Distal surface caries in the mandibular second molar is a common outcome of impaction of the adjacent third molar. To determine the prevalence of this type of caries in the Netherlands, 250 radiographs were evaluated. The prevalence among the research population was 26%. Risk factors for developing distal surface caries in the mandibular second molar were partially erupted/impacted third molars with a mesioangular or horizontal angulation, an LD loss of ≥ 2mm and an increased modified DMFT score. The relation between the point of contact of the third molar and the adjacent mandibular second molar, on the one hand, and, on the other, the cemento-enamel junction of the mandibular second molar and the prevalence of distal surface caries almost reached statistical significance (p = 0.052). The relatively low prevalence of distal surface caries in the Netherlands is probably related to the current clinical guidelines recommending early extraction of partially erupted/impacted third molars, particularly in cases of third molars with a mesioangular or horizontal angulation.

Introduction Impacted mandibular third molars can be asymptomatic or cause some pathologies, such as distal caries and odontogenic cysts and tumours. This study investigates the prevalence of distal caries of the mandibular second molar adjacent to the mesioangular or horizontally partially erupted mandibular third molar and its relationship regarding decayed, missing and filled teeth (DMFT) risk group, age, side (left-right) and sex.Methods The study included patients who had an appointment for mesioangular or horizontally positioned and partially erupted mandibular third molar surgery. Before the surgery, the DMFT score, age, sex and side were recorded. After extraction, the second molar was clinically examined for distal caries. The prevalence of distal caries and the correlation between the DMFT risk group, age, sex and side were determined.Results The study was conducted on 514 patients and involved 639 teeth. Men and patients in the moderate- or high-risk group are significantly prone to developing distal caries of the second molar. No statistically significant relationship exists regarding age group, side and position.Conclusion Sex and DMFT risk groups affect the prevalence of distal caries in second molars adjacent to the mesioangular or horizontally partially erupted third molars. Introduction: Wisdom teeth extraction is a common procedure, generally considered as safe by patients. However, complications are possible, especially periodontal ones. The aim of this study was to evaluate the frequency and the consequences of periodontal complications at the level the 2nd molars after extraction of the wisdom teeth., Material and Methods: A single-center retrospective observational multi-operator study was conducted at the university hospital of Besançon - France. The files of all the adult patients who underwent extraction of four impacted wisdom teeth by mean of a standardized surgical technique between November 2012 and November 2014 and who could be followed 1 year postoperatively at least and that precisely mentioned the periodontal status of the surgical sites were included. Postoperative complications, gingival and plaque indexes according to Loë and Silness, periodontal attachment level and periodontal second molar probing were recorded. The main judgment criterion was the occurrence of a periodontal complication in the second molar areas., Results: The files of 20 patients (15 women - 5 men), operated on by five different surgeons, met the inclusion criteria. Two patients suffered from dry socket at one of the avulsion sites. No patient had a gingival or plaque index greater than 2. No gingival recession or periodontal pocket over 4mm was found., Discussion: Extraction of impacted third molars in young healthy adults didn't have any impact on the second molars periodontal environment in our study. Aims: The aim of this study is to retrospectively determine the prevalence of impacted second molars and associated dental and radiographic findings in South Indian population., Settings and Design: Cross-sectional design., Materials and Methods: Dental records of 4976 patients depending on the selection criteria from various dental clinics in South India were retrospectively analyzed in this study. All selected radiographs and records were examined by the two authors. The angle of impacted second molars was also recorded on panoramic radiographs. Data were tabulated and analyzed., Results: The prevalence of impacted second mandibular molars was found to be 0.16%. In seven cases, the impaction was unilateral with three on the left and four on the right, and in one case, it was bilateral. The prevalence was found to be more in females than males. The angle of impaction was found to range from 19° to 80°., Conclusions: Although the prevalence of impacted second molars is low, it is crucial to diagnose early for optimal treatment.

Prophylactic extraction of unerupted asymptomatic third molars is the most common oral surgery procedure in the United States. However, limited evidence exists to justify its costs and associated morbidity. We analyzed data collected over 25 years from 416 adult men enrolled in the Veterans Affairs Dental Longitudinal Study to evaluate the association of retained asymptomatic third molars with risk of adjacent second molar pathology (caries and/or periodontitis), based on third molar status (i.e., absent, erupted, or unerupted). Unerupted molars were further categorized as either "soft tissue" or "bony" impacted. We found that the lowest prevalence and incidence of second molar pathology occurred when the adjacent third molar was absent. The presence of a third molar that was soft tissue impacted increased the risk of incident second molar pathology 4.88-fold (95% confidence interval: 2.62, 9.08). Having an erupted or "bony" impacted third molar increased the risk of incident second molar pathology by 1.74 (95% confidence interval: 1.34, 2.25) and 2.16 (95% confidence interval: 1.56, 2.99), respectively. The retention of third molars is associated with increased risk of second molar pathology in middle-aged and older adult men.

