Background: When performing dynamic gastric emptying scintigraphy with continuous acquisition in children, a single posterior view acquisition is preferred because it allows the young patient to more easily interact with a parent or technologist even though this method tends toward overestimating gastric emptying.Objectives: The objective of our study was to develop a new attenuation correction (AC) method to improve the accuracy of the time activity curve and the measurement of residual gastric emptying from 1-h posterior images of gastric emptying scintigraphy with continuous acquisition.Materials and Methods: We developed a frame-count-based AC for gastric emptying scintigraphy from the posterior view (posterior AC method). We retrospectively reviewed 122 gastric emptying studies performed in children using conjugated posterior and anterior views, and evaluated the statistical differences between posterior only (without AC) and posterior AC using the geometric mean method as a reference standard.Results: The residual values obtained using posterior AC were not significantly different (P=0.813) compared to those using the geometric mean while the values using the posterior only were significantly different (P<0.001) from the geometric mean.Conclusion: The proposed method can replace the geometric mean method to estimate gastric emptying residual fraction using patient-friendly posterior view without a significant difference in 1-h gastric emptying scintigraphy with continuous acquisition. Purpose: The purpose of this study was to evaluate the variability in quantitation of positron emission tomography (PET) data acquired within the context of a multicenter consortium. Methods: PET quantitation phantoms designed by American Association of Physicists in Medicine/Society of Nuclear Medicine Task Group 145 were sent to the ten member sites of the Pediatric Brain Tumor Consortium (PBTC), a NIH-funded research consortium investigating the biology and therapies for brain tumors in children. The phantoms were water-filled cylinders (18.6 cm inside height and 20.4 cm inside diameter) based on the standard ACR phantom with four small, “hot” cylinders of varying diameters (8, 12, 16, 25 mm, all with 38 mm height), consisting of an equilibrium mixture of 68Ge/68Ga in an epoxy matrix. At each site, the operator added the appropriate amount of 18F to the water in the background in order to attain a feature-to-background ratio of roughly 4:1. The phantom was imaged and reconstructed as if it were a brain PET scan for the PBTC. An approximately 12 mm circular region of interest (ROI) was placed over each feature and in a central area in the background. The mean and maximum pixel values for each ROI were requested from local sites in units of activity concentration (Bq/ml) and the standard uptake value (SUV) (g/mL) based on bodyweight. The activity concentration was normalized by the decay-corrected known activity concentration for the features, and reported as the absolute recovery coefficient (RC). In addition, central analyses were performed by two observers. Results: The ten sites successfully imaged the phantom within 5 months and submitted the quantitative results and the phantom image data to the PBTC Operations and Biostatistics Center. The local site-based and central analyses yielded similar mean values for RC. Local site-based SUV measurements of the hot cylindrical features yielded greater variability than central analysis (COV range of 29.9%–42.8% compared to 7.7%–23.2%). Correcting for miscalculations in the local site reported SUVs substantially reduced the variation to levels similar to the central analysis (COV range of 8.8%–18.4%) and also led to the local sites providing a similar mean of the SUV values to those from the central analysis. In the central analysis, the use of mean SUV in place of maximum SUV for an ROI of fixed size substantially reduced the variation in the SUV values (COV ranges of 7.7%–11.3% vs 9.3%–23.2%). Conclusions: Based on this investigation, a SUV variability in the range of 10%–25% due solely to instrument and analysis factors can be expected in the context of a multicenter consortium if a central reading is used and quality assurance and quality control procedures are followed. The rationale of this study was to investigate the feasibility of three-dimensional (3D) methods to analyze 18F-fluoro-deoxy-glucose (FDG) uptake in children with anaplastic astrocytoma (AA) in a multi-institutional trial, to compare 3D and two-dimensional (2D) methods and explore data associations with progression-free survival (PFS). 3D tumor volumes from pretreatment MR images (fluid attenuation inversion recovery and postgadolinium) of children with recurrent AA on a phase I trial of imatinib mesylate were coregistered to FDG positron emission tomography (PET) images. PET data were normalized. Four metrics were defined: the maximum ratio (maximum pixel value within the 3D tumor volume, normalized), the total ratio (cumulative pixel values within the tumor volume, normalized) and tumor mean ratio (total pixel value divided by volume, normalized). 2D analysis methods were compared. Cox proportional hazards models were used to estimate the association between these methods and PFS. Strongest correlations between 2D and 3D methods were with analyses using postcontrast T1 images for volume of interest (VOI). The analyses suggest 3D maximum tumor and mean tumor ratios, whether normalized by gray matter or white matter, were associated with PFS. This study of a series of pretreatment AA patients suggests that 3D PET methods using VOIs based on postcontrast T1 correlate with 2D methods and are related to PFS. These methods yield an estimate of metabolically active tumor burden and may add prognostic information after tumor grade is determined. Abstract: Introduction: Myocardial infarction is the leading cause of death in western countries, and positron emission tomography (PET) plays an increasing role in the diagnosis and treatment planning for this disease. However, the absence of an 18F-labeled PET myocardial perfusion tracer hampers the widespread use of PET in myocardial perfusion imaging (MPI). We recently reported a potential MPI agent based on 18F-labeled rhodamine B. The goal of this study was to more completely define the biological properties of 18F-labeled rhodamine B with respect to uptake and localization in an animal model of myocardial infarction and to evaluate the uptake 18F-labeled rhodamine B by cardiomyocytes. Methods: A total of 12 female Sprague Dawley rats with a permanent ligation of the left anterior descending artery (LAD) were studied with small-animal PET. The animals were injected with 100–150μCi of 18F-labeled rhodamine B diethylene glycol ester ([18F]RhoBDEGF) and imaged two days before ligation. The animals were imaged again two to ten days post-ligation. After the post-surgery scans, the animals were euthanized and the hearts were sectioned into 1mm slices and myocardial infarct size was determined by phosphorimaging and 2,3,5-triphenyltetrazolium chloride staining (TTC). In addition, the uptake of [18F]RhoBDEGF in isolated rat neonatal cardiomyocytes was determined by fluorescence microscopy. Results: Small-animal PET showed intense and uniform uptake of [18F]RhoBDEGF throughout the myocardium in healthy rats. After LAD ligation, well defined perfusion defects were observed in the PET images. The defect size was highly correlated with the infarct size as determined ex vivo by phosphorimaging and TTC staining. In vitro, [18F]RhoBDEGF was rapidly internalized into rat cardiomyocytes with ~40 % of the initial activity internalized within the 60min incubation time. Fluorescence microscopy clearly demonstrated localization of [18F]RhoBDEGF in the mitochondria of rat cardiomyocytes. Conclusion: Fluorine-18-labeled rhodamine B diethylene glycol ester ([18F]RhoBDEGF) provides excellent image quality and clear delineation of myocardial infarcts in a rat infarct model. In vitro studies demonstrate localization of the tracer in the mitochondria of cardiac myocytes. In combination, these results support the continued evaluation of this tracer for the PET assessment of myocardial perfusion. [Copyright &y& Elsevier]

We recently reported the development of the 2-[18F]fluoroethylester of rhodamine B as a potential positron emission tomography (PET)tracer for myocardial perfusion imaging. This compound, which wasprepared using a [18F]fluoroethyl prosthetic group, hassignificant uptake in the myocardium in rats but also demonstratesrelatively high liver uptake and is rapidly hydrolyzed in vivo inmice. We have now prepared 18F-labeled rhodamine B usingthree additional prosthetic groups (propyl, diethylene glycol, andtriethylene glycol) and found that the prosthetic group has a significanteