Four groups of sandwich scaffold configurations were investigated with alteration of (PMMA: PCL) composition.Results: The sandwich scaffold composed of 2%PCL/4%PMMA/1%6-MP showed the best miscibility, good homogeneity and produced the smoothest nanofibers and low crystallinity. All fabricated 6-MP-loaded-PCL/PMMA scaffolds exhibited antimicrobial properties on the bacterial and fungal organisms, where the cytotoxicity evaluation proved the safety of scaffolds on normal cells, even at high concentration. Scaffolds provided a sustained-drug release profile that was strongly dependent on (PCL: PMMA). As (PCL: PMMA) decreased, the sustained 6-MP release from PCL/PMMA scaffolds increased. Results established that ∼ 18% and 20% of 6-MP were released after 23h from (4%PCL/4%PMMA/1%6-MP) and (2%PCL/4%PMMA/1%6-MP), respectively, where this release was maintained for more than 20 days. The anti-cancer activity of all fabricated scaffolds was also investigated using different cancerous cell lines (e.g., Caco-2, MDA, and HepG-2) results showed that 6-MP-loaded-nanofibrous mats have an anti-cancer effect, with a high selective index for breast cancer. We observed that viability of a cancer cell was dropped to about 10%, using nanofibers containing 2%PCL/4%PMMA/1%6-MP.Conclusion: Overall, the PCL: PMMA ratio and sandwich configuration imparts a tight control on long-term release profile and initial burst of 6-MP for anticancer treatment purposes.Keywords: sandwich scaffold, PCL/PMMA, 6-MP, sustained drug release, anticancer, 6-Mercaptopurine, polycaprolactone/poly methyl methacrylate

Purpose: The aim of this study is to investigate the power of disease severity scores to predict the development of Severe Acute Pancreatitis (SAP) and mortality in the early period over 65 years old diagnosed with acute pancreatitis in the emergency department. Materials and Methods: We calculated RANSON (on admission) and Computed Tomography Severity Index (CTSI) in addition to Bedside Index for Severity in Acute Pancreatitis (BISAP) score on admission to the emergency department. Results: One hundred and sixty patients (46.9% over 80 years of age) were included in the study. We observed statistically higher length of hospitalization, longer duration of stay in the intensive care unit, SAP and higher mortality in patients over 80 years of age. When we examined the ROC curve, we determined that the AUC values of the BISAP score were highest in both SAP and mortality estimation (AUC: 0.911, 95% CI 0.861-0.962; AUC: 0.918, 95% CI 0.864-0.9722, respectively). Binary logistic analysis indicated a 4.7-fold increased risk for SAP and a 12.3-fold increased mortality for each unit increase in BISAP score value. Conclusion: BISAP may be a good predictor for SAP and mortality estimation on admission to the emergency department in patients over 65 years of age with acute pancreatitis.

Purpose: The aim of this retrospective observational study is to compare C-reactive protein to albumin ratio and CURB-65 score in the emergency department in terms of predicting mortality in patients over the age of 18 who were hospitalized for COVID-19 pneumonia. Materials and Methods: The study includes 613 patients hospitalized between March 15 and April 30, 2020 due to COVID-19 pneumonia detected on thorax computed tomography at the emergency department pandemic area. Hospitalized patients were divided into groups according to positive and negative real-time polymerase chain reaction results. Results: While 73.1% (n: 448) of 613 patients included in the study were hospitalized in the ward, 26.9% (n: 165) were hospitalized in intensive care. 8.6% (n: 53) of the total patients died. In non-survivors patients the mean CURB 65 score was 4±1 (and C-Reactive Protein to Albumin Ratio was 5.6±4.2 Multivariate logistic regression analysis showed that CURB 65 and high C-Reactive Protein to Albumin Ratio are independent risk factors for COVID-19 pneumonia. Conclusion: The C-reactive protein to albumin ratio is as sensitive as CURB 65 and can guide the clinician in the early detection of patients with poor prognosis COVID-19 pneumonia.

Amaç: Bu çalışmada; acil servise yılan ısırması ile başvuran hastalarda kullanılan iki tip polivalan yılan antivenomun etkinliğinin karşılaştırılması ve olası lokal ve sistemik yan etkilerin belirlenmesi amaçlanmıştır. Gereç ve Yöntem: Bu retrospektif çalışmaya yılan ısırması şikayeti olan 30 hasta dahil edildi. Hastalar aldıkları antivenom tipine (PoliseraTM veya PolivalanTM) göre gruplandırıldı. Hastaların demografik özelliklerinin yanı sıra, vital bulguları, lokal doku bulguları ve laboratuvar parametreleri standart veri formuna kaydedildi. Her grupta kaç vial yılan anti serumu kullanıldığı, kaç kez ek doz venom ihtiyacı olduğu; antivenoma karşı herhangi bir reaksiyon gelişip gelişmediği kaydedildi. Bulgular: Çalışmaya 30 hasta dahil edildi. 16 hastaya Polisera TM (Grup 1) yılan antivenomu, 14 hastaya PolivalanTM (Grup 2) yılan antivenomu verildi. Grup 1'deki hastalara ortalama 9.1±7.3 vial antivenom verilirken, Grup 2'deki hastalara ortalama 11.6±12.7 vial verildi. PolivalanTM antivenom alan grupta daha fazla alerjik reaksiyon-ürtiker, ateş ve selülitgözlendi. Sonuç: Pirojen kontaminasyonu, üretimden kaynaklanan farklı metodolojiler ve ambalaj farklılıkları aynı doz ve içeriğe sahip antivenom ürünlerinde bile farklı etkilere ve yan etkilere neden olabilir.

