Objective. To estimate the direct costs associated with the diagnosis, treatment, and control of measles cases in Brazil from 2018 to 2020. Methods. This cost-of-illness study utilized a prevalence-based approach, considering direct costs incurred by the Brazilian Public Health System (SUS) related to measles outbreaks, including costs of inpatient care, outpatient care, and laboratory tests, as well as measles-containing vaccines and laboratory tests (viral isolation) used for outbreak control. Costs are presented in 2020 US dollars. Univariate and bivariate sensitivity analyses were performed. Results. There were 36 236 confirmed measles cases from 2018 to 2020. The estimated outbreaks cost was USD 107 960 122, with the cost per case ranging from USD 2 601 to USD 3 654 (mean USD 2 979). Conclusions. These findings highlight the substantial economic burden imposed by measles outbreaks in Brazil and emphasize the importance of measles prevention and control measures. Policymakers and public health authorities can use these results to plan and allocate resources, to mitigate the economic impact of future outbreaks.

Objetivo. Descrever a implementação e a atuação pioneira do comitê de farmacovigilância em vacinas do estado de São Paulo, Brasil, na análise de eventos supostamente atribuíveis à vacinação ou imunização (ESAVI) no período da pandemia de covid-19. Método. Trata-se de um estudo de caso retrospectivo, qualiquantitativo, com enfoque na atuação do comitê de farmacovigilância nos anos de 2021 e 2022. A partir dos registros das reuniões do comitê, foi descrito o modo de funcionamento do comitê e contabilizados os casos de ESAVI discutidos e resolvidos nas reuniões de especialistas. O número de doses de vacinas aplicadas no período de interesse, assim como o número de ESAVI notificados nesse período, foi obtido de sistemas de informação estaduais. Resultados. Em 55 encontros realizados em 2021 e 2022, foram discutidos 118 casos de ESAVI nas reuniões do comitê de farmacovigilância, majoritariamente relacionados às vacinas de covid-19. Contabilizou-se a aplicação de 126 778 252 doses de vacinas de covid-19 nesse período, tendo havido 42 893 notificações de ESAVI. Entre os casos elencados para discussão, 71,2% ocorreram após a primeira dose da vacina, 32,8% representaram eventos neurológicos e apenas 5% dos casos apresentaram associação causal com os imunizantes. Conclusão. O comitê de farmacovigilância teve um papel importante no sistema de vigilância de ESAVI no período analisado, auxiliando na avaliação de casos de maior complexidade, com potencial para contribuir positivamente no fortalecimento do programa de imunização, em especial no âmbito do sistema de vacinação segura.

Abstract Introduction Determining the VDO is one of the most important requirements in restorative dentistry, as it is crucial for good aesthetic and functional harmony. However, due to its empirical nature, a combination of methods is clinically recommended, and cephalometric analysis is one of the procedures available. Objective The objective of this study is to evaluate the coherence between VDO clinically established with VDO determined in Seraidarian-Tavano cephalometric analysis in completely edentulous patients. Material and method Registrations were carried out on completely edentulous patients using a combination of clinically recommended methods (metric method from VDR, swallowing method, and facial fullness method). With the registration in the mouth, lateral cephalometric X-ray images were taken with the patient in occlusion for cephalometric analysis. Result The sample consisted of 18 patients, 11 females (61.1%) and 7 males (38.9%) with a mean age of 65.8 ± 1.9 years. For the 18 patients, the mean measurements of the upper and middle angles were 54.1 and 54.0, respectively. No statistically significant difference was found between the two angles (p 0.772). These results show that the VDO considered ideal from a clinical point of view was confirmed by the cephalometric analysis used. Conclusion Based on the data, it seems fair to conclude that the tested cephalometric analysis can be a method used to assist the clinical determination of VDO.

Abstract COVID-19 pandemic brought chemosensory impairment to the forefront of medicine, revealing gaps in the knowledge of pathophysiological mechanisms, true prevalence and preventive/therapeutic alternatives. This is a sub-study of the ORCHESTRA cohort focusing on post-COVID-19 chemosensory symptoms. Risk factors for neurosensorial cluster of post-COVID-19 syndrome (NSc-PCS) were assessed through multivariable analysis. Psychophysical validated tests were applied on a sub-population of 50 patients. Qualitative chemosensory symptoms as well as nasal and oral chemesthesis were evaluated through anamnestic interview and the quality of life through the SF-36 questionnaire. Chemosensory symptoms evolution and olfactory training’s outcome were assessed through phone-call interviews. Background: This study aimed to conduct a narrative review approaching the effects of exclusive breastfeeding on neuropsychomotor development. The goal was to provide evidence-based knowledge to inform healthcare practices and policies and promote optimized infant feeding strategies. Methods: Our study reviewed the relevant literature from May and June 2024, covering the publication period between 2013 and 2024. The PubMed database was utilized and searched for articles using keywords such as “Brain”, “Growth”, “Development”, and “Breastfeeding”, employing Boolean operators such as “AND”, “OR”, and “NOT.” Results: Our search initially screened 15,412 studies, resulting in 600 articles. Eleven studies met our inclusion criteria and provided relevant information on the topic. Several studies have shown that exclusive breastfeeding and its duration are beneficial for neural development. Research suggests that breastfeeding improves brain architecture, white matter development, and cognitive performance. Additionally, studies indicate that the mother’s intake of omega-3 fatty acids can enhance infant brain development, and specific micronutrients in breast milk, such as myo-inositol, may contribute to neural connectivity. Some findings also suggest that the child’s sex may play a role in how breast milk benefits the brain. Furthermore, there is evidence of the strong influence of epigenetic compounds on the neurodevelopmental benefits of exclusive breastfeeding. Conclusions: This narrative review revealed findings that indicate breast milk has a positive impact on brain development. This emphasizes that breast milk has a positive impact on brain development. It underscores the importance of conducting additional research to understand how breastfeeding specifically influences neurodevelopment.

