Purpose: We aimed to investigate the role of [68 Ga]Ga-FAPI-04 PET/CT and uptake patterns of primary and metastatic lesions in patients with renal cell carcinoma (RCC). Methods: Twenty patients with a suspicious lesion considered primary renal malignancy or a history of RCC were included in our study. Two patients were excluded from further analyses due to other confirmed malignancies. Six patients were newly diagnosed, while the indication of 12 patients was restaging. All patients underwent [68 Ga]Ga-FAPI-04 and [18F]F-FDG PET/CT. SUVmax and tumor-to-background ratio (TBR) of primary (n = 7) and local recurrent lesions (n = 6) and lymph node (n = 26), lung (n = 32), bone (n = 5), and other metastases (n = 14) were compared between the two tracers. Results: We detected 90 lesions in 18 patients with varying FAPI and FDG uptake values on both PET/CT. The median TBR of FAPI-PET/CT of all lesions was higher than TBR of FDG-PET/CT with statistically significance (5.6 vs. 2.1, p < 0.001). In primary and recurrent lesions, the median SUVmax, TBR, and tumor volume on FAPI-PET/CT were higher than FDG-PET/CT. The median SUVmax of lung lesions on FAPI-PET/CT was statistical significantly higher than FDG-SUVmax (3.8 vs. 1.8, p = 0.02). The median of FAPI-SUVmax on primary lesions was lower in the early stage based on TNM compared to the advanced stage. FAPI-SUVmax in 49% of all lesions were SUVmax ≥ 6, and 13% were SUVmax ≥ 10. In patient-based analyses, seven patients (39%) had at least one lesion with FAPI-SUVmax ≥ 10; 12 patients (67%) had at least one lesion with FAPI-SUVmax ≥ 6. Conclusion: This study showed the potential utility of [68 Ga]Ga-FAPI-04 PET/CT showing promising results in RCC. We have presumed that FAPI-PET/CT may be performed for complementary imaging modality providing prognosis and possibility of theranostic application in selected patients. [ABSTRACT FROM AUTHOR]

Our purpose was to determine whether there is a clinical benefit to add lower-limb imaging in 68Ga-labeled prostate-specific membrane antigen (PSMA) PET/CT scans for patients with prostate cancer. Methods: In total, 701 patients with prostate cancer who underwent 68Ga-PSMA PET/CT were evaluated retrospectively. All patients underwent additional lower-limb imaging. Images were reanalyzed by experienced nuclear medicine physicians, and metastatic sites were documented. The prostate-specific antigen (PSA) level and Gleason score were also compared with 68Ga-PSMA PET/CT findings. Results: In 601 patients (85.7%), at least 1 tumoral lesion was observed on 68Ga-PSMA PET/CT. The number of patients with bone metastasis in 2 forms was 278 patients (39.6%); 108 (15.4%) were oligometastatic (<4 metastases) and 170 (24.2%) were multimetastatic (≥4 metastases). In lower-limb imaging, bone metastasis was detected in 61 patients (8.7%), the specific locations of which were as follows: middle-distal femur (n = 54), tibia (n = 19), fibula (n = 24), and calcaneus (n = 1). Lower-limb metastasis was detected mostly in symptom-positive patients (70.1%) but in only 4% of the symptom-negative group. All patients with lower-extremity metastasis also had multiple bone metastases shown on limited whole-body 68Ga-PSMA PET/CT. The median PSA level was significantly higher in multimetastatic patients with lower-limb metastasis than in those without lower-limb metastasis (P < 0.001, Mann-Whitney U test), but no statistical differences was found in terms of Gleason score (χ2 = 0.042, P = 0.837). According to receiver-operating-characteristic analysis, PSA has a good predictive value for detecting lower-limb metastasis, with 76.6% sensitivity and 72% specificity (using a reference cutoff PSA level of 24 ng/mL [area under the curve, 0.81; 95% confidence interval, 0.74-0.87]). Conclusion: Lower-limb imaging did not change the metastatic status of disease or significantly affect the therapeutic approach. However, if multimetastatic