Biomedical Quality Engineers (QEs) ensure that medical devices are safe, reliable, and consistent. To better understand career pathways for biomedical QEs, we (1) quantified the prevalence of QE skills in entry-level biomedical engineering (BME) job listings and (2) interviewed seven biomedical Quality Engineers with a BME bachelor's degree. We mapped participant responses to the mechanisms in Social Cognitive Career Theory and identified common career pathways for biomedical QEs. To our surprise, over 40% of entry-level BME jobs were QE positions and 70% required QE-related skills. The interview participants were unaware of careers in QE careers as undergraduates and learned about QE while entering the job market--a surprising finding given the prevalence of entry-level QE jobs. The participants had low outcome expectations for QE careers and higher outcome expectations for research and development positions; instructors should be aware that a design-focused curriculum can bias students against QE careers. BME departments should introduce QE topics and experiences to help students make informed career decisions and be competitive in the sizable biomedical QE job market.

Abstract This research takes on a crucial task- exploring the optimal placement of Renewable Distributed Generators such as Solar Photovoltaic, wind turbines and Electric Vehicles into the Radial Distribution System (RDS). This is a strategic move aimed at minimising power loss (PLoss) and improving the voltage profile and stability index. The RDGs are integrated into RDS with and without considering the uncertainty of the different load demands for 24 h. The probability function of Beta and Weibull distribution functions are employed to attain the solar irradiance and wind speed in a particular region. In addition, EVs are also integrated into RDS, employing meta-heuristic algorithms intended to reduce power loss (PLoss) and improve the voltage profile. The study uses an Indian 28-bus test system mimicking a balanced radial distribution network to integrate distributed generators (DGs) and EV charging stations. The simulated results demonstrate that integrating DGs into power systems has offered considerable benefits, including reduced PLoss, heightened efficiency, decreased dependency on centralised generation, and improved environmental sustainability. It is discovered that the Multi-population Evolution Whale Optimization Algorithm (MEWOA) produces better results than other methods in the literature and is valuable and practical for handling these nonlinear optimisation situations.

Abstract Post-harvest losses caused by insect pests, particularly the rice weevil, Sitophilus oryzae, pose a significant challenge in food storage facilities worldwide. To combat this pest, synthetic insecticides and fumigants are widely used. However, effective contact insecticides are scarce. Hence, the present study explored the alternatives by evaluating the contact toxicity of various insecticides against S. oryzae using glass, jute, and floor tiles as surface substrates and further examining the effect on progeny production of promising candidate insecticides. Among the insecticides tested, malathion exhibited the highest toxicity on glass and jute surfaces regardless of the exposure period. On the other hand, spinetoram proved to be the most effective on tile surface with an 8 h exposure period. Among the alternate insecticides (spinosad, spinetoram, chlorfenapyr and lambda-cyhalothrin), spinetoram was most effective with LC50 values of 155.8 and 116.9 mg/m² for 4 h and 8 h exposure, respectively, on tile surface; 204.6 and 129.0 mg/m² for 4 h and 8 h exposure, respectively, on glass surface; and 271.5 and 199.5 mg/m² for 4 h and 8 h exposure, respectively, on jute surface. Relative toxicity assessments revealed spinetoram to be 2.11 and 2.51 times more effective than deltamethrin on tile surface for 4 and 8 h of exposure, respectively whereas it was 1.14 times more effective than malathion on tile surface at 8 h exposure. Principal component analysis indicated a higher demand for insecticide doses closely associated with the structural properties of surfaces, particularly evident with jute surfaces. Furthermore, the effect on adult mortality and progeny production by malathion, spinetoram, and lambda-cyhalothrin revealed malathion as the most effective insecticide followed by spinetoram. Carboxylesterase, acetylcholinesterase, and Glutathione S-transferase (GST) activities were notably higher in deltamethrin-treated insects compared to other insecticides. The studies concluded that spinetoram can be considered an alternative to conventional insecticides for the management of S. oryzae under different storage conditions.

Abstract Trachinotus blochii is a high-value tropical mariculture species. The present study evaluated the gut microbial impact of therapeutic exposure (80 mg/day/kg biomass for 10 days) to oxytetracycline, the most common aquaculture antibiotic in T. blochii. The cultivable counts, α-diversity measures of taxonomic and functional metagenomics, microbial dysbiosis (MD) index, and microbial taxon abundances showed the resilience of gut microbiota at 16–26 days of treatment. A significant reduction in bacterial abundance, diversity measures, Firmicutes and Actinobacteria and an increase in γ-Proteobacteria was recorded on the 6th and 11th day of treatment. The increased metagenomic stress signatures, decreased beneficial bacterial abundances, decreased abundance of microbial pathways on energy metabolism, and MD index indicated short-term transient stress during the initial days of therapeutic withdrawal, warranting health management measures. Therapeutic exposure reduced the abundance of fish pathogens, including Vibrio spp., kanamycin and ampicillin-resistant bacteria. Strikingly, oxytetracycline treatment did not increase tetracycline-resistant bacterial counts and the predicted abundance of tetracycline resistance encoding genes in the gut, illustrating that therapeutic application would not pose a risk in the context of antimicrobial resistance in short term. Altogether, the present study provides a foundation for oxytetracycline treatment to develop suitable risk minimization tactics in sustainable aquaculture.

Abstract With the advances in technology, humans tend to explore the world underwater in a more constructive way than before. The appearance of an underwater object varies depending on depth, biological composition, temperature, ocean currents, and other factors. This results in colour distorted images and hazy images with low contrast. To address the aforesaid problems, in proposed approach, initially White balance algorithm is carried out to pre-process original underwater image. Contrast enhanced image is achieved by applying the Contrast Limited Adaptive Histogram Equalization algorithm (CLAHE). In CLAHE, tile size and clip limit are the major parameters that control the enhanced image quality. Hence, to enhance the contrast of images optimally, Firefly algorithm is adopted for CLAHE. Dark Channel Prior algorithm (DCP) is modified with guided filter correction to get the sharpened version of the underwater image. Multiscale fusion strategy was performed to fuse CLAHE enhanced and dehazed images. Finally, the restored image is treated with optimal CLAHE to improve visibility of enhanced underwater image. Experimentation is carried out on different underwater image datasets such as U45 and RUIE and resulted in UIQM = 5.1384, UCIQE = 0.6895 and UIQM = 5.4875, UCIQE = 0.6953 respectively which shows the superiority of proposed approach.

Abstract Earthworm, Eudrilus eugeniae cannot survive and regenerate without clitellum segments. In regenerating worms, the clitellum’s epithelial and circular muscular layers are reduced to one-third, and longitudinal cell layers to half. In C2C12 cells, Clitellum Factors (CF − 5, 25 and 50%) and Regenerative Clitellum Factors (RCF − 5, 25, 50, 75%) ameliorate the cell viability up to 20–28% and 30–38% respectively than the control. In contrast, extracts from body segments negatively influence cell viability up to 80%. In a scratch-wound assay, 25% RCF and 5% CF achieved 99.86% and 81.54% wound closure in 24 h, respectively, compared to 40% in controls. RCF and CF also possess enhanced anti-microbial activity against gram + ve bacteria. Western Blotting reveals that Wnt3a, HoxD3 and VEGF were remarkably upregulated in RCF and CF treated samples and their upregulated stemness property is effectively regulated by p53, TCTP, H2AX, Cleaved Caspase-3 proteins. Immunofluorescence data clearly states that Wnt3a and Caspase-3 signals are more profoundly observed in nuclear over cytoplasm in RCF treated samples and H2AX shows less nuclear signals than CF. In in-vivo earthworm model conditions, RCF remarkably promotes the survivability and wound healing ability by promoting the Wnt3a and VEGF expression together with downregulation of Cox2.

Abstract Background Neoantigen vaccines can induce or enhance highly specific antitumor immune responses with minimal risk of autoimmunity. We have developed a neoantigen DNA vaccine platform capable of efficiently presenting both HLA class I and II epitopes and performed a phase 1 clinical trial in triple-negative breast cancer patients with persistent disease on surgical pathology following neoadjuvant chemotherapy, a patient population at high risk of disease recurrence. Methods Expressed somatic mutations were identified by tumor/normal exome sequencing and tumor RNA sequencing. The pVACtools software suite of neoantigen prediction algorithms was used to identify and prioritize cancer neoantigens and facilitate vaccine design for manufacture in an academic GMP facility. Neoantigen DNA vaccines were administered via electroporation in the adjuvant setting (i.e., following surgical removal of the primary tumor and completion of standard of care therapy). Vaccines were monitored for safety and immune responses via ELISpot, intracellular cytokine production via flow cytometry, and TCR sequencing. Results Eighteen subjects received three doses of a neoantigen DNA vaccine encoding on average 11 neoantigens per patient (range 4–20). The vaccinations were well tolerated with relatively few adverse events. Neoantigen-specific T cell responses were induced in 14/18 patients as measured by ELISpot and flow cytometry. At a median follow-up of 36 months, recurrence-free survival was 87.5% (95% CI: 72.7–100%) in the cohort of vaccinated patients. Conclusion Our study demonstrates neoantigen DNA vaccines are safe, feasible, and capable of inducing neoantigen-specific immune responses. Clinical trial registration number NCT02348320.