Purpose: In incomplete spinal cord injury (SCI), there is a partial decrease in motor or sensory or autonomic function. Mainly due to the motor impairment in SCI, a muscle–machine interface is a tool that can bring functional benefits to this population. Objective: To investigate the feasibility of the non-invasive myoelectric signal–functional electrical stimulation (MES-FES) interface on the response of the quadriceps muscle in an individual with incomplete SCI. Methods: This is a quasi-experimental, uncontrolled, longitudinal case report study carried out with an individual with incomplete SCI in the chronic phase. The assessments performed before (pre) and after eight (post8) interventions were neuromuscular assessment (surface electromyography (EMG) in rectus femoris (RF) and vastus lateralis (VL) muscles); muscle strength (load cell); knee extension range of motion (goniometry); spasticity (Modified Ashworth Scale); and quality of life (Spinal Cord Injury Quality-of-Life Questionnaire (SCI-QoL.Br)). The MES-FES interface was associated with physical therapy exercises on the extension knee joint muscle group. Results: Improvement in neuromuscular activation (normalized increase in EMGRMS of 2% (RF) and 3.3% (VL)) and synchronism of the motor units (normalized reduction in EMGMDF of 22.8% (RF) and 5.9% (VL)); 1.4 kgf increase in quadriceps strength; 10.6° increase in knee joint extension amplitude; 1 point spasticity reduction; improved quality of life, confirmed by a 12-point reduction in the SCI-QoL.Br score. Moreover, along with interventions, the participant increased the correct FES activation rate, indicating a user learning curve (ρ = 0.78, p-value = 0.04). Conclusions: The MES-FES interface associated with physical therapy promotes neuromuscular and quality of life improvements in the SCI participant.

Abstract In this study, we propose an approach to detect deception during investigative interviews by integrating response latency and error analysis with the unexpected question technique. Sixty participants were assigned to an honest (n = 30) or deceptive group (n = 30). The deceptive group was instructed to memorize the false biographical details of a fictitious identity. Throughout the interviews, participants were presented with a randomized sequence of control, expected, and unexpected open-ended questions about identity. Responses were audio recorded for detailed examination. Our findings indicate that deceptive participants showed markedly longer latencies and higher error rates when answering expected (requiring deception) and unexpected questions (for which premeditated deception was not possible). Longer response latencies were also observed in participants attempting deception when answering control questions (which necessitated truthful answers). Moreover, a within-subject analysis highlighted that responding to unexpected questions significantly impaired individuals’ performance compared to answering control and expected questions. Leveraging machine-learning algorithms, our approach attained a classification accuracy of 98% in distinguishing deceptive and honest participants. Additionally, a classification analysis on single response levels was conducted. Our findings underscore the effectiveness of merging response latency metrics and error rates with unexpected questioning as a robust method for identity deception detection in investigative interviews. We also discuss significant implications for enhancing interview strategies.

The market placing of a medical device in compliance with the requirements of EU Regulation 2017/745 (Medical Device Regulation) demands advanced regulatory expertise and a high level of detail and depth, inevitably leading to significant human and time resources. In an era where Artificial Intelligence (AI) is already present in various aspects of daily life, the potential and opportunity to use AI tools in the scientific field, such as the CE marking process of medical devices, are being explored. This process consists of several phases and related activities, some of which have been chosen as significant examples to evaluate how and to what extent AI can add value in achieving their compliance. The article presents the overall results in terms of performance and reliability derived from generative AI tests using Large Language Models, such as ChatGPT, applied to some of the processes necessary for the market placing of a medical device. The method used focuses on the relationship between prompt quality and output quality, demonstrating the importance of prompt engineering in using these tools effectively alongside regulatory processes. It also emphasizes the need for end-users to have education, training, and understanding of the mechanisms of generative AI to optimize performance.

ABSTRACT Objective To characterize laparoscopy teaching in Medical Residency Programs in Gynecology and Obstetrics in Brazil, and to evaluate preceptors’ characteristics in laparoscopy programs and map laparoscopic training practice scenarios. Methods This descriptive cross-sectional study evaluated questionnaire responses from coordinators of the Medical Residency Programs in Gynecology and Obstetrics from 2019 to 2021. Results The questionnaire was sent to 175 programs, and 90 responses were received (51.4%). From the 85 valid responses, it was noted that 67 programs had laparoscopic training. Of the 64 responses received regarding location, 32 replies (50%) indicated the Southeast of Brazil, particularly some country’s capitals. In 37.3% (n=25) of the cases, the program coordinator performed laparoscopy. The chief of the laparoscopy sector has advanced experience in most 52.5% (n=10) medical residency services; the preceptors also had advanced experience in 89.4% (n=59) of the services. Residents received laboratory training in 39.4% (n=26) of the services. In most cases, training was performed using a physical simulator. Of the 26 medical residency services with laparoscopy training outside the operating room, 80.8% (n=21) performed them as part of the curriculum, 61.5% (n=16) had a schedule for the same, and only 3.9% (n=1) were objectively evaluated. Conclusion Laparoscopy teaching in Brazil is heterogeneous, with only a few programs offering any training in laparoscopy. The preceptors had advanced experience and participated in laboratory and operating room training. Only a few programs have their own laboratories or training centers, and most teaching programs do not plan to set up training centers.

Overall health relies on features of skeletal muscle that generally decline with age, partly due to mechanisms associated with mitochondrial redox imbalance and bioenergetic dysfunction. Previously, aged mice genetically devoid of the mitochondrial NAD(P)+ transhydrogenase (NNT, encoded by the nicotinamide nucleotide transhydrogenase gene), an enzyme involved in mitochondrial NADPH supply, were shown to exhibit deficits in locomotor behavior. Here, by using young, middle-aged, and older NNT-deficient (Nnt−/−) mice and age-matched controls (Nnt+/+), we aimed to investigate how muscle bioenergetic function and motor performance are affected by NNT expression and aging. Mice were subjected to the wire-hang test to assess locomotor performance, while mitochondrial bioenergetics was evaluated in fiber bundles from the soleus, vastus lateralis and plantaris muscles. An age-related decrease in the average wire-hang score was observed in middle-aged and older Nnt−/− mice compared to age-matched controls. Although respiratory rates in the soleus, vastus lateralis and plantaris muscles did not significantly differ between the genotypes in young mice, the rates of oxygen consumption did decrease in the soleus and vastus lateralis muscles of middle-aged and older Nnt−/− mice. Notably, the soleus, which exhibited the highest NNT expression level, was the muscle most affected by aging, and NNT loss. Additionally, histology of the soleus fibers revealed increased numbers of centralized nuclei in older Nnt−/− mice, indicating abnormal morphology. In summary, our findings suggest that NNT expression deficiency causes locomotor impairments and muscle dysfunction during aging in mice.

Abstract Most research on trinucleotide repeats (TRs) focuses on human diseases, with few on the impact of TR expansions on plant gene expression. This work investigates TRs' effect on global gene expression in Psidium guajava L., a plant species with widespread distribution and significant relevance in the food, pharmacology, and economics sectors. We analyzed TR-containing coding sequences in 1,107 transcripts from 2,256 genes across root, shoot, young leaf, old leaf, and flower bud tissues of the Brazilian guava cultivars Cortibel RM and Paluma. Structural analysis revealed TR sequences with small repeat numbers (5–9) starting with cytosine or guanine or containing these bases. Functional annotation indicated TR-containing genes' involvement in cellular structures and processes (especially cell membranes and signal recognition), stress response, and resistance. Gene expression analysis showed significant variation, with a subset of highly expressed genes in both cultivars. Differential expression highlighted numerous down-regulated genes in Cortibel RM tissues, but not in Paluma, suggesting interplay between tissues and cultivars. Among 72 differentially expressed genes with TRs, 24 form miRNAs, 13 encode transcription factors, and 11 are associated with transposable elements. In addition, a set of 20 SSR-annotated, transcribed, and differentially expressed genes with TRs was selected as phenotypic markers for Psidium guajava and, potentially for closely related species as well.

