This study proposes a bioinspired exotendon routing configuration for a tendon-based mechanism to provide finger flexion and extension that utilizes a single motor to reduce the complexity of the system. The configuration was primarily inspired by the extrinsic muscle–tendon units of the human musculoskeletal system. The function of the intrinsic muscle–tendon units was partially compensated by adding a minor modification to the configuration of the extrinsic units. The finger kinematics produced by this solution during flexion and extension were experimentally evaluated on an artificial finger and compared to that obtained using the traditional mechanism, where one exotendon was inserted at the distal phalanx. The experiments were conducted on nine healthy subjects who wore a soft exoskeleton glove equipped with the novel tendon mechanism. Contrary to the traditional approach, the proposed mechanism successfully prevented the hyperextension of the distal interphalangeal (DIP) and the metacarpophalangeal (MCP) joints. During flexion, the DIP joint angles produced by the novel mechanism were smaller than the angles generated by the traditional approach for the same proximal interphalangeal (PIP) joint angles. This provided a flexion trajectory closer to the voluntary flexion motion and avoided straining the interphalangeal coupling between the DIP and PIP joints. Finally, the proposed solution generated similar trajectories when applied to a stiff artificial finger (simulating spasticity). Musculoskeletal findings in MPS can progress after enzyme replacement. Our aim was to examine synovial recesses, tendons, retinacula and pulleys using ultrasonography for structural and inflammatory changes. The wrist, metacarpophalangeal (MCP), proximal and distal interphalangeal (PIP and DIP) joints, the finger flexor tendons and the knee including entheses of quadriceps and patella tendons were assessed clinically. Ultrasonography of the various synovial recesses of the wrist as well as the extensor retinaculum, carpal tunnel, MCP, PIP and DIP joints of the second finger, extensor and flexor tendons, A1-5 pulleys and the knee joint including relevant entheses followed. Significance of differences between patient values and available normative data were assessed using t-tests. Ultrasonography showed significant abnormal intraarticular material in the wrist without a clear distribution to synovial recesses and without effusions. Doppler signals were found in a perisynovial distribution and not intrasynovial as expected in in inflammatory arthritis. Findings were similar in the knee but not the fingers. Flexor and extensor tendons were also mostly normal in their structure but significant thickening of retinaculae and the flexor tendon pulleys was seen (p<0.0001 compared to normal). MPS I patients showed intraarticular deposition of abnormal material in the wrist and knee but not in the finger joints where significant thickening of retinaculae/pulleys controlling tendon position was dominant. In this research a biologically inspired finger-like mechanism similar to human musculoskeletal system is developed based on Shape Memory Alloys (SMAs). SMA actuators are inspiring the design of a modular finger part with compact and compliant actuation. A three-segmented finger-like mechanism is designed and fabricated. This mechanism is composed of six bending Shape Memory Alloy (SMA) actuators. As a result, our finger mechanism is compact and compliant. The insider three SMA actuators are used for finger flexion while the outsider three SMA actuators are for extension. Each segment of this mechanism can be bent and/or extended independently by actuating a corresponding bending SMA actuator. Furthermore, full bending motion can be achieved by applying coordinated control of the three SMA actuators. Toward this goal a mathematical model of the SMA combined finger has been developed. The developed mathematical model is then used to design a proportional-derivative controller for control compliant actuation of the finger-mechanism. The performance of this mechanism has been experimentally evaluated. Our experimental results verify that the SMA-based finger module can achieve the desired postures similar to a human finger. Purpose: Knowledge of rare variants of the FDP is of high clinical importance for physicians examining patients for tendon lacerations and especially for hand surgeons operating tendon injuries. Methods: During routine dissection at our Department of Anatomy both cases were observed. Results: Variations of flexor digitorum superficialis and flexor digitorum profundus muscles of the little finger were observed in two cadavers. In both cases, the flexor digitorum profundus muscle for the little finger was absent. Moreover, in the first case, the flexor digitorum superficialis muscle for the little finger was hypoplastic and in the second case it featured variable insertion. Conclusion: There were found only four cases in previous literature describing absent flexor digitorum profundus tendon without any muscle attachment to the base of the distal phalanx. Furthermore, all previously described cases were observed in living patients. To our best knowledge, a case report in cadaver has yet not been reported and is of high importance for hand surgeons examining the hand for tendon injuries. [ABSTRACT FROM AUTHOR]

When we perform a movement we generally have a clear distinction between which parts of the world constitute our body and which parts do not. However, how the sense of ownership over our body supports movement is not yet fully understood. We aimed to see whether a sense of ownership over the hand supports the performance of rapid hand movements. In three experiments (n = 48, n = 30, n = 24), we presented participants with congruent and incongruent visuotactile and visuoproprioceptive information regarding their own hand. In keeping with previous experiments, multisensory disintegration resulted in a reduction in the subjective sensation of ownership over the hand, as reflected in questionnaire responses. Following sensory stimulation, participants were required to rapidly abduct their index finger whilst the movement was tracked. We examined the hypothesis that, should a sense of ownership over the limb be necessary for generating rapid movements with that limb, reaction time would increase when hand ownership was reduced, whilst the acceleration and velocity of the movement would decrease. We observed that reductions in own hand ownership did not interfere with rapid index finger abduction, suggesting that the motor system may not be reliant on a subjective sense of ownership over the body in order to generate movement. Visual hand gesture recognition systems are promising technologies for Human Computer Interaction, as they allow a more immersive and intuitive interaction. Most of these systems are based on the analysis of skeleton information, which is in turn inferred from color, depth, or near-infrared imagery. However, the robust extraction of skeleton information from images is only possible for a subset of hand poses, which restricts the range of gestures that can be recognized. In this paper, a real-time hand gesture recognition system based on a near-infrared device is presented, which directly analyzes the infrared imagery to infer static and dynamic gestures, without using skeleton information. Thus, a much wider range of hand gestures can be recognized in comparison with skeleton-based approaches. To validate the proposed system, a new dataset of near-infrared imagery has been created, from which good results that outperform other state-of-the-art strategies have been obtained. Little is known regarding how repetitive finger movement performance impacts other fine motor control tasks, such as circle drawing, in persons with Parkinson’s disease (PD). Previous research has shown that impairments in repetitive finger movements emerge at rates near to and above 2 Hz in most persons with PD. Thus, the purpose of this study was to compare circle drawing performance in persons with PD that demonstrate impairment in repetitive finger movement and those that do not. Twenty-two participants with PD and twelve healthy older adults completed the study. Only participants with PD completed the repetitive finger movement task. From the kinematic data for the repetitive finger movement task, participants were grouped into Hasteners and Non-Hasteners. Participants with PD and the healthy older adults completed a series of circle drawing tasks at two different target sizes (1 cm and 2 cm) and three pacing conditions (Self-paced, 1.25 Hz, and 2.5 Hz). Kinematic and electromyography data were recorded and compared between groups. Results revealed that, in general, persons with PD demonstrate impairments in circle drawing and associated electromyography activity compared to healthy older adults. Moreover, persons with PD that hasten during repetitive finger movements demonstrate significantly increased movement rate during circle drawing, while those persons with PD that do not hasten demonstrate a significant increase in width variability. This suggests that differing motor control mechanisms may play a role in the performance of fine motor tasks in persons with PD. Albeit differences in climbing-specific strength of the forearms have been demonstrated between lead and boulder climbers, little is known about the potential differences in force and power output