2. Recommandations françaises du Comité de cancérologie de l'AFU – Actualisation 2024–2026 : cancer de la prostate – traitement de la récidive et de la maladie métastatique.

Le Comité de cancérologie de l'Association française d'urologie propose une mise à jour des recommandations dans la prise en charge du cancer de la prostate (CaP) récidivé et/ou métastatique. Une revue systématique de la littérature de 2022 à 2024 a été conduite par le CCAFU concernant les éléments de prise en charge thérapeutique du CaP en récidive après traitement local ou métastatique, en évaluant les références avec leur niveau de preuve. L'imagerie moléculaire est le bilan standard de la récidive après traitement local. Sa réalisation ne doit pas retarder le traitement de rattrapage précoce. La suppression androgénique (SAd) est le pilier du traitement du CaP métastatique. L'intensification de la SAd est désormais un standard dans la prise en charge du CaP métastatique, associant au moins une hormonothérapie de nouvelle génération (HTNG). Pour les patients d'emblée métastatiques au diagnostic de haut volume, un traitement par docétaxel en plus de l'association SAd + HTNG peut être discuté pour les patients éligibles. Chez les patients avec un CaP résistant à la castration (CPRC), les inhibiteurs de PARP et radiothérapie interne vectorisée par ligands du PSMA sont de nouvelles options thérapeutiques. La combinaison et le séquençage des traitements dépendent de nombreux facteurs (caractéristiques du patient et de la maladie, lignes précédentes, statut génomique, imagerie moléculaire). Cette actualisation des recommandations françaises doit contribuer à améliorer la prise en charge des patients porteurs d'un CaP en récidive ou métastatique. The Oncology Committee of the French Urology Association is proposing updated recommendations for the management of recurrent and/or metastatic prostate cancer (PCa). A systematic review of the literature from 2022 to 2024 was conducted by the CCAFU on the elements of therapeutic management of PCa in recurrence after local or metastatic treatment, evaluating the references with their level of evidence. Molecular imaging is the standard assessment of recurrence after local treatment. It should not delay early remedial treatment. Androgen deprivation therapy (ADT) is the main stay of treatment for metastatic PCa. Intensification of ADT is now the standard treatment for metastatic PCa, involving at least one new-generation hormone therapy (ARPI). For patients with high-volume metastatic disease at the time of diagnosis, treatment with docetaxel in addition to ADT + ARPI may be discussed for eligible patients. In patients with castration-resistant PCa (CRPC), PARP inhibitors and PSMA radioligand therapy are new treatment options. The combination and sequencing of treatments depend on many factors (patient and disease characteristics, previous lines, genomic status, molecular imaging). Metastatic gastrointestinal cancer is not an uncommon situation, especially for pancreatic, gastric, and colorectal cancers. In this setting, few data are available on the impact of the treatment of the primary tumour. Oligometastatic disease is associated with longer survival in comparison with more advanced disease. Metastasis-directed therapy, such as stereotactic body radiotherapy, seems related to better outcomes, but the level of evidence is low. 14. Un syndrome des orteils bleus révélant un cancer de la prostate métastatique : une observation avec analyse de la littérature.