Abstract Supplementing minerals beyond dietary requirements can increase the risk of toxicity and mineral excretion, making the selection of more bioavailable sources crucial. Thus, this work aimed to use metalloproteomics tools to investigate possible alterations in the hepatic proteome of broilers fed with diets containing two sources (sulfate and hydroxychloride) and two levels of copper (15 and 150 ppm) and manganese (80 and 120 ppm), totaling four treatments: low Cu/Mn SO4, high Cu/Mn SO4, low Cu/Mn (OH)Cl and high Cu/Mn (OH)Cl. The difference in abundance of protein spots and copper and manganese concentrations in liver and protein pellets were analyzed by analysis of variance with significance level of 5%. The Cu and Mn concentrations determined in liver and protein pellets suggested greater bioavailability of hydroxychloride sources. We identified 19 Cu-associated proteins spots, 10 Mn-associated protein spots, and 5 Cu and/or Mn-associated protein spots simultaneously. The analysis also indicated the induction of heat shock proteins and detoxification proteins in broilers fed with high levels of copper and manganese, suggesting the involvement of these proteins in metal tolerance and stress.

ABSTRACT Objective: This study aimed to evaluate the cardiovascular risk of patients with post-surgical hypoparathyroidism through coronary calcium score (CACS) evaluation and cardiovascular risk calculators. Subjects and methods: Patients with post-surgical hypoparathyroidism (HG = 29) were compared to a control group (CG = 29), matched by sex and age. Demographic and clinical data were captured by a questionnaire or patient files. Both groups performed a thoracic-computed tomography to evaluate the CACS and the cardiovascular risk was calculated by two risk calculators. Results: In the HG, the supplementation of calcium varied between 500 to 2,000 mg/day and the mean calcitriol was 0.5 ± 0.29 mcg/day. The mean serum calcium and phosphorus were 8.32 ± 0.68 and 4.92 ± 0.87 mg/dL, respectively, and in the range recommended for hypoparathyroidism. The Brazilian Society of Cardiology's risk calculator showed a difference among groups, with no patient in the HG with low risk, but the CACS was similar. A positive CACS in the HG was associated with obesity and high BMI but not with calcium and/or vitamin D supplementation. Conclusion: In conclusion, patients with hypoparathyroidism did not show increased CACS, and it was not related to supplementation.

Abstract Human accuracy in detecting deception with intuitive judgments has been proven to not go above the chance level. Therefore, several automatized verbal lie detection techniques employing Machine Learning and Transformer models have been developed to reach higher levels of accuracy. This study is the first to explore the performance of a Large Language Model, FLAN-T5 (small and base sizes), in a lie-detection classification task in three English-language datasets encompassing personal opinions, autobiographical memories, and future intentions. After performing stylometric analysis to describe linguistic differences in the three datasets, we tested the small- and base-sized FLAN-T5 in three Scenarios using 10-fold cross-validation: one with train and test set coming from the same single dataset, one with train set coming from two datasets and the test set coming from the third remaining dataset, one with train and test set coming from all the three datasets. We reached state-of-the-art results in Scenarios 1 and 3, outperforming previous benchmarks. The results revealed also that model performance depended on model size, with larger models exhibiting higher performance. Furthermore, stylometric analysis was performed to carry out explainability analysis, finding that linguistic features associated with the Cognitive Load framework may influence the model’s predictions.

Breakthrough COVID-19 (occurring in fully vaccinated people) has been described. Data on its characteristics among immune-mediated rheumatic disease (IMRD) patients are scarce. This study describes breakthrough COVID-19 occurring in IMRD patients participating in the SAFER-study, a Brazilian multicentric cohort evaluating the safety, effectiveness, and immunogenicity of SARS-CoV-2 vaccines in patients with autoimmune diseases. A descriptive analysis of the population and a binary logistic regression model were performed to evaluate the predictors of COVID-19-related hospitalization. A p-value < 0.05 was significant. The included 160 patients were predominantly females (83.1%), with a mean (SD) age of 40.23 (13.19) years. The patients received two (19%), three (70%), or four (11%) vaccine doses. The initial two-dose series was mainly with ChAdOx1 (Oxford/AstraZeneca) (58%) or BBIBP-CorV (Sinopharm-Beijing) (34%). The first booster (n = 150) was with BNT162b2 (BioNtech/Fosun Pharma/Pfizer) (63%) or ChAdOx1 (29%). The second booster (n = 112) was with BNT162b2 (40%) or ChAdOx1 (26%). The COVID-19 hospitalization rate was 17.5%. IMRD moderate/high activity (OR: 5.84; CI: 1.9–18.5; p = 0.002) and treatment with corticosteroids (OR: 2.94; CI: 1.02–8.49; p = 0.0043) were associated with higher odds of hospitalization, while increasing the number of vaccine doses was protective (OR: 0.37; CI: 0.15–0.9; p = 0.032). These findings, along with previous reassuring results about the safety of the COVID-19 vaccines, argue in favor of booster vaccination in IMRD patients.

Abstract Pump sizing is the process of dimensional matching of an impeller and stator to provide a satisfactory performance test result and good service life during the operation of progressive cavity pumps. In this process, historical data analysis and dimensional monitoring are done manually, consuming a large number of man-hours and requiring a deep knowledge of progressive cavity pump behavior. This paper proposes the use of graph neural networks in the construction of a prototype to recommend interference during the pump sizing process in a progressive cavity pump. For this, data from different applications is used in addition to individual control spreadsheets to build the database used in the prototype. From the pre-processed data, complex network techniques and the betweenness centrality metric are used to calculate the degree of importance of each order confirmation, as well as to calculate the dimensionality of the rotors. Using the proposed method a mean squared error of 0.28 is obtained for the cases where there are recommendations for order confirmations. Based on the results achieved, it is noticeable that there is a similarity of the dimensions defined by the project engineers during the pump sizing process, and this outcome can be used to validate the new design definitions.

