Silicone oil is the one of the artificial materials used in vitreoretinal surgery for retinal detachment treatment. Since the silicone oil is sometimes applied along with intraocular lens (IOL) implantation the direct influence of silicone oil on the artificial implant should be taken into account. Presented study was performed in order to determine the time-dependent impact of silicone oil on hydrogel based ophthalmic materials. Two kinds of IOLs based on hydroxyethyl 2-methacrylate (HEMA) hydrogel material were immersed in silicone oil based on linear poly(dimethylsiloxane) (PDMS). Incubation in oil medium was performed in 37 °C for 1, 3 and 6 months. After appropriate period of the incubation samples were examined by means of FTIR-ATR method as the technique of surface study as well as Positron Annihilation Lifetime Spectroscopy (PALS) as the method of internal structure investigation. Results obtained during the study revealed that silicone oil is not capable to penetrate the internal structure of investigated materials and its impact has come down to interaction with the samples surfaces only.

Introduction Breast cancer in women with cosmetic breast implants is increasingly common. Over the past decade, there has been a push for mastectomy and reconstruction in these patients, based on a fear of poor aesthetic results from small breast volume, and radiation-induced capsular contracture. At the Paris Breast Centre, augmented women routinely undergo lumpectomy with whole-breast irradiation (BCT). Materials and methods A consecutive cohort of 50 augmented women, who had attempted BCT for early breast cancer at our institution between 2003 and 2018, were retrospectively identified. Post-treatment complications, oncologic outcomes, capsular contracture rates, long-term cosmetic outcomes, and patient-reported outcomes were evaluated. Results The median follow-up was 51 months. Margins were involved in 7 women (14%); 4 of whom underwent successful re-excision, and 3 had a mastectomy, for an early mastectomy rate of 6%. There were no early complications, nor cases of early implant loss. Long-term aesthetic results were evaluated using our 5-point scale: An excellent (5), or good (4) result was obtained in 68%. Significant capsular contracture (Baker grade 3 or 4) developed in 34%, of which, 5 women underwent capsulotomy and fat grafting; 4 of 5 downstaging their Baker grade. The estimated 5-year local recurrence rate was 2.3%. Ninety-five percent of participants would recommend BCT to augmented women. Conclusion BCT is feasible and safe in augmented women with good long-term aesthetic results, and should be considered to avoid unnecessary mastectomy.

The paper describes the tDCS method with regards to the effectiveness of its application, which depends on the technical conditions adopted during a stimulation. Transcranial constant current stimulation is a non-invasive, easy and exceptionally available method, however, this does not transfer into it being effective. It turns out that although stimulation can be conducted quickly and relatively simply, its effectiveness depends, to a large extent, on the technical parameters of equipment and the skills of the person who qualifies for, as well as the person who performs the stimulation. The correctness of using the method depends on the appropriately selected technical parameters for each case. The work is based on available current literature.

Item does not contain fulltext Improved performance has been reported for computer aided detection (CADe) methods using information from multiple mammographic views over single-view CADe approaches. Linkage across the views is based on assuming that location and image features from the same lesion depicted in both views will be similar. In this study we investigate if the location features can be improved and what effect such an improvement has on the linkage of lesions across ipsilateral views. Performance of different methods to define the location features was first assessed with respect to the location of 137 manually annotated and linked masses. Taking the median result from five complementary methods (based on pectoral muscle boundary, breast shape and intensity signature) increased the mean accuracy compared to the current standard (7.1 vs. 6.3mm). Thereafter the impact of this best method on the automatic linkage of detected regions across views was assessed for a second, independent dataset of 131 mammogram pairs. Linkage was based on the combination of location and single-view image features by a linear discriminate analysis classifier trained to differentiate between links of corresponding true-positive (TP) regions versus links including TP and false-positive (FP) regions. Nested cross-validation results showed that using the improved location features significantly increased the classification performance and the percentage of correctly linked regions.