Abstract Background Phenotypic age (PhenoAge), a widely used marker of biological aging, has been shown to be a robust predictor of all-cause mortality and morbidity in different populations. Existing studies on biological aging have primarily focused on individual domains, resulting in a lack of a comprehensive understanding of the multi-systemic dysregulation that occurs in aging. Methods PhenoAge was evaluated based on a linear combination of chronological age (CA) and 9 clinical biomarkers in 952 multi-ethnic Asian women of reproductive age. Phenotypic age acceleration (PhenoAgeAccel), an aging biomarker, represents PhenoAge after adjusting for CA. This study conducts an in-depth association analysis of PhenoAgeAccel with clinical, nutritional, lipidomic, gut microbiome, and genetic factors. Results Higher adiposity, glycaemia, plasma saturated fatty acids, kynurenine pathway metabolites, GlycA, riboflavin, nicotinamide, and insulin-like growth factor binding proteins were positively associated with PhenoAgeAccel. Conversely, a healthier diet and higher levels of pyridoxal phosphate, all-trans retinol, betaine, tryptophan, glutamine, histidine, apolipoprotein B, and insulin-like growth factors were inversely associated with PhenoAgeAccel. Lipidomic analysis found 132 lipid species linked to PhenoAgeAccel, with PC(O-36:0) showing the strongest positive association and CE(24:5) demonstrating the strongest inverse association. A genome-wide association study identified rs9864994 as the top genetic variant (P = 5.69E-07) from the ZDHHC19 gene. Gut microbiome analysis revealed that Erysipelotrichaceae UCG-003 and Bacteroides vulgatus were inversely associated with PhenoAgeAccel. Integrative network analysis of aging-related factors underscored the intricate links among clinical, nutritional and lipidomic variables, such as positive associations between kynurenine pathway metabolites, amino acids, adiposity, and insulin resistance. Furthermore, potential mediation effects of blood biomarkers related to inflammation, immune response, and nutritional and energy metabolism were observed in the associations of diet, adiposity, genetic variants, and gut microbial species with PhenoAgeAccel. Conclusions Our findings provide a comprehensive analysis of aging-related factors across multiple platforms, delineating their complex interconnections. This study is the first to report novel signatures in lipidomics, gut microbiome and blood biomarkers specifically associated with PhenoAgeAccel. These insights are invaluable in understanding the molecular and metabolic mechanisms underlying biological aging and shed light on potential interventions to mitigate accelerated biological aging by targeting modifiable factors.

Introduction: Psoriasis is a prevalent, long-lasting inflammatory skin condition characterised by various clinical manifestations and arising from the interaction of genetic, environmental, and immunological factors. It has been found to be associated with several medical conditions, such as hypertension, diabetes, Cardiovascular (CV) diseases, and metabolic syndrome. Aim: To compare the Electrocardiographic P Wave Dispersion (PWD) in individuals with and without psoriasis and to correlate PWD with the Psoriasis Area Severity Index (PASI) score. Materials and Methods: This was a hospital-based cross-sectional study comparing the electrocardiographic PWD in 30 patients with psoriasis to that of 30 age- and gender-matched subjects without psoriasis who attended Government Medical College, Trivandrum, Kerala, India, during a period of one year (February 2021 to January 2022). A structured questionnaire was used to record history and examination findings. A 12-lead Electrocardiogram (ECG) was performed on study participants from both groups in a supine position following 15 minutes of rest. Categorical and quantitative variables were expressed as frequency (percentage) and mean±SD, respectively. The Karl Pearson test was used to find correlations. A p-value of ≤0.05 was considered statistically significant. Results: Ten (33.3%) patients belonged to the age group of 41-50 years. The duration of the disease ranged from six months to 21 years. Seventeen (56.7%) patients had a PASI score between 10-20, and 5 (16.7%) patients had a PASI score of less than 10. The maximum value of the P wave and PWD was higher in the case group and was statistically significant (p≤0.001 and p≤0.001, respectively). A statistically significant correlation was found between the severity of psoriasis and PWD (r=0.564, p≤0.001) and between the duration of psoriasis and PWD (r=0.372, p=0.04) in this study. Conclusion: PWD was found to be higher in psoriasis patients, with a significant correlation between PWD and both the severity and duration of the disease. Therefore, ECG should be considered as a screening test for Atrial Fibrillation (AF) in all psoriasis patients.

Abstract Sentiment analysis (SA) of several user evaluations on e-commerce platforms can be used to increase customer happiness. This method automatically extracts and identifies subjective data from product evaluations using natural language processing (NLP) and machine learning (ML) methods. These statistics may eventually reveal information on the favourable, neutral, or negative attitudes of the consumer base. Due to its capacity to grasp the complex links between words and phrases in reviews as well as the emotions they imply, deep learning (DL) is very useful for SA tasks. A unique approach termed Weighted Parallel Hybrid Deep Learning-based Sentiment Analysis on E-Commerce Product Reviews (WPHDL-SAEPR) is introduced by the proposed system. Accurately distinguishing between distinct sentiments found in online store reviews is the aim of the WPHDL-SAEPR technique. Additional data pre-processing processes are implemented within the WPHDL-SAEPR architecture to guarantee compatibility. Words are embedded into the paper using the word2vec model, while sentiment is classified using the WPHDL model. The Restricted Boltzmann Machine (RBM) and Singular Value Decomposition (SVD) models are combined in this model. The results of the WPHDL-SAEPR approach’s simulation were assessed using a consumer review database, with the results being emphasized at each stage.

Introduction Forensic odontology plays a pivotal role in identifying individuals in legal contexts. This study analyses the efficacy of lip prints, tongue prints and mandibular canine width in determining gender, aiming to contribute to the evolving field of forensic dentistry. Methods A cross-sectional study was conducted involving 100 participants, analysing lip prints based on Tsuchihashi’s classification, recording tongue shapes and features through the use of alginate impressions and measuring mandibular canine width using a vernier calliper. The findings were reported in accordance with the Strengthening the Reporting of Observational Studies in Epidemiology checklist. Results In lip prints, Tsuchihashi’s Type I lip print was predominantly found in females, whereas Tsuchihashi’s Type III prevailed in males. Tongue shapes revealed a predominance of pentagonal shapes in males and ovoid shapes in females. The lingual apex was blunt in males and sharp in females. Mandibular canine width was higher in males than females on both the left and right sides. Conclusion The findings of this study support the use of lip prints, tongue prints and canine width in determining gender as ancillary and supporting methods.

Background/Aims By understanding the prevalence and causes of childhood blindness, stakeholders can work toward comprehensive strategies that encompass prevention, treatment, rehabilitation, and support, ultimately improving the quality of life for affected children. The information about the pattern and causes of childhood blindness in Northern Indian states is limited, it highlights the need for further research and data collection to better understand and address the specific challenges in this region. Therefore, the present cross-sectional study was planned to ascertain the different causes and patterns of childhood blindness and to understand the barriers to the use of LVAs and its compliance among the students attending schools for the blind in a North Indian State. Study Design and Setting Students from two schools for blind were examined in Chandigarh and its vicinity in North India. The different causes of severe visual impairment/blindness were classified as per the World Health Organization/Prevention of Blindness standard recording form. Results A total of 89 students attending schools for the blind were examined. The mean age was 13.10 years (range = 4–16 years). Male, female ratio was 3.3:1. The major causes of blindness were congenital anomalies (whole globe anomalies 37.08%), followed by retinal conditions (16.7 = 8%), undetermined/other (12.3), and lenticular conditions (112%). More than one-third of children (34.8%) were blind due to avoidable causes of blindness. Conclusion The current study data append on the existing national available data about childhood blindness in India. Though the sample size of the current is comparatively low, observed different results as compared to previously published reports from other regions, emphasize the strengthening of pediatric ophthalmology services as well as of low vision services in this particular region of North India.

