BACKGROUND: Preservation of quality of life regarding fecal continence after abdominoperineal excision (APE) in cancer is challenging. Simultaneous soft tissue coverage and restoration of continence mechanism can be provided through an interdisciplinary collaboration of colorectal and plastic reconstructive surgery. OBJECTIVE: Evaluation of surgical procedure and outcome combining soft tissue reconstruction using a central perforated vertical rectus abdominis myocutaneous flap (VRAM), implementing a perineostoma and restoring anorectal angle augmenting the levator ani by neurostimulated graciloplasty. METHODS: 14 Patients underwent APE due to cancer. In all patients coverage was achieved by pedicled VRAM and simultaneous pull-through descendostomy (perineostoma). 10 of those patients received a levator augmentation additionally. Postoperative complications, functional measures of continence as well as quality of life were obtained. RESULTS: Perineal minor complication rate was 43% without need of surgical intervention. All but one VRAM survived. Continence measures and disease specific life quality showed a good preservation of continence in most patients. CONCLUSION: The results present a complex therapy option accomplished by a collaboration of two highly specialized partners (visceral and plastic surgery) after total loss of the sphincter function and consecutive fecal insufficiency after APE.

Microsurgical free flap transfer plays a key role in soft tissue reconstruction of the lower extremities. Through close cooperation between plastic and orthopedic surgery, great progress and success in limb salvage could be achieved over the last decades. The risk for extremity malperfusion is especially high in older patients and after trauma. To maximize the success rate for free flap transfer there is need for interdisciplinary clinical examination and diagnostics. In addition to clinical methods radiological procedures are necessary to evaluate and optimize lower extremity perfusion before surgery.Vascular ultrasound provides important information about the arterial and venous status; however, DSA, CTA and MRA are well-established and exact methods to evaluate arterial inflow. The use of less invasive methods makes it much more feasible, economic and comfortable to perform preoperative selection of patients requiring interventional procedures.In the case of intraluminal stenosis without any option for PTA, a vascular surgeon can be involved at an early stage to evaluate further surgical options. In some cases, similar surgical revascularization and free flap transfer can be performed in a single surgery. The aim of this study is to implement a standardized algorithm for preoperative examination and radiological diagnostics before reconstructive surgery of the lower extremity.

The reverse sural artery flap (RSAF) was first described approximately four decades ago and has since been used frequently for reconstruction of soft tissue defects in the distal part of the leg. Although the popularity of this flap never reached the extent of that of free perforator flaps, it still serves as a reliable alternative. This has been demonstrated by the increased rate of publication on the technique in recent years. The number of authors reporting data has risen up to 20 reports a year. During this time, several new modifications of the flap were inaugurated in order to boost the efficiency and reliability of the flap. The goal of this systematic analysis of the literature was to gain information on the influences of the modifications of the RSAF with regard to consistency and complication rates. An overall survival rate of 95% and a rate of complications of 14% were reported in all the analyzed cases. Some modifications were able to improve flap viability but not at a statistically significant extent. Venous supercharging and the adipofascial variant of the RSAF provided the best results. The RSAF is still a remarkable flap design, especially as a backup solution or in circumstances without a microsurgical skilled surgeon. Further investigation with controlled randomized prospective trials is vital to confirm this finding with more evidence.

Hintergrund Die Fotodokumentation von offenen Frakturen, Wunden, Dekubitalulzera, Tumoren oder Infektionen ist ein wichtiger Bestandteil der digitalen Patientenakte. Bisher ist unklar, welchen Stellenwert diese Fotodokumentation bei der Abrechnungsprüfung durch den Medizinischen Dienst der Krankenkassen (MDK) hat. Fragestellung Kann eine Smartphone-basierte Fotodokumentation die Verteidigung von erlösrelevanten Diagnosen und Prozeduren sowie der Verweildauer verbessern? Material und Methoden Ausstattung der Mitarbeiter mit digitalen Endgeräten (Smartphone/Tablet) in den Bereichen Notaufnahme, Schockraum, OP, Sprechstunden sowie auf den Stationen. Retrospektive Auswertung der Abrechnungsprüfung im Jahr 2019 und Identifikation aller Fallbesprechungen, in denen die Fotodokumentation eine Erlösveränderung bewirkt hat. Ergebnisse Von insgesamt 372 Fallbesprechungen half die Fotodokumentation in 27 Fällen (7,2 %) zur Bestätigung eines Operationen- und Prozedurenschlüssels (OPS) (n = 5; 1,3 %), einer Hauptdiagnose (n = 10; 2,7 %), einer Nebendiagnose (n = 3; 0,8 %) oder der Krankenhausverweildauer (n = 9; 2,4 %). Pro oben genanntem Fall mit Fotodokumentation ergab sich eine durchschnittliche Erlössteigerung von 2119 €. Inklusive Aufwandpauschale für die Verhandlungen wurde somit ein Gesamtbetrag von 65.328 € verteidigt. Diskussion Der Einsatz einer Smartphone-basierten Fotodokumentation kann die Qualität der Dokumentation verbessern und Erlöseinbußen bei der Abrechnungsprüfung verhindern. Die Implementierung digitaler Endgeräte mit entsprechender Software ist ein wichtiger Teil des digitalen Strukturwandels in Kliniken., Background Photographic documentation of wounds, decubitus ulcers, tumors, open fractures and infections is an important part of digital patient files. It is unclear whether the photographic documentation has an effect on medical accounting with health insurance companies. Objective It was hypothesized that Smartphone-based systematic photographic documentation can improve the confirmation of proceeds-relevant diagnoses and procedures as well as the duration. Material and methods Staff in the emergency room, operating theater, outpatient clinic and on the wards were equipped with digital devices (Smartphone, tablet) including a photo-app. Medical accounting with the health insurance companies and identification of all case conferences in which the photographic documentation had effected a change in proceeds were analyzed for 2019 in a retrospective manner. Results Overall, 372 cases were discussed of which 27 cases were affected by the digital photographic documentation. Photographic documentation was used for clarification of the operative procedure (n = 5), primary diagnosis (n = 10), secondary diagnosis (n = 3), and length of hospitalization (n = 9). An average of 2119 € was negotiated and added per case affected by photographic documentation. Hereby, a level 1 trauma center gained an estimated 65,328 € in revenue. Discussion The use of Smartphone based photographic documentation can improve the overall quality of patient files and thus avoid loss of revenue. The implementation of digital devices with corresponding software is an important component of the digital structural change in hospitals.

Zusammenfassung Hintergrund Im Rahmen der SARS-CoV-2-Pandemie wurde zur Bewältigung der erwarteten COVID-19-Patienten eine Reduktion der ambulanten und stationären elektiven Patientenbehandlung an den dafür vorgesehenen Kliniken gefordert. Je nach örtlichen Gegebenheiten und Versorgungsauftrag kam es während der SARS-CoV-2-Restriktionen zu unterschiedlichen Auswirkungen auf die Patientenversorgung. Betrachtet werden sollte der Anteil der notwendigen Versorgungsleistungen während der SARS-CoV-2-Restriktionen in einer Plastischen Chirurgie einer Großklinik. Methode Untersucht wurde die OP-Auslastung einer Abteilung für Plastische Chirurgie einer universitären Klinik im Zeitraum vom 16.03.2020 bis 27.04.2020. Diese Daten wurden mit Daten desselben Zeitraumes der Jahre 2017–2019 verglichen. Ergebnisse Auf Grund des vollständigen Stopps an elektiven Operationen kam es in dem Zeitraum der Restriktionen zu einer OP-Auslastung von 57,3 % verglichen mit den Jahren zuvor. Das Verhältnis von notfallmäßigen (2020: 56,4 %; 2017–2019: 54,9 %) und dringlichen (2020: 44,6 %; 2017–2019: 45 %) Operationen zu der Gesamtanzahl an OPs zeigte keine deutliche Veränderung. Des Weiteren zeigten sich keine ausschlaggebenden Unterschiede der verhältnismäßigen Verteilung der notfallmäßigen und dringlichen Operationen bezogen auf die Plastisch-Chirurgischen Teilbereiche, die Verletzungsursache oder den Versicherungsstatus (BG/Nicht-BG). Zusammenfassung Auf Grund der vorliegenden Daten kann die Relevanz des Fachgebiets für die allgemeine Patientenversorgung belegt werden. Daraus ergibt sich eine eindeutige Erforderlichkeit der Vorhaltung von Plastisch-Chirurgischen OP-Kapazitäten und Infrastrukturen auch während Krisenzeiten.