Les manifestations cutanées et vasculaires des cancers sont nombreuses. Parmi les acrosyndromes vasculaires paranéoplasiques, nous rapportons un cas d'un syndrome des orteils bleus révélant un cancer de la prostate en analysant les causes de ce syndrome avec les données de la littérature. Un homme de 56 ans, ayant un phénomène de Raynaud des membres supérieurs depuis 2 à 3 ans, consultait pour 4 orteils bleus du pied gauche évoluant depuis 18 mois, sans ulcération, les pouls étant bien perçus. Les explorations vasculaires (écho-doppler, angio-IRM) et cardiaques étaient normales. Il n'y avait aucune anomalie biologique et immunologique, excepté une élévation des PSA. Les biopsies prostatiques confirmaient le diagnostic de carcinome prostatique et le scanner abdomino-pelvien les localisations osseuses et ganglionnaires. La révélation d'un cancer de la prostate avec métastases osseuses par un syndrome des orteils bleus est une situation rare. Devant un syndrome des orteils bleus sans anomalie clinique évidente, notamment athéromateuse, les explorations doivent comprendre la recherche d'un cancer qui peut en être révélateur. Cutaneous and vascular manifestations of cancer are numerous. Among paraneoplastic acral vascular syndrome, we report a case of blue toe syndrome as the first manifestation of a prostate cancer following with analysis of this syndrome according literature. A 56-year-old man, with Raynaud's phenomenon of the upper limbs for 2 to 3 years, had 4 blue toes of the left foot evolving for 18 months, without ulceration, the pulses being present. Vascular and cardiac explorations (ultrasound, angio-MRI) were normal. There was no biological or immunological abnormality except an elevated PSA level. Prostate biopsies confirmed diagnosis and abdomino-pelvic CT scan proved the bone and lymph node metastasis. The revelation of a prostate cancer with bone metastases by a blue toe syndrome is a rare situation. In a patient with a blue toe syndrome with no obvious clinical or biological abnormality, especially atheromatous, investigations should include a search for cancer, which can be revealed by blue toes. 15. Biopsies vertébrales assistées par robot dans les spondylodiscites infectieuses.

Le recours à une biopsie vertébrale reste très fréquemment d'actualité, dans la prise en charge des patients atteints de spondylodiscites infectieuses, sans germe identifié ou métastases vertébrales, sans cancer primitif mis en évidence, afin d'obtenir une preuve bactériologique et/ou histologique de la maladie. Les services de rhumatologie qui restent complètement autonomes dans la réalisation de ces biopsies vertébrales sont peu nombreux. Nous décrivons ici une technique de biopsies vertébrales sous assistance robotisée. Celle-ci a pour objectifs d'être plus précise que les techniques actuelles, plus confortable pour le patient, et de réduire la durée moyenne de séjour des patients hospitalisés en rhumatologie. La biopsie est réalisée au bloc opératoire par un duo rhumatologue/neurochirurgien guidé par un robot. La précision de la biopsie repose sur l'acquisition de l'anatomie rachidienne tridimensionnelle par un scanner à capteur plan couplé au robot. Cette technique de biopsies vertébrales s'acquiert facilement et permet au rhumatologue d'être autonome de la première consultation au diagnostic étiologique, puis au traitement des spondylodiscites infectieuses. Cette technique est cependant dépendante de la disponibilité d'un bloc opératoire équipé. The use of a vertebral biopsy remains very frequently relevant in the management of patients with infectious spondylodiscitis without identified germs or vertebral metastases without identified primary cancer in order to obtain bacteriological and/or histological proof of the disease. There are few Rheumatology departments which remain completely independent in carrying out these vertebral biopsies. We describe here a technique of spinal biopsies under robotic assistance. This aims to be more precise than current techniques, more comfortable for the patient, and to reduce the average length of stay of patients hospitalized in rheumatology. The biopsy is performed in the operating room by a rheumatologist/neurosurgeon duo guided by a robot. The precision of the biopsy is based on the acquisition of three-dimensional spinal anatomy by a plane sensor scanner coupled to the robot. This technique of vertebral biopsies is easily acquired and allows the rheumatologist to be independent from the first consultation to the aetiological diagnosis, then to the treatment of infectious spondylodiscitis. Objective: to evaluate and correlate the quality of life domains of patients with liver metastasis of colorectal cancer undergoing chemotherapy. Method: cross-sectional, observational study with 106 patients selected in three hospitals in southern Brazil. Data were collected using the Quality of Life Questionnaire Core 30 and Quality of Life Questionnaire Colorectal Liver Metastases, analyzed descriptively and by Spearman’s correlation test. Results: sexual life (45.08%), emotional function (43.08%) and fatigue (40.15%) had the worst scores with the Quality of Life Questionnaire Colorectal Liver Metastases. All correlations between the domains are significant, demonstrating that a domain with a low score interferes in all others. Conclusion: patients with liver metastasis from colorectal cancer undergoing chemotherapy showed changes in quality of life domains. [ABSTRACT FROM AUTHOR]