Abstract Despite being a global public health problem, there are no antiviral agents for dengue. Plants are the sources of most approved drugs and many phytochemicals have exhibited in vitro antiviral activity. We explored the antiviral potential of the aqueous extract of Glycyrrhiza glabra roots (GGaq) on dengue viruses. Plaque reduction antiviral assay for half maximal inhibitory concentrations (IC50) was done in Vero cells infected with dengue serotypes 1–4 and exposed to varying concentrations of GGaq. Half-maximal cytotoxic concentration (CC50) of the GGaq was 651.9 µg/mL. The IC50 of the four dengue serotypes (10–50 µg/mL) indicated moderate inhibition by GGaq. 98–100% inhibition of all dengue serotypes was seen with GGaq at the maximum nontoxic dose. Preparative thin layer chromatography of GGaq, isolated sub fractions E and F which had low to moderate antiviral activity (IC50 12.65–85.95 and IC50 13.14–69.27 respectively). They exhibited good therapeutic potential only for dengue serotype-4. Virus adsorption to cells was significantly inhibited by GG (50%) and sub fraction E (24.9%). Chloroquine diphosphate was used as the positive control in all assays. The aqueous extract of GG was non-toxic and had better antiviral activity than sub fractions E and F.

Abstract Robust verification protocols are crucial for maintaining the security and reliability of sensitive information due to the increasing complexity of cyber-attacks. This paper introduces a novel 5G Secure Handover Protocol aimed at addressing security and effectiveness issues encountered in existing systems. The proposed protocol is robust against various attacks, including de-synchronization, replay, man-in-the-middle (MITM), denial of services (DoS), and jamming, ensures perfect forward key secrecy, safeguarding communication confidentiality. The proposed protocol utilizes a combination of spiking neural network and fuzzy logic (SNN-FL) techniques that must choose the goal cell as carefully as possible before initiating the transfer process. By combining fuzzy logic and spiking neural networks to reduce handover latency and thwart several types of cyberattacks, the proposed 5G Secure Handover Protocol improves security. Extensive simulations show its efficacy and emphasize its potential for safe communication in large-scale cybersecurity applications. The paper presents a novel secure authentication protocol that significantly reduces handover delays and improves efficiency. Simulations show its resilience against common security threats, protecting sensitive information and maintaining secure communication channels. The protocol, with low communication expenses, complex spatial, and latency for changeover verification, is ideal for large-scale cybersecurity applications, contributing to the development of secure digital authentication mechanisms.

Abstract This paper introduces the design and comprehensive performance evaluation of a novel Multi-Load and Multi-Source DC-DC converter tailored for electric vehicle (EV) power systems. The proposed converter integrates a primary battery power source with a secondary renewable energy source—specifically, solar energy—to enhance overall energy efficiency and reliability in EV applications. Unlike conventional multi-port converters that often suffer from cross-regulation issues and limited scalability, this converter ensures stable power distribution to various EV subsystems, including the motor, air conditioning unit, audio systems, and lighting. A key feature of the design is its ability to independently manage multiple power loads while maintaining isolated outputs, thus eliminating the inductor current imbalance that is common in traditional systems. Experimental validation using a 100 W prototype demonstrated the converter’s ability to deliver stable 24 V and 48 V outputs from a 12 V input, with output voltage deviations kept within ± 1%, significantly improving upon the ± 5% deviations typically seen in existing converters. Furthermore, the system achieved an impressive 93% efficiency under variable load conditions. The modular nature of the converter makes it not only suitable for EV applications but also for a broader range of industries, including renewable energy systems and industrial power supplies. This paper concludes by discussing optimization strategies for future improvements and potential scaling of the technology for commercial use in sustainable energy applications.

Abstract Hypertension is a significant public health concern and a modifiable risk factor for increased mortality worldwide. It is reported to be influenced by gene-environment interactions and micronutrient intake. This study aims to understand the relationship between global DNA methylation levels and hypertension, independently and in the context of micronutrient status, among rural population in Punjab, India. A total of 2300 individuals, aged 30–75 years, (54.9% females) were screened for blood pressure. Of 2300 screened individuals, 900 (age sex matched 450 cases and 450 controls of hypertension) individuals were selected to examine the relationship between hypertension, global DNA methylation (5mC), and biochemicals (Folate, Vitamin B12, and Homocysteine). Folate, vitamin B12, and homocysteine levels were estimated using chemiluminescence technique. The ELISA-based colorimetric technique was used for performing peripheral blood leucocyte (PBL) global DNA methylation (5mC). Statistical analyses were performed using SPSS version 22.0. Hypertensives were found to have significantly lower levels of global DNA methylation than normotensives (0.65 vs. 0.72 respectively; p-value = 0.01*). Individuals in the 1st quartile of 5mC were at significantly (OR: 1.671; 95% CI: 1.206–2.315; p-value = 0.01*) increased risk for hypertension in comparison to those in the 4th quartile (reference). Further hypertensives on medication with controlled blood pressure (BP) were significantly hypermethylated as compared to hypertensives on medication with uncontrolled BP (0.70 vs. 0.62 respectively; p-value = 0.04*). Folate appeared to mediate global DNA methylation among hypertensives on medication-controlled BP. Further hypertension driven hypomethylation hints towards accelerated biological aging among younger hypertensives. Hypertension may be associated with Global DNA hypomethylation in the studied rural population of Punjab, India. Folate sufficiency may prove to be an aid in improving the methylation levels among the cases of hypertension who were on medication and had controlled BP.

Abstract Deep eutectic electrolytes (DEEs) show promise for future electrochemical systems due to their adjustable buffer capacities. This study utilizes machine learning algorithms to analyse the carbon dioxide reduction reaction (CO2RR) in DEEs with a buffer capacity of approximately 10.21 mol/pH. The objective is to minimize undesired hydrogen evolution reactions (HER) and render CO2RR dominant in a membrane cell. The CO2RR process was found to be non-adiabatic, as the time of nuclear motion for CO3 2− in K2CO3 product, through CO2 ●− trapping, is 0.368 femtoseconds shorter than the 1.856 × 10−3s charge transfer relaxation time. Microkinetic analysis reveals that the rate of CO2RR to CO2 ●− is 2.14 × 103 mol/cm2/s2 with a rate constant of 2.1 × 1010 cm/s. Our findings demonstrate that ensemble and k-Nearest Neighbours algorithms learn the CO2RR dataset, achieving a prediction accuracy of over 99%. The models were verified visually and quantitatively by overlaying predicted and experimental dataset. Diagnostic and SHAP analyses highlighted the gradient boost ensemble algorithm, predicting asymptotic current densities of -4.114 mA/cm2 or -13.340 mA/cm2, with high turnover frequencies (TOF) of 3.79 × 1010 h-1 or 12.30 × 1010 h-1 for CO2 ●− or K2CO3 generation on silver electrodes, respectively. These results consider both accuracy and robustness against overfitting, providing an opportunity to optimize future non-aqueous electrolytes for convenient TOF measurements at industrially relevant current densities.

Abstract Meibomian Gland Dysfunction (MGD) and Dry Eye Disease (DED) comprise two of the most significant eye diseases, impacting millions of sufferers worldwide. Several etiological factors influence the early symptoms of DED. Early diagnosis and treatment of erectile dysfunction may significantly improve the Quality of Life (QoL) for people. The current study introduces the ESAE-ODNN, an improved stacked autoencoder-aided optimised deep neural network, as a new way to predict DED using feature selection (FS), feature extraction (FE), and classification. The approach described here is novel because it merges chaotic maps into FS, employs SLSTM-STSA for improved classification accuracy (CA), and optimizes with the adaptive quantum rotation of the Enhanced Quantum Bacterial Foraging Optimisation Algorithm (EQBFOA). The present study enhances prediction functions by extracting MGD-related features and complicated relationships from the DED dataset. To ensure essential feature identification, the ESAE minimizes irrelevant and redundant features. To predict the DED, the ESAE first applies FE and then implements an ODNN classifier. This method fine-tunes the ODNN framework to enhance the effectiveness of the classification. The proposed ESAE-ODNN classification system efficiently assists in the early diagnosis of DED. Combining advanced Deep Learning (DL) methods with optimization can help us understand MGD features better and sort the data with the best accuracy (96.34%). The experimental evaluation with relevant performance metrics indicates that the proposed method is efficient in diverse aspects: accurate identification, reduced complexity, and fine-tuned performance. The ESAE-ODNN’s robustness in handling intricate feature indications and high-dimensional data outperforms the existing state-of-the-art techniques.