patients present relevant symptoms for lower-limb metastasis, it could be beneficial to consider including lower-limb imaging for possible palliative therapies. Purpose: The objective of this study is to determine the range of SUV of 68Ga-DOTATATE in normal organs and tumoral lesions and establish uptake unrelated to NET. Materials and methods: One hundred and twenty patients (57 men, 63 women), who underwent Ga-DOTATATE PET/CT imaging in our institution were analyzed. Patients were indicated for Ga-DOTATATE PET/CT imaging to detect primary tumor or metastasis of suspected or previously known NET, to determine SSTR positivity and to detect occult source of ectopic Cushing syndrome. Normal range of uptake was calculated for the organs that were proven to have no pathology by either conventional radiological imaging or clinical follow-up, using SUV as a semiquantitative measure. Uptake and tumor to background ( T/ B) ratios of tumoral lesions in liver, pancreas, bone, brain and lymph nodes were calculated. Uptakes due to lesions unrelated to NET were also documented. Results: Significant uptake was found in spleen, kidneys, adrenal glands, liver and pituitary gland with mean SUV of 24.67, 14.30, 13.73, 9.12 and 9.74 respectively. Uptake was measured separately for the pancreatic head and body separately, however, besides a slightly heterogeneous uptake; the difference was not statistically significant. Uptake in the tumoral lesions had high ( T/ B) ratios with mean SUV of 28.72, 25.21, 18.28, 34.73 and 12.59 for liver, pancreas, bone, brain and lymph nodes, respectively. Incidental benign tumoral lesions were detected in 3 patients (2.5 %) which were meningioma and fibrous dysplasia demonstrating significant and breast fibroadenoma demonstrating mild Ga-DOTATATE uptake. Non-neoplastic processes were detected in 4 patients (14.1 %), including postsurgical inflammation, reactive lymph nodes, arthritis and demonstrated faint to mild Ga-DOTATATE uptake, with the exception of significant uptake in accessory spleen. Conclusion: Ga-DOTATATE has high T/ B ratio with physiological biodistribution comparable to its counterparts. However, the presence of SSTRs in benign and malignant lesions unrelated to NET may be challenging in interpretation particularly where the physiological uptake is variable. [ABSTRACT FROM AUTHOR]

Purpose: The aim of this study was to evaluate the potential role of [18F]FDG positron emission tomography/computed tomography (PET/CT) in the characterization of thymic epithelial tumors (TETs). Materials and Methods: A total of 73 patients who underwent preoperative [18F]FDG PET/CT were included in this study. Visual total score (VTS), maximum standard uptake values (SUVmax), metabolic tumor volume (MTV), total lesion glycolysis (TLG), and heterogeneity index (HI) parameters were analyzed to investigate the prediction of histopathologic grade and advanced stage. Results: The cohort included 26 patients with low-grade thymoma (LGT), 36 patients with high-grade thymoma (HGT), and 11 patients with thymic carcinoma (TC). Ninety-one percent of TC had VTS >2, whereas 31% of LGT and 75% of HGT had VTS >2. SUVmax, MTV, and TLG were statistically significantly higher in the TC group than in both thymoma and HGT. Using the cutoff value of 7.25 for SUVmax, TC was differentiated from thymomas with 91% sensitivity and 74% specificity. TC had significantly lower HI values than thymomas. HI parameters showed good diagnostic ability to differentiate TC from thymoma and TC from HGT. SUVmax, MTV, and TLG were significantly higher in advanced-stage disease than in early-stage disease. Conclusions: Visual and quantitative parameters can reliably predict both advanced disease and the grade of primary tumor in TETs. Therefore, as a promising metabolic imaging method, [18F]FDG PET/CT makes important contributions to preoperative evaluation in routine clinical practice. [ABSTRACT FROM AUTHOR]
