Abstract This study aims to evaluate the applicability of the high-resolution WaveFront Phase Imaging Sensor (WFPI) in eyes with Fuchs’ Endothelial Corneal Dystrophy (FECD) through qualitative and quantitative analysis using a custom-designed Automatic Guttae Detection Method (AGDM). The ocular phase was measured using the t $$\cdot$$ · eyede aberrometer and then was processed to obtain its High-Pass Filter Map (HPFM). The subjects were pathological and healthy patients from the Fundación Jiménez-Díaz Hospital (Madrid, Spain). The AGDM was developed and applied in pupils with 3 and 5 mm of diameter. A set of metrics were extracted and evaluated like the Root-Mean-Square error (RMS), Number of guttae, Guttae Area, and Area of Delaunay Triangulation (DT). Finally, a Support Vector Machine (SVM) model was trained to classify between pathological and healthy eyes. Quantitatively, the HPFM reveals a dark spots pattern according to the ophthalmologist’s description of the slit-lamp examination of guttae distribution. There were significant statistical differences in all the metrics when FECD and Healthy groups were compared using the same pupil size; but comparing both pupil sizes for the same group there were significant differences in most of the variables. This sensor is a value tool to objectively diagnose and monitor this pathology through wavefront phase changes.

Abstract Introduction To date, there are no sufficient studies aimed to determine a correlation between personal, academic, and psychological variables with academic achievement, measured with the grade point average (GPA) and intelligence in university students according to each sex. Study aim To determine the correlation between studying strategies, personal and psychological factors with GPA and intelligence in a sample of health sciences university students. Methods Health Sciences university students, were invited to participate, those who accepted were cited in a computer room where they signed an informed consent and filled an electronic questionnaire with sociodemographic, behavioral, psychological variables and studying strategies (from the MLSQ instrument) afterwards they performed a verbal and non-verbal intelligence test (Shipley-2). Results A total of 439 students were included, from which 297 (67.7%) were women. The mean of age was 20.34 ± 2.61 years old. We found that no differences in GPA where observed between sexes. We detected a higher correlation between combined intelligence and GPA in women than in men. In addition, most studying strategies showed a higher correlation with GPA than intelligence scores in men´s sample. All these findings coincide with the fact that preparatory GPA was the most correlated variable with university GPA in both sexes. Finally, women showed higher levels of the sum of diseases, somatization, anxiety, depression and academic stress than men, and all these variables showed low significant correlations with the combined intelligence score only in women´s sample. Conclusion Verbal and non-verbal intelligence scores show a lower association to GPA in men than in women, while studying strategies showed a higher association with GPA in men than in women.

Chagas disease is a parasitic infection found mainly in continental Latin America. However, it is now present globally due to increasing international mobility and its lifelong persistence in the absence of timely treatment. In Switzerland, this neglected tropical disease presents a significant challenge because of its high potential for transmission, severe clinical manifestations and complications, and a lack of medical and public health programs at both the cantonal and national levels. This paper reviews the medical and public health interventions addressing Chagas disease in countries outside Latin America. It provides a specific perspective on the challenges, opportunities, and strategies for the elimination of Chagas disease as a public health problem within the Swiss context. This article concludes with the introduction of the newly created Swiss Chagas Network, which aims to develop strategies for improving the management of Chagas disease in Switzerland.

BackgroundThe current standard of interpretation provision is not efficacious or not acceptable to Deaf patients who communicate using sign language. In-person or video relay interpretation (VRI) sign language interpretation is largely unavailable. There are no clear data on the availability of VRI or in-person interpretation. Given the limited number of available sign language interpreters and the cost, VRI may be more available than in-person. Existing evidence tends to focus on assessing personal preferences of Deaf users regarding interpretation and interpreters’ preferences. Although respecting preferences is essential, there is a vacuum of knowledge on how the format of access to interpretation impacts the quality of communication between Deaf persons and health personnel. ObjectiveThis study aims to look at the effectiveness of the VRI system in improving communication outcomes between Deaf patients and doctors versus the available standard of care of the usual communication tools, including informal interpretation, lip- or note-reading, and using their mobile phones to contact a formal or informal interpreter, for Deaf patients aged 18 years and older in Bogota, Colombia. MethodsThis is a randomized controlled trial with a total sample size of 216 participants, divided into 2 groups: an intervention group, which receives a medical appointment using VRI, and a control group, which receives a medical appointment using standard communication. Both the Deaf participants and the health care professionals will be blinded to the allocation, as they will not know whether the appointment will involve VRI or standard communication until they arrive at the office. The primary outcome measure will be an assessment of communication using a Doctor-Patient Communication Scale. This scale was translated into Colombian Sign Language following a rigorous cultural adaptation and translation procedure. Furthermore, the database contains key clinical variables and recommendations provided by the doctor during a general medicine appointment. We will compute associations. ResultsRecruitment opened on August 24, 2023. As of July 2024, 180 participants had been enrolled. The intervention and data collection were finalized in October 2024. The findings of this study are expected to be submitted for publication in early 2025. ConclusionsThis study will provide rigorous evidence regarding information and communications technology intervention in health care, addressing empirical challenges in using inclusive research designs in public health. In addition, effective VRI models that address the challenges faced by Deaf people will be tested, implemented, and maintained in low- and middle-income countries. A disability-inclusive evaluative tool for quality communication mediated by VRI in health care is also tested. Ultimately, this will lead to evidence-based recommendations for implementing the Convention on the Rights of Persons with Disabilities (CRPD) in mobile health contexts. Se realizó un estudio de las 700 primeras oclusiones tubarias por laparoscopía realizadas en el Departamento de Obstetricia y Ginecología del Hospital Cayetano Heredia entre 1974 y 1983. 100 por electrocauterización y 600 por aplicación de bandas de silastic. El mayor porcentaje de pacientes que solicitaron este método anticonceptivo estuvieron entre los 30 y 39 años (75.9%) y con cinco o más gestaciones (78.6%). La complicación más frecuente fué enfisema subcutáneo (7.0°/o) y con menor incidencia laceración de trompas (1.6%) y perforación uterina (1.4%). El procedimiento se realizó en forma ambulatoria y en el 96.0% se utilizó neuroleptoanalgesia más anestesia local. Hubo cuatro gestaciones, con una incidencia de 0.57% y una confiabilidad de 0.15 por 100 años mujer.

Addiction is commonly characterized by escalation of drug intake, compulsive drug seeking, and continued use despite harmful consequences. However, the factors contributing to the transition from moderate drug use to these problematic patterns remain unclear, particularly regarding the role of sex. Many preclinical studies have been limited by small sample sizes, low genetic diversity, and restricted drug access, making it challenging to model significant levels of intoxication or dependence and translate findings to humans. To address these limitations, we characterized addiction-like behaviors in a large sample of >500 outbred heterogeneous stock (HS) rats using an extended cocaine self-administration paradigm (6 hr/daily). We analyzed individual differences in escalation of intake, progressive ratio (PR) responding, continued use despite adverse consequences (contingent foot shocks), and irritability-like behavior during withdrawal. Principal component analysis showed that escalation of intake, progressive ratio responding, and continued use despite adverse consequences loaded onto a single factor that was distinct from irritability-like behaviors. Categorizing rats into resilient, mild, moderate, and severe addiction-like phenotypes showed that females exhibited higher addiction-like behaviors, with a lower proportion of resilient individuals compared to males. These findings suggest that, in genetically diverse rats with extended drug access, escalation of intake, continued use despite adverse consequences, and PR responding are highly correlated measures of a shared underlying construct. Furthermore, our results highlight sex differences in resilience to addiction-like behaviors.

