A strategy is needed on how to treat the growing number of obese children with the limited resources available. We compared the long-term (24 months) effectiveness of therapist-led groups (TLG) v. self-help groups (SHG) for parents on changes in children's adiposity and dietary intake. The study included ninety-nine children (forty-eight girls) who were referred to obesity treatment (7–12 years, BMI z-scores ≥ 2, attendance of at least one parent). Parents (ninety-one mothers, fifty-four fathers) were randomised to TLG aimed at increasing parents' competence to accomplish lifestyle changes (n 47), or SHG (n 52), both with fifteen sessions. All children participated in children's groups, and all families attended individual counselling by a clinical dietitian and physiotherapist. Percentage of body fat (BF) was measured by dual-energy X-ray absorptiometry, BMI z-score was calculated by international reference values and dietary intake was calculated from 4 d estimated food records at baseline and after 6 and 24 months. No significant between-group differences were detected in the children's changes in adiposity or dietary intake after 6 and 24 months. BF, BMI z-scores and energy intake were significantly decreased after 6 months (P< 0·05) in both intervention groups, and this persisted throughout 24 months without compromising the diet macronutrient composition. In conclusion, the TLG and SHG intervention groups appear to be equally effective in improving long-term adiposity and dietary intake in obese children. Further research should be performed to clarify whether the SHG should be preferred to parental group treatment for similar children with obesity.

Objective: The aim of the current study was to examine to what extent changes in reported energy intake and physical activity predict changes in body fat during a family-based outpatient treatment of obesity in children., Methods: Total body fat (DXA), reported energy intake (4-day diet record), and physical activity (accelerometer) was measured in 99 children (age 7-12 years, mean BMI SDS = 2.99) at baseline as well as after 6 months 2 years of treatment. Repeated measures (GLM), growth modeling, and structural equation modeling (SEM) were applied in the data analyses., Results: There was significant decrease in body fat, reported energy intake, and physical activity at both follow-ups (p < 0.001) compared to baseline. Changes in reported energy intake from baseline to 6 months predicted a decrease in body fat from baseline to 6 months (β = 0.68, p < 0.001). In addition, changes in reported energy intake had a strong indirect effect on body fat at 2-year follow-up, mediated by changes in body fat from baseline to 6 months (indirect β = 0.50, p < 0.001). Changes in physical activity did not predict changes in body fat during treatment., Conclusions: Changes in reported energy intake significantly affected body fat at 6 months and indirectly predicted the amount of body fat at 2-year follow-up. The indirect effect was mediated by a decrease in body fat obtained during the first phase of treatment.

Purpose: The purpose of the current study was to explore psychopathology as a mediator of quality of life (QOL) in children and adolescents with obesity. The notion that psychopathology and QOL are two distinct constructs was also tested., Methods: A sample of treatment-seeking children and adolescents with obesity (n = 185, average age = 11.5, mean BMI SDS = 3.03) was matched to a community sample of children (n = 799, average age = 11.5). Both self- and parent-reported measures of QOL (KINDL-R) and psychopathology (CBCL/YSR) was completed., Results: Parent-reported QOL was impaired, and both self-reported and parent-reported psychopathology was elevated in children and adolescents with obesity. Psychopathology accounted for all the variance of the effect of obesity on parent-reported QOL. The distinction between QOL and psychopathology was supported through confirmatory factor analysis., Conclusion: Impaired parent-reported QOL in children and adolescents with obesity was attributable to their elevated levels of psychopathology.

Children's eating behavior influences energy intake and thus weight through choices of type and amount of food. One type of eating behavior, food responsiveness, defined as eating in response to external cues such as the sight and smell of food, is particularly related to increased caloric intake and weight. Because little is known about the potential determinants of such behavior, we focus herein on child and parent predictors of food responsiveness in a large community sample of Norwegian 6-year-olds, followed up at ages 8 and 10. To measure children's food responsiveness, parents completed the Children's Eating Behavior Questionnaire. Potential predictors were children's inhibition and symptoms of attention-deficit/hyperactivity disorder and depression, and parents' instrumental and controlling feeding practices as well as parental restrained eating. After accounting for children's initial levels of food responsiveness within a hybrid fixed effects method that takes into consideration all unmeasured time-invariant confounders, more child attention-deficit/hyperactivity disorder symptoms and greater restrained eating by parents predicted more food responsiveness at both ages 8 and 10. Parental feeding practices and children's eating behavior are consistently related to childhood obesity. However, it is not known whether parents' feeding practices predict obesogenic eating behavior or vice versa. In a Norwegian cohort (n = 797), it was found that greater parental use of food as a reward (instrumental feeding) when children were 6 predicted increased emotional overeating and food responsiveness, whereas greater parental encouragement to eat forecasted increased enjoyment of food 2 years later. No evidence of child effects emerged. Although children's eating behavior is relatively stable and established at an early age, findings suggest that parental feeding practices can serve as targets of intervention to prevent the development of obesogenic eating behavior.

