International audience; Primary membranous nephropathy (MN) is an autoimmune glomerular disease in which autoantibodies are directed against podocyte proteins. In about 80% of cases the main targeted antigen is the phospholipase A2 receptor 1 (PLA2R1). Anti-PLA2R1 antibodies are mainly immunoglobulin G type 4 (IgG4). However, the antigenic target remains to be defined in 20% of cases. MN can be associated with chronic inflammatory demyelinating polyneuropathy, an autoimmune disease of the peripheral nervous system where a common antigenic target has yet to be identified. To ascertain a possible novel target antigen, we analyzed kidney biopsies from five patients positive for anti-contactin 1 antibodies and presenting with MN combined with chronic inflammatory demyelinating polyneuropathy. Eluted IgG from biopsy sections against contactin 1 and nerve tissue were screened. Western blot revealed contactin 1 expression in normal kidney glomeruli. Confocal microscopic analysis showed the presence and colocalization of contactin 1 and IgG4 on the glomerular basement membrane of these patients. Glomerular contactin 1 was absent in patients with anti-PLA2R1-associated MN or membranous lupus nephritis or a healthy control. The eluted IgG from contactin 1-positive biopsy sections but not the IgG eluted from patients with PLA2R1 MN bound conactin 1 with the main eluted subclass IgG4. Eluted IgG could bind paranodal tissue (myelinated axon) and colocalized with commercial anti-conactin 1 antibody. Thus, contactin 1 is a novel common antigenic target in MN associated with chronic inflammatory demyelinating polyneuropathy. However, the precise pathophysiology remains to be elucidated.

Cerebellar ataxia with neuropathy and vestibular areflexia syndrome (CANVAS) is a recessively inherited multisystem ataxia compromising cerebellar, vestibular, and sensory nerves, which has been associated to a pathogenic AAGGG(n) biallelic expansion repeat in the RFC1 gene. Our objective was to assess its prevalence in a French cohort of patients with idiopathic sporadic late-onset ataxia (ILOA), idiopathic early-onset ataxia (IEOA), or Multiple System Atrophy of Cerebellar type (MSA-C). 163 patients were recruited in 3 French tertiary centers: 100 ILOA, 21 IEOA, and 42 patients with possible or probable MSA-C. A pathogenic biallelic RFC1 AAGGG(n) repeat expansion was found in 15 patients: 15/100 in the ILOA group, but none in the IEOA and MSA-C subgroups. 14/15 patients had a CANVAS phenotype. Only 1/15 had isolated cerebellar ataxia, but also shorter biallelic expansions. Two RFC1 AAGGG(n) alleles were found in 78% of patients with a CANVAS phenotype. In one post-mortem case, the pathophysiological involvement of cerebellum and medullar posterior columns was found. Our study confirms the genetic heterogeneity of the CANVAS and that RFC1 repeat expansions should be searched for preferentially in case of unexplained ILOA associated with a sensory neuronopathy, but not particularly in patients classified as MSA-C.

Homozygous mutations in MAG, encoding the myelin‐associated glycoprotein, a transmembrane component of the myelin sheath, have been associated with SPG 75 recessive spastic paraplegia. Here, we report the first patient with two compound heterozygous novel MAG mutations (p.A151V and p.S373R) and early developmental delay with a progressive complex phenotype characterized by spastic paraplegia, peripheral sensorimotor neuropathy, intellectual disability, and sensorial dysfunctions with severe optic atrophy and hearing involvement. Brain imaging showed progressive global cerebellar atrophy. We propose that complex hereditary spastic paraplegia, with axonal and demyelinating polyneuropathy, sensorial impairment and intellectual disability might suggest MAG mutations.

International audience; Since 1973, a few cases of concomitant inflammatory demyelinating polyradiculoneuropathy and nephrotic syndrome have been reported. The neuropathy was either an acute inflammatory demyelinating polyneuropathy (AIDP) or a chronic inflammatory demyelinating polyneuropathy (CIDP), and the nephrotic syndrome was related to either a focal segmental glomerulosclerosis (FSGS) or a membranous glomerulonephritis (MGN).Time course, severity, and response to treatment were heterogeneous, and evolution of neuropathy and nephropathy could be independent. This suggests that common epitopes are targeted by autoantibodies within the peripheral nerve and the glomerular filtration barrier. However, the antigenic targets are unknown. Here, we report a patient who developed CIDP associated with anti-contactin-1 immunoglobulin G4 (IgG4) antibodies and nephrotic syndrome.

