auch zu einer Verbesserung der Schlafqualitat.

Epidemiologische Erhebungen weisen Rauchen der Eltern und Stillverzicht als Risikofaktoren des plotzlichen Sauglingstodes aus. Aus klinisch-physiologischer Sciht gelten obstruktive Schlafapnoen und periodische Atmung als Risikomarker. So galt es, zu uberprufen, ob die genannten klinische-physiologischen Faktoren in einer abhangigen Beziehung zu den epidemiologischen stunden. Bei 302 Sauglingen wurden der Risikostatus erfast und eine Polysomnographische Untersuchung durchgefuhrt. Mit dem statistischen Verfahren der logistischen Regression wurden die moglichen Interkorrelationen uberpruft. Starkes Rauchen der Mutter wahrend der Schwangerschaft (≥10 Zigaretten/Tag) erwies sich als entscheidende Einflusgrose fur das Auftreten obstruktiver Schlafapnoen bei den Sauglingen. Kinder vielrauchender Mutter hatten im ersten Lebensjahr im Vergleich zu Kindern nicht rauchender Mutter ein etwa doppelt so hohes Risiko fur obstruktive Atempausen. Die Starke dieser Interkorrelation war altersabhangig, sie war im Alter von 14 bis 42 Tagen besonders hoch (O.R. 8.6, 95% CI 2.0;36.1, p=0.003). Das Auftreten periodischer Atmung wahrend mindestens 1% der Meszeit war bei gestillten Kindern von der 7. bis zur 10. Lebenswoche besonders haufig (O.R. 8.0, 95% CI 2.9;22.3, p=0.0001). Unabhangig vom Alter trat bei weiblichen Sauglingen periodische Atmung deutlich haufiger auf, als bei mannlichen (O.R. 2.3, 95% CI 1.4;3.9, p=0.002). Wir folgern, das starkes Rauchen der Schwangeren das Auftreten obstruktiver Apnoen bei ihren Kindern gerade fur den Zeitraum begunstigt, fur den die Epidemiologie den starksten Einflus solchen Verhaltens auf die SID-Inzidenz erweist. Die Auspragung periodischer Atmung wird von elterlichem Rauchen nicht verstarkt. Demgegenuber ist periodische Atmung positiv mit epidemiologischen Faktoren korreliert, die mit eher niedrigem SID-Risiko assoziiert sind. Dieser Befund stutzt die Auffassung, das periodische Atmung bei gesunden, reifgeborenen Sauglingen kein pathologisches Atemmuster darstellt.

Fragestellung: In der fruhen Krankheitsphase der Osteomyelitis ist die Sicherung der Diagnose schwierig, da charakteristische radiologische Befunde erst nach einigen Tagen auftreten. Daher wurde die Aussagekraft von Sonographie, Skelettszintigraphie und Kernspintomographie bei klinischer Manifestation einer akuten Osteomyelitis uberpruft. Methode: In den Jahren von 1980–1997 wurden 138 Patienten unter der Verdachtsdiagnose Osteomyelitis stationar aufgenommen, bei denen die Symptome weniger als 5 Tage bestanden. Die Aussagekraft von Sonographie, Skelettszintigraphie und Kernspintomographie wurde bei 66 Patienten mit gesicherter Osteomyelitis und 27 Patienten mit sicherem Ausschlus der Diagnose analysiert. Ergebnisse: Die Sonographie zeichnete sich durch eine grose Spezifitat (93%) und einen grosen positiv pradiktiven Wert (92%) bei geringer Sensitivitat (60%) und geringem negativ pradiktiven Wert (61%) aus. Die Skelettszintigraphie besas eine Sensitivitat von 80%, eine Spezifitat von 78%, einen negativ pradiktiven Wert von 68% und einen positiv pradiktiven Wert von 87%. Die groste Spezifitat (100%) wurde durch die Kernspintomographie erzielt (Sensitivitat 90%). Schlusfolgerung: Sonographien konnen die Verdachtsdiagnose weitgehend sichern, wahrend ein Ausschlus der Diagnose Osteomyelitis nicht moglich ist. Die Skelettszintigraphie eignet sich weder zur Sicherung noch zum Ausschlus der Verdachtsdiagnose. Differentialdiagnosen konnen v.a. durch die Kernspintomographie abgegrenzt werden. Da jedes bildgebende Verfahren Fehlinterpretationen zulast, kann die Diagnose einer Osteomyelitis nur durch Synopse aus klinischem Befund, Labordaten, Erregernachweis, Verlauf und bildgebender Diagnostik gestellt werden.