Articular cartilage is an exceptional connective tissue which by a network of fibrillar collagen and glycosaminoglycan (GAG) molecules allows both low- friction articulation and distribution of loads to the subchondral bone (Armiento et al., 2018, Ulrich-Vinther et al., 2003). Because of its very limited ability to self-repair, chondral defects following traumatic injury increase the risk for secondary osteoarthritis (OA) (Muthuri et al., 2011). Still, current OA treatments such as common nonsteroidal anti-inflammatory drugs (NSAIDs) and joint replacement primarily address end-stage symptoms (Tonge et al., 2014). As low-grade inflammation plays a pivotal role in the pathogenesis of OA (Robinson et al., 2016), there is a strong demand for novel therapeutic concepts, such as integrating application of anti-inflammatory agents into cartilage cell- based therapies in order to effectively treat OA affected joints in early disease stages. The polyphenolic phytoalexin resveratrol (RSV), found in the skin of red grapes, berries, and peanuts, has been shown to have effective anti-inflammatory properties (Shen et al., 2012). However, its long-term effects on 3D chondrocyte constructs cultured in an inflammatory environment with regard to tissue quality have remained unexplored so far. Therefore, in this study, pellets made from expanded porcine articular chondrocytes were cultured for 14 days with either the pro-inflammatory cytokine interleukin-1β (IL-1β) (1 - 10 ng/ml) or RSV (50 μM) alone, or a co-treatment with both agents. Constructs treated with chondrocyte medium only served as control. Treatment with IL-1β at 10 ng/ml resulted in a significantly smaller pellet size and reduced DNA content. However, RSV counteracted the IL-1β-induced decrease and significantly enhanced diameter and DNA content. Also, in terms of GAG deposition, treatment with IL-1β at 10 ng/ml resulted in a tremendous depletion of absolute GAG content and GAG/DNA. Again, RSV co-treatment counteracted the inflammatory stimulus and led to a partial recovery of GAG content. Histological analysis utilizing safranin-O staining confirmed these findings. Marked expression of the cartilage-degrading enzyme matrix metalloproteinase 13 (MMP13) was detected in IL-1β-treated pellets, but none upon RSV co- treatment. Moreover, co-treatment of IL-1β-challenged constructs with RSV significantly increased absolute collagen content. However, under non- inflammatory conditions, RSV induced gene expression and protein accumulation of collagen type X, a marker for undesirable hypertrophy. Taken together, in the present thesis, RSV was demonstrated to elicit marked beneficial effects on the extracellular matrix composition of 3D cartilaginous constructs in long-term inflammatory culture in vitro, but also induced hypertrophy under non-inflammatory conditions. Based on these findings, further experiments examining multiple concentrations of RSV under various inflammatory conditions appear desirable concerning potential therapeutic applicability in OA.

Zusammenfassung Hintergrund Die Weichgewebsrekonstruktion bei alten Patienten stellt eine Herausforderung dar. Der freie Gewebetransfer kann bei gesunden Patienten trotz hohen Alters mit hoher Erfolgsrate durchgeführt werden. Bei multimorbiden Patienten, die für den freien Gewebetransfer nicht in Betracht kommen, werden häufig lokale Lappenplastiken eingesetzt, welche mit einer hohen Komplikationsrate assoziiert sind. Gerade solche Rettungseingriffe müssen so gewählt werden, dass eine Amputation durch die Wahl der Entnahmestelle nicht nachteilig beeinflusst wird oder gar unmöglich wird. Methode Drei distal basierte lokale Lappenplastiken wie Suralis‑, Peronaeus-brevis- und Propellerlappenplastik werden im Hinblick auf die Platzierung der Entnahmestelle sowie die Komplikationen der Wundheilungsstörung diskutiert. Ergebnisse Die Entnahmestelle der Suralislappenplastik ist nachteilig, da die proximale, dorsale Wadenregion betroffen ist, die im Falle einer Unterschenkelamputation die Weichteildeckung des Stumpfes ermöglicht. Schlussfolgerung Soll eine lokale Lappenplastik als „Rettungseingriff“ als Versuch der Vermeidung einer Unterschenkelamputation bei für einen freien Gewebetransfer nichtgeeigneten Patienten zum Einsatz kommen, gilt es, Lappenplastiken zu wählen, die die Entnahmestelle außerhalb der dorsalen, proximalen Wade platzieren, um die Weichteildeckung einer Stumpfbildung potenziell zu ermöglichen.

BACKGROUND Monteggia-like lesions are rare injuries. We retrospectively reviewed 40 consecutive patients with Monteggia-like lesions treated at a level I trauma center from 2009 to 2016. Clinical results were evaluated with a focus on the use of modern locking plates. OBJECTIVE AND METHODS Twenty-eight patients had Bado type II fractures, 11 had Bado type I, and 1 had Bado type III. All patients were treated by plate fixation of the ulna with a locking compression plate or double mini-plates. Fixation or replacement was performed for radial head fractures. We characterized all fractures and recorded the range of motion (ROM); 100-point visual analog scale (VAS) scores for pain and function; Disabilities of the Arm, Shoulder and Hand (DASH) score; and Mayo Elbow Performance Score (MEPS). A follow-up rate of 78% was reached at an average of 36 months (range, 12-95 months). RESULTS Stable fixation without recurrent instability was accomplished. The average ROM for flexion reached 130∘ with an extension deficit of 13∘. The sum of supination and pronation was 136∘. The VAS scores for pain and function were 20 and 75, respectively. The average MEPS and DASH scores were 84 and 28, respectively. Bado type I fractures had better outcomes for all variables, especially forearm rotation (p< 0.05). The complication rate was 22.5% and revision rate was 17.5%. CONCLUSION This fracture repair technique can safely achieve good and excellent outcomes in most cases and is partly superior to previously published techniques. However, Bado type II fractures still have a risk of poor results.

Complex regional pain syndrome (CRPS) typically develops after fracture or trauma. Many of the studies so far have analyzed clinical and molecular markers of CRPS in comparison with healthy or pain controls. This approach, however, neglects mechanisms occurring during physiological trauma recovery. Therefore, we compared the clinical phenotype, sensory profiles, patient-reported outcomes, and exosomal immunobarrier microRNAs (miRs) regulating barrier function and immune response between CRPS and fracture controls (FCs) not fulfilling the CRPS diagnostic criteria. We included upper-extremity FCs, acute CRPS I patients within 1 year after trauma, a second disease control group (painful diabetic polyneuropathy), and healthy controls. Fracture controls were not symptoms-free, but reported some pain, disability, anxiety, and cold pain hyperalgesia in quantitative sensory testing. Patients with CRPS had higher scores for pain, disability, and all patient-reported outcomes. In quantitative sensory testing, ipsilateral and contralateral sides differed significantly. However, on the affected side, patients with CRPS were more sensitive in only 3 parameters (pinprick pain and blunt pressure) when compared to FCs. Two principal components were identified in the cohort: pain and psychological parameters distinguishing FC and CPRS. Furthermore, the immunobarrier-protective hsa-miR-223-5p was increased in plasma exosomes in FCs with normal healing, but not in CRPS and healthy controls. Low hsa-miR-223-5p was particularly observed in subjects with edema pointing towards barrier breakdown. In summary, normal trauma healing includes some CRPS signs and symptoms. It is the combination of different factors that distinguish CRPS and FC. Fracture control as a control group can assist to discover resolution factors after trauma.

Introduction Epidemiological injury surveillance in professional sports is often based on online media analysis in order to collect necessary data. However, the validation of this study protocol is lacking. Therefore, this study aimed to identify the validity of injury surveillance in men’s professional team sports based on media reports. Methods In a retrospective cohort study, the validity of media-data-relating injuries was investigated in participating teams of the highest two German divisions in men’s professional basketball (BB) and handball (HB) in the season 2018/2019. Injury protocols completed by the team physicians were compared to those of sports media injury reports. Results The study population was composed of 133 athletes (54 BB and 79 HB). Of 343 injuries reported by the team physicians, 151 (44%) could be identified by means of sports media reports. Severe injuries (n = 75, 72%) were reported more likely in sports media compared to less severe injuries (n = 76, 32%, p Conclusion For severe injuries, media analysis may be a sufficient method for data collection in popular men’s professional ball sports. An underestimation of true injury prevalence lies within the range of previous reported investigations concerning the validation of injury surveillance methods. Non-severe injuries could not be verified via media analysis in professional handball and basketball.

