Alpha1-antitrypsin (A1AT) is an anti-inflammatory mediator with antiprotease activity associated with anti-inflammatory and immunomodulatory effects in various inflammatory conditions. A1AT deficiency (A1ATD) has been associated with various hyperinflammatory diseases, such as lung disease (emphysema and bronchiectasis), liver disease (chronic hepatitis, cirrhosis, and hepatoma), and skin diseases (panniculitis). Migraine with aura is one of the common migraine subtypes associated with neuroimmunologic activation and neuroinflammation which is associated with cortical spreading depression and glial hyperinflammation in etioradiopathogenesis, and the main mechanisms explained so far are hyperinflammation of pro-inflammatory mediators, sensitivity of trigeminal nerve fibers and pain-conjugated glial cells activation. In this case report, a causative perspective of migraine with aura and A1ATD was presented through etioradiopathogenetics mechanisms that show the central reflections of systemic hyperinflammatory processes, and the importance of peripheral hyperinflammatory conditions in migraine etiology was examined.

Amaç: Sağlıklı yaşam biçimi, kişinin sağlığını etkileyebilecek davranışlarını kontrol edebilmesi, günlük aktivitelerini düzenlerken kendi sağlık durumuna uygun davranışları seçmesi olarak tanımlanmaktadır. Bu çalışmanın amacı, hemşirelik ve tıp eğitimi gören üniversite öğrencilerinin sağlığı geliştirme davranışlarının değerlendirilmesi ve bu davranışları etkileyen sosyodemografik özelliklerin saptanmasıdır. Materyal ve Metot: Bu araştırma kesitsel tipte bir araştırma olup, araştırmanın evrenini Yıldırım Beyazıt Üniversitesi Tıp Fakültesinde ve Sağlık Bilimleri Fakültesi Hemşirelik Bölümünde okuyan toplamda 1367 öğrenci oluşturdu. Örneklem seçimi için tabakalı örnekleme yöntemi kullanıldı. Veriler, sosyodemografik özellikler için oluşturulan bir anket formu ve Sağlıklı Yaşam Biçimi Davranışları-II (SYBD-II) ölçeği aracılığıyla Mart -Mayıs 2016 tarihleri arasında toplandı. Bulgular: Çalışmaya 283 tıp öğrencisi, 251 hemşirelik öğrencisi olmak üzere toplamda 534 kişi dahil edildi. Yaş ortancası 21,50 yıl (min-max 19-24) olarak saptandı. Çalışmaya katılan bireylerin %26,78’ i (n=143) erkek, %73,22’ si (n=391) kadındı. SYBD-II ölçeği puan ortalaması 130,00 ± 17,62 olarak saptandı. En yüksek puanın tinsellik (manevi gelişim), en düşük puanın fiziksel aktivite alt ölçeğinden alındığı saptandı. Hemşirelik öğrencilerinin genel ölçek, sağlık sorumluluğu, beslenme, kişilerarası ilişki puanlarının tıp öğrencilerinden anlamlı olarak daha yüksek olduğu saptandı(p

Purpose: We aimed to evaluate frequency of electroconvulsive therapy (ECT) use among hospitalized patients in a psychiatry clinic of university hospital and to determine the diagnostic, socioodemographic features of these ECT treated patients retrospectively.Material and Method: Of 759 inpatients, 32 who was treated with ECT in psychiatry clinic of Ministery of Health – Ordu University Training and Research Hospital were included in the study. The data evaluated in the study were obtained from patient files retrospectively. Results: Among the 32 (4.21%) patients treated with ECT, 23 (71.9%) were male with a mean age of 44.4 and 9 (28.1%) were female with a mean age of 44.5. The mean hospitization time was 28,7 days in male patients whereas it was 38.33 days in female patients. The mean number of ECT sessions was 9.4 for males and 9.8 for females. Conclusion: ECT treatment was performed in 4.24 % of the patients in our clinic for the past 5.5 years. Although the application is restricted because of false beliefs and prejudiced opinions, ECT is a reliable treatment method with very low risk of serious complications. New methods should be implemented for healthy acknowledgment to overcome these opinions and beliefs about ECT.