Die Kardinalsymptome des okulozerebrorenalen Syndroms (Lowe-Syndrom) sind kongenitale Katarakt, zerebrale Entwicklungsstorung, muskulare Hypotonie sowie tubulare Funktionsstorungen der Niere. Die Pathophysiologie ist unklar, vermutet wird eine Storung im Inositolphosphatstoffwechsel in Folge von Mutationen des auf dem X-Chromosom liegenden Gens. Wir berichten uber einen 4 Jahre alten Jungen, bei dem aufgrund der Familienanamnese, einer kongenitalen Katarakt, einer auffalligen Facies, einer muskularen Hypotonie, einer Entwicklungsverzogerung und einer tubularen Azidose die Diagnose eines Lowe-Syndroms gestellt wurde. Die augenarztliche Untersuchung der Mutter zeigte Linsentrubungen als Hinweis auf einen Konduktorinnenstatus. Die auf DNA-Ebene durchgefuhrte Mutationsanalyse ergab eine G-A-Transition an der Exon-Intron-Schnittstelle des vorletzten Exons im Gen. Diese Veranderung wurde bei der Mutter in heterozygoter Form nachgewiesen. Diskussion: Die Mutation, die aufgrund der besonderen Lokalisation neben dem Austausch einer Aminosaure ebenso das fehlerhafte Herausschneiden des Exons oder eine Verschiebung des Leserasters hervorrufen kann, ist die molekulargenetische Ursache der klinischen Auspragung des Lowe-Syndroms bei unserem Patienten.

Between 1991 and 1996 three cases of MCAD-deficiency (medium-chain-acyl-Co-A dehydrogenase deficiency) were diagnosed in the Vestische Kinderklinik. All patients showed hypoketotic hypoglycaemia with hyperuricaemia. In the group of hypoketotic hypoglycaemia without lactat acidosis MCAD-deficiency is the only metabolic disease presenting regularly with hyperuricaemia. Thus, hyperuricaemia in a patient with hypoketotic hypoglycaemia is a strong indicator for MCAD-deficiency. Measurement of uric acid is easily available before sophisticated metabolic analysis are completed.

Whereas in adults the sleep apnea syndrome is a well-defined illness, in childhood and infancy the relevance of different types of apneas is discussed controversially. In paediatric sleep laboratories experts usually decide more or less intuitively based on slopes of different important parameters measured by polysomnography. During the routine analysis of a night recording generally hundreds of apneas have to be classified. In order to accelerate this time consuming process and to emphasize the relevance of different apnea events we introduce a new apnea severity factor ASF which is calculated automatically. The algorithm is based on the expertise of four well trained physicians. at implies the examination of type and duration of each single apnea as well as the changes in oxygen saturation and heart rate variation. This apnea severity profile offers a non-subjective method of decision making and enables the physician to diagnose severe events in greater detail. For validation this method was applied to 9 newborn infants with bronchopulmonary dysplasia and to 11 children of a control group in a total amount of 32 apneas. It allowed a classification of respiratory disturbances by the first glance.