Zusammenfassung Hintergrund Intensiv- und Beatmungskapazitäten sind für die Behandlung COVID-19-erkrankter Patienten essenziell. Unabhängig davon beanspruchen auch schwer verletzte Patienten häufig Intensiv- und Beatmungskapazitäten. Daraus ergibt sich folgende Fragestellung: Führt eine Ausgangsbeschränkung zu einer Reduktion schwer verletzter Patienten, und kann hierdurch mit frei werdenden Intensivkapazitäten gerechnet werden? Material und Methoden Es erfolgte eine retrospektive Auswertung schwer verletzter Patienten mit einem Injury Severity Score (ISS) ≥16 zwischen dem 17.03.2020 und 30.04.2020 (landesweiter Shutdown) an einem überregionalen Traumazentrum. Erfasst wurden der Unfallmechanismus, ISS, Versicherungsträger (BG vs. GKV/PKV), ob es sich um einen dokumentierten Suizidversuch handelte, und ob eine operative Intervention innerhalb der ersten 24 h erforderlich war. Als Kontrollgruppe wurden die Daten des gleichen Zeitraums der Jahre 2018 und 2019 ausgewertet. Ergebnisse Es konnte keine wesentliche Veränderung bezüglich der Anzahl an schwer verletzten Patienten festgestellt werden (2018 n = 30, 2019 n = 23, 2020 n = 27). Es zeigten sich insgesamt keine deutlichen Veränderungen der absoluten Zahlen bezüglich der Intensivpflichtigkeit in den ersten 24 h und der Beatmungspflichtigkeit beim Verlassen des Schockraums. Die Anzahl an Patienten, die eine Operation innerhalb der ersten 24 h nach Eintreffen im Schockraum benötigten, war 2020 sogar leicht erhöht, jedoch nicht statistisch signifikant. Der durchschnittliche ISS blieb konstant. Bezüglich der Unfallursache zeigte sich 2020 kein Motorradfahrer, der einen nicht berufsgenossenschaftlich versicherten Unfall erlitt (2018 n = 5, 2019 n = 4, 2020 n = 0). Es wurde 2020 ein erhöhter Anteil an Arbeitsunfällen mit einem ISS ≥16 festgestellt (2018: 10 %, 2019: 26,1 %, 2020: 44,4 %). Diskussion Eine Ausgangsbeschränkung führte zu keiner Reduktion verletzter- und intensivpflichtiger Patienten am untersuchten Zentrum. Auch unter einer landesweiten Ausgangsbeschränkung muss für dieses Patientenkollektiv eine ausreichende Menge an Intensiv- und OP-Kapazitäten vorgehalten werden. Die Bestätigung dieser Ergebnisse durch Auswertung nationaler Register steht noch aus.

Bouldering has become a sport of growing interest, but little prospective evidence exists about injury proportions and patterns. The purpose of this study was to prospectively evaluate the cause of injuries sustained during indoor bouldering, proportion of affected body location, and injury severity.Proportions and patterns of injury among German-speaking indoor boulderers were evaluated prospectively in an explorative cohort study. Participants completed a baseline questionnaire assessing anthropometric data and sport-specific potential preventive and risk factors, followed by monthly injury questionnaires including injury location and injury severity over a period of 12 mo.Out of 507 boulderers, 222 (44%) sustained 305 injuries. Of those, 78% (n=238) were classified as Union Internationale de Associations d'Alpinisme (UIAA) 1, 19% (n=57) as UIAA 2, and 3% (n=10) as UIAA 3. Injuries of the upper extremities accounted for 63% (n=191) of all injuries. Injuries of the lower extremities accounted for 23% (n=71) but were more often classified as UIAA≥2 (P=0.0071; odds ratio [OR] 2.23; 95% CI 1.23-4.04) and were more often caused by falling (P=0.0005; OR 2.92; 95% CI 1.57-5.42) and jumping off the wall (P0.0001; OR 4.39; 95% CI 2.25-8.56) than injuries of other body locations. There was no statistically significant protective effect of the evaluated potential preventive measures. Participants who used heavily downturned climbing shoes had a higher risk of sustaining a UIAA ≥2 injury (P=0.0034; OR 2.58; 95% CI 1.34-4.95).Injuries in indoor bouldering are common. Lower extremity injuries are associated with higher injury severity. Preventive measures need to be established to reduce bouldering injuries, especially during falls and landings.

Aims and Objectives: The rupture of the acl is considered to be one of the most common knee injuries in sports traumatology. Many patients report that they felt the sensation of a luxated patella during their knee distorsion. In many cases patients patients with the suspicion of a traumatic patella luxation showed in the mri a complete rupture of the acl. Materials and Methods: We report about 10 patients consulting our department within the last year after a significant knee trauma. In the mri as well as during the arthroscopic evaluation they showed a complete rupture of the acl as well as the findings of a fresh, traumatic patella luxation. We evaluated and analysed the preoperative and intraoperative pictures (mri and arthrosopy). Results: All patients showed in the posttraumatic mri a complete rupture of the acl as well as an at least partial rupture of the mpfl and a rupture of the medial retinaculum. 4 patients showed a persistent subluxation of the patella, 8 patients showed also a rupture of the medial collateral ligament. During the operation we verified a significant instability of the patella in 8 patients so that a stabilization of the patella was performed (open medial stabilization or mpfl reconstruction) in the same operation the acl was reconstructed (semi-t). Eventhough the patients were treated with only one operation and the regaining of motion was prolonged compared with isolated acl-reconstruction, after 12 months postoperatively all patients had a satisfactory range of motion. Conclusion: The typical trauma mechanism for an acl rupture is a valgus position of the knee in combination with an external rotation of the tibia, which leads to an enlargement of the q-angle which is considered a predisposition for a patella luxation. If an acl rupture occurs, the patella can luxate within the same trauma. In the posttraumatic mri the rupture of the mpfl is often misinterpreted as a rupture of the medial collateral ligament, particularly if the patella is, as in most cases, in full reposition. Within an acl-rupture a luxation of the patella can occur simultaneously, leading to a significant instability which needs to be treated (medial stabilization or mpfl-reconstruction). Our data shows that a single stage treatment with acl-reconstruction and medial stabilization is technical practicable and mid-term investigation shows good clinical results without a higher risk of an arthrofibrosis.

Study Design: Abstract consensus paper with systematic literature review. Objective: The aim of this study was to establish recommendations for treatment of thoracolumbar spine fractures based on systematic review of current literature and consensus of several spine surgery experts. Methods: The project was initiated in September 2008 and published in Germany in 2011. It was redone in 2017 based on systematic literature review, including new AOSpine classification. Members of the expert group were recruited from all over Germany working in hospitals of all levels of care. In total, the consensus process included 9 meetings and 20 hours of video conferences. Results: As regards existing studies with highest level of evidence, a clear recommendation regarding treatment (operative vs conservative) or regarding type of surgery (posterior vs anterior vs combined anterior-posterior) cannot be given. Treatment has to be indicated individually based on clinical presentation, general condition of the patient, and radiological parameters. The following specific parameters have to be regarded and are proposed as morphological modifiers in addition to AOSpine classification: sagittal and coronal alignment of spine, degree of vertebral body destruction, stenosis of spinal canal, and intervertebral disc lesion. Meanwhile, the recommendations are used as standard algorithm in many German spine clinics and trauma centers. Conclusion: Clinical presentation and general condition of the patient are basic requirements for decision making. Additionally, treatment recommendations offer the physician a standardized, reproducible, and in Germany commonly accepted algorithm based on AOSpine classification and 4 morphological modifiers.

To evaluate the use of active controlled motion (ACM) after unstable ankle fractures needing initial partial weight-bearing.Prospective randomized controlled trial.Inpatient and outpatient clinic.A total of 50 patients with unstable ankle fractures and the need for partial weight-bearing for six weeks.Randomization in two groups: physiotherapy alone or physiotherapy with an additional ACM device.Follow-up after 6 and 12 weeks. Range of motion, visual analogue scale for foot and ankle (VAS FA), Philip score, Mazur score, American Orthopaedic FootAnkle Society (AOFAS) score and dynamic pedobarography.Range of motion was better in the ACM group at six weeks (mean 49° ± 11.1° vs. 41.3° ± 8.1°). Questionnaires revealed better outcome after six weeks in the VAS FA (56 ± 13.7 vs. 40.6 ± 10.5), Mazur score (64.4 ± 12.3 vs. 56.7 ± 11) and AOFAS score (71.2 ± 12 vs. 63.6 ± 8.7) ( P0.02 for all). Better outcome after 12 weeks in all questionnaires (VAS FA, 77.7 ± 13.8 vs. 61.4 ± 16.3; Philip score, 79.1 ± 10.9 vs. 60.1 ± 21.7; Mazur score, 83.9 ± 10.7 vs. 73.1 ± 14.1; AOFAS score, 87.5 ± 7.9 vs. 75.2 ± 11.7) ( P0.01 for all). Pressure balance was better under the midfoot region after 12 weeks in the ACM group (Δ P 4.4 N vs. 34.0 N; P = 0.01). The ACM group had an earlier return to work after 10.5 (range, 3-17) versus 14.7 (range, 9-26) weeks ( P = 0.02).The use of ACM for patients needing initial partial weight-bearing after operatively treated unstable ankle fractures in the first six postoperative weeks leads to better clinical and functional results and an earlier return to work.