A 16-year-old boy with recurrent transient visual loss had bilateral papilledema. MRI revealed giant arachnoid granulations (GAG) in both transverse sinuses and signs of intracranial hypertension (figure). CSF opening pressure was 420 mm H2O. After depletive lumbar puncture, the size of GAG decreased and sinus lumen stenosis resolved (figure). Arachnoid granulations (AG) are CSF herniations through dural defects into sinuses.1 Since CSF removal led to their regression, GAG might be a consequence of intracranial hypertension. Our case supports the Krisch theory, which considers AG as a buffer of CSF compartment rather than a site of CSF absorption.2

Introduction: Conduction block (CB) has been included in the Rajabally criteria for axonal Guillain–Barre syndrome (GBS). Because the nerve roots may be affected early in GBS, detection of proximal CB by the triple stimulation technique (TST) can be useful. Methods: We describe TST findings in 2 patients who presented with the pharyngeal–cervical–brachial (PCB) variant of axonal GBS. Results: In the first patient, although conventional nerve conduction studies (NCS) did not fit electrodiagnostic criteria for axonal GBS, the TST detected proximal CB in the median and ulnar nerves. In the second patient, NCS fulfilled criteria for axonal GBS, and the TST detected proximal CB in the median nerve. After plasmapheresis, NCS and TST findings were normalized, suggesting reversible conduction failure rather than demyelinating CB. Conclusion: The TST may be useful for diagnosis of PCB when NCS remain inconclusive. The technique provides additional clues for classifying PCB into the acute nodo-paranodopathies. Lesions of the corpus callosum (CC) are seen in a multitude of disorders including vascular diseases, metabolic disorders, tumours, demyelinating diseases, trauma and infections. In some diseases, CC involvement is typical and sometimes isolated, while in other diseases CC lesions are seen only occasionally in the presence of other typical extra-callosal abnormalities. In this review, we will mainly discuss the MRI characteristics of acquired lesions involving the CC. Identification of the origin of the CC lesion depends on the exact localisation of the lesion(s) inside the CC, presence of other lesions seen outside the CC, signal changes on different MRI sequences, evolution over time of the radiological abnormalities, history and clinical state of the patient, and other radiological and non-radiological examinations.

Introduction Sensory chronic inflammatory demyelinating polyneuropathy (CIDP) can be difficult to diagnose. Methods We report 22 patients with chronic sensory polyneuropathy with ≥1 clinical sign atypical for chronic idiopathic axonal polyneuropathy (CIAP) but no electrodiagnostic criteria for CIDP. Results Clinical signs atypical for CIAP were: sensory ataxia (59%), generalized areflexia (36%), cranial nerve involvement (32%), rapid upper limb involvement (40%), and age at onset ≤55 years (50%). Additional features were: normal sensory nerve action potentials (36%), abnormal radial/normal sural pattern (23%), abnormal somatosensory evoked potentials (SSEPs) (100%), elevated cerebrospinal fluid (CSF) protein (73%), and demyelinating features in 5/7 nerve biopsies. Over 90% of patients responded to immunotherapy. We conclude that all patients had sensory CIDP. Conclusions Sensory CIDP patients can be misdiagnosed as having CIAP. If atypical clinical/electrophysiologic features are present, we recommend performing SSEPs and CSF examination. A 20-year-old man presented with subacute dysarthria, along with bilateral facial and brachial palsy, after a febrile odynophagia. Initially brisk, his upper limb deep tendon reflexes decreased with muscle wasting concerning the C5-T1 myotomes. MRI showed bilateral prefrontal gyri and thalami lesions with anterior cervical spinal cord lesion (figures 1 and 2). Upper limb lower motor neuron involvement was confirmed by EMG. CSF analysis showed lymphocytic pleiocytosis, elevated protein, and normal glucose levels . Epstein-Barr virus (EBV) serology revealed acute infection with the presence of viral capsid antigen (VCA) immunoglobulin M and VCA immunoglobulin G (IgG), in the absence of Epstein-Barr nuclear antigen–1 IgG. EBV DNA load was increased in blood and CSF. Other investigations including HIV screening remained negative. Simultaneous upper and lower motor neuron injury, well-described in HIV, is an unusual finding in EBV infection.1

Polyarteritis nodosa (PAN) is a systemic necrotizing vasculitis mediated by intravascular deposition of immune complexes. We report a 55-year-old man with PAN revealed by CNS vasculitis with subarachnoid hemorrhage and brainstem vasogenic oedema, associated with essential mixed cryoglobulinemia. In spite of aggressive treatment (steroids and cyclophosphamide), the patient became tetraplegic. A pathophysiological relationship between cryoglobulin and PAN can be suspected. Mixed cryoglobulinemia may trigger vasculitis with PAN features, possibly explaining the lack of treatment response.