Physiological parameters of infants and children with Prader-Willi syndrome were examined in order to clarify whether there were indicators of disturbed respiratory control mechanisms in the pre-obesity stage of the syndrome. From January 1993 to March 1995 in eight patients with Prader-Willi syndrome (five boys, three girls, aged 6 weeks-12.5 years), polysomnography was performed and compared with 28 children matched for gestational age, sex, birth weight and age at sleep study. The recordings included thoracic and abdominal breathing movements, nasal airflow, tcPO2, tcPCO2, oxygen saturation, EEG, EOG and ECG. Respiratory responses to hypercapnia during quiet sleep were obtained from five Prader-Willi patients and ten peers. The Prader-Willi group showed an increased number of apnoeas per hour of sleep, a decreased nadir of oxygen saturation, increased maximum of the instantaneous heart rate and decreased respiratory responses to hypercapnia during quiet sleep.These findings indicate a primary disturbance of central respiratory control in patients with Prader-Willi syndrome which may be worsened by the development of obesity.

SUMMARY In three young infants with Joubert's syndrome polygraphic recordings were carried out in the awake state and during REM and NREM sleep in order to characterize the respiratory disorder associated with this condition. In all three states disturbed respiration parameters were found in variable amounts. The highest breath rate occurred in the awake state followed by REM and NREM sleep. The highest incidence of apnoea and periodic breathing was observed in NREM sleep, followed by REM sleep and the awake state.

We report the case of a mature newborn infant, which aroused attention 8 hours after birth due to an unexplainable paroxysmal tachypnea together with subsequently prolonged apnea. The combination of tachypnea, athetoid movement patterns, apraxia of the tongue, nystagmus and dysplasia of the cerebellum with dilation of the 4th ventricle, led to the diagnosis of Joubert-Syndrome. The child also showed a variation of a connatal unilateral facial nerve palsy, which has not previously been described in connection with a Joubert-Syndrome.

Newborn infants with inborn errors of lipid metabolism are relatively rare. There are only a few reports of hyperlipoproteinemia in infancy. In a six-day-old "healthy" newborn infant with a weight of 2960 gm a milky blood serum was found. Diagnosis of primary hyperlipoproteinemia type I was established by exclusion of other diseases and by analysis of blood lipids. Lack of lipoprotein lipase activity in postheparin plasma confirmed the diagnosis. The parents of the patient were normolipemic but showed reduced lipoprotein lipase activity. Thus, they were recognized as heterozygotes. A low fat (6-19 gm fat/d, 2-3 gm fat/kg bodyweight/d) diet with a high proportion of medium-chain triglycerides (78% MCT) lowered the initially extremely high serum triglyceride level (3674 mg/dl) significantly (214-711 mg/dl). Until the age of 7 months body weight and length followed the 10. percentile, head circumference followed the 50. percentile. Unexpectedly the patient died of pulmonary infection at the age of 8 months.

In den verschiedenen Bereichen der medizinischen Aus-,Fort- und Weiterbildung stehen traditionelle mundliche Prufungen seit Jahren in der kritischen Diskussion. Ihre Problematik ergibt sich aus der ausgepragten Theorielastigkeit. Eine Standardisierung der Fragen und der erwarteten Antworten ist nicht moglich. Neben einem fundierten theoretischen Wissen erfordert die patientennahe arztliche Berufstatigkeit in der Krankenversorgung weitere unverzichtbare Kompetenzen: hilfsbedurftige Lagen von Menschen erkennen konnen; klinisches Denken in Zusammenhangen, ausgehend von dem konkreten Problem eines Patienten; Gesprachsfuhrung; Beratungskompetenz; manuelle Geschicklichkeit und vieles mehr.Diese Kompetenzen eines „guten Arztes“ konnen in einer traditionellen mundlichen Prufung nicht systematisch erfasst werden.Die von Ronald Harden 1975 erstmals vorgestellte Prufungsform des „Objective structured clinical examination“ (OSCE) bietet hier eine Alternative. Der OSCE ist ein standardisiertes, auf Checklisten basierendes mundlichpraktisches Prufungsformat,mit dem nicht nur das Produkt Wissen, sondern auch Prozesse wie Handlungsablaufe und Problemlosungsstrategien bewertet werden konnen.

