The flexion relaxation phenomenon (FRP) is characterized by the reduction of paraspinal muscle activity at maximum trunk flexion. The FRP is reported to be altered (persistence of spinal muscle activity) in nonspecific chronic low back pain (NSCLBP) and is considered a promising biomarker. The aim of this systematic review was to synthetize current knowledge on FRP in the NSCLBP population regarding prevalence, the reliability of FRP measurement using surface electromyography (sEMG), the average value, and variation of the relaxation ratios (RR). Five databases were searched (PubMed, EMBASE, Web of Sciences, Cochrane Library, Pedro). A qualitative analysis was done for all included studies and meta-analysis studying prevalence, mean value of flexion relaxation ratio (FRR) and extension relaxation ratio (ERR), and difference between asymptomatic and NSCLBP FRR. Twenty-seven studies were included for qualitative analysis and 21 studies for meta-analysis. The prevalence of the altered FRP was 55% (95%CI [32–79%]) in the NSCLBP population. The studies on reliability reported good within-session and between-session reproducibility. In the NSLBP population, the mean FRR was 2.96 (95%CI [2.02; 3.90]) and the mean ERR was 4.07 (95%CI [2.08; 6.07]). The difference between asymptomatic and NSCLBP FRR was − 1.19 (95%CI [− 1.92, − 0.45]). In all meta-analysis, the I2 index was > 80%. An altered FRP is frequently found in NSCLBP population using sEMG and the test has a good reproducibility. The difference between asymptomatic and NSCLBP FRR was significant. Nevertheless, considering the high heterogeneity observed, additional research is required to confirm the value of RR.

Background Trunk flexion is a common exercise during daily activities. Flexion relaxation phenomenon (FRP) occurs during forward bending in which there is a sudden silence of erector spinae (ES) muscles. The pattern of forward bending differs in yoga practitioners. This learned pattern probably predisposes yogis to injuries. The hypothesis of this study was that FRP differs in yogis in comparison to non-yogis individuals. Methods This observational cross-sectional study was performed on 60 women assigned into yogis and non-athlete groups. Each participant was asked to bend forward and then return to the initial position. ES activity was recorded at L3 level, 4 cm from mid line during the trial. Trunk inclination and lumbar flexion angles were calculated at FRP onset and cessation moments. Results The FRP occurred in 80% of yoga practitioners in comparison to 96.7% in the control group. Trunk inclination angle was significantly greater at FRP initiation in yogis compared to control group. Lumbar flexion angle was not different between the groups. Conclusions It is concluded that the altered pattern of forward bending observed in yogis may change patterns of ES muscles activity if it becomes part of a person's daily lifestyle which might predispose these muscles to fatigue and subsequent injuries; however, further studies are warranted for clarification.

Axial spondyloarthritis (axSpA) is a chronic rheumatic disease characterized by the presence of inflammatory back pain. In patients with chronic low back pain, the lumbar flexion relaxation phenomenon measured by surface electromyography (sEMG) differs from that in healthy individuals. However, sEMG activity in axSpA patients has not been studied. The purpose of this study was to analyze the flexion relaxation phenomenon in axSpA patients. A study evaluating 39 axSpA patients and 35 healthy controls was conducted. sEMG activity at the erector spinae muscles was measured during lumbar full flexion movements. sEMG activity was compared between axSpA patients and the controls, as well as between active (BASDAI ≥ 4) and non-active (BASDAI &lt, 4) patients. The reliability (using intraclass correlation coefficients (ICC)), criterion validity and discriminant validity using the area Under the curve (AUC) for the inverse flexion/relaxation ratio (1/FRR) were evaluated. Significant differences (p &lt, 0.05) were observed between axSpA patients and the control group in lumbar electric activity, especially during flexion, relaxation, and extension and in FRR and 1/FRR (0.66 ± 0.39 vs. 0.25 ± 0.19, respectively). In addition, significant differences were found between active and non-active but also between non-active and healthy subjects. The sEMG showed good reliability (ICC &gt, 0.8 for 1/FRR) and criterion validity. ROC analysis showed good discriminant validity for axSpA patients (AUC = 0.835) vs. the control group using 1/FRR. An abnormal flexion/relaxation phenomenon exists in axSpA patients compared with controls. sEMG could be an additional objective tool in the evaluation of patient function and disease activity status.