Introduction Young infants 7–59 days old with fast breathing pneumonia presented to a primary level health facility receive a 7-day course of amoxicillin as per the WHO guideline. However, community-level health workers (CLHW) are not allowed to treat these infants. This trial evaluated the community level treatment of non-hypoxaemic young infants with fast breathing pneumonia by CLHWs.Methods This cluster-randomised, open-label, non-inferiority trial was conducted in rural areas of Bangladesh, Ethiopia, India and Malawi. We randomly allocated clusters (first-level health facility) 1:1, stratified by the population size, to an intervention group (enhanced community case management) or control group (standard community case management). Infants aged 7–59 days with a respiratory rate of ≥60 breaths/min and oxygen saturation (SpO2) ≥90% were enrolled. In the intervention clusters, these infants were treated with a 7-day course of oral amoxicillin (according to WHO weight bands) and were regularly followed up by CLHWs. In the control clusters, CLHWs continued the standard management (assess and refer after pre-referral antibiotic dose) and followed up according to the national programme guideline. The primary outcome of treatment failure was assessed in both groups by independent outcome assessors on days 6 and 14 after enrolment. Secondary outcomes (accuracy and impact of pulse oximetry) were also assessed.Results Between September 2016 and December 2018, we enrolled 2334 infants (1168 in intervention and 1166 in control clusters) from 208 clusters (104 intervention and 104 control). Of 2334, 22 infants with fast breathing were excluded from analysis, leaving 2312 (1155 in intervention clusters and 1157 in control clusters) for intention-to-treat analysis. The proportion of treatment failure was 5.4% (63/1155) in intervention and 6.3% (73/1157) in the control clusters, including two deaths (0.2%) in each group. The adjusted risk difference for treatment failure between the two groups was −1.0% (95% CI −3.0% to 1.1%). The secondary outcome showed that CLHWs in the intervention clusters performed all recommended steps of pulse oximetry assessment in 94% (1050/1115) of enrolled patients.Conclusions The 7-day amoxicillin treatment for 7–59 days old non-hypoxaemic infants with fast breathing pneumonia by CLHWs was non-inferior to the currently recommended referral strategy.Trial registration numbers CTRI/2017/02/007761 and ACTRN12617000857303.

Amaç: Çalışmamızda acil servise başvuran hastalarda ne sıklıkla amilaz testi istendiğini ve bu testin etkinliğini göstermeyi amaçladık.Gereç ve Yöntem: Amilaz tetkiki istenilen 32.922 hastadan amilaz yüksekliği (>100 IU/L) saptanan 4318’nin dosya bilgileri incelendi. Geriye kalan 1543 hasta çalışmaya dahil edildi. Hastaların demografik verileri ile laboratuvar değerleri, hastaların nihai tanıları ve ilaç bilgileri değerlendirildi.Bulgular: Acil servise başvuran hastaların %15,1’inden amilaz tetkiki istenilmiş olup, %1,98’inde amilaz yüksekliği saptandı. Acil servisteki tanı alan akut pankreatit insidansı 100.000’de 60 olarak saptandı. Serum amilaz kestirim değeri normalin üst limitinin 3 kat ve üzeri esas alındığında amilazın akut pankreatit tanısı koymadaki sensitivitesi ve spesifitesi amilaz düzeyini yükselttiği bilinen hastalıklarla ayırımda %85,4 ve %98,0 idi. Amilazın akut pankreatit tanısı koymadaki sensitivitesi ve spesifitesi amilazı yükselttiği bilinen bir hastalık veya ilaç kullanımı olmayan hastalarla ayırımında %85,4 ve %95,3 olarak saptandı. Sonuç: Amilaz yüksekliği akut pankreatit ile beraber diğer gastrointestinal, jinekolojik, kardiyovasküler, nörolojik hastalıklar ve renal fonksiyon bozukluklarında da görülmektedir. Acil servislerde hastalardan kliniğine göre amilaz tetkiki istenilmesi gereksiz tetkik istenilmesini önleyecektir.

Amaç: Bu çalışmanın amacı, acil servise baş dönmesi olan hastaların demografik özelliklerini, şikayete neden olan etyolojik nedenleri, laboratuvar bulgularını ve prognozu değerlendirmektir.Gereç ve yöntem: Adana Numune Eğitim ve Araştırma Hastanesi Acil Tıp Kliniğine Ocak 2013-Ocak 2015 tarihleri ​​arasında başvuran toplam 5.056 hastanın dosya verileri retrospektif olarak incelendi. Verilerin tanımlayıcı istatistiklerinde ortalama, standart sapma, ortanca, en düşük, en yüksek, frekans ve oran değerleri kullanılmıştır.Bulgular: İki bin dokuz yüz dört hasta (% 57.4) kadın, 2.152'si erkek (% 42.6) ve yaş ortalaması 53.6 (± 18.2) idi. Şikâyetlerin 51-70 yaş arası periferik periferik vertigo (n: 4.130,% 81.6), sistemik nedenler (n: 664,% 13.1) ve santral vertigo (n: 249) arasında yaygın olduğu saptanmıştır. En sık görülen etiyolojiler ise% 4.9'du. Acil serviste gerekli prosedürleri takiben hastaların% 83'ünün taburcu olduğu tespit edildi.Sonuç: Baş dönmesi, hastaları acil servise getiren en önemli belirtilerden biridir. Pek çok etiyolojinin bir parçası olmasına rağmen, nörolojik ve kardiyolojik değerlendirmelerin yapılması gerektiği belirlenir. Hastalar genellikle basit manevra veya tıbbi tedavi seçenekleri ile acil servislerde yönetilebilir.