Objetivos: evaluar la evolución de la resistencia a los antibióticos de los microorganismos aislados de pie diabético infectado en pacientes atendidos en el Hospital Clínico Viedma, en las gestiones 2014, 2016, 2018 y 2021. Métodos: investigación observacional, descriptiva, retrospectiva, transversal con enfoque cuantitativo, 268 cultivos y antibiogramas de pacientes con diagnóstico de infección de pie diabético fue la muestra analizada con un 2,82 % de error máximo admitido. Resultados: se identificaron 312 microorganismos, con 71,5 % las bacterias Gram negativas fueron las más frecuentes, de estas la Klebsiella spp con 17,9 % y Escherichia coli con 17,6 % predominaron, cuya resistencia a la ciprofloxacina, sulfatrimetoprim y gentamicina aumentó, los bacilos Gram negativos no fermentadores más aislados fueron las Pseudomonas spp y el Acinetobacter spp cuya resistencia a las cefalosporinas, ciprofloxacina y carbapenémicos incrementó. Por otro lado, con 16,3 % el Staphylococcus aureus fue la bacteria Gram positiva más identificada, la resistencia a la meticilina de 17 % en 2014 aumentó a 25 % en 2021. Conclusiones: las enterobacterias fueron los microorganismos aislados con más frecuencia, cuya resistencia se asoció a la producción de betalactamasas de espectro extendido (BLEE).

Objetivo: comparar la actividad antibacteriana in vitro del extracto, aceite esencial de hojas de muña (Minthostachys mollis), ciprofloxacina, doxiciclina y amoxicilina frente a dos géneros y cinco cepas bacterianas patógenas. Materiales y métodos: estudio experimental, enfoque cuantitativo, actividad antibacteriana determinada por el método modificado de pozos en agar de Kirby-Bauer. Resultados: el rendimiento del aceite esencial fue 1,19%. Las medias de los halos de inhibición del aceite esencial al 100% frente al S. aureus fue 27 mm mayor al extracto y la doxiciclina; sumamente sensible ≥ 20 mm contra el género Proteus spp, y cepas de K. pneumoniae, E coli, P. aeruginosa; además, fue más efectivo con 17 mm frente al género Enterobacter spp, y la cepa E. faecalis con relación a los ≤ 12,8 mm del extracto y los controles positivos. Abstract High-density lipoprotein cholesterol (HDL-c) removes cholesterol, an essential component in lipid rafts, and this cholesterol removal can regulate protein attachment to lipid rafts, modulating their functionality in the immune cell response. Although severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection can alter the lipid profile, there is little information on the role of HDL-c and other lipids in prognostic of the coronavirus disease 2019 (COVID-19) in Mexican population. This study aims to evaluate the predictive value of HDL-c and lipid profile on severity and survival of 102 patients infected with SARS-CoV-2 during the COVID-19 first wave. Our findings, derived from univariate and multivariate Cox proportional hazards regression models, highlighted age and hypertension as significant predictors of survival (HR = 1.04, p = 0.012; HR = 2.78, p = 0.027), while gender, diabetes, and obesity showed no significant impact. Triglycerides and HDL-c levels notably influenced mortality, with elevated triglycerides and lower HDL-c associated with higher mortality risk (p = 0.032). This study underscores the importance of lipid profiles alongside traditional risk factors in assessing COVID-19 risk and outcomes. It contributes to the understanding of COVID-19 patient management and emphasizes the need for further investigation into the role of dyslipidemia in influencing COVID-19 prognosis, potentially aiding in refined risk stratification and therapeutic strategies.

Abstract Over the last quarter century, increasing honey bee colony losses motivated standardized large-scale surveys of managed honey bees (Apis mellifera), particularly in Europe and the United States. Here we present the first large-scale standardized survey of colony losses of managed honey bees and stingless bees across Latin America. Overall, 1736 beekeepers and 165 meliponiculturists participated in the 2-year survey (2016–2017 and 2017–2018). On average, 30.4% of honey bee colonies and 39.6% of stingless bee colonies were lost per year across the region. Summer losses were higher than winter losses in stingless bees (30.9% and 22.2%, respectively) but not in honey bees (18.8% and 20.6%, respectively). Colony loss increased with operation size during the summer in both honey bees and stingless bees and decreased with operation size during the winter in stingless bees. Furthermore, losses differed significantly between countries and across years for both beekeepers and meliponiculturists. Overall, winter losses of honey bee colonies in Latin America (20.6%) position this region between Europe (12.5%) and the United States (40.4%). These results highlight the magnitude of bee colony losses occurring in the region and suggest difficulties in maintaining overall colony health and economic survival for beekeepers and meliponiculturists.

Abstract The aim of this work is to quantitatively assess the wavefront phase of keratoconic eyes measured by the ocular aberrometer t·eyede (based on WaveFront Phase Imaging Sensor), characterized by a lateral resolution of 8.6 µm without requiring any optical element to sample the wavefront information. We evaluated the parameters: root mean square error, Peak-to-Valley, and amplitude of the predominant frequency (Fourier Transform analysis) of a section of the High-Pass filter map in keratoconic and healthy cohorts. Introducción. Durante el desarrollo de la pandemia por SARS-CoV-2 en Antioquia se presentaron picos epidemiológicos relacionados con las variantes α, ɣ, β, ƛ y δ, donde δ tuvo la mayor incidencia y prevalencia. Este linaje se considera una variante de preocupación dadas las manifestaciones clínicas que desencadena y sus características epidemiológicas. Se han informado 253 sublinajes δ en la base de datos PANGOLIN. La identificación de estos sublinajes mediante análisis genómico ha permitido rastrear su evolución y propagación. Objetivo. Caracterizar la diversidad genética de los diferentes sublinajes δ de SARSCoV-2 en Antioquia y determinar su prevalencia. Materiales y métodos. Se recopiló información sociodemográfica de 2.675 muestras y de 1.115 genomas del repositorio GISAID entre el 12 de julio de 2021 y el 18 de enero de 2022. Se seleccionaron 501 por su alto porcentaje de cobertura (>90 %) para realizar análisis filogenéticos e inferencia de frecuencias alélicas de mutaciones de interés. Resultados. Se caracterizaron 24 sublinajes donde el más prevalente fue AY.25. En este sublinaje se identificaron mutaciones de interés como L452R, P681R y P681H, que comprendían una frecuencia cercana a 0,99. Conclusiones. Este estudio permitió identificar que el sublinaje AY.25 tiene una ventaja de transmisión en comparación con los otros sublinajes δ. Esto puede estar relacionado con la presencia de las mutaciones L452R y P681R que en otros estudios se han visto asociadas con una mayor transmisibilidad, evasión del sistema inmunitario y menor eficacia de los medicamentos contra SARS-CoV-2.