Hintergrund Die Symphysensprengung mit entsprechender Diastase kann durch eine Symphysenplatte stabilisiert werden. Fragestellung Welche Beckenverletzungen werden mit einer Symphysenplatte stabilisiert und wie ist das Outcome? Material und Methoden Retrospektive Auswertung von 64 Patienten über einen Untersuchungszeitraum von 24 Monaten. Ergebnisse Es waren 56 Patienten männlich, 8 weiblich und das mittlere Alter betrug 44 Jahre (SD ± 17). Unfälle im Straßenverkehr waren der führende Grund für die Beckenverletzung. Die Verteilung nach AO-Klassifikation zeigte sich wie folgt: 14-mal B1-, 10-mal B2-, 5‑mal B3-, 23-mal C1-, 9‑mal C2- und 3‑mal C3-Verletzungen. Die Verteilung nach Young und Burgess ergab: 9‑mal APC-I-, 18-mal APC-II-, 13-mal APC-III-, 9‑mal LC-I-, 3‑mal LC-II-, 2‑mal LC-III- und 10-mal VS-Verletzungen. Der mittlere Injury Severity Score (ISS) betrug 32 und die mittlere stationäre Verweildauer 29 Tage (pos. Korrelation p ≤ 0,001). Im Verlauf war eine radiologische Implantatlockerung bei 52 Patienten nachweisbar. Therapierelevante Komplikationen gab es in 14 Fällen. Hierbei war das Implantatversagen (n = 8) der Hauptgrund für eine operative Revision. Diskussion Obwohl die radiologische Implantatlockerung häufig beobachtet wird, ist sie nur selten Grund für einen Revisionseingriff. Kommt es hingegen zum vollständigen Implantatversagen, tritt dies meist innerhalb der ersten postoperativen Wochen auf und ist revisionsbedürftig. Eine frühzeitige Abklärung durch Röntgenbildgebung sollte bei Verdacht erfolgen., Background Separation of the pubic symphysis with corresponding diastasis can be stabilized by symphyseal plating. Objective Which pelvic injuries are stabilized with symphyseal plating and what is their outcome? Material and methods A retrospective evaluation of 64 patients who underwent symphyseal plating over a period of 24 months was conducted. Results Of the patients 56 were male and 8 female with a mean age of 44 years (SD ± 17 years). The main cause of the pelvic injuries were traffic accidents. The distribution according to the AO fracture classification was 14 × B1, 10 × B2, 5 × B3, 23 × C1, 9 × C2 and 3 × C3 injuries. The distribution according to the Young and Burgess classification showed 9 × APC I, 18 × APC II, 13 × APC III, 9 × LC I, 3 × LC II, 2 × LC III and 10 × VS injuries. The mean Injury Severity Score (ISS) was 32 (SD ± 17) and the mean inpatient stay was 29 days (SD ± 16 days; positive correlation p ≤ 0.001). Radiological implant loosening occurred in 52 patients. 14 patients required treatment for severe complications. The leading reason for revision surgery was implant failure (n = 8). Conclusion These observations suggest that radiological signs of implant loosening are commonly observed but rarely the reason for revision surgery. Complete implant failures, however; occur mainly within the first postoperative weeks and require early revision. A timely clarification by additional X-ray imaging should be carried out if this is suspected.

Background Motorcycle accidents account for a large fraction of the patients with polytrauma treated in German hospitals. Clinical experience indicates that an in- creasing number of older motorcyclists are having accidents. We studied whether such individuals are subject to higher mortality and longer hospital stays. Methods We retrospectively evaluated data from the Traumaregister DGU® (TR- DGU) concerning all patients (n = 13 850) who were registered in the TR-DGU as having sustained trauma in a motorcycle accident from 2002 to 2015 and who had an Injury Severity Score (ISS) greater than 8. The patients were divided into four age groups for further study. Results Despite a nearly identical severity of anatomical injury according to the ISS, persons sustaining trauma in motorcycle accidents who were over 65 years of age (n = 892) needed longer and more intensive treatment than their younger counter- parts. They were invasively ventilated for a longer time (+ 1.2 days), kept for a longer time on the intensive care unit (+ 1.7 days), and stayed in the hospital three days longer. These older persons injured in motorcycle accidents had a disproportionate mortality in comparison to other polytrauma patients and a significantly elevated mor- tality in comparison to their younger counterparts-15.8%, compared to 7.2% among patients aged 45 to 64. Older trauma patients are more likely than younger ones to develop lethal complications in the later course of their hospitalization, while younger trauma patients who die generally do so as a direct result of the traumatic injury. Conclusion Patients over age 65 who sustain trauma in motorcycle accidents have a higher mortality, a longer duration of ventilation, and longer stays in the intensive care unit and in the hospital overall than their younger counterparts. These patients present a special challenge to the treating medical team.

BACKGROUND Treatment of distal intra-articular humerus fractures is still a technical challenge. Until now, little is known about the regain of strength and elbow agility after surgical treatment of these fractures. Due to small collectives there is only limited data. OBJECTIVE Investigation of regained strength and elbow agility in patients with intra-articular distal humerus fractures. METHODS A total of 28 patients were treated with distal intra-articular humerus fractures followed up for an average period of 62.3 months. The following parameters were examined: Arthrosis, heterotopic ossification, functional outcome (MEPS, DASH score, LES) and isometric strength of the elbow in extension and flexion was tested in 30∘, 60∘ and 90∘ in a custom-made positioning device. RESULTS There was a high complication rate with 32%. At the 60th month post injury, range of motion (ROM) of the elbow was 114∘ with a reduction of 32∘ compared to the contralateral uninjured side (p< 0.001). The highest reduction was seen in extension with an average loss of 16∘ (p< 0.001). Loss of motion correlated with the fracture severity regarding the AO-classification (r= 0.54, p< 0.01). The average regained muscle force was 81.5% in flexion and 92% in extension in comparison to the contralateral healthy side. Patients over 60 years had less range of motion and inferior results in the DASH score compared to younger patients. CONCLUSIONS Functional impairment in terms of reduced ROM and muscle force is a common complication after distal intra-articular humerus fracture. Patients over 60 years have a higher deficit of motion in the injured elbow joint and an inferior clinical outcome. Superiority of modern angle-stable implants could yet not be shown.

BACKGROUND: Critically elevated compartment pressures after complicated tibial fractures may result in fibrosis and therefore scarring of muscles with impaired function. Several studies have shown a relationship between angiogenesis and more effective muscle regeneration. Cysteine-rich angiogenic inducer 61 (CYR61) is associated with angiogenesis but it is not clear whether it would restore muscle force, reduce scarring or improve angiogenesis after acute musculoskeletal trauma. OBJECTIVE: We researched whether local application of CYR61 (1) restores muscle force, (2) reduces scar tissue formation, and (3) improves angiogenesis. METHODS: We generated acute soft tissue trauma with temporary ischemia and increased compartment pressure in 22 rabbits and shortened the limbs to simulate surgical fracture debridement. In the test group, a CYR61-coated collagen matrix was applied locally around the osteotomy site. After 10 days of limb shortening, gradual distraction of 0.5 mm per 12 hours was performed to restore the original length. Muscle force was measured before trauma and on every fifth day after trauma. Forty days after trauma we euthanized the animals and histologically determined the percentage of connective and muscle tissue. Immunohistology was performed to analyze angiogenesis. RESULTS: Recovery of preinjury muscle strength was significantly greater in the CYR61 group (2.8 N; 88%) as compared to the control (1.8 N; 53%) with a moderate reduction of connective tissue (9.9% vs. 8.5%). Immunohistochemical staining showed that blood vessel formation increased significantly (trauma vs. control 38.75 ± 27.45 mm2 vs. 24.16 ± 19.81 mm2). CONCLUSIONS: Local application of CYR61 may improve restoration of muscle force and accelerate muscle force recovery by improving angiogenesis and moderately reducing connective tissue.

There are different treatment options for extensor hallucis longus injuries. For primary repair, the end-to-end suture is recommended. The treatment of reruptures or tendon defects is challenging, and a wide range of procedures have been used in this regard, including primary and secondary repairs with and without auto- and allografts. To overcome the disadvantages of second-site morbidity and to achieve high primary stability, we demonstrate a technique using a local tendon graft in combination with a strong Pulvertaft suture technique in a case of rerupture of the extensor hallucis longus tendon.

