Throughout the history of the development of treatments proposed for pain, medicinal plants and their secondary metabolites have played a key role. However, even more effective therapeutic options for the treatment of painful conditions are necessary. In this sense, the present study aimed to evaluate the effect of the inclusion complex of β-cyclodextrin (β-CD) containing carvacrol, a phenolic monoterpene present in the essential oil of medicinal plants of the genus Origanum, Thymus (Lamiaceae), on the oncology nociception (induced by tumor cell sarcoma 180, S180) in experimental animals. This thesis is formed by three chapters, consisting of a review of patents and two regular papers. Initially, the review Terpenes and derivatives as a new perspective for pain treatment: a patent review was elaborated, which describes patents on the analgesic properties of terpenes and prospects created from these records. After conducting the experimental protocols, the article Evidence for the Involvement of descending pain- inhibitory mechanisms in the attenuation of cancer pain induced by carvacrol , a monoterpene of Origanum oil was constructed, in which it has been shown that carvacrol (CARV) reduces nociception induced by tumor cells (S180), decreases the number of Fos positive neurons in lamina I of the Lumbar spinal cord and activates the brain nuclei involved in the descending inhibitory pain pathway, such as the periaqueductal gray, nucleus raphe magnus and locus coeruleus. Subsequently, the inclusion complex consisting of carvacrol and β-cyclodextrin (CARV/β-CD) was developed and characterized physicochemically through the method of co-evaporation. CARV complexation was able to increase the time of the antinociceptive effects of this compound in animals with S180 in 15 hours and improved the pharmacological response at lower doses, in addition to promoting a clear increase in solubility and stability. These data formed the patent submitted and approved by the Center for Innovation and Technology Transfer (CINTEC UFS) and forms the scientific basis of the article Encapsulation of β-cyclodextrin in carvacrol improves the pharmacological response on cancer pain . From these findings, we conclude that the analgesic effect of carvacrol on cancer pain can be enhanced by employing inclusion complexes containing β-cyclodextrin and may represent an interesting object of further translational research and clinical trials in order clinical evidence to be obtained, which is necessary for the development of new drugs for cancer-associated pain management. Ao longo da história do desenvolvimento de propostas terapêuticas para o tratamento da dor, as plantas medicinais e seus metabólitos secundários tem exercido um papel fundamental. No entanto, ainda são necessárias opções terapêuticas mais eficazes para o tratamento de condições dolorosas. Neste sentido, o presente trabalho buscou avaliar o efeito do complexo de inclusão de β-ciclodextrina (β-CD) contendo carvacrol, um monoterpeno fenólico presente no óleo essencial de plantas medicinais do gênero Origanum e Thymus (Lamiaceae), sobre a nocicepção oncológica (induzida por células tumorais sarcoma 180, S180) em animais de experimentação. A tese é composta por três capítulos, constituídos de uma revisão de patentes e dois artigos científicos. Inicialmente foi elaborada a revisão Terpenes and derivatives as a new perspective for pain treatment: a patent review , a qual descreve as patentes sobre as propriedades analgésicas dos terpenos e as perspectivas criadas a partir destes registros. Após a realização dos protocolos experimentais foi elaborado o artigo Evidence for the involvement of descending pain-inhibitory mechanisms in the attenuation of cancer pain induced by carvacrol, a monoterpene of Origanum oil , no qual foi demonstrado que o carvacrol (CARV) reduz a nocicepção induzida por células tumorais (S180), diminui o número de neurônios Fos positivos na lâmina I da medula lombar e ativa núcleos cerebrais envolvidos na via descendente inibitória da dor, como a substância cinzenta periaquedutal, núcleo magno da rafe e locus ceruleus. Posteriormente, foi desenvolvido e caracterizado físico-quimicamente o complexo de inclusão constituído de β-ciclodextrina e carvacrol (CARV/β-CD) através do método de co-evaporação. A complexação do CARV foi capaz de aumentar o tempo do efeito antinociceptivo deste composto nos animais com o S180 em 15 horas e melhorar a resposta farmacológica em doses menores, além de promover nítido aumento de solubilidade e estabilidade. Estes dados compuseram a patente submetida e aprovada pelo Centro de Inovação e Transferência de Tecnologia (CINTEC UFS) e constitui a base científica do artigo Encapsulation of carvacrol in β-cyclodextrin improve the pharmacological response on cancer pain . A partir destes achados é possível concluir que o efeito analgésico do carvacrol sobre a dor oncológica pode ser aprimorado pelo emprego de complexos de inclusão contendo β-ciclodextrina e pode representar um interessante objeto de novas pesquisas translacionais e ensaios clínicos a fim de que sejam obtidas as evidências clínicas necessárias para o desenvolvimento de novos medicamentos destinados ao manejo da dor associada ao câncer.