OBJECTIVE: Glioblastoma multiforme is the most frequent primary brain tumor in adults. The major obstacle for successful treatment is the invasive growth pattern. Metabolically, glioblastomas are highly glycolytic, leading to increased levels of lactic acid production. The carbonic anhydrase IX (CAIX) moderates the extrusion of hydrogen ions into the extracellular space, which may enhance tumor invasion by activating proteolytic enzymes. We therefore induced glycolysis in glioblastoma cells and investigated the extracellular pH, cathepsin B expression, and its subcellular distribution and secretion parallel to the invasive behavior of the cells upon CAIX knockdown. METHODS: U251 glioblastoma cells were transfected with a CAIX siRNA construct and cultured in a Biocoat Matrigel invasion chambers with an 8-mm pore size membrane. The chambers were incubated in a humified 5% CO2 modular with either 21% oxygen and 25 mM glucose (control) or 0% oxygen plus 125 mM glucose (glycolysis). Invasion was quantified by counting the cells on the lower membrane surface. Extracellular pH was measured using a pH-meter. Cathepsin B expression and localization was investigated by RT-PCR, Western blot, and immunofluorescence staining, respectively. The cathepsin B secretion into the supernatant was measured using a cathepsin B activity assay. RESULTS: In vitro glycolysis caused a significant increase of cathepsin B expression and secretion combined with massive invasion of glioblastoma cells. In addition, the subcellular distribution of the enzyme was shifted to the cell periphery. The extracellular pH dropped significantly under glycolytic conditions, antagonized by CAIX knockdown. In addition, CAIX knockdown did not influence cathepsin B expression but attenuated the intracellular change of cathepsin B distribution and reduced both the cathepsin B secretion and glioma cell invasion. CONCLUSION: Our data demonstrate that CAIX moderates invasion in glycolytic glioma cells via acidification of the extracellular milieu and enhanced secretion of cathepsin B.

Item does not contain fulltext False positive (FP) marks represent an obstacle for effective use of computer-aided detection (CADe) of breast masses in mammography. Typically, the problem can be approached either by developing more discriminative features or by employing different classifier designs. In this paper, the usage of support vector machine (SVM) classification for FP reduction in CADe is investigated, presenting a systematic quantitative evaluation against neural networks, k-nearest neighbor classification, linear discriminant analysis and random forests. A large database of 2516 film mammography examinations and 73 input features was used to train the classifiers and evaluate for their performance on correctly diagnosed exams as well as false negatives. Further, classifier robustness was investigated using varying training data and feature sets as input. The evaluation was based on the mean exam sensitivity in 0.05-1 FPs on normals on the free-response receiver operating characteristic curve (FROC), incorporated into a tenfold cross validation framework. It was found that SVM classification using a Gaussian kernel offered significantly increased detection performance (P = 0.0002) compared to the reference methods. Varying training data and input features, SVMs showed improved exploitation of large feature sets. It is concluded that with the SVM-based CADe a significant reduction of FPs is possible outperforming other state-of-the-art approaches for breast mass CADe.

Studienziel: Seit Mitte der 90er-Jahre ist die Ganzkorper-CT zur Primardiagnostik bei Polytraumatisierten immer mehr in den Vordergrund geruckt, wird jedoch kontrovers diskutiert. Vor allem bei Kindern ist die hohe Strahlenexposition ein Argument, welches immer wieder gegen die CT vorgebracht wird. Ziel unserer Studie war eine Messung der Strahlenbelastung durch eine Ganzkorperspiral-CT im Vergleich zu konventionellem Rontgen in der Polytraumadiagnostik im Tiermodell. Daraus sollte der Stellenwert der CT in der Diagnostik polytraumatisierter Kinder diskutiert werden. Material und Methoden: Um die Strahlenbelastung von Kindern wahrend des Schockraumprotokolles durch konventionelles Rontgen versus Ganzkorper-Spiral-CT zu messen haben wir ein Tiermodell gewahlt. Probanden waren 3 weibliche Hausschweine unterschiedlicher Grose und mit unterschiedlichem Gewicht. Es wurden pro Schwein 4 Filmdosimeter je Untersuchung in diese implantiert; ein Dosimeter wurde auf das Auge geklebt, eines unter die Mamma geschoben, eines transthorakal auf die Brustwirbelsaule gelegt und eines in das kleine Becken in den Douglas-Raum platziert. Zuerst wurden die Schweine konventionell mit dem Standard-Polytraumaprotokoll (Schadel in 2 Ebenen, HWS, BWS und LWS in 2 Ebenen, Thorax a.-p. und Becken a.-p.) gerontgt. Danach wurden die Dosimeter gegen neue ausgetauscht und die Schweine alle im Body-Scan mit einem 64-Zeilen-CT nach unserem speziellen Kinderprotokoll „CT Ganzkorper“ mit Gewichtsadaptation untersucht. Ergebnisse: Als Ergebnis sieht man eine etwa 7-fach hohere Strahlenbelastung durch die Ganzkorper-Spiral-CT-Untersuchung im Vergleich zum konventionellen Rontgenprotokoll fur das Kind. Dagegen steht eine Untersuchungszeit im CT von durchschnittlich 8 min gegen eine Untersuchungszeit von durchschnittlich 18 min im konventionellen Rontgen. Schlussfolgerung: Basierend auf unseren Daten und der Analyse der Literatur wird die Ganzkorper-Spiral-CT-Untersuchung - entsprechend unserem Schockraumalgorithmus fur Erwachsene - weiter das diagnostische Mittel der Wahl bei kreislaufstabilen, intubierten oder bewusstlosen Traumakindern bleiben. Aim: Whole body spiral CT scans have become a routine method in the radiological imaging of severely injured patients in emergency rooms of an increasing number of hospitals. The routine use of CT scans is, however, still discussed controversially, especially with regard to its use in children. This is mainly due to the reportedly higher level of exposure to radiation of CT scans as compared to plain radiographs. The aim of the present study was to compare the dose of exposure to radiation of a whole body CT scan to that of a plain radiograph protocol in an animal model for severely injured children. Material and Methods: We chose 3 female pigs of different weights to serve as a model for children at different ages. 4 film radiation dosimeters (positioned on the eye, under the breast, paravertebrally on the thoracic spine and in the small pelvis, respectively) were implanted into every pig for each examination. Plain radiographs of the chest and the pelvis in one plane and of the skull and the complete spine in two planes were performed. The CT scan included skull, cervical spine and the whole body from the thorax to pelvis. Results: The radiation dose of CT scans was 7 times higher as compared to the X‐ray protocol, but the performance of CT scans was faster (8 vs. 18 min). The radiation dose of the whole body CT scan was at about 15 mSv. Conclusion: Based on our data and a review of the literature we will use whole body spiral CT scans as the preferred method in the primary radiological imaging of severely injured patients. In our opinion, the evident benefit of a reduced time of imaging in combination with superior image information outweighs the higher level of exposure to radiation.

