Inflow control of a rapidly expanding or ruptured femoral anastomotic "pseudoaneurysm" can be fraught with hazard. Occlusion of an anastomotic femoral aneurysm with a balloon catheter offers the surgeon a simple method of gaining inflow control prior to surgery. After achieving inflow control with the balloon catheter, the surgeon can incise the anastomotic aneurysm without significant blood loss, control back bleeding with balloon occlusion catheters, and with relative ease and safety repair or replace the anastomotic aneurysm as indicated. An illustrative case is presented.

During a six-year period, 47 patients underwent operation for ruptured abdominal aortic aneurysm (rAAA) with a mortality rate of 38% (18/47): of the 39 patients surviving the initial surgery, 9 (23%) developed ischemic colitis. All 9 patients demonstrated hyperdynamic cardiovascular changes consistent with sepsis: decreased systemic vascular resistance (SVR) (mean 852 dynes/sec/cm2) and increased cardiac index (mean 3.8 L/min/m2). This "septic picture" in the postoperative period, before the onset of any gastrointestinal (GI) symptoms (diarrhea with/without blood), led the authors to question bowel viability. Flexible sigmoid colonoscopy identified ischemic colitis in all patients and sigmoid colectomy was performed in 4; 3 of these patients survived. Of the 5 remaining patients managed nonoperatively, 3 survived. When hyperdynamic cardiovascular changes were recognized "early" (< forty-eight hours) and ischemic colitis was diagnosed (5 patients), all patients survived. However, when ischemic colitis was diagnosed "late" (> forty-eight hours) after operation in 4 patients, only 1 (25%) survived. In patients who suffer an rAAA and demonstrate cardiodynamic signs of sepsis (especially falling SVR) in the immediate postoperative period (< forty-eight hours), immediate bedside sigmoid colonoscopy to rule out ischemic colitis is warranted. Similarly, when patients develop "late" cardiodynamic signs of sepsis during recuperation from rAAA, even before the onset of GI symptoms, sigmoid colonoscopy should be performed. If severe ischemic colitis is documented, aggressive management with frequent colonoscopy and possibly sigmoid resection is indicated, for this may offer the only chance of survival in these catastrophically ill patients.

A 10 year retrospective study of 103 patients with amaurosis fugax was done. Sixty-two patients with symptoms of amaurosis fugax underwent arteriography, which demonstrated ulcerated carotid plaque in 36 and hemodynamically significant stenoses (greater than 75% diameter reduction) in 26. These 62 patients underwent carotid endarterectomy. The other 41 patients who had proven ulcerated plaque (33 patients) or hemodynamic stenoses (eight patients) were not treated surgically and served as a control series. No strokes or deaths occurred in the immediate postoperative period. Follow-up of the 62 operated patients extending to 10 years (mean 4.2 years), revealed one (1.6%) patient with recurrent amaurosis fugax symptoms, two (3.2%) with transient ischemic attacks, and one (1.6%) with a stroke in the operated hemisphere. In the nonoperated group, despite aspirin or warfarin treatment, four (9.7%) patients had ongoing amaurosis fugax symptoms, and two (4.8%) developed transient ischemic attacks that led to carotid endarterectomy. One (2.4%) other patient developed sudden, permanent monocular blindness, and two (4.8%) suffered hemispheric strokes, one of which was fatal. The cumulative morbidity (ongoing ocular or transient ischemic attack symptoms, perioperative and late stroke) in the operated group was 6.4% (four patients), while the cumulative morbidity in the nonoperated group was significantly higher at 21.9% (nine patients) (p = 0.02). When patients present with symptoms of amaurosis fugax and have demonstrable carotid bifurcation disease, carotid endarterectomy is recommended. Amaurosis fugax should be regarded as a harbinger of monocular blindness and stroke.

During a three-year period 51 patients with abdominal aortic aneurysm (AAA) underwent magnetic resonance imaging (MRI) computed tomography (CT) and aortography to evaluate the diagnostic accuracy and role of each study. Ct and MRI were 100% accurate in establishing the diagnosis of AAA, whereas aortography did not visualize the AAA in 3 patients nor the associated iliac aneurysms in 3 other patients. MRI and aortography were superior to CT in evaluating the celiac, superior mesenteric, and renal arteries. CT should be the screening method of choice to identify AAA, and when operation is contemplated, aortography will best demonstrate the mesenteric and renal anatomy. If aortography is deemed too hazardous, MRI will yield the most information and should then be the noninvasive test of choice.