Prospective analysis of patients who underwent minimally invasive posterior instrumentation. The purpose of this study was to evaluate the safety of minimally invasive pedicle screw placement in patients with unstable thoracic and lumbar spine fractures using the conventional fluoroscopy technique. Although wound infection, haematoma, and new neurological deficit due to screw malplacement remain a common source of morbidity, estimates of their rates of occurrence remain relatively limited. 2052 percutaneous pedicle screws in 433 consecutive patients were evaluated. The accuracy of pedicle screw placement was based on evaluation of axial 3-mm slice computed tomography scans. Morbidity and mortality data were collected prospectively. A total of 2029 of 2052 screws (99%) had a good or excellent position. 5 screws (0.2%) showed a higher grade violation of the medial pedicle wall. Seven patients (1.8%) needed revision due to screw malposition (3 pat.), surgical site infection, postoperative haematoma, implant failure (2 pat.), and technical difficulties. Minimally invasive transpedicular instrumentation is an accurate, reliable, and safe procedure to treat thoracic and lumbar spine fractures.

Zusammenfassung Die Hand gehort aufgrund ihrer exponierten Position und des aktiven Einsatzes im Ballsport zu den haufig verletzten Korperregionen. Im professionellen Sport entfallen sportartubergreifend bis zu 10% der Verletzungen auf den Handbereich. Neben den auch bei anderen Sportarten anzutreffenden Verletzungsmechanismen wie Sturz oder Zugbelastung kommt beim Ballsport zusatzlich die Verletzung durch axiale Stauchungstraumen durch den Ball zum Tragen. In dieser Ubersichtsarbeit liegt der Fokus auf den ballspezifischen Verletzungen, die von distal nach proximal aufgearbeitet werden. Von entscheidender Bedeutung ist nicht nur die Wahl der adaquaten Therapie – konservativ oder operativ – sondern auch, wann der (Profi-) Sportler zuruck zum Sport kehren kann.

The purpose of this study was to compare two sutures; a knotted polydioxane with a knotless barbed in a 4-strand Kirchmayr-Kessler suture technique. Human flexor digitorum tendons were separated into four groups. Group 1 – polydioxane; Group 2 - barbed suture; Group 3 and 4 – same as group 1 and 2 with an additional peripheral running suture. In each group the repaired tendons were subjected to linear and cyclical loads. No difference in maximum tensile strength after linear and cyclical force could be detected between the knotted polydioxane suture and the knotless barbed suture. On linear force tests an additional circumferential repair increased the maximum tensile strength of both sutures. Cyclical force loading did not lead to a reduction of maximum strength. Following linear and cyclical loading the 4-strand barbed suture achieved maximum tensile strengths comparable to the 4-strand repair using the polydioxane suture. Barbed suture repair may offer the advantage of knotless suture techniques.

Treating acetabular fractures with open reduction and internal fixation (ORIF) may lead to poorer outcomes in older patients. As data for osteoporotic acetabular fractures are limited, we compared primary total hip replacement (THR) with ORIF for treating osteoporotic acetabular fractures, including those with posterior column instability. All patients with osteoporotic acetabular fractures, treated with ORIF or primary THR from 2005 to 2015, were assessed retrospectively for clinical and radiologic outcomes. Indication criteria for selecting primary THR were evaluated. Twenty-three patients were treated with ORIF and nine with primary THR (performed with an anti-protrusion cage). If the posterior column was unstable, THR was combined with posterior column bridge plating. Indications for THR were the presence of a comminuted fracture pattern, approach-related risk factors for ORIF, and mobilization issues. Biomechanical reconstruction was acceptable with THR. Acetabular component loosening was observed only once. Secondary THR was indicated in 45 % of the ORIF cases. ORIF for acetabular fracture in older patients is unsatisfactory and may be even worse for osteoporotic fractures. Immobilization for long-term restricted weight bearing after ORIF is hazardous. Primary THR may enable early post-operative full weight-bearing. Various techniques, affording fixation of the unstable acetabular columns, are described. In THR with an anti-protrusion cage, only dorsal column instability needs to be addressed. The described technique achieved sufficient acetabular component stability. Primary THR with an anti-protrusion cage is an advantageous option to ORIF and should be strongly considered for osteoporotic acetabular fractures.

Je nach Verletzung primare Bandnaht, transossare Naht, Refixation mittels Knochenanker, Kirschner-Draht- oder Schraubenosteosynthese bei knochernen Ausrissen. Bandplastik bei chronischer Instabilitat oder alterer Verletzung. Gipsschiene mit Einschluss des MP-Gelenks bis zur Abschwellung; Daumenbandage fur 6 Wochen; fur 3 Monate Mobilisation ohne forcierte Radialdeviation im MP-Gelenk. Vollstandige Bandstabilitat 3 Monate postoperativ bei 34 Patienten mit traumatischer Ruptur des ulnaren Kollateralbands des Daumen-MP-Gelenks.

Ziel der Operation ist es, minimal-invasiv eine ubungsstabile Situation zu erreichen und somit die Immobilisationszeit und Dauer der Arbeitsunfahigkeit deutlich zu reduzieren. Skaphoidfrakturen Typ A2, B1 und B2 nach Herbert, die keine bis minimale Dislokationen aufweisen. Wunsch des Patienten nach einer nichtimmobilisierenden, fruhfunktionellen Nachbehandlung. Relativ: Ausgepragte Dislokation der Fraktur, Skaphoidzysten, sehr proximal gelegene Frakturen, Begleitverletzungen des Handgelenks. Absolut: Pseudarthrose, Luxationsfraktur. Minimal-invasive perkutane Schraubenosteosynthese mittels kanulierter Doppelgewindeschraube. Postoperativ bis zum Abklingen der Schwellung und der Schmerzen Ruhigstellung im gut gepolsterten Unterarmgipsverband mit Daumeneinschluss fur 1–3 Wochen, dann Beginn aktiver Beubung unter krankengymnastischer Anleitung, keine schwere Belastung der Hand fur 6 Wochen postoperativ. Im Zeitraum 2005 bis 2011 wurden 70 Patienten mit einer nicht- oder minimal-dislozierten Skaphoidfraktur in unserer Klinik mittels perkutaner Verschraubung therapiert. Zu regelmasigen Nachuntersuchungen erschienen 57 Patienten (81 %). Eine nichtverheilte Fraktur wurde bei 4 Patienten (5,7 % von der Gesamtzahl) 6 Monate postoperativ nachgewiesen. Bei einem Patienten war postoperativ bei Schraubenuberlange der Wechsel indiziert. Keiner der Patienten entwickelte eine postoperative Infektion, ein Hamatom oder ein komplexes regionales Schmerzsyndrom. Als mogliche Risikofaktoren bei den nichtverheilten Frakturen wurden Rauchen sowie die zu fruhe schwere Belastung der Hand sofort nach Schienenabnahme angesehen.

Purpose Fascia flaps are a preferred method to reconstruct the soft tissue envelope of the hand when a thin and pliable flap is required to cover exposed tendons. The aim of this study was to report on our experience with the fascia-only reversed posterior interosseous artery flap. Contrary to commonly used fascia flaps, this flap does not require free tissue transfer. Methods In this retrospective review, 5 patients were identified, each of whom underwent soft tissue reconstruction with a reversed posterior interosseous artery fascia flap. The operative technique is similar to the harvest of a fasciocutaneous flap except that only the fascia is harvested through a straight incision. Results No flap loss occurred. In 2 patients a distal wound dehiscence occurred, which healed by secondary intention. No venous congestion or iatrogenic lesion of the motor nerves to the extensor muscles was encountered. Conclusions The fascia-only reverse posterior interosseous artery flap represents a locally available, pedicled option. With regard to the quality of the transferred tissue, this flap is comparable to the temporalis fascia flap. Major advantages are that the donor site is confined to the ipsilateral extremity and microsurgery is not required. Contrary to the fasciocutaneous version, no skin graft has to be applied to the donor site, which improves cosmesis. We consider this flap a worthwhile alternative to other fascia flaps. Surgical adhesives are useful supplements in surgery, but their benefit in tendon repair is uncertain. The purpose of this study was to evaluate the effect of BioGlue™ on strength of flexor tendon repair. A total of 60 porcine flexor tendons were divided into three groups. In group one, a conventional core and peripheral suture repair was used. In group two, a core suture and BioGlue™ were used. In group three, a conventional core and peripheral suture repair and BioGlue™ were used. We performed static and cyclic axial load testing and measured diameter of the repair site. We found that BioGlue™ did not improve the tensile strength when added to a core and peripheral suture and that there was an increase in bulk at the repair site. We conclude that BioGlue™ application cannot replace a peripheral suture as tensile strength significantly decreases without a peripheral suture, and it does not benefit a tendon already repaired with a core and peripheral suture. Introduction: Although rare, subtalar dislocations are severe injuries with long-term alterations such as clinical dysfunction or painful posttraumatic arthritis. The objective of this study was to investigate long-term morphological changes of subtalar dislocations and to correlate them to clinical function. Based on the conclusions, suggestions for therapy guidelines were made in order to improve the functional outcome. Methods: Twenty-two patients (12 with a medial, 9 with a lateral and 1 with an anterior dislocation) were re-examined with an average follow-up time of 10 years. Radiological results of a computer tomography examination of the hindfoot were related to the clinical outcome, and both were discussed in the context of further parameters, such as additional injuries, time until reduction, and post-reduction treatment. Results: Additional injuries were found to affect the clinical outcome and/or the radiological changes. Predictive factors for limited range of motion were severe skin trauma and traumatic brain injuries. Factors that predicted both poor clinical function and clear signs of arthritis included complex talus, ankle and calcaneus fractures, long time until reduction, and infection. Conclusions: In many cases, radiological results can be correlated to clinical outcomes. However, the modulating effects of additional injuries should be considered when planning therapy. In cases with additional fractures of the talus and the calcaneus, suffering pain may be reduced by an early arthrodesis of the talo-cal-canear joint. In cases involving a long stay on an intensive care unit, early functional treatment by passive motion should be discussed in special cases to improve the clinical outcome.