Improving health outcomes in countries with the greatest burden of under-5 child mortality requires implementing innovative approaches like integrated community case management (iCCM) to improve coverage and access for hard-to-reach populations. ICCM improves access for hard-to-reach populations by deploying community health workers to manage malaria, diarrhoea and pneumonia. Despite documented impact, challenges remain in programme implementation and sustainability. An analytical review was conducted using evidence from published and grey literature from 2010 to 2019. The goal was to understand the link between governance, policy development and programme sustainability for iCCM. A Governance Analytical Framework revealed thematic challenges and successes for iCCM adaptation to national health systems. Governance in iCCM included the collective problems, actors in coordination and policy-setting, contextual norms and programmatic interactions. Key challenges were country leadership, contextual evidence and information-sharing, dependence on external funding, and disease-specific stovepipes that impede funding and coordination. Countries that tailor and adapt programmes to suit their governance processes and meet their specific needs and capacities are better able to achieve sustainability and impact in iCCM.

Purpose: The aim of this study was to determine the admission complaints, demographic characteristics, etiology, diagnosis, and outcome of patients who admitted to emergency department with eye-related complaints.Material and Methods: All patients, over the age of 18, who admitted to emergency department with eye-related complaints between June 2011 and May 2012, were included prospectively in this study. Demographic characteristics, etiology, admission complaints, diagnosis, and outcome of the patients in the emergency department were evaluated with statistical analysis.Results: At the time of the study, a total of 201427 adult patients admitted to our emergency department and 3256 (1.6%) were included in the study. 77.5% of the patients (2524) were males. The most frequent diagnosis was trauma (30.3%) in female gender, whereas it was superficial foreign bodies (46.2%) in male gender. The most common cause of trauma was home accidents (57.4%) in females, whereas it was work accidents (73%) in males. Of patients admitted due to vision loss (0.46%), it was identified that these patients were older and visual loss is more dependent on non-traumatic ophthalmic emergencies. Conclusions: The majority of cases of ophthalmic emergencies are work accidents in young active workingmen patients group. The most common complaints of patients are trauma and superficial foreign bodies. Vision loss depends mostly on non-traumatic ophthalmic emergencies seen in the elderly.

Purpose: We evaluated whether abdominal tomography is necessary for patients who have been admitted to the emergency service because of non traumatic abdominal pain and effect of C-Reactive Protein (CRP) and tomography to the patient management. Materials and Method: On retrospective study, we recorded demographic data, tomography diagnoses, CRP and leucocyte values operation and following decisions of 199 patient who were 18 years old and over and were admitted to emergency service because of abdominal pain and were scanned abdominal tomography. 104 patient were hospitalized and their CRP values, tomography diagnoses, end of operation diagnoses were recorded from patient files. The statistical analysis was performed using the Chi-square test and the diagnostic value was assessed through the logistic regression test. Results: A statistically significant relationship was observed between anormal tomography findings and high CRP and leukocyte values of the 199 patients included in the study. The tomography findings and CRP values were found to be efficient in discharge and hospitalization rates. When tomography finding and CRP value were combined, the operation decision was effected. Conclusion: Based on our study; in the event of abdominal pain, CRP can predict abnormal tomography finding and it is more valuable than leucocyte. Amaç: Çalışmamızda, acil servise travma dışı karın ağrısı başvurularında abdominal tomografinin gerekliliğini değerlendirdik. Bu sayede, C-Reaktif Protein (CRP) ve tomografi bulgularının hastanın yönetimine etkisini ölçmeyi amaçladık. Materyal ve Metod: Geriye dönük yapılan çalışmada acil servise karın ağrısı ile başvurup abdominal tomografi çekilen 18 yaş ve üzeri 199 hastanın demografik özellikleri, tomografi bulguları, CRP ve lökosit değerleri, yatış veya taburculuk kararları ile hastaneye yatışı yapılan 104 hastanın CRP değerleri, tomografi tanıları, operasyon sonu tanıları hasta dosyalarından kaydedildi. İstatistiksel analizde ki-kare, tanısal değerliliğin ölçülmesinde ise lojistik regresyon testi kullanıldı. Bulgular: Çalışmaya alınan 199 hastanın anormal tomografi bulgusu ile CRP ve lökosit yüksekliği arasında anlamlı ilişki bulundu. Tomografi ve CRP’nin taburculuk ve yatış oranını etkilediği görüldü. Tomografi bulgusu ve CRP birlikte kullanıldığında, operasyon kararını etkilediği bulundu. Sonuç: Çalışmamıza göre; karın ağrısında CRP yüksekliği, anormal tomografi bulgusunu ön görebilir ve lökositten daha değerlidir. Tomografi ve CRP birlikte kullanıldığında operasyon kararı etkilenmektedir.

