Background and purpose: Some epilepsy syndromes (sleep‐related epilepsies, SREs) have a strong link with sleep. Comorbid sleep disorders are common in patients with SRE and can exert a negative impact on seizure control and quality of life. Our purpose was to define the standard procedures for the diagnostic pathway of patients with possible SRE (scenario 1) and the general management of patients with SRE and comorbidity with sleep disorders (scenario 2). Methods: The project was conducted under the auspices of the European Academy of Neurology, the European Sleep Research Society and the International League Against Epilepsy Europe. The framework entailed the following phases: conception of the clinical scenarios; literature review; statements regarding the standard procedures. For the literature search a stepwise approach starting from systematic reviews to primary studies was applied. Published studies were identified from the National Library of Medicine's MEDLINE database and Cochrane Library. Results: Scenario 1: Despite a low quality of evidence, recommendations on anamnestic evaluation and tools for capturing the event at home or in the laboratory are provided for specific SREs. Scenario 2: Early diagnosis and treatment of sleep disorders (especially respiratory disorders) in patients with SRE are likely to be beneficial for seizure control. Conclusions: Definitive procedures for evaluating patients with SRE are lacking. Advice is provided that could be of help for standardizing and improving the diagnostic approach of specific SREs. Rhythmic movement disorder (RMD) consists of rhythmic movements (RMs) that occur on falling asleep or during sleep, can involve any part of the body and have a reported frequency ranging from 0.5 to 2 Hz. RMs have been reported to occur in a high proportion of normal children as a self-limiting phenomenon starting and remitting within early infancy. However, there have also been descriptions of forms of RMD occurring against a background of mental retardation or persisting beyond childhood, or having onset in adulthood. So, the occurrence of RMs can be regarded as both a physiological and a pathological phenomenon. The few polysomnographic studies conducted in this field have shown that, in some forms of RMD, RMs are highly linked to arousal fluctuations. However, the mechanisms that underlie the genesis of RMs and are capable of leading to both physiological and pathological patterns of RMs are not fully understood. Here we emphasise the possibility that the central motor pattern generator, recently hypothesised to play a role in the genesis of motor phenomena during sleep in the cases of parasomnia and epileptic seizures, might account for the occurrence of RMs in both physiological and pathological conditions. Introduction: Sleep quality is expected to be poor in epilepsy patients with sleep-related seizures as a consequence of the disruptive effect of seizures themselves on sleep patterns, a potentially heightened pre sleep emotional arousal due to patient’s worries about having seizures during sleep and associated sleep comorbidities. This study is aimed at investigating subjective sleep quality and features in adult patients with seizures occurring exclusively or predominantly during sleep. Patients affected with Nocturnal Frontal Lobe Epilepsy were not included. Materials and methods: Fifty-seven subjects ( 18–72years of age; 27 males), 27 (47.3%) diagnosed with sleep related undetermined epilepsy and 30 (52.7%) with focal epilepsy based on clinical, awake/sleep EEG and brain MRI findings, were investigated. Most of the patients (94%) had <1 seizure per month, with the seizures being in most cases generalized convulsive seizures during sleep. All the patients were given Pittsburgh questionnaire (PQ), Morningness – Eveningness Questionnaire (MEC), STAI 1 and 2, Beck Depression inventory (BDI). Patients reporting snoring and/or witnessed apneas during sleep underwent a nocturnal polysomnographic screening for obstructive sleep apnea (OSA). At the time of the questionnaire compiling the patients had had at least 1 seizure during the previous month (seizures recency) in 19% of the cases and they all were on antiepileptic drugs at bed time, with valproate, levetiracetam and carbamazepine, alone (89.5%) or in combination (10.5%) being the most frequently used drugs. Fifty-seven healthy subjects matched for age and sex served as controls. Univariate analyses were preliminary performed in comparing categorical (chi square test) and continuous variables (T-test) between patients and controls and patients with PSQI >5 and those with PSQI <5. Results: Neither PSQI total score (5.2 3.6 in patients and 5.3 3.4 in controls) nor its components differ significantly between patients and controls. PSQI >5 patients (39%) and PSQI <5 patients (61%) do not differ significantly for age (47.1 15.0 vs. 43.9 15.0years), sex (41% vs. 50% males), STAI 1 ( 46.2 13.2 vs. 37.9 9.6) and STAI2 scores (44.2 10.5 vs. 39 10.7), BDI (12.8 8.0 vs. 9.1 9.0), MEQ chronotype, OSA prevalence (15.8% vs. 7.1%). As for illness-related parameters, PSQI >5 patients proved to have a significantly longer illness duration than PSQI <5 patients (17.5 18.2years vs 8.2 0.3years; p =0.035) and a higher rate of seizures recency (54.5% vs. 15.4%; p =0.04). Conclusion: Our data indicate that subjective sleep quality in adult patients with seizures occurring exclusively or predominantly during sleep is not poorer than in healthy controls. Patients’ sleep quality is influenced by some illness related parameters, with poor sleep quality being associated with longer disease duration and seizure recency. The relatively low frequency of seizures along with a blunting effect of antiepileptic treatment on arousal instability during sleep may explain the finding of a substantially good sleep quality in our patients series. However also epilepsy patients’ life style and their good compliance to sleep hygiene rules may account for a preserved sleep quality. Background: Nocturnal stridor and respiratory abnormalities are important features of multiple system atrophy (MSA) with relevance to patient survival, and they are detected and evaluated mainly through video-polysomnography (video-PSG). Diurnal laryngoscopy seems to yield abnormal findings only in the presence of significant vocal cord (VC) dysfunction.Aim: To assess whether specific electrophysiological patterns of diurnal EMG of VC muscles may indicate nocturnal stridor or respiratory dysfunctions in MSA patients.Materials and Methods: Seventeen patients with probable MSA were examined. A full-night video-PSG to collect standard breathing parameters (apnea/hypopnea index, mean HbSAO2, oxygen desaturation index, total sleep time with HbSaO2 below 90%) was performed in all the patients. Laryngoscopy and EMG investigation of adductor (thyroarytenoid-TA) and abductor (posterior cricoarytenoid-PCA) muscles of the VCs were also performed.Results: Both the laryngeal EMG abnormalities (based on MUAP analysis and kinesiologic EMG investigation of VC muscles) and the laryngoscopic alterations correlated with video-PSG respiratory abnormalities. Specific patterns of EMG findings were consistently found in MSA subjects with nocturnal stridor detected at PSG. In particular, the following EMG findings were related to the severity of breathing abnormalities and the presence of stridor on video-PSG: neurogenic pattern on MUAP analysis of the PCA, paradoxical activation of the TA during inspiration and tonic EMG activity of the TA during quiet breathing.Conclusions: Electromyographic/kinesiologic investigation of VC muscles during wakefulness provides additional information on the pathophysiology of the respiratory abnormalities in MSA patients that could be useful for guiding the choice of the best appropriate treatment and care.