Abstract BackgroundLeprosy leads to nerve damage and slow-healing ulcers, which are treatable with routine therapy. There has been a recent resurgence of interest in the use of honey for the treatment of different kinds of wounds. ObjectiveThe aim of this study, Honey Experiment on Leprosy Ulcer (HELP), is to evaluate the healing properties of raw, unadulterated African honey in comparison with normal saline dressing for the treatment leprosy ulcers. MethodsThis is a multicenter, comparative, prospective, single-blinded, parallel-group, and 1:1 individually randomized controlled trial to be conducted at The Leprosy Referral Hospital, Chanchaga in Minna, Niger State, North Central Nigeria, and St. Benedict Tuberculosis and Leprosy Rehabilitation Hospital in Ogoja, Cross River State, South-South Nigeria. Raw, unadulterated honey will be used in the ulcer dressing of eligible, consenting participants in the intervention group, whereas those in the control group will be treated by dressing with normal saline. The main outcomes will be the proportion of complete healing and the rate of healing up to 84 days after randomization. Follow-up will be conducted 6 months after randomization. We aim to enroll 90‐130 participants into the study. Blinded observers will examine photographs of ulcers to determine the outcomes. ResultsThe recruitment of trial participants began on March 14, 2022, and has been continuing for approximately 24 months. ConclusionsOur study will provide an unbiased estimate of the effect of honey on the healing of neuropathic ulcers.

Abstract Following the reaction of biological membranes to external stimuli reveals fundamental insights into cellular function. Here, self-assembled lipid monolayers act as model membranes containing photoswitchable azobenzene glycolipids for investigating structural response during isomerization by combining Langmuir isotherms with X-ray scattering. Controlled in-situ trans/cis photoswitching of the azobenzene N = N double bond alters the DPPC monolayer structure, causing reproducible changes in surface pressure and layer thickness, indicating monolayer reorientation. Interestingly, for monolayers containing azobenzene glycolipids, along with the expected DPPC phase transitions an additional discontinuity is observed. The associated reorintation represents a crossover point, with the surface pressure and layer thickness changing in opposite directions above and below. This is evidence that the azobenzene glycolipids themselves change orientation within the monolayer. Such behaviour suggests that azobenzene glycolipids can act as a bidirectional switch in DPPC monolayers providing a tool to investigate membrane structure–function relationships in depth.

ABSTRACT Objective To describe the temporal trend of notifications of physical, sexual and emotional violence and neglect against children in Brazil between 2011 and 2019. Methods This was an ecological time-series study based on notifications of violence against children aged 0-9 years held on the Brazilian Notifiable Health Conditions Information System. Age-adjusted notification rates were calculated for Brazil as a whole, by national macro-region and by sex. Trends were assessed using Joinpoint Regression. Results We analyzed 88,820 notifications of physical violence, 87,141 notifications of sexual violence, 52,359 notifications of emotional violence and 166,664 notifications of neglect. A rising trend was identified for notifications of physical, sexual and emotional violence and neglect for Brazil as a whole and for both sexes. Neglect accounted for the highest rate (95.24 notifications per 100,000 children in 2019). The Northeast and Southeast macro-regions had rising trends for all forms of violence. Conclusion Notifications of physical, sexual and emotional violence and neglect showed rising trends in Brazil in the period studied.

Abstract The temporal pole (TP) plays a central role in semantic memory, yet its neural machinery is unknown. Intracerebral recordings in patients discriminating visually the gender or actions of an actor, yielded gender discrimination responses in the ventrolateral (VL) and tip (T) regions of right TP. Granger causality revealed task-specific signals travelling first forward from VL to T, under control of orbitofrontal cortex (OFC) and neighboring prefrontal cortex, and then, strongly, backwards from T to VL. Many other cortical regions provided inputs to or received outputs from both TP regions, often with longer delays, with ventral temporal afferents to VL signaling the actor’s physical appearance. The TP response timing reflected more that of the connections to VL, controlled by OFC, than that of the input leads themselves. Thus, visual evidence for gender categories, collected by VL, activates category labels in T, and consequently, category features in VL, indicating a two-stage representation of semantic categories in TP.

Abstract We compared seven node vaccination strategies in twelve real-world complex networks. The node vaccination strategies are modeled as node removal on networks. We performed node vaccination strategies both removing nodes according to the initial network structure, i.e., non-adaptive approach, and performing partial node rank recalculation after node removal, i.e., semi-adaptive approach. To quantify the efficacy of each vaccination strategy, we used three epidemic spread indicators: the size of the largest connected component, the total number of infected at the end of the epidemic, and the maximum number of simultaneously infected individuals. We show that the best vaccination strategies in the non-adaptive and semi-adaptive approaches are different and that the best strategy also depends on the number of available vaccines. Furthermore, a partial recalculation of the node centrality increases the efficacy of the vaccination strategies by up to 80%.

Pulmonary sequestration is an uncommon congenital malformation of the lung, generally diagnosed in childhood or adolescence, corresponding to dysplastic lung tissue not communicating with the rest of vascular or bronchial lung system but receiving an arterial blood supply from systemic arteries. Currently, surgical resection is usually indicated in order to prevent or treat related symptoms or complications, although controversy exists regarding its use in asymptomatic patients and adults. We present the case of a 32-year-old pregnant woman with acute chest pain and vomiting diagnosed with intralobar sequestration at 32+2 weeks of gestation and treated with pulmonary lobectomy after giving birth by cesarean section at 33+0 weeks of gestation.

Objective The objective of the present study was to define the minimum number of monitoring days required for the adequate cross-sectional assessment of sedentary behaviour in individuals with chronic obstructive pulmonary disease (COPD). Methods In this cross-sectional study, the sedentary behaviour of individuals with COPD was assessed using two physical activity monitors during awake time for seven consecutive days. Time spent per day in activities requiring ≤1.5 metabolic equivalents (METs) and in sitting, lying and sitting+lying positions was calculated taking into account the average of 7 days (as a reference in all analyses) and of all 119 possible combinations of 2­­–6 days. Intraclass correlation coefficients (ICCs) and linear regression analyses were performed for all combinations. Results 91 individuals were analysed (47 female, 66±9 years, forced expiratory volume in 1 s 50±15% predicted). A multicenter cross-sectional survey study involving four Italian University Hospitals was performed to test the hypothesis that negative affect and positive affect (affective dimensions) mediate the association between risk perception (perceived risk of infection and death; cognitive dimensions) and the feeling of work exhaustion (WE) among obstetrics healthcare providers (HCPs) during the Coronavirus Disease 2019 (COVID-19) pandemic. Totally, 570 obstetrics HCPs were invited to complete the 104-item IPSICO survey in May 2020. A theoretical model built on the tested hypothesis was investigated by structural equation modelling. The model explained 32.2% of the WE variance. Only negative affect mediated the association between cognitive dimensions and WE and also the association between WE and psychological well-being before the pandemic, experiences of stressful events, female gender, and dysfunctional coping. Non-mediated associations with WE were observed for work perceived as a duty, experience of stressful events, support received by colleagues, and the shift strategy. Only previous psychological well-being, support by colleagues, and shift strategies were inversely associated with WE. Based on study results, monitoring negative than positive affect appears superior in predicting WE, with practical implications for planning psychological interventions in HCPs at the individual, interpersonal, and organizational levels.