Background The pathophysiological mechanism of white matter lesions (WML) and dilated Virchow–Robin spaces (DVRS), both frequently encountered in myotonic dystrophy 1 (DM1), is unclear. Methods We studied the spatial co-existence of WML and DVRS in 6 DM1 patients with a 1.5 T or 3 T brain MRI. Results Co-localisation with DVRS was found in 86% of the WML. Some WML had a droplet, ovoid, of finger-like aspect following the exactly the orientation of the DVRS. Some WML were linear (indistinguishable from DVRS on T2-weighted imaging) whereas some other WML had a streak- or a dotted-like aspect. Conclusion Our data suggest a pathophysiological relationship between the presence of WML and DVRS in DM1. Some WML might represent, pathologically modified, DVRS.

International audience; INTRODUCTION:Cerebral punctate and curvilinear gadolinium enhancements (PCGE) correspond to opacification of small vessel lumen or its perivascular areas in case of blood-brain barrier (BBB) disruption. We will discuss the possible causes of intra-parenchymal central nervous system PCGE.METHODS:Our review is based on French database including patients presenting with central nervous system PCGE and literature search using PubMed database with the following keywords: punctate enhancement, linear enhancement, and curvilinear enhancement. Disorders which displayed linear leptomeningeal or periventricular enhancements without intra-parenchymal PCGE are excluded of this review.RESULTS:Among our 39 patients with PCGE, 16 different diagnoses were established. After combining our PCGE causes with those described in the literature, we propose a practical approach. Besides physiologic post-contrast enhancement of small vessels, three pathologic conditions may exhibit PCGE: (1) small collateral artery network seen in Moyamoya syndrome, (2) small veins congestions related to developmental or acquired venous outflow disturbance, and (3) disorders causing small vessels BBB disruption indicated by T2 and FLAIR hyperintensities in the corresponding areas of PCGE. Disruption of the BBB could be caused by a direct injury of the endothelial cell, as in posterior reversible encephalopathy syndrome, Susac syndrome, and radiochemotherapy-induced injuries, or by an angiocentric cellular infiltrate, as in inflammatory disorders, demyelinating diseases, host immune responses fighting against infections, prelymphoma states, lymphoma, and in CLIPPERS.CONCLUSION:PCGE may conceal several causes, including physiological and pathological conditions. Nevertheless, a practical approach could improve its management and limit the indications of brain biopsy to very specific situations

International audience; We report the third case of amyotrophic lateral sclerosis related to p.E121G Superoxide dismutase-1 (SOD1) mutation. Besides a sporadic presentation and a slow progressive course, as described in the 2 previously cases, our patient presented with prominent sensory and cerebellar signs. This case report strengthens that p.E121G should be considered as a causal mutation. Slowly upper and lower motor neuron degeneration, even with non-motor clinical features, should prompt a sequencing of SOD1.