In this work, a classifier combination approach for computer aided diagnosis (CADx) of breast mass lesions in mammography (MG) and magnetic resonance imaging (MRI) is investigated, using a database with 278 and 243 findings in MG resp. MRI including 98 multimodal (MM) lesion annotations. For each modality, feature selection was performed separately with linear Support Vector Machines (SVM). Using nonlinear SVMs, calibrated unimodal malignancy estimates were obtained and fused to a multimodal (MM) estimate by averaging. Evaluating the area under the receiver operating characteristic curve (AUC), feature selection raised AUC from 0.68, 0.69 and 0.72 for MG, MRI and MM to 0.76, 0.73 and 0.81 with a significant improvement for MM (P=0.018). Multimodal classification offered increased performance compared to MG and MRI (P=0.181 and P=0.087). In conclusion, unimodal feature selection significantly increased multimodal classification performance and can provide a useful tool for generating joint CADx scores in the multimodal setting.

Understanding, and accurately being able to predict, breast cancer risk would greatly enhance the early detection, and hence treatment, of the disease. In this paper we describe a new metric for mammographic structure, "orientated mammographic entropy", via a comprehensive classification of image pixels into one of seven basic image feature (BIF) classes. These classes are flat (zero order), slope-like (first order), and maximum, minimum, light-lines, dark-lines and saddles (second order). By computing a reference breast orientation with respect to breast shape and nipple location, these classes are further subdivided into 23 orientated BIF classes. For a given mammogram a histogram is constructed from the proportion of pixels in each of the 23 classes, and the orientated mammographic entropy, Hom, computed from this histogram. Hom, shows good correlation between left and right breasts (r2=0.76, N=478), and is independent of both mammographic breast area, a surrogate for breast size (r2=0.07, N=974), and breast density, as estimated using VolparaTM software (r2=0.11, N=385). We illustrate this metric by examining its relationship to familial breast cancer risk, for 118 subjects, using the BOADICEA genetic susceptibility to breast and ovarian cancer model.

Despite tremendous advances in modern imaging technology, both early detection and accurate diagnosis of breast cancer are still unresolved challenges. Today, a variety of imaging modalities and image-guided biopsy procedures exist to identify and characterize morphology and function of suspicious breast tissue. However, a clinically feasible solution for breast imaging, which is both highly sensitive and specific with respect to breast cancer, is still missing. As a consequence, unnecessary biopsies are taken and tumours frequently go undetected until a stage where therapy is costly or unsuccessful. Currently, the exact diagnosis of suspicious breast tissue is ambiguous in many cases. To resolve this, computer aided diagnosis methods are developed which use knowledge extracted from large multimodal case databases. Clinical workstations must be developed to allow clinicians to use additional image modalities in an optimal way. Dedicated tools are required to guide clinicians in establishing a diagnosis, and should ultimately lead to more specific and accurate diagnostic decisions.