Abstract In 2020/2021, several European brown hare syndrome virus (EBHSV) outbreaks were recorded in European hares (Lepus europaeus) from Catalonia, Spain. Recombination analysis combined with phylogenetic reconstruction and estimation of genetic distances of the complete coding sequences revealed that 5 strains were recombinants. The recombination breakpoint is located within the non-structural protein 2C-like RNA helicase (nucleotide position ~ 1889). For the genomic fragment upstream of the breakpoint, a non-pathogenic EBHSV-related strain (hare calicivirus, HaCV; GII.2) was the most closely related sequence; for the rest of the genome, the most similar strains were the European brown hare syndrome virus (EBHSV) strains recovered from the same 2020/2021 outbreaks, suggesting a recent origin. While the functional impact of the atypical recombination breakpoint remains undetermined, the novel recombinant strain was detected in different European brown hare populations from Catalonia, located 20–100 km apart, and seems to have caused a fatal disease both in juvenile and adult animals, confirming its viability and ability to spread and establish infection. This is the first report of a recombination event involving HaCV and EBHSV and, despite the recombination with a non-pathogenic strain, it appears to be associated with mortality in European brown hares, which warrants close monitoring.

Background The COVID-19 pandemic affected global access to health services, including contraception We sought to explore effects of the pandemic on family planning (FP) service provision and use in South Africa and Zambia, including on implant and intrauterine device (IUD) users’ desire and ability to obtain removal. Methods Between August 2020 and April 2021, we conducted surveys with 537 women participating in an ongoing longitudinal contraceptive continuation study. We also carried out in-depth interviews with 39 of the survey participants and 36 key informants involved in FP provision. We conducted descriptive analysis of survey responses and thematic analysis of interviews. Results Contraceptive use changed minimally in this sample with the emergence of COVID-19. Fewer than half of women (n=220) reported that they tried to access FP since the start of the pandemic. The vast majority of those seeking services were using short-acting methods and 95% were able to obtain their preferred method. The proportion of women not using a method before and after the start of the pandemic did not change in Zambia (31%), and increased from 8% to 10% in South Africa. Less than 7% of implant or IUD users in either country reported wanting removal. Among the 22 who sought removal, 10 in Zambia and 6 in South Africa successfully obtained removal. In qualitative interviews, those reporting challenges to service access specifically mentioned long queues, deprioritization of contraceptive services, lack of transportation, stock-outs, and fear of contracting COVID-19 at a facility. Key informants reported stock-outs, especially of injectables, and staff shortages as barriers. Conclusions We did not find a substantial impact of COVID-19 on contraceptive access and use among this sample; however, providers and others involved in service provision identified risks to continuity of care. As the COVID-19 pandemic wanes, it continues to be important to monitor people’s ability to access their preferred contraceptive methods.

Objectives In the midst of the pandemic, face-to-face data collection for national censuses and surveys was suspended due to limitations on mobility and social distancing, limiting the collection of already scarce disability data. Responses to these constraints were met with a surge of high-frequency phone surveys (HFPSs) that aimed to provide timely data for understanding the socioeconomic impacts of and responses to the pandemic. This paper provides an assessment of HFPS datasets and their inclusion of disability questions to evaluate the visibility of persons with disabilities during the COVID-19 pandemic.Design We collected HFPS questionnaires conducted globally from the onset of the pandemic emergency in March 2020 until December 2022 from various online survey repositories. Each HFPS questionnaire was searched using a set of keywords for inclusion of different types of disability questions. Results were recorded in an Excel review log, which was manually reviewed by two researchers.Methods The review of HFPS datasets involved two stages: (1) a main review of 294 HFPS dataset-waves and (2) a semiautomated review of the same dataset-waves using a search engine-powered questionnaire review tool developed by our team. The results from the main review were compared with those of a sensitivity analysis using and testing the tool as an alternative to manual search.Results Roughly half of HFPS datasets reviewed and 60% of the countries included in this study had some type of question on disability. While disability questions were not widely absent from HFPS datasets, only 3% of HFPS datasets included functional difficulty questions that meet international standards. The search engine-powered questionnaire review tool proved to be able to streamline the search process for future research on inclusive data.Conclusions The dearth of functional difficulty questions and the Washington-Group Short Set in particular in HFPS has contributed to the relative invisibility of persons with disabilities during the pandemic emergency, the lingering effects of which could impede policy-making, monitoring and advocacy on behalf of persons with disabilities.

The volcano rabbit (Romerolagus diazi) is a lagomorph endemic to the central mountains of the Trans-Mexican Volcanic Belt and is classified as threatened at extinction risk. It is a habitat specialist in bunchgrass communities. The annual wildfires that occur throughout its distribution range are a vulnerability factor for the species. However, the effects of wildfires on volcano rabbit populations are not fully understood. We evaluated the occupancy and change in the volcano rabbit relative abundance index in the burned bunchgrass communities of the Ajusco-Chichinautzin Mountain Range during an annual cycle of wildfire events. Additionally, we assessed the factors that favor and limit occupation and reoccupation by the volcano rabbit using the relative abundance index in burned plots as an indicator of these processes. The explanatory factors for the response of the volcano rabbit were its presence in the nearby unburned bunchgrasses, the height of three species of bunchgrass communities, the proportion of different types of vegetation cover within a 500 m radius around the burned plots, heterogeneity of the vegetation cover, and the extent of the wildfire. Statistical analyses indicated possible reoccupation in less than a year in burned bunchgrass communities adjacent to unburned bunchgrass communities with volcano rabbits. The relative abundance index of volcano rabbits was not favored when the maximum height of the Muhlenbergia macroura bunchgrass community was less than 0.77 m. When the vegetation around the burned plots was dominated by forest (cover >30% of the buffer) and the fire was extensive, the number of latrines decreased per month but increased when the bunchgrass and shrub cover was greater around the burned plots. While the statistical results are not conclusive, our findings indicate a direction for future projects, considering extensive monitoring to obtain a greater number of samples that contribute to consolidating the models presented.

El Dr. Escomel cursó sus estudios superiores en la Universidad de San Agustín y se graduó en el año 1902, de médico, en la Escuela de Medicina de San Fernando, su tema de tesis fue "El Verrucoma de la Enfermedad de Carrión". Realizó estudios en Francia, y de regreso al país realizó una fecunda labor científica, principalmente en su tierra natal, Arequipa, no solamente como médico sino como observador minucioso de la flora, fauna y ambiente natural, así como de los agentes productores de diversas enfermedades prevalentes en el país, observaciones que han dado lugar a múltiples publicaciones en libros, revistas naciones y extranjeras , reflejando la rigurosidad científica y la importancia para la salud y el avance en el conocimiento del país y de Arequipa en especial.

Se reconoce como hemorragia digestiva baja (HDB) a todo sangrado proveniente de cualquier localización distal al ángulo de Treitz. 10 a15%de todos los sangrados digestivos pertenecen a este tipo de hemorragia; sin embargo las cifras son mayores en el adulto mayor (20-30%). El espectro clínico es amplio debido a que involucra diferentes etiologías y varía desde una trivial hematoquezia hasta una hemorragia exanguinante. LaHDBaguda puede provocar importantes cambios hemodinámicos, por eso es fundamental definir la magnitud de la pérdida, categorizar el riesgo del paciente e iniciar las medidas de reposición de la volemia. Se revisa el algoritmo diagnóstico y terapéutico, se revisa el rol de la colonoscopia, la arteriografía y el uso de isótopos radiactivos; también se consideran la enteroscopia, la cápsula endoscópica y las indicaciones quirúrgicas.