Abstract This retrospective observational study, building on prior research that demonstrated the efficacy of the Digital Twin (DT) Precision Treatment Program over shorter follow-up periods​​, aimed to examine glycemic control and reduced anti-diabetic medication use after one-year in a DT commercial program. T2D patients enrolled had adequate hepatic and renal function and no recent cardiovascular events. DT intervention powered by artificial intelligence utilizes precision nutrition, activity, sleep, and deep breathing exercises. Outcome measures included HbA1c change, medication reduction, anthropometrics, insulin markers, and continuous glucose monitoring (CGM) metrics. Of 1985 enrollees, 132 (6.6%) were lost to follow-up, leaving 1853 participants who completed one-year. Abstract The Indian spot-billed duck, Anas poecilorhyncha is a large dabbling and non-migratory breeding bird. The identification and phylogenetic relationship of A. poecilorhyncha remain uncertain due to the presence of overlapping meristic characters and hybridization with closely related species. Molecular data aids when there are challenges in morphological identification. However, genetic characterization of A. poecilorhyncha has been paid less attention. Apart from their functional and physiological role, mitochondrial genome can also be used for various purposes, including species identification, phylogenetic analysis, understanding the domestication history of species etc. Therefore, the present study aimed to sequence the mitochondrial genome of A. poecilorhyncha and its closely related domestic species A. platyrhynchos (mallard duck) to understand their mitochondrial genome structure and phylogenetic relationships. The length of mitochondrial genome of A. poecilorhyncha and A. platyrhynchos was 16,608 and 16,604 bp respectively. Mitochondrial genome contained 37 genes and a non-coding control region. Overall, the characteristics of mitochondrial genome of both species were found to be conserved. The phylogenetic tree exhibited seven major clades (A to G) with a high bootstrap support. Notably, the Indian A. poecilorhyncha population formed a distinct clade (C) whereas the A. poecilorhyncha that were probably sampled from China grouped along with A. zonorhyncha (clade B). Besides, one of the A. poecilorhyncha probably sampled from China was placed in the clade A, which predominantly consisted of A. platyrhynchos. It suggests that Indian A. poecilorhyncha population is genetically different from Chinese A. poecilorhyncha population. Further, it sheds light on the importance of conducting a comprehensive phylogenetic study on these species. The newly sequenced mitochondrial genome of A. poecilorhyncha and A. platyrhynchos would be useful not only to have a better understanding of the phylogeny and evolution of Anas species but also to help in the conservation of A. poecilorhyncha which is under constant threat from rapid urbanization, interspecific hybridization and other human activities.

Abstract Antibody escape mutations pose a significant challenge to the effectiveness of vaccines and antibody-based therapies. The ability to predict these escape mutations with computer simulations would allow us to detect threats early and develop effective countermeasures, but a lack of large-scale experimental data has hampered the validation of these calculations. In this study, we evaluate the ability of the MD+FoldX molecular modeling method to predict escape mutations by leveraging a large deep mutational scanning dataset, focusing on the SARS-CoV-2 receptor binding domain. Our results show a positive correlation between predicted and experimental data, indicating that mutations with reduced predicted binding affinity correlate moderately with higher experimental escape fractions. We also demonstrate that higher precision can be achieved using affinity cutoffs tailored to distinct SARS-CoV-2 antibodies from four different classes rather than a one-size-fits-all approach. Further, we suggest that the quartile values of optimized cutoffs reported for each class in this study can serve as a valuable guide for future work on escape mutation predictions. We find that 70% of the systems surpass the 50% precision mark, and demonstrate success in identifying mutations present in significant variants of concern and variants of interest. Despite promising results for some systems, our study highlights the challenges in comparing predicted and experimental values. It also emphasizes the need for new binding affinity methods with improved accuracy that are fast enough to estimate hundreds to thousands of antibody-antigen binding affinities.

Abstract Malaria remains a global health problem, and the standard membrane feeding assay (SMFA) is a key functional assay for development of new interventions to stop malaria transmission from human to mosquito. For SMFA, media with ~ 10% of human serum has been used for infectious gametocyte cultures, however, there are multiple challenges to obtain a suitable human serum. Here we show a human-serum-free culture medium (HSF), which was a mixture of two stem cell culture media and AlbuMAX, supported infectious gametocyte growth. Moreover, the HSF-induced gametocytes elicited significantly higher numbers of oocysts compared to gametocytes cultured with conventional human serum medium (Conv). While some caution is required when comparing percent transmission reducing activity data generated from HSF-SMFA and Conv-SMFA, the HSF method can facilitate the establishment of gametocyte cultures or SMFA by bypassing the need for human serum. Thus, this study will support future development of P. falciparum transmission-blocking interventions.

Abstract While selenium is a cofactor of several antioxidant enzymes against cancer and is essential for human health, its excess intake may also be harmful. Though a safe intake of selenium has recently been recommended, it is not well understood in the Asian population. We aimed to determine the association between dietary intake of selenium and cancer risk in a case–control study of 3758 incident cancer cases (i.e., stomach, colon, rectum, lung cancers, and other sites) and 2929 control subjects in Vietnam. Daily intake of selenium was derived from a semiquantitative food frequency questionnaire. The unconditional logistic regression model was used to calculate the odds ratios (ORs) and 95% confidence intervals (CIs) for the association between selenium intake and cancer risk. We observed a U-shaped association between selenium intake and cancer risk. A safe intake ranged from 110.8 to 124.4 µg/day (mean 117.8 µg/day). Compared to individuals with the safe intake of selenium, individuals with the lowest intake (i.e., 27.8–77.2 µg/day) were associated with an increased risk of cancer (OR = 3.78, 95% CI 2.89–4.95) and those with the highest intake (169.1–331.7 µg/day) also had an increased cancer risk (OR = 1.86, 95% CI 1.45–2.39). Abstract We report direct measurements of the magnetic field screening at the limits of the Meissner phase for two superconducting niobium (Nb) samples. The samples are processed with two different surface treatments that have been developed for superconducting radio-frequency (SRF) cavity applications—a “baseline” treatment and an oxygen-doping (“O-doping”) treatment. The measurements show: (1) that the screening length is significantly longer in the “O-doping” sample compared to the “baseline” sample; (2) that the screening length near the limits of the Meissner phase increases with applied field; (3) the evolution of the screening profile as the material transitions from the Meissner phase to the mixed phase; and (4) a demonstration of the absence of any screening profile for the highest applied field, indicative of the full flux entering the sample. Measurements are performed utilizing the $$\beta$$ β -detected nuclear magnetic resonance ( $$\beta$$ β -NMR) technique that allows depth resolved studies of the local magnetic field within the first 100 nm of the surface. The study takes advantage of the $$\beta$$ β -SRF beamline, a new facility at TRIUMF, Canada, where field levels up to 200 mT are available parallel to the sample surface to replicate radio frequency fields near the Meissner breakdown limits of Nb.

Abstract Background The COVID 19 lockdown created a shift in medical education from the traditional physical classroom to online learning. Objectives To explore the lived experiences of students in various years of medical education attending a medical college in Chennai, India. Methods In this qualitative exploration of lived experiences we conducted 4 focus group discussions among students of the four years in the medical college with the help of a checklist. We recorded the interviews, transcribed them and performed a thematic content analysis. Results There was a gendered impact of the lockdown on the online learning experiences with women students finding it challenging to attend classes balancing their gender roles of performing household chores. Online learning offered some advantages in the form of increased participation and engagement due to the partial anonymity. The greatest disadvantage of online learning was lack of clinical learning experience. The students resorted to fabricating case studies for discussion, which some students found useful and some commented that it can never replace real life clinical discussions. A generational gap between adoption of technology between the senior professors and the students hampered the online learning. Online assessments were challenging, and many students resorted to cheating in these exams. Conclusions Though online learning offers several advantages, it has serious limitations in offering the clinical learning experience. While planning adoption of online learning into routine medical education adequate time must be set aside for real life clinical exposure in addition to the online lectures and demonstrations for conceptual understanding.