ABSTRACT Background Major depressive disorder is a difficult-to-treat psychological disorder. Approximately 30% of patients with major depressive disorder do not respond to conventional therapies; thus, the efficacy of alternative therapies for treating major depressive disorder, such as neurofeedback, a non-invasive neuromodulation method used in the treatment of psychiatric diseases, must be investigated. Objective We aimed to evaluate the efficacy of neurofeedback in minimizing and treating major depressive disorder and its application as a substitute to or an adjuvant with conventional therapies. Methods We searched for experimental studies published between 1962–2021 in Scopus, PubMed, Web of Science, and Embase databases and identified 1,487 studies, among which 13 met the inclusion exclusion criteria. Results We noted that not all patients responded to neurofeedback. Based on depression scales, major depressive disorder significantly improved in response to neurofeedback only in a few individuals. Additionally, the number of training sessions did not influence the results. Conclusion Neurofeedback can reduce depression symptoms in patients; however, not all patients respond to the treatment. Therefore, further studies must be conducted to validate the effectiveness of neurofeedback in treating major depressive disorder.

Introduction Leprosy occurs among very poor people who may be stigmatised and pushed further to the margins of society. Programmes to improve social integration and stimulate economic development have been implemented to help break the vicious cycle of poverty, reduced quality of life and ulcer recurrence. These involve forming groups of people, with a common concern, to provide mutual support and form saving syndicates—hence the term ‘self-help groups’ (SHGs). While there is literature on the existence and effectiveness of SHGs during the funded periods, little is known about their sustainability. We aim to explore the extent to which SHG programme activities have continued beyond the funding period and record evidence of sustained benefits.Methods and analysis In India, Nepal and Nigeria, we identified programmes funded by international non-governmental organisations, primarily aimed at people affected by leprosy. In each case, financial and technical support was allocated for a predetermined period (up to 5 years).We will review documents, including project reports and meeting minutes, and conduct semistructured interviews with people involved in delivery of the SHG programme, potential beneficiaries and people in the wider environment who may have been familiar with the programme. These interviews will gauge participant and community perceptions of the programmes and barriers and facilitators to sustainability. Data will be analysed thematically and compared across four study sites.Ethics and dissemination Approval was obtained from the University of Birmingham Biomedical and Scientific Research Ethics Committee. Local approval was obtained from: The Leprosy Mission Trust India Ethics Committee; Federal Capital Territory Health Research Ethics Committee in Nigeria and the Health Research Ethics Committee of Niger State Ministry of Health; University of Nigeria Teaching Hospital and the Nepal Health and Research Council. Results will be disseminated via peer-reviewed journals, conference presentations and community engagement events through the leprosy missions.

Abstract Background Experiential learning through patient care is fundamental to graduate medical education. Despite this, the actual content to which trainees are exposed in clinical practice is difficult to quantify and is poorly characterized. There remains an unmet need to define precisely how residents’ patient care activities inform their educational experience. Methods Using a recently-described crosswalk tool, we mapped principal ICD-10 discharge diagnosis codes to American Board of Internal Medicine (ABIM) content at four training hospitals of a single Internal Medicine (IM) Residency Program over one academic year to characterize and compare residents’ clinical educational experiences. Frequencies of broad content categories and more specific condition categories were compared across sites to profile residents’ aggregate inpatient clinical experiences and drive curricular change. Results There were 18,604 discharges from inpatient resident teams during the study period. The crosswalk captured > 95% of discharges at each site. Infectious Disease (ranging 17.4 to 39.5% of total discharges) and Cardiovascular Disease (15.8 to 38.2%) represented the most common content categories at each site. Several content areas (Allergy/Immunology, Dermatology, Obstetrics/Gynecology, Ophthalmology, Otolaryngology/Dental Medicine) were notably underrepresented (≤ 1% at each site). There were significant differences in the frequencies of conditions within most content categories, suggesting that residents experience distinct site-specific clinical content during their inpatient training. Conclusions There were substantial differences in the clinical content experienced by our residents across hospital sites, prompting several important programmatic and curricular changes to enrich our residents’ hospital-based educational experiences.

Abstract Objective: to analyze the difference in the number of primary teeth dental procedures performed within the Brazilian National Health System (SUS) in the state of Rio Grande do Sul, before and during the COVID-19 pandemic. Methods: this was a descriptive ecological study, using secondary data from the SUS Outpatient Information System (SIA-SUS), from 2018 to 2021, in the state and in its seven health macro-regions; we calculated the relative and absolute frequencies and the percentage difference of the dental procedures performed. Results: 94,443 and 36,151 dental procedures were recorded before and during the pandemic, respectively, corresponding to a 61.7% reduction; relevant percentage reductions were found in restorative procedures, which reached 20% in the southern region of the state; an increase in the percentage of exodontic and endodontic procedures was found. Conclusion: the results suggest that the COVID-19 pandemic had negative repercussions on the performance of primary teeth dental procedures in Ro Grande do Sul.

Health-related quality of life is frequently included in patient-reported outcomes aimed at evaluating the effectiveness of disease-modifying drugs for multiple sclerosis, but recent data about Italian patients are missing. A multicenter observational and cross-sectional study was performed by students of hospital pharmacy to update existing data on quality of life and to correlate it with the pharmacological and medical history of patients. Quality of life (QoL) was assessed using the MS-QoL54 questionnaire, and the pharmacist collected patients’ characteristics, medical and pharmacological history, and Expanded Disability Status Scale (EDSS). Three hundred and forty-nine patients with multiple sclerosis were recruited from 16 centers between May 2018 and June 2019 (median age = 44.1 years; 68.9% women). The composite indexes of physical and mental well-being showed direct correlation with each other (R = 0.826; p < 0.001), and EDSS disability was an independent negative predictor of both indexes (R2 = 35.08% p < 0.001 and R2 = 15.74% p < 0.001, respectively). A trend of association between Physical Health Composite Score and different classes of oral disease-modifying drugs (DMDs) was observed. Our study found a decrease in QoL correlated with teriflunomide, which deserves further investigation. This experience demonstrates that joint action between scientific society and students association can be successful in conducting a no-profit multicenter observational study in a real-world setting.