Objective: The incidence of Type 1 Diabetes Mellitus (DM), which is one of the most common chronic diseases of childhood, is increasing all over the world and brings a series of physical, emotional, and social problems. In this study, we aimed to observe the effects of active expressionist art therapy techniques on depression, anxiety, quality of life and diabetes management in adolescents with Type 1 DM. Materials and Methods: This study included 21 volunteer adolescents with Type 1 DM between the ages of 13-18. All adolescents were evaluated by the Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI) and Quality of Life Scale for Children (PedsQL). Then, the program of art therapy techniques was carried out for 12 weeks for 90 minutes a day for every week. Pre-test scales were repeated after the application. HbA1c levels were compared before and after the art therapy techniques program without classification. Results: In this study, 13 girls (61.9%), 8 boys (38.1%) were included. Prostaglandin E2 (PGE2) is a key paracrine mediator of ovulation. Few specific PGE2-regulated gene products have been identified, so we hypothesized that PGE2 may regulate the expression and/or activity of a network of proteins to promote ovulation. To test this concept, Ingenuity Pathway Analysis (IPA) was used to predict PGE2-regulated functionalities in the primate ovulatory follicle. Cynomolgus macaques underwent ovarian stimulation. Follicular granulosa cells were obtained before (0 h) or 36 h after an ovulatory dose of human chorionic gonadotropin (hCG), with ovulation anticipated 37–40 h after hCG. Granulosa cells were obtained from additional monkeys 36 h after treatment with hCG and the PTGS2 inhibitor celecoxib, which significantly reduced hCG-stimulated follicular prostaglandin synthesis. Granulosa cell RNA expression was determined by microarray and analyzed using IPA. No granulosa cell mRNAs were identified as being significantly up-regulated or down-regulated by hCG + celecoxib compared with hCG only. However, IPA predicted that prostaglandin depletion significantly regulated several functional pathways. Cell cycle/cell proliferation was selected for further study because decreased granulosa cell proliferation is known to be necessary for ovulation and formation of a fully-functional corpus luteum. Prospective in vivo and in vitro experiments confirmed the prediction that hCG-stimulated cessation of granulosa cell proliferation is mediated via PGE2. Our studies indicate that PGE2 provides critical regulation of granulosa cell proliferation through mechanisms that do not involve significant regulation of mRNA levels of key cell cycle regulators. Pathway analysis correctly predicted that PGE2 serves as a paracrine mediator of this important transition in ovarian structure and function.

Objective: Although a strong inflammatory basis has been demonstrated, the pathophysiology of attention deficit hyperactivity disorder (ADHD) has not been defined clearly. The aim of the present study was to investigate whether soluble urokinase plasminogen activator receptor (suPAR), one of the inflammatory disruptors, plays a role in the etiology of ADHD. Materials and Methods: The study population comprised 50 patients aged 7–13 years, diagnosed with ADHD according to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, without any chronic or other psychiatric disease, and 25 healthy controls. Parents of the children in the study group completed the Conners' Parent Rating Scale—Revised Short, and teachers completed the Conners' Teacher Rating Scale—Revised Short. Enzyme-linked immunosorbent assay kits were used to measure suPAR levels in plasma samples. Results: The mean plasma suPAR level of patients with ADHD was 2.92+-1.74 ng/ml, the suPAR level of the controls was 2.54+-1.05 ng/ml, and there was no significant difference in suPAR levels between ADHD and controls (Z=0.084, p=0.933). No correlation was found between plasma suPAR levels and ADHD severity as assessed by Conners' parent and teacher scales. Conclusion: The role of inflammatory systems and mediators in ADHD was emphasized in many studies, and many important data on ADHD etiopathology were obtained. However, we found no significant relationship between ADHD and suPAR levels. Further research is needed with large samples.