We present a 48-year-old mother [with history of seizures, migraine, diabetes mellitus, neurosensory hearing loss, short stature, cognitive deficit, and ataxia, and onset of stroke-like episodes 3 years after initial magnetic resonance imaging (MRI)] and her 20-year-old son (with history of one strokelike episode at the moment of the initial MRI, and another stroke-like episode 3 months later, in absence of other signs) with mitochondrial encephalopathy, lactic acidosis, and stroke-like episodes (MELAS, m.3243A [G tRNALeu[UUR] mutation), both showing on computed tomography (CT) bipallidal microcalcifications and on initial 3-T T2*-weighted MRI (TE = 13 ms, TR = 475 ms) large bilateral hypointensities in the pallidum and the substantia nigra (Fig. 1). Basal ganglia showed normal intensities on T1-weighted and fluid-attenuated inversion recovery (FLAIR) imaging, and very slight hypointense (i.e., nearnormal) signal on T2-weighted imaging (Fig. 1). One strokelike lesion (accompanying stroke-like symptoms) was seen on initial MRI in the son, and another stroke-like lesion occurred 3 months later, and the mother presented strokelike lesions 3 years after the initial MRI (Fig. 2). Both patients also showed cerebral and cerebellar atrophy, and slight bilateral posterior leukoencephalopathy on MRI (Fig. 2). MRI follow-up (3 years in the mother, and 3 months in the son) did not show evolution of the T2*-weighted hypointensities in the pallidum and the substantia nigra. Radiological abnormalities (sometimes absent or discrete, especially in the early stage of the disease) encountered in MELAS include cerebral and cerebellar atrophy, basal ganglia signal changes, stroke-like lesions, and leukodystrophy. Extensive calcifications are the most frequently encountered basal ganglia abnormalities in MELAS, most frequently described on CT and only rarely on MRI in the literature [1]. Basal ganglia calcifications in our two patients were discrete on CT. The extent of the MRI abnormalities, when compared with CT, may be explained by the magnetic susceptibility effect of T2*weighted imaging (potentially exaggerated by the highfield 3-T MRI used in our patients) and/or the presence of microscopic calcium and iron deposition [2, 3]. Both calcium and iron deposition in the globus pallidus have been demonstrated earlier on autopsy in MELAS [2]. T2and diffusion-weighted MRI changes and necrotizing lesions on autopsy have been reported in the substantia nigra in another mitochondrial syndrome, i.e., Leigh syndrome [4, 5]. In a general population, incidence of basal ganglia calcifications on CT scan varies between 0.6 and 0.8 % [6, 7]. Incidence increases with increasing age [7, 8]. In the elderly, basal ganglia calcifications are encountered in 38.7 % of patients on CT scan, with the globus pallidus as the most frequently involved structure [9]. The precise agematched incidence, the MRI-based incidence of age-related basal ganglia calcifications, and the pathophysiology explaining the predominant involvement of certain substructures (e.g., the pallidum in our MELAS patients and in the elderly) are unknown. When MELAS is suspected clinically, T2*-weighted MRI imaging (and thus not only CT scan) may be an interesting additional radiological tool to detect discrete calcium and/or iron deposition, especially in young patients (in whom age-related basal ganglia calcifications are less D. Background. Thalamic lesions are seen in a multitude of disorders including vascular diseases, metabolic disorders, inflammatory diseases, trauma, tumours, and infections. In some diseases, thalamic involvement is typical and sometimes isolated, while in other diseases thalamic lesions are observed only occasionally (often in the presence of other typical extrathalamic lesions).Summary. In this review, we will mainly discuss the MRI characteristics of thalamic lesions. Identification of the origin of the thalamic lesion depends on the exact localisation inside the thalamus, the presence of extrathalamic lesions, the signal changes on different MRI sequences, the evolution of the radiological abnormalities over time, the history and clinical state of the patient, and other radiological and nonradiological examinations.

A 70-year-old woman presented with recurrent episodes of headache and (left-sided and sometime bilateral) upper limb paresthesias associated with progressive mild cognitive deficit, in the absence of ataxia or hearing loss. MRI showed the presence of superficial siderosis (SS) of the CNS in the brain (predominant in the posterior fossa) and along the spinal cord (Fig. 1). Contrast-enhanced MRI (Fig. 1) and Time-resolved imaging of contrast kinetics (TRICKS; GE Healthcare, Milwaukee, WI; Supplementary Video) revealed the presence of a vertebral hemangioma at T4 level [1]. CSF analysis revealed the presence of nine red blood cells/mm, in the absence of other abnormalities. CNS SS has been described in association with a multitude of (often vascular) CNS abnormalities [2]. In our case, SS was probably due to repetitive subarachnoid bleeding of the vertebral hemangioma. Goodpasture's disease with central nervous system involvement is extremely rare. We report a 62-year-old woman with Goodpasture's disease (GD) associated with the presence of perinuclear anti-neutrophil cytoplasmic antibodies, complicated by spinal subarachnoid hematomas and cerebral infarctions. In spite of aggressive treatment, the patient died. GD and anti-neutrophil cytoplasmic antibodies vasculitis should be suspected in patients presented with renal insufficiency with spinal and/or brain involvement.

Sir, We read with great interest the recent article published in Brain by Pittock et al. (2010), who reported eight patients with clinical, radiological and pathological features of brainstem involvement responsive (but dependent) to steroid treatment. All the reported patients shared common clinical and MRI findings: gait ataxia, diplopia, patchy increased T2 signal and gadolinium enhancement within the pons and the middle cerebellar peduncles, the presence of oligoclonal bands on CSF analysis, and normal cerebral angiography. Involvement of the medulla, the cerebellum, the midbrain, the basal ganglia, the corpus callosum and the spinal cord has also been described. Brain biopsy of four patients found white matter T-lymphocytic infiltrate with perivascular predominance. Steroids led to clinical and neuroradiological improvement in all patients, with steroid dependency in three (mean follow-up: 22 months, range: 7–144 months).