Abstract Brown propolis from permanent preservation and reforestation areas of southern Brazil have attracted international commercial interest and have a unique composition, although little is known about their botanical origins, which are the plant resins used by bee foragers to produce propolis. Hence, the volatile profiles of organic and non-organic brown propolis and resins of suspected botanical origins—Araucaria angustifolia, Pinus elliott and Pinus taeda—were determined using static headspace gas chromatography coupled to mass spectrometry (SHS-GCMS) and compared. IntroductionPeople affected by leprosy are at increased risk of impairments and deformities from peripheral nerve damage. This mostly occurs if diagnosis and treatment is delayed and contributes to continued transmission within the community. Champa district of Chhattisgarh state in India is an endemic area with the highest national annual case detection and disability rates for leprosy. The Replicability Model is a system strengthening intervention implemented by the Leprosy Mission Trust India in Champa that aims to promote early diagnosis and treatment of leprosy, improve on-going management of the effects of leprosy and improve welfare for the people affected by leprosy. This protocol presents a plan to describe the overall implementation of the Replicability Model and describe the barriers and facilitators encountered in the process. We will also quantify the effect of the program on one of its key aims- early leprosy diagnosis.MethodsThe replicability model will be implemented over four years, and the work described in this protocol will be conducted in the same timeframe. We have two Work Packages (WPs). In WP1, we will conduct a process evaluation. This will include three methods i) observations of replicability model implementation teams' monthly meetings ii) key informant interviews (n = 10) and interviews with stakeholders (n = 30) iii) observations of key actors (n = 15). Our purpose is to describe the implementation process and identify barriers and facilitators to successful implementation. WP2 will be a quantitative study to track existing and new cases of leprosy using routinely collected data. If the intervention is successful, we expect to see an increase in cases (with a higher proportion detected at an early clinical stage) followed by a decrease in total cases.ConclusionThis study will enable us to improve and disseminate the Replicability Model by identifying factors that promote success. It will also identify its effectiveness in fulfilling one of its aims: reducing the incidence of leprosy by finding and tracking cases at an earlier stage in the disease.

A few epidemiological studies are evaluating the prevalence and mortality rates of Alzheimer's disease, with no one using a nationwide sample of Brazilian elderlies. This study aims to calculate the prevalence of Alzheimer's disease and investigate possible associations with sociodemographic and lifestyle factors and the presence of diseases non-communicable, and the prevalence and mortality for all Brazilian state capitals. This is an ecological design study made with secondary public data provided by the Ministry of Health. Prevalence rates were calculated based on the analysis of the dispensing of Alzheimer's disease-specific drugs. Correlation analyzes were performed between rates and factors, and a multiple linear regression analysis was used to analyze possible associations between variables, controlled for each other. AD prevalence was 313/100,000. Prevalence rates were positively associated with primary health care coverage factors and negatively associated with ultra-processed food consumption and physical activity levels. AD mortality was 98/100,000. Mortality rates were positively associated with the proportion of obese elderly and elderly living on up to half the minimum wage and were inversely associated with the proportion of elderly with diabetes factors. We found positive and negative associations of sociodemographic, behavioral and diabetes indicators with Alzheimer's disease prevalence and mortality, which provide data that can be investigated by studies with different designs.

Laternula elliptica (P. P. King, 1832) is the sole representative of the anomalodesmatan family Laternulidae and the largest bivalve in the Antarctic and Subantarctic. A keystone species of the regional benthic communities, it has reached model status, having been studied in hundreds of scientific works across many biological disciplines. In contrast, its anatomy has remained poorly known, with prior published data limited to partial descriptions based on chemically preserved specimens. Based on observations of aquarium-maintained living animals at the Brazilian Comandante Ferraz Antarctic Station, gross-morphological dissections, and histological sectioning, the comparative anatomy, functional morphology, and aspects of behavior of L. elliptica are described and discussed. Special focus is placed on the pallial organs (including elucidation of cleansing and feeding sorting mechanisms in the mantle cavity) and the musculature. Among the noteworthy findings are the presence of well-developed siphons furnished with sensory tentacles at its tips, some of which bearing eyes; large, folded gills and labial palps capable of sorting the material entering the mantle cavity; an inter-chamber communication in the posterior region of the mantle cavity; an ample ventral mantle fusion with an anterior pedal gape; the absence of a 4th pallial opening; and the absence of a ligamental lithodesma in adult specimens. This study reevaluates the available anatomical data in the literature, both supplementing and correcting previously published accounts.

Abstract Alzheimer’s disease is a neurodegenerative condition that causes changes in memory and cognition, in addition to behavioral disorders, and most commonly affects the elderly. Several studies in the literature have presented therapeutic measures in an attempt to interfere with the pathogenic mechanisms of the disease and to mitigate its clinical manifestations. Some factors, such as excitotoxicity, cholinergic dysfunctions, oxidative stress, tau protein hyperphosphorylation, changes in amyloid-beta peptide metabolism, herpes viruses, apolipoprotein E, glycogen synthase kinase 3, insulin resistance, and the endocannabinoid system seem to be related to pathophysiology of Alzheimer’s disease. Given this, a literature review was carried out to address the molecular mechanisms associated with the pathophysiological hypotheses previously mentioned, aiming to better understanding their underlying causes and contributing to possible pharmacological strategies about treatment of the disease.

ABSTRACT BACKGROUND: Inflammatory bowel diseases affect mostly young patients and have a huge impact on their quality of life and growing treatment costs. Currently, there are few Brazilian studies concerning their epidemiological profile. OBJECTIVE: The aim of this study was to describe the regional clinical and epidemiological profile of these pathological conditions in Caxias do Sul, Brazil. DESIGN AND SETTING: Cross-sectional study in Caxias do Sul (RS), Brazil. METHODS: A search for patients was conducted in the municipality's special medications pharmacy using the International Classification of Diseases, and medical records were manually reviewed for data collection. Sixty-seven patients were included. RESULTS: The patients’ mean age was 46.5 years and females predominated (71.6%). Ulcerative colitis was the most prevalent disease (70%) and Montreal E3 was the most prevalent presentation. The mean age at diagnosis was 39 years. Most patients had recently undergone colonoscopy (67%). Only five patients (7.4%) had records of hospital admission due to the disease, while 12 (18%) underwent a surgical procedure during follow-up. Sixty patients (89.5%) were using aminosalicylates, while less than one fifth were using immunosuppressants or immunobiological drugs: 19.4% and 14.9%, respectively. CONCLUSION: The profile of inflammatory bowel disease patients in this region of Brazil is similar in some characteristics to other published Brazilian data, although it differs in others such as higher frequency of pancolitis. A prospective study on these patients is planned in this region, in order to improve the data quality.