of the upper body pulling-apparatus between disciplines. The aim of this study was to compare the climbing-specific upper-body strength and finger flexor endurance between lead and boulder climbers, as well as to examine the relative utilization of force when testing on a ledge hold compared to a jug hold. Sixteen boulder climbers (red-point climbing grade 17.9 ± 3.3) and fifteen lead climbers (red-point climbing grade 20.5 ± 3.5) performing on an advanced level volunteered for the study. Peak force, average force and rate of force development (RFD) were measured during an isometric pull-up, average velocity in dynamic pull-up, and finger flexor endurance in an intermittent test to fatigue. The isometric pull-up was performed on a ledge hold (high finger strength requirements) and on a jug hold (very low finger strength requirements). Boulder climbers demonstrated a higher maximal and explosive strength in all strength and power measurements (26.2–52.9%, ES = 0.90–1.12, p = 0.006–0.023), whereas the finger flexor endurance test showed no significant difference between the groups (p = 0.088). Both groups were able to utilize 57–69% of peak force, average force and RFD in the ledge condition compared to the jug condition, but the relative utilization was not different between the groups (p = 0.290–0.996). In conclusion, boulder climbers were stronger and more explosive compared to lead climbers, whereas no differences in finger flexor endurance were observed. Objective: Mycobacterium marinum causes a rare cutaneous disease known as fish tank granuloma (FTG). The disease manifestations resemble those associated with Cutaneous Leishmaniasis (CL). The aim of this study was to determine whether FTG was the cause of cutaneous lesions in patients who were referred to the Parasitology laboratory of Imam Reza Hospital in Mashhad to be investigated for CL. Materials/Methods: One hundered patients, clinically diagnosed with CL between April 2014 and March 2015, were included in this study. Ziehl-Neelsen staining was performed to identify acid-fast Mycobacterium in addition to bacterial cultures using Löwenstein-Jensen medium. Skin lesion samples were also collected and kept on DNA banking cards for PCR testing. Results: Twenty-nine of the 100 individuals with skin lesions, and therefore suspected of suffering from CL, tested positive for Mycobacterium marinum by PCR. Of these, 21 (72.4%) were male and 8(27.6%) were female. In 97% of these cases the lesions were located on hands and fingers. These patients had a history of manipulating fish and had been in contact with aquarium water. A sporotrichoid appearance was observed in 58.6% of the patients with mycobacterial lesions; 67% of patients had multiple head appearance. Conclusion: Patients suspected to have CL and who test negative for CL could be affected by FTG. Therefore, after obtaining an accurate case history, molecular diagnosis is recommended for cases that give a negative result by conventional methods. [ABSTRACT FROM AUTHOR]

Hippocampal interactions with the motor system are often assumed to reflect the role of memory in motor learning. Here, we examine hippocampal connectivity with sensorimotor cortex during two tasks requiring paced movements, one with a mnemonic component (sequence learning) and one without (repetitive tapping). Functional magnetic resonance imaging activity was recorded from thirteen right-handed subjects; connectivity was identified from sensorimotor cortex correlations with psychophysiological interactions in hippocampal activity between motor and passive visual tasks. Finger movements in both motor tasks anticipated the timing of the metronome, reflecting cognitive control, yet evidence of motor learning was limited to the sequence learning task; nonetheless, hippocampal connectivity was observed during both tasks. Connectivity from corresponding regions in the left and right hippocampus overlapped extensively, with improved sensitivity resulting from their conjunctive (global) analysis. Positive and negative connectivity were both evident, with positive connectivity in sensorimotor cortex ipsilateral to the moving hand during unilateral movements, whereas negative connectivity was prominent in whichever hemisphere was most active during movements. Background: Hands are a route of transmission for fecal-oral pathogens. This analysis aimed to assess associations between hand E. coli contamination and child age and determine if observed hand cleanliness can serve as a proxy for E. coli contamination on young children’s hands. Methods: Trained field workers collected hand rinse samples from children aged 1–14 months in 584 households in rural Bangladesh and assessed the visual cleanliness of child hands (fingernails, finger pads and palms). Samples were analyzed using the IDEXX most probable number (MPN) methodto enumerate E. coli. We assessed if child age (immobile children aged 1–4 months vs. mobile children aged 5–14 months) is associated with log10 E. coli counts on hands using generalized estimating equations (GEE). We estimated the log10 difference in hand E. coli counts associated with the cleanliness of different hand parts using a multivariable GEE model.We calculated the sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) for dirty fingernails, fingerpads, palms and overall hands (the three observed parts combined) against binary E. coli presence on hands. Results: E. coli was detected on 43% of child hands. Children in the mobile age range had 0.17 log10 MPN higher E. coli on hands than those in the immobile age range (Δlog10 = 0.17, 95% CI = 0.02, 0.32, p = 0.03). Children with visible dirt particles on finger pads had 0.46 log10 MPN higher E. coli on hands than those with clean finger pads (Δlog10 = 0.46, 95% CI = 0.05, 0.87, p = 0.03). Dirty fingernails indicated binary E. coli presence with 81% sensitivity and 26% specificity while dirty fingerpads and palms indicated E. coli presence with 29% sensitivity and 75–77% specificity. The PPV was 45–48% and NPV 59–65% for all three types of observations. Conclusion: Hand contamination with E. coli was prevalent among young children in rural Bangladesh, with higher levels of contamination among mobile children. Studies should assess if strategies to remove animal feces from the courtyard, provide designated hygienic play spaces for children and deliver targeted messaging to mothers to wipe or wash children’s hands after contact with animals and animal feces reduce child hand contamination. Visible hand cleanliness was a poor predictor of E. coli presence on young children’s hands so other low-cost field measurements are needed to accurately detect fecal contamination on hands. This study presents a modelling framework in which information on muscle fiber direction and orientation during contraction is derived from diffusion tensor imaging (DTI) and incorporated in a computational model of the surface electromyographic (EMG) signal. The proposed model makes use of the principle of reciprocity to simultaneously calculate the electric potentials produced at the recording electrode by charges distributed along an arbitrary number of muscle fibers within the muscle, allowing for a computationally efficient evaluation of extracellular motor unit action potentials. The approach is applied to the complex architecture of the first dorsal interosseous (FDI) muscle of the hand to simulate EMG during index finger flexion and abduction. Using diffusion tensor imaging methods, the results show how muscle fiber orientation and curvature in this intrinsic hand muscle change during flexion and abduction. Incorporation of anatomically accurate muscle architecture and other hand tissue morphologies enable the model to capture variations in extracellular action potential waveform shape across the motor unit population and to predict experimentally observed differences in EMG signal features when switching from index finger abduction to flexion. The simulation results illustrate how structural and electrical properties of the tissues comprising the volume conductor, in combination with fiber direction and curvature, shape the detected action potentials. Using the model, the relative contribution of motor units of different sizes located throughout the muscle under both conditions is examined, yielding a prediction of the detection profile of the surface EMG electrode array over the muscle cross-section. In the size–weight illusion, when two objects of identical weight but different volume are lifted, the smaller object is typically perceived to weigh more than the larger object. A well-known explanation for this and other weight illusions is provided by the hypothesis that perceived weight results from the contrast between actual and expected weight. More recently, it has been suggested that an object’s size may exert a direct and automatic effect on its perceived weight, independently of expected weight. Here we test these two hypotheses by exploring two illusions that have been known for a long time but have remained relatively underexplored, namely the shape–weight and brightness–weight illusions. Specifically, we measured the influence of visually perceived shape and brightness on the perceived size, the expected weight, and the perceived weight of 3D plastic objects. A numerical rating task was used in Experiment 1, and a paired comparison task was used in Experiment 2. The results showed that spheres were perceived to be heavier