ABSTRACT Objective: To identify and analyze the scientific literature, both national and international, concerning the primary maternal concerns about caring for premature newborns at home. Methods: This integrative review is based on the guiding question: “What scientific publications from 2012 to 2021 address maternal concerns about the care of premature newborns at home?”. Searches were conducted in the electronic databases: Embase, Medline, Web of Science, Lilacs, Scielo, and Cochrane Library. Results: A total of 21 articles were identified. The qualitative analysis showed that maternal concerns pertained to breastfeeding, hygiene, sunbathing practices, managing infant colic, identifying signs, symptoms, and clinical changes, temperature control, and the third phase of the kangaroo method. Conclusions: Maternal uncertainties underscore the importance of enhancing strategies focused on supporting families and ensuring continued care for neonates at home.

Objective: to understand the perception of nurses about the complexity of care for thermal control of the premature infant. Methods: qualitative study conducted with 13 nurses from a public maternity hospital, through virtual interviews, with semi-structured script, submitted to Content Analysis and interpreted by the theoretical support of the complex adaptive care system model. Results: the factors present in nurses' care in thermal control of premature infants are related to the complexity permeated by environmental factors such as the temperature of the unit and distribution of air conditioning currents; institutional factors such as the availability and correct use of technologies, and those related to the professional, such as performance, training, and qualification. Conclusion: adjusting the temperature of the unit; better distribution of the air conditioning currents;availability of technologies; a policy of continued education in the service; training in handling the equipment; supervision and requirement for preventive and corrective maintenance of the devices can influence the reduction of thermal instability events in premature infants. Contributions to practice: stimulating professional qualification, planning of intersectoral actions for technological structuring and preventive maintenance of the units, systematization of assistance to premature infants in thermal control minimizing risk of neonatal morbimortality due to thermal instability.

ABSTRACT Objective To describe and compare the physiological signs presented by premature infants in traditional and humanized weighing. Method A quasi-experimental crossover study, with a sample of 30 premature infants randomly assigned and allocated to the control group (traditional) and the intervention group (humanized), from March 2019 to March 2020, with the collection of general data, vital signs before and after the procedures. Statistical analyses included description of relative and absolute frequencies, measure of central tendency and dispersion. Results Premature infants showed less increase in heart rate (53.3%) and respiratory rate (43%) in the verification of humanized weighing than in the traditional way, with 83.3% of neonates and 80%, respectively. Conclusion It was observed that the humanized form provided less physiological instability, especially in heart and respiratory rates, making it necessary to encourage discussions about the humanization of care and perform this practice routinely in health units.

ABSTRACT Objective: To learn the experiences of mothers of infants who were born premature in the Covid-19 pandemic. Method: Qualitative study based on the Bioecological Theory of Human Development, developed in a public maternity hospital in Paraíba, Brazil, between June and July 2020 with 21 mothers of infants who were born premature, through semi-structured interview. The data were submitted to inductive thematic analysis. Results: Mothers know the measures for prevention, transmission, and symptoms of the disease, as reported in the media, but have different perceptions about social isolation. Their experiences were permeated by financial difficulties, reduced social support and emotional stress. Final considerations: It is recommended to monitor child health and provide social support to the mother-child binomial remotely, so that there is no break in the continuity of the bond and care for the family in this moment of vulnerability.

Abstract Objective: To analyze the prevalence of hypothermia in the first hour of life of preterm infants with birth weight 1,500 g or less. Method: A cross-sectional study performed in a Neonatal Intensive Care Unit. Data obtained from 359 computerized records of premature infants admitted between 2012 and 2016. Descriptive Statistics and Poisson Regression were used. Results: Premature infants (66.9%) presented hypothermia in the first hour of life, with axillary temperature of 36.2ºC (35.7-36.6), associated with: diagnosis of preeclampsia (p = 0.001), small for gestational age (p = 0.029), and the need for chest compression in the delivery room (p = 0.001). In cases of peri-intraventricular hemorrhage grade III (75%) and death (78.9%), there was a prevalence of premature infants with hypothermia in the first hour of life. Conclusion: Hypothermia in the first hour of life was prevalent in preter m infants, being associated with clinical complications. The prevention of hypothermia in the first hour of life is fundamental in the reduction of diseases related to prematurity.