Purpose: Trauma is one of the most common reason of admissions to emergency departments. In this study, it was aimed to determine the demographic characteristics, etiology, morbidity and mortality rates and prognosis of adult trauma patients admitted to our emergency department (ED). Materials and Methods: Patients over the age of 18 years, who admitted to ED between 01 March 2011 and 31 August 2011 were included in this retrospective study. Patient examination cards, hospitalization files and records entered with ICD 10 codes to hospital automation system were analyzed. Patients with inaccessible data were excluded from the study. Results: During the study period, total number of 110495 patients admitted to ED, and 13585 (12,29%) of them admitted with trauma. Simple extremity injuries (38,28%) and falls (31,7%) were most common etiological factors. Glasgow coma scales of 99,71% of the patients were between 13 and 15. Of the patients with trauma, 9,6% had a Computed Tomography (CT) scan, and 84,5% of CT scans were evaluated as normal, and cranial CT was the most requested one. Only 6% of the patients were hospitalized, and 0,9% of the trauma patients died. Falls from height in females and traffic accidents in males were the leading causes of death. Conclusion: Most of the patients with simple traumas admitted to ED can be discharged from the hospital with a complete history and careful examination. The rate of unnecessary medical tests, loss of time and waste money should be reduced, and the time and labor allocated to severe patients can be increased by this way. Amaç: Travma acil servise en yaygın başvuru nedenlerinden birisidir. Bu çalışmada, acil servisimize başvuran erişkin travma hastalarının demografik özellikleri, etyolojileri, mortalite ve morbidite oranlarının belirlenmesi amaçlanmıştır. Materyal ve Metod: Acil servise 01 Mart 2011 ile 31 Ağustos 2011 tarihleri arasında travma ile başvuran 18 yaş üzeri hastalar çalışmaya dahil edilmiştir. Hasta muayene kartları, yatış dosyaları ve hastane otomasyon sistemine ICD 10 kodları ile girilen kayıtlar incelenmiştir. Bilgilerine eksiksiz ulaşılamayan hastalar çalışma dışı bırakılmıştır. Bulgular: Çalışma süresi boyunca 110495 hastanın acil servise başvurduğu ve bunların 13585’inin (%12,29) travmaya bağlı başvuru olduğu belirlenmiştir. Basit ekstremite travması (%38,28) ve düşmenin (%31,7) en yaygın etyolojik nedenler olduğu, hastaların %99,71’inin Glaskow Koma Skalası skorunun 13 ile 15 arasında olduğu tespit edilmiştir. Travmalı hastaların %9,6’sına Bilgisayarlı Tomografi (BT) çekildiği, bunların %84,5’inin normal olduğu ve en sık Beyin BT çekildiği belirlenmiştir. Hastaların sadece %6’sının hastaneye yatırıldığı, %0,9’unun ise eksitus olduğu görülmüştür. Bayanlarda yüksekten düşmenin, erkeklerde ise trafik kazalarının en sık ölüm nedeni olduğu saptanmıştır. Sonuç: Acil servise basit travma ile başvuran hastaların büyük çoğunluğunun ayrıntılı hikaye ve dikkatli fizik muayene sayesinde taburcu edilebileceği belirlenmiştir. Gereksiz tıbbi tetkikler, zaman kaybı ve para israfı azaltılmalıdır. Bu yolla ağır hastalara daha fazla zaman ve işgücü ayrılabilir.

Meyan kökü glycyrrhizic acid içeren Glycyrrhiza glabra"nın köküdür. Aşırı meyan kökü alımı sodyum ve su retansiyonu, hipokalemi, hipertansiyon, metabolik alkaloz, düşük renin aktivitesi ve hipoaldosteronizm ile karekterize hipermineralokortikoidizm benzeri sendroma yol açabilir. Bu yazıda aşırı meyan kökü şerbeti içimi sonrası acil servise solunum sıkıntısı, kol ve bacaklarında güçsüzlük nedeniyle getirilen hipopotasemik paralizili 34 yaşındaki erkek hasta sunuldu. Acil Servise paralizi ve solunum sıkıntısıyla gelen hastalarda aşırı meyan kökü alımı olup olmadığının sorgulanması gerekliliğine dikkat çekilmesi amaçlandı. Kan potasyum düzeyi 1.4 mmol/L olan hastanın potasyum replasmanı sonrası solunum sıkıntısı ve kas kuvvet kaybı tamamen düzeldi.

Licorice is the root of Glycyrrhiza glabra, which has a herbal ingredient, glycyrrhizic acid. Excessive intake of licorice may cause a hypermineralocorticoidism-like syndrome characterized by sodium and water retention, hypokalemia, hypertension, metabolic alkalosis, low-renin activity, and hypoaldosteronism. In this paper, an 34 years old man who admitted to the emergency department with respiratory failure and marked muscle weakness of all extremities that progressed to paralysis after excessive intake of licorice syrup was presented. It was aimed to draw attention to the necessity of questioning whether there is excessive intake of licorice or not in patients who admitted to emergency department with paralysis and dyspnea. Plasma potassium concentration of the patient was 1.4 mmol/L. The patient\'s respiratory distress and loss of muscle strength recovered completely after potassium replacement. Amaç: Bu çalışma, eğitim araştırma hastanesinin acil bölümüne giriş yapan geriatrik travma hastalarının demografik karakteristiklerini, travma nedenlerini, fizik muayene bulgularını, kırıkların varlığını ve sonuç statülerini ortaya çıkarmayı amaçlamaktadır. Materyal ve Metod: Bu çalışma, 1 Eylül 2011-31 Ağustos 2012 tarihleri arasında acil bölümüne giriş yapmış travmalı 65 yaş üstü hastaları içermektedir. Acil bölümünde ki hastaların travma nedenleri, fizik muayene bulguları ve sonuç statülerinin yanı sıra yaş, cinsiyet, başvuru tarihi gibi demografik karakteristikleri de değerlendirilmiştir. Çalışma prospektif olarak yapıldı. Elde edilen dataların istatistik analizleri için SPSS V.20 programı kullanıldı. Bulgular: Çalışmaya 74 (%42,28)"ü erkek, 101 (%57,72)"i kadın olmak üzere 175 kişi dahil edildi. Kadın hastaların yaş ortalaması 76.10 ± 7.353 iken erkek hastaların yaş ortalaması 75.01 ± 6.557 idi. Her 2 cinsiyette de en yaygın travma nedeni; düşmelerdi. Bu oran kadın hastalarda %91,1 iken erkek hastalarda %8,9"du. Kadın hastaların %40,6"sı, erkek hastaların ise %27"si herhangi bir travma nedeni olmadan önce hastaneye baş vurdu. Her iki cinsiyette de vücut lokalizasyonuna göre en yaygın travma; ekstremite travmasıydı. Bu oran kadınlarda %51,5, erkeklerde ise %56,8 olarak bulundu. Kadın hastaların 30"unda (%29,7) ve erkek hastaların 13"ünde (%17,6) bacak kırığı vardı. Tüm hastaların %78,3"ü acil bölümünden taburcu edildi, %21,7"si ise hastanede yatmaya devam etti. Acil bölümünde hastaların hiç biri ölmedi ve başka bir kuruma sevk edilmedi. Hastanede yatan toplam 38 hastanın 32"si taburcu edildi, 2"si başka bir kuruma sevk edildi ve 4"ü öldü. Hastanede yatan 38 hastanın 26"sı cerrahi müdahaleye maruz kaldı ki bunların 20"si ortopedik ameliyatlardı. Sonuç: Geriatrik travmaların çoğu trafik kazaları ve yüksekten düşmeler sonucu meydana gelmektedir. Bu yaralanmalar yaşlı kişilerde ciddi hastalıklara ve ölümlere neden olabilir. Bu problemler, bakıcılara ve aile bireylerine iyi bir eğitim ve gerekli bilgiler verilerek önemli derecede önlenebilir.