Abstract Pathogens affect wild bird populations worldwide, contributing to their decline. Considering the scarce health data regarding the endangered Pyrenean Capercaillie (Tetrao urogallus aquitanicus), we molecularly surveyed selected pathogens (Newcastle disease virus, Avian influenza virus, Chlamydia psittaci, avian pathogenic Escherichia coli, Campylobacter jejuni, and Salmonella spp.) in 30 Pyrenean Capercaillie feces collected in the field (Catalonia, northeastern Spain). Additionally, swab and tissue samples from eight wild Pyrenean Capercaillies of Catalonia and Andorra were molecularly tested for herpesvirus and hemosporidians (Plasmodium spp., Haemoproteus spp., and Leucocytozoon spp.). All fecal samples were negative for the pathogens tested. Nevertheless, we detected a novel herpesvirus in 50% (4/8) of the Pyrenean Capercaillies, and hemosporidian DNA in 62.5% (5/8) of the tissue samples (i.e., Haemoproteus sp. [4 of 8] and/or Leucocytozoon sp. [3 of 8]). To our knowledge, this is the first detection of herpesvirus and hemosporidians infections in Pyrenean Capercaillies. The putative novel herpesvirus belongs to the genus Iltovirus. The presence of hemosporidian parasites in this mountain bird species is of concern, and could be related to the marked increase in the average temperature in the Pyrenees as a consequence of climate change. Our findings are fundamental to improve the conservation plans for the endangered Pyrenean Capercaillie population.

Abstract Orai1 is the pore-forming subunit of the Ca2+-release activated Ca2+ channels that mediate store-operated Ca2+ entry (SOCE) in excitable and non-excitable cells. Two Orai1 forms have been identified in mammalian cells, the full-length variant Orai1α, and the short form Orai1β, lacking the N-terminal 63 amino acids. Stem cells were isolated from non-tumoral breast epithelial cells of the MCF10A cell line, and the most representative ER+ , HER2 or triple negative breast cancer cell lines MCF7, SKBR3 and MDA-MB-231, respectively. Orai and TRPC family members expression was detected by RT-PCR and Western blotting. Changes in cytosolic Ca2+ concentration were analyzed by confocal microscopy using Fluo 4 and the spheroid-forming ability and self-renewal was estimated in culture plates coated with pHEMA using a cell imaging system. Here, we have characterized the expression of Orai family members and several TRPC channels at the transcript level in breast stem cells (BSC) derived from the non-tumoral breast epithelial cell line MCF10A and breast cancer stem cells (BCSC) derived from the well-known estrogen receptor positive (ER+), HER2 and triple negative cell lines MCF7, SKBR3 and MDA-MB-231, respectively. Furthermore, we have evaluated the mammosphere formation efficiency and self-renewal of the BSC and BCSC. Next, through a combination of Orai1 knockdown by iRNA and the use of MDA-MB-231 KO cells, missing the native Orai1, transfected with plasmids encoding for either Orai1α or Orai1β, we show that Orai1 is essential for mammosphere formation and self-renewal efficiency in BCSC derived from triple negative and HER2 subtypes cell cultures, while this channel has a negligible effect in BCSC derived from ER+ cells as well as in non-tumoral BSC. Both, Orai1α, and Orai1β support SOCE in MDA-MB-231-derived BCSC with similar efficiency, as well as COX activation and mammosphere formation. These findings provide evidence of the functional role of Orai1α and Orai1β in spheroid forming efficiency and self-renewal in breast cancer stem cells.

Liver cancer is the fourth leading cause of cancer-related death worldwide. In Mexico, there is a high burden of liver cancer mortality in rural states, affecting both women and men equally. Thus, we aimed to describe the demographic and clinical characteristics of hepatocellular cancer (HCC) by sex in Mexico. Demographic and clinical information was extracted retrospectively from the medical records of patients with HCC initially treated (2015–2022) at institutions participating in a national survey across the country. The male-to-female ratio was calculated at the national and regional levels, and the results were stratified by sex. Among 697 HCC patients, the age at diagnosis was 65.4 ± 11.9 years and 20% were diagnosed at ≥75 years. The male-to-female ratio was 1.4:1, ranging from 1:1 in the northwestern and southwestern regions, to 2.1:1 in the western region. The proportion of cirrhosis was similar between the sexes; however, the etiology of cirrhosis differed: cryptogenic cirrhosis was higher in women and alcohol consumption was higher in men. Men had a higher proportion of advanced HCC, poor/undifferentiated tumors, and ≥4 nodules than women. HCC in the Mexican population affects both men and women at a 1.4:1 male-to-female ratio. This unique proportion by sex could be explained by the differences in the prevalence of risk factors across our heterogeneous country.

Abstract Peruvian anchovy (Engraulis ringens) represents the largest single-species fishery worldwide. Knowledge on how temperature and prey availability influences growth and age estimation during marine fish early life stages is critical for predicting bottom-up processes impacting stock productivity under changing environmental conditions. We reared Peruvian anchovy larvae at two temperatures (14.5 and 18.5 °C) and prey concentrations [high (HF), and low (LF)] from 6 to 30 days post-hatch (dph) to measure growth rate and examine daily deposition of otolith increments. Peruvian anchovy larvae grew faster at 18.5 °C compared to 14.5 °C. Larvae reared at low prey concentration (18.5-LF) and low temperature (14.5-HF) grew 61 and 35% slower, respectively, than those at high prey and warm temperature (18.5-HF). Age and growth rates of larvae were well depicted in the otolith microstructure of well-fed larvae at 18.5 °C. However, larvae reared at 18.5-LF or 14.5-HF, had only 55 and 49% of the expected number of daily otolith increments. Our results suggest caution when attempting to explore how ocean processes regulate small pelagic stocks, the productivity of which are largely driven by changes in the survival and growth of young larvae.

La presente investigación de mapeo sistemático tuvo como objetivo principal identificar las publicaciones científicas que hay sobre violencia y dependencia emocional en los últimos dos años. La metodología utilizada consistió en una búsqueda de documentos en las siguientes bases de datos: Web of Science, Scopus a través de la búsqueda “risk* of violence* AND emotion* dependen*”, y para Scielo y La Referencia “Violencia Dependencia emocional”. Donde se encontraron 19 estudios de investigación, los cuales conforman artículos, tesis de maestría y doctorados. Se halló que la mayoría de investigaciones se han realizado en Latinoamérica, especialmente en Perú en los cuales se ha empleado mayormente el Cuestionario de Dependencia Emocional (CDE)de Lemos y Londoño, el Inventario de Dependencia Emocional (IDE), con relación a la variable de violencia se utilizan indistintos tipos de instrumentos. Se pudo detectar que en una gran parte de las exploraciones hay una conexión cercana entre violencia y dependencia emocional, produciendo problemas en la salud integral de la persona. Cabe recalcar que ninguna de las investigaciones pertenece al cuartil nivel uno, lo que llevaría a la conclusión que sería beneficioso llevar a cabo investigaciones innovadoras sobre esta temática a fin de optimizar futuros estudios en relaciones de pareja.