The aim of the present study was to characterize verbal contents expressed by preterm neonates' mothers during psychological support intervention. The sample was composed by 20 mothers of pre-term and very low birthweight neonates, hospitalized in NICU, allocated into two groups: 10 mothers with emotional clinical symptoms (MECS) and 10 mothers with no symptoms (MNECS), assessed by the Beck Depression Inventory and the State-Trait Anxiety Inventory. The verbal behavior expressed by mothers in the psychological support group was recorded, transcribed and categorized. The results show that three verbal categories were more frequent: negative feelings or reactions, followed by positive feelings or reactions and communication with the health staff. The comparison between groups reveals that MECS presented more expressions in the first category in comparison to MNECS. The identification of the feelings and reactions of pre-term neonates' mothers and emotional clinical symptoms should be included for appropriate intervention in developmental care in NICU.El objetivo de este estudio fue caracterizar los contenidos verbales maternos expresados durante una intervención de apoyo psicológico. Objective: To identify risk factors and causes for prematurity in newborns (NBs) in a hospital institution. Method: Documentary, descriptive study with a quantitative approach, carried out in a hospital in the interior of the state of Rio Grande do Sul. The population consisted of 58 premature NBs, who had complete records at the institution. The search for premature NBs was carried out in the birth book of the institution's Obstetric Center and data were collected from the clinical records of the NBs and their mothers. For data tabulation and organization, we used the Microsoft Excel 2013 software and statistical analysis, the Statistical Package for Social Science (SPSS), version 21 software. Results: The results obtained in the research on possible risk factors related to prematurity in NBs were: maternal age, obstetric, genetic and physiological history, lifestyle and number of prenatal consultations. And the causes evidenced were: female genital tract, placental changes and hypertensive syndromes. Also, data on the two types of preterm births: spontaneous and elective are present in the study. Conclusion: it is essential to have knowledge of health professionals about the risk factors and the causes that allow the pregnant woman to have a premature birth during her prenatal monitoring through consultations during the pregnancy period. Thus, the results found in this research become relevant in academic training and technical-scientific development for professional practice, which can provide subsidies to optimize the planning of health policies aimed at this specific audience. Objective: to investigate the nurses' perception about the use of Nursing Care Systematization in the care provided to premature newborns, in the Intermediate Care Unit. Methodology: This is a field research, with a qualitative approach, of a descriptive character, where the data were obtained through a semi-structured interview and a questionnaire, composed of 05 open questions, carried out with 05 nurses allocated at the ICU Neonatal in a public hospital in Pará; the saturation technique was used for the answers to the questionnaire and the content analysis was based on Bardin. Result: four professionals made up the study sample, being young adults and female; most had specialization in the area, working for more than two years, contracted or tendered. It was found that the nursing care provided is aimed at performing technical procedures using protocols; the nurses' knowledge in relation to what is the Nursing Care Systematization is consistent, however they demonstrated difficulty in differentiating it from the Nursing Process; institutional and professional barriers hinder the application of the systematization of nursing care, however they are applied in care aimed at premature newborns. Final considerations: the knowledge and understanding of the subject in question has great benefits, both for the professional and the premature newborn, and to demonstrate that the obstacles of daily nursing care cannot be greater than the need to offer assistance Of Quality. Background: Kangaroo mother care has been highlighted as an effective intervention package to address high neonatal mortality pertaining to preterm births and low birth weight. However, KMC uptake and service coverage have not progressed well in many countries. The aim of this case study was to understand the institutionalisation processes of facility-based KMC services in three Asian countries (India, Indonesia and the Philippines) and the reasons for the slow uptake of KMC in these countries. Methods: Three main data sources were available: background documents providing insight in the state of implementation of KMC in the three countries; visits to a selection of health facilities to gauge their progress with KMC implementation; and data from interviews and meetings with key stakeholders. Results: The establishment of KMC services at individual facilities began many years before official prioritisation for scale-up. Three major themes were identified: pioneers of facility-based KMC; patterns of KMC knowledge and skills dissemination; and uptake and expansion of KMC services in relation to global trends and national policies. Pioneers of facility-based KMC were introduced to the concept in the 1990s and established the practice in a few individual tertiary or teaching hospitals, without further spread. A training method beneficial to the initial establishment of KMC services in a country was to send institutional health-professional teams to learn abroad, notably in Colombia. Further in-country cascading took place afterwards and still later on KMC was integrated into newborn and obstetric care programs. The patchy uptake and expansion of KMC services took place in three phases aligned with global trends of the time: the pioneer phase with individual champions while the global focus was on child survival (1998–2006); the newborn-care phase (2007–2012); and lastly the current phase where small babies are also included in action plans. Conclusions: This paper illustrates the complexities of implementing a new healthcare intervention. Although preterm care is currently in the limelight, clear and concerted country-led KMC scale-up strategies with associated operational plans and budgets are essential for successful scale-up.