Summary: This systematic review and meta‐analysis aimed to quantify associations between Child Eating Behavior Questionnaire (CEBQ) and Baby Eating Behavior Questionnaire (BEBQ) appetitive traits (food approach: Food Responsiveness [FR], Enjoyment of Food [EF], Emotional Overeating [EOE], and Desire to Drink [DD]; food avoidant: Satiety Responsiveness [SR], Slowness in Eating [SE], Emotional Undereating [EUE], Food Fussiness [FF]) and measures of child adiposity. Searches of six databases up to February 2019 identified 72 studies (CEBQ, n = 67; BEBQ, n = 5), and 27 met the meta‐analysis criteria. For cross‐sectional studies reporting unadjusted correlations with body mass index z‐scores (BMIz) (n = 19), all traits were associated with BMIz in expected directions (positive: FR, EF, EOE, and DD; negative: SR, SE, EUE, and FF). Pooled estimates ranged from r = 0.22 (FR) to r = −0.21 (SR). For cross‐sectional studies reporting regression coefficients (n = 10), three traits (FR, EF, and EOE) associated positively, and three traits (SR, SE, and EUE) negatively, with BMIz (β = −0.31 [SR] to β = 0.22 [FR]). Eleven studies reported prospective relationships from appetite to adiposity measures for six scales (positive: FR, EF, EOE, and DD; negative: SR and SE). Five studies reported relationships from adiposity measures to appetite for five traits (positive: FR, EF, and EOE; negative: SR). All BEBQ traits were consistently cross‐sectionally associated with adiposity measures. Overall, CEBQ/BEBQ‐assessed appetitive traits show consistent cross‐sectional relationships with measures of child adiposity. [ABSTRACT FROM AUTHOR]

Objectives: To examine appetite traits, level of physical activity, and television (TV) time as predictors of change in Body Mass Index Standard Deviation Score (BMI SDS) from age 6 to 8 and to explore the effect of BMI SDS (from age 4) on appetite traits.Methods: In all, 995 Norwegian children participated at age 4; 760 and 687 of these children took part in the assessment at ages 6 and 8, respectively. Appetite traits were assessed using the Children's Eating Behavior Questionnaire, activity was measured using accelerometers, and TV time was based on parental reports.Results: High food responsiveness predicted a steeper increase in BMI SDS. A reversed effect was also observed: High BMI SDS predicted increased food responsiveness and decreased satiety responsiveness. Physical activity and TV time were unrelated to BMI SDS.Conclusion: Children whose eating is especially triggered by the sight and smell of food show prospective increased weight gain. Summary: Aim: Research supports the use of family-based interventions in the treatment of obesity in children, but there is a lack of knowledge about what factors affect parents’ ability to carry out the lifestyle changes necessary to reduce their child‘s obesity. The aim of the present study was to examine whether parents’ self-efficacy, perceived emotional barriers, subjective norms, and attitudes could predict change in their children''s body fat at 6 month and 2 year follow-ups after a family-based treatment of obesity. Methods: Body Mass Index Standard Deviation Scores (BMI SDS) were calculated and body fat (dual-energy X-ray absorptiometry) were measured in 99 treatment-seeking children with obesity (ages 7–12; 48 girls, 51 boys; mean BMI SDS=2.99) at baseline, after 6 month and after 2 year follow-up. Parental cognitions regarding diet and physical activity were examined by parent-completed questionnaires. Structural equation modeling (SEM) was used to test whether the selected health cognitions could predict treatment outcome. Results: Parental perceived emotional barriers was a significant predictor of change in body fat at 6 month (β =−.32, p =.001) and 2 year (β =−.38, p =.002) follow-up when the initial body fat values were controlled. Self-efficacy, subjective norms and attitudes did not improve the amount of variance explained. Conclusion: Parents’ perceived emotional barriers significantly predict change in total body fat in children treated for obesity. In order to increase treatment-efficacy, perceived emotional barriers should be addressed. [Copyright &y& Elsevier]

Meta-analyses suggest shorter sleep as a risk factor for obesity in children. The prevailing hypothesis is that shorter sleep causes obesity by impacting homeostatic processes. Sleep duration and adiposity are both heritable, and the association may reflect shared genetic aetiology. We examined the association between a body mass index (BMI) genetic risk score (GRS) and objectively-measured total sleep time (TST) in a cohort of Norwegian children (enrolled at age four in 2007-2008) using cross-sectional data at age six. The analytical sample included 452 six-year old children with complete genotype and phenotype data. The outcome was actigraphic total sleep time (TST) measured at age six years. Genetic risk of obesity was inferred using a 32-single nucleotide polymorphism (SNP) weighted GRS of BMI. Covariates were BMI-Standard deviation scores (SDS) (which takes into account age and sex) and, in a sensitivity analysis socioeconomic status. Analyses consisted of Pearson's correlations and linear regressions. In our sample, 54% of participants were male; mean (SD) TST, age and BMI were 9.6 (0.8) hours, 6.0 (0.2) years and 15.3 (1.2) kg/m2, respectively. BMI and TST were not correlated, r = -0.003, p = 0.946. However, the BMI GRS was associated with TST after adjusting for BMI-SDS, standardised β = -0.11; 95% confidence interval (CI) = -0.22, -0.01. To our knowledge, this is the first study to establish a relationship between genetic risk of obesity and objective sleep duration in children. Findings suggest some shared genetic aetiology underlying these traits. Future research could identify the common biological pathways through which common genes predispose to both shorter sleep and increased risk of obesity. [ABSTRACT FROM AUTHOR]