Introduction: The most crucial abilities for a Paediatric Dentist involve evaluating and managing children based on their behaviour. Children are often motivated to choose coloured restorative materials. They tend to hold their new fillings in high regard and are usually fascinated by the new materials in their oral cavity. Consequently, they are more likely to maintain the restoration, which greatly enhances their overall oral hygiene and fosters a favourable attitude towards therapy. Aim: To evaluate the behaviour of children and their acceptance of multicoloured and conventional composite materials: A randomised controlled trial. Materials and Methods: A split-mouth, single-blinded, randomised clinical study was conducted on 25 patients with Class I occlusal caries at the Department of Paediatric and Preventive Dentistry, Government Dental College and Hospital (GDCH), Nagpur, Maharashtra, India, from July 2023 to January 2024. Healthy children aged between 5 and 10 years, who were visiting the dentist for the first time, were selected from the Department of Paediatric and Preventive Dentistry. The chidren were divided in two groups-group 1, 13 (52%) children with blue coloured composite restorations and group 2, 12 (48%) chidren who received conventional composite restorations. Behaviour was assessed using the modified behaviour rating scale before and after treatment. Data analysis was performed using Statistical Package for Social Sciences (SPSS) software version 16.0 with the Wilcoxon’s Signed-rank Test. Results: Among total of 25 children, 14 (56%) were males and 11 (44%) were females with a mean age of 8.47±1.59 years. Abstract This paper introduces a novel design for a universal DC-DC and DC-AC converter tailored for DC/AC microgrid applications using Approximate Dynamic Programming and Artificial Neural Networks (ADP-ANN). The proposed converter is engineered to operate efficiently with both low-power battery and single-phase AC supply, utilizing identical side terminals and switches for both chopper and inverter configurations. This innovation reduces component redundancy and enhances operational versatility. The converter's design emphasizes minimal switch usage while ensuring efficient conversion to meet diverse load requirements from battery or AC sources. A conceptual example illustrates the design's principles, and comprehensive analyses compare the converter's performance across various operational modes. A test bench model, rated at 3000W, demonstrates the converter's efficacy in all five operational modes with AC/DC inputs. Experimental results confirm the system's robustness and adaptability, leveraging ADP-ANN for optimal performance. The paper concludes by outlining potential applications, including microgrids, electric vehicles, and renewable energy systems, highlighting the converter's key advantages such as reduced complexity, increased efficiency, and broad applicability.

Background: Rabies is an acute and 100% fatal viral disease transmitted to humans through the bite or lick of an infected animal. It can be successfully prevented if a complete course of anti-rabies vaccine (ARV) is taken following an animal bite. The present study reveals the compliance of animal bite victims to post-exposure prophylaxis (PEP). Aims and Objectives: The objectives of the study are as follows:(1) To determine the adherence to PEP among animal bite victims and factors influencing it. Materials and Methods: A retrospective record-based cross-sectional study was conducted at the anti-rabies clinic of VIMS Hospital Ballari. Information regarding sociodemographic variables, animal bite history, category of bite, treatment received, and completion of ARV schedule of all the animal bite cases were collected from the animal bite register and treatment card during the period from July 2023 to December 2023. Results: Out of the total 350 animal bite victims, all of them received the first dose of vaccine, 86.28% completed two doses, 72% completed three doses, and only 51.14% completed the schedule by taking all four doses, 37% of them completed the schedule without any delay, 14% delayed one or more doses, and the rest 49% did not complete the schedule. Conclusion: Adherence to PEP is a dire necessity as rabies is a fatal disease. Counseling the animal bite victims about the importance of adhering to the complete PEP schedule at the time of administering the first dose of vaccine is crucial.

Abstract Several farmed fish species, including carps, tilapia, salmon, and catfish, have experienced significant economic losses in aquaculture due to motile Aeromonas septicemia caused by Aeromonas hydrophila. In the present study, a novel lytic bacteriophage infecting hypervirulent Aeromonas hydrophila (vAh) was isolated and characterized. This is the first report of a phage against vAh. Phage AhFM11 demonstrated lytic activity against both vAh strains and the A. hydrophila reference strain ATCC 35654. The AhFM11 genome was sequenced and assembled, comprising 168,243 bp with an average G + C content of 41.5%. The genome did not harbor any antibiotic resistance genes. Genomic information along with transmission electron microscopy revealed that phage AhFM11 belongs to the Straboviridae family. Therapeutic application of monophage AhFM11 in fish showed 100% survival in injection, 95% in immersion and 93% in oral feeding of phage top-coated feed. Fish and chicken meat spiked with A. hydrophila and phage showed significant reduction of A. hydrophila. These findings support that phage AhFM11 can be used as a biocontrol agent against vAh as an alternative to antibiotics.

Introduction: Helicobacter pylori (H. pylori), despite its high prevalence in the Indian population, has been subjected to limited studies concerning its potential role as a risk factor for Metabolic Syndrome (MetS) and Insulin Resistance (IR). Proposed mechanisms include inflammatory mediators, atherogenic lipid profiles, and vasoconstriction. Aim: To determine the association between H. pylori infection and MetS components, focusing on Highly sensitive C-Reactive Protein (hs-CRP) levels, to enhance understanding and management of these conditions. Materials and Methods: This cross-sectional study was conducted at the Department of Medicine, Dr. D.Y. Patil Medical College, Hospital and Research Centre, Pimpri, Pune, Maharashtra, India spanning from September 2017 to August 2019. This study involved 100 dyspeptic patients aged 18 years or older undergoing upper Gastrointestinal (GI) endoscopy. Data collection included fasting/postprandial blood parameters, serum lipids, hs-CRP, and H. pylori detection via both rapid urease test and Histopathological Examination (HPE). Physical assessments covered height, weight, Waist Circumference (WC), and blood pressure. MetS was evaluated using the National Cholesterol Education Program Adult Treatment Panel III (NCEP ATP III) criteria. Statistical analysis employed unpaired t-tests, Chi-square tests, and Fisher-Exact tests using IBM Statistical Package for Social Sciences (SPSS) version 21.0. Mucopolysaccharidosis (MPS) type II, or Hunter’s syndrome, is an X-linked recessive disorder with a defect in Glycosaminoglycan (GAG) metabolism resulting in a deficiency of the enzyme Iduronate sulfatase. Hydrocephalus (HC) is a condition in which the volume of Cerebrospinal Fluid (CSF) in the cerebral ventricles is abnormally elevated. HC patients may show macrocephaly and delayed development. A Ventriculoperitoneal (VP) shunt is the most commonly used treatment option. Hereby, the authors present a case report of a nine-year-old male child diagnosed with Hunter’s syndrome and HC. The manuscript provides a comprehensive overview of the dental management strategies involving a team of specialists, including a paediatric dentist and a paediatrician. Key interventions included emphasising assessment of oral health issues related to the syndrome and condition, modification of treatment plans like implementation of a tailored oral hygiene regimen and fluoride treatments, use of sedative techniques to manage anxiety and facilitate thorough dental work, regular follow-up appointments to monitor oral health and accommodate the child’s physical and developmental needs, preventive care, modified treatment techniques, and close monitoring of systemic health. Effective dental management of patients with HC and Hunter’s Syndrome requires a comprehensive, multidisciplinary approach. Tailoring dental care to accommodate the unique needs of these patients can lead to successful outcomes and improved quality of life. This case underscores the importance of collaboration between dental and medical professionals in managing complex cases.

Introduction: Pituitary apoplexy is a rare endocrine emergency that is occasionally life-threatening, most often involving pituitary macroadenomas. Most pituitary macroadenomas are nonsecretory and are diagnosed in the late stages. They remain undiagnosed until they cause compression of the surrounding vital structures and lead to subsequent symptoms. The clinical feature depends on the size of the tumor and the direction of the spread. There are two approaches for surgery. The less invasive endoscopic transnasal transsphenoidal approach is favored more than the transcranial approach. Case Report: 46-year-old female patient presented to the outpatient department (OPD) with complaints of headache and drooping of the right eyelid for 1.5 months. On neurological examination, right severe ptosis was present and extraocular movements of the right eye are restricted for the muscles supplied by the third nerve. Pupils were reactive to light. On perimetry, the left eye showed temporal hemianopia. Right eye perimetry was not done due to ptosis. MRI Brain gave differential diagnosis as pituitary macroadenoma with hemorrhage or necrosis and pituitary apoplexy without macroadenoma. Endoscopic transnasal transsphenoidal excision of the tumor under general anesthesia was performed. Conclusion: MRI brain is the preferred imaging for cases with high suspicion of pituitary apoplexy. Endoscopic transnasal transsphenoidal excision of the tumor is an excellent choice for surgical resection of pituitary adenomas, with improved rates of complete tumor removal and reduced incidence of complication.