This study quantified the neck posture and fatigue using the flexion relaxation phenomenon (FRP) and craniovertebral angle (CVA), further, it compared the difference between the level of fatigue and neck posture induced by two types of monitors (regular fixed monitor and moving monitor). Twenty-three male participants were classified into two groups&mdash, the low-flexion relaxation ratio (FRR) group and the normal-FRR group, depending on the FRR value. All participants performed a document task for 50 min using both types of monitors. It was found that the FRR values significantly decreased after the documentation task. The CVA analysis showed that the moving monitor&rsquo, s frequency of forward head posture (FHP) was lower than that for the fixed monitor. Overall, the moving monitor worked better than the fixed monitor, this can be interpreted as proof that such monitors can reduce neck fatigue.

The purpose of the current study was to assess the changes in lumbar tissue load sharing mechanism and lumbar range of motion at different trunk flexion postures. Eight participants performed the tasks of changing lumbar posture from extended to flexed posture while maintaining fixed trunk postures (30°, 60°, or 90°). Results of the present study showed that FRP onset was observed in almost all the trials at two conditions of trunk posture (i.e. 60° and 90°). The results also showed that lumbar range of motion changed significantly between the different levels of trunk posture. Findings of the current study could help for better understanding of the dynamics of lumbar active and passive tissue loading during static trunk flexion.

[Purpose] The objective of this research was to examine the impact of cognitive load on the flexion relaxation phenomenon (FRP) during trunk flexion and return from flexion task. [Subjects and Methods] Twenty-two healthy subjects (18 males, 4 females) participated in the study. Each participant was exposed to 3 experimental conditions: no cognitive task, easy cognitive task and difficult cognitive task. Surface electromyography was used to measure lumbar erector spinae muscles activity level. Flexion relaxation ratio (FRR) was compared in order to assess the differences between the three experimental conditions during flexion and extension (FLX FRR and EXT FRR). [Results] The FRR was decreased with increase in cognitive difficulty; the difficult cognitive task was associated with significant lower value of FLX FRR in both sides. However, these changes were not significant in easy cognitive task. In addition, the EXT FRR was decreased in cognitive task conditions, but these results were not statistically significant except for difficult cognitive task condition in comparison to no cognitive task condition in left side. [Conclusion] These findings suggest that cognitive loading can affect FRP in healthy subjects.

Although several studies have investigated the back-muscle flexion–relaxation phenomenon (FRP), the effect of individual flexibility on the FRP has been discussed infrequently, with very limited data on the influence of flexibility training on the FRP. This study thus examined the effect of flexibility training on the change of back-muscle FRP pattern in relatively inflexible young men. We collected and analyzed the valid data from 20 male participants (10 each with high and low flexibility included in the control and trained groups, respectively) when flexing their trunks at seven trunk flexion positions (0°–90°, in increments of 15°); their erector spinae and hamstring activation, pelvic tilt, and lumbosacral angle were then recorded. After 7 weeks of flexibility training for the low-flexibility group, no difference in flexibility was discovered between this group and the control (originally high-flexibility) group. The trunk flexion experiment was then repeated. The results showed that before the training stage, the low-flexibility group had lower erector spinae and higher hamstring activation, a larger pelvic tilt, and a smaller lumbosacral angle. By contrast, after training, the erector spinae and hamstring activation, pelvic tilt, and lumbosacral angle were significantly changed, and no intergroup differences were observed in FRP patterns. The study results suggest that flexibility training changes lumbopelvic movement and thereby reduces the degree of the back-muscle FRP when trunk flexion is performed.

Summary Introduction Neck pain is a common musculoskeletal disorder, especially among skilled workers who must keep their necks in a flexed position frequently during the day. The present study investigated changes in cervical flexion-relaxation phenomenon parameters after sustained neck flexion. Methods The participants were 40 healthy subjects grouped by gender (20 females, 20 males). They were exposed to static neck flexion at the full angle of cervical flexion for 10 min. Each subject underwent three trials of cervical flexion and re-extension before and after this period. Differences in onset and cessation angle of flexion-relaxation phenomenon, maximum neck flexion angle, amplitude of neck muscle activation and flexion-relaxation ratio were evaluated. Results The maximum neck flexion angle significantly increased after sustained flexion. The onset of flexion-relaxation was significantly delayed during flexion, but cessation angle remained unchanged. Myoelectric activity of the cervical erector spinae muscles increased significantly after maintaining flexion, especially in female subjects. The flexion-relaxation ratio also decreased significantly. Conclusion It was concluded that 10 min of static flexion results in a delay in flexion-relaxation phenomenon and a shortened silence period. Also the cervical erector spinae muscles are required to be active longer and generate more activity. These neuromuscular changes may be a risk factor for neck pain.

