Pyomiositis is a bacterial infection of skeletal muscle tissue occurring mostly in immunocompromised pa­tients, including diabetics. The main agents are Gram-positive cocci . We report a case of a 42 years old, male, diabetic, who presented the emergency room referring pain in thighs and inability to flex his legs. We performed a magnetic resonance which revealed extensive purulent collections in both lower limbs, thus confirming clinical suspicion, and allowing proper antibiotic treatment. In this case, we showed that imaging tests facilitate early diagnosis and treatment through direct location of the lesions, and guides invasive procedures such as biopsy and abscess aspiration when needed.

Cysts in the anterior cruciate ligament is difficult to diagnose due to the lack of specific symptoms and signs. Incidence of this disorder was the highest for 20–40 years of age group. Occurs predominantly in males. Our aim was to report a case and review the literature about this uncommon lesion. We carried out a review of medical records, photographic record of diagnostic method, and review from the literature. 18 years-old man with pain in the right knee for 04 months. He plays two hours of soccer everyday for a few years. Physical examination with specific knee negative tests, without bruise or edema. Denies torsion, trauma and surgeries. Magnetic resonance imaging demonstrates intrasubstantial lobulated cyst in the anterior cruciate ligament. This report demonstrates the difficulty of diagnosing this lesion, since its physical examination is non-specific, and it can simulate more serious pathologies, requiring complementary tests for its correct diagnosis.

The spine is the main stabilizer and load bearer of the axial skeleton. It is also important for the protection of neural structures, such as the spinal cord, nerve roots, and cauda equina. In the healthy skeleton, most injuries are a consequence of high-energy trauma and can lead to severe dysfunction, such as tetraplegia or paraplegia. In order to avoid such disabilities, it is important to recognize details that will guide treatment, and that will determine the necessity or not to have surgery. Familiarity with radiography, CT, and MRI in evaluating spine trauma is necessary, as, in some cases, all three methods will be useful in determining management and surgical planning. The most important factor in determining management in the thoracolumbar spine is the posterior ligamentous complex (PLC). Therefore, familiarity with its anatomy, primary and secondary signs of its injuries, is essential for radiologists in the emergency setting. Spine fractures are a very heterogeneous group of disorders. Management can be both conservative and surgical. It is important for radiologists to be aware of classifications and patterns for these injuries.

Most stress fractures occur in the lower limbs and are rarely observed in the , upper limbs. The second metacarpal is the longest of all the metacarpals and has the largest base, articulating with the trapezium, trapezoid, capitate, and third metacarpal. In athletes, stress fractures in non-weight bearing joints are uncommon. Therefore, the shaft of the second metacarpal bone undergoes a higher load - the maximum tension at the base of the second metacarpal is amplified when the hand grasps a tool such as a tennis racquet.

Bouveret syndrome is a rare disease—a cause of gastric outlet obstruction. It represents less than 3% of cases of gallstone ileus, which is already rare—0.3–0.5% of patients with cholelithiasis. Persistent inflammation provokes the development of the fistula between the gallbladder and the duodenum and, with an elevation of the intraluminal pressure, the wall ischemia starts, enabling the gallstone entrance in the bowel. The risk factors are female, gallstones larger than 2.5 cm, and elderly. Morbidity and mortality are between 15 and 33%. Herein, we present a case of a Bouveret syndrome diagnosed by a CT scan.

Haglund syndrome is a cause of posterior heel pain and was first described by Patrick Haglund, in 1928, to occur in patients who had a prominence of the posterosuperior surface of the calcaneus.

A 4-year-old male patient presented with headaches that started 18 months ago and were associated with progressive loss of balance and difficulty walking. Magnetic resonance imaging (MRI) demonstrated an expansile extra-axial mass lesion in the posterior aspect of the posterior fossa on the left, compressing the adjacent brain, as well as the cerebellar hemispheres; it also distorts the fourth ventricle with severe obstructive hydrocephalus upstream. Histological evaluation diagnosed aggressive osteoblastoma with a secondary aneurysmal bone cyst of the skull. We report a case of aggressive osteoblastoma in which the lesion in the MRI was mimicking brain tumor or intracranial primary tumor rather than primary bone tumor.

Introdução: A coalizão tarsal representa uma comunicação anormal entre ossos do tarso e pode ser óssea, cartilaginosa ou fibrosa, se desenvolvendo de forma secundária a insuficiência da diferenciação e segmentação do mesênquima primitivo nas primeiras fases do desenvolvimento, diminuindo a mobilidade e deformando os pés, quando congênita. Quando adquirida ocorre devido à artrite inflamatória, infecção, trauma, neoplasia, entre outras causas. Relato de Caso: Relatamos o caso de uma mulher de 37 anos com queixa há um mês, desmontando a importância da investigação da causa e a necessidade de atenção por parte do radiologista para a avaliação das coalizões tarsais visto que, uma coalizão, independentemente de quais ossos envolvidos, pode levar a uma sintomatologia limitante. Here, we report the case of a 46-year-old male who present-ed with a three-year history of pain in the right knee. He stated that he had experienced no trauma or torsion, had not under-gone any surgery, and did not engage in sports. In the physical examination, he tested positive for a meniscal tear. Calcaneal apophysitis is a common cause of heel pain in children and is also known as Sever´s disease. Rarely causes important disability and is transient in most of the times, but it can interfere with walking and physical performance in sports, causing concern to the patient and parents. It is the most common cause of heel pain in the physically active growing children–8-25 years-old–and is considered a benign, self-limiting condition of childhood and adolescence. Boys constitute two-thirds of the patients and the sport that tends to dominate is soccer. The diagnosis of calcaneal apophysitis is usually made with a typical history and physical examination. Pain with activity, especially running and jumping, and a positive “squeeze test”: pain is produced by medial and lateral compression of the heel where the calcaneal apophysis attaches to the main body of the calcaneus. Children with Sever’s disease don´t have growth disturbances in the calcaneal apophysis or any vary in bone density compatible with a necrotic or a repair process. Children with Sever’s disorder respond variably, poorly, or not at all to anti-inflammatory drugs. Heel cup fixating the heel pad provided better pain relief in young boys with Sever’s injury compared with the traditional heel wedge.

A 42-year-old man presented with a 3-year history of headache. Neurologic examination was unremarkable. Brain MRI showed multiple peripheral cerebellar nodules, some containing fat (figure). Biopsy revealed neurocytic cancer cells scattered with mature fat tissue. The approach was conservative with serial imaging follow-up. Cerebellar liponeurocytoma is a rare tumor classified as grade II by the World Health Organization,1 now considered a clinicopathologic entity distinct from medulloblastoma.1 It is almost always solitary and almost always cerebellar. The clear multifocality of the MRI findings makes this case unusual.2