than tetrahedrons and cubes, and cubes were perceived to be heavier than tetrahedrons. We did not find any consistent relationship between brightness and perceived weight. A systematic comparison between perceived size, expected weight, and perceived weight showed that the visual shape–weight and brightness–weight illusions are partially inconsistent with the hypothesis that perceived weight results from the contrast between actual and expected weight and with the hypothesis that perceived weight results from the contrast between actual weight and perceived size. The results appear to suggest that there may be a dissociation between the processing of variables that contribute to the conscious experience of size, such as brightness and vertical height, and the processing of variables that contribute to perceived weight, such as surface area. Background: New developments, based on the concept of wearable soft-robotic devices, make it possible to support impaired hand function during the performance of daily activities and intensive task-specific training. The wearable soft-robotic ironHand glove is such a system that supports grip strength during the performance of daily activities and hand training exercises at home. Design: This pilot randomized controlled clinical study explored the effect of prolonged use of the assistive ironHand glove during daily activities at home, in comparison to its use as a trainings tool at home, on functional performance of the hand. Methods: In total, 91 older adults with self-perceived decline of hand function participated in this study. They were randomly assigned to a 4-weeks intervention of either assistive or therapeutic ironHand use, or control group (received no additional exercise or treatment). All participants performed a maximal pinch grip test, Box and Blocks test (BBT), Jebsen-Taylor Hand Function Test (JTHFT) at baseline and after 4-weeks of intervention. Only participants of the assistive and therapeutic group completed the System Usability Scale (SUS) after the intervention period. Results: Participants of the assistive and therapeutic group reported high scores on the SUS (mean = 73, SEM = 2). The therapeutic group showed improvements in unsupported handgrip strength (mean Δ = 3) and pinch strength (mean Δ = 0.5) after 4 weeks of ironHand use (p≤0.039). Scores on the BBT and JTHFT improved not only after 4 weeks of ironHand use (assistive and therapeutic), but also in the control group. Only handgrip strength improved more in the therapeutic group compared to the assistive and control group. No significant correlations were found between changes in performance and assistive or therapeutic ironHand use (p≥0.062). Conclusion: This study showed that support of the wearable soft-robotic ironHand system either as assistive device or as training tool may be a promising way to counter functional hand function decline associated with ageing. This study explored gender differences in correct response rates and response times on a task involving left or right arrow selection and another involving the transformation of mental rotation of the hand. We recruited 15 healthy, right-handed men (age 24.5 ± 6.4) and 15 healthy, right-handed women (age 21.3 ± 4.9). For the tasks, we used pictures of left and right arrows and 32 hand pictures (left and right, palm and back) placed in cons (each at 45° from 0° to 315°). Hand and arrow pictures alternated and were shown at random. Participants decided as quickly as possible whether each picture was left or right. To compare the time taken for the transformation of mental rotation of the hand, we subtracted the average arrow response time from that for the left and right hand pictures for each participant. Correct response rates did not differ significantly between men and women or left and right for either arrow or hand pictures. Regardless of gender, the response time was longer for the left arrow picture than right arrow picture. The response time for the hand picture was longest for both men and women for pictures at rotation angles that were most difficult to align with participants’ hands. While there was no difference between men’s responses for left and right hand pictures, the responses of women were longer for left than right hand pictures and also than those of men. These findings suggest that both men and women mainly perform the hand mental rotation task with implicit motor imagery. On the other hand, the gender difference in performance might be explained by the difference in balance with other strategies, such as visual imagery, and by cognitive, neurophysiological, and morphological differences. While designing a dynamic hand orthosis to assist during activities of daily living, the designer has to know whether a concept will have sufficient grasp performance to support these activities. This is often estimated by measuring the interaction force at the contact interface. However, this requires a prototyping step and limits the practicality of comparing several concepts in an early design stage. Alternatively, this study presents and compares basic static and dynamic models to numerically estimate grasp performance. This was applied on an exemplary concept for a hydraulically operated hand orthosis grasping a circular object. The models were validated with an experimental set-up that does not require sensors at the contact interface. Static and dynamic model results were almost identical, where the static model could be around 10 times faster and is generally more robust to a high contact stiffness. Both models were unable to make accurate quantitative predictions, which is believed to be due to differences in used contact stiffness. However, the models were able to make correct qualitative comparisons, making it a valid method to compare and choose concepts in an early design stage. [ABSTRACT FROM AUTHOR]

A progressive decline in upper limb function is associated with ageing and disease. In this cross-sectional study we assessed the performance of 367 healthy individuals aged of 20 to 95 years across a battery of upper limb clinical tests, which we have termed the upper limb Physiological Profile Assessment (PPA). The upper limb PPA was designed to quantify the performance of the multiple physiological domains important for adequate function in the upper extremities. Included are tests of muscle strength, unilateral movement and dexterity, position sense, skin sensation, bimanual coordination, arm stability, along with a functional task. We report age and gender normative values for each test. Test-retest reliability ranged from good to excellent in all tests (intra-class correlation coefficients from 0.65 to 0.98) with the exception of position sense (0.31). Ten of the thirteen tests revealed differences in performance between males and females, twelve showed a decline in performance with increasing age, and eight discriminated between older people with and without upper limb functional impairment. Furthermore, most tests showed good external validity with respect to age, an upper limb functional test and self-reported function. This profiling approach provides a reference range for clinical groups with upper limb sensory and motor impairments and may assist in identifying undiagnosed deficits in the general population. Giancardo et al. recently introduced the neuroQWERTY index (nQi), which is a novel motor index derived from computer-key-hold-time data using an ensemble regression algorithm, to detect early-stage Parkinson’s disease. Here, we derive a much simpler motor index from their hold-time data, which is the standard deviation (SD) of the hold-time fluctuations, where fluctuation is defined as the difference between successive natural-log of hold time. Our results show the performance of the SD and nQi tests in discriminating early-stage subjects from controls do not differ, although the SD index is much simpler. Hand motor impairment is common after stroke but there are few comprehensive data on amount of hand movement. This study aimed to compare the amount of thumb and finger movement over an extended period of time in people with stroke and able-bodied people. Fifteen stroke subjects and 15 able-bodied control subjects participated. Stroke subjects had impaired hand function. Movement of the thumb and index finger was recorded using stretch sensors worn on the affected hand (stroke subjects) or the left or right hand (control subjects) for ∼4 hours during the day. A digit movement was defined as a monotonic increase or decrease in consecutive sensor values. Instantaneous digit position was expressed as a percentage of maximal digit flexion. Mixed linear models were used to compare the following outcomes between groups: (1) average amplitude of digit movement, (2) digit cadence and average digit velocity, (3) percentage of digit idle time and longest idle time. Amplitude of digit movement was not different between groups. Cadence at the thumb (between-group mean difference, 95% CI, p value: -0.6 movements/sec, -1.0 to -0.2 movements/sec, p = 0.003) and finger (-0.5 movements/sec, -0.7 to -0.3 movements/sec, p<0.001) was lower in stroke than control subjects. Digit velocity was not different between groups. Thumb idle time was not different between groups, but finger idle time was greater in stroke than control subjects (percentage of idle time: 6%, 1 to 11%, p = 0.02; longest idle time: 375 sec, 29 to 721 sec, p = 0.04). Rehabilitation after