Aims and Objectives: Knee-luxations are severe and rare injuries. Within the last years there is an incline on low velocity trauma leading to a nee-luxation. The luxated knee should be reduced immediately to minimize collateral damage to soft-tissue structures, nerves, vessels, etc. Materials and Methods: We report about a 87-year old man who was brought to our emergency room after he fell out of his bed. He showed a to the lateral side luxated tibial-head, which could not be reduced in analog sedation. The foot pulses could be measured with the Doppler, the patient was under massive pain so we decided to perform an immediate operative intervention. Even under maximum relaxtion the knee stayed in medial luxation. Results: The open reposition showed the destruction of all medial structures including the subcutaneous fat. The medial condyle was completely uncovered. The sartorious fascia, the ruptured M. vastus medialis and fibres of the medial collateral ligament have been trapped in the medial tibial joint line. The reposition was made possible by incision of the sartorious fascia. The tibially ruptured acl was refixated in a modified canula technique, the medial structures were adapted with sutures and additionally stabilized with ligament bracing for the medial collateral ligament. Postoperatively, the knee was immobilized in an external fixation for two week. Afterward we started with functional rehabilitation. The postoperatively obtained CT-angiography showed now traumatic vascular damage. This case is completely foto-documented. Conclusion: The irreducible knee-luxations are rare complications. So far there are 4 cases published. In all cases there has been an interposition of the musculus vastus medialis an the surrounding structures. An immediate open intervenvtion to reposition the knee has to be performed to avoid severe damage to. Especially the elderly patient benefits from a single-staged procedure to reconstruct the ruptured ligaments. To realize the best outcome early functional rehabilitation under close supervision of the surgeon is necessary.

Purpose.To analyse the biomechanical characteristics of locking plates under cyclic loading compared to a nonlocking plate in a diaphyseal metacarpal fracture.Methods.Oblique diaphyseal shaft fractures in porcine metacarpal bones were created in a biomechanical fracture model. An anatomical reduction and stabilization with a nonlocking and a comparable locking plate in mono- or bicortical screw fixation followed. Under cyclic loading, the displacement, and in subsequent load-to-failure tests, the maximum load and stiffness were measured.Results.For the monocortical screw fixation of the locking plate, a similar displacement, maximum load, and stiffness could be demonstrated compared to the bicortical screw fixation of the nonlocking plate.Conclusions.Locking plates in monocortical configuration may function as a useful alternative to the currently common treatment with bicortical fixations. Thereby, irritation of the flexor tendons would be avoided without compromising the stability, thus enabling the necessary early functional rehabilitation.

Operationsziel Problemwunden im distalen Drittel der unteren Extremitat sind eine Herausforderung fur die operative Therapie. Die Technik der distal gestielten adipofaszialen A.-suralis-Lappenplastik ist eine einfache und sichere Modifikation der klassischen A.-suralis-Insellappenplastik und ermoglicht somit eine einfache Rekonstruktion innerhalb dieser Problemzone. Das chirurgische Ziel besteht in einer Weichteilrekonstruktion mit lokalem belastungs- und einpassungsfahigem Gewebe in Kombination mit einer Hauttransplantation bei geringer Hebedefektmorbiditat.

Plastic deformation of sutures creates an irreversible extension during load. To test our hypothesis that such plastic deformation causes gap formation after tendon repair, we determined the plasticity of five different suture materials commonly used in tendon surgery. Prolene, Polydioxanone (PDS), Ethibond, Vicryl, and FiberWire sutures were biomechanically tested to determine their offset yield strength, initial extension, creep, relaxed elongation, peak-to-peak displacement, stiffness, and maximum tensile strength under static, constant, and cyclic loading. In addition, 35 porcine hindlimb tendons were used to evaluate gap formation and the biomechanical behavior of the suture materials after tendon repair. Prolene had a low offset yield strength and high initial extensions at 30, 60, and 90N combined with relatively large creep, relaxed elongation, and peak-to-peak displacement. Aside from the low maximum tensile strength and stiffness, these parameters indicate an early plastic deformation during loading. The material properties of PDS were generally better for suturing than those of Prolene, but no difference was found in offset yield strength or initial extension. In contrast to the monofilament materials Prolene and PDS, the braided Ethibond, Vicryl, and FiberWire materials showed significantly less plastic deformation. The lowest amount of plastic deformation was found in the FiberWire and the results for Ethibond and Vicryl were equal. Gap formation occurred at the lowest tension force in the Prolene group, but only FiberWire required a significantly larger tension force to produce gapping at the repair site, indicating a higher resistance to gap formation. The results of this study show that plastic deformation occurs at a lower tension force in the monofilament sutures Prolene and PDS than in the braided materials Ethibond, Vicryl, and FiberWire. After tendon repair, FiberWire likely prevents gap formation, whereas Prolene induces gapping through low-tension plastic deformation. Therefore, plastic deformation should be considered when selecting suture materials for tendon repair surgery.

Axial burst fractures of the distal tibia are challenging to treat and often lead to restricted function of the lower limb. The purpose of this study was to investigate the clinical outcome and changes in gait pattern in such patients.Thirty-five patients in a level 1 trauma centre were followed up clinically and by gait analysis. The American Orthopaedic Foot and Ankle Society (AOFAS), visual analogue scale (VAS) foot and ankle scale and Phillips scores were applied. Dynamic pedography (emed-M; Novel, Germany) with analyses of load, pressure and force-time integral were undertaken to investigate possible changes in gait pattern.Mean follow-up was 50 (19-100) months. Mean AOFAS, VAS foot and ankle and Phillips scores were 65, 63 and 55 points, respectively. There were clear correlations between fracture severity in the Arbeitsgemeinschaft für Osteosynthesefragen (AO) classification and functional outcome in AOFAS (-0.63; p 0.01), VAS foot and ankle scale (-0.56; p 0.01) and Phillips (-0.64; p 0.01) scores. There was a high correlation of 0.74 (p 0.01) between the severity of the injury in the AO-classification and onset of post-traumatic arthrosis. Dynamic pedography revealed lesser load bearing for the total foot, medial foot, heel, first metatarsal and medial forefoot for the affected limb, and increased load bearing was seen in the lateral midfoot region.Fractures of the tibial pilon lead to restricted function of the lower limb. Clinical outcome correlates with fracture severity in the AO classification, the onset of post-traumatic arthrosis and changes in gait patterns.

Complicated tibial fractures with severe soft tissue trauma are challenging to treat. Frequently associated acute compartment syndrome can result in scarring of muscles with impaired function. Several studies have shown a relationship between angiogenesis and more effective muscle regeneration. Vascular endothelial growth factor (VEGF) is associated with angiogenesis but it is not clear whether it would restore muscle force, reduce scarring, and aid in muscle regeneration after acute musculoskeletal trauma.Therefore, we asked whether local application of VEGF (1) restores muscle force, (2) reduces scar tissue formation, and (3) regenerates muscle tissue.We generated acute soft tissue trauma with increased compartment pressure in 22 rabbits and shortened the limbs to simulate fracture débridement. In the test group (n = 11), a VEGF-coated collagen matrix was applied locally around the osteotomy site. After 10 days of limb shortening, gradual distraction of 0.5 mm per 12 hours was performed to restore the original length. Muscle force was measured before trauma and on every fifth day after trauma. Forty days after shortening we euthanized the animals and histologically determined the percentage of connective and muscle tissue.Recovery of preinjury muscle strength was greater in the VEGF group (2.4 N; 73%) when compared with the control (1.8 N; 53%) with less connective and more muscle tissue in the VEGF group. The recovery of force was related to the percentage of connective tissue versus muscle fibers.Local application of VEGF may improve restoration of muscle force by reducing connective tissue and increasing the relative amount of muscle fibers.VEGF may be useful to improve skeletal muscle repair by modulating muscle tissue regeneration and fibrosis reduction after acute trauma.