Objective: To determine the prevalence of impacted mandibular second molar (MM2) and the association between MM2 impaction and crowding. The clinical significance of the angle between first and second mandibular molar and of the space between the first mandibular molar (MM1) and the anterior margin of mandibular ramus in MM2 impaction were also evaluated., Material and Methods: In this retrospective study , from the dental records of 2,945 caucasian young orthodontics patients , 40 subjects with MM2 impaction were included in a study group (SG) and compared with a control group (CG) of 200 subjects without MM2 impactions. The crowding , the angle of inclination of MM2, the distance between MM1 and mandibular ramus, the canine and molar relationships, and the lower centre line discrepancy were measured. For the statistical analysis , descriptive statistics and t-Student for independent sample groups were used., Results: The prevalence of impacted MM2 was 1.36%. The independent-Samples t-Test between SG and CG showed: the presence of crowding (P ≤ 0.001), an higher angle values of MM2 inclination (P ≤ 0.001) and a smaller distance between MM1 and the anterior margin of mandibular ramus (P ≤ 0.001) in the SG., Conclusion: The impaction of MM2 is a relatively rare occurrence in orthodontic caucasian populations. The crowding, a higher angle values of MM2 inclination and a reduced distance between MM1 and the anterior margin of mandibular ramus, at the time of one third of MM2 root formation (T1), characterize MM2 impaction.

Objectives: To determine the prevalence of supernumerary molars in a sector of the population of Madrid, as well as the possible complications associated with the presence of hypergenetic molars., Materials and Methods: A retrospective, descriptive study was carried out, which included a data assessment of all patients (13557) seeking dental care at a hospital's department of oral surgery across 4 years. The proposed methodology consisted of preparing a medical record in order to obtain data on the medical record number, age and sex of the patient, radiological findings such as location and type of supernumerary molar, retained/erupted molar, related accidents and tooth morphology., Results: Supernumerary molars were found to be present in 130 patients, representing a prevalence of 0.96% of the total population studied. These patients had a total of 173 hypergenetic molars, consisting of 137 distomolars and 36 paramolars Mechanical-obstructive pathology was associated with 28.9%, whereas enlargement of the follicular sack bigger than 3 mm was present in 16.2% of the sample., Conclusion: Supernumerary molars are usually diagnosed as a coincidental radiological finding without any associated pathology. However a higher percentage of comorbidity was found than initially expected.

Objective: To determine the prevalence and severity of impacted permanent mandibular second molars (MdM2s) in the general population of Taiwan as well as to evaluate treatment outcomes with an innovative method for impacted MdM2s., Materials and Methods: Dental records of 21,580 patients were retrospectively screened in this study. The impactions of MdM2s were registered. The chosen study population consisted of 96 patients with 125 impacted MdM2s who received orthodontic treatment in our clinics. Standardized periapical radiographs were taken to measure the angles and depths of the impacted MdM2s. In addition, the initial uprighting periods of the impacted MdM2s were recorded., Results: The prevalence of MdM2 impaction in the Taiwanese population was 0.65%. Most of the angles and depths of the MdM2s were 31-60° and 9-12 mm, respectively. The mean initial uprighting period of the impacted MdM2 was 3.6 months. There were no significant differences between genders in the initial uprighting period based on patient ages, impacted angles, or impacted depths (P > .05). Correlation analysis revealed that the impacted depth was highly and positively correlated with the initial uprighting period (r > .75)., Conclusions: The prevalence of MdM2 impaction in this study was low but slightly higher than previous reports investigating whites. Although such impactions are rare, it is recommended to diagnose early for an optimal treatment time and reduction of complications. In addition, the effective molar uprighting appliance used in this study has displayed good treatment outcomes with natural improvement of the adjacent infrabony defects of the first molars.