19. Les atteintes digestives secondaires des carcinomes mammaires de type lobulaire infiltrant.

Résumé: Chez la femme, le cancer du sein est le plus fréquent et la première cause de mortalité par cancer. Le carcinome mammaire de type lobulaire infiltrant en est un sous-type histologique représentant 10 à 15 % de l'ensemble des cancers du sein. Il est caractérisé par la perte de l'expression membranaire de la protéine d'adhésion E-cadhérine codée par le gène CDH1 , lui conférant des caractéristiques histologiques particulières. Son mode de dissémination métastatique est sensiblement différent, caractérisé par une moindre fréquence des localisations secondaires pleuro-pulmonaires, ganglionnaires et cérébrales et une plus grande fréquence des localisations digestives, péritonéales et ovariennes. Cependant, les atteintes digestives secondaires restent rares en pratique clinique et sont parfois non diagnostiquées. L'atteinte gastrique en est la localisation la plus fréquente avec pour la majorité des cas un tableau de linite gastrique secondaire. Le diagnostic différentiel avec les linites gastriques primitives peut être difficile devant une symptomatologie, une description endoscopique, écho-endoscopique et en imagerie non spécifiques, et en particulier lorsqu'il existe un très long délai entre le diagnostic initial de la lésion mammaire et celui de l'atteinte gastrique. Cependant, l'enjeu thérapeutique est majeur et leur pronostic différent. L'examen anatomopathologique et surtout l'étude immunohistochimique sont les éléments clés du diagnostic. L'atteinte d'autres segments digestifs est également possible, en particulier le côlon, le rectum et l'intestin grêle, isolée ou associée à une atteinte gastrique. Enfin, la cooccurrence chez une même patiente d'un carcinome lobulaire infiltrant et d'une linite primitive gastrique est rare et doit évoquer la possibilité d'une mutation constitutionnelle du gène CDH1. In women, breast cancer is the most common cancer and the leading cause of cancer death. Invasive lobular breast carcinoma is a histological subtype representing 10 to 15% of all breast cancers. It is characterized by the loss of membrane expression of the adhesion protein E-Cadherin encoded by the CDH1 gene, giving it specific histological characteristics. Its means of spread is significantly different, characterized by a lower frequency of secondary pleuropulmonary, lymph node and cerebral localizations and a higher frequency of digestive, peritoneal and ovarian localizations. However, secondary digestive involvement remains rare in clinical practice and sometimes goes undiagnosed. Gastric involvement is the first location, with in most cases presenting as secondary gastric linitis plastica. The differential diagnosis with primary gastric linitis plastica might be difficult in the face of a non-specific symptomatology, endoscopic, echo-endoscopic and imaging description, and in particular when there is a very long delay between the initial diagnosis of the breast lesion and that of the gastric involvement. However, the therapeutic issue is major and their prognosis different. Anatomopathological examination and especially immunohistochemical study are the key facts about the diagnosis. Involvement of other digestive segments is also possible, in particular the colon, rectum and small bowel, isolated or associated with gastric involvement. Lastly, the association of invasive lobular carcinoma and primary gastric linitis plastica in the same patient is rare and should raise the possibility of a constitutional mutation of the CDH1 gene. Penile metastasization is an uncommon condition, mostly derived from primitive advanced abdominal cancers, with disabling symptoms. Palliative treatment, in reason of poor prognosis patients, is frequently surgical with destructive management. We report two cases of penile metastasis, from primitive prostatic adenocarcinoma and primitive urothelial carcinoma, effectively managed with radiation treatment at our institution. A three-dimensional conformal radiation therapy with 42 Gy to the planning target volume in 14 fractions was delivered. Radiation treatment was safely delivered, with low toxicity profile and achieved adequate symptoms control without compromising genitourinary functions. Résumé: Le cancer colorectal (CCR) constitue un problème de santé publique en France, il est métastatique dans la moitié des cas (synchrones dans 25 % des cas). Dès le début de la prise en charge, outre le diagnostic histologique positif et le bilan d'extension standard, il est important de déterminer le profil clinique du patient (état