Nach brusterhaltender Operation ist eine Nachbestrahlung indiziert. Es stellt sich die Frage, wie stark ausgepragt die Effekte der Bestrahlung auf das Lokalrezidivrisiko und Uberleben sind und ob nicht bei bestimmten Subgruppen auf eine Bestrahlung verzichtet werden kann. Auserdem ist interessant, ob auch kurzere Fraktionierungsschemata angewendet werden konnen sowie in welchen Fallen und in welcher Technik ein Boost appliziert wird. Des Weiteren sollte eine Stellungsnahme zur Rebestrahlung erfolgen. Evidenzbasierter Review zur Klarung von Indikation, Fraktionierung, Technik von adjuvanter Homogenbestrahlung der Brust und Boost sowie Moglichkeit der Rebestrahlung. Mittels systematischer Literaturrecherche von Pubmed und manueller Suche wurden relevante Publikationen zwischen 1985 (tierexperimentelle Daten) und 2013 untersucht. Der Publikation wurde insbesondere die Metaanalyse der Early Breast Cancer Trialists Collaborative Group (EBCTCG) mit 10.801 Patientinnen aus randomisierten Studien und bis zu 15 Jahren Nachbeobachtungszeit zugrunde gelegt. Nach brusterhaltender Operation ist eine Nachbestrahlung der Brust in allen Subgruppen indiziert. Zusatzlich kann zur Senkung des Lokalrezdivrisikos ein Boost appliziert werden. Langzeitergebnisse weisen darauf hin, dass neben der normofraktionierten Strahlentherapie auch hypofraktionierte Bestrahlungsschemata bei Patientinnen mit niedrigem Ruckfallrisiko (altere Patientinnen ohne lokoregionaren Lymphknotenbefall, die keine Chemotherapie benotigen, mit 15 Jahre fehlen. Rebestrahlungen sind moglich; allerdings sollte ein Zeitabstand von mindestens 6 Monaten eingehalten werden. Grundsatzlich handelt es sich bei der adjuvanten Strahlentherapie nach brusterhaltender Operation um eine gut untersuchte, nebenwirkungsarme Therapie mit der Moglichkeit, das Lokalrezidivrisiko um den Faktor 3 zu senken und mit 4 verhinderten Lokalrezidiven statistisch einen Todesfall zu vermeiden.

Brustkrebs ist mit einer jahrlichen Inzidenzrate von 172.700 die weltweit haufigste Erkrankung der Frau und weltweit fur annahernd eine halbe Million Todesfalle pro Jahr verantwortlich. Am wahrscheinlichsten handelt sich um ein multifaktorielles Geschehen. Zwar machen triple-negative Mammakarzinome nur 20% aller Mammakarzinome aus, sie sind jedoch mit einer sehr schlechten Prognose assoziiert. Die Therapiemoglichkeiten sind im Vergleich zu den hormonabhangigen und Her2-neu exprimierenden Mammakarzinomen sehr eingeschrankt. Triple-negative Mammakarzinome sind eine wenig erforschte, molekularbiologisch, pathologisch und klinisch sehr heterogene Gruppe. Ein Teil zeigt Defekte genetischer Reparaturmechanismen, wie sie ahnlich bei Tumoren mit Keimbahnmutationen des BRCA-1-Gens vorliegen („BRCAness“). Die medikamentose Blockade alternativer Reparaturmechanismen stellt einen zukunftsweisenden Therapieansatz fur die Behandlung von triple-negativen Mammakarzinomen dar.

BACKGROUND An alternative to implant-based breast reconstruction using acellular dermal matrix is the use of a titanium-coated polypropylene mesh. The mesh was approved for implant-based breast reconstruction in Europe in 2008, but only limited clinical data are available. METHODS Two hundred seven patients (231 breasts) with skin-sparing/nipple-sparing or modified radical mastectomy and immediate or delayed implant-based breast reconstruction using titanium-coated polypropylene mesh were evaluated retrospectively. The primary endpoints were identification of patient-related and surgical factors that were predictive for an adverse outcome and the development of recommendations for patients eligible for implant-based breast reconstruction using the mesh. Complications were divided into major (need for additional surgery), minor (conservative treatment), and implant loss. Univariate and multivariate logistic regression analyses were performed to determine the influence of the patient- and procedure-related characteristics on postoperative complications and implant loss. RESULTS No risk factors were observed for patient-associated complications. Major complications occurred in 13.4 percent, minor complications in 15.6 percent, and implant loss in 8.7 percent of patients. Univariate analysis revealed procedure-related risk factors for postoperative complications with a bilateral procedure (p = 0.013) or skin expansion before implant surgery (p = 0.043). Multivariate analysis confirmed these risk factors and revealed an increased risk for implant loss in patients with skin necrosis (p < 0.001) and capsule fibrosis (p < 0.001). CONCLUSIONS This titanium-coated polypropylene mesh shows acceptable complication rates and can be a helpful device in implant-based breast reconstruction. The mesh should only be used in primary cases and, when adhering to the proposed indications, is a safe and convenient option in implant-based breast reconstruction. 112 Background: Intraoperative boost irradiation as part of breast-conserving therapy is a perfect method to adequately capture the high risk tumor relapse area. The most homogeneous dose distribution is achieved with electrons. Intraoperative radiotherapy (IOERT) as a boost for breast cancer releases a high single dose of radiation to the breast tissue; therefore acute toxicity is of particular attention. To date there is only inadequate information available on breast cancer patients treated with IORT using electrons applied as a boost. We therefore analyzed the acute toxicity after radiotherapy with 10 Gy as a boost with a minimum follow-up of 3 months. Methods: A total of 69 patients treated with IOERT (10Gy with 5, 7 and 9 MeV electrons) with a dedicated robotic linac (NOVAC 7, New Radiant Technology, Aprilia, Italy) to the tumor bed during breast-conserving surgery as a boost followed by whole-breast radiotherapy (WBRT, 50.4 Gy; 1.8 Gy per fraction) were included in this study. All patients underwent a retrospective follow-up (median, 8 months; range 1-17 months) regarding acute side effects within the first three months. Toxicities were documented using the common toxicity criteria (CTC 4.0 of the European Organization for Research and Treatment of Cancer). Results: The IOERT was well tolerated. As a side effect there was one patient with seroma. Two patients developed chronic pain in the irradiated breast. Two patients developed a secondary wound healing. The remaining patients did not develop any grade 3 or 4 side effects. The observed toxicity rates were not influenced by age, tubus size, electron energy or systemic therapy. Conclusions: After IOERT of the breast using electrons we did not find any unexpected acute toxicity rates.
