In this study, we aimed to investigate the prognostic impact of splenectomy on post-operative complications and the long-term oncologic results of cytoreductive surgery with hyperthermic intraperitoneal chemotherapy for treating peritoneal metastasis. All consecutive patients who underwent cytoreductive surgery with hyperthermic intraperitoneal chemotherapy for peritoneal metastasis were analysed between 2007 and 2020. In addition to clinicopathological features, peritoneal cancer index, completeness of cytoreduction, (neo-)adjuvant chemotherapy, operative time, need for surgical intensive care unit, use of albumin, erythrocyte suspension and fresh frozen plasma, infectious and non-infectious post-operative complications were evaluated. The data of 660 patients were analysed, and splenectomy was performed in 92 patients (13.9%). The 3- and 5-year-survival rates were significantly lower in the splenectomy group (42.9% and 25.6% vs. 50.2% and 34.0%; p=.038). High peritoneal cancer index score (OR, 2.34; 95%CI, 1.40–3.90; p =.001) and the number of resected organs (OR, 4.34; 95%CI, 2.58–7.29; p =.000) were significant risk factors in the splenectomy group. Age (HR, 1.01; 95%CI, 1.01–1.03; p =.002), high peritoneal cancer index score (HR, 1.59; 95%CI, 1.18–2.15; p =.002), smoking (HR, 1.39; 95%CI, 1.04–1.87; p =.028), intraoperative erythrocyte suspension (HR, 2.37; 95%CI, 1.69–3.31; p <.001), intraoperative fresh frozen plasma (HR, 0.66; 95%CI, 0.46–0.93; p =.019) usage and infection (HR, 1.51; 95%CI 1.11–2.06; p =.009) were independent, robust prognostic factors in Cox regression analysis. Splenectomy was associated with increased morbi-mortality and post-operative infections and decreased overall survival in patients with peritoneal metastasis who underwent cytoreductive surgery with hyperthermic intraperitoneal chemotherapy. Lung is primary capillary filter of venous drainage for many organs. As a result of this, it is the most common metastasis site of extrapulmonary organ tumours. Cystic and cavitary forms of lung metastases are unusual. In evaluation of patients with these lesions benign and malignant lesions should be distinguished from each other. Background: The purpose of our study was to investigate the immunohistochemical expression of TGF-β1 and p27 in pancreatic adenocarcinomas and to compare the findings with the clinicopathological features and survival. We also aimed to evaluate the expression of TGF-β1 and p27 in the context of other cell cycle and proliferation markers such as cyclin D1 and Ki-67. Methods: We examined TGF-β1 and p27 expression immunohistochemically in 63 cases of invasive ductal adenocarcinoma of the pancreas. Standard streptavidin-biotin immunperoxidase method was used for immunostaining and the stained slides were examined microscopically using semiquantitative criteria. Results: TGF-β1 stained the cytoplasms of the tumor cells in 43 cases [68.3%]. There was a statistically significant difference among TGF-β1 staining scores in terms of clinicopathologic factors such as blood vessel invasion, stage and distant metastasis [p < 0.05]. Of the 63 tumors evaluated 23 [36.5%] were positive for p27 within the nucleus. An inverse correlation was found between p27 immunoreactivity and grade [p < 0.05]. But no significant correlation was found between p27 and other parameters. Among the patients with survival data 27 patients had RO resections and these cases were considered in survival analysis. In the univariate analysis, neither TGF-β1 nor p27 expression was related with patient survival. Conclusion: Our findings suggest that in pancreatic carcinoma, TGF-β1 expression is related to tumor growth and metastasis. But it is not associated with cell cycle proteins. p27 expression is reduced in pancreatic adenocarcinomas and decreased protein levels of p27 may play a role in the differentiation of pancreatic cancer. Aims Solid-pseudopapillary neoplasm (SPN) is an uncommon malignant tumor of the pancreas with a favorable prognosis unlike other pancreatic neoplasms. We investigated the clinicopathological features of 20 patients with SPN in details. Methods The patients diagnosed as SPN in Dokuz Eylul University Hospital between January 2005 and March 2016 were reviewed in terms of clinical and histopathological data. Results Mean age of the patients was 33. Three of our cases were male and 4 were children. Some patients had synchronuous malignancies. Nine patients were diagnosed by fine needle aspiration cytology (FNAC). One of our tumors had 2 × 1 mm of pancreas endocrine neoplasm in addition to SPN. One case had foci of atypical and multinucleated giant cells. All cases were positive for vimentin and CD10 antibodies and most were positive for PR and β-catenin. The mean follow-up duration was 40 mo (range 2–110 mo). Only one case showed liver metastasis. Conclusion Herein we present a series of 20 patients with 3 male and 4 pediatric cases, almost half of which were diagnosed with FNAC findings, and most of which are clinically being followed with one patient showing progression. Our series includes rare examples like collision tumor of SPN and pancreas endocrine neoplasm, SPN with multinucleated giant cells. Also cases with no surgical treatment and no progression, as well as cases with synchronous malignancies are presented. We believe that FNAC findings of any pancreatic mass should be investigated in detail for the designation of a therapy plan especially for the patients with high operation risks. The findings in our series also show that extensive necrosis, angioinvasion, perineurial invasion and larger tumor size might be predictive for worse prognosis and these patients should be more closely followed up.