Objective Fixation of ankle fractures in elderly patients is associated with reduced stability conditioned by osteoporotic bone. Therefore, fixation with implants providing improved biomechanical features could allow a more functional treatment, diminish implant failure and avoid consequences of immobilisation. Materials and methods In the actual study, we evaluated a lateral conventional contoured plate with a locking contoured plate stabilising experimentally induced distal fibular fractures in human cadavers from elderly. Ankle fractures were induced by the supination-external rotation mechanism according to Lauge-Hansen. Stage II fractures (AO 44-B1) were fixed with the 2 contoured plates and a torque to failure test was performed. Bone mineral density (BMD) was measured by quantitative computed tomography to correlate the parameters of the biomechanical experiments with bone quality. Results The locking plate showed a higher torque to failure, angle at failure, and maximal torque compared to the conventional plate. In contrast to the nonlocking system, fixation with the locking plate was independent of BMD. Conclusion Fixation of distal fibular fractures in osteoporotic bone with the contoured locking plate may be advantageous as compared to the nonlocking contoured plate. The locking plate with improved biomechanical attributes may allow a more functional treatment, reduce complications and consequences of immobilisation.

Reconstruction of the tip of the thumb using a neurovascular flap. Transverse defects of the thumb’s tip or large defects of the palmar pulp (max. 2.0–2.5 cm) with exposure of bone and/or tendons. Extensive crush injury, heavy wound contamination, circulatory disorders, acute infection, very large defects (> 2.0–2.5 cm finger length), circumferential soft tissue defects, and previous defects/operations (relative). Supine position, hand supinated, tourniquet, loupe magnification. Mid-lateral incisions along both sides of the finger running from the defect to the interphalangeal joint (small defect) or proceeding further proximally. Careful elevation of the flap including both neurovascular bundles leaving dorsal branches of the bundles (long fingers only) and the flexor tendon sheath intact. Suture of the flap in either flexion position (i.e., advancement flap) (Moberg) or by creating an island-flap through an additional transverse skin incision along the flap’s base (O’Brien). Finally, closure of the defect at the flap’s base using a full thickness skin graft, Z plasty, or V-Y plasty. Plaster cast (finger slightly flexed) for 2 weeks. Reliable method. Good functional results with good sensibility and only minor reduction in range of motion.

Das kompetitive Auflisten von Leistungen universitarer Einrichtungen auf Bundeslandebene oder deutschlandweit als Benchmarking ist heute ein ubliches Vorgehen. Ziel der folgenden Zusammenstellung ist es zu uberprufen, ob es prinzipiell moglich ist, nach den vorhandenen Kennzahlen eine derartige Auflistung im Sinne eines F&L-Ranking (Forschung und Lehre) zu machen. Zugrunde gelegt wurden alle verfugbaren Daten aus dem Bereich Forschung und Lehre. Weder betriebswirtschaftliche Zahlen noch Drittmittelakquise liesen einen direkten Vergleich der Einrichtungen zu, wobei teilweise der Modus der internen Mittelvergabe nicht bekannt war. Als Vergleichsdaten waren nur wie ublich der Impact-Faktor nutzbar oder Faktoren wie durchschnittlicher Impact je Publikation bzw. eingeworbene Drittmittel ohne Gewichtung je m2 Forschungsflache. Ein universitares Benchmarking unfallchirurgischer Abteilungen ist demnach auserhalb der Bundeslandebene nicht moglich.

Die Kenntnis uber den Entstehungsmechanismus und das detaillierte Ausmas einer Sprunggelenkverletzung bedingen deren adaquate Behandlung. Supinations-Eversions- (SE-) Verletzungen wurden in 29 Unterschenkelpraparaten induziert und deren Knochendichte bestimmt. Die Literaturrecherche umfasst die experimentellen Arbeiten uber diese Verletzungsentitat. Wir konnten das Stadium II der Verletzung in 42% induzieren; die Inzidenz konnte durch ein laterales Widerlager erhoht werden. In 50% blieb die Syndesmose bei induzierter Fibulafraktur auf Syndesmosenhohe intakt. Das Stadium IV wurde in 25% beobachtet. Die insgesamt geringe Knochendichte zeigte eine positive Korrelation zum Entstehungswinkel der Fibulafraktur. Wir haben SE-Verletzungen in osteoporotischen Unterschenkelpraparaten induziert. Im Stadium II der Verletzung war die vordere untere tibiofibulare Syndesmose in 50% der Experimente intakt. Innenknochelfrakturen konnten erstmals nach Lauge-Hansen und Michelson reproduziert werden. Etablierte Frakturmodelle liefern Erkenntnisse uber die Pathogenese der Verletzung und sind fur die biomechanische Implantattestung essentiell.

Purpose The reconstruction of tendon defects is challenging. The palmaris longus and plantaris tendon are generally considered best for tendon grafting. Only a few studies have examined whether these tendons, when present, meet criteria for successful grafting. The purpose of this study was to evaluate these tendons in regard to adequacy as tendon grafts. Methods To evaluate adequacy for grafting, the palmaris longus and plantaris tendons were harvested from 92 arms and legs of 46 cadavers. Macroscopic evaluation and measurements concerning presence, length, and diameter of the tendons were obtained. Criteria for adequacy were a minimum length of 15 cm with diameter of 3 mm or, alternatively, 30 cm with a diameter of 1.5 mm. Results The palmaris longus tendon was present bilaterally in 36 cases and was absent bilaterally in 4 cases. The plantaris tendon was present bilaterally in 38 cases and absent bilaterally in 4 cases. In 29 cadavers, the palmaris longus tendon did not meet the criteria to be used as a tendon graft. Only in 8 cases were the tendons satisfactory for grafting bilaterally. The plantaris tendon met criteria for grafting in 20 cases bilaterally. In 17 cases, the tendons were considered inadequate bilaterally. Conclusions Despite their presence, the palmaris longus and plantaris tendons are adequate for grafting less often than previously thought. In less than 50%, the tendons, although present, would serve as useful grafts. Clinical Relevance Our findings underscore the importance of choosing a second donor site before surgery in case the primarily selected tendon is not found to be suitable.

Barbed suture material for tendon repair opens up the possibility of a knotless reconstruction due to an increased suture-tendon interaction. The aim of this study was to compare the tensile strength of a knotted technique with a monofilament polydioxane suture to that of a knotless technique with a barbed suture material, by using a multistrand, modified Kirchmayr-Kessler tenorrhaphy.Sixty human flexor digitorum tendons were randomized into 4 groups. A modified, knotted, multistrand Kirchmayr-Kessler technique with an absorbable, monofilament polydioxane suture was compared with a modified, knotless, multistrand Kirchmayr-Kessler technique with an absorbable, unidirectional barbed glycolic-carbonate suture. Tendons were distracted to failure. Mode of failure and load to failure were recorded.The knotless 2-strand Kirchmayr-Kessler barbed suture shows a significantly lower tensile strength than the knotted 2-strand polydioxane suture (p.001). The comparison of the maximum tensile strength of the knotless (glycolic-carbonate) technique with that of the knotted (polydioxane) 4-strand technique resulted in no significant difference in either technique utilized (p = .737). The tensile strength of the 4-strand technique was greater than that of the corresponding 2-strand technique (p.001).The 2-strand Kirchmayr-Kessler barbed suture proved to be insufficient and significantly weaker than the 2-strand polydioxane suture, and therefore it cannot be recommended. With the knotless 4-strand Kirchmayr-Kessler technique, the barbed suture material has the potential to be used in flexor tendon surgery, but it has no advantage over the 4-strand polydioxane suture.