stroke should encourage the performance of functional tasks that involve movements at faster cadences, and encourage more frequent movement of the digits with shorter periods of inactivity. The ability to efficiently perform actions immediately following instructions and without prior practice has previously been termed Rapid Instructed Task Learning (RITL). In addition, it was found that instructions are so powerful that they can produce automatic effects, reflected in activation of the instructions in an inappropriate task context. RITL is hypothesized to rely on limited working memory (WM) resources for holding not-yet implemented task rules. Similarly, automatic effects of instructions presumably reflect the operation of task rules kept in WM. Therefore, both were predicted to be influenced by WM load. However, while the involvement of WM in RITL is implicated from prior studies, evidence regarding WM involvement in instructions-based automaticity is mixed. In the current study, we manipulated WM load by increasing the number of novel task rules to be held in WM towards performance in the NEXT paradigm. In this task, participants performed a series of novel tasks presented in mini-blocks, each comprising a) instructions of novel task rules; b) a NEXT phase measuring the automatic activation of these instructed rules, in which participants advance the screen using a key-press; and c) a GO phase in which the new rules are first implemented and RITL is measured. In three experiments, we show a dissociation: While RITL (rule implementation) was impaired by increased WM load, the automatic effects of instructions were not robustly influenced by WM load. This study examines two contrasting explanations for early tendencies to fight and flee. According to a stimulus-driven explanation, goal-incompatible stimuli that are easy/difficult to control lead to the tendency to fight/flee. According to a goal-directed explanation, on the other hand, the tendency to fight/flee occurs when the expected utility of fighting/fleeing is the highest. Participants did a computer task in which they were confronted with goal-incompatible stimuli that were (a) easy to control and fighting had the highest expected utility, (b) easy to control and fleeing had the highest expected utility, and (c) difficult to control and fleeing and fighting had zero expected utility. After participants were trained to use one hand to fight and another hand to flee, they either had to choose a response or merely observe the stimuli. During the observation trials, single-pulse Transcranial Magnetic Stimulation (TMS) was applied to the primary motor cortex 450 ms post-stimulus onset and motor-evoked potentials (MEPs) were measured from the hand muscles. Results showed that participants chose to fight/flee when the expected utility of fighting/fleeing was the highest, and that they responded late when the expected utility of both responses was low. They also showed larger MEPs for the right/left hand when the expected utility of fighting/fleeing was the highest. This result can be interpreted as support for the goal-directed account, but only if it is assumed that we were unable to override the presumed natural mapping between hand (right/left) and response (fight/flight). Prior to goal-directed actions, somatosensory target positions can be localized using either an exteroceptive or an interoceptive body representation. The goal of the present study was to investigate if the body representation selected to plan reaches to somatosensory targets is influenced by the sensory modality of the cue indicating the target’s location. In the first experiment, participants reached to somatosensory targets prompted by either an auditory or a vibrotactile cue. As a baseline condition, participants also performed reaches to visual targets prompted by an auditory cue. Gaze-dependent reaching errors were measured to determine the contribution of the exteroceptive representation to motor planning processes. The results showed that reaches to both auditory-cued somatosensory targets and auditory-cued visual targets exhibited larger gaze-dependent reaching errors than reaches to vibrotactile-cued somatosensory targets. Thus, an exteroceptive body representation was likely used to plan reaches to auditory-cued somatosensory targets but not to vibrotactile-cued somatosensory targets. The second experiment examined the influence of using an exteroceptive body representation to plan movements to somatosensory targets on pre-movement neural activations. Cortical responses to a task-irrelevant visual flash were measured as participants planned movements to either auditory-cued somatosensory or auditory-cued visual targets. Larger responses (i.e., visual-evoked potentials) were found when participants planned movements to somatosensory vs. visual targets, and source analyses revealed that these activities were localized to the left occipital and left posterior parietal areas. These results suggest that visual and visuomotor processing networks were more engaged when using the exteroceptive body representation to plan movements to somatosensory targets, than when planning movements to external visual targets. In recent years, safety issues surrounding robots have increased in importance, as more robots are in close contact with humans, both in industrial fields and elsewhere. Safety standards for industrial robots operating in specific spaces have been established, but no such standards have been specified for collaborative and service robots. To establish safety standards for such robots, we assessed pressure pain thresholds for collisions between humans and robots, under the assumption that the pain threshold is lower than the mild injury threshold. The pressure pain threshold for collision with a robot was measured in 90 male Korean adults using a homemade collision system. The pain thresholds were measured three times at 15 sites, including the forehead. The highest threshold was 196.1 ± 85.8 N/cm2 at the back of the hand, and the lowest was 65.1 ± 22.6 N/cm2 at an arm nerve. Moderate thresholds, i.e., 100–120 N/cm2, were noted on the forehead, neck muscle, ball of the thumb, and shin. The thresholds of participants < 30 years of age were lower, by 3–33%, than those of participants aged > 30 years. Thresholds differed by body mass index only at certain sites, including the shoulder joint, neck, and back of the hand. The pressure pain threshold depended on individual characteristics, body site, and age. The threshold relevant to potential human-robot collisions was determined to be between 65.1 ± 22.6 and 196.1 ± 85.8 N/cm2. [ABSTRACT FROM AUTHOR]

What concepts and words do communicative gestures activate in the minds of people who view them? It’s widely believed that many gestures grow from iconic origins—they look like what they mean—but also that at some point they may become emblematic—conventionalized as culturally agreed-upon symbols. How long do links between physical movements of the body and the things in the world they denote persist in the minds of gesture-users? A pair of experiments asks this question for the Middle-Finger, a cross-culturally recognized obscene gesture. The prevailing view is that the gesture originates in a phallic symbol. Yet it is now predominantly used as an emblematic gesture displaying contempt (among other things). It is currently unknown whether the iconic origins of gestures persist through the emblematic stage in the minds of gesture users. Two experiments tested the hypothesis that viewing the Middle-Finger primes thoughts about penises or the word penis. The results showed that the Middle-Finger induced no priming of penis compared with control, unlike another obscene penis-representing gesture (Finger-Bang), which did. This suggests that the Middle-Finger no longer activates thoughts of penises in the minds of contemporary American English speakers. Emblematic gestures with iconic origins may undergo historical change not just in the functions they serve but also in the effects they have on the minds of people who use them. It is well known that people who read print or braille sometimes make eye or finger movements against the reading direction. The way these regressions are elicited has been studied in detail by manipulating linguistic aspects of the reading material. Actually, it has been shown that reducing the physical intensity or clarity of the visual input signal can also lead to increased regressions during reading. We asked whether the same might be true in the haptic realm while reading braille. We set the height of braille dots at three different levels (high, medium, and low) and asked adult blind, practiced braille readers to read standardized texts without any repetition of content. The results show that setting the braille dot height near the tactile threshold significantly increased the frequency of regressive finger movements. Additionally, at the lowest braille dot height, braille reading speed significantly diminished. These effects did not occur at braille dot heights that were closer to the height of standard braille (medium and high). We tentatively conclude that this effect may be due to a heightened sense of uncertainty elicited by perception near the threshold that seems to be common to the reading process, independent of the sensory input modality. In the Eriksen flanker task as well as in the Simon task irrelevant activation produces a response