2. Severe Cutaneous Candidiasis in a Liver Transplant Patient

Candida infections are common diseases in immunocompromised patients. A 19-year-old boy with liver transplantation, necrotic skin lesion, jaundice, dyspnea, and ascites was admitted to Namazi Hospital, Shiraz, southern Iran. The mycological examination for the skin lesion was requested. The skin sample was cultured on Sabouraud dextrose agar and evaluated by direct microscopic smear. Identification of isolated yeast was performed with RFLP-PCR. In direct smear, pseudohyphae, blastopores and yeasts were observed. Candida species was isolated from the media and identified as Candida albicans by molecular method. He died before starting any treatments. A skin lesion may present as the only sign of a systemic fungal infection in immunocompromised people. Careful attention and follow up are therefore recommended.

3. Antibiotic Resistance Pattern of Gram-Negative Bacteria in Gorgan

Abstract Background and Objective: The excessive use of broad-spectrum antibiotics will lead to drug resistance of microorganism and specially nosocomial organisms. Because of high incidence of antibiotic resistance in hospitals, we aimed to study antibiotic resistance to gram negative bacteria. Material and Methods: This cross-sectional study was conducted on the data of biological samples (2006-2008), with positive culture result. Using antibiogram, microbial resistance to isolated microorganism was determined, and after culturing the samples, bacteria were identified by using differential media and antiserum. Then, antibiotic resistance was performed by disk diffusion. Results: The most common gram-negative microorganism obtained from all cultures was Ecoli with the lowest drug resistance to Nitrofurantoin. Conclusion: Based on the results, antimicrobial resistance pattern is not the same in different places and furthermore it is ever changing. Therefore, further research is needed to be done to have an accurate pattern of antibiotic resistance to provide effective treatment regimens. 4. Seroprevalence of Campylobacter Jejuni in Patients with Irritable Bowel Syndrome and in Healthy Individuals

Abstract Background and Objective: The patients with Post-Infectious Irritable bowel syndrome (PI-IBS), a subtype of irritable bowel syndrome, suffer from bacterial gastroenteritis. Since campylobacter Jejuni (CJ) is one of the most common agents in this syndrome, we aimed to evaluate the prevalence of Campylobacter Jejuni in patients with Irritable Bowel Disease. Material and Methods: This case - control study was conducted on 160 patients divided into 2 equal groups of healthy and unhealthy. The presence of Anti- CJ antibody (IgG and IgA) was evaluated by ELISA and the comparison was performed by chi-square test. Results: The mean age of case (31.51) and control (31.84) was not statistically different (P = 0.87). Titer of Anti- Campylobacter Jejuni antibody IgG was positive in 25% of patients and 18.8% of the healthy ones (p=0.02). IgA Seropositivity in patients was 7.5% but no one in control group was positive (p =0.01) Conclusion: Seroprevalence of Cj in patients with IBS was higher significantly than that of control group. Thus, Cj can be known as one of the causes of Post-infection in patients with IBS in our region and it should be paid more attention in diagnostic assessment of these patients. 5. Adsorption of bacterial spores from air using bone char

Background and aims : Microorganisms are one of the most important sources of contamination in indoor air and occupational environments especially in hospitals. The aim of this study was to evaluate the adsorption characteristics of bone char to adsorb B. Subtilis in the air. Methods: In this study B.subtilis spores were used as a model of bacterial spores. Also bone char was used as an adsorbent in this study that was prepared by thermal method from waste bones and the characterization of prepared adsorbent was determined. Then the adsorption capacity of bone char for B.subtilis was evaluated. Results: The Present study show that bone char as a mineral adsorbent has high efficiency in adsorption of bacterial spores. The adsorption capacity of this mineral was 3698 spores per gram. Conclusion: bone char has a high capacity for adsorption of bacterial spores. Mineral structure of bone char can delay the reproduction and biofilm formation of microorganism on its surface. The result of this study could be used in development of control equipment of biological air pollutant.