The relationship between flexibility and the pattern formed by the surface electromyography activity of the back muscles while performing a dynamic trunk flexion-extension task is not yet thoroughly understood, although many previous studies have adopted it as their focus in the literature. Additionally, several studies have proposed technologies and algorithms to analyse the flexion-relaxation phenomenon, which is defined by myoelectric silence that occurs when the subject's torso exceeds a certain flexion angle. Before participating in the flexion-relaxation test, subjects involved in the data collection underwent medical examinations, in which their physical condition, perceived pain, and level of disability were reported in their anamnesis. During the flexion-relaxation test, which was conducted with 25 subjects with and without low back pain, subjects wore four surface electromyography electrodes positioned over the back muscles, as well as an inertial sensor to estimate trunk inclination.

[Purpose] This study evaluated the differences in the flexion-relaxation phenomenon (FRP) of the right and left erector spinae muscles in asymptomatic subjects and the effect of lumbar stabilization exercises on these differences. [Subjects and Methods] Twenty-six participants (12 in the exercise group and 14 in the control group) with a difference in the FRP in the right and left erector spinae muscles were recruited from among healthy students attending Silla University. The exercise group performed two lumbar stabilization exercises (back bridge exercise and hand-knee exercise) for 4 weeks. The control group did not exercise. [Results] No significant group-by-exercise interaction was found. The right and left erector spinae muscles did show a difference in FRP between the control and exercise groups (119.2 ± 69.2 and 131.1 ± 85.2 ms, respectively). In addition, the exercise group showed a significant decrease in post-exercise (50.0 ± 27.0 ms) compared to pre-exercise (112.3 ± 41.5 ms) differences in the right and left FRP. [Conclusion] These results suggest that lumbar stabilization exercises may counter asymmetry of the FRP in the erector spinae muscles, possibly preventing low back pain in the general population.

A cross-sectional comparative study.This study aimed to investigate the flexion-relaxation phenomenon (FRP), in standing trunk flexion and slumped sitting tasks, by comparing children and adolescents suffering from nonspecific chronic low back pain (NSCLBP) with controls (CTRL).The absence of the FRP can accurately discriminate adults with NSCLBP from those without during standing trunk flexion and slumped sitting tasks. Even if the FRP has been extensively studied in adults with NSCLBP, only one study has evaluated the FRP in adolescents, during a slumped sitting task, and this suggested that the FRP was also present in adolescents with NSCLBP.Thirty-seven children and adolescents with NSCLBP and 23 CTRL performed standing trunk flexion and slumped sitting tasks. All participants were equipped with surface electromyography (EMG) electrodes on the erector spinae longissimus (ESL) and multifidus (M) muscles and reflective markers on the spinous processes of C7, L1, and S1. Global (C7-S1), thoracic (C7-L1), and lumbar (L1-S1) trunk flexion absolute angle were measured. The FRP was reported using visual inspection and a flexion-relaxation ratio (FRR). A self-reference threshold was used to identify the time of FRP onset. Repeated-measures analysis of variance (ANOVA) was used to determine the main and interaction effects of task, group and muscle on FRR, and the relative maximal angle at FRP onset of the global trunk (C7-S1).Results showed three main findings: (1) the FRP's low sensitivity in discriminating between NSCLBP and CTRL participants in groups, tasks, or muscles; (2) similar observed maximal flexion angles in both groups during flexion tasks; and (3) similar observed relative maximal global trunk flexion angles at FRP onset in groups, tasks, and muscles.These results are not consistent with the literature on adults and could lead to modified therapeutic management of NSCLBP in children and adolescents.3.

Background The flexion relaxation phenomenon (FRP) has been widely investigated. Nevertheless, no study has been reported on the FRP as well as spasm response to large lifting load. The aim of this study was to evaluate the effect of large lifting load on the FRP response and spasm during execution of a flexion-extension exercise. Methods Twenty-two healthy male university students without low back pain history participated this study. Subjects randomly performed three trials of trunk flexion-extension cycles of 5 s flexion and 5 s extension in each of 4 conditions (three large lifting loads of 15, 20 and 25 kg and one lifting load of 0 kg for comparison). Surface EMG from bilateral erector spinae was recorded during the performance of a trunk anterior flexion-extension exercise. The relaxation phase was determined through the onset of electromyography (EMG) signals. Spasm was evaluated in the relaxation period. The mean normalized electromyography (NEMG) was derived from the raw EMG. Results Spasm was observed in more than 45% of the individuals and the intensity of muscle activation was increased by more than 78% in the relaxation phase. Conclusions A large lifting load could lead to a high prevalence of spasms as well as a high intensity of muscle activations on erector spinae muscle in the relaxation period, which may be associated with the development of low back disorder during the performance of a flexion-extension exercise.