conflict that has to be resolved by mental control mechanisms. Despite these similarities, however, the tasks differ with respect to their delta functions, which express how the congruency effects develop with response time. The slope of the delta function is mostly positive for the flanker task, but negative for the Simon task. Much effort has been spent to explain this difference and to investigate whether it results from task-specific control. A prominent account is that the temporal overlap between irrelevant and relevant response activation is larger in the flanker task than in the Simon task. To test this hypothesis, we increased the temporal distance in a flanker task by presenting the flankers ahead of the target. This not only produced negatively sloped delta functions but also caused reversed congruency effects. We also conducted a Simon-task experiment in which we varied the proportion of congruent stimuli. As a result, the delta function was negatively sloped only if the proportion was low. These results demonstrate that a long temporal distance is necessary but not sufficient for observing negatively sloped delta functions. Finally, we modeled the data with drift-diffusion models. Together, our results show that differently sloped delta functions can be produced with both tasks. They further indicate that activation suppression is an important control mechanism that can be adapted rather flexibly to the control demands. [ABSTRACT FROM AUTHOR]

The present study examines the visual recognition of action simulations by finger gestures (ASFGs) produced by sighted and blind individuals. In ASFGs, fingers simulate legs to represent actions such as jumping, spinning, climbing, etc. The question is to determine whether the common motor experience of one’s own body is sufficient to produce adequate ASFGs or whether the possibility to see gestures from others are also necessary to do it. Three experiments were carried out to address this question. Experiment 1 examined in 74 sighted adults the recognition of 18 types of ASFGs produced by 20 blindfolded sighted adults. Results showed that rates of correct recognition were globally very high, but varied with the type of ASFG. Experiment 2 studied in 91 other sighted adults the recognition of ASFGs produced by 10 early blind and 7 late blind adults. Results also showed a high level of recognition with a similar order of recognizability by type of ASFG. However, ASFGs produced by early blind individuals were more poorly recognized than those produced by late blind individuals. In order to match data of recognition obtained with the form that gestures are produced by individuals, two independant judges evaluated prototypical and atypical attributes of ASFG produced by blindfolded sighted, early blind and late blind individuals in Experiment 3. Results revealed the occurrence of more atypical attributes in ASFG produced by blind individuals: their ASFGs transpose more body movements from a character-viewpoint in less agreement with visual rules. The practical interest of the study relates to the relevance of including ASFGs as a new exploratory procedure in tactile devices which are more apt to convey action concepts to blind users/readers. Miniaturised frogs form a fascinating but poorly understood amphibian ecomorph and have been exceptionally prone to taxonomic underestimation. The subfamily Cophylinae (family Microhylidae), endemic to Madagascar, has a particularly large diversity of miniaturised species which have historically been attributed to the single genus Stumpffia largely based on their small size. Recent phylogenetic work has revealed that several independent lineages of cophyline microhylids evolved towards highly miniaturised body sizes, achieving adult snout–vent lengths under 16 mm. Here, we describe five new species belonging to three clades that independently miniaturised and that are all genetically highly divergent from their relatives: (i) a new genus (Mini gen. nov.) with three new species from southern Madagascar, (ii) one species of Rhombophryne, and (iii) one species of Anodonthyla. Mini mum sp. nov. from Manombo in eastern Madagascar is one of the smallest frogs in the world, reaching an adult body size of 9.7 mm in males and 11.3 mm in females. Mini scule sp. nov. from Sainte Luce in southeastern Madagascar is slightly larger and has maxillary teeth. Mini ature sp. nov. from Andohahela in southeast Madagascar is larger than its congeners but is similar in build. Rhombophryne proportionalis sp. nov. from Tsaratanana in northern Madagascar is unique among Madagascar’s miniaturised frogs in being a proportional dwarf, exhibiting far less advanced signs of paedomorphism than other species of similar size. Anodonthyla eximia sp. nov. from Ranomafana in eastern Madagascar is distinctly smaller than any of its congeners and is secondarily terrestrial, providing evidence that miniaturisation and terrestriality may be evolutionarily linked. The evolution of body size in Madagascar’s microhylids has been more dynamic than previously understood, and future studies will hopefully shed light on the interplay between ecology and evolution of these remarkably diverse frogs. The use of wrist-worn accelerometers has recently gained tremendous interest among researchers and clinicians as an objective tool to quantify real-world use of the upper limbs during the performance of activities of daily living (ADLs). However, wrist-worn accelerometers have shown a number of limitations that hinder their adoption in the clinic. Among others, the inability of wrist-worn accelerometers to capture hand and finger movements is particularly relevant to monitoring the performance of ADLs. This study investigates the use of finger-worn accelerometers to capture both gross arm and fine hand movements for the assessment of real-world upper-limb use. A system of finger-worn accelerometers was utilized to monitor eighteen neurologically intact young adults while performing nine motor tasks in a laboratory setting. The system was also used to monitor eighteen subjects during the day time of a day in a free-living setting. A novel measure of real-world upper-limb function—comparing the duration of activities of the two limbs—was derived to identify which upper limb subjects predominantly used to perform ADLs. Two validated handedness self-reports, namely the Waterloo Handedness Questionnaire and the Fazio Laterality Inventory, were collected to assess convergent validity. The analysis of the data recorded in the laboratory showed that the proposed measure of upper-limb function is suitable to accurately detect unilateral vs. bilateral use of the upper limbs, including both gross arm movements and fine hand movements. When applied to recordings collected in a free-living setting, the proposed measure showed high correlation with self-reported handedness indices (i.e., ρ = 0.78 with the Waterloo Handedness Questionnaire scores and ρ = 0.77 with the Fazio Laterality Inventory scores). The results herein presented establish face and convergent validity of the proposed measure of real-world upper-limb function derived using data collected by means of finger-worn accelerometers. Purpose: To compare the diagnostic performance and raters´confidence of radiography, radiography equivalent dose multi-detector computed tomography (RED-MDCT) and radiography equivalent dose cone beam computed tomography (RED-CBCT) for finger fractures. Methods: Fractures were inflicted artificially and randomly to 10 cadaveric hands of body donors. Radiography as well as RED-MDCT and RED-CBCT imaging were performed at dose settings equivalent to radiography. Images were de-identified and analyzed by three radiologists regarding finger fractures, joint involvement and confidence with their findings. Reference standard was consensus reading by two radiologists of the fracturing protocol and high-dose multi-detector computed tomography (MDCT) images. Sensitivity and specificity were calculated and compared with Cochrane´s Q and post hoc analysis. Rater´s confidence was calculated with Friedman Test and post hoc Nemenyi Test. Results: Rater´s confidence, inter-rater correlation, specificity for fractures and joint involvement were higher in RED-MDCT and RED-CBCT compared to radiography. No differences between the modalities were found regarding sensitivity. Conclusion: In this phantom study, radiography equivalent dose computed tomography (RED-CT) demonstrates a partly higher diagnostic accuracy than radiography. Implementing RED-CT in the diagnostic work-up of finger fractures could improve diagnostics, support correct classification and adequate treatment. The current study seeks confirmation for the hypothesis that 2D:4D (positively) predicts prosociality when people are more likely to rely on intuition than deliberation. We assess intuition and deliberation using the Cognitive Reflection Test (CRT) and measure prosociality via the validated Social Value Orientation (SVO) slider measure. Although our results do not provide collective evidence for our main proposition, we observe in the data that for low (right) 2D:4D men, the more intuitive they are, the less prosocial they become, whereas for high (right) 2D:4D men the thinking style does not affect their prosociality. Importantly, we find that two alternative measures of cognitive reflection, CRT and CRT-2, differently relate to prosocial decision making such that only CRT-2 (but not the