Laser-assisted balloon angioplasty has been shown to be effective in the treatment of superficial femoral artery (SFA) occlusive disease. In order to avoid the inherent trauma of balloon angioplasty, 30 patients underwent laser angioplasty of the SFA as "sole therapy." Laser angioplasty was performed using a laser coupled to a 600 micron fiber and a 3.5 mm probe (22-28 watts). Fourteen patients had laser angioplasty for claudication and 16 for limb salvage. The initial success rate was 83% (25/30) (lesion length 2-14 cm). In follow-up extending to nine months, 84% (21/25) of laser angioplastied SFAs have remained patent. By avoiding the disruptive effect on the arterial wall architecture caused by balloon angioplasty, while at the same time ablating and vaporizing more atheromatous material with larger laser probes, the long-term patency rates of laser angioplasty as sole therapy may prove to be superior to those of laser- assisted balloon angioplasty.

Purpose: With 70 cm "cutter" valvulotomes for valvulotomy and an electronically steerable nitinol catheter to occlude venous tributaries with platinum coils, endovascular in situ saphenous vein (EISV) bypass can be safely perofrmed from within the saphenous vein. To determine whether EISV bypass could reduce hospital length of stay (LOS) and perioperative morbidity without compromising patency, another 53 EISV bypasses for limb salvage were performed. Methods: Tributary occlusion was accomplished with only fluoroscopic surveillance with a new, smaller, and more steerable silicone-tipped nitinol catheter. Results: Two (3.7%) wound complications occurred. The mean hospital LOS after operation was 4.2 days (range 2 to 29 days). All tributaries initially embolized remained occluded, and three "missed" arteriovenous fistulas were identified during follow-up extending to 15 month (mean 8.4 months). Eighty-eight percent (49 of 54) of phase II bypasses remained patent, whereas life-table analysis of all bypasses (phase I and II) was 77% (69/99) at 24 months follow-up (mean 13.6 months). By comparison, 41 infrainguinal saphenous vein in situ bypasses with "classic" open techniques were performed concurrently. The mean postoperative LOS was 11.6 days (range 4 to 42 days), wound complications occurred in 24% (10) of patients, and two "missed" arteriovenous fistulas were identified during follow-up. Eighty-three percent (34 of 41) of bypasses remain patent at 24 months follow-up (mean 16.2 months). Conclusions: If EISV bypass long-term patency rates remain similar to classic in situ bypass patency results, the additional benefits of decreased hospital LOS, reduced wound-related complications, shortened recuperation, and therefore increased health care savings gives this endovascular technique strong consideration as the possible future operation for infrainguinal saphenous veins in situ bypass. Penetrating arterial injuries can result in the formation of a traumatic false aneurysm or an arteriovenous fistula. Traumatic arteriovenous fistulas of the mesenteric circulation are extremely rare, with only 15 operated cases reported in the English language literature that involved the superior mesenteric artery and vein. Although surgical intervention has been considered the most successful method to treat traumatic mesenteric arteriovenous fistulas, percutaneous transcatheter embolization has been occasionally advocated in the management of small iatrogenic fistulas. We report a case of a gunshot wound patient who had an 8 cm abdominal aortic false aneurysm, as well as a high-flow arteriovenous communication between the superior mesenteric artery and vein, which were successfully treated by a combination of aorto-superior mesenteric bypass and postoperative percutaneous transcatheter embolization. A review of the literature is also included.

To evaluate the benefits and disadvantages of autologous intraoperative transfusion during major aortic reconstructive procedures, we retrospectively studied 50 patients who had major aortic revascularization procedures without the use of autologous transfusion devices (group 1) and prospectively evaluated a second 50-patient cohort having similar procedures, but with the use of the autologous transfusion device for salvaging and reinfusing lost blood. Both groups were assessed for preoperative risk factors and postoperative complications. We found a somewhat lower morbidity in the autotransfusion group and more complete replacement of blood loss. Autologous transfusion accounted for approximately 75% of all transfused blood in group 2, tremendously reducing blood bank requirements. In addition to reduction of immediate postoperative morbidity, further risks associated with homologous transfusion such as hemolysis, posttransfusion hepatitis, transfusion-related acute lung injury, anaphylaxis, and transfusion-transmitted acquired immunodeficiency syndrome (AIDS) were markedly reduced or eliminated.