Two hybrid flow shop scheduling lines must be coordinated to assemble batches of terminated products at their last stage. Each product is thus composed of two jobs, each produced in one of the lines. The set of jobs is to be processed in a series of stages to minimize the makespan of the scheduling, but jobs forming a product must arrive at the assembly line simultaneously. We propose a mixed integer linear programming model. Then, based on the model, we propose a pull-matheuristic algorithm. Finally, we present two metaheuristics, a greedy randomized adaptive search procedure and a biased random key genetic algorithm, and compare all the methodologies with real-based instances of a production scheduling problem in the automobile manufacturing industry. The greedy algorithm yields high-quality solutions, while the genetic one offers the best computational times.

Introducción: La colelitiasis es un problema grave en la medicina moderna y son los procedimientos más comunes realizados en cirugía general. Actualmente la mayoría de las colecistectomías se realizan por vía laparoscópica. Objetivo: describir las ventajas, diferencias y conversión entre la colecistectomía abierta y laparoscópica. Método: Este estudio se desarrolló a través de un análisis documental aplicándose el sistema PRISMA. Se realizó una búsqueda sistemática en las bases de datos de PubMed y Google Académico de artículos publicados entre 2019 y 2023. Los términos de búsqueda fueron: colecistectomía o laparoscópica y resultado del tratamiento o complicaciones o complicaciones intraoperatorias o complicación posoperatoria. Desarrollo: La colecistectomía laparoscópica (CL) da como resultado una tasa general de complicaciones más baja y una estancia hospitalaria postoperatoria más corta en comparación con la colecistectomía abierta. Los casos de conversión, de laparoscópica a abierta, se asocian a complicaciones postoperatorias infecciosas, a un mayor riesgo de procedimientos adicionales y una mayor tasa de reingreso dentro de los 30 días. Además, mayores tasas de morbilidad y mortalidad. Conclusiones: La colecistectomía laparoscópica es una de las mejores opciones para el tratamiento de la enfermedad biliar y la colecistitis. Esta ofrece una ventaja incuestionable sobre la colecistectomía abierta, tanto para el paciente como para el sistema de salud.

Background Population distribution of reduced diffusing capacity of the lungs for carbon monoxide (DLCO) in smokers and main consequences are not properly recognised. The objectives of this study were to describe the prevalence of reduced DLCO in a population-based sample of current and former smoker subjects without airflow limitation and to describe its morphological, functional and clinical implications.Methods A sample of 405 subjects aged 40 years or older with postbronchodilator forced expiratory volume in 1 s/forced vital capacity (FVC) >0.70 was obtained from a random population-based sample of 9092 subjects evaluated in the EPISCAN II study. Baseline evaluation included clinical questionnaires, exhaled carbon monoxide (CO) measurement, spirometry, DLCO determination, 6 min walk test, routine blood analysis and low-dose CT scan with evaluation of lung density and airway wall thickness.Results In never, former and current smokers, prevalence of reduced DLCO was 6.7%, 14.4% and 26.7%, respectively. Current and former smokers with reduced DLCO without airflow limitation were younger than the subjects with normal DLCO, and they had greater levels of dyspnoea and exhaled CO, greater pulmonary artery diameter and lower spirometric parameters, 6 min walk distance, daily physical activity and plasma albumin levels (all p

Solute carrier (SLC) transporters constitute a vast superfamily of transmembrane proteins tasked with regulating the transport of various substances such as metabolites, nutrients, ions, and drugs across cellular membranes. SLC transporters exhibit coordinated expression patterns across normal tissues, suggesting a tightly regulated regulatory network governing normal cellular functions. These transporters are crucial for the transport of various metabolites, including carbohydrates, proteins, lipids, and nucleic acids. However, during tumor development, metabolic changes drive an increased demand for energy and nutrients. Consequently, tumor cells alter the expression of SLC transporters to meet their heightened nutrient requirements. Targeting SLCs through inhibition or activation presents a promising therapeutic approach in cancer treatment. Certain SLCs also serve as intriguing chemo-sensitizing targets, as modulating their activity can potentially alter the response to chemotherapy. This review underscores the significance of various SLCs in tumor progression and underscores their potential as both direct and indirect targets for cancer therapy.

Abstract Applying contemporary trapping standards when managing wildlife should no longer be an option, but a duty. Increasing wild boar populations originate a growing number of conflicts and hunting is the only cost-effective management option in most cases. However, new scenarios where hunting is unfeasible emerge and trapping necessities cope with lacking regulatory frameworks and technical guidelines. In this research, we evaluated drop nets, a capture method not considered by the international trapping standards, to capture Eurasian wild boar (Sus scrofa), a wildlife species not included in the list of mammal species under the scope of the Agreement on International Humane Trapping Standards (AIHTS). Less than 20% of the captured wild boars presented moderate or severe injuries attributable to the capture method, hence fulfilling the acceptance thresholds of the outdated AIHTS. Based on the new standards thresholds of acceptance, the humaneness of drop-nets in our study ranged 66–78%, under the 85% required. The capture success and selectivity were 100%, as ensured by operator-driven triggering, which should be considered the main strengths of this method, together with the minimization of animal suffering owing the short duration of the stressful situation. Additionally, in spite of the socially adverse environment, with people contrary to wild boar removal, no disturbances against the capture system or operations occurred. This is the first assessment of a drop-net capture method according to internationally accepted mammal trapping standards, with unconclusive results. However, there is a need for adapted procedures and thresholds of acceptance aimed at not-mechanical traps in general, and specifically at drop-nets. Compared to other live-capture methods, drop-nets minimize the duration of the stressful situation —at the expense of a strong adrenergic acute response—, maximize the probabilities of capturing entire sounders of prosocial species, which may be also considered as more humane, and has the ability to coordinate higher values of capture success, absolute selectivity and adaptability to difficult environments.

Objetivos. Objetivos. Caracterizar el estado del hierro y proponer factores de ajuste de hemoglobina por altitud, en niños de 6 a 8 meses de Lima, Arequipa, Cusco y Puno. Materiales y métodos. Estudio transversal en niños de 6 a 8 meses de edad en cuatro ciudades, se midió hemoglobina y otros biomarcadores de hierro, Proteína C reactiva (PCR), entre otros. Para estimar la ecuación de ajuste, se aplicó una regresión exponencial y excluimos a los niños con deficiencia de hierro (DH) y/o inflamación. Resultados. Las proporciones de DH fueron mayores en Puno y Arequipa, mientras que la inflamación no superó el 19% en ninguna de las ciudades. La hemoglobina mostró un incremento exponencial a mayor altitud. La ecuación de ajuste fue: 10,34249 x (1,00007 ^ Alt). Conclusiones. Los niños residentes en Arequipa y Puno mostraron mayores proporciones de DH y menores reservas de hierro; además el aumento de la hemoglobina por altitud fue exponencial evidenciando la necesidad de ajustar la hemoglobina en altura.