The flexion–relaxation phenomenon (FRP) in back muscles is related to the lower back load. Although the FRP has been widely discussed, the effect of wearing jeans—a common attire in workplaces—on the FRP is unexplored. This study recruited 20 male participants with high and low flexibilities (10 each) and measured the related muscle activity and lumbopelvic movement at five trunk flexion angles (0°–90°) when wearing two types of bottom wears (swim trunks and fit jeans). Results revealed that the bottom wear type and trunk angle significantly affected all responses, whereas flexibility affected only erector spinae and hamstring activities. Participants with high flexibility and wearing swim trunks exhibited greater erector spinae activity than did those with low flexibility and wearing jeans. Thus, those who have low flexibility or are wearing jeans are likely to experience more activity reductions in erector spinae. However, the result with hamstring activity was the opposite. Wearing jeans limited participants’ pelvic rotation, forcing them to bend their lumbar spine further to complete trunk flexion, thus reducing erector spinae activity. A looser attire should be chosen to avoid constraining the pelvic rotation when work involves a deep trunk flexion. Relevance to industry People wearing jeans have limited pelvic movement and decreased erector spinae activity during deep trunk flexion, and this may further strain their interspinous ligaments. Thus, people should not wear fit jeans when in workplaces where their work involves stooping or trunk flexion exceeding 45° to minimize the load on their lower back.

Occupational tasks performed on uneven ground surfaces are common in agriculture and construction industries. The influence of unilaterally elevated ground surface on lumbar muscle flexion relaxation phenomenon (FRP) during trunk flexion and extension motion has been investigated in the current study. Ten subjects performed trunk flexion and extension motion on flat ground and two unilaterally elevated ground conditions while lumbar muscle EMG and trunk kinematics data were recorded. Results of this study demonstrated clear difference in bilateral lumbar muscle FRP under elevated ground conditions. The current finding can be used to better understand uneven ground surfaces as a risk factor for the development of low back pain.

The cervical flexion relaxation phenomenon (FRP) is a neck extensor myoelectric "silence" that occurs during complete cervical and lumbar flexion. In contrast to low back pain, the changes that occur during FRP in chronic neck pain (CNP) patients are still not clear. The aim of this study was to assess the characteristics of this phenomenon in the cervical region in CNP patients and controls.Twenty-two women (23 ± 2.62 years) with chronic non-specific neck pain and 21 healthy women (23.4 ± 1.68 years) participated in this study. They accomplished a cervical flexion and extension from neutral position. Neck angle and surface electromyographic activity of cervical erector spinae (CES) and upper trapezius muscles were recorded. Appearance, onset and offset angle of the FRP were analysed and compared between the two groups.There were significant differences in the appearance of FRP between the two groups (P ≤ 0.001). The FRP in the CES muscles was observed in 85.7 % of healthy subjects and in 36.3 % of CNP patients, and no FRP was observed in the upper trapezius. Results of this study show that the onset and offset of FRP parameters were significantly different between the two groups (P ≤ 0.001).The results of the present study indicate that FRP in CNP patients was seen less than the healthy subjects, and moreover the FRP period was reduced in CNP patients. Our results also suggest that the changes in FRP of CNP patients may be due to the increased CES activity in these patients.

The purpose of the study was to investigate the effect of spinal shrinkage on the characteristic of flexion relaxation phenomenon in erector spinae muscle. Twelve male university students volunteered for this study. The spinal shrinkage was created with a load of 20% body weight on shoulder in 10 min. Each performed three trials of lumbar flexion–extension with the cycle of 5 s flexion and 5 s extension in standing before and after shrinkage. Surface electromyography from right erector spinae muscle as well as trunk flexion performance was recorded synchronously in video camera. A one-way ANOVA with repeated measures was used to evaluate the effect of shrinkage. The results showed that there was a significant (p < 0.001) stature change with mean magnitude of shrinkage 5.9 mm. The erector spinae was active through a significantly larger angle during flexion and began activity significantly earlier during extension after shrinkage. It was concluded that shrinkage caused by prolonged compressive loading will elicit the change of flexion relaxation characteristic, which may be closely related to low back disorder.

The flexion-relaxation phenomenon (FRP) is well documented at end-range lumbar spine flexion in both standing and sitting however, the FRP has been insufficiently investigated in cervico-thoracic musculature. The aim of this study was to determine whether the FRP occurs during forward flexion of the neck, in lumbo-pelvic sitting, amongst a pain-free population. Surface electromyography (EMG) was used to measure muscle activation in 20 (10 men, 10 women) asymptomatic subjects in selected cervico-thoracic muscles during four, 5-s phases (upright posture, forward flexion, full flexion and return to upright) while subjects were positioned in lumbo-pelvic sitting. Spinal kinematics were simultaneously measured using an electromagnetic motion tracking device. No FRP was observed in upper trapezius or thoracic erector spinae (T4). When using visual methods to determine the presence/absence of the FRP, five subjects were believed to show evidence of the FRP in the cervical erector spinae. However, when using various non-visual criteria to determine the existence of the FRP, substantial variations (0-13 subjects) were evident. We recommend that criteria based upon relatively large differences in muscle activation should be considered when defining the FRP. These findings are of significance for future investigations examining specific cervical pain disorders.