Introduction: Detection of mandibular second molar (MM2) impaction is imperative for orthodontic diagnosis and treatment. In this study, we examined a possible genetic trait in MM2 impaction in 2 populations and defined distinctive characteristics., Methods: Initial panoramic radiographs of patients of Israeli (n = 3500) and Chinese-American (n = 3000) origin, aged 11 to 15 years, were examined. Twelve distinctive characteristics were compared between the unilateral impacted and the nonimpacted sides., Results: A total of 120 subjects with MM2 impaction were found (1.8%). The Chinese-American population had a higher prevalence (n = 71, 2.3%) of MM2 impaction compared with the Israeli population (n = 49, 1.4%; P = 0.004). For the subjects with MM2 impaction, the Israelis had significantly (P = 0.039) fewer bilateral impactions (27%) than did the Chinese-Americans (45%). Mesially inclined impacted MM2s were more common (88% and 89%) in the Israeli and Chinese-American populations, respectively. The unilateral impacted side demonstrated reductions in the distance between the mandibular first molar and the ramus (P <0.001), the length of the mesial root of the MM2 (P <0.001), and the height between the MM2 and the mandibular third molar, and increases in the angulations of the MM2 (P <0.001) and the mandibular third molar (P <0.003)., Conclusions: An autosomal genetic trait is present in MM2 impaction with greater penetrance in the Chinese-American population. Objective: To investigate the prevalence of impaction of permanent mandibular second molars and associated dental and radiographic characteristics of Chinese children in Hong Kong., Materials and Methods: Dental and radiographic records of a group of Chinese schoolchildren were studied retrospectively. Cases of impaction of 1 or both permanent mandibular second molars were selected. Demographic data and dental and radiographic findings were recorded and analyzed. The angle of the impacted second molars was measured on panoramic radiographs., Results: Among the 3,053 records reviewed, 32 cases of impaction of permanent mandibular second molars were identified and studied. Mesial impaction was found in all but 1 case. The angle of the mesially impacted teeth ranged from 13 degrees to 75 degrees . Mandibular crowding and the presence of third molars were common. Dens evaginatus, taurodontism and Class III malocclusions were significantly more prevalent in the study group than in the general population of Chinese children of similar age., Conclusions: The prevalence of impaction of permanent mandibular second molars in Chinese children was estimated to be 1%. Thorough and regular examination of the child"s developing dentition is recommended to diagnose dental impaction promptly.

Objective: To elucidate the prevalence of ectopic eruption, impaction, and primary and secondary retention as well as agenesis of the permanent second molar (M2) among adolescents., Materials and Methods: After a sample size calculation, dental records, including radiographs, of 1543 patients (722 girls and 821 boys), from three clinics in the city of Malmoe, Sweden, were retrospectively analyzed. Series of annual records and radiographs were examined for all patients from 10 to 16 years of age and were carried out during 2004-2006. The prevalence of ectopic eruption, impaction, and primary and secondary retention as well as agenesis of M2s was registered in a standardized manner and according to preset definitions. In addition, the times of emergence of the M2s were recorded., Results: The prevalence of ectopic eruption of M2 was 1.5%, the prevalence of primary retention was 0.6%, and the prevalence of impaction was 0.2%. This means that the overall prevalence of eruption disturbances was 2.3%. In addition, the prevalence of agenesis was 0.8%. The prevalence of ectopic eruption was significantly higher in the mandible. Those patients with eruption disturbances and agenesis of M2 showed significantly delayed eruption of their other M2s compared to the individuals without any eruption disturbances., Conclusions: The prevalence of eruption disturbances was higher than reported earlier, and, even if the disturbances do not occur frequently, it is important to develop an early diagnosis in order to start the treatment at the optimal time.

Background: Patients who suffered from impacted teeth increase in number every year They have a wide range of ages and also have symptomatic or asymtomatic impacted molar teeth. The most common symptoms were pain, dental carious problem, gingivitis and oral infection. This was different in each patient., Objectives: 1) to study the relation of age and common symptoms among patients with an impacted molar tooth. 2) to study the type of impacted molar tooth, Setting: Department of Dentistry, King Chulalongkorn Memorial Hospital, Research Design: Descriptive study, Material and Method: Six hundred and eighty patients who had an impacted tooth were treated at the Department of Dentistry, King Chulalongkorn Memorial Hospital from June 2002 to December 2005. They were categorized in 5 age groups, namely: < or = 20 years, 21-30 years, 31-40 years, 41-50 years, and > 50 years. Data was collected from history taking, oral and radiographic examinations. The details of the patient's data and tooth's data were recorded, i.e., gender, age, location and type of the impacted tooth including the details of symptoms including pain, dental caries, gingivitis, oral and throat infections., Results: Four common symptoms including pain, dental caries, gingivitis and oral infection were recorded in each age group. The Pearson Chi - square test showed that in the statistical difference of variation of ages, p-values were 0.001, 0.037, < 0.001, 0.036 for the symptoms of pain, dental carious problem, gingivitis, and infection, respectively. One hundred and seventy eight maxillary third molar impacted teeth and 502 mandibular third molar impacted teeth were found. The impacted molar teeth were classified into five types. There was a mesioangular (402 cases), horizontal (125 cases), vertical (116 cases), distoangular (30 cases) and linguoversion or buccoversion impacted tooth (7 cases). There was a significant difference of type of impacted tooth, p-values were 0.013, 0.002 for the symptom ofpain and oral infection, respectively, but non significant difference for the dental carious problem and gingivitis, p-values were 0.702, 0.367 respectively., Conclusion: The most frequent age group of patients with an impacted molar tooth was 21-30 years. Patient under 30 years had dental carious problems more than other symptoms, while gingivitis was more common in the older group. There was a statistical significant difference of variation of age and symptom. The most common type of impacted molar tooth was mesioangular type.