Renal failure is characterized by loss of kidney function. During the course of the disease, mostpatients develop the mineral and bone disorder of chronic kidney disease (CKD-BMD). The early identification of the clinical manifestations allows conducts to be performed to minimize bone pain and improve the functional capacity, bringing to patients affected by this pathology a better quality of life. For this reason, this study aimed to evaluate pain index in patients with osteometabolic disorder of chronic kidney disease, submitted to hemodialysis, and its impact on the quality of life. An observational, epidemiological cross-sectional epidemiological study was conducted with a sample of 195 patients submitted to hemodialysis. Biochemical parameters associated to bone metabolism were evaluated, such as: Calcium, phosphorus, PTH, alkaline phosphatase, calcitriol and aluminum. The qualification and quantification of pain was measured using the questionnaires: Brief Pain Inventory (IBD) and McGill, respectively. The evaluation of patients' quality of life was performed using the Quality of Life Questionnaire in Renal Disease (KDQOL-SF). This study was approved by CEP / UFS under the number of CAAE: 60158216.6.0000.5546. Of the total number of patients, 37.44% presented altered values for calcium. Concerning phosphorus, approximately 50% of the patients presented alterations below or above the reference values. Of the total, 17.44% of the volunteers presented the product of calcium and phosphorus levels ≥ 55 mg / dL. As for PTH, 74.87% of the patients presented values above or below the reference range. Evaluating calcitriol and alkaline phosphatase, it was observed that 34.36% and 45.65% of the patients presented serum levels outside the reference levels, respectively. From these data, 104 (53.33%) patients were classified as having BMD. Black patients had a higher risk (RR = 1.36) for developing the disorder. In addition, longer treatment times also showed a relationship with the development of BMD. It was also verified the high prevalence of patients with BMD, especially BMD with high remodeling. Through the use of IBD, it was possible to observe that 66 patients (33.85%) had experienced pain in the last 24 hours, with a mean pain score of 4.59 ± 2.33. Patients with musculoskeletal pain had high levels of alkaline phosphatase. In general, these pain conditions impacted on general activity, mood, walking ability and sleep. Through the McGill questionnaire, 109 patients (55.90%) reported experiencing pain, of which 38 (19.49%) reported pain at the time of interview, ranging from weak to unbearable. The sensorial and affective dimensions had a higher index of pain classification. The mean total quality of life score of patients with CKD was 64.72%, being an intermediate quality of life index. Only the status of work had a high impact on the quality of life of patients submitted to hemodialysis. Evaluating the pain dimension, it can be observed that patients with present BMD present lower quality of life. Through this study it was possible to verify that more than half of the patients on hemodialysis reported pain, varying from weak to unbearable, with a higher prevalence for mild and moderate pain, with emphasis on sensory and affective dimensions. In addition, although patients presented a mean total quality of life intermediate score, pain has a significant impact on the quality of life of BMD patients indicating that this disorder aggravates the patient's clinical condition. A insuficiência renal é caracterizada pela perda da função dos rins. Ao longo da evolução da doença grande parte dos pacientes desenvolvem o distúrbio mineral e ósseo da doença renal crônica (DMO-DRC). A identificação precoce das manifestações clínicas permite que sejam realizadas condutas para minimizar dores ósseas e melhorar a capacidade funcional trazendo para os pacientes acometidos por essa patologia uma melhor qualidade de vida. Por esta razão, este trabalho buscou avaliar índice álgico em pacientes com distúrbio osteometabólico da doença renal crônica, submetidos à hemodiálise, e seu impacto sobre a qualidade de vida. Foi realizado um estudo epidemiológico observacional analítico do tipo transversal com uma amostra de 195 pacientes submetidos à hemodiálise. Foram avaliados parâmetros bioquímicos associados ao metabolismo ósseo, como: Cálcio, fósforo, PTH, fosfatase alcalina, calcitriol e alumínio. A qualificação e quantificação da dor foi mensurada através da aplicação dos questionários: Inventário Breve da dor (IBD) e McGill, respectivamente. A avaliação da qualidade de vida dos pacientes foi realizada através da utilização do Questionário de qualidade de vida em doença renal (KDQOL-SF). Esse estudo foi aprovado pelo CEP/UFS com o número de CAAE: 60158216.6.0000.5546. Do total de pacientes 37,44% apresentaram valores alterados para o cálcio. Quanto ao fósforo, aproximadamente 50% dos pacientes apresentaram alterações abaixo ou acima dos valores de referência. Do total, 17,44% dos voluntários apresentaram o produto dos níveis de cálcio e fósforo ≥ 55 mg/dL. Tratando-se do PTH, 74,87% dos pacientes apresentaram valores superiores ou inferiores ao intervalo de referência. Avaliando o calcitriol e a fosfatase alcalina, observou-se que 34,36% e 45,65% dos pacientes apresentaram níveis séricos fora dos níveis de referência, respectivamente. A partir destes dados, 104 (53,33%) pacientes foram classificados como portadores do DMO. Os pacientes negros apresentaram maior risco (RR=1,36) de desenvolver o distúrbio. Além disso, um maior tempo de tratamento também mostrou relação com o desenvolvimento do DMO. Verificou-se ainda a alta prevalência de pacientes com DMO, com destaque para DMO de alta remodelação. Através do emprego do IBD foi possível observar que 66 pacientes (33,85%) haviam sentido dor nas últimas 24 horas, com escore médio de dor de 4,59 ± 2,33. Os pacientes queapresentaram dor musculoesquelética estavam com níveis de fosfatase alcalina elevada. Em geral, estas condições álgicas impactaram na atividade geral, humor, habilidade de caminhar e no sono. Através do questionário de McGill verificou-se que 109 pacientes (55,90%) relataramsentir dor, dos quais 38 (19,49%) reportaram dor no momento da entrevista, variando de fraca a insuportável. As dimensões sensorial e afetivo apresentaram maior índice de classificação da dor. O escore médio total de qualidade de vida dos pacientes com DRC foi de 64,72%, sendo um índice intermediário de qualidade de vida. Apenas a dimensão status de trabalho apresentou elevado impacto sobre a qualidade de vida dos pacientes submetidos a hemodiálise. Avaliando a dimensão dor, pode-se observar que os pacientes com DMO presente apresentam menor qualidade de vida. Através deste estudo foi possível verificar que mais da metade dos pacientes em hemodiálise relataram sentir dor, variando de fraca a insuportável, com maior prevalência para dores fracas e moderadas, com destaque para as dimensões sensoriais e afetivas. Além disso, embora os pacientes tenham apresentado escore médio total de qualidade de vida intermediário, a dor apresenta impacto significativo sobre a qualidade de vida dos portadores de DMO, indicando que este distúrbio agrava a condição clínica do paciente.