Background: The clinical debut of erythropoiesis-stimulating medicines (ESAs) radically altered anaemia care. The effect of erythropoietin (EPO), pegylated erythropoietin and mircera on anaemia in chronic kidney disease (CKD) patients receiving continuous haemodialysis, peritoneal dialysis, or no dialysis was studied in this retrospective analysis. Methods: The retrospective analysis comprised of 490 CKD patients with erythropoietin hyporesponsiveness who were either on haemodialysis or peritoneal dialysis or not on dialysis. The patients’ records were separated into three groups: (a) those with CKD 3-5 (without dialysis), (b) those on haemodialysis, and (c) those on peritoneal dialysis (PD). Serum ferritin, haemoglobin, and transferrin saturation were measured. Serum ferritin was determined using a two-site sandwich immunoassay based on direct chemiluminometric technique, which employs two antiferritin antibodies at constant concentrations that are collated and statistically observed using bivariate analysis. Result: The participants’ mean ages were 46.9±20.4, 60.5±9.7, and 48.2±10.1 years for peritoneal dialysis patients, CKD patients on haemodialysis, and CKD 3 5 patients without dialysis, respectively. Abstract Meniscus-derived stem cells (MeSCs), a unique type of MSC, have outstanding advantages in meniscal cytotherapy and tissue engineering, but the effects and molecular mechanisms of PBM on MeSCs are still unclear. We used 660-nm LED light with different energy densities to irradiate six human MeSC samples and tested their proliferation rate via cell counting, chondrogenic differentiation capacity via the DMMB assay, mitochondrial activity via the MTT assay, and gene expression via qPCR. The proliferation ability, chondrogenic capacity and mitochondrial activity of the 18 J/cm2 group were greater than those of the 4 J/cm2 and control groups. The mRNA expression levels of Akt, PI3K, TGF-β3, Ki67 and Notch-1 in the 18 J/cm2 group were greater than those in the other groups in most samples. After chondrogenic induction, the expression of Col2A1, Sox9 and Aggrecan in the 18 J/cm2 group was significantly greater than that in the 4 J/cm2 and control groups in most of the samples. The variation in the MTT values and Src, PI3K, Akt, mTOR and GSK3β levels decreased with time. The results showed that 660-nm LED red light promoted proliferation and chondrogenic differentiation and affected the gene expression of MeSCs, and the effects on gene expression and mitochondrial activity decreased with time.

Introduction: The success of fixed restorations depends on marginal integrity, particularly in subgingival areas. Gingival retraction facilitates proper placement of impression material in the displaced gingival sulcus; however, a lack of consensus on evaluation criteria hinders comparative studies of gingival retraction systems. Aim: To evaluate the effectiveness of mechanical retraction cord and chemical gingival retraction paste compared to a control group with no retraction. Materials and Methods: This study was a cross-over clinical trial conducted at Saveetha Dental College and Hospitals in Chennai, Tamil Nadu, India, involving 20 patients requiring single crowns from May 2024 and June 2024. Patients were allocated into three groups-no retraction, chemical retraction, and mechanical retraction-based on randomisation. Patients with healthy gingival and periodontal status, exhibiting no bleeding on probing, were included in the study. Impressions were taken at baseline, and subsequent gingival displacements on days 7 and 27 were performed using chemical and mechanical methods according to random allocation. A Visual Analogue Scale (VAS) score was used to analyse post-operative comfort. Gingival displacement was measured with a stereomicroscope, and results were tabulated. Data analyses were performed using Statistical Package for Social Sciences (SPSS) software (version 26.0). Statistical significance was set at a threshold of p chemical retraction > no retraction. For VAS scores, statistically significant results were observed for mechanical retraction compared to no retraction (0.9±0.052) and mechanical retraction (3.40±0.049), as well as between no retraction and chemical retraction (2.6±0.057) (p0.05). Conclusion: Although there was a statistically significant difference in the amount of displacement between the chemical and mechanical systems, both were within the clinically acceptable range (220 microns). Hence, chemical retraction can be used as a substitute for mechanical retraction techniques.

Abstract Oligodeoxynucleotides containing CpG motifs (CpG-ODN) can promote antimicrobial immunity in chickens by enriching immune compartments and activating immune cells. Innate memory, or trained immunity, has been demonstrated in humans and mice, featuring the absence of specificity to the initial stimulus and subsequently cross-protection against pathogens. We hypothesize that CpG-ODN can induce trained immunity in chickens. We delivered single or multiple administrations of CpG-ODN to birds and mitochondrial oxidative phosphorylation (OXPHOS) and glycolysis of peripheral blood mononuclear cells were quantified using Seahorse XFp. Next, chickens were administered with CpG-ODN twice at 1 and 4 day of age and challenged with Escherichia coli at 27 days of age. The CpG-ODN administered groups had significantly higher mitochondrial OXPHOS until 21 days of age while cellular glycolysis gradually declined by 14 days of age. The group administered with CpG-ODN twice at 1 and 4 days of age had significantly higher survival, lower clinical score and bacterial load following challenge with E. coli at 27 d of age. This study demonstrated the induction of trained immunity in broiler chickens following administration of CpG-ODN twice during the first 4 days of age to protect birds against E. coli septicemia at 27 days of age.

Abstract Background Rare disorders comprise of ~ 7500 different conditions affecting multiple systems. Diagnosis of rare diseases is complex due to dearth of specialized medical professionals, testing labs and limited therapeutic options. There is scarcity of data on the prevalence of rare diseases in different populations. India being home to a large population comprising of 4600 population groups, of which several thousand are endogamous, is likely to have a high burden of rare diseases. The present study provides a retrospective overview of a cohort of patients with rare genetic diseases identified at a tertiary genetic test centre in India. Results Overall, 3294 patients with 305 rare diseases were identified in the present study cohort. These were categorized into 14 disease groups based on the major organ/ organ system affected. Highest number of rare diseases (D = 149/305, 48.9%) were identified in the neuromuscular and neurodevelopmental (NMND) group followed by inborn errors of metabolism (IEM) (D = 47/305; 15.4%). Majority patients in the present cohort (N = 1992, 61%) were diagnosed under IEM group, of which Gaucher disease constituted maximum cases (N = 224, 11.2%). Under the NMND group, Duchenne muscular dystrophy (N = 291/885, 32.9%), trinucleotide repeat expansion disorders (N = 242/885; 27.3%) and spinal muscular atrophy (N = 141/885, 15.9%) were the most common. Majority cases of β-thalassemia (N = 120/149, 80.5%) and cystic fibrosis (N = 74/75, 98.7%) under the haematological and pulmonary groups were observed, respectively. Founder variants were identified for Tay-Sachs disease and mucopolysaccharidosis IVA diseases. Recurrent variants for Gaucher disease (GBA:c.1448T > C), β-thalassemia (HBB:c.92.+5G > C), non-syndromic hearing loss (GJB2:c.71G > A), albinism (TYR:c.832 C > T), congenital adrenal hyperplasia (CYP21A2:c.29–13 C > G) and progressive pseudo rheumatoid dysplasia (CCN6:c.298T > A) were observed in the present study. Conclusion The present retrospective study of rare disease patients diagnosed at a tertiary genetic test centre provides first insight into the distribution of rare genetic diseases across the country. This information will likely aid in drafting future health policies, including newborn screening programs, development of target specific panel for affordable diagnosis of rare diseases and eventually build a platform for devising novel treatment strategies for rare diseases.

Aim: To quantify and compare salivary levels of S100A8, S100A9 and S100A8/9 in periodontal health, gingivitis or stage 1 periodontitis and stages 3 or 4 periodontitis. Materials and Methods: A cross-sectional analytical study was carried out in the Department of Periodontics, Saveetha Dental College and Hospital, Chennai, India, from December 2022 to February 2023. Periodontal examination and saliva sample collection were done for sixty-eight consecutively enrolled subjects who met the inclusion and exclusion criteria. Study subjects were categorised as group 1, which comprised participants with clinical periodontal health (n=20), group 2, subjects with gingivitis or stage 1 periodontitis (n=20), and group 3, which included patients with stages 3 or 4 periodontitis (n=28). Detailed clinical examination and periodontal charting were done in all study subjects. Saliva samples were processed and stored at -80°C, and enzyme-linked immunosorbent assay was done to quantify S100A8, S100A9 and S100A8/9. Data were analysed using International Business Machines (IBM) Statistical Package for the Social Sciences (SPSS) software version 25.0. Salivary levels of S100A8, S100A9 and S100A8/9 were expressed as mean and standard deviation values. Mean values of the three salivary proteins were compared using the non parametric Kruskal-Wallis test. Bonferroni adjusted pairwise comparisons were also done. The p-values less than 0.05 were considered statistically significant. Results: The mean age of study participants was 38.97±7.62 years. Salivary levels of S100A8 were 41.34±14.34, 141.95± 185.25 and 901.37±65.60 pg/mL in groups 1, 2 and 3, respectively. Abstract This research paper introduces an avant-garde poly-input DC–DC converter (PIDC) meticulously engineered for cutting-edge energy storage and electric vehicle (EV) applications. The pioneering converter synergizes two primary power sources—solar energy and fuel cells—with an auxiliary backup source, an energy storage device battery (ESDB). The PIDC showcases a remarkable enhancement in conversion efficiency, achieving up to 96% compared to the conventional 85–90% efficiency of traditional converters. This substantial improvement is attained through an advanced control strategy, rigorously validated via MATLAB/Simulink simulations and real-time experimentation on a 100 W test bench model. Simulation results reveal that the PIDC sustains stable operation and superior efficiency across diverse load conditions, with a peak efficiency of 96% when the ESDB is disengaged and an efficiency spectrum of 91–95% during battery charging and discharging phases. Additionally, the integration of solar power curtails dependence on fuel cells by up to 40%, thereby augmenting overall system efficiency and sustainability. The PIDC’s adaptability and enhanced performance render it highly suitable for a wide array of applications, including poly-input DC–DC conversion, energy storage management, and EV power systems. This innovative paradigm in power conversion and management is poised to significantly elevate the efficiency and reliability of energy storage and utilization in contemporary electric vehicles and renewable energy infrastructures.