Significant differences in surface electromyography (SEMG) have been reported between persons with low back pain (LBP) and normal, healthy controls. This manuscript presents a systematic meta-analytic review of studies examining SEMG differences between these groups. Forty-four articles were identified using MEDLINE and a review of reference lists in articles. For static SEMG, the largest effect size was observed for SEMG while standing, with subjects having LBP demonstrating higher SEMG. The effect size for flexion/relaxation measures was found to be very high (d 1.71). Studies examining SEMG during isometric exercise or muscle recovery following exercise produced inconsistent findings. Sensitivity and specificity of SEMG for dynamic SEMG measures averaged 88.8% and 81.3%. Most classification schemes were statistically determined and utilized a combina- tion of measures. Only one published study prospectively validated a classification scheme. SEMG measures of flexion-relaxation appear to distinguish LBP subjects from controls with good accuracy, and the sensitivity and specificity of SEMG can be increased by using multiple measures. Further research is needed to determine the combination of measures that are cost-effective, reliable, valid and discriminate with a high degree of accuracy between healthy persons and those with LBP. Perspective: SEMG is a simple and noninvasive measure of muscle activity. SEMG measures hold promise as an objective marker of LBP.

This study evaluated the flexion-relaxation phenomenon (FRP) and flexion-relaxation ratios (FR-ratios) using surface electromyography (sEMG) of the cervical extensor muscles of computer workers with and without chronic neck pain, as well as of healthy subjects who were not computer users. This study comprised 60 subjects 20-45years of age, of which 20 were computer workers with chronic neck pain (CPG), 20 were computer workers without neck pain (NPG), and 20 were control individuals who do not use computers for work and use them less than 4h/day for other purposes (CG). FRP and FR-ratios were analyzed using sEMG of the cervical extensors. Analysis of FR-ratios showed smaller values in the semispinalis capitis muscles of the two groups of workers compared to the control group. The reference FR-ratio (flexion relaxation ratio [FRR], defined as the maximum activity in 1s of the re-extension/full flexion sEMG activity) was significantly higher in the computer workers with neck pain compared to the CG (CPG: 3.10, 95% confidence interval [CI95%] 2.50-3.70; NPG: 2.33, CI95% 1.93-2.74; CG: 1.99, CI95% 1.81-2.17; p0.001). The FR-ratios and FRR of sEMG in this study suggested that computer use could increase recruitment of the semispinalis capitis during neck extension (concentric and eccentric phases), which could explain our results. These results also suggest that the FR-ratios of the semispinalis may be a potential functional predictive neuromuscular marker of asymptomatic neck musculoskeletal disorders since even asymptomatic computer workers showed altered values. On the other hand, the FRR values of the semispinalis capitis demonstrated a good discriminative ability to detect neck pain, and such results suggested that each FR-ratio could have a different application.

Background Characterizing the lumbar muscle flexion-relaxation phenomenon is a clinically relevant approach in understanding the neuromuscular alternations of low back pain patients. Previous studies have indicated that changes in stance posture could directly influence trunk kinematics and potentially change the lumbar tissue synergy. In this study, the effects of stance width and foot posture on the lumbar muscle relaxation responses during trunk flexion were investigated. Methods Thirteen volunteers performed trunk flexion using three different stance widths and ‘toe-forward’ or ‘toe-out’ foot postures (six conditions in total). Lumbar muscle electromyography was collected from the L3 and L4 level paraspinals; meanwhile three magnetic motion sensors were placed over the S1, T12, and C7 vertebrae to track lumbar and trunk kinematics. The lumbar angle at which muscle activity diminished to a near resting level was recorded. At the systemic level, the boundary where the internal moment started to shift from active to passive tissues was identified. Findings For the L3 paraspinals, the flexion relaxation lumbar angle reduced 1.3° with the increase of stance width. When changed from ‘toe-forward’ to ‘toe-out’ foot posture, the flexion relaxation lumbar angle reduced 1.4° and 1.1° for the L3 and L4 paraspinals respectively. However, the active and passive lumbar tissue load shifting boundary was not affected. Interpretation Findings of this study suggest that changes in stance width and foot posture altered the lumbar tissue load sharing mechanism. Therefore, in a clinical setting, it is critical to maintain consistent stance postures when examining the characteristics of lumbar tissue synergy.