général, comorbidités), la résécabilité des métastases et la biologie moléculaire (RAS , BRAF , MSI) de la tumeur. Ces éléments vont nous guider dans le choix de la stratégie thérapeutique de première ligne mais également des lignes ultérieures. Dans cet article, nous décrivons les différentes stratégies thérapeutiques en fonction du profil du patient et de la tumeur, ainsi que la place des nouveaux traitements dans ces stratégies (anti-BRAF, anti-HER2, immunothérapie et inhibiteurs de NTRK) et le rôle prometteur de l'ADN tumoral circulant dans le suivi de la maladie, la réponse au traitement et l'orientation thérapeutique. Grâce aux progrès thérapeutiques et à la biologie moléculaire qui a permis un affinement du diagnostic, on peut parler aujourd'hui d'un traitement personnalisé du CCR au vue de la grande hétérogénéité de ces tumeurs, avec en corollaire une amélioration de la survie des patients qui atteint aujourd'hui 30 mois de médiane. Colorectal cancer (CRC) constitutes a public health problem in France, it is metastatic in half of the cases (synchronous in 25% of cases). From the start of treatment, in addition to the positive histological diagnosis and the standard extension assessment, it is important to determine the patient's clinical profile, the resecability of the metastases, the molecular biology of the tumor ; all this elements guide us in the choice of the therapeutic strategy in first line, but also the in the more advanced lines. In this article, we describe the different therapeutics strategies according to the patient and tumor profiles, but also the place of new treatments in this strategy (BRAF inhibitor, anti-HER2, immunotherapy and NTRK inhibition) and the role of circulating tumor DNA in the treatment monitoring, evaluation of tumor response and therapeutic choice. Thanks to therapeutic advances and molecular biology, which allow a diagnostic refinement, today we can talk about personalized treatment of CRC given the great heterogeneity of these tumors, with an improvement in patient survival which reach a median of 30 months. The objective of this pictorial essay is to demonstrate what can be identified within inguinal hernias. As evidências atuais sugerem que a cirurgia oncológica, usada no tratamento de tumores sólidos, aumenta o risco de metástase. Nesse sentido, uma ampla gama de células tumorais expressa Canais de Sódio Dependentes de Voltagem (CSDV), cujos papéis biológicos não estão relacionados à produção de potencial de ação. Nas células epiteliais tumorais, o CSDV é parte integrante de estruturas celulares denominadas invadópodes, que participam da proliferação, migração e metástase celular. Estudos recentes mostraram que a lidocaína pode diminuir a recorrência do câncer através de efeitos diretos nas células tumorais e de propriedades imunomoduladoras na resposta ao estresse. O objetivo desta revisão narrativa é analisar o papel do CSDV nas células tumorais e descrever o possível efeito antiproliferativo da lidocaína na patogênese das metástases. Foi realizada uma revisão crítica da literatura de Abril de 2017 a Abril de 2019. Os artigos encontrados no PubMed (2000 − 2019) foram analisados. Pesquisamos textos de linguagem livre e descritores MeSH‐lidocaína; canais de sódio dependentes de voltagem; células tumorais; invadópodes; estresse cirúrgico; proliferação celular; metástase; recorrência do câncer − em artigos publicados em inglês, espanhol e português. Foram selecionadas 62 publicações. Em estudos empregando animais, a lidocaína atua bloqueando o CSDV e outros receptores, diminuindo a migração, invasão e metástase. Esses estudos precisam ser replicados em humanos submetidos a cirurgia oncológica. The current evidence suggests that oncological surgery, which is a therapy used in the treatment of solid tumors, increases the risk of metastasis. In this regard, a wide range of tumor cells express Voltage‐Gated Sodium Channels (VGSC), whose biological roles are not related to the generation of action potentials. In epithelial tumor cells, VGSC are part of cellular structures named invadopodia, involved in cell proliferation, migration, and metastasis. Recent studies showed that lidocaine could decrease cancer recurrence through its direct effects on tumor cells and immunomodulatory properties on the stress response. The aim of this narrative review is to highlight the role of VGSC in tumor cells, and to describe the potential antiproliferative effect of lidocaine during the pathogenesis of metastasis. A critical review of literature from April 2017 to April 2019 was performed. Articles found on PubMed (2000 − 2019) were considered. A free text