Abstract Influential lines of research propose dual processes-based explanations to account for both the cognitive cost implied in lying and for that entailed in the resolution of the conflict posited by Simon tasks. The emergence and consistency of the Simon effect has been proved to be modulated by both practice effects and transfer effects. Although several studies provided evidence that the lying cognitive demand may vary as a function of practice, whether and how transfer effects could also play a role remains an open question. We addressed this question with one experiment in which participants completed a Differentiation of Deception Paradigm twice (baseline and test sessions). Crucially, between the baseline and the test sessions, participants performed a training session consisting in a spatial compatibility task with incompatible (condition 1) or compatible (condition 2) mapping, a non-spatial task (condition 3) and a no task one (condition 4). Results speak in favour of a modulation of individual performances by means of an immediate prior experience, and specifically with an incompatible spatial training.

Abstract Background Whilst there is little uncertainty about the deleterious impact of pollution on human and planetary health, pollution’s impact on adolescent mental health is less well understood. This is particularly true for young people in underdeveloped and developing world contexts, about whom research is generally lacking. Furthermore, although adolescent resilience continues to be a research priority, little attention has been paid to adolescent pathways of resilience in the face or aftermath of pollution exposure. The objective of this study will be to examine the associations between pollution and mental health in 10- to 24-year-olds (i.e. adolescents). Methods We designed and registered a study protocol for a systematic review of studies which link pollution and mental health in adolescents. We will include observational studies (e.g. cohort, case-control, time series analyses) that assess the associations between exposure to any form of pollution and the mental health of 10- to 24-year-olds. The primary outcome will be symptoms associated with neurodevelopmental disorders; disruptive, impulse-control, and conduct disorders; depressive disorders; anxiety disorders; substance disorders; and schizophrenia. No secondary outcomes will be considered. Literature searches will be conducted in multiple electronic databases (from inception onwards), including PubMed, MEDLINE, SCOPUS, Web of Science, CINAHL, PsycINFO, SciELO, ERIC, and Africa-Wide. Two investigators will independently screen all citations, full-text articles, and abstract data. The methodological quality (or bias) of included studies will be appraised using appropriate tools. We will provide a narrative synthesis of the evidence. Discussion This systematic review will evaluate the evidence on the associations between pollution and the mental health of 10- to 24-year-olds. Our findings will be of potential interest to multiple audiences (including adolescent patients/clients, their families, caregivers, healthcare professionals, scientists, and policy makers) and could be used to develop prevention and intervention strategies as well as focus future research. Enquadramento: O modelo de cuidados prestados durante o parto na água é interpessoal, integral e respeitado. Objetivo: Conhecer a perceção das mulheres sobre os cuidados recebidos pelas enfermeiras especialistas de saúde materna e obstétrica durante o parto na água na perspetiva da teoria de enfermagem de Jean Watson. Metodologia: Investigação empírica, qualitativa, interpretativa com 24 mulheres portuguesas que vivenciaram o parto na água no hospital ou no domicílio, com tratamento dos dados baseados no método de análise de conteúdo de Bardin. Resultados: A partir das narrativas, emergiram cinco categorias - ambiente acolhedor; a mulher no controlo do seu próprio parto; relação de confiança; expectativas correspondidas e o companheiro como parte do processo. Conclusão: O Processo Clinical Caritas foi percebido nas narrativas através do respeito e do carinho durante os cuidados prestados, proporcionando uma relação e cooperação, que promoveu posteriormente uma cura interior através desse processo de cuidados. A perceção das participantes do estudo quanto aos cuidados recebidos revelou um ambiente seguro, íntimo, respeitado, acolhedor, sem intervenções desnecessárias, favorável e amparado pelo companheiro.

Severe asthma is associated with increased morbidity, mortality, healthcare costs and impaired quality of life. Asthma is no longer considered as a single entity but as a heterogeneous disease with different clinical presentations (phenotypes) and variable underlying mechanistic biological pathways (endotypes). Two different endotypes are based on the inflammatory Type 2 T-helper response: T2-high and T2-low. The understanding of these endotypes has revolutionised the management of severe asthma. Recent guidelines from the 2019 European Respiratory Society/American Thoracic Society (ERS/ATS) and Global Initiative for Asthma (GINA) 2021 specifically address the diagnosis and the management of severe asthma in adults, but less evidence exists for the paediatric population. Presently, five biologics for the treatment of severe asthma are approved, i.e., omalizumab (anti-IgE antibody), mepolizumab and reslizumab (anti-IL-5 antibody), benralizumab (anti-IL-5 receptor antibody) and dupilumab (anti-IL-4 receptor alpha antibody). This article reviews the pathological mechanisms of severe asthma, clinical biomarkers related to the T2-high endotype, and their use for the prediction of the severity of the disease and response to biological therapy. Furthermore, future developments of biologics for severe asthma are presented.

Objective To identify factors associated with accessing and utilisation of healthcare and provision of health services in slums.Design A scoping review incorporating a conceptual framework for configuring reported factors.Data sources MEDLINE, Embase, CINAHL, Web of Science and the Cochrane Library were searched from their inception to December 2021 using slum-related terms.Eligibility criteria Empirical studies of all designs reporting relevant factors in slums in low and middle-income countries.Data extraction and synthesis Studies were categorised and data were charted according to a preliminary conceptual framework refined by emerging findings. Results were tabulated and narratively summarised.Results Of the 15 469 records retrieved from all years, 4368 records dated between 2016 and 2021 were screened by two independent reviewers and 111 studies were included. The majority (63 studies, 57%) were conducted in Asia, predominantly in India. In total, 104 studies examined healthcare access and utilisation from slum residents’ perspective while only 10 studies explored provision of health services from providers/planners’ perspective (three studies included both). A multitude of factors are associated with accessing, using and providing healthcare in slums, including recent migration to slums; knowledge, perception and past experience of illness, healthcare needs and health services; financial constraint and competing priorities between health and making a living; lacking social support; unfavourable physical environment and locality; sociocultural expectations and stigma; lack of official recognition; and existing problems in the health system.Conclusion The scoping review identified a significant body of recent literature reporting factors associated with accessing, utilisation and provision of healthcare services in slums. We classified the diverse factors under seven broad categories. The findings can inform a holistic approach to improving health services in slums by tackling barriers at different levels, taking into account local context and geospatial features of individual slums.Systematic review registration number https://osf.io/694t2.

Abstract New tools of mapping systems may allow to detect residual conduction around the atriotomy and could eliminate the subjectivity of the human eye, analyzing thousands of electrograms in a matter of seconds and providing a clear and fast identification.

