To assess the association between smoking, anti-cyclic citrullinated peptide (anti-CCP) antibody status, and clinical efficacy of biological therapies in rheumatoid arthritis (RA) patients.This retrospective clinical practice setting study included 1349 RA patients from the METEOR database (aged18 years). We collected data on sociodemographics, smoking status (smoker,10, 10-19, and20 cigarettes/day; ex-smoker; non-smoker), baseline disease activity parameters and anti-CCP, previous disease-modifying anti-rheumatic drugs (DMARDs), biological therapy, combined therapy (steroids and DMARDs), and follow-up disease activity. Clinical efficacy was assessed by European League Against Rheumatism (EULAR) good/moderate response rates for all aggregated biological therapies, based on both smoking and anti-CCP status.The non-smoking RA patients were more often female at biological therapy initiation than the ex-smokers and smokers (91.1% vs 60.4% and 67.9%, respectively, p 0.001), and ex-smokers were older than non-smokers and smokers (mean ± sd 56.5 ± 11.1, 53.5 ± 13.3 and 51.3 ± 11.0 years old, respectively; p 0.001). In total, 845 (62.6%) were non-smokers, 214 (15.9%) ex-smokers, and 290 (21.5%) smokers [daily cigarettes smoked: 148 (11%)11; 61 (4.5%) 11-20; and 81 (6%)20]. Anti CCP-antibody status was similar in both groups. Non-smokers showed higher baseline DAS28 than ex-smokers and smokers (5.0 ± 1.5 vs 4.7 ± 1.4 and 4.7 ± 1.4, respectively; p 0.001) and used more baseline steroids and DMARDs. A higher EULAR response rate was observed in non-smokers than in ex-smokers and smokers (73% vs 65% and 64.1%, respectively; p = 0.004). Drug survival was higher in non-smokers compared to ex-smokers and smokers [57.7 months (46.4-53.8), 38.6 (30.3-46.8), and 50.1 (41.8-58.4); p0.001, respectively].In daily clinical practice, non-smokers respond better than smokers to biological therapy, but this does not result in better drug survival.

Background:Managing patients with chronic rheumatic diseases is increasingly relying on the collaboration of a multidisciplinary team, including the rheumatology nurse who can aid assessment of disease activity among other healthcare tasks.However, this practice, although common in Europe and North America, is still poorly implemented in the Middle eastern Arab countries (MEAC).Objectives:To evaluate the concordance between the physician’s and the nurse’s assessment of disease activity in patients with rheumatoid arthritis (RA) in the MEAC.Methods:During a routine patient visit to one of 9 rheumatology clinics located in 7 Middle Eastern Arab countries, the physician and the rheumatology nurse (who was already working in the clinic) performed the disease activity score 28 (DAS), Clinical Disease Activity Index (CDAI) and Simple Disease Activity Index (SDAI) in a blinded manner. The concordance regarding the 3 continuous scores was calculated using paired t-test. The agreement between physician- and nurse-DAS categories (remission, low, moderate and high disease activity) was calculated using weighted kappa for category comparison. Predictive factors of positive concordance between physician- and nurse-DAS were identified using binary logistic regression.Results:The study included 373 patients’ measurements over a period of two years (2018 to 2019). The mean age of the patients was 49.6 years (±28.2), 82.6% were females, and the mean disease duration was 11.3 years (±7.5).The mean physician-DAS was slightly higher (3.97 (±1.52)) than the nurse-DAS (3.90 (±1.54)) (p=0.002), with a mean difference of +0.08 [95%CI 0.03; 0.12] (Table 1). Background:An increased risk of severe COVID-19 outcomes may be seen in patients with autoimmune diseases on moderate to high daily doses of glucocorticoids, as well as in those with comorbidities. However, specific information about COVID-19 outcomes in SLE is scarce.Objectives:To determine the characteristics associated with severe COVID-19 outcomes in a multi-national cross-sectional registry of COVID-19 patients with SLE.Methods:SLE adult patients from a physician-reported registry of the COVID-19 GRA were studied. Variables collected at COVID-19 diagnosis included age, sex, race/ethnicity, region, comorbidities, disease activity, time period of COVID-19 diagnosis, glucocorticoid (GC) dose, and immunomodulatory therapy. Immunomodulatory therapy was categorized as: antimalarials only, no SLE therapy, traditional immunosuppressive (IS) drug monotherapy, biologics/targeted synthetic IS drug monotherapy, and biologic and traditional IS drug combination therapy. We used an ordinal COVID-19 severity outcome defined as: not hospitalized/hospitalized without supplementary oxygen; hospitalized with non-invasive ventilation; hospitalized with mechanical ventilation/extracorporeal membrane oxygenation; and death. An ordinal logistic regression model was constructed to assess the association between demographic characteristics, comorbidities, medications, disease activity and COVID-19 severity. This assumed that the relationship between each pair of outcome groups is