When forming a memory of an experience that is unfolding over time, we can use our schematic knowledge about the world (constructed based on many prior episodes) to predict what will transpire. We developed a novel paradigm to study how the development of a complex schema influences predictive processes during perception and impacts sequential memory. Participants learned to play a novel board game (‘four-in-a-row’) across six training sessions and repeatedly performed a memory test in which they watched and recalled sequences of moves from the game. We found that participants gradually became better at remembering sequences from the game as their schema developed, driven by improved accuracy for schema-consistent moves. Eye tracking revealed that increased predictive eye movements during encoding, which were most prevalent in expert players, were associated with better memory. Our results identify prediction as a mechanism by which schematic knowledge can improve episodic memory.

Diabetes mellitus is the main cause of progression of chronic kidney disease and dialysis. The therapeutic intervention considers an HbA1C level that avoids adverse effects associated with hypoglycemic therapy, as occurs in patients with chronic kidney disease, despite not reaching normoglycemia. The present case describes an intervention based on Lifestyle Medicine in a patient with diabetic nephropathy and chronic kidney disease, whose biomarkers showed improvement in kidney damage due to reduction of proteinuria or reduction of HbA1C, LDL and triglycerides in its metabolic control. The intervention with lifestyle medicine requires a multidisciplinary approach and the time of intervention does not discriminate a degree of chronic kidney disease that limits its intervention

Objective. To determine the titer of antibodies against the receptor binding domain (RBD) of the spike protein (S) in health personnel between the 4th and 12th week after receiving the BBIBP-CorV vaccine (Sinopharm). Materials and methods. We included a total of 168 healthcare workers from two hospitals in the region, who complied with the complete Sinopharm vaccine schedule; serum antibodies were measured using the Elecsys® Anti-SARS-CoV-2 test. Results. All participants developed antibodies to the RBD domain. The lowest antibody titer level was 1.78 U/mL. Levels equal to or above 250 were found in 70 (41.7%) participants. The geometric mean was 82.6 (95% CI: 67.8-100.6). Women had higher antibody levels. Participants whose antibodies were measured between 4- and 7-weeks post-vaccination showed significantly higher antibody levels than patients whose antibody levels were measured between 10- and 12-weeks post-vaccination. Among patients with a history of COVID-19, antibody levels were found to be at or above 250 U/mL in 88% of cases, compared to 6% among those without a history of COVID-19, (p0.001). Conclusion. All participants immunized with BBIBPCorV vaccine were positive for antibodies against the SARS-CoV-2 spike protein RBD. The correlation between the titer level and protection against COVID-19, as well as the length of the protection provided by the vaccine, needs to be evaluated.

Climate-related phenomena in Peru have been slowly but continuously changing in recent years beyond historical variability. These include sea surface temperature increases, irregular precipitation patterns and reduction of glacier-covered areas. In addition, climate scenarios show amplification in rainfall variability related to the warmer conditions associated with El Niño events. Extreme weather can affect human health, increase shocks and stresses to the health systems, and cause large economic losses. In this article, we study the characteristics of El Niño events in Peru, its health and economic impacts and we discuss government preparedness for this kind of event, identify gaps in response, and provide evidence to inform adequate planning for future events and mitigating impacts on highly vulnerable regions and populations. This is the first case study to review the impact of a Coastal El Niño event on Peru's economy, public health, and governance. The 2017 event was the third strongest El Niño event according to literature, in terms of precipitation and river flooding and caused important economic losses and health impacts. At a national level, these findings expose a need for careful consideration of the potential limitations of policies linked to disaster prevention and preparedness when dealing with El Niño events. El Niño-related policies should be based on local-level risk analysis and efficient preparedness measures in the face of emergencies.

The release of vast quantities of sulfide from the sediment into the water column, known as a sulfidic event, has detrimental consequences on fish catches, including downstream effects on other linked element cycles. Despite being frequent occurrences in marine upwelling regions, our understanding of the factors that moderate sulfidic event formation and termination are still rudimentary. Here, we examined the biogeochemical and hydrodynamic conditions that underpinned the formation/termination of one of the largest sulfur plumes to be reported in the Peruvian upwelling zone. Consistent with previous research, we find that the sulfur-rich plume arose during the austral summer when anoxic conditions (i.e., oxygen and nitrate depletion) prevailed in waters overlying the upper shelf. Furthermore, the shelf sediments were organically charged and characterized by low iron-bound sulfur concentrations, further enabling the diffusion of benthic-generated sulfide into the water column. While these biogeochemical conditions provided a predicate to sulfidic event formation, we highlight that attenuations in local wind intensity served as an event trigger. Namely, interruptions in local wind speed constrained upwelling intensity, causing increased stratification over the upper shelf. Moreover, disturbances in local wind patterns likely placed additional constraints on wind-driven mesoscale eddy propagation, with feedback effects on coastal elemental sulfur plume (ESP) formation. We suggest ESP development occurs as a result of a complex interaction of biogeochemistry with regional hydrodynamics.

Gene mutations causing loss of dystrophin result in the severe muscle disease known as Duchenne muscular dystrophy (DMD). Despite efforts at genetic repair, DMD therapy remains largely palliative. Loss of dystrophin destabilizes the sarcolemmal membrane, inducing mechanosensitive cation channels to increase calcium entry and promote cell damage and, eventually, muscle dysfunction. One putative channel is transient receptor potential canonical 6 (TRPC6); we have shown that TRPC6 contributed to abnormal force and calcium stress-responses in cardiomyocytes from mice lacking dystrophin that were haplodeficient for utrophin (mdx/utrn+/– [HET] mice). Here, we show in both the HET mouse and the far more severe homozygous mdx/utrn–/– mouse that TRPC6 gene deletion or its selective pharmacologic inhibition (by BI 749327) prolonged survival 2- to 3-fold, improving skeletal and cardiac muscle and bone defects. Gene pathways reduced by BI 749327 treatment most prominently regulated fat metabolism and TGF-β1 signaling. These results support the testing of TRPC6 inhibitors in human trials for other diseases as a novel DMD therapy.