Abstract Multiple sclerosis is an autoimmune disease that affects the myelinated central nervous system (CNS) neurons and triggers physical and cognitive disabilities. Conventional therapy is based on disease-modifying drugs that control disease severity but can also be deleterious. Complementary medicines have been adopted and evidence indicates that yeast supplements can improve symptoms mainly by modulating the immune response. In this investigation, we evaluated the therapeutic potential of Saccharomyces cerevisiae and its selenized derivative (Selemax) in experimental autoimmune encephalomyelitis (EAE). Female C57BL/6 mice submitted to EAE induction were orally supplemented with these yeasts by gavage from day 0 to day 14 after EAE induction. Both supplements determined significant reduction in clinical signs concomitantly with diminished Th1 immune response in CNS, increased proportion of Foxp3+ lymphocytes in inguinal and mesenteric lymph nodes and increased microbiota diversity. However, Selemax was more effective clinically and immunologically; it reduced disease prevalence more sharply, increased the proportion of CD103+ dendritic cells expressing high levels of PD-L1 in mesenteric lymph nodes and reduced the intestinal inflammatory process more strongly than S. cerevisiae. These results suggest a clear gut-brain axis modulation by selenized S. cerevisiae and suggest their inclusion in clinical trials.

Inter-row vegetation in vineyards is classified as a service crop as it provides many ecosystem services. The vegetation is often removed but maintaining them can mitigate the negative effects on the environment. However, the type of species or mixture choice can affect their success. A field trial was conducted in an organically-managed vineyard of Cabernet sauvignon Vitis vinifera L. cultivars in north-eastern Italy, in which three blends of grass species (Shedonorus arundinaceus, Lolium perenne and Festuca rubra) and two grass-legume mixtures were grown in the inter-rows and compared with resident vegetation and regularly tilled bare soil. Each vegetation type, including resident vegetation, was subjected to mulching and non-mulching treatments. The aim of this study was to evaluate the use of seeded species or mixtures in the inter-row spaces of the vineyard in north-Italy as an alternative management to resident vegetation and tilled soil. The experiment was conducted over two years to monitor the persistence of the sown vegetation and the influence of vegetation types on vine performance and grape composition, and on soil compaction and erosion. The relative abundances of each species, vegetation height, percentage green cover and normalised difference vegetation index (NDVI), vine shoot length, number of leaves per vine shoot, leaf area, bunch weight, vine NDVI, soil compaction and erosion, and depth and width of tractor tyre prints were measured. Over time, weed invasion altered the botanical composition of all vegetation types except for the S. arundinaceus blend, which remained stable throughout the study period. Our results showed that vine parameters were not affected by the type of vegetation in the inter-rows, nor were there differences between the grassed and bare soil inter-rows. Soil compaction and erosion, and tractor tyre prints were not directly affected by the type of vegetation cover, but they were affected by tillage in the plots with bare soil in the inter-rows or where it was used to prepare the soil for sowing. Soil compaction and erosion were related to the percentage vegetation cover. Mulching did not affect any of the parameters measured. Therefore, species selection plays a crucial role in inter-row vegetation management and in minimising environmental impacts. S. arundinaceus gave high protection against soil erosion due to its high persistence throughout the year and had the lowest growth rate thus requiring fewer cuttings.

This study aimed to evaluate HPV seroprevalence, cervical HPV prevalence, genotype distribution, and frequency of HPV-related cervical lesions in SOT recipients in comparison to immunocompetent women.MethodsCross-sectional study including SOT and immunocompetent women aged 18 to 45 years who denied previous HPV-related lesions. Cervical samples were screened for HPV-DNA by a polymerase chain reaction (PCR)-based DNA microarray system (PapilloCheck®) and squamous intraepithelial lesions (SIL) by liquid-based cytology. A multiplexed pseudovirion-based serology assay (PsV-Luminex) was used to measure HPV serum antibodies.Results125 SOT and 132 immunocompetent women were enrolled. Cervical samples were collected from 113 SOT and 127 immunocompetent women who had initiated sexual activity. HPV-DNA prevalence was higher in SOT than in immunocompetent women (29.6% vs. 20.2%, p = 0.112), but this difference was not statistically significant. High-risk (HR)-HPV was significantly more frequent in SOT than in immunocompetent women (19.4% vs. 7.9%, p = 0.014). Simultaneous infection with ≥2 HR-HPV types was found in 3.1% of SOT and 0.9% of immunocompetent women. HPV seropositivity for at least one HPV type was high in both groups: 63.8% of 105 SOT and 69.7% of 119 immunocompetent women (p = 0.524). Low-grade (LSIL) and high-grade SIL (HSIL) were significantly more frequent in SOT (9.7% and 5.3%, respectively) than in immunocompetent women (1.6% and 0.8%, respectively) (p = 0.001).ConclusionsThese results may reflect the increased risk of HPV persistent infection and disease progression in SOT women due to chronic immunosuppression.

In the 21st century, we still need to talk about gender inequality in science. Even with the sharp growth of studies on this theme over the last decades, we are still trying to convince our peers that diversity matters and, if embraced, makes better science. Part of this drawback can be related to the need for data to support effective proposals to change the academic scenario. In order to close some of those gaps, we here analyze 1) the profile of Brazilian researchers based on production, impact, and membership to the Brazilian Academy of Sciences, 2) participation in the Editorial boards of Brazilian journals dedicated to Entomology, and, 3) the academic scenario of Brazilian Entomology focusing on the sex of the first and last authors in peer-reviewed international publications related to Entomology. We aimed to provide a deeper look on the Brazilian Entomology scenario and to expand the amount of data availability to stimulate and foster a mind-change in the current academic structure. We performed scientometric searches and analysis using different platforms and found that the number and impact of the publications by female researchers, as observed by relative numbers, are not less than that of males. Despite that, female researchers are less represented at the Brazilian Academy of Sciences and editorial boards, reinforcing the lack of women recognition in science. Thus, we observe that some narratives related to the productivity gap can be misleading to a perpetuation of our internal and structural biases. We here expanded data from a previous paper where we scrutinized the Brazilian Entomology scenario and discussed the patches and systems that maintain gender gap in science.

Faking in a psychological test is often observed whenever an examinee may gain an advantage from it. Although techniques are available to identify a faker, they cannot identify the specific questions distorted by faking. This work evaluates the effectiveness of term frequency-inverse document frequency (TF-IDF)-an information retrieval mathematical tool used in search engines and language representations-in identifying single-item faked responses. We validated the technique on three datasets containing responses to the 10-item Big Five questionnaire (total of 694 participants, respectively 221, 243, and 230) in three faking situations. Each participant responded twice, once faking to achieve an objective in one of three contexts (one to obtain child custody and two to land a job) and once honestly. The proposed TF-IDF model has proven very effective in separating honest from dishonest responses-with the honest ones having low TF-IDF values and the dishonest ones having higher values-and in identifying which of the 10 responses to the questionnaire were distorted in the dishonest condition. We also provide examples of the technique in a single-case evaluation.