classic CRT) positively predicts prosociality. Given that previous research on the role of cognitive reflection and 2D:4D in prosocial decision making provided inconsistent results, the present study findings are highly valuable to get a better understanding in this domain of study. It has been hypothesized that force can be transmitted between adjacent muscles. Intermuscle force transmission violates the assumption that muscles act in mechanical isolation, and implies that predictions from biomechanical models are in error due to mechanical interactions between muscles, but the functional relevance of intermuscle force transmission is unclear. To investigate intermuscle force transmission between human flexor pollicis longus and the index finger part of flexor digitorum profundus, we compared finger flexion force produced by passive thumb flexion after one of three conditioning protocols: passive thumb flexion-extension cycling, thumb flexion maximal voluntary contraction (MVC), and thumb extension stretch. Finger flexion force increased after all three conditions. Compared to passive thumb flexion-extension cycling, change in finger flexion force was less after thumb extension stretch (mean difference 0.028 N, 95% CI 0.005 to 0.051 N), but not after thumb flexion MVC (0.007 N, 95% CI -0.020 to 0.033 N). As muscle conditioning changed finger flexion force produced by passive thumb flexion, the change in force is likely due to intermuscle force transmission. Thus, intermuscle force transmission resulting from passive stretch of an adjacent muscle is probably small enough to be ignored. [ABSTRACT FROM AUTHOR]

Imitation and perspective taking are core features of non-verbal social interactions. We imitate one another to signal a desire to affiliate and consider others’ points of view to better understand their perspective. Prior research suggests that a relationship exists between prosocial behaviour and imitation. For example, priming prosocial behaviours has been shown to increase imitative tendencies in automatic imitation tasks. Despite its importance during social interactions, far less is known about how perspective taking might relate to either prosociality or imitation. The current study investigates the relationship between automatic imitation and perspective taking by testing the extent to which these skills are similarly modulated by prosocial priming. Across all experimental groups, a surprising ceiling effect emerged in the perspective taking task (the Director’s Task), which prevented the investigation of prosocial priming on perspective taking. A comparison of other studies using the Director’s Task shows wide variability in accuracy scores across studies and is suggestive of low task reliability. In addition, despite using a high-power design, and contrary to three previous studies, no effect of prosocial prime on imitation was observed. Meta-analysing all studies to date suggests that the effects of prosocial primes on imitation are variable and could be small. The current study, therefore, offers caution when using the computerised Director’s Task as a measure of perspective taking with adult populations, as it shows high variability across studies and may suffer from a ceiling effect. In addition, the results question the size and robustness of prosocial priming effects on automatic imitation. More generally, by reporting null results we hope to minimise publication bias and by meta-analysing results as studies emerge and making data freely available, we hope to move towards a more cumulative science of social cognition. The rubber hand illusion describes a phenomenon in which participants experience a rubber hand as being part of their body by the synchronous application of visuotactile stimulation to the real and the artificial limb. In the recently introduced robotic hand illusion (RobHI), a robotic hand is incorporated into one’s body representation due to the integration of synchronous visuomotor information. However, there are no setups so far that combine visuotactile and visuomotor feedback, which is expected to unravel mechanisms that cannot be detected in experimental designs applying this information in isolation. We developed a robotic hand, controlled by a sensor glove and equipped with pressure sensors, and varied systematically and separately the synchrony for motor feedback (MF) and tactile feedback (TF). In Experiment 1, we implemented a ball-grasping task and assessed the perceived proprioceptive drift of one’s own hand as a behavioral measure of the spatial calibration of body coordinates as well as explicit embodiment experiences by a questionnaire. Results revealed significant main effects of both MF and TF for proprioceptive drift data, but we only observed main effects for MF on perceived embodiment. Furthermore, for the proprioceptive drift we found that synchronous feedback in one factor compensates for asynchronous feedback in the other. In Experiment 2, including a new sample of naïve participants, we further explored this finding by adding unimodal conditions, in which we manipulated the presence or absence of MF and/or TF. These findings replicated the results from Experiment 1 and we further found evidence for a supper-additive multisensory effect on spatial body representation caused by the presence of both factors. Results on conscious body perception were less consistent across both experiments. The findings indicate that sensory and motor input equally contribute to the representation of spatial body coordinates which for their part are subject to multisensory enhancing effects. The results outline the potential of human-in-the-loop approaches and might have important implications for clinical applications such as for the future design of robotic prostheses. Previous studies revealed that healthy individuals consistently misjudge the size and shape of their hidden hand during a localisation task. Specifically, they overestimate the width of their hand and underestimate the length of their fingers. This would also imply that the same individuals misjudge the actual location of at least some parts of their hand during the task. Therefore, the primary aim of the current study was to determine whether healthy individuals could accurately locate the actual position of their hand when hidden from view, and whether accuracy depends on the type of localisation task used, the orientation of the hidden hand, and whether the left or right hand is tested. Sixteen healthy right-handed participants performed a hand localisation task that involved both pointing to and verbally indicating the perceived position of landmarks on their hidden hand. Hand position was consistently misjudged as closer to the wrist (proximal bias) and, to a lesser extent, away from the thumb (ulnar bias). The magnitude of these biases depended on the localisation task (pointing vs. verbal), the orientation of the hand (straight vs. rotated), and the hand tested (left vs. right). Furthermore, the proximal location bias increased in size as the duration of the experiment increased, while the magnitude of ulnar bias remained stable through the experiment. Finally, the resultant maps of perceived hand location appear to replicate the previously reported overestimation of hand width and underestimation of finger length. Once again, the magnitude of these distortions is dependent on the task, orientation, and hand tested. Grasp affordances in robotics represent different ways to grasp an object involving a variety of factors from vision to hand control. A model of grasp affordances that is able to scale across different objects, features and domains is needed to provide robots with advanced manipulation skills. The existing frameworks, however, can be difficult to extend towards a more general and domain independent approach. This work is the first step towards a modular implementation of grasp affordances that can be separated into two stages: approach to grasp and grasp execution. In this study, human experiments of approaching to grasp are analysed, and object-independent patterns of motion are defined and modelled analytically from the data. Human subjects performed a specific action (hammering) using objects of different geometry, size and weight. Motion capture data relating the hand-object approach distance was used for the analysis. The results showed that approach to grasp can be structured in four distinct phases that are best represented by non-linear models, independent from the objects being handled. This suggests that approaching to grasp patterns are following an intentionally planned control strategy, rather than implementing a reactive execution. [ABSTRACT FROM AUTHOR]

