However, when a diagnosis of PAF is being considered, it is necessary to work through an exhaustive differential diagnosis list covering a wide spectrum of pathologies whose prognosis and treatment approaches may substantially differ from those of PAF [[1]]. Those alternative diagnoses include other alpha-synucleinopathies, such as Parkinson disease and multiple systemic atrophy, endocrinopathies (e.g. diabetes mellitus), drug-induced orthostatic hypotension, amyloidosis [[2]] and the recently described autoimmune autonomic ganglionopathies and neuronopathies [[1]-[3]]. However, the symptoms did not change after treatment with corticoids or immunoglobulins and autonomic function testing performed after the treatment showed no change in results.