Acute soft tissue trauma influences callus formation and fracture healing. Several studies showed a relationship between angiogenesis and bone formation during distraction osteogenesis. The purpose of this study was to investigate the influence of controlled release of vascular endothelial growth factor (VEGF) on callus formation in a limb-shortening distraction procedure after acute compartment syndrome. Acute soft tissue trauma with critical increased compartment pressure was generated in 22 rabbits, and the limb was shortened simulating fracture site debridement. In the test group (n = 11), a VEGF-coated collagen matrix was locally applied around the fracture, while no collagen was applied in the control group (n = 11). Following 10 days in limb shortening, a gradual distraction of 0.5 mm/12 h was performed using an external fixation device and followed up for 40 days. Osseous consolidation occurred in all animals. Average callus diameter (1.54 ± 0.8 vs. 1.27 ± 0.14 mm) and torsional strength (72% vs. 46% of normal) were significantly higher in the test versus the control group. Blood vessel formation increased with a significantly higher number of vessels (6.3 vs. 3.81/mm2) and larger cross-sectional area (>40 µm, 90.5% vs. 86%) in the test versus control group. The results showed that locally applied VEGF stimulates fracture healing after acute soft tissue trauma and might be an option for fracture treatment in cases with severe soft tissue damage. © 2011 Orthopaedic Research Society Published by Wiley Periodicals, Inc. Perforator flaps have been successfully used for reconstruction of pressure sores. Although V-Y advancement flaps approximate debrided wound edges, perforator-based propeller flaps allow rotation of healthy tissue into the defect. Perforator-based propeller flaps were planned in 13 patients. Seven pressure sores were over the sacrum, five over the ischial tuberosity, and one on the tip of the scapula. Three patients were paraplegic, six were bedridden, and five were ambulatory. In three patients, no perforators were found. In 10 patients, propeller flaps were transferred. In two patients, total flap necrosis occurred, which was reconstructed with local advancement flaps. In two cases, a wound dehiscence occurred and had to be revised. One hematoma required evacuation. No further complications were noted. No recurrence at the flap site occurred. Local perforator flaps allow closure of pressure sores without harvesting muscle. The propeller version has the added benefit of transferring tissue from a distant site, avoiding reapproximation of original wound edges. Twisting of the pedicle may cause torsion and venous obstruction. This can be avoided by dissecting a pedicle of at least 3 cm. Propeller flaps are a safe option for soft tissue reconstruction of pressure sores.

To evaluate a possible beneficial effect of low-energy nuclear magnetic resonance (NMR) on cartilage in moderate and severe posttraumatic osteoarthrosis in the rabbit using a macroscopic and a histological grading system.Following transection of the anterior cruciate ligament of both knees in 24 skeletally mature New Zealand White rabbits, we observed different stages of osteoarthrosis (OA) 6 and 12 weeks postoperatively. Animals were randomized into four groups: Group 1 (eight animals) was treated after 6 weeks by NMR (magnetic field: 20-40 G, interference field: 2.35 mT, 100 kHz; MBST Device, MedTec, Germany), with 1 h of treatment for seven consecutive days. Group 2 was treated in the same pattern after 12 weeks. The sham-operated groups 3 and 4 received no treatment. Seven days after the last treatment, OA was macroscopically graded and hyaline cartilage of the load bearing area was evaluated histologically according to the Mankin scale.Macroscopically, there was less OA in group 1 (p0.01), but did not reveal significance in group 2 (p = 0.11) compared to the sham groups. There was no significant difference in the Mankin score in both of the treated groups compared to the control groups (group 1: p = 0.36; group 2: p = 0.81).The results showed some beneficial macroscopic effect in mild OA with less macroscopic OA signs in the treated animals but without a histological effect in the Mankin scale. There was no effect found in the pattern later OA. On behalf of these results, NMR for the treatment of posttraumatic OA cannot be recommended at this point of time.

Introduction. Soft tissue defects exposing the Achilles tendon are challenging. Local perforator flaps represent a valuable option gaining increasing popularity. Despite preoperative planning an adequate perforator cannot always be found intraoperatively. The free peroneal artery perforator flap can serve as a back-up option limiting the donor site morbidity to the same extremity without sacrificing major vessels or nerves. Methods. Nine patients with soft tissue defects exposing the Achilles tendon were treated with local perforator flaps, seven were scheduled for 180° propeller flap coverage after Doppler-ultrasound examination. However, in two patients (22%) no adequate perforators were found intraoperatively. As the perforators for the free peroneal artery perforator flap were routinely mapped out, this flap was harvested for microsurgical reconstruction. Results. One patient with a 180° propeller flap developed a partial flap necrosis, another patient developed superficial epidermolysis, both requiring skingrafting. No complications were seen with free tissue transfer. Conclusion. Pedicled perforator flaps as propeller flaps add options to the armamentarium of microsurgeons. Despite thorough preoperative planning the surgeons must be prepared to perform a different method of reconstruction if inadequate vessels are encountered. To limit additional donor site morbidity, local options are preferred. The free peroneal artery perforator flap represents a good option as it matches the original tissue properties closely. The complication rate of propeller flaps in this series is tolerable. Propeller flaps should therefore be considered an alternative but not as a replacement of local fasciocutaneous flaps. © 2010 Wiley-Liss, Inc. Microsurgery 30:608–613, 2010.

Breast reduction by liposuction alone is an appealing technique that has failed to gain widespread acceptance. Despite numerous studies on liposuction, the majority of surgeons remain skeptical. This study aimed to review the indication and limitations of this procedure. Two groups of patients that qualify for this procedure can be defined. However, the aesthetic result will be pleasing only for young patients. For elderly patients, liposuction breast reduction will simply achieve a weight reduction of the breast without improving the shape. Universally good liposuction results for breast weight reduction and elevation of the nipple–areolar complex are reported. Improvement in breast shape and correction of ptosis cannot be achieved for elderly patients. Young patients with a preoperatively pleasing breast shape can expect a preservation of the shape with the benefit of minimal scarring. Liposuction breast reduction is appealing due to selective removal of fat, ease of the procedure, and minimal scarring. The main disadvantage is that a correction of shape and ptosis is not possible with liposuction, and only young patients can expect an aesthetically pleasing result. Elderly patients may benefit from faster recovery times, a less invasive procedure, and low costs. The application of a new technique to a cancer-prone organ represents a potentially serious medicolegal issue because follow-up imaging may be impaired and a possible spread of cancer cells cannot be ruled out. Despite its technical appeal, breast reduction by liposuction alone mandates a cautious approach.

Die perkutane kanulierte Verschraubung nichtdislozierter Schenkelhalsfrakturen ist eine etablierte Methode mit dem Vorteil der geringen Invasivitat insbesondere bei alteren Patienten mit haufig begleitenden Komorbiditaten. Die Frage nach der optimalen Technik ist nach wie vor nicht abschliesend geklart. Eine besonders schwere Komplikation ist das Auftreten einer subtrochantaren Femurfraktur, die in der Literatur noch sehr sparlich beschrieben ist. Aufgrund des aktuellen Auftretens dieser Komplikation in unserer Klinik haben wir unsere Patienten vom 01.04.2004 bis 30.09.2009 retrospektiv analysiert und eine Literaturrecherche durchgefuhrt mit besonderem Augenmerk auf die Schraubenkonfiguration und die Inzidenz subtrochantarer Femurfrakturen. In unserem Kollektiv von 35 Patienten >65 Jahre trat diese Komplikation in 2 Fallen (5,7%) auf, ohne dass ein adaquates Trauma vorlag.

The use of locking plates increases the primary load to failure, thereby reducing the rate of implant-related failure. The good clinical and biomechanical results of locking plates in long bones might be applicable to treatment of metacarpal fractures. The purpose of this study was to determine strength and stiffness of locking plates in a metacarpal fracture model with mono- and bicortical screw fixation in comparison to non-locking plate mono- and bicortical screw fixation, with both types of plates placed at the dorsal side of the bone.Fresh second metacarpals from domestic pigs (n=40) were randomized in 4 equal groups. Short, oblique, mid-shaft fractures were generated, using a standardized 3-point bending method. Fractures were plated with non-locking, titanium, 1-mm-thick monocortical (group 1, n=10) or bicortical (group 2, n =10) plates (Leibinger-Stryker; Stryker Corp, Freiburg, Germany). Newly designed locking titanium plates with the same width and thickness (Leibinger-Stryker) were used in the same manner for groups 3 (monocortical) and 4 (bicortical). The metacarpals were then tested to load to failure in a cantilever bending mode.Bicortical, non-locking fixation (group 2, 359 +/- 90 N) had a higher load to failure than monocortical non-locking fixation (group 1, 250 +/- 56 N) in testing the maximum load to failure (p.01). There was no significant difference in stiffness between group 1 (46 +/- 12 N/mm) and group 2 (56 +/- 21 N/mm). The difference in maximum load to failure between monocortical (group 3, 440 +/- 85N) and bicortical (group 4, 378 +/- 116 N) locking plate stabilization was not significant. Also, there was no significant difference in stiffness between monocortical (group 3, 83 +/- 35 N/mm) and bicortical locking plates (group 4, 70 +/- 31 N/mm). Comparing non-locking (group 1) and locking plates in a monocortical fixation technique (group 3) demonstrated significant differences in maximum load to failure (group 1, 250 +/- 56 N; group 3, 440 +/- 85 N) and stiffness (group 1, 46 +/- 12 N/mm; group 3, 83 +/- 35 N/mm). The stability of monocortical locking plates was stronger, although not statistically significant, than the non-locking bicortical plates (load to failure, 440 +/- 85 N vs 359 +/- 90 N; stiffness, 83 +/- 35 N/mm vs 56 +/- 21 N/mm).The new generation of locking plates can be used to achieve a higher stability for fixation of metacarpal fractures. Monocortical, stable fixation can minimize flexor tendon interference and probably reduce bone and soft tissue trauma.