and MeSH‐lidocaine; voltage‐gated sodium channels; tumor cells; invadopodia; surgical stress; cell proliferation; metastasis; cancer recurrence − for articles in English, Spanish and Portuguese language − was used. A total of 62 were selected. In animal studies, lidocaine acts by blocking VGSC and other receptors, decreasing migration, invasion, and metastasis. Background: Allogeneic blood transfusion is avoidable in many oncological interventions by the use of cell salvage or mechanical autotransfusion (MAT). As irradiation is elaborate and expensive, the safety of leucocyte depletion filters (LDF) for autologous blood from the surgical field might be a more acceptable alternative for the prevention of cancer recurrences. A previous meta-analysis could not identify an increased risk of cancer recurrence. The aim of this review article is to provide an update of a previous meta-analysis from 2012 as well as a safety analysis of cell salvage with LDF due to the improved data situation.Material and Methods: This systematic review included all studies in PubMed, Cochrane, Cochrane Reviews and Web of Science on cell salvage or autotransfusion combined with outcomes, e.g. cancer recurrence, mortality, survival, blood transfusion, length of hospital stay (LOS) after the use of MAT without irradiation and with or without LDF. The grades of recommendations (GRADE) assessment of underlying evidence was applied.Results: A total of seven new observational studies and seven meta-analyses were found that compared unfiltered or filtered cell salvage with autologous predeposition, allogeneic transfusion or without any transfusion. No randomized controlled trials have been completed. A total of 27 observational and cohort studies were included in a meta-analysis. The evidence level was low. The risk of cancer recurrence in recipients of autologous salvaged blood with or without LDF was reduced (odds ratio, OR 0.71, 95% confidence interval, CI 0.58-0.86) as compared to non-transfused subjects, allogeneic or predeposited autologous transfusion. The transfusion rate could not be assessed due to the substantial selection bias and large heterogeneity. Cell salvage does not change mortality and LOS. Leucocyte depletion studies reported a removal rate of cancer cells in the range of 99.6-99.9%.Conclusion: Randomized controlled trials on a comparison of MAT and allogeneic blood transfusion as well as LDF and irradiation would be desirable but are not available. From observational trials and more than 6300 subjects and various tumors, cell salvage in cancer surgery with or without LDF appears to be sufficiently safe. The efficacy of leucocyte depletion of autologous salvaged blood is equivalent to irradiation. Unavailability of radiation is not a contraindication for cell salvage use in cancer surgery. By usage of leucocyte depleted salvaged autologous blood, the risks of allogeneic transfusion can be avoided. [ABSTRACT FROM AUTHOR]- Published

Skin metastases from undifferentiated carcinoma of the nasopharynx are a rare clinical entity with unfavorable prognosis. We here report the case of a 16-year-old patient with single scalp metastasis with rapid evolution. This occurred 6 months after the end of concomitant radiotherapy and chemotherapy for the treatment of undifferentiated carcinoma of the nasopharynx. The patient died a month after the diagnosis of the metastasis. Given their poor prognosis, skin metastases should always be suspected in patients with skin lesions after a cancer. Fluorescence‐guided cytoreductive surgery is one of the most promising approaches for facile elimination of tumors in situ, thereby improving prognosis. Reported herein is a simple strategy to construct a novel chainlike NIR‐II nanoprobe (APP‐Ag2S‐RGD) by self‐assembly of an amphiphilic peptide (APP) into a nanochain with subsequent chemical crosslinking of NIR‐II Ag2S QDs and the tumor‐targeting RGD peptide. This probe exhibits higher capability for cancer cell detection compared with that of RGD‐functionalized Ag2S QDs (Ag2S‐RGD) at the same concentration. Upon intraperitoneal injection, superior tumor‐to‐normal tissue signal ratio is achieved and non‐vascularized tiny tumor metastatic foci as small as about 0.2 mm in diameter could be facilely eliminated under NIR‐II fluorescent imaging guidance. These results clearly indicate the potential of this probe for fluorescence‐guided tumor staging, preoperative diagnosis, and intraoperative navigation. [ABSTRACT FROM AUTHOR]