Object manipulation requires well-coordinated force vectors involving both magnitudes and directions. Despite extensive studies about force magnitudes during manipulation, relatively little is known how the muscle fatigue could affect the directional coordination of fingertip forces. This study aims to examine the effects of muscle fatigue on inter-digit coordination of force directions during precision grip. Sixteen female subjects performed precision grip with their thumb and index finger before and after fatigue tasks, which required subjects to produce continuous submaximal pinch strength on the apparatus for a duration more than 200 s. Both their left and right hands were evaluated using the same testing protocol. The means and standard deviations of the coordination angle and the projection angle were applied to quantify the directional coordination across the digits and the force vector direction of each individual digit. Results showed that fatigue led to significant reduction in the mean values of coordination angle and that of projection angle of the index finger in the ipsilateral hand (p < 0.05). Meanwhile, fatigue induced increases in both the standard deviations of coordination angle and projection angle of both digits in the ipsilateral hand (p < 0.05). These results imply that the muscle fatigue could interfere with the grasping stability by altering the directional coordination of all the involved digits and the control of force directions for each individual digit. Audio-motor coordination is a fundamental requirement in the learning and execution of sequential actions such as music performance. Predictive motor control mechanisms determine the sequential content and timing of upcoming tones and thereby facilitate accurate performance. To study the role of auditory-motor predictions at early stages of acquiring piano performance skills, we conducted an experiment in which non-musicians learned to play a musical sequence on the piano in synchrony with a metronome. Three experimental conditions compared errors and timing. The first consisted of normal auditory feedback using conventional piano key-to-tone mappings. The second employed fixed-pitch auditory feedback consisting of a single tone that was given with each key stroke. In the third condition, for each key stroke a tone was randomly drawn from the set of tones associated with the normal sequence. The results showed that when auditory feedback tones were randomly assigned, participants produced more sequencing errors (i.e., a higher percentage of incorrect key strokes) compared to when auditory feedback was normal or consisted of a single tone of fixed pitch. Furthermore, synchronization with the metronome was most accurate in the fixed-pitch single-tone condition. These findings suggest that predictive motor control mechanisms support sequencing and timing, and that these sensorimotor processes are dissociable even at early stages of acquiring complex motor skills such as music performance. Observing others’ actions desynchronizes electroencephalographic (EEG) rhythms and modulates corticospinal excitability as assessed by transcranial magnetic stimulation (TMS). However, it remains unclear if these measures reflect similar neurofunctional mechanisms at the individual level. In the present study, a within-subject experiment was designed to assess these two neurophysiological indexes and to quantify their mutual correlation. Participants observed reach-to-grasp actions directed towards a small (precision grip) or a large object (power grip). We focused on two specific time points for both EEG and TMS. The first time point (t1) coincided with the maximum hand aperture, i.e. the moment at which a significant modulation of corticospinal excitability is expected. The second (t2), coincided with the EEG resynchronization occurring at the end of the action, i.e. the moment at which a hypothetic minimum for action observation effect is expected. Results showed a Mu rhythm bilateral desynchronization at t1 with differential resynchronization at t2 in the two hemispheres. Beta rhythm was more desynchronized in the left hemisphere at both time points. These EEG differences, however, were not influenced by grip type. Conversely, motor potentials evoked by TMS in an intrinsic hand muscle revealed an interaction effect of grip and time. No significant correlations between Mu/Beta rhythms and motor evoked potentials were found. These findings are discussed considering the spatial and temporal resolution of the two investigated techniques and argue over two alternative explanations: i. each technique provides different measures of the same process or ii. they describe complementary features of the action observation network in humans. A basic process in regulating behavior that helps us to disentangle meaningful from distracting information is the binding of stimulus and response features into stimulus-response episodes or “event files”. Recent studies have shown that even irrelevant information is bound into event files; distractor repetition on the next trial can trigger the response encoded in this episode, which is indicated by faster reaction times. The present study was conducted to get further insight into the electrophysiological underpinnings of those distractor-based retrieval. For that, we analyzed the N2, a negative deflection in event-related potentials that has been associated with a multitude of processes occurring when relevant and irrelevant stimuli compete with each other within a given trial or even in sequences of trials. Our study showed that distractor which did not provide useful information regarding the required behavior led to more negative N2 amplitudes, whereas distractors that provide useful response-related information were associated with less negative N2 amplitudes. Our results are explained as an adaptive mechanism that helps to hedge against invalid stimulus-response-bindings before an error occurs to increase efficiency of human behavior. The Rubber Hand Illusion (RHI) has been widely used to investigate the perception of the bodily self. Commonly used measures of the illusion are self-report questionnaires and proprioceptive drift of the participants’ hands towards the rubber hand. Recent studies have shown that these measures can be dissociated, suggesting they may arise from distinct mechanisms. In previous studies using questionnaires, participants were asked to base responses on their subjective feelings of body ownership, rather than their beliefs. This makes sense given the obvious fact that whereas participants may feel like the rubber hand is part of their body, they do not believe that it is. It is not clear, however, whether a similar dissociation between feelings and beliefs also exists for proprioceptive drift. Here, we investigated the presence of a dissociation between feeling and belief in the context of the RHI. When participants reported their feelings there was an increase both in the sense of body ownership over the fake hand as well as in the proprioceptive drift, compared to when they reported their beliefs. Strikingly, unlike the sense of ownership, proprioceptive drift was unaffected by the synchrony of stimulation. This may be an important way in which the two measures of the RHI differ. Symptomatic hyperventilation (SH) is a pathological condition that manifests with breathlessness, dyspnea, light-headedness, anxiety, and paresthesia. However, little is known about the prevalence of SH and its association with health-related quality of life (HRQoL) in a young population. The Nijmegen questionnaire (NQ), which measures severity of SH, had not previously been cross-culturally translated into Korean. In this study, the NQ was cross-culturally translated into Korean (KNQ), using translation and back-translation methods. To examine the reliability and validity levels of the KNQ, as well as its association with HRQoL, 237 college students (21.38 ± 2.45 years) were asked to complete the KNQ, the Korean version of the general health questionnaire (K-GHQ-30) and the short form-36 (K-SF-36). The KNQ showed satisfactory reliability (Cronbach’s α = 0.878). In the construct validity test, four factors (neuropsychological, respiratory, neurogastrointestinal, and neuromuscular) were extracted (% of total variance = 59.8). Using a KNQ cut-off score of 23 points, the prevalence of SH was 22.8%. Physical and mental HRQoL levels estimated by the K-GHQ-30 score and the 8 subscale scores of the K-SF-36 were lower in the SH group than in those of the non-SH group. It is concluded that the cross-culturally translated KNQ is reliable and valid, and management of SH may prevent a reduction in physical and mental HRQoL in a young population. Purpose: To investigate the difference between sonographic findings in extensor pollicis longus tendons rupture and other finger tendons rupture in patients sustaining hand and wrist trauma. Methods: Twenty-four patients who presented with signs and symptoms clinically suspicious for tendon injury and surgically confirmed tendon rupture were included in this study. We analyzed 6 sonographic features: discontinuity of the tendon, pseudomass formation, decreased echogenicity of the tendon, retraction of the ruptured tendon, fluid collection within the tendon sheath, and the motion of the tendon. We compared the sonographic features of ruptured extensor pollicis longus tendons with the other ruptured finger tendons. Results: Discontinuity of the tendon was the most common sonographic findings and retraction of the ruptured tendon was the second most common findings. Fourteen of 16 cases with a dynamic study on sonography showed loss of normal motion of the tendon. Pseudomass formation was the second most common feature in ruptured extensor pollicis longus tendons, in contrast to the other ruptured finger tendons (p < 0.05). Conclusion: Using ultrasonography, detection of discontinuity of the tendon, retraction of the ruptured tendon, and limitation of tendon motion could be very helpful for diagnosing a tendon rupture in hand and wrist trauma. Scinax is a speciose genus of Neotropical hylid frogs. We describe a new species from western Ecuador (elevations between 0 and 1207 