The purpose of this study was to determine the characteristics and occurrence of the ectopic eruption of the maxillary first permanent molar in a sample of growing Spanish children. A descriptive, observational, retrospective study was done using the radiographs of 509 consecutive patients, who were in the first phase of mixed dentition. A method was designed to evaluate the amount of pathologic resorption of the second maxillary primary molar and the impaction of the first permanent molar measured in millimeters. Statistical analysis of the studied variables was done using chi-square, analysis of variance, and Pearson correlation coefficient. The frequency of occurrence was 4.3% with no differences in both sexes. Of these, 36.4% were unilateral and 63.6% bilateral with a right:left relation of 3:1. Of the 36 ectopic molars, 69.4% self-corrected spontaneously. In such cases, the pathological resorption of the root of the second maxillary primary molar and its adverse clinical implications were persistent. The relation between self-correction and impaction was 2.27:1. The average impaction on the right side was 2.91 mm and 1.6 mm on the left side. Correlation between the magnitude of resorption and grade of impaction was not observed. Although resorption was found on grades I and II, spontaneous self-correction could occur without arch length loss. However, on grade III or more, therapeutic intervention has to be done. The benefit of early diagnosis and treatment of the maxillary first permanent molar is the prevention of the premature loss of second maxillary deciduous molar and the resulting malocclusion.

The purpose of this research was to study the prevalence of ectopic eruption of the first permanent molars and possible etiologic factors. A group of 4,232 Thai students, from 6 to 9 years old, was examined. The prevalence of ectopic eruption of the first permanent molars in the subjects was 0.75%. Both the severity of the ectopic eruption and the amount of root resorption on the second primary molars were more pronounced in the maxilla than in the mandible. The important etiologic factors were the eruption path of the first permanent molars relative to reference lines and the size of the mandibular second primary molars. The amount of proximal caries did not seem to affect the prevalence of ectopic eruption.

This retrospective study reports the radiographic prevalence of embedded primary molar roots within Saudi adult dentulous patients. Embedded roots of primary molars were viewed radiographically by evaluation of the records of 300 patients. Embedded primary molar roots were detected in 7 percent of the viewed patient radiographs. Fifty-five percent of the embedded roots demonstrated the concomitant existence of partial bony ankylosis. The remaining 45 percent of the embedded roots were surrounded by an intact lamina dura and periodontal ligament space. The frequency of occurrence demonstrated an age-based relationship, showing 9 percent prevalence in the older age-group (41-50 yr.) as compared to a 45 percent occurrence seen in the younger (15-20 yr.) group. Seventy-seven percent of the embedded roots were found within the mandible as compared with a 23 percent occurrence in the maxilla.

Supernumerary teeth may be found in both the primary and permanent dentition, although they are more common in the permanent dentition. Presence of a fourth molar is rare, and such a tooth is almost invariably impacted. Dental practitioners should be aware of the possibility of encountering this rare supernumerary, its diagnosis and treatment. The authors of this article conducted a survey of patients in Montreal, looking specifically at the prevalence, aetiology, diagnosis, pathology and treatment of fourth molars. Their findings are reported here, and compared with data from the literature over the past 15 years.

The condition of first permanent molars in a cohort of children born in Cuba, during the week comprised from March the 1st. to 7, 1973, is presented now when they are 11 years old. Maxillary teeth presented a better conservation level than mandibular teeth. Non valuable differences were found in the condition of teeth of one or another hemiarch. It was found that 80.9% of the children had one or more molars affected by caries.

Disturbed eruption of the second lower molar is a condition not often described in the dental literature. To study this aberration, 88 referred patients with 108 totally or partially impacted lower second molars were examined. The mean age of these patients was 15 years; 64 percent were male. A third molar was found adjacent to the affected second molar in 95 percent of cases. Local space shortage was seen in 90 percent of the cases. The prevalence of this disturbance overall is 1.5 per 1,000 individuals. Impactions in mesioangular and distoangular positions seemed to have been caused mostly by lack of space, whereas additional local factors had complicated many vertical impactions. Bite supervision should include early observance of the molar area for detection of disturbance in eruption of the lower second molar.

The incidence of lower second molar impaction was investigated, comparing the records of two samples of 200 orthodontic patients referred consecutively, in 1976 and 1986. A further study compared 50 cases showing both bilateral and unilateral impactions, with a non-impacted control group. This allowed possible causes of the impactions to be examined, particularly concerning the effect of premature loss of deciduous teeth on the distribution of crowding.