Introduction: Increased attention has been paid to the role of lipophilic antioxidants in childhood nutrition and diseases during recent years. The lipophilic antioxidant coenzyme Q10 (CoQ10) is known as an effective inhibitor of oxidative damage. In contrast to other lipophilic antioxidants like α-tocopherol the plasma concentrations of CoQ10 in childhood are poorly researched. The aim of this study was to determine plasma level and redox status (oxidized form in total CoQ10 in %) of CoQ10 in clinically healthy infants, preschoolers and school-aged children. Methods: Plasma level and redox status of CoQ10 were measured by HPLC in 199 clinically healthy children, three groups of infants [1st-4th month (n = 35), 5th-8th month (n = 25), 9th-12th month (n = 25) ], preschoolers (n = 60) and school-aged children (n = 54). The CoQ10 plasma levels were related to plasma cholesterol concentrations. The median and the 5th and 95th percentile were calculated. Results: Plasma levels and redox status of CoQ10 in infants were significantly higher than in preschoolers and school-aged children. The CoQ10 redox status in the 1st-4th month was significantly increased when compared to the remaining subgroups of infants. In elder children the CoQ10 redox status stabilized. Conclusion: This is the first study concerning age-related values of plasma level and redox status of CoQ10 in apparently healthy children. Decreased CoQ10 values could be involved in various pathological conditions affecting childhood. Therefore, the application of age-adjusted reference values may provide more specific criteria to define threshold values for CoQ10 deficiency in plasma.

A 13-year-old girl presented with hyersomnia and incontinence of urine. The clinical course was complicated by strabism, muscle weakness and coordination and balance disturbances. VEP recording were pathologic. Myelin basic protein in the cerebrospinal fluid was elevated. Nuclear resonance imaging of the brain revealed multiple areas of demyelination. These findings supported the diagnosis of multiple sclerosis. There was a marked disturbance of the sleep-waking-pattern during the acute phase of the illness. The hypnogram resembled the pattern of polysymptomatic narcolepsy. Regression of symptoms was noted with dexamethasone therapy.

Klinische Diagnosekriterien des Restless-legs-Syndroms (RLS) wurden 1995 durch die International Restless Legs Syndrome Study Group (IRLSSG) erarbeitet. Angesichts neuerer Forschungsergebnisse und klinischer Erfahrung wurden die Diagnosekriterien bei einem Konsensustreffen der IRLSSG revidiert. Es wurde als besonders wichtig erachtet, spezifische Diagnosekriterien fur zwei Untergruppen auszuarbeiten: 1. fur Kinder und 2. fur kognitiv beeintrachtigte Patienten im hoheren Lebensalter. Beiden Gruppen gemeinsam ist die Schwierigkeit, ihre Beschwerden adaquat auszudrucken, wodurch die Diagnosestellung erheblich erschwert werden kann. Vorschlage fur Diagnosekriterien des RLS im Kindesalter wurden bereits 2002 von Mitgliedern der Arbeitsgruppen „Motorik und Schlaf“ und „Padiatrie“ der Deutschen Gesellschaft fur Schlafforschung und Schlafmedizin erarbeitet. Diese wurden in der Empfehlung der IRLSSG mit berucksichtigt. Bei den Diagnosekriterien handelt es sich um Empfehlungen, die in kunftigen klinischen Studien validiert werden mussen. In der vorliegenden Arbeit werden nach einem Uberblick zu RLS im Kindesalter die vorgeschlagenen Diagnosekriterien vorgestellt und die Besonderheiten der Diagnosestellung bei Kindern zusammengefasst.