m) using morphology, vocalizations, and DNA sequences. We also present a new phylogeny for Scinax based on mitochondrial DNA genes 12S rRNA, Cytochrome Oxidase sub-unit I, Cytochrome B, 16S rRNA, NADH dehydrogenase subunit 1, and adjacent tRNAs. The new species, Scinax tsachila sp. nov. was previously confused with S. quinquefasciatus, a morphologically similar sympatric species. They differ by having markedly different advisement calls, distinct skin texture in the dorsum, and different bone coloration. The new species is sister to S. elaeochroa, a species that differs in advertisement call and color pattern. The autonomic response to pain might discriminate among consciousness disorders. Therefore, aim of this study was to describe differences between minimally conscious state (MCS) and unresponsive wakefulness syndrome (UWS) patients in their autonomic response to a nociceptive stimulus. ECG, respiration, finger blood pressure (BP) and total peripheral resistances (TPR) were continuously recorded before, during and after a standardized noxious stimulus in 20 adult brain-injured patients, 14 in UWS and 6 in MCS. Occurrence of fast autonomic responses synchronous with the stimulus was detected by visual inspection of the tracings; short-term (<20 s) and long-term (between 20s and 50 s from the stimulus) responses were evaluated by beat-by-beat quantitative analysis. The noxious stimulus elicited fast responses in both groups, but only MCS patients showed a significant short-term response in TPR and long-term response in HR. Thus, short- and long-term cardiovascular responses to pain might integrate neuro-behavioural assessments to discriminate between MCS and UWS. [ABSTRACT FROM AUTHOR]

We describe a new rainfrog species (Pristimantis), from the wetland complex Oña, Nabón, Saraguro and Yacuambi, in the Andes of southern Ecuador, at altitudes ranging between 3000–3400 m a.s.l. Pristimantis tiktik sp. nov. is a small frog, displaying sexual dimorphism (the males with dorsum of various shades of gray, brown, orange or green and a whitish or pinkish yellow venter; females with brownish gray or gray dorsum and a reticulated white and black venter), with SVL ranging between 19.7–20.4 mm in females (n = 3) and 16.1–18.4 mm in males (n = 6). The skin on dorsum is tuberculated, that on venter is coarsely areolate, dorsolateral folds are absent, tympanic membrane is absent but the tympanic annulus is evident, cranial crests are absent, discs on fingers just slightly expanded, heel is lacking enlarged tubercles, inner edge of tarsus is bearing a long fold, Toe V is slightly longer than Toe III and the iris coloration is bronze with fine black reticulations. The males have a large subgular vocal sac that extends onto the chest and vocal slits but lack nuptial pads. The unique advertisement call consists of long duration series of periodically repeated clicks: “tik”. Molecular analyses place the new species in the recently resurrected P. orestes group, as the sister species of the assemblage that contains P. bambu, P. mazar, P. simonbolivari and an undescribed species. Background: Hydroquinone-containing creams cause false increases in capillary glycemia. However, the magnitude of this false increase, and the means to reverse it have not been investigated. Objective: To evaluate the technical and clinical impact of hydroquinone-containing creams on capillary glycemia and investigate the efficacy of hand washing and other common practices, in reversing cream effects. Methods: We included 91 participants in a quasi-experimental study in Buea, Cameroon. After determining the hydroquinone content of a cream, Caro Light, we used two glucometers with different enzymatic systems (Accu-Chek Active and OneTouch Ultra 2) to measure fasting glycemia after: initial hand washing (reference), application of 1 ml of hydroquinone-containing cream, finger swabbing with wet gauze, sanitizer application and a series of three hand washings following cream application. Reference glycemia was compared to those obtained after various interventions. Statistical significance was assessed by paired sample t-test, clinical significance by total error allowable (TEa), and clinical impact by Parke’s error grid analysis. Results: The mean differences in capillary glycemia (Intervention—reference) measured by Accu-Chek Active in mg/dl were 28, 27, 38, 16, 4, and -2 after cream application, finger swabbing, sanitizer application, one, two, and three hand washings respectively. Corresponding values for OneTouch Ultra2 were 41, 44, 64, 22, 5 and -5. These differences, except after two and three hand washings were both statistically (p < 0.0001) and clinically significant (TEa). After cream application, Accu-Check had 9.9% of values in Parke’s Zones C-E, while OneTouch had 18.7%. Conclusion: Hydroquinone-containing creams cause significant false increase in capillary glycemia irrespective of the enzymatic system of the glucometer used, and can lead to potentially wrong clinical decisions. Background: Medical Infrared Imaging (MII) is an investigative method that can be potentially used in emergency care to non-invasively detect thermal signatures associated with change in blood flow. We have developed a protocol for the use of MII in the Emergency Department (ED) and shown that it is feasible. To derive initial data for sample size calculations, we performed an exploratory study in patients with fever and sepsis. Methods: The Leicester MII protocol was used to image the temperature patterns along the arm among three patient groups (control, fever and sepsis) of a total 56 patients. Anatomical markers were used to divide this gradient into upper arm, forearm, hand and finger regions. Variations in measurements within and between these regions were described. Results: The thermal gradient down the arm was successfully extracted in all patients. The distribution of values in each region of the arm was described in control, fever and sepsis patients. There was a significant gradient between upper arm and finger in controls (2.75, p < 0.0001), but no gradient in fever (p = 0.944) or sepsis (p = 0.710). This was reflected in the finger/arm difference, which was of -2.74°C (±3.50) in controls, -0.39C (±2.48) in fever, and -1.80°C (±3.09) in sepsis. Conclusions: This study found different thermal gradients along the arm in control and febrile groups, and defined the degree of individual variation. It is likely that the difference between upper arm temperature and finger temperature (representing the temperature gradient down the arm) may be more useful than absolute measurements in future studies. Iterative prototyping is costly and time-consuming. Particularly when designing medical instruments, human factors related design choices significantly impact performance and safety. A tool is presented that allows for the evaluation of steerable instrument controls before the onset of the prototyping stage. The design tool couples gestural input to virtually simulated instrument motions using hand motion tracking. We performed a human-subject evaluation of two manual control strategies that differed in their degrees of freedom (DOF). 2DOF thumb control was compared to 4DOF thumb-index finger control. Results identified regions within the instrument workspace that are difficult to reach and showed participants to favor using the thumb for gross and fine-tuning motions at both control strategies. Index finger ab/adduction was found to be least functional. A strong learning effect was observed at 4DOF control. Based on the results, gesture-based instrument design is a viable design tool. Background: Macrodactyly is a congenital malformation characterized by aggressive overgrowth of multiple tissues, including subcutaneous fat, nerves, and bones in digits or limbs. In type II macrodactyly, the peripheral nerve is enlarged; however, the morphological and functional characteristics of the affected peripheral nerves have rarely been evaluated. Methods: In this research, six macrodactyly patients and three polydactyly patients (control) were studied. Pre-operative sensory nerve action potential and intra-operative nerve action potential tests were performed. The microstructure and ultrastructure of the enlarged nerves were observed and neurofilament (NF) expression was evaluated using immunofluorescent staining. Results: Axon impairment of the digital nerves originating from the median nerve (MN) was observed. A compensatory reinnervation from the ulnar nerve (UN) was found in two of the six patients, and significant morphological changes were observed in the enlarged nerve. The myelinated nerve fibers decreased, the lamellar structure of the myelin sheath changed, and the density of the NFs of the unmyelinated fibers decreased. There was aberrant distribution of NFs in the macrodactylous nerve tissues. In patients with compensatory UN reinnervation, the number of myelinated and unmyelinated fibers increased to normal levels; however, the diameter of the myelinated fibers apparently decreased. Conclusions: The morphology and function of the macrodactylous enlarged nerve was impaired in type II macrodactyly patients; however, the unaffected UN partially compensated for the lost function of the affected MN under specific situations. Electrophysiological tests should be performed to determine the function of the affected nerve and surgical treatment for type II macrodactyly could be refined. [ABSTRACT FROM AUTHOR]