Amaç: Bu çalışmada kardiyak arrest tanısı konulan hastalarda kardiyopulmoner resüsitasyon uygulamasındaki başarı oranının ve bu hastaların demografik özelliklerinin saptanması amaçlandı. Materyal ve Metod: Geriye dönük olarak yapılması planlanan çalışmaya Adana Numune Eğitim ve Araştırma Hastanesi Acil Tıp Kliniği"ne 01.01.2011 ve 31.12.2012 tarihleri arasında başvuran ve kardiyopulmoner resüsitasyon uygulanan hastalar alındı. Güncel ileri yaşam desteği protokollerine uygun olarak kardiyopulmoner resüsitasyon uygulanan hastaların yaş, cinsiyet, adli olgu durumu, kardiyak arrest nedeni ve zamanı, arrestte ilk gözlenen ritm, arrest öncesinde var olan hastalığı, acil servise geliş şekli, kardiyopulmoner resüsitasyon süresi, acil kliniği"nde uygulanan kardiyopulmoner resüsitasyon"un sonucu, yattığı klinik, operasyon varlığı, hastaların sonlanımı standart veri giriş formuna kaydedildi. Bulgular: Verilerine eksiksiz ulaşabildiğimiz toplam 290 hasta çalışmaya alındı. Bu hastaların çoğu erkekti (% 65.2). Yaş ortalaması erkeklerde 61 ± 19 yıl, kadınlarda ise 67 ± 14 yıl idi (p=0.018). Hastaların özgeçmişleri incelendiğinde en sık iskemik kalp hastalığı ve kalp yetmezliği tanısı aldıkları saptandı. Hastaların 92"sinin (% 31.7) acil servise ölü duhul olarak getirildiği, ve bu hastaların hiçbirinde kardiyopulmoner resüsitasyon"da başarılı olunamadığı belirlendi. Hastaların 198"inin (% 68.3) acil serviste arrest olduğu ve 102"sinde kardiyopulmoner resüsitasyon uygulamasında başarılı olunduğu tespit edildi. En sık kardiyak arrest nedenleri miyokard infarktüsü ve kalp yetmezliği idi. Monitörde ilk gözlenen ritm en fazla asistoli idi. Ventriküler fibrilasyon ve ventriküler taşikardi olan hastalarda kardiyopulmoner resüsitasyona cevap oranı daha yüksek bulundu. Hastaların en fazla koroner yoğun bakım ünitesi"ne yatırıldığı ve perkütan koroner girişim yapılan 21 hastanın 11"inin şifa ile taburcu edildiği belirlendi. Çalışmaya dahil edilen tüm hastaların toplam 15"inin (% 5) şifa ile taburcu edildiği tespit edildi. Sonuç: Kardiyopulmoner resüsitasyon"da başarı oranının düşük olması kardiyopulmoner resüsitasyon yaşam zincirinin tüm basamaklarında aksaklıklar olduğunu, düzeltmeye yönelik adımlar atılması gerektiğini göstermektedir.