To evaluate the role of carotid endarterectomy (CE) in patients 80 years and older an 8-year study of 172 nonrandomized cases of octogenarians with cerebrovascular disease was done. Ninety octogenarians underwent CE whereas 82 octogenarians, with arteriographically established carotid artery disease, were not operated on and served as a control series. The stroke rate after CE was 6%. In follow-up extending to 8 years the late stroke rate was only 2%, whereas the cumulative long-term stroke rate in the nonoperated group was 16%. These late strokes were appropriate to the side of the arteriographically demonstrated disease. In 1008 nonoctogenarians who underwent CE during this same time interval, the stroke rate after CE was 2% and the mortality rate was 0.6%. In the octogenarian population, however, the mortality rate after stroke was an alarming 40% in the operated group and 62% in the nonoperated control group. Arteriographic flow-limiting (>75% stenosis) intracranial occlusive disease was identified in 53% of the octogenarians undergoing operation and in all patients who suffered a postoperative neurologic deficit. This incidence of severe intracranial disease was nearly five times that of the nonoctogenarian patients undergoing CE. Although the stroke rate after CE in the octogenarian patient was 6%, the late stroke rate was only 2% compared with the cumulative stroke rate of 16% in the nonoperated octogenarian patients. Severe intracranial occlusive disease and, therefore, flow deprivation may play a more significant role as a cause of postoperative deficits than in younger patients, but CE is appropriate for selected octogenarians on the basis of physiologic rather than chronologic age. A fluid-filled oculoplethysmography (OPG) was used to assess carotid artery flow in 50 patients with symptoms of cerebral ischemia. Based on positioned OPG and angiographic studies, 20 patients had angioplastic procedures. At operation, positional electromagnetic flow measurements were obtained to confirm the preoperative noninvasive findings. Electromagnetic flow reductions were identified in 18 patients with positive preoperative positional OPGs. A high correlation between noninvasive testing data and intraoperative flow measurements suggests that positional OPG testing is helpful in identifying hemodynamically significant kinking of the internal carotid artery.

The benefit of carotid endarterectomy (CE) in preventing recurrent stroke and improving survival in the patient who has sustained a reversible ischemic neurologic deficit (RIND) or stroke is still controversial. To determine the long-term benefits and value of CE in these patients, a 10-year review of 253 patients who suffered a RIND or stroke was conducted. All patients had CT brain scans, as well as arch, extracranial, and intracranial arteriography; any patients without demonstrated carotid bifurcation disease were excluded from the study. On the basis of clinical symptoms and CT scan findings, 66 patients were categorized as having sustained a RIND and 187 a stroke. One hundred fifty-one patients who suffered a RIND or stroke had CE, whereas 102 patients with RIND or stroke did not have CE and served as a control group. All endarterectomies were performed with a temporary indwelling shunt. Postoperative complications included two deaths (1%), six strokes (4%), and 10 transient neurologic deficits (7%). • The recent utilization of the axillo-axillary bypass graft for revascularization of proximal subclavian or innominate artery occlusions has proved to be a successful operation, technically simple with a low operative morbidity. We have reviewed the reported experience to date and have added our experience of nine additional cases. The procedure has produced excellent results in all patients, with no evidence of postoperative "steal" from the donor limb. Preoperative hemodynamic defects were corrected. Our experience has led us to the conclusion that this simpler and safer extrathoracic approach should be considered as the procedure of choice when arterial lesions allow for its selection and use. Many patients who suffer a massive pulmonary embolus die despite emergent therapy. In these desperately ill patients an aggressive, combined method of management was initiated to improve their chances and quality of survival. During a 5-year period 10 patients were treated with (1) low-dose topical, intrapulmonary thrombolytic therapy to dissolve thrombus, (streptokinase or urokinase); (2) anticoagulation to prevent thrombus propagation (heparin); and (3) the simultaneous insertion of a Greenfield filter to prevent the early, recurrent, and therefore potentially fatal pulmonary embolus--"triple-armed therapy." Thrombolytic therapy was administered through a Swan-Ganz catheter wedged against the pulmonary embolus. During the same interval 10 other patients also sustained massive pulmonary emboli but were treated only with systemic heparin. Serial pulmonary arteriography was performed daily. The patients treated by triple-armed therapy responded favorably with a rapid (less than 6 hours), significant improvement in PaO2, pulmonary artery pressure, cardiac output, pulmonary vascular resistance, and blood pressure, compared with patients treated with continuous heparin alone. Nine patients in the triple-armed therapy group survived whereas only six in the heparin group survived. Two additional patients were treated by triple-armed therapy and had thrombolysis with triple-armed therapy with tissue plasminogen activator; these patients demonstrated the most rapid improvement in cardiorespiratory dynamics and arteriographic clearance of emboli. This management protocol shows promise for patients who sustain a massive pulmonary embolus, because it reduces the morbidity associated with pulmonary embolectomy while avoiding the hazards associated with systemic thrombolytic therapy.