Antecedentes: La detección y resección de lesiones colónicas elevadas (pólipos) sésiles o planas, con polipectomía clásica o compleja detiene la secuencia adenoma-cáncer. La mucosectomía endoscópica (EMR) fue introducida en los setentas y perfeccionada en los ochentas como un procedimiento avanzado para el tratamiento de pólipos grandes o complejos. Una adecuada realización de la técnica puede evitar procedimientos quirúrgicos mayores. Objetivos: Evaluar los resultados y complicaciones de la técnica de mucosectomía (EMR) realizada por gastroenterólogos-endoscopistas en un centro de referencia del Perú. Revisión de indicaciones, éxito, complicaciones y seguimiento. Material y método: Se realizó un análisis descriptivo, retrospectivo y observacional de pacientes tratados con técnica de mucosectomía endoscópica en un centro de endoscopia de referencia nacional en Lima, Perú, desde enero de 2004 a diciembre de 2018. Se aplicó la técnica de elevación y corte controlado en lesiones polipoideas mayores a 1 cm. Se realizó la resección en bloque en lesiones hasta 3 cms y técnica de “piecemeal” o sacabocado en mayores de 3 cms. Se evaluaron resultados, eventos adversos y recurrencia. Resultados: Se analizaron 756 lesiones en el mismo número de pacientes. Hombres 46.8 % (298) y mujeres 53.2 % (338). La edad promedio fue de 61.9 (rangos 37-91). El tamaño promedio de las lesiones fue de 20,3 mm (10 – 50 mm). El tiempo promedio por procedimiento fue de 46 minutos (rango 20-123 minutos). Se logró resección en bloque en 78.04 % de pólipos (590 lesiones). Se realizó técnica sacabocado en 166 (21.96 %) lesiones. La tasa de complicaciones en nuestra serie fue del 6.74 %, todos manejados endoscópicamente más tratamiento conservador médico sin cirugía. El seguimiento promedio fue de 18 meses (3 – 24 meses) y la tasa global de recidiva local fue de 2.49 %. El tratamiento quirúrgico post procedimiento y con pieza analizada se indicó en 15 casos por adenomas avanzados con adenocarcinoma intramucoso bien diferenciado (ADCA-IM). A los 12 meses, 13 de 15 recidivas fueron tratadas endoscópicamente y 2 casos refractarios fueron operados. Conclusiones: La mucosectomía (RME ó EMR) es un procedimiento que, realizado por endoscopistas-gastroenterólogos bien entrenados muestra baja tasa de recurrencia y complicaciones aisladas permitiendo la obtención de adecuado material para el estudio anátomo-patológico y reduciendo necesidad de cirugía abierta o laparoscópica.

Objetivo: Describir los resultados de la biopsia prostática transperineal por fusión de la resonancia magnética cognitiva y ultrasonido en la detección del cáncer. Materiales y métodos: Estudio de serie de casos, retrospectivo, realizado en Clínica Delgado entre julio del 2019 y octubre del 2021. Se incluyó pacientes con examen digital de próstata y/o Antígeno Prostático Específico anormal e imágenes de Resonancia Magnética de próstata con lesiones categoría PI-RADS 4 o 5. Excluyendo a pacientes con trastorno de coagulación, lesiones rectales, imposibilidad para abducción, comorbilidades para anestesia. Registramos las características clínicas, demográficas, datos de antígeno prostático específico, volumen de la próstata, lesiones PI-RADS, tasas de cáncer, complicaciones. Resultados: Se evaluaron 18 pacientes, con edad media de 69,33 ± 7,67 años. La media del antígeno prostático específico de 14,94 ± 12,42 ng/mL. La media del volumen de próstata de 57,72 ± 28,68 cc. Al examen digital de la próstata 16/18 (88.88 %) pacientes tenían sospecha de cáncer. Biopsias en pacientes con lesiones PI-RADS 4 en 6/18 (33,33%) y 5 en 12/18 (66,67%). Biopsia con 19 núcleos a 6/18 (33,33%) y 16 núcleos a 12/18 (66,67%) pacientes. Pacientes con cáncer 15/18 (83,33%), todos Adenocarcinoma Acinar, con Gleason 6 en 2/18 (11,11%) y Gleason ≥ 7 en 13/18 (72,22%) pacientes. Complicaciones leves como hematuria 1/18 (5,6%), disuria terminal 8/18 (44,4%) y molestia perineal 7/18 (38,8%) y ninguna infecciosa. Conclusiones: la biopsia prostática transperineal por fusión de imágenes de resonancia magnética cognitiva y ultrasonido es factible, segura, con tasas altas de positividad y sin infecciones.

ABSTRACT Objective: Diabetes mellitus (DM) has a high healthcare system cost worldwide. Educational strategies are important to improve self-care and control this disease. This study aimed to evaluate satisfaction and clinical efficacy of a Short Message Service (SMS) educational intervention in self-care and nutrition at a Brazilian university hospital. Materials and methods: We conducted a trial of educational intervention and assigned eligible patients with DM to either receive weekly educational SMS for 6 months (intervention group [IG]) or no SMS at all (control group). A satisfaction questionnaire was applied before and after the intervention in both groups. Laboratory (fasting glucose, hemoglobin [Hb] A1c, total cholesterol, triglycerides, high-density lipoprotein, and low-density lipoprotein) and clinical (blood pressure) data were also collected. Data were analyzed using nonparametric tests with the Statistical Package for the Social Sciences. Results: We included 128 patients (64 in each group). Responses to the satisfaction questionnaire with self-care and healthcare professionals from 112 patients revealed an improvement in the perception of receiving information regarding helpful eating habits and in healthy eating behavior and an improvement in satisfaction with their diabetes care in the IG. In the post-intervention period, improved systolic blood pressure and HbA1c levels were observed in the IG as illustrated by delta % (post-intervention minus pre-intervention data divided by pre-intervention data multiplied by 100) reductions of 2.3% and 3.9%, respectively Conclusion: SMS intervention was useful as an educational tool for improving satisfaction and glycemic and blood pressure control of patients with DM observed at a Brazilian university hospital.

Introduction: The SARS-CoV-2 coronavirus is a new pathogen causing COVID-19 disease, ranging from an asymptomatic infection to a critical condition dependent on mechanical ventilation. Rehabilitation interventions contribute for the prevention and treatment of related complications and associated disability. Objective: Evaluate the rehabilitation interventions for the care of patients with COVID-19 according to the available evidence. Methods: A systematic review was conducted through electronic and library searches of national and foreign medical journals over a 10-year horizon in Spanish, English, Chinese and Italian, as well as in textbooks with information related to COVID-19 and Physical Medicine and Rehabilitation. Conclusions: Rehabilitation interventions are essential and indispensable resources for the transdisciplinary treatment of patients with COVID-19, despite of the low quality of the available evidence.

Abstract Epstein–Barr virus (EBV), human herpesvirus 6A/B (HHV-6A/B) and multiple sclerosis (MS)-associated retrovirus (MSRV) have been described as possible MS triggers. We analysed antibody titres against EBV and HHV-6, and MSRV envelope (env) mRNA expression, in the serum of pregnant multiple sclerosis patients (P-MS) to study their possible link to the clinical activity of MS during pregnancy and postpartum and their possible role as relapse predictors. For that purpose, serum samples were collected from 71 pregnant women (50 pregnant MS and 21 pregnant healthy controls—P-HC) during pregnancy and postpartum. Relating to antibody titres, IgM antibody titres against HHV-6A/B were significantly higher in P-MS than in P-HC both in each pregnancy trimester and in the postpartum period. Moreover, IgM antibody titres against HHV-6A/B were higher in P-MS who suffered a relapse during the postpartum. Regarding MSRV env mRNA expression, the prevalence in the first trimester of pregnancy was significantly higher in P-MS who suffered relapses during pregnancy. Summing it up, high IgM antibody titres against HHV-6A/B and MSRV env mRNA expression during the first trimester of pregnancy could act as relapse predictors for the gestation/postpartum periods.