of the same direction and magnitude.Results:Of 1069 SLE patients included, 1047 (89.6%) were female, with a mean age of 44.5 (SD: 14.1) years. Patient outcomes included 815 (78.8%) not hospitalized/hospitalized without supplementary oxygen; 116 (11.2) hospitalized with non-invasive ventilation, 25 (2.4%) hospitalized with mechanical ventilation/extracorporeal membrane oxygenation and 78 (7.5%) died. In a multivariate model (n=804), increased age [OR=1.03 (1.01, 1.04)], male sex [OR =1.93 (1.21, 3.08)], COVID-19 diagnosis between June 2020 and January 2021 (OR =1.87 (1.17, 3.00)), no IS drug use [OR =2.29 (1.34, 3.91)], chronic renal disease [OR =2.34 (1.48, 3.70)], cardiovascular disease [OR =1.93 (1.34, 3.91)] and moderate/high disease activity [OR =2.24 (1.46, 3.43)] were associated with more severe COVID-19 outcomes. Compared with no use of GC, patients using GC had a higher odds of poor outcome: 0-5 mg/d, OR =1.98 (1.33, 2.96); 5-10 mg/d, OR =2.88 (1.27, 6.56); >10 mg/d, OR =2.01 (1.26, 3.21) (Table 1).Table 1.Characteristics associated with more severe COVID-19 outcomes in SLE. Background:Acute Respiratory Distress Syndrome (ARDS) is a life-threatening complication of COVID-19 and has been reported in approximately one-third of hospitalized patients with COVID-191. Risk factors associated with the development of ARDS include older age and diabetes2. However, little is known about factors associated with ARDS in the setting of COVID-19, in patients with rheumatic disease or those receiving immunosuppressive medications. Prediction algorithms using traditional regression methods perform poorly with rare outcomes, often yielding high specificity but very low sensitivity. Machine learning algorithms optimized for rare events are an alternative approach with potentially improved sensitivity for rare events, such as ARDS in COVID-19 among patients with rheumatic disease.Objectives:We aimed to develop a prediction model for ARDS in people with COVID-19 and pre-existing rheumatic disease using a series of machine learning algorithms and to identify risk factors associated with ARDS in this population.Methods:We used data from the COVID-19 Global Rheumatology Alliance (GRA) Registry from March 24 to Nov 1, 2020. ARDS diagnosis was indicated by the reporting clinician. Five machine learning algorithms optimized for rare events predicted ARDS using 42 variables covering patient demographics, rheumatic disease diagnoses, medications used at the time of COVID-19 diagnosis, and comorbidities. Model performance was assessed using accuracy, area under curve, sensitivity, specificity, positive predictive value, and negative predictive value. Adjusted odds ratios corresponding to the 10 most influential predictors from the best performing model were derived using hierarchical multivariate mixed-effects logistic regression that accounted for within-country correlations.Results:A total of 5,931 COVID-19 cases from 67 countries were included in the analysis. Mean (SD) age was 54.9 (16.0) years, 4,152 (70.0%) were female, and 2,399 (40.5%) were hospitalized. ARDS was reported in 388 (6.5% of total and 15.6% of hospitalized) cases. Statistically significant differences in the risk of ARDS were observed by demographics, diagnoses, medications, and comorbidities using unadjusted univariate comparisons (data not shown). Gradient boosting machine (GBM) had the highest sensitivity (0.81) and was considered the best performing model (Table 1). These results identify patients who may be at higher risk of severe COVID-19 outcomes. Further studies are necessary to validate the proposed prediction model in external cohorts and to evaluate its clinical utility. Background:Involving the patients with rheumatoid arthritis (RA) in the assessment of their disease may increase their adherence to treatment, improve the disease outcomes and facilitate the application of telehealth. We previously reported an excellent concordance between the Disease Activity Score (DAS-28) performed by physicians and patients at the baseline visit of this prospective study (1).Objectives:To evaluate the persistence of the concordance between the physician’ and the patient’s assessment of disease activity in RA using DAS-28 after one year.Methods:At the baseline visit, patients with RA from 7 Middle Eastern Arab Countries (MEAC) were briefed about DAS-28 by their rheumatologist during a routine consultation and given smartphone access to a video in Arabic language explaining the performance of DAS-28. At 3, 6 and 12 months (± 3 months), the patients were asked to self-report DAS-28, blinded to the physician’s assessment. Concordance between the continuous DAS-28 at each visit was calculated using paired t-test numerically and the Bland-Altman method graphically. Agreement between physician- and patient-DAS categories (remission, low-, moderate- and high disease activity) was calculated at each visit using weighted kappa for category comparison. Weighted kappa of the different agreements were compared over time using their respective confidence intervals (CIs). Predictive factors of positive concordance between physician and patient-DAS were identified using binary logistic regression.Results:The study included 428 patients over a period of three years (2018 to 2020). The mean age of participants was 49.8 years, 82.5% were females, 44.3% had a university degree and the mean disease duration was 11.4 years.At baseline, the average patient-DAS was higher (4.06 (±1.52)) than the physician-DAS (3.97 (±1.52)). The mean difference was -0.09 [95%CI -0.14; -0.04] and most of the pairs were within the limit of agreement in the Bland-Altman graph, indicating a good concordance, particularly in cases of remission.During the study follow-up, 299 patients consulted for visit 2 (69.9% of the total population), 232 for visit 3 (54.2%) and 199 for visit 4 (46.5%). The weighted kappa was 0.80 [95%CI 0.76;0.85] at visit 1 and 0.79 [95%CI 0.72;0.88] at visit 4 (Figure 1 showing kappa for DAS-28, CDAI and SDAI as well). A minor numerical decrease in kappa was observed over time; however, the CIs were overlapping over the four visits and the agreement was considered stable, remaining in the excellent range. At visit 4, a positive concordance between the physician- and the patient-DAS was associated with the profession (lower in blue collar, p=0.001), the educational level (higher in high school and university, p=0.034) and the baseline physician’s DAS (higher in high disease activity, p=0.46).Conclusion:The agreement between the DAS-28 performed by the physician and by the patient was excellent at baseline and remained stable over one year. A positive concordance was associated with the profession, the educational level and the level of disease activity. Background:Rheumatoid arthritis is implicated in causing adverse pregnancy outcomes including high rates of prematurity and low birth weight. But little is known about the impact of the disease when it’s controlled as most of the information is extracted from retrospective data.Objectives:To examine the adverse obstetric outcomes after controlling disease during pregnancy. We also took into account many confounders that might affect the outcome.Methods:This is an ongoing Case-Control Prospective Cohort. It is implemented in a tertiary center where cases are recruited from a single specialized pregnancy and rheumatic disease clinic to ensure standardized management. These cases were fulfilling the ACR 2010 classification criteria for rheumatoid arthritis. Disease activity was measured using CDAI once before pregnancy and once in each trimester. We excluded subjects with chronic morbidities or twin pregnancy. Data were collected in pre-specified data sheets. Routine blood tests in addition to C-reactive protein were obtained. Cases were recruited at different disease activity stages, but treatment was escalated to reach remission as possible by the third trimester. Data were analyzed using SPSS software for descriptive and comparative analyses.Results:Since 2017 we have recruited 215 subjects. A total of 190 completed pregnancies were analyzed in this report (114 controls and 76 cases). Five subjects were excluded as their disease was not controlled by 27 weeks of gestation. Baseline characteristics of age, baseline BMI and anemia were similar. Exposure to passive smoking was significantly higher in the control group. There was no statistical difference in the incidence of gestational diabetes, pre-eclampsia and infections. Rates of abortions and cesarean sections were significantly higher in the cases group. The incidence of PROM & low birth weight was not statistically different. Three cases of IUFD were reported among controls versus none in the cases (Table 1). Prematurity rate was numerically higher in the control group but did not reach a statistical difference. Congenital anomalies and NICU admission rates were comparable between the groups. But the incidence of neonatal morbidities was significantly higher in the control group (p. value 0.006), but the majority of morbidities were due to jaundice that resolved with phototherapy. we have evaluated the incidence of group B streptococcal Agalactae as a possible contributor to morbidities but it was similar between the groups. All cases were on DMARDs during pregnancy. Hydroxychloroquine was the most commonly used (55%) followed by sulfasalazine (40%). Steroid was used for variable duration in pregnancy in 23 cases. In most of them, it was tapered and stopped by the end of pregnancy. Background Using a treatment target in rheumatoid arthritis (RA), such as DAS-28 and involving patients in their disease management can improve prognosis. Objectives To develop and implement an educational video that instructs patients with RA for self-assessment of disease activity using DAS-28. Methods Step 1:Initiation. Rheumatologists from some Middle Eastern Arab countries were invited to participate in this study. International experts and societies were contacted to access pre-existing educational material. Step 2: Development. Different material sources were synthetized and translated into Arabic