Objectives There are various methods to quantify the flexion-relaxation phenomenon (FRP); however, there is little standardization. This study aimed to evaluate the performance of various quantification methods in terms of their ability to identify lumbar erector spinae flexion-relaxation during standing forward trunk flexion. Methods The study was a cross-sectional design in a laboratory setting. Lumbar erector spinae activation levels were measured in 12 male participants performing full trunk flexion movements. Electromyographical signals were assessed using 16 criteria within 4 quantification methods (visual, statistical, threshold, ratio), and the sensitivity of each was assessed relative to the benchmark criterion (visual inspection of raw electromyography data). Results Visual inspection and most of the threshold and ratio criteria displayed the highest sensitivity. On average (SD) across the 16 criteria, FRP was positively identified 21.6 (6.2) times of 24 data sets (12 participants, 2 muscles). The visual inspection criteria positively identified FRP in all 24 trials, whereas the statistical method did not identify FRP at all ( P = .44 and P = .46 for the left and right sides, respectively). The threshold and ratio criteria positively identified FRP 23.2 (1.5) and 22.5 (3.7) times, on average, respectively. Results from criteria based on differences between upright and fully flexed muscle activation tended to be conservative in FRP identification. The methods were classified as reliable or nonreliable, based on their sensitivity when specific characteristics were evident in the electromyography signals. Conclusions Although many of the criteria identified FRP with 100% sensitivity, others produced unrealistic results. The latter may be suitable for other experimental designs or may require reevaluation regarding their ability to identify FRP. Although visual inspection, threshold, or ratio methods performed well and may be appropriate for either biomechanical or clinical research, the threshold method provided the optimal trade-off between performance, consistency, and feasibility for these data.

Investigations corresponding to the affected factors of the cross-correlation of pair muscles are limited though muscle activation patterns of bilateral erector spinae (ES) during trunk flexion–extension performance in standing have been utilized as an indicator in the evaluation of low back pain condition. The purpose of the study is to evaluate the effect of leg support on the cross-correlation of bilateral ES, and to test if the average of bilateral ES could weaken this effect. Twenty male university students volunteered for this study. Each performed the trunk flexion–extension in three leg support conditions randomly with the condition of single left leg support, double leg support and single right leg support, respectively. Each condition included three trials of trunk flexion–extension with the cycle of 5 s flexion and 5 s extension in standing. Surface electromyography from the right ES muscle as well as from the left one was recorded. The cross-correlation both in pair muscle of right–left ES and in pair muscle of right-average of bilateral ES was calculated in the flexion as well as extension period. A one-way ANOVA with repeated measures was used. The results showed that leg support has some effect on cross-correlation of bilateral ES, which causes the absolute value of phase lag to be significantly larger in flexion period. It is suggested that this effect could be weakened by the average of bilateral ES through significantly increasing the cross-correlation coefficient, and decreasing the absolute value of phase lag.

Objective: The paraspinal muscles often fail to relax on forward flexion in many persons with low-back pain. The goal of this prospective study was to determine whether this abnormal lack of a flexion-relaxation phenomenon corrects after lumbar diskectomy for symptoms of radiculopathy with low-back pain. Design: Electromyographic testing was performed on 17 patients before and 30 days after lumbar diskectomy. Results: Although pain improved significantly (P < 0.05), the flexion-relaxation phenomenon did not improve. Conclusions: Failure to recover muscle relaxation while pain is relieved suggests another mechanism for paraspinal activity.

This paper describes the investigation of Surface Electromyography (sEMG) signals of back muscles during forward flexion and extension in healthy and low back pain (LBP) women. Flexion Relaxation Phenomenon (FRP) of EMG was observed and quantitatively analyzed. There were three groups of Malaysian women aged between 20 to 50 years old volunteers participated in this study. The groups were group 1, 2, and 3 which consist of 5 healthy women, 5 LBP patients with FRP and 5 LBP patients without FRP with respectively. Every participant was trained to perform two types of forward flexion; maximum forward flexion and 90° forward flexion with hands on the knees. The epidemiology classify cyclic lumbar flexion as a risk factor for the development of cumulative low back disorder. Experimental biomechanical data confirming the epidemiology in humans are lacking. The purpose of this study, therefore, is to investigate the flexion relaxation response to sustained cyclic lumbar flexion in humans.Twelve normal college aged males performed deep cyclic lumbar flexion at 0.1 Hz for 9 min while recording lumbar paraspinal electromyogram and kinematic data.The most important observation of the study was the significant increase in the angular excursion of myoelectric silence during the deep part of the flexion phase. The observed increase in myoelectric silence consisted of earlier cessation of EMG during flexion and delayed activation of trunk extensors during extension. EMG magnitude, during flexion, increased approximately 30% over trials (P0.0001), and a smaller (10%), but, significant (P0.02) EMG magnitude increase was also observed during the extension phase of the cycle. Spasms, an indication of micro damage to viscoelastic tissues, were sporadic and appeared more frequently later in the session and mostly during the silent period.It was concluded that increased myoelectric silence during prolonged cyclic flexion-extension demonstrates an enhanced flexion-relaxation phenomenon which reduces lumbar stability and may be detrimental to low back health. The presence of spasms confirm that sustained cyclic lumbar flexion results in micro damage in the viscoelastic tissues. Overall, a neuromuscular disorder was evoked due to a relatively short period of unloaded cyclic lumbar flexion.