6. Yersinia Enterocolitica in Cases of Diarrhea in Gorgan, Northern Iran

Abstract Background and objectives: After respiratory infection, Diarrhea is the second cause of mortality. Yersinia enterocolitica is the second important cause of infectious diarrhea in children of some countries. In this study, we evaluated the frequency of Yersinia entocolitica of diarrheal specimens in Gorgan, Iran. Material and Methods: This descriptive cross - sectional Study was carried out on diarrheal stools of 455 patients referred to medical centers and laboratory of Gorgan in 2004-2005. DNA extraction using phenol chloroform was performed for all samples. Using two specific primers (genus-specific16s rRNA and ail- specific species genus of Yersinia enterocolitica), we did PCR sample. Results: Yersinia genome was identified in 12 patients(2.63%) and 11 of them was Yersinia enterocolitica. The frequency infection in of girls (3%) was more than boys (2.4%), and the prevalence in winter (4%) was more them other seasons, and under one- year- group (3.4%) and 1-5 years (3.1%) is more than other age groups. It was not observed significant difference. (P> 0.05). Conclusion: The frequency of Yersinia in cases of diarrhea in Gorgan is similar to most regions of Iran and in children under 5 years is observed more in winter. Delayed onset muscle soreness (DOMS) appears after unaccustomed exercise and peaks 24-48 h after exercise. Vitamin D micronutrient and vibration therapy may have an effect on DOMS. The present study investigated the effects of vitamin D micronutrient and vibration therapy on DOMS. Sixty female students were randomly assigned to one of the four groups (n=15 in each group): vitamin D, vibration therapy, vitamin D + vibration therapy, and control. The participants of vitamin D groups received vitamin D (3,800 IU, 1 session daily for 7 days), while the participants of the control groups received placebo. The participants of vibration therapy groups received vibration therapy (50 Hz; 3 sets of 1 min, 1 session daily for 7 days). One day later, the participants performed eccentric exercise (a quadriceps leg extension exercise). Immediately after this exercise protocol, the participants received vitamin D or vibration therapy on basis of their groups. Pain perception, creatine kinase (CK), interleukin (IL)-6, superoxide dismutase and malondialdehyde (MDA) concentration were measured at baseline (before 7 days of intervention), after 7 days of intervention (before eccentric exercise) and 24, 48 and 72 h after eccentric exercise. Statistical analysis was employed and P≤0.05 was considered as the significant level. CK and IL-6 concentrations, as well as pain perception, were significantly lower in the vibration therapy and vitamin D groups compared to the control group 24 to 48 h after eccentric exercise. MDA concentration was significantly lower in the vibration therapy and vitamin D groups compared to the control group 48 to 72 h after eccentric exercise. In conclusion, the present study suggests that vibration therapy and vitamin D supplement may have effects against eccentric exercise-induced delayed onset muscle soreness in female students.

Summary Background & aims Several genome-wide-association-studies have identified genetic variants in a region on chromosome 9p21 that are associated with an increased risk of Cardiovascular disease (CVD) and diabetes. Here we have explored the interaction of a genetic variant of the CDKN2A/B-rs10811661 gene locus with cardiovascular risk factors and environmental-exposures (e.g., diet and physical activity) in 1165 individuals recruited from the Mashhad-Stroke and Heart-Atherosclerotic-Disorders cohort. Methods Genotyping was carried out using TaqMan-real-time-PCR based method. The association of CDKN2A/B-rs10811661 locus and its interaction with dietary intake in association with the main determinants of dyslipidemia, and cardiovascular-risk-factors were assessed in 2 cohorts. Results Our data showed that obese subjects with a TT genotype had a higher level of TG, TG/HDL ratio and Hs-CRP, compared to the subjects with the wild type genotype, or individuals with a normal BMI. Moreover, the presence of a TT genotype was associated with increased risk of hypercholesterolemia, insulin resistance and CVD. These effects were more pronounced in the sub-group with low physical activity and a high dietary energy intake (e.g., the interaction between TT genotype and total energy intake on serum cholesterol was positive (RERI: 0.2, 95%CI (−0.96–1.3), AP: 0.1, 95%CI (−0.5–0.7) and SI: 1.2, 95%CI (0.3–5.1))). Conclusions We have found a significant association between the CDKN2A-rs10811661 polymorphism with cardiovascular risk factors and dyslipidemia in a non-diabetic population. It is possible that a low energy diet and high physical activity could ameliorate the unfavorable effects of T allele of CDKN2A/B locus. Functional analysis is warranted to investigate the value of this genetic biomarker of CVD risk in obese people.