Cerrahi hastalarında, normoterminin sürdürülmesi; hasta güvenliği, hasta memnuniyeti, olumlu cerrahi sonuçların elde edilmesi ve kaliteli bakımın sürdürülmesi için önemlidir. Ameliyat sırası ve sonrası dönemde normoterminin sürdürülmesi; taburculuk süresini %40’a kadar kısaltmakta ve cerrahi alan enfeksiyonu riskini %64’e kadar azaltmaktadır. Ameliyat sırasında ısı yalıtım mekanizmasının bozulması ve anestezi ısı kaybını arttırarak normoterminin sürdürülmesini engellemektedir. Hastanın ısı kaybını önlemek ve normotermiyi sürdürmek için pasif izolasyon, aktif eksternal ısıtma sistemleri ve internal ısıtma yöntemleri kullanılmaktadır. Aktif ısıtma yöntemleri ile hastaya dışarıdan iletim, ışınım, taşınım yollarından biri veya birden fazlasıyla ısı verilmektedir. Pasif ısıtma yöntemleri; hastanın vücut ısısının korunmasına yönelik yöntemlerdir. Pasif ısıtma; cerrahi örtüler, boneler, çoraplar, çarşaflar, pikeler, battaniyeler, metalize plastik örtüler gibi hastayı dış ortam sıcaklığından koruyacak malzemeler kullanılarak sağlanmaktadır. Pasif ısıtmada örtülen vücut alanı ile korunan ve/veya kazanılan ısı miktarı doğru orantılı olduğundan; çalışmalar battaniyeler üzerinde yoğunlaşmıştır. Hastaların vücut ısısını korumada kullanılan battaniyelerin üretiminde değişik hammaddelerin kullanılabilmesine rağmen daha önce yapılan çalışmalarda pamuklu battaniye kullanıldığı dikkat çekmektedir. Bir kat ısıtılmış pamuklu battaniyenin ısı kaybını %33, İki kat pamuklu battaniyenin ısı kaybını ek %18 azalttığı, daha fazla katın fayda sağlamadığı bildirilmiştir. Pasif ısıtma yöntemlerinden pamuklu battaniyelerin vücut ısısını koruyabildikleri fakat arttıramadıkları belirtilmekte, bu nedenle aktif ısıtma yöntemleri ile birlikte kullanılmaları önerilmektedir. Bu çalışmada postoperatif hipoterminin kontrolü ile ilgili literatür özetlenmiştir

Giriş: Sağlık bakım kurumlarında hemşirenin rol ve sorumlulukları teknolojinin etkisi ile değişmekte ve bireye hizmet daha kaliteli hale gelmektedir. Bilgisayarın; hemşirelerin günlük ve mesleki yaşamına hızla girmesine karşın, sağlık bakımında bilgisayar kullanımına hemşireler direnç göstermektedirler. Amaç: Hemşirelerin bilgisayar kullanım durumlarını değerlendirmektir. Yöntem: Araştırma Mart- Nisan 2015 tarihleri arasında Kastamonu Dr. Münif İslamoğlu Devlet Hastanesi, Kastamonu Dr. Münif İslamoğlu Ek Binası, Kastamonu Fizik Tedavi ve Rehabilitasyon Merkezi'nde yapılmıştır. Araştırmanın evrenini, bu hastanelerde çalışan 410 hemşire örneklemini ise 120 hemşire oluşmuştur. Veriler anket formu ile toplanmıştır. Verilerin değerlendirilmesinde tanımlayıcı istatistikler (sayı ve yüzde) kullanılmıştır. Etik boyut: Çalışmanın yürütülebilmesi için kurumlardan yazılı, araştırmaya katılan bireylerden sözel onam alınmıştır. Bulgular: Araştırmamıza katılan hemşireler 31-40 yaşında(%56.7), evli(%82.5) kadınlardır(%83.3). %49.2'si kliniklerde görev yapan(%90.0), 11-15 yıldır çalışan(%25.0) lisans mezunlarıdır. %96.7'sinin bilgisayarı vardır, %49.2'si orta düzeyde bilgisayar kullanmaktadır. %92.5'i bilgisayar kullanma konusunda kendini yeterli hissetmektedir. %39.2’si günde 1-3 saat, %40.8'i 10 yıldan uzun süredir bilgisayar kullanmaktadır. Hemşireler hemşirelikte bilgisayar kullanımı ile ilgili olumlu ifadelere genellikle “katılıyorum” (%27.0-%58.3) demişlerdir. Olumsuz ifadelerden “bilgisayar kullanımı hasta mahremiyetini ihmal eder” için %49.2 oranında; “sağlık bakım sisteminde bilgisayar kullanımı hemşirelerin iş yükünü arttırır” için % 44.2 oranında katılmıyorum ifadesini kullanmışlardır. Sonuç: Hemşirelerin bilgisayar kullanım oranları yüksek bulunmuştur. Ayrıca hemşireler bilgisayar kullanımı ile ilgili olumlu ifadelere genellikle “katılmıyorum” yanıtını verirken, bilgisayar kullanımı ile ilgili olumsuz ifadelere katılım durumları değişkenlik göstermektedir. Öneriler: Hemşirelerin bilgisayar kullanımı ile ilgili olumsuz ifadelere katılım durumlarının azaltılmasıyla ilgili eğitim verilmesi ve farkındalık oluşturulması önerilebilir.

