International audience; Ultrasonography is the first-step exam for the diagnosis of alveolar echinococcosis. Liver involvement commonly appears as an ill-defined infiltration of the liver parenchyma. Lesions are heterogeneous and in most cases hyperechoic, related to fibro-parasitic tissue associated to scattered calcifications. Hypoechoic foci can also be observed, related to necrosis. In 25 % of the cases, ultrasonography discovers atypical aspects that must be known by radiologists working in endemic region for alveolar echinococcosis. Specific serological tests, particularly ELISA and western blot, usually confirm the diagnosis. Therefore, per-cutaneous punction is exceptionally needed to assess the diagnosis. Other imaging techniques are very useful to complete the diagnosis step and to specify vascular and biliary extension, a crucial information for the therapeutic choice. Computed tomography may show small additional parasitic foci, non visualized by ultrasonography. It allows an accurate examination of the lesions particularly in case of very calcified images that could have made ultrasonographic analysis more difficult. Magnetic resonance imaging may be useful for diagnosis showing on T2 weighted images, in cases of fertile lesions, numerous clustered small cysts. Moreover, it is an excellent technique to analyse vascular involvement, particularly for vena cava and hepatic veins, and to diagnose involvement of adjacent organs. Coupled to cholangio-MR, it allows a precise examination of the biliary tree invasion, particularly in the hilum area.

Abstract

International audience; Cystic echinococcosis (CE), caused by Echinococcus granulosus, and alveolar echinococcosis (AE), due to E. multilocularis, are endemic to large areas of north and northwest China. Here we review features of a severe focus of AE and CE in Ningxia Hui Autonomous Region (NHAR), northwest China. We describe for NHAR the results of surveys of hospital surgical and clinical records, including treatment of AE and CE, and active community detection of asymptomatic/early-stage cases and patient follow-up using questionnaire analysis, ultrasound examinations and serology. We compare the key risk factors for both AE and CE in this setting with those reported from other areas. In addition, we document the socio-economic factors impacting on treatment and control of AE and CE. This update indicates that echinococcosis continues to be highly endemic in NHAR and exemplifies the serious public health problem that the disease presents, particularly in poor rural areas. Extensive community surveys of echinococcosis are required throughout northwest China, especially in rural communities. These will enable earlier detection of echinococcosis cases, thereby improving treatment outcomes, and reveal the true epidemiological picture regarding the disease in this region, with a view to developing and implementing new strategies for future control.

Abstract

International audience; Alveolar echinococcosis is characterized by a long asymptomatic period but, without treatment, up to 80% of patients may die within ten years of diagnosis. Owing to a lack of fast-acting and fully effective chemotherapy, partial radical hepatic resection is the only chance of cure. One-third of patients are now treated in this way, and complex vascular and biliary reconstruction procedures are sometimes necessary. Liver transplantation may also be indicated for highly selected patients (about 5%) with life-threatening complications after failure of other treatments. Interventional radiology and endoscopy can be used to drain liver abscesses and/or infected and obstructed bile ducts, either as palliative procedures or as a bridge to radical resection. Parasitostatic benzimidazole therapy, especially based on continuous albendazole administration, is mandatory for at least two years after radical resection, and for life in inoperable patients.

Abstract

International audience; OBJECTIVE: To determine the true community prevalence of human cystic (CE) and alveolar (AE) echinococcosis (hydatid disease) in a highly endemic region in Ningxia Hui, China, by detecting asymptomatic cases. METHODS: Using hospital records and "AE-risk" landscape patterns we selected study communities predicted to be at risk of human echinococcosis in Guyuan, Longde and Xiji counties. We conducted community surveys of 4773 individuals from 26 villages in 2002 and 2003 using questionnaire analysis, ultrasound examination and serology. FINDINGS: Ultrasound and serology showed a range of prevalences for AE (0-8.1%; mean 2%) and CE (0-7.4%; mean 1.6%), with the highest prevalence in Xiji (2% for CE, 2.5% for AE). There were significant differences in the prevalence of CE, AE and total echinococcosis between the three counties and villages (with multiple degrees of freedom). While hospital records showed 96% of echinococcosis cases attributable to CE, our survey showed a higher prevalence of human AE (56%) compared to CE (44%). Questionnaire analysis revealed that key risk factors for infection were age and dog ownership for both CE and AE, and Hui ethnicity and being female for AE. Drinking well-water decreased the risk for both AE and CE. CONCLUSION: Echinococcosis continues to be a severe public health problem in this part of China because of unhygienic practices/habits and poor knowledge among the communities regarding this disease.

Abstract

International audience; Data from two cross-sectional investigations on 7,138 subjects were used to explore risk factors of human alveolar echinococcosis (AE) in Tibetan communities. The overall human AE prevalence was 3.1% (223 of 7,138), females had a higher prevalence (3.6%, 132 of 3,713) than males (2.7%, 91 of 3,425; P = 0.011), and herdsmen had a higher prevalence (5.2%, 154 of 2,955) than farmers (1.8%, 12 of 661; P < 0.001) and urban populations (2.1%, 49 of 2,360; P < 0.001). Age in all populations, number of dogs kept, fox skin ownership in farmers, not preventing flies from landing on food in herdsmen, using open streams as drinking water sources, and playing with dogs in urban populations were statistically significant risk factors. The results suggest that AE is highly endemic in the eastern Tibetan plateau, in Sichuan Province, the role of the dog is important for human infection, and other factors associated with environmental contamination may vary according to structure and practices of communities.