Objective It has been shown that several environmental and physiological factors can affect on the serum levels of calcium and phosphate. The objective of the present study was explored the relationship between serum calcium and phosphate levels with anthropometric and hematological markers. Methods 908 subjects were recruited from the Mashhad stroke and heart atherosclerosis disorder (MASHHAD) program. Anthropometric parameters, liver/kidney function tests (e.g., Urea nitrogen, creatinine, urea and uric acid, creatinine, AST, ALT) were determined in all participants. Serum concentrations of calcium and phosphate were measured using Autoanalyzer BT3000P (Pars Azmoon kit, Tehran, Iran). SPSS software was used for statistical analyses. Results We observed that obese subjects had a lower level of serum calcium (p˂0.05). Moreover, a relationship was detected between serum phosphate level and different menopausal status (p˂0.05). Serum calcium and phosphate did not change by increasing age in the population. Additionally, there was a correlation between lymphocyte count with serum phosphate level (p˂0.05). No statistically different were detected for the levels of calcium/phosphate with respect to smoking status, physical activity, lipid profile, liver and renal function markers. Conclusion We found an association between serum calcium and BMI as well as with serum phosphate and menopausal status.

Objective To explore the effect of hepatocyte growth factor (HGF) and fibroblast growth factor (FGF) on preimplantation embryo development of mouse. Methods Mated mice were killed by cervical dislocation to collect two pronucleous (2 PN) zygotes from oviduct of pregnant 1 d NMRI mice and were cultured to the hatching blastocyst stage and the number of embryo in different stages was recorded under an invert microscope. The cleavage rates of formed 2 PN zygotes were compared with blastocyst and hatching blastocyst stage in drops of T6 medium with or without HGF or FGF (0 ng/ml, 10 ng/ml, 20 ng/ml, 50 ng/ml, 1 000 ng/ml). Results HGF and FGF enriched embryo culture media promoted the development from 2 PN stage embryos to blastocyst. Adding 20 ng/ml of FGF or HGF to the culture medium significantly increased the percentage of 2PN embryos that developed into blastocysts (P Conclusion Exogenous HGF and FGF at low concentrations promote in-vitro mouse blastocyst formation.

SUMMARY One hundred eleven mares were examined and selected on the basis of some criteria (history of recent genital discharge and/or abortion, dystocia, retained placenta, rectovaginal rupture and/or failing to conceive after repeated services), rectal palpation, and ultrasound examination of genital tract. The mares were classified in 3 groups (young, middle age and old). During estrus, a specimen was taken by uterine swab and endometrial biopsy. Endometritis was diagnosed and graded on the basis of pathology results and treatment was done on the basis of culture and antibiogram tests. At the first step of treatment, uterine lavage with warm saline followed by intrauterine administration of a specific antibiotic, or povidone iodine solution, was done daily during estrus. If the mare had not responded to the first treatment, in the next steps systemic antibiotic therapy was also added to the mentioned treatment. The results showed that Escherichia coli was the most frequently isolated pathogen and response to the treatments was significantly better in the young mares with category IIA fibrosis of uterine pathology than that of two other groups. It was also found that there is no relationship among age and kind of pathogen to mare endometritis.

We use drift and diffusion coefficients to reveal interactions between different oscillatory processes underlying a complex signal and apply the method to EEG delta and theta frequencies in the brain. By analysis of data recorded from rats during anaesthesia we consider the stability and basins of attraction of fixed points in the phase portrait of the deterministic part of the retrieved stochastic process. We show that different classes of dynamics are associated with deep and light anaesthesia, and we demonstrate that the predominant directionality of the interaction is such that theta drives delta.