Question Does repeated anodal transcranial direct current stimulation (A-tDCS) of the left dorso-lateral prefrontal cortex (DLPFC) enhance cognitive rehabilitation effects on memory and attention in patients with traumatic brain injury (TBI). Methods Twenty-three adult patients, 4–92 months post severe TBI, were randomly allocated to two groups. The experimental group received A-tDCS (10 min; 1 mA; in the DLPFC), followed by 1-h cognitive training, daily for 15 days. Controls received A-tDCS for 25 s (sham condition) with the same rehabilitation programme. Effects of the treatment were assessed using a battery of memory and attention tests (Rey’s Auditory Verbal Learning Test, Paced Auditory Serial Addition Test, Cambridge Neuropsychological Test Automated Battery), which included visual and auditory modalities. Additionally, European Brain Injury Questionaire was used to assess functional gains in daily activities. Participants were tested twice before beginning rehabilitation (to control for spontaneous recovery), after rehabilitation completion, and four months later. Results Tests scores in both groups were similar at three weeks before and immediately before treatment. After treatment, the experimental group exhibited larger effect sizes in six out of eight cognitive outcome measures, but they were not significantly different from controls. At follow-up, experimental group’s effect sizes were higher in five outcome measures. However, differences remained insignificant. No serious adverse effects were observed during the programme. Conclusion In contrast to previous studies, our study did not provide sufficiently strong evidence to support the efficacy of repeated A-tDCS over the left DLPFC for enhancing rehabilitation of memory and attention in patients after severe TBI. However, effect sizes suggested a positive response to stimulation.

The purpose of the present study was to investigate the possibility that the low‐frequency leakage of sound from beneath supra‐aural earphone cushions increases as adults age, giving the appearance of a low‐frequency hearing loss. The subjects were four groups (n=6) defined by age (younger than 30 and older than 65 years) and gender. The probes for ER‐7C microphones were placed 0.5 in. into each of a subject’s ear canals. Continuous pure tones at 11 1/3‐oct frequencies (100–1000 Hz) were presented via TDH‐39 headphones mounted in MX‐41/AR cushions. Tones were presented at 80 dB SPL. The procedure was replicated once. The variable analyzed was the difference between sound pressure level measured in the ear canal and that measured in an NBS 9A coupler. The low‐frequency leak was present in our results in that the average leak increased at a rate of 12 dB/oct, beginning at 400 Hz. The slope of the leak, however, was different for right and left ears and was different for the four age‐by‐sex groups. These results strongly suggest that caution be used in interpreting sensitivity differences obtained with supra‐aural earphones. [Work supported by DC00174. The unidirectional fluxes of 20, 100, 500, and 2,000 microM rho-aminohippurate (PAH) were measured under open- and short-circuit conditions in canine tracheal epithelium mounted as flat sheets in Ussing chambers. In tissues pretreated with mucosal indomethacin (10(-6) M) and amiloride (10(-4) M), unidirectional PAH fluxes under short-circuit conditions increased with increasing bath concentrations but there was no significant net PAH transport. After stimulation of chloride secretion by mucosal cyclic adenosine 3′,5′ -cyclic monophosphate (cAMP 10(-3) M), there was a significant increase in the secretory flux of PAH and a significant decrease in the absorptive flux of PAH. This resulted in net PAH secretion that demonstrated saturation kinetics with an apparent Michaelis-Menten constant of 754 microM by Lineweaver-Burk analysis. Intracellular concentrations of PAH were 0.4–1.2 times bath concentrations after pretreatment with indomethacin and amiloride and increased to 2.6–3.3 times bath concentrations after cAMP. Under open-circuit conditions, secretory PAH flux decreased and absorptive flux increased resulting in net PAH absorption. We conclude from these early studies that the canine tracheal epithelium possesses a specialized system for the transport of organic anions in the airways and that this transport system may share many similarities with organic anion transport in the kidney.

Presently submitted and previously available data indicate that the recommended calcium requirement of 800 mg/day is inadequate for a large segment of the population, particularly the elderly. With an intake of 800 mg of calcium/day, calcium balance was only slightly positive, without consideration of dermal losses or other risk factors that may play a role in calcium bioavailability and calcium retention. A calcium intake of 1000-1200 mg/day is preferable, as calcium balance increased significantly when calcium intake was increased from 800 to 1200 mg/day; there was no further improvement in calcium balance on higher calcium intakes, up to 2300 mg/day.