Introduction: Prematurity is the leading cause of newborn death in Uganda, accounting for 38% of the nation's 39,000 annual newborn deaths. Kangaroo mother care is a high-impact; cost-effective intervention that has been prioritized in policy in Uganda but implementation has been limited. Methods: A standardised, cross-sectional, mixed-method evaluation design was used, employing semi-structured key-informant interviews and observations in 11 health care facilities implementing kangaroo mother care in Uganda. Results: The facilities visited scored between 8.28 and 21.72 out of the possible 30 points with a median score of 14.71. Two of the 3 highest scoring hospitals were private, not-for-profit hospitals whereas the second highest scoring hospital was a central teaching hospital. Facilities with KMC services are not equally distributed throughout the country. Only 4 regions (Central 1, Central 2, East-Central and Southwest) plus the City of Kampala were identified as having facilities providing KMC services. Conclusion: KMC services are not instituted with consistent levels of quality and are often dependent on private partner support. With increasing attention globally and in country, Uganda is in a unique position to accelerate access to and quality of health services for small babies across the country.Key words: Delivery of health care, implementation, infant premature, Kangaroo Mother Care, neonatal, newborn, preterm, program evaluation, Uganda

ABSTRACT Objective To identify the factors associated with the development of skin allergies in the first year of life in moderate and late preterm infants. Method: This is a cross-sectional study with 151 moderate and late preterm infants, born between May 2016 and May 2017. Participants were evaluated in the 3rd, 6th, 9th and 12th months of life, in telephone interviews. Statistical analyzes were performed in the SPSS software with frequency comparison tests and logistic regression. Results: The prevalence of skin allergy, in the perception of caregivers, among late and moderate preterm infants was 16%. Factors such as being admitted to neonatal intensive care (p = 0.006) and not being breastfed (p = 0.041) showed a significant association with the development of skin allergies in the 3rd and 12th months of life, respectively. Conclusion: Skin allergy, in the perception of caregivers, is more severe in newborn infants who have clinical respiratory and gastrointestinal manifestations, be it conditioning or cause-effect. Breastfeeding proved to be a protective factor in the first year of life.

The virtual learning object was organized in modules, consisting of a presentation, image gallery, theoretical content about Nursing Diagnosis in Neonatology, automatic presentations of clinical cases of preterm newborns at the neonatal intermediate care unit, and simulation exercises of clinical cases of preterm newborns. Face and content evaluations of the virtual object regarding the aspects related to presentation, organization, use and overall impression were performed by experts in computer science (12) and nursing (31). All items received positive evaluations by over 80% of the evaluators, except for the information density criterion on the evaluation by computer science experts. The product is considered adequate to be used for students and continuous education in nursing, in the pedagogical framework of problem-posing.

Background: Some countries have undertaken programs that included scaling up kangaroo mother care. The aim of this study was to systematically evaluate the implementation status of facility-based kangaroo mother care services in four African countries: Malawi, Mali, Rwanda and Uganda. Methods: A cross-sectional, mixed-method research design was used. Stakeholders provided background information at national meetings and in individual interviews. Facilities were assessed by means of a standardized tool previously applied in other settings, employing semi-structured key-informant interviews and observations in 39 health care facilities in the four countries. Each facility received a score out of a total of 30 according to six stages of implementation progress. Results: Across the four countries 95 per cent of health facilities assessed demonstrated some evidence of kangaroo mother care practice. Institutions that fared better had a longer history of kangaroo mother care implementation or had been developed as centres of excellence or had strong leaders championing the implementation process. Variation existed in the quality of implementation between facilities and across countries. Important factors identified in implementation are: training and orientation; supportive supervision; integrating kangaroo mother care into quality improvement; continuity of care; high-level buy in and support for kangaroo mother care implementation; and client-oriented care. Conclusion: The integration of kangaroo mother care into routine newborn care services should be part of all maternal and newborn care initiatives and packages. Engaging ministries of health and other implementing partners from the outset may promote buy in and assist with the mobilization of resources for scaling up kangaroo mother care services. Mechanisms for monitoring these services should be integrated into existing health management information systems.