In this situation, two orally administered drugs, regorafenib and trifluridine/tipiracil, have become therapeutic standards after having demonstrated a survival improvement as compared to best supportive care in randomised controlled trials. This review article presents the pharmacokinetic, efficacy and safety data for these two drugs, as well as current data on the search for predictive biomarkers of their efficacy, and other treatment options beyond the second line. This article contains a morphological description of a case of early presentation lymphoma in a lamb, characterised by its unusual clinical history and advanced lesions. The primary tumour was located in the skin in the carpal region, and metastases were observed in the brain, right eye, all the pulmonary lobes, the myocardium and endocardium of the heart, left adrenal gland, abdominal portion of the vagal nerve, serosa of the rumen, the body of lumbar vertebra L3, and the submandibular, mediastinal and mesenteric lymph nodes. Histopathological examination revealed that it was a large cell lymphoma. Immunohistochemical analysis was carried out with antibodies for MUM1/IRF4, CD79a, CD3, CD10 and Ki67, used here for the first time as markers to characterise a lymphoma in this species. National audience; Testis tumors are uncommon in oncology, and testicular metastasis from distant solid tumors are even rarer. We present two cases encountered in our department of pathology in CHU de Rennes, France. Moreover, we collected all reported cases in the Medline/PubMed databases of non-hematopoietic secondary testis tumors in adults, excluding autopsy studies, to propose an integrative study on this topic. In total, we report 98 cases of secondary testis lesions to prostate (n=38, 38.77 %), colorectal (n=19, 19.39%), gastric (n=12, 12.24%), kidney (n=7, 7.14%), lung (n=6, 6.12%) and other primary cancers. Le cancer de la prostate (CaP) représente la 3ème cause de mortalité par cancer chez l’homme en France. Il est reconnu pour son potentiel pouvoir métastatique au niveau de l’os et préférentiellement au niveau du squelette axial. Nous en décrivons dans ce travail les particularités diagnostiques, thérapeutiques actuels et évolutifs d’une métastase crânienne du CaP.Patient de 69 ans, sans antécédents pathologiques connus, reçu en consultation au service d’Urologie du CHU Yalgado Ouédraogo du Burkina Faso pour des SBAU. L’examen clinique a retrouvé un globe vésical, une prostate d’allure suspecte après la vidange vésicale et une voussure crânienne de l’os pariétal droit. L’élévation de la valeur du PSA total à 110 ng/ml a permis de faire une biopsie prostatique échoguidée qui a révélé à l’histologie un adénocarcinome ISUP 4. La tomodensitométrie thoraco-abdomino-pelvienne a montré un envahissement du plancher vésical, des méats urétéraux et du rectum. La scintigraphie osseuse a montré de multiples lésions métastatiques, dont l’os pariétal droit du crâne. Le diagnostic retenu était un adénocarcinome prostatique classé pT4N1M1c. Le patient a choisi comme traitement, une castration chirurgicale (pulpectomie) après un counseling bien conduit. Les suites opératoires ont été simples. A M3 post opératoire, il y avait une nette régression de la voussure crânienne.Les métastases crâniennes du CaP sont rares. La prise en charge thérapeutique par suppression androgénique donne de bons résultats. Prostate cancer is the third leading cause of cancer death in men in France. It is recognized for its potential metastatic power to bone and preferentially to the axial skeleton. We describe in this work the diagnostic, current and evolving therapeutic particularities of a cranial metastasis of prostate cancer. 69-year-old patient, with no known pathological history, seen in the Urology Department of the Yalgado Ouedraogo Teaching Hospital in Burkina Faso for SBAU. Clinical examination found a distended bladder, a suspicion of prostate cancer after bladder emptying and a cranial arch of the right parietal bone. Raising the total PSA value to 110 ng / ml allowed ultrasound-guided prostate biopsy to be performed, which revealed on histology an ISUP 4 adenocarcinoma. Thoraco-abdominal-pelvic computed tomography showed invasion of the bladder floor, ureteral meatus and rectum. The bone scintigraphy showed multiple metastatic lesions, including the right parietal bone of the cranium. The diagnosis was a prostatic adenocarcinoma classified as pT4N1M1c. The patient chose surgical castration (pulpectomy) as a treatment after well-conducted counseling. The postoperative follow-up was straightforward. At M1 postoperative, there was a clear regression of the cranial arch.Cranial prostate cancer metastases are rare. Therapeutic management by androgen suppression gives good results.