Objectives. To determine if repeated spinal flexion and loading modulate the deactivation of lumbar muscles near full flexion (flexion–relaxation). Design. Repeated measures experimental study of the effect of repetitive trunk flexion and added mass on the flexion–relaxation phenomenon. Background. Repeated flexion causes muscular fatigue, creep of passive tissues and diminished protective reflexes. However, flexion–relaxation has not been studied in repeated trunk flexion, and could be related to the increased risk of low-back disorders. Methods. Thirty healthy young subjects performed 100 trunk flexion movements between standing and full flexion. Erector spinae electromyography and lumbar spine flexion were measured during cycles 1–10 (no load), 11–20 (performed holding a mass in the hands), 81–90 (mass in the hands) and 91–100 (no load). The spinal flexion angle at myoelectric silence and full flexion were extracted from each movement cycle. Results. Twenty-three of the 30 subjects showed flexion–relaxation throughout the repeated trunk flexion. The flexion–relaxation and maximum flexion angles increased at the end of the experiment; the flexion–relaxation angle relative to the maximum flexion angle also increased. This effect depended on the load condition; the flexion–relaxation and maximum flexion angles showed a greater increase in the unloaded than loaded condition. Conclusions . The flexion–relaxation phenomenon was changed due to repeated trunk flexion. The increases in flexion–relaxation angle likely involve changes to the neuromuscular control system. Relevance The deactivation of the erector muscles near full flexion occurs at a greater spinal flexion angle and a greater proportion of maximum spinal flexion following repeated spinal flexion. This may be related to the increased risk of injury associated with repeated flexion.

Objective. The purpose of this study was to examine the myoelectric activity of the erector spinae muscles of the back in order to determine if the flexion relaxation phenomenon occurs in seated forward flexion or slumped postures. Background. The flexion relaxation phenomenon during standing forward flexion is well documented. However, flexion relaxation in seated forward flexion has not been studied. It is possible that flexion relaxation could be linked with low back pain that some individuals experience during seated work. Methods. Twenty-two healthy subjects (11 males, 11 females) participated in the study. Surface electromyography was used to measure the level of muscle activity at the thoracic and lumbar levels of the erector spinae muscles. An electromagnetic tracking device measured the three-dimensional movement of the lumbar spine. Five trials each of standing and seated forward flexion were performed. Results. A slumped sitting posture yielded flexion relaxation of the thoracic erector spinae muscles, whereas the lumbar erector spinae muscle group remained at relatively constant activation levels regardless of seated posture. Thoracic erector spinae silence occurred at a smaller angle of lumbar flexion during sitting than the flexion relaxation angle observed during standing flexion relaxation. Conclusions. Since the myoelectric activity of the lumbar erector spinae did not increase, it is likely that the passive tissues of the vertebral column were loaded to support the moment at L4/L5. Ligaments contain a large number of free nerve endings which act as pain receptors and therefore could be a potential source of low back pain during seated work. Relevance. STUDY DESIGN Nonrandomized control trial. OBJECTIVE To determine if the variations of speed and loading conditions during trunk flexion-extension could influence the times of occurrence and disappearance of the electrical silence of the erector spinae muscles, the degrees of lumbar flexion at those instants, and the relative lumbar motion time. SUMMARY OF BACKGROUND DATA It has been suggested that varying either the speed of movement or the load on the trunk during trunk flexion-extension movements may influence the flexion-relaxation phenomenon or the kinesiologic data. However, no study dealt with the simultaneous effect of the speed of movement on the spine rhythm and on the occurrence of the electrical silence of the erector spinae. METHODS A total of 22 pain-free volunteers performed a series of trunk flexion-extension movements varying the speed and load. The motion of the lumbar spine ( degrees ) and the integrated electromyography (microV) of erector spinae muscles were simultaneously recorded. Two measures were calculated: the percentage of the maximum lumbar spine flexion at the instants when changes of electrical activity represented the beginning and end of the electrical silence and the relative lumbar spine motion time during trunk flexion and extension movements. RESULTS The increase in speed of movement significantly increased the relative lumbar flexion time and significantly reduced the relative lumbar extension time (t = 2.49 and t = 2.25, P < 0.05); furthermore, it significantly delayed the appearance of the electrical silence in the range of flexion (t = 3.52, P < 0.01). There was no significant effect from a change in load. CONCLUSIONS The relative spine motion time differed depending on the direction of movement, being longer during trunk flexion and shorter during extension. The increase in speed of movement produced greater differences in the relative time between trunk flexion and extension; furthermore, it delayed the appearance of the electrical silence of the erector spinae muscles in the range of flexion.