and validated by the rheumatologists. The educational video was shot by a professional team with a real patient and a rheumatologist on set. Step 3. Validation. The final products (leaflets and video) were validated by the rheumatologists and tested with patients (figure 1). Obstacles were noted at the various study levels. Results 23 rheumatologists from 7 Middle Eastern Arab countries participated in the study. A one-page educational leaflet was developed in Arabic and English languages. An educational video presenting the treat-to-target concept and the basics of DAS performance was produced in Arabic language with English subtitles. Obstacles at the rheumatologist, patient, cultural and logistic levels were identified and the potential solutions were addressed by the study team. Conclusions Collaboration between several countries sharing the same language and similar cultural backgrounds was possible and able to produce an educational video aiming at the empowerment of RA patients for the self-assessment of their disease activity. The video will serve for future studies in the Arabic-speaking countries and will be available later for clinical use according to the rheumatologist’s clinical judgment. References [1] Smolenet al. Background Comorbid conditions are frequently associated with rheumatoid arthritis (RA) which not only increase morbidity but also has impact on treatment and may shorten the life span of those patients. Objectives To study the prevalence of different comorbid conditions among patients with RA. Methods Data was collected from rheumatoid arthritis Registry in Qatar. Patients fulfilling 2010 ACR/EULAR criteria for RA were included in this observational study from period of June 2013 to November 2015. Data about Baseline demographics, treatment pattern, disease scores and details of comorbid conditions were recorded in this observational study Results Data of 496 patients was analyzed. Demographic and disease characteristic of our RA cohort was as follows: 75.8% were female, 74.9% were positive for rheumatoid factor, 79.8% were positive for anti CCP and 31.3% have erosion at the time of data collection. One hundred thirty patients (26.4%) were receiving biologic drugs, 71.8 were on synthetic DMARDS (either as monotherapy or different combination) and 39.7% were receiving concomitant steroid. The most commonly associated comorbid conditions were hypertension (24.2%) followed by diabetes mellitus (20.6%), dyslipidemia 10.9%, hypothyroidism 10.9%, asthma and chronic obstructive disease 1.4%. Steroid was used by 39.2% of patients with diabetes and hypertension. Cardiovascular events (Ischemic heart disease and ischemic stroke) occurred in 2.2% of patients and 45.5% of patients with cardiovascular events were receiving concomitant steroid. Infections requiring hospital visit were recorded in 1.8% of patients; 77.8% of patients with infection were on biologic DMARD and 33.3% were receiving concomitant steroid. Out of Two hundred seventy six patients who underwent DXA scanning for estimation of bone mineral density, 48.6% were having decrease bone density (37% osteopenia, 11.6% osteoporosis). Steroid use was significantly associated with decrease bone density. Conclusions Comorbid conditions are frequently associated with Rheumatoid arthritis as observed in our cohort of patients. Patient care should not be focused only on arthritis care. All RA patients should be screened for comorbidities and treated accordingly in order to avoid their deleterious effect on patient health. Background To date, there is no rheumatoid arthritis registry in Middle East. Rheumatoid arthritis registry in Qatar was established in 2013 with the aim of collecting data on RA patients seen in routine care at Hamad General Hospital, Qatar. It is observational study collecting data about demographic features of RA, RF and Anti CCP status, type of medication and their adverse reactions. CDAI, SDAI, DAS28, RAPID3 and HAQ questionnaire are also recorded. Objectives To describe demographic, disease activity score and treatment pattern in rheumatoid arthritis patients in Qatar. Methods All consecutive rheumatoid arthritis patients who were attending routine rheumatology outpatient clinic at Hamad General Hospital from period of June 2013 to September 2015 were included in this study. Results Data of 496 patients were analyzed: 75.8% female, 54.2% Arabs, 45.8% non Arabs, 74.9% were positive for rheumatoid factor, 79.8% were positive for anti CCP and 31.3% have erosion at the time of data collection. One hundred thirty one patients (26.4%) were receiving biologic drugs: (9.3% etanercept, 6% rituximab, 5.6% tocilizumab, 4.4% adalimumab, 0.4% abatacept, 0.4% certolizumab, and 0.2% infliximab). Among patients who are receiving biologic therapy, 42 patients (32%) are on biologic monotherapy. (9.9% on rituximab monotherapy, 9.9% on tocilizumab, 6.9% on etanercept and 5.3% on adalimumab monotherapy). Similarly, 49.23% of those patients receiving biologic, are also using concurrent methotrexate. Methotrexate (MTX) was the most