Abstract This study systematically evaluates biomimicry research within the context of sustainable development goals (SDGs) to discern the interdisciplinary interplay between biomimicry and SDGs. The alignment of biomimicry with key SDGs showcases its interdisciplinary nature and potential to offer solutions across the health, sustainability, and energy sectors. This study identified two primary thematic clusters. The first thematic cluster focused on health, partnership, and life on land (SDGs 3, 17, and 15), highlighting biomimicry's role in healthcare innovations, sustainable collaboration, and land management. This cluster demonstrates the potential of biomimicry to contribute to medical technologies, emphasizing the need for cross-sectoral partnerships and ecosystem preservation. The second thematic cluster revolves around clean water, energy, infrastructure, and marine life (SDGs 6, 7, 9, and 14), showcasing nature-inspired solutions for sustainable development challenges, including energy generation and water purification. The prominence of SDG 7 within this cluster indicates that biomimicry significantly contributes to sustainable energy practices. The analysis of thematic clusters further revealed the broad applicability of biomimicry and its role in enhancing sustainable energy access and promoting ecosystem conservation. Emerging research topics, such as metaheuristics, nanogenerators, exosomes, and bioprinting, indicate a dynamic field poised for significant advancements. By mapping the connections between biomimicry and SDGs, this study provides a comprehensive overview of the field's trajectory, emphasizing its importance in advancing global sustainability efforts.

Abstract Objective miR-497/195, located at 17p13.1, is a highly conserved miRNA cluster whose abnormal expression is a key regulator of carcinogenesis. We performed a comprehensive analysis of the miR-497/195 cluster to determine its prognostic utility and role in cervical cancer (CC) using publicly available datasets. Results In silico analysis and validation revealed that this cluster is downregulated in CC. A total of 60 target genes of miR-497/195 cluster were identified as differentially expressed between normal and CC samples. ShinyGO, STRING, CytoHubba, Timer 2.0, HPA, and HCMBD were used for functional enrichment, PPIN network construction, hub gene identification, immune infiltration correlation, histopathological expression, and determination of the metastatic potential of miR-497/195 cluster and their target genes. PPIN analysis identified CCNE1, CCNE2, ANLN, RACGAP1, KIF23, CHEK1, CDC25A, E2F7, CDK1, and CEP55 as the top 10 hub genes (HGs). Furthermore, the upregulation of RECK, ATD5, and BCL2, downregulation of OSBPL3, RCAN3, and HIST1H3H effected overall survival of CC patients. We identified 6 targets (TFAP2A, CLSPN, RASEF, HIST1H3H, AKT3, and ITPR1) of miR-497/195 cluster to influence metastasis. In addition, 8 druggable genes and 38 potential drugs were also identified. Our study identified miR-497/195 cluster target genes and pathways that could be used for prognostic and therapeutic applications in CC.

Introduction: Sleep is a vital biological, reparative, and restorative phenomenon, and poor sleep has a serious detrimental effect on health. Poor sleep quality is a serious problem as it adversely affects a person’s ability to function physically and socially, as well as their potential for profession and quality of life. The degree of hypertension directly affects sleep quality, and poor sleep can exacerbate hypertensive symptoms. This study assesses hypertension patients’ sleep quality in a rural health training centre in Chennai. Objectives: 1. To study the prevalence of poor sleep quality among hypertensive patients. 2. To determine the association between selected socio-demographic factors and sleep quality among these patients. Materials and Methods: Hypertensives attending the outpatient department of a rural health and training centre were selected in a systematic manner and administered a semi-structured interview schedule and the Pittsburgh Sleep Quality Index (PSQI) questionnaire followed by a basic clinical examination. Using the PSQI, they were categorised as having either poor or good sleep quality and the association between selected socio-demographic factors and sleep quality was assessed. Results: Out of the 179 study participants, there were almost equal numbers of male and female participants. The mean age of participants was 57.80 years (standard deviation (SD) = 9.8 yrs.). Among the study participants, 44.13% had completed secondary level of school as an education qualification and 6.70% were illiterate. By the modified BG Prasad classification 2022, most of the study participants (34.08%) came under the class II socioeconomic classification. A large proportion (78.8%) of study participants had poor sleep quality. The difference in sleep quality was observed with respect to gender, socio-economic class, alcohol consumption and body mass index (BMI). Conclusion: This observational study shows that individuals with hypertension suffer significantly from poor sleep. A poor quality of sleep was reported by 78.77% of the patients in this study. Statistically significant factors that influenced sleep quality were gender, alcohol consumption and BMI.

Context: Rabies is a fatal zoonotic disease that is endemic in India. Both forms of rabies, namely, encephalitic and furious, invariably lead to coma and death. Lack of awareness is the most critical barrier in accessing timely post-exposure prophylaxis that can prevent the progression to clinical disease. Aims: To report the case series of fourteen clinically suspected rabies cases. Settings and Design: We followed a retrospective study design. Data were collected from the Anti Rabies Section of Government Medical College, Jammu, which is a tertiary care hospital of Jammu Division. Methods and Material: We collected data of all patients over a period of three years, from 2017 to 2020, who were diagnosed as cases of clinically suspected rabies on the basis of classic signs and symptoms and who finally succumbed to the disease. Statistical Analysis Used: Descriptive statistics was used to describe the results of the study Results: In the present description, the mean age of the patients was 31 ± 18.6 years. Out of 14 suspected cases, 9 had not received any post-exposure prophylaxis—neither vaccine nor serum—and instead visited spiritual faith healers. The remaining patients had history of either incomplete, unknown, or incorrect vaccination status. Median duration of presentation of symptoms after dog bite was 42 days, with a range of 15–180 days. Mean survival of patients after presentation of symptoms was around 1.8 days Conclusions: Low level of awareness resulted in non-seeking of health-care services after animal bites.

Introduction: Renography is a non invasive technique routinely used by clinicians to provide information about kidney structure and function. However, its role in the diagnosis of renovascular disease (especially in patients with renal insufficiency), the exclusion of obstruction, and during the follow-up of patients undergoing pyeloplasty is still controversial. Aim: To study the changes in Anteroposterior Pelvic Diameter (APD), cortical thickness, split renal function and T½ post pyeloplasty at three months and to determine the usefulness of these parameters in assessing successful pyeloplasty for Pelvic Ureteric Junction Obstruction (PUJO). Materials and Methods: This prospective non randomised interventional study was conducted in the Department of Nuclear Medicine, PGIMER, Chandigarh, India and Department of Paediatrics, JIPMER, Karaikal, Puducherry, India, from July 2020 to July 2021. A total of 31 infants with persistent postnatal Hydronephrosis (HDN) on Ultrasonography (USG) with no vesicoureteral reflux were included in the study and underwent 99mTc EC renography. The diagnosis of obstruction was determined by visual interpretation, renogram curves, Time to peak (Tmax), and Time from Tmax to T½max parameters. Patients with obstructed patterns in 99mTc EC renography underwent Anderson Hyne’s dismembered pyeloplasty. After three months of surgery, 99mTc EC renography and USG were performed on all enrolled patients. The Chi-square test was used for the comparison of the difference in proportion, and the Student’s t-test was used for the comparison of the mean difference, between two groups. Results: The mean age of enrolled infants was 5.79±3.36 months with a male preponderance (27 males and 4 females). Out of 31 infants, 23 (74%) showed an obstructive pattern of drainage on diuretic renogram and underwent Anderson-Hynes dismembered pyeloplasty. The observed difference in the preoperative renal USG anteroposterior diameter (27.85±14.3 mm) and parenchymal thickness (9.6±3.3 mm) vs postoperative anteroposterior diameter (8±3.19 mm) and parenchymal thickness (15.5±4.19 mm) was statistically significant (p-value=0.001). The follow-up renogram scan conducted at three months showed a significant reduction in clearance half-time (T½). However, there was no statistically significant variation in split renal function at three months after pyeloplasty. Conclusion: Both 99mTc EC renography and USG indicate the likelihood of successful pyeloplasty, and in settings with limited resources, USG may be a viable substitute for early follow-up after pyeloplasty.