Amaç: Araştırma; cerrahi kliniklerinde yatan yaşlı hastaların ilaç kullanımlarını etkileyebilecek bilgi ve uygulamalarının belirlenmesi amacıyla tanımlayıcı olarak yapılmıştır. Örneklemi Hacettepe Üniversitesi Erişkin Hastanesi’nin Beyin Cerrahi, Genel Cerrahi, Toraks/Kalp/Damar Cerrahi ve Üroloji kliniklerinde yatan 60 yaş ve üzeri 128 hasta oluşturmuştur. Yöntem: Veri toplama aracı olarak, araştırmacı tarafından ilgili literatüre doğrultusunda geliştirilen soru formu kullanılmıştır. Veriler araştırmacı tarafından, yüz yüze görüşme yöntemi ile toplanmıştır. Verilerin değerlendirilmesinde yüzde hesaplamaları, McNemar ve CohranQ testleri kullanılmıştır. Bulgular: Araştırmaya katılan hastaların bir yılda ortalama 1.8±1.7 kere hekime/sağlık kuruluşuna başvurduğu ve kendilerine ortalama 16.4±1.8 reçete yazıldığı; %22.7’sinin reçetesiz ilaç, %25’inin bitkisel ürün kullandığı, %16.4’ünün ilaçlarını düzenli kullanmadığı belirlenmiştir. İlaçlarını düzenli kullanmadığını belirten hastaların (%16.4) yaklaşık üçte biri (%38.7) ilaçlarını düzenli kullanmama sebebi olarak “ilaç kullanımı nedeniyle yaşanan sorunlar” ı göstermiştir. Sonuç: Bu çalışmadan elde edilen bulgular doğrultusunda; ilaçların akılcı ilaç kullanımı (AİK) ilkelerine uygun kullanılmalarını engelleyebilecek özelliklerin, yaşlı hastaların yaklaşık dörtte birinde görüldüğü saptanmıştır.

Kanser dünyada temel bir sağlık sorunudur ve kronik bir hastalıktır. Kanser hastaları ve aileleri; sosyal izolasyon, rol değişiklikleri ve para sorunları ile sonuçlanan, acil yardım gerektiren bir kriz yaşarlar. Psikososyal bakımın tıbbi bakım ile birlikte gerekli olmasının nedeni de budur. 1970'li yıllardan bu yana sağlık profesyonellerinin bakımın psikososyal yönlerine önem vermesi gerektiği belirtilmektedir. Hastalar ve aileleri ile çok daha fazla zaman geçiren bir mesleğin üyeleri olarak hemşireler; psikososyal bakımın, hastaların ve ailelerinin yaşam kalitesini artıran bir faktör olduğunu hesaba katmalıdırlar. Bu çalışmanın amacı, T hücreli ALL klinik tanısı ile izlenen bir hastanın yakını ile baş etme konusunda, psikososyal destek birimi hemşirelerinin kosültasyon-liyezon hizmetlerinin klinik hemşirelere etkisini sunmaktır.

Although; data from clinical risks in the literature is limited. In this study, we aimed to determine the clinical risk factors of sexual abuse. Methods: 22 female and 12 male, sexual abused children and 34 healthy volunteer children were involved in this study. Patients and volunteers were assessed with the Parental Bonding Instrument (PBI), Reading Mind from Eyes Test (RMET) and the UPPS impulsive behavior scale. Results: There were no significant differences in maternal attachment scores in either group but the paternal attachment scores were statistically significantly lower in the study group. The RMET scores were statistically significantly lower in the study group than in the control group. The UPPS impulsive behavior scale unpersistence subscores were statistically significantly higher in the study group than in the control group. Conclusion: Sexual abuse is known as the most difficult type of trauma to treat and preventing the risk of identification is of the most importance. Further studies with a large sample are needed to determine the clinical risk factors of sexual abuse.