Amaç: Bu çalışmada; acil servise göğüs ağrısı şikayeti ile başvuran ve miyokard infarktüsü (MI) tanısı konulan hastalarda tümör nekroz faktör-beta (TNF-) A329G gen polimorfizminin etkinliğinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Prospektif, randomize, kontrollü olarak yapılması planlanan çalışmamıza Cukurova University Tıp Fakültesi Etik Kurulu’ndan onay alındıktan sonra başlandı. Çalışmaya onam veren hastalardan kan örneği alındı. Alınan kan örneklerinin analizi Mersin Üniversitesi Tıp Fakültesi Biyokimya Laboratuvarı’nda çalışıldı. DNA izolasyonu, DNA izolasyon kiti (High Pure PCR Template Preparation kit, Roche Diagnostic, Germany) ile yapıldı. TNF-β A329G gen polimorfizmi Light Cycler mutasyon belirleme kiti (Roche diagnostic, GmbH, Mannheim, Germany) kullanılarak Light Cycler cihazında Real Time PCR ile saptandı. Verilerin istatistiksel analizi SPSS 11.5 paket programı ile yapıldı. Tüm testlerde istatistiksel önem düzeyi p < 0.05 olarak alındı. Bulgular: Bu çalışmaya miyokard infarktüsü tanısı konulan 90 hasta (78 erkek, 12 kadın) ve kontrol grubu olarak 78 sağlıklı birey (28 erkek, 50 kadın) dahil edildi. Tümör nekroz faktör-beta A329G polimorfizmi ile miyokard infarktüsü arasında istatistiksel olarak anlamlı bir ilişki bulunamadı. Miyokard infarktüs’ lü hastaların % 42.2’sinde AG genotipi ve % 6.7’sinde ise GG genotipi saptandı. MI olan hastalarda kontrol grubuna göre LDL düzeyinin istatistiksel olarak anlamlı derecede yüksek olduğu (p = 0.021), HDL düzyinin ise istatistiksel olarak anlamlı derecede düşük olduğu (p = < 0.001) saptandı. Sonuç: TNF-β A329G gen polimorfizmi ile Miyokard İnfarktüsü arasında bir ilişki olmadığı saptandı. Yüksek LDL ve düşük HDL düzeylerinin Mİ için risk faktörleri olduğu tespit edildi. Ülkemizde Türk toplumuna yönelik TNF-β A329G gen polimorfizmi ile MI arasındaki ilişkiyi belirlemeye yönelik yapılan ilk çalışma olması açısından bu çalışma önemlidir. Bu çalışma, diğer çalışmalara ışık tutabilir ve tümör nekroz faktörün diğer polimorfizmleri araştırılarak muhtemel olası anlamlı bulgular elde edilebilir.

Purpose: In this study, we aimed to determine the success rate of cardiopulmonary resuscitation performed in the patients with diagnosis of cardiac arrest, and demographic characteristics of these patients. Material and Methods: The patients admitted to Adana Numune Education and Research Hospital, Department of Emergency Medicine between 01.01.2011 and 31.12.2012, and who underwent cardiopulmonary resuscitation were included to this study planned as retrospectively. The age, gender, status of judicial cases, causes and time of cardiac arrest, first observed arrest rhythm, the diseases prior to the arrest, means of arrival to emergency department, duration of cardiopulmonary resuscitation, results of cardiopulmonary resuscitation, the name of the hospitalised clinic, the existence of the operation, and outcome of the patients who underwent cardiopulmonary resuscitation in accordance with current advanced life support protocols were recorded in standard data entry form. Results: A total of 290 patients with completely accessible data were included to the study. Most of these patients were men (65.2%). The mean ages were 61 ± 19 years for men, 67 ± 14 years for women (p = 0.018). The most common diagnosis were ischemic heart disease and heart failure according to the analysis of the patient's medical history. 92 patients (31.7%) were brought to the emergency department after death, and all of these patients were unsuccessful following to cardiopulmonary resuscitation. 198 patients (68.3%) had cardiac arrest in the emergency department, and we determined that cardiopulmonary resuscitation application of 102 patients were successful. The most common causes of cardiac arrest were myocardial infarction and heart failure. Mostly first observed rhythm in the monitor was asystole. The response rate of cardiopulmonary resuscitation in patients with ventricular fibrillation and ventricular tachycardia was higher. Most patients were hospitalised to the coronary intensive care unit, and 11 of the 21 patients who underwent percutaneous coronary intervention were discharged from hospital in good health. Total 15 (5%) of all patients included to the study were discharged in good health. Conclusion: The lower rate of success in cardiopulmonary resuscitation showed the presence of defects in all stages of the chain of life, and suggests that some actions should be performed to correct them. Objective: The aim of this study is to reveal the demographic characteristics, causes of trauma, physical examination findings, the presence of fractures and the status of the outcome of the geriatric trauma patients admitted to the emergency department of an educational research hospital. Material and Methods: This study covers all the cases over 65 years who were admitted to emergency department with trauma between September 1 2011-31 August 2012. The demographic characteristics of the patients such as, age, gender, date of application and as well as the causes of trauma, physical examination findings and outcome situation in the emergency department were evaluated. The study was performed prospectively. SPSS V.20 was used for statistical analysis of the data obtained. Results: Total 175 patients were included to the study, 74 were male (42.28%) and 101 were female (57.72%). The mean age of male patients were 75.01 ± 6.557 while the mean age of female patients were 76.10 ± 7.353. The most common cause of trauma in both gender was falls. This rate was 91.1% in female and 8.9% in male patients. 40.6% of the female patients and 27% of the male patients were admitted to the hospital before because of any trauma. The most common form of trauma according to exposed body localization in both gender was extremity traumas. It was seen in 51.5% of the females and 56.8% of the males. 30 female patients (29.7%) and 13 male patients (17.6%) had fracture in limbs. 78.3% of all patients were discharged from the emergency department and 21.7% of the patients were hospitalised. None of the patients were died in emergency department and none of the patients were referred to another institution from the emergency department. Total 38 patients were hospitalised, 32 of them were discharged, 2 of them were referred to another institution, and 4 of them were died. 26 of 38 hospitalised patients had undergone surgery while 20 of them were orthopedic surgeries. Conclusions: Most of the geriatric trauma patients constitute of traffic accidents and falls from height. These injuries can lead to serious morbidity and mortality in elderly people, and these problems can be prevented significantly by giving information and education to family members and caregivers in terms of preventive measures. Amaç: Bu çalışmadaki amaç acil tıp kliniği" ne başvuran adli olguların demografik ve epidemiyolojik özelliklerini, düzenlenen adli raporların hayati tehlike içeriğini, basit tıbbi müdahale durumlarını ve acilde sonlanmalarını açığa çıkarmaktır. Gereç ve Yöntem: Geriye dönük yapılan bu çalışmaya acil tıp kliniğine 01.12.2009 - 31.12.2010 tarihleri arasında başvuran ve kayıtlarına ulaşılan 18 yaş ve üzeri erişkin adli olgular dahil edildi. Elde edilen verilerden kategorik ölçümler sayı ve yüzde olarak, sayısal ölçümler ise ortalama ve standart sapma olarak özetlendi ve SPSS 17.0 paket programı ile analiz edildi. Bulgular: Çalışmadaki 5870 adli olgunun % 63.78"i erkek, % 36.22"si kadındı. Yaş ortalaması 33.75 ± 12.4 idi. Adli duruma sebep olan olaylardan, ilk üç sırada görülen trafik kazası (% 27.3), zehirlenme (% 23.4) ve darp (% 17.6) idi. Trafik kazalarının en sık 26-33 yaş grubu erkeklerde, zehirlenmelerin en sık 18-25 yaş grubu kadınlarda görüldüğü tespit edildi. En fazla yaralanmanın 2404 olgu ile ekstremite yaralanması olduğu görüldü. Acile başvuran adli olguların % 73.3"ü taburcu edilirken, % 26.3"ü yatırıldı. % 0.3"ünün (19 olgu) acil serviste öldüğü, % 0.1"inin (4 olgu) ölü duhul olduğu belirlendi. Ölüm en fazla TK ve yüksekten düşmelerde görüldü. Vakalara düzenlenen adli raporlar değerlendirildiğinde erkeklere düzenlenen raporların % 28.8"inin, kadınlara düzenlenen raporların da % 11.3"ünün basit tıbbi müdahale ile giderilemez şeklinde olduğu görüldü. Adli raporlarda tüm olguların sadece 3336"sının (% 56.8) hayati tehlike durumunun belirtildiği tespit edildi. Hayati tehlike durumu belirtilen olguların % 21.1"inin hayati tehlikesi mevcut idi. Sonuç: Adli vakalar en çok erkek hastalarda, en çok 26-33 yaş grubunda görülmektedir. Tanıların sıklığı erkek ve bayan hastalarda değişmektedir. Adli vakaların % 26,3 gibi yüksek oranda hastaneye yatış gerektirmesine, % 21" inin hayati tehlikesinin olmasına rağmen, acilde mortalite oranı % 0.3"dür. Adli raporlarda mutlaka belirtilmesi gerekli hayati tehlike durumu raporların yarıya yakın kısmında yazılmamıştır. Kişilerin hak kaybı ve gereksiz mağduriyetini önlemek için, hekimlerin yaralıyı iyi tedavi etmesinin yanında, kanun ve kurallara uygun, açık, net anlaşılabilir adli rapor yazma sorumluluğu da vardır.