commonly prescribed synthetic DMARD (65.3%); 27% MTX monotherapy, 9.9% combined MTX and hydroxychloroquine, 6% combined MTX and sulfasalazine, 4.8% combined MTX and leflunomide and 3.6% triple therapy. Hydroxychloroquine was used by 29.4% of patients (4.6% patients were only on hydroxycholoquine monotherapy). Sulfasalazine was used by 21.2% patients (3.2% were only on sulfasalazine monotherapy). Leflunomide was used by 13.7% of patients (3.4% were only on leflunomide monotherapy). Good number of patients (39.7%) was receiving corticosteroid in our cohort. Mean dose of methotrexate, sulfasalazine and prednisolone was 15.88 ± 4.98 mg per week, 1.64 ± 0.57 gram daily and 5.45 ± 1.81 mg per day respectively. Different disease activity scores in our cohort were as follows: DAS28 (4) 3.2 ± 1.2, CDAI 8.5 ± 7.9 and SDAI 9.7 ± 9.1. Mean HAQ score was 0.9 ± 0.7. Disease activity categorized by these indices differs considerably; 18% and 17.5% patients were in remission according to CDAI and SDAI scores respectively while 30.3% patients were in DAS 28 remission. Similarly, 23.1% patients were in low disease activity according to DAS28 (4) score while 51.6% patients were in low disease activity according to CDAI and SDAI scores. Conclusions This observational study describes a cohort of RA patients seen at Hamad General Hospital, Doha Qatar. Treatment pattern is somewhat similar to what is reported worldwide. One fourth of patients are receiving biological DMARD. Approximately two third of patients are categorized as having low disease activity or in remission. Mesenteric cysts are uncommon and mesenteric chylous cysts are extremely rare. The most common presenting symptoms are abdominal pain and abdominal distention. We report two cases of mesenteric chylous cyst in a young man and a young woman. Pre-operative, intra-operative and histological findings are discussed.

Background: The currently available TNF blocking agents include ifliximab, etanercept and adalimumab. They are approved for the treatment of rheumatoid arthritis, ankylosing spondylitis, and psoriatic arthritis in the United States and Europe. Also TNF blockers are used currently in other rheumatic and non-rheumatic disease. Objective: Primary objective: To evaluate our practice of using TNF blockers in the outpatient rheumatology clinics at Hamad Medical Corporation in Qatar, to evaluate the response rate to TNF blockers, and the rate of switching from one TNF blockers to another. Secondary objective: to assess rate of serious infection and TB. Methods: The study involved a retrospective chart review of 82 patients in rheumatology outpatient clinic since the introduction of TNF-Blockers at Hamad Medical Corporation in 2002 till December of2005. Results: Twenty two patients out of 82 received Infliximab (26.8%), 51 patients received etanercept (62.1%) and 22 patients received adalimumab (26.8%) including switchers and non switchers. Response rate was 82% to initial TNF blocker in RA, and almost 100% in Psoriatic arthritis and ankylosing spondylitis. Reason of switching was either due to no response or side effects. No significant serious adverse events in our cohort and no TB reactivation. Conclusion: Our data are comparable with the international guidelines; regarding the use of TNF blockers in rheumatic disease.also they reflect the daily practice in the rheumatology outpatient clinic in Qatar.

We report two patients with pachydermoperiostosis. Both patients presented with pain and swelling of the large joints, characteristic facial features, clubbing and enlargement of distal parts of the extremities.

Pregnancy in women with rheumatic disorders is known to be associated with risks for both the mother and fetus; however, these risks can be minimized with proper planning and careful management of the disease. In the Middle East, there are specific cultural challenges that may have a negative impact on the care that women with rheumatic disorders receive. There is a need for cross-collaboration between specialist physicians, improved awareness of rheumatic disorders among the general public and more open discussion with patients about the potential complications of pregnancy. Women in the region are often unwilling to discuss their disease with their partner and are even less likely to seek advice regarding family planning from their physician. The objective of this review is to highlight the specific challenges of pregnancy management and to discuss why establishing specialist pregnancy clinics for women with rheumatic disorders could be an effective solution. Such clinics can provide high quality care before, during and after pregnancy as shown in several European and US centers. Additionally, such clinics could be useful for the collection of pregnancy outcomes data from the Middle East, which may currently be lacking in the region, in order to highlight where further improvements can be made. With specialist care and analysis of pregnancy outcomes, the standard of care for women with rheumatic disorders in this area could be significantly improved.