Listeriosis infection is a severe disease, with high morbidity and mortality in the immunocompromised patient, especially with disseminated and fatal presentations in cancer patients. A descriptive study was developed to describe the clinical and epidemiologic characteristics in oncologic patients with listeriosis in the Instituto Nacional de Enfermedades Neoplásicas between the years 2005-2015. A total of 29 patients were included; 23 (79.3%) of the listeriosis cases showed up in patients with hematological neoplasia, of which 52.1% was acute lymphatic leukemia and 39.1% non-Hodgkin’s lymphoma. The 72.4% of the isolated species correspond to Listeria monocytogenes. Twenty-seven (93.1%) patients met sepsis criteria and twenty-four (82.7%) had neurologic affection. Bacteremia was the most common presentation, followed by eningoencephalitis (20.6%). Global mortality was 75.8%. In conclusion in cancer patients, listeriosis implies high morbidity and mortality. Therefore, the suspicion of this entity is mandatory in onco-hematologic patients with sepsis and acute neurologic symptoms.

Introduction: Cervical cancer is the third most common malignant tumor in the female population and the fourth cause of death from cancer in women in Brazil. The squamocolumnar junction and the transformation zone concentrate 90% of pre-invasive and invasive cervical lesions. Objective: To evaluate the prevalence of cytology without cells of the squamocolumnar junction and feasibility of active search. Methods: Cross-sectional study at a university hospital between 2017 and 2018. The prevalence of cytology without squamocolumnar junction cells was calculated. A convenience sample was obtained and mean age and relationship with presence of transformation zone cells were calculated. An active search was performed and cytology collected, with estrogen preparation if indicated. Medical records of the other women were analyzed. Results: Squamocolumnar junction cells were not found in 28.84% of samples. Mean age was 53 years, without association with presence of squamocolumnar junction cells (p=0.409). Seventy-six women returned, 36 of which (47.37%) used estrogen. Level 2 or 3 cervical intraepithelial neoplasia, microinvasive carcinoma or cancer was not identified. Objectives: To evaluate the predictive capacity of gestational weight gain recommendations regarding low birth weight (LBW) and neonatal macrosomia, proposed by the Institute of Medicine (IOM) and the Latin American Center of Perinatology (CLAP). Materials and methods: The bibliographic search was performed in PubMed, Embase (via Ovid), Cochrane Library, EBSCOhost, Scopus, LILACS and SciELO. Methodological quality was evaluated using QUADAS 2. Results: A total of 1,192 articles were identified, only 5 articles met the inclusion criteria, no study evluated the CLAP recomendations. Sensitivity and specificity to predict LBW and macrosomia varied widely depending on which country the study took place. In the Latin American cohorts, the sensitivity for predicting LBW ranged from 62.8% to 74% and the specificity from 61.7% to 68%, while the sensitivity for predicting macrosomia was 28.8% and the specificity 43.8%. In most studies the positive predictive value was less than 25%, and the negative predictive value was more than 90%. Most studies had high risk of bias and applicability problems in patient selection. Conclusions: The limited methodological quality and representativeness of the studied cohorts, probable unadjusted confounding factors and modest values of sensitivity and specificity suggest the need to develop studies aimed at providing recommendations that fit the epidemiological characteristics of the Peruvian population.

ABSTRACT Objective The consequences of sleep deprivation in type 1 diabetes (T1D) patients are poorly understood. Our aim was to determine how sleep disorders influence lipid profile and insulin sensitivity in T1D patients. Materials and methods This was a cross-sectional study at a public university hospital. Demographic information and medical histories were obtained during regular scheduled visit of T1D patients to the outpatient clinic. Insulin sensitivity was obtained using the estimated glucose disposal rate (eGDR) formula. Sleep quality was assessed using the Pittsburgh Sleep Quality Index, Epworth Sleepiness Scale and Berlin Questionnaire. Results The adult participants (n = 66, 62% women) had a median age of 28.0 years (interquartile range 21.8-33.0). Six patients (9%) had metabolic syndrome according to the International Diabetes Federation criteria. Thirty patients (46%) were considered poor sleepers according to the Pittsburgh Sleep Quality Index. The LDL-c and total cholesterol levels of poor sleepers were higher than those of good sleepers (103 v. 81; p = 0.003 and 178.0 v. 159.5 mg/dL; p = 0.009, respectively). Three patients (4%) were at high risk of obstructive sleep apnea syndrome (OSAS) according to the Berlin Questionnaire. The eGDR was lower in the group of patients with high probability of having OSAS (6.0 v. 9.1 mg.kg-1.min-1;p = .03). Conclusions Poor subjective quality of sleep and higher risk of OSAS were correlated with a worsened lipid profile and decreased insulin sensitivity, respectively. Therefore, T1D patients with sleep disturbances might have an increased cardiovascular risk in the future.

La sepsis es la respuesta desordenada del organismo a la infección y se caracteriza por un daño a los órganos que puede ser irreversible y mortal. El microbioma intestinal regula a un grupo de mecanismos homeostáticos en el huésped, como la función inmunológica y la protección de la barrera intestinal, la pérdida de la estructura y la función microbiana intestinal normal; además, se ha asociado con el inicio de enfermedades de características diversas. La evidencia reciente ha demostrado un nexo entre el microbioma intestinal y la sepsis: la alteración del microbioma intestinal aumenta la susceptibilidad a la sepsis a través de varios mecanismos como la expansión de bacterias intestinales patógenas, la respuesta proinflamatoria marcada y la disminución de la formación de productos microbianos beneficiosos como los ácidos grasos de cadena corta. Una vez establecida la sepsis, la alteración del microbioma intestinal empeora y aumenta la susceptibilidad a la disfunción del órgano terminal. Existen pruebas limitadas de que las terapias basadas en microbiomas (que incluyen a probióticos y a la descontaminación digestiva selectiva) pueden disminuir el riesgo de sepsis y mejorar sus resultados en poblaciones de pacientes seleccionadas, pero las preocupaciones sobre la seguridad causan una aceptación limitada. Si bien gran parte de la evidencia que vincula el microbioma intestinal y la sepsis se ha establecido en estudios preclínicos, aún es necesaria la evidencia clínica en distintas áreas.

Introducción: la eritrocitosis excesiva afecta a los habitantes de grandes alturas como La Paz, Bolivia, a 3600 msnm. El objetivo del presente trabajo es determinar la existencia de hipertensión arterial pulmonar en pacientes fumadores con eritrocitosis excesiva, residentes de la gran altura entre 20 y 60 años de edad, casos estudiados en el Instituto Boliviano de Biología de Altura (IBBA) a lo largo de tres décadas. Materiales y métodos: estudio retrospectivo en el que revisamos sistemáticamente expedientes clínicos del IBBA. Se hallan 540 expedientes clínicos de la unidad de fisiología y fisiopatología respiratoria, de los cuales el 7% (30) fueron incluidos. Se buscaron correlaciones lineales entre edad, tabaquismo, hemoglobina sérica, altitud de residencia, presión arterial pulmonar y pruebas de función pulmonar. Resultados: existe correlación entre presión arterial pulmonar sistólica y hemoglobina sérica con R=0,579 (p=0,005). El índice de Tiffeneau (TIFF.O) tiene una correlación con la presión arterial media sistémica (PAM) (p=0,006). Conclusión: en pacientes con eritrocitosis excesiva fumadores residentes de grandes alturas, existe una correlación positiva entre presión arterial pulmonar sistólica y concentración de hemoglobina sin existir correlación con valores espirométricos que sugieran patrones obstructivos.