Giriş: Çalışmamızda akut organofosfat zehirlenmesine bağlı elektrokardiyografik (EKG) değişiklikler ile kolinesteraz düzeylerinin mortaliteye etkilerini araştırmayı amaçladık. Materyal ve Metod: Prospektif, randomize olarak 2 yıl süresince, 01.08.2009 – 31.08.2011 tarihleri arasında acil tıp kliniği’ne organik fosfat zehirlenmesi ile gelen hastaların alınmasının planlandığı çalışmaya Etik Kurul’dan onay alındıktan sonra başlandı. Acile başvuru sonrası, tedavi (atropin ve pralidoksim uygulaması) öncesinde çalışmada kullanılmak üzere EKG çekildi ve kan örneği alınarak, serum ve eritrosit kolinesteraz düzeyleri hastanemiz laboratuvarı’nda çalışıldı. Hastaların EKG bulguları, kolinesteraz değerleri, sonlanım durumu standart veri giriş formuna kayıt edildi. Verilerin istatistiksel analizinde SPSS 18.0 paket programı kullanıldı. Bulgular: Çalışmaya alınan 39 hastanın 5’i (% 12.8) tedavileri sırasında eksitus oldu. Hastalarda görülen elektrokardiyografik bulgular sıklık sırasına göre; sinüs taşikardisi (% 48.7), uzamış QT mesafesi (% 20.5) ve sağ dal bloğu (% 20.5), ST-T dalga değişikliği (% 12.8), atriyal fibrilasyon (% 7.7), sağ aks sapması (% 5.1), uzamış PR mesafesi (% 2.5) idi. Eksitus ve taburcu edilen hastaların acile ilk başvurusu sonrasında çekilen EKG bulguları karşılaştırıldığında gruplar arasında istatistiksel olarak fark olmadığı tespit edildi (p ≥ 0.05). Eksitus olan hastaların ilk geliş anında bakılan ortalama serum kolinesteraz düzeyleri ortalaması taburcu edilen hastalara göre düşüktü ve aralarında istatistiksel olarak anlamlı bir fark mevcuttu (p=0.006). Ancak eksitus ve taburcu edilen hastaların eritrosit kolinesteraz düzeyleri arasında anlamlı farklılık yoktu (p= 0.984). Sonuç: Organofosfat zehirlenmesinin şiddeti ile EKG bulguları arasında mortalite açısından anlamlı bir ilişki olmadığı, şiddetli zehirlenmelerde serum kolinesteraz düzeyi düşüklüğü ile mortalite arasında istatistiksel olarak anlamlı bir ilişki olduğu belirlendi. Organofosfat zehirlenmelerinde görülebilecek EKG bulguları ve mortaliteye etkileri hakkında acil hekimlerine fikir vermesi, klinik deneyimlerine katkıda bulunması açısından bu çalışmanın faydası olacaktır. Ancak bu konuda yapılacak daha detaylı çalışmalara ihtiyaç vardır.