Dissertação de mestrado em Biophysics and Bionanosystems, O câncro da mama é um dos tipos de câncer mais comuns e uma das principais causas de mortalidade por câncer em mulheres1. A metástase é a causa básica da mortalidade relacionada ao câncer e, como o câncer é uma doença dinâmica e heterogênea, é crucial monitorizar a progressão da doença para combater a sua evolução2. Os métodos atualmente utilizados na clínica não são ideais, pois não representam o real estado do tumor. Para promover uma maior precisão na biomedicina a fim de melhorar o diagnóstico clínico e as decisões terapêuticas, é necessário desenvolver novas estratégias de detecção dos biomarcadores3. Por meio da biópsia líquida (LB), que é um método minimamente invasivo, é possível aceder informações sobre o tumor em tempo real, superando as limitações da biópsia solida4, permitindo um diagnóstico mais preciso, precoce e uma terapia personalizada. LB analisa vários biomarcadores de câncer, incluindo ctDNA5. O DNA tumoral circulante (ctDNA) é um DNA fragmentado que se origina no tumor ou em células tumorais circulantes (CTCs) que circulam livremente no sangue do paciente, resultantes do fenômeno da metástase. Ao ser proveniente das células, é liberado após sua morte, por diferentes processos como apoptose, necrose e autofagia, assim o ctDNA apresenta menor uniformidade de tamanho e integridade quando relacionado ao DNA livre de células6. Estudos anteriores sobre o ctDNA mostraram que ele representa um biomarcador valioso para identificar mutações específicas que fornecem informações valiosas para o manejo clínico de pacientes com câncer, pois representa uma fonte em tempo real de informações relacionadas ao tumor, relevantes para a progressão do câncer e personalização do tratamento do paciente para melhorar a sua resposta. No entanto, as técnicas atuais de extração de ctDNA são complicadas e pouco eficientes7–9. Microfluídica é a área da ciência de manipulação e controlo de fluidos e partículas em microcanais10. Apresenta múltiplas vantagens no manuseio de amostras biológicas, como a possibilidade de dispositivos portáteis, descartáveis e de baixo custo, e a oferta de capacidade de integração para que toda a gama de protocolos laboratoriais de bancada, desde o manuseio de amostras até a reação, separação e deteção, possam ser incorporados e automatizados num único chip 11., Breast cancer is one of the most common cancer types and a leading cause of cancer-related mortality in women1 . Metastasis is the underlying cause of cancer-related mortality, and since cancer is a dynamic and heterogeneous disease, it is crucial to monitor disease progression to fight cancer evolution2 . The methods currently used in the clinic, are not ideal as they fail to represent the real state of the tumor. To promote greater precision in biomedicine to improve clinical diagnosis and therapeutic decisions, it is necessary to develop new biomarker detection strategies3 . Through liquid biopsy (LB), which is a minimally invasive method, it is possible to access information about the tumor in real-time, overcoming the limitations of tissue biopsy4 , and allowing a more precise and early diagnosis and personalized therapy. LB analyses several cancer biomarkers, including ctDNA5 . Circulating tumor DNA (ctDNA) is fragmented DNA that originates in the tumor or in circulating tumor cells (CTCs) circulating freely in the patient's blood, resulting from the phenomenon of metastasis. When coming from cells, it is released upon their death, by different processes such as apoptosis, necrosis, and autophagy, thus ctDNA exhibits less uniformity in size and integrity when related to cell-free DNA6 . Previous ctDNA studies have shown that it represents a valuable biomarker to identify specific mutations that provide valuable information for the clinical management of cancer