Abstract Background Efficient doctor-patient communication is essential for improving patient care. The impact of educational interventions on the communication skills of male and female students has not been systematically reviewed. The aim of this review is to identify interventions used to improve communication skills in medical curricula and investigate their effectiveness in improving the communication skills of male and female medical students. Methods A systematic review of the literature was conducted using the PRISMA guidelines. Inclusion criteria were as follows: used intervention strategies aiming to improve communication skills, participants were medical students, and studies were primary research studies, systematic reviews, or meta-analyses. Results 2913 articles were identified based on search terms. After title, abstract, and full-text review, 58 studies were included with interventions consisting of Training or Drama Courses, Curriculum-Integrated, Patient Learning Courses, and Community-Based Learning Courses. 69% of articles reported improved communication skills for both genders equally, 28% for women more than men, and 3% for men more than women. 16 of the 58 articles reported numerical data regarding communication skills pre-and post-intervention. Coronal fracture of the anterior teeth is a commonly occurring dental trauma often seen in young individuals. Such fractures can be conservatively managed with fragment reattachment procedure when the fragment is available and preserved in correct storage media. Due to advancements in adhesive dentistry, fragment reattachment is a simple, esthetic and fast treatment option. This article reports a case of fragment reattachment of a complicated crown root fracture using fibre post as an intraradicular splint.

Abstract The purpose of the current study was to examine chemical composition, antioxidant, anti-inflammatory and antibacterial properties of leaves extract of Ajuga integrifolia Buch.-Ham. The antibacterial and antioxidant properties of three different solvents i.e. methanol (AIM), hexane (AIH), and water (AIW) were tested against two bacterial strains Staphylococcus aureus and Escherichia coli. The presence of antioxidant and antibacterial chemicals, such as hexanedioic acid, hexadecanoic acid, nonadecadiene, hexadecen-1-ol, octadecadienoic acid, nonane, phytol, henicosanal, stearyl aldehyde, and neophytadiene, were depicted in the GCMS chromatograms of three extracts. After the extracts' FTIR peaks were examined, it was discovered that phenols, amines, hydroxy groups, and components linked to amino acids were present. Compared to the Hexane and Water extracts, the Methanol extract showed a greater phenolic (196.16 ± 0.0083 mg gallic acid equivalent/gram), flavonoid (222.77 ± 0.002 mg rutin equivalents/g) and phosphomolybdate assay for total antioxidant activity (557.62 ± 0.0023 mg AAE/g). Methanol extract showed the highest scavenging activity with a minimum IC50 value was observed in DPPH assay. AIM showed its maximum anti-denaturation activity i.e. 3.75 ± 0.28%. For antibacterial activities, best zone of inhibition (ZOI) and minimum inhibitory concentration (MIC) was observed in case of the methanol extract as compared to other extracts against methicillin-resistant Staphylococcus aureus and β-lactam-resistant Escherichia coli.

Abstract Flue gas emissions are the waste gases produced during the combustion of fuel in industrial processes, which are released into the atmosphere. These identical processes also produce a significant amount of wastewater that is released into the environment. The current investigation aims to assess the viability of simultaneously mitigating flue gas emissions and remediating wastewater in a bubble column bioreactor utilizing bacterial consortia. A comparative study was done on different growth media prepared using wastewater. The highest biomass yield of 3.66 g L−1 was achieved with the highest removal efficiencies of 89.80, 77.30, and 80.77% for CO2, SO2, and NO, respectively. The study investigated pH, salinity, dissolved oxygen, and biochemical and chemical oxygen demand to assess their influence on the process. The nutrient balance validated the ability of bacteria to utilize compounds in flue gas and wastewater for biomass production. The Fourier Transform–Infrared Spectrometry (FT–IR) and Gas Chromatography–Mass Spectrometry (GC–MS) analyses detected commercial-use long-chain hydrocarbons, fatty alcohols, carboxylic acids, and esters in the biomass samples. The nuclear magnetic resonance (NMR) metabolomics detected the potential mechanism pathways followed by the bacteria for mitigation. The techno-economic assessment determined a feasible total capital investment of 245.74$ to operate the reactor for 288 h. The bioreactor’s practicability was determined by mass transfer and thermodynamics assessment. Therefore, this study introduces a novel approach that utilizes bacteria and a bioreactor to mitigate flue gas and remediate wastewater.

Abstract In response to the COVID-19 pandemic, the American Board of Anesthesiology transitioned from in-person to virtual administration of its APPLIED Examination, assessing more than 3000 candidates for certification purposes remotely in 2021. Four hundred examiners were involved in delivering and scoring Standardized Oral Examinations (SOEs) and Objective Structured Clinical Examinations (OSCEs). More than 80% of candidates started their exams on time and stayed connected throughout the exam without any problems. Only 74 (2.5%) SOE and 45 (1.5%) OSCE candidates required rescheduling due to technical difficulties. Of those who experienced “significant issues”, concerns with OSCE technical stations (interpretation of monitors and interpretation of echocardiograms) were reported most frequently (6% of candidates). In contrast, 23% of examiners “sometimes” lost connectivity during their multiple exam sessions, on a continuum from minor inconvenience to inability to continue. 84% of SOE candidates and 89% of OSCE candidates described “smooth” interactions with examiners and standardized patients/standardized clinicians, respectively. However, only 71% of SOE candidates and 75% of OSCE candidates considered themselves to be able to demonstrate their knowledge and skills without obstacles. When compared with their in-person experiences, approximately 40% of SOE examiners considered virtual evaluation to be more difficult than in-person evaluation and believed the remote format negatively affected their development as an examiner. The virtual format was considered to be less secure by 56% and 40% of SOE and OSCE examiners, respectively. The retirement of exam materials used virtually due to concern for compromise had implications for subsequent exam development. The return to in-person exams in 2022 was prompted by multiple factors, especially concerns regarding standardization and security. The technology is not yet perfect, especially for testing in-person communication skills and displaying dynamic exam materials. Nevertheless, the American Board of Anesthesiology’s experience demonstrated the feasibility of conducting large-scale, high-stakes oral and performance exams in a virtual format and highlighted the adaptability and dedication of candidates, examiners, and administering board staff.

Abstract Background Effective mentorship is an important component of medical education with benefits to all stakeholders. In recent years, conceptualization of mentorship has gone beyond the traditional dyadic experienced mentor-novice mentee relationship to include group and peer mentoring. Existing theories of mentorship do not recognize mentoring’s personalized, evolving, goal-driven, and context-specific nature. Evidencing the limitations of traditional cause-and-effect concepts, the purpose of this review was to systematically search the literature to determine if mentoring can be viewed as a complex adaptive system (CAS). Methods A systematic scoping review using Krishna’s Systematic Evidence-Based Approach was employed to study medical student and resident accounts of mentoring and CAS in general internal medicine and related subspecialties in articles published between 1 January 2000 and 31 December 2023 in PubMed, Embase, PsycINFO, ERIC, Google Scholar, and Scopus databases. The included articles underwent thematic and content analysis, with the themes identified and combined to create domains, which framed the discussion. Results Of 5,704 abstracts reviewed, 134 full-text articles were evaluated, and 216 articles were included. The domains described how mentoring relationships and mentoring approaches embody characteristics of CAS and that mentorship often behaves as a community of practice (CoP). Mentoring’s CAS-like features are displayed through CoPs, with distinct boundaries, a spiral mentoring trajectory, and longitudinal mentoring support and assessment processes. Conclusion Recognizing mentorship as a CAS demands the rethinking of the design, support, assessment, and oversight of mentorship and the role of mentors. Further study is required to better assess the mentoring process and to provide optimal training and support to mentors.