Aim: In our study, we aimed to investigate the relationship between the mortality and acute inflammatory markers, serum - erythrocyte cholinesterase levels in patients with organophosphorus poisoning. Material and Methods: We planned to take patients who administered to emergency department with organic phosphorus poisoning prospectively for 2 years after approval by the Ethics Committee of Cukurova University. Patients with hereditary cholinesterase deficiency, liver disorders, malnourished, anemia, using cocaine, morphine, codeine, and medications such as succinylcholine were excluded from the study. Aim: In our study we aimed to investigate the relationship between electrocardiographic (ECG) changes, cholinesterase levels and mortality due to acute organophosphate poisoning. Material and Methods: We planned to take patients who administered to emergency department with organic phosphorus poisoning randomized and prospectively for 2 years between 01.08.2009 – 31.08.2011 after approval by the Ethics Committee. The electrocardiograms of the patients were taken after admission to the emergency department, before the treatment (atropine and pralidoxime application). Blood samples were taken and studied in the laboratory of our hospital. ECG findings, cholinesterase values and the final situation of the patients were recorded at the standard data entry form. SPSS 18.0 package program was used for statistical analysis of data. The statistical significance level of all tests was p < 0.05. Results: The five of thirty nine patients (12.8 %) included in the study have died during the treatment. The electrocardiographic findings seen in patients in order of frequency; sinus tachycardia (48.7 %), prolonged QT interval (20.5 %), and right bundle branch block (20.5 %), ST-T wave changes (12.8 %), atrial fibrillation (7.7 %), right axis deviation (5.1 %), prolonged PR (2.5 %). There was no statistically significant difference between the ECG findings of the patient groups who died or discharged (p ≥ 0.05). The mean serum cholinesterase levels of the patients who died were statistically lower than the discharged patients (p = 0.006). Conclusion: There is not a significant relationship between ECG findings and the severity of organophosphate poisoning.There is a statistically significant relationship between mortality and low levels of serum cholinesterase due to severe poisoning. ECG findings and their effects to the mortality in organophosphate poisoning can give an opinion to the emergency physicians and contribute to their clinical experiences. Amaç: Bu çalışmada acil servisimize kafa travması nedeniyle başvuran hastaların epidemiyolojik özelliklerini, mortalite ve morbidite oranlarını belirlemeyi amaçladık. Gereç ve Yöntem: Bu çalışmada, 01.12.2009 - 31.12.2010 tarihleri arasında acil tıp kliniği"ne kafa travması nedeni ile getirilen, ayaktan ve yatırılarak tedavi edilen 18 yaş üzerindeki hastalar geriye dönük olarak incelendi. Hasta verileri standart veri giriş formuna kaydedildi. Verilerin istatistiksel analizinde SPSS 17.0 paket programı kullanıldı. Tüm testlerde istatistiksel önem düzeyi p < 0.05 olarak alındı. Bulgular: Çalışmaya toplam 5200 hasta alındı. Hastaların yaş ortalaması 39.97±16.66 yıl idi. Hastaların 4682"si (% 90) acil servisten taburcu edilmişti. Acil servisten taburcu edilen hastaların en sık başvuru nedeni düşme (% 41.81) idi. Çalışmaya alınan hastalardan 518"i (% 10) hastaneye yatırılmıştı. Bu hastaların 110"u kadın (% 21.24), 408"i erkek (% 78.76) idi. Bu hastaların 256"sı (% 48.35) trafik kazası sonucu yaralanmıştı. Yatan hastaların BBT bulguları incelendiğinde 201 (% 38.8) hastanın BBT"sinin normal olduğu, 89 hastanın (% 17.2) travmatik subaraknoid kanama"sı (SAK) olduğu tespit edildi. Hastalarda ek patoloji olarak en fazla lomber vertebra fraktürü (% 12) saptandı. Kafa travması nedeni ile yatırılan hastaların 75"i (% 14.5) eksitus olmuş idi (Tüm hastaların %1,44"ü). Eksitus olan hastalarda en sık rastlanan ek patoloji servikal vertebra fraktürü idi (14 hasta, % 18.68). İkinci sıklıkta ise toraks travması (13 hasta, % 17.33) saptandı. Sonuç: Acil servise başvuran kafa travmalı olguların çoğu basit travmadır. Dikkatli ve tam bir fizik muayene sonrasında basit tıbbi müdahale ile bu hastalar acil servisten taburcu edilebilir. Hastaneye kafa travmasıyla yatırılan hastaların çoğu erkek, en sık başvuru sebebi trafik kazası, BBT"de rastlanan en sık bulgu SAK"tır. Trafik kazaları ölüm sebeplerinde ilk sırada gelmekle birlikte ateşli silah yaralanmalarında ölüm oranı daha yüksektir. Acil hekimlerinin kafa travmalı hastalara yaklaşımı ve klinik deneyimlerine katkıda bulunması açısından bu çalışmanın faydası olacaktır. Yapılacak daha detaylı çalışmalarla ülkemize özgü acil travma protokolleri oluşturulabilir.