Eclampsia is defined as one or more generalised convulsions in the setting of pre-eclampsia. Only 0.5% of pre-eclamptic woman develop eclampsia, and about 15–30% of cases occur after delivery.1 2 A single case of reversible cerebral oedema with brainstem involvement in an eclamptic woman has been reported with transient gaze-evoked nystagmus.3 Other cases of reversible brainstem oedema in various medical conditions did not include eclampsia.4 5 In these cases, posterior white matter was not spared as in our case, and so we see this reversible eclamptic leucoencephalopathy syndrome separate from ‘classic’ reversible posterior leucoencephalopathy syndrome. A HPLC-method was developed for the simultaneous detection of the lipophilic antioxidants ubiquinol-10, ubiquinone-10, alpha-tocopherol, and gamma-tocopherol in plasma macrosamples (100 microl) as well as microsamples (10 microl) using electrochemical detection with internal standardization (gamma-tocotrienol, ubiquinol-9, ubiquinone-9). The use of a mobile phase containing ammonium formate instead of lithium perchlorate as conductivity salt and the use of a PEEK column instead of a steel column improved the reproducibility of the method. The components were separated on a RP C18 column. The detection limits for all components were between 1 and 28 fmol. The within-day precision varied between 3 and 13% for all measured substances. The analytical recovery was between 93 and 109%. The plasma levels from 10 healthy donors were determined in microsamples and macrosamples taken by micropuncture or venous puncture, respectively. A good correlation of the antioxidant levels for both methods strengthened the reliability and the transferability of the results. The present method can be used to assess the role of these antioxidants in diseases associated with oxidative damage in childhood.

Fluorine-18 fluorodeoxyglucose and positron emission tomography (FDG-PET) is a new imaging modality used in the follow-up of patients with differentiated thyroid cancer if the results of (131)I scintigraphy are negative in spite of an elevated thyroglobulin level. The aim of this retrospective analysis was to estimate the value of FDG-PET regarding the operability of patients with positive findings. From January 1994 to October 1997, we investigated 60 patients with differentiated thyroid carcinoma by FDG-PET. Thirteen patients were operated on after positive findings. Most of these lesions were suspected of having lymph-node involvement or local recurrences in the thyroid bed. One patient showed a solitary distant metastasis in the scapula. Thirteen of 16 operations in these 13 patients confirmed the suspected involvement of thyroid cancer. The false-positive findings were caused by inflamed lymph nodes in two cases and benign thymus tissue in one case. We conclude that PET is a useful diagnostic tool to guide early surgical therapy in patients with (131)I negative differentiated thyroid carcinoma.

The integrin CD11b is an important adhesion molecule mediating the transendothelial migration of circulating polymorphonuclear granulocytes into an inflammatory region. The expression of CD11b is closely related to the ability to polymerize actin, a major component of the cytoskeleton within the phagocyte. In this study we compared the CD11b expression as well as the polymerization of actin of isolated neutrophils from patients endangered by sepsis with cells from healthy donors. The patient population was subdivided into a group of patients with severe thermal injuries and a group of patients who were admitted to an intensive care unit on suspicion of sepsis. The following results were obtained: (1) cells from burn patients, but not from non-burn patients, showed a reduced basal expression of CD11b during the first week after the burn trauma; (2) stimulation with the chemotactic peptide formyl-Met-Leu-Phe (FMLP) led to a strong overexpression of CD11b on the cells from the burn patients, this effect was not observed using cells of the second subgroup; (3) the content of polymerized actin was reduced within resting and stimulated cells from burn patients during the first 2 weeks postinjury, non-burn patient cells showed an enhanced F-actin content within the first week; (4) the ability of burn and non-burn patient cells to polymerize actin after stimulation with FMLP was slightly impaired during the first week post injury/admission. The results demonstrate that cells from patients endangered by sepsis show dysfunctions on the level of adhesion molecule expression and the strongly related actin polymerization. There are remarkable differences between patients with burn injuries and patients who were endangered by sepsis due to other injuries or diseases.

Polygraphic recordings in the awake state and during sleep were performed in two girls with Rett syndrome, aged 9 y and 17 y, in order to characterize the clinically manifest breathing disorder of these patients. Primary hyperventilation was detected, leading to hypocapnia and compensatory apnoea. Hypoxaemia occurred as the consequence of prolonged apnoea. In the younger patient these disturbances were observed only in the awake state, whereas in the older patient they were found in the awake state and during sleep.
