Peripheral giant cell granuloma (PGCG) is a reactive exophyt-ic lesion occurring on the gingiva and alveolar ridge usually as a result of local irritating factors. PGCG is mostly located on the mandibular anterior region and it is frequently seen in 4-6 decades. PGCG is generally smaller than 2 cm, may cause mobility on related teeth and its clinical appearance looks like a liver. Surgical excision is the main treatment choice and recurrence occurs rarely. In this report a 18-year-old female patient's lesion, located on maxillary premolar-molar region and histo-pathologically diagnosed as PGCG is presented with its treatment and 20 months follow-up period. The case is unusual compared to the literature regarding its location and size. The aim of this case report is to show that, exophytic lesions which are seen in the oral and maxillofacial region may reach enormous dimensions if they are neglected by the patients or local irritating factors are not eliminated.