The hemodynamic significance of the kinked internal carotid artery and cerebrovascular insufficiency are demonstrated. Several procedures have been devised to correct a carotid kink, but these involve resection or excision of the internal, external or common carotid artery. We currently prefer a reimplantation technique whereby preservation of the external carotid is maintained, resection of the internal carotid artery is avoided and, when necessary, endarterectomy may be safely employed.

Traumatic injury to the middle and distal thirds of the profunda femoris artery can be extremely difficult to surgically repair. The present article out lines an alternative form of treatment in which angiotherapeutic techniques of embolization are used in such an injury. A survey of the literature indicates that this may be the preferred method of therapy with lesions in this difficult area to surgically approach.

Using various modalities, we analyzed 110 patients with thoracic outlet compression syndrome who had extensive evaluation. Nerve conduction velocities and Directional Doppler studies are the most useful adjuncts in diagnosis. Diagnosis and selection of the proper treatment of these patients will be outlined.

To evaluate the diagnostic accuracy of Doppler ultrasound (DU) and impedance plethysmography (IPG) in deep vein thrombosis (DVT), hospital records of 904 patients having 1,084 studies of blood flow in the lower extremities were retrospectively reviewed, and 544 were found abnormal. In 128, both DU and IPG showed abnormalities, 86 had only DU evidence of abnormalities, and 330 had only IPG abnormalities. Fifty-one patients with abnormal results of both DU and IPG studies had ascending phlebography, which in 47 documented the presence of DVT, for a diagnostic accuracy of 92%. On the basis of DU/IPG testing, or extenuating clinical circumstances, 135 patients considered at high risk for pulmonary emboli had ventilation-perfusion lung scans. Of these, 44 had roentgenographic evidence of pulmonary emboli. While ascending phlebography remains the definitive study for documenting the presence of DVT, DU/IPG testing has a high accuracy rate, is noninvasive, readily reproducible, inexpensive, and simple to perform. When results of DU/IPG studies are abnormal a baseline ventilation lung scan should be done which, if questionable, should be followed by pulmonary arteriography.

We reviewed 125 patients who had had stroke or reversible ischemic neurologic deficits (RIND) and who underwent carotid endarterectomy, with follow-up extending to six years. Phase I patients (n = 36) had endarterectomy and shunt placement at operation by surgeons' preference. The rate of postoperative neurologic deficit was 8%. Phase II patients (n = 36) had endarterectomy monitored by EEG and stump pressures. Postoperative deficits occurred in 9%. The EEGs and stump pressures in these patients were "normal"; therefore, no shunt was used. Phase III patients (n = 41) had endarterectomy again monitored by EEG and stump pressure. Despite normal EEGs or stump by EEG and stump pressure. Despite normal EEGs or stump pressure, all patients underwent endarterectomy with a temporary indwelling shunt. No complications occurred. In 368 patients operated on for transient ischemic attacks alone, the operative stroke rate was 1.6%. Stump pressure and EEG are unreliable indicators of cerebral perfusion during carotid endarterectomy in patients who have suffered stroke or RIND. Use of a temporary shunt is indicated in all of these patients.

Bilateral fibromuscular disease of the segmental renal arteries, producing severe renovascular hypertension, was surgically treated in a young male patient. The technique used was graduated internal dilatation of the involved segments. An excellent reduction in the blood pressure was obtained, and the patient remains normotensive 16 months postoperatively. The successful surgical result was documented by arteriography. This technique would appear to be the treatment of choice for this dysplasia involving the renal artery divisional branches.
