Background: Patients with intestinal failure who require long-term parenteral nutrition (PN) develop catheter thrombosis as a complication. This patient group may also develop choline deficiency because of a defect in the hepatic transsulfuration pathway in the setting of malabsorption. This study was undertaken to determine whether choline deficiency is a risk factor for development of catheter thrombosis., Methods: Plasma free and phospholipid-bound choline concentrations were measured in a group of 41 patients that required long-term PN. Episodes of catheter thrombosis from onset of PN to the time of blood testing were recorded., Results: Sixteen (39%) patients developed catheter thrombosis, and 5 of these had recurrent catheter thrombosis. Plasma free choline was 7.7 +/- 2.7 nmol/mL in patients with no history of catheter thrombosis and 6.2 +/- 1.7 nmol/mL in patients with previous catheter thrombosis (p = .076 by Wilcoxon rank-sum test). The partial correlation between plasma free choline concentration and the frequency of clots after controlling for catheter duration was r = -0.33 (p = .038). The relative risk for catheter thrombosis in subjects with a plasma free choline concentration <8 nmol/mL was 10.0, 95% confidence interval (1.134-88.167). Plasma phospholipid-bound choline concentration was 2191.7 +/- 679.0 nmol/mL in patients with previous catheter thrombosis and 2103.3 +/- 531.2 nmol/mL in patients without history of catheter thrombosis (p = NS)., Conclusion: Choline deficiency is a significant risk factor for development of catheter thrombosis in patients with intestinal failure who require PN. more...

Objective: To analyze the outcome of children with short bowel syndrome (SBS) who required long-term parenteral nutrition (PN)., Study Design: Retrospective analysis of children (n=78) with SBS who required PN >3 months from 1975 to 2000., Statistics: univariate analysis, Kaplan-Meier method, and Cox proportional regression model were used., Results: We identified 78 patients. Survival was better with small bowel length (SBL) >38 cm, intact ileocecal valve (ICV), intact colon, takedown surgery after ostomy (all P <.01), and primary anastomosis (P <.001). PN-associated early persistent cholestatic jaundice (P <.001) and SBL of <15 cm (P <.01) were associated with a higher mortality. Intestinal adaptation was less likely if SBL <15 cm (P <.05), ICV was removed, colonic resection was done (both P <.001), >50% of colon was resected (P <.05), and primary anastomosis could not be accomplished (P <.01). Survival was 73% (57), and 77% (44) of survivors had intestinal adaptation., Conclusions: SBL, intact ICV, intestinal continuity, and preservation of the colon are important factors for survival and adaptation. Adaptation usually occurred within the first 3 years. Need for long-term PN does not preclude achieving productive adulthood. Bone disease with total parenteral nutrition (TPN) has been attributed to aluminum loading or vitamin D therapy. We studied 17 patients who first received TPN containing casein hydrolysate with high Al and ergocalciferol (25 µg/d) for 6-72 mo followed by TPN containing amino acids with reduced Al and ergocalciferol (5 µg/d) for 9-58 mo. We also did a cross-sectional study of 22 patients receiving casein and ergocalciferol (25 µg/d) compared with 46 patients receiving amino acids and ergocalciferol (5 µg/d) for 6-58 mo. Bone formation was higher and osteoid area, bone-surface stainable Al and total bone Al were lower with amino acid TPN than with casein TPN. Bone formation varied inversely with both plasma Al and bone-surface Al, suggesting that plasma or bone-surface Al, acquired during TPN, can reduce bone formation and lead to patchy osteomalacia. Taurine concentrations were measured in plasma and blood cells of 40 adults undergoing long-term parenteral nutrition, without intravenous taurine, for 43.8 ± 35.1 (SD) mo. Patients were classified into Group 1 (21 patients) or Group 2 (19 patients) according to whether their estimated enteral absorption of calories was less or greater than 25% of their daily requirement, respectively. In Group 1, taurine concentrations were reduced to 35-49% of normal control values in plasma (p < 0.01), platelets (p < 0.001), lymphocytes (p < 0.005), and erythrocytes (p < 0.001). Granulocyte taurine was not different from normal. A smaller decrease in taurine concentration was found in Group 2 patients; however, taurine levels were significantly below normal in their plasma and red cells. Thus, many patients undergoing long-term parenteral nutrition with little or no taurine intake are depleted of taurine in plasma and most blood cells.