Purpose of study: The flexion-extension (FR) phenomenon represents lumbar extensor muscle relaxation in electromyograph (EMG) reading near full flexion. It can be reproducibly assessed in normal subjects and found intermittently in CLBP patients. Few recent studies have examined a potential relationship between inclinometric lumbar range-of-motion (ROM) measures and the surface EMG signal. This two-part study described the use of FR in assessing CLBP patients before and after rehabilitation, and establishing a normative database (Part 1), for subsequent use in comparison to CLBP patients (Part 2). We discuss the clinical utility of combined EMG and ROM measurements in assessing the FR phenomenon as a test to potentially assist in planning rehabilitation programs, guide patients' rehabilitation progress and identify early posttreatment failures. Methods used: In Part 1, 12 normal subjects were evaluated to examine reliability of EMG signal readings and ROM measures at both FR and maximum voluntary flexion (MVF). The mean sEMG signal, as well as the gross, true and sacral lumbar ROM measurements were recorded as normative data. In Part 2, 54 CLBP patients referred for tertiary rehabilitation underwent EMG and ROM measurement before rehabilitation. Of this group, 20 patients were not appropriate for rehabilitation or failed to complete the course of treatment and were assessed only once. Program completers were retested with the identical methodology after rehabilitation (n = 34), using the empirically derived cutoff scores for EMG readings at FR and ROM from Part 1 and prior scientific literature. Sensitivity, specificity and predictive values of the EMG to identify abnormal motion were assessed. of findings: The ability of experienced testers to reliably measure ROM and EMG at FR was high (r .92, p < .001). All normal subjects achieved FR at a tightly clustered range of mean EMG signals from 1 to 2.3 μV. Most of the variation between motion at FR and MVF occurred through the hip (sacral) motion component of the gross (or total) motion measured at T12. In Part 2, posttreatment reliability for ROM, EMG and the ability to detect the FR point was also high (r 0.82, p < .001). Sensitivity was variable, but specificity was 100% before and after rehabilitation because of the absence of false positives (patients failing to achieve FR who demonstrated normal motion by any of the ROM criteria). Similarly, positive predictive value was also 100% before and after rehabilitation. Over 30% of all 54 patients demonstrated the ability to achieve FR before treatment, with FR usually associated with higher ROM than the non-FR patients. After treatment, 94% of program completers achieved FR, including all those who achieved FR before treatment. FR was associated with major improvement in ROM. Relationship between findings and existing knowledge: FR measures a point at which true lumbar flexion ROM approaches its maximum in normal subjects. This is also the point at which lumbar extensor muscle contraction relaxes, allowing the lumbar spine to hang on its posterior ligaments. The gluteal hamstring muscles then lower the flexed trunk even further, by allowing the pelvis to rotate around the hips. Overall significance of findings: The FR phenomenon, absent in most CLBP patients, was subsequently found in Part 2 to offer a promising method to individualize rehabilitation treatment, decrease unnecessary utilization, identify potential postrehabilitation treatment failures and assess permanent impairment rating validity. In quiet standing positions involving substantial trunk flexion, myoelectric activity in the back muscles diminishes to low levels. Aspects of that "flexion-relaxation" phenomenon were explored through measurements of myoelectric activities in 11 young men during performance of 19 isometric tasks in flexed positions. Biomechanical model analyses were used to predict the internal loads imposed on the lumbar trunk structures during those performances. Flexion-relaxation consistently occurred in quiet flexed standing, but marked increases in myoelectric activity were found on imposition of external loads in flexed positions. Increases in myoelectric activity per unit increase in back muscle contraction force increase were nearly the same as those found in upright postures. Whether or not flexion-relaxation occurs, large trunk flexions load the spine heavily.

Purpose To investigate if there is an effect of sustained trunk axial twisting on the development of low back pain. Methods Sixteen male pain-free university students volunteered for this study. The trunk axial twisting was created by a torsion moment of 50 Nm for 10-min duration. The axial rotational creep was estimated by the transverse camera view directly on the top of the head.