patients as it represents a real-time source of tumor-related information, relevant to cancer progression and personalize patient treatment to improve their response. However, current techniques for ctDNA extraction are complicated and not very efficient7–9 . Microfluidics is the area of science of handling and controlling fluids and particles in microchannels10. It presents multiple advantages to handling biological samples, such as the possibility of portable, disposable, and inexpensive devices, and the offer of integration capability so that the entire range of benchtop laboratory protocols, from sample handling to reaction, separation, and detection, can be incorporated and automated on a single chip11.

In breast, the adenomyoepithelioma is a lesion which terminology suggests to be a benign tumor. Making a review in a literature, it shows with potential to develop metastasis in axillary nodes. In this paper the authors show a case occurred after breast feeding. The lesion was treated under local surgery and the patient is maintained in follow-up. O adenomioepitelioma é uma lesão cuja terminologia sugere ser um tumor benigno, mas observado na literatura médica com potencialidade para o aparecimento de metástases, principalmente em gânglios axilares. Os autores apresentam um caso ocorrido após a lactação. A lesão foi tratada com cirurgia local e a paciente continua em segmento contínuo.

Secondary subcutaneous metastasis originating from vesicular thyroid carcinoma is unusual. It accounts for more than 5.8% of most common sites of metastasis for thyroid carcinoma. We here report the case of a 60-year old woman presenting with slowly evolving giant hypervascular lesion in the subcutaneous tissue of the skull occurred 7 years after right thyroid lobectomy. Anatomopathological examination enabled the diagnosis of vesicular thyroid carcinoma. Surgery was based on reduction of the skull lesion as well as on thyroidectomy. Currently, the patient is waiting for radioactive iodine treatment. Metastasis from vesicular carcinoma of the scalp, even though rare, can occur. Meningiomas are the most frequent neoplasms involving the brain in dogs and cats, and are occasionally observed in the spinal cord. They cause compression of the central nervous system; however, do not infiltrate the neuropile and rarely metastasize to other organs. The present study describes a case of anaplastic spinal meningioma with extra neural metastasis in a 20 years-old cat. Clinically, the feline presented a clinical history of 120 days with paresis of the hind limbs and loss of the tail's movements, with subsequent death. At necropsy, there was an irregular and soft whitish mass involving the meninges from the lumbar intumescence to the sacral region of the spinal cord. Similar white nodular masses were observed in the lungs, liver and kidneys. Microscopically, both were composed of a poorly limited and infiltrative neoplastic proliferation composed by spindle, round and epithelioid cells, with a high cellular pleomorphism. On IHC, there was a severe immunostaining for vimentin and S100. Histopathologic and IHC analysis are important tools for definitive diagnosis of meningiomas in cats, and differentiation of other common neurologic disorders in these animals. [ABSTRACT FROM AUTHOR]

Endocervical Adenocarcinoma In Situ (ACIS) is an injury associated with the columnar epithelium and by definition does not invade the cell stroma. We report the case of a 49- year-old woman with a history of total hysterectomy in 2012 due to uterine myomatosis and in situ adenocarcinoma in which no signs of stromal invasion were observed in the sections studied. Patient underwent bilateral oophorectomy, in 2016, due to proven ACIS metastatic ovarian involvement, due to probable spoliation, through a positive immunohistochemical study of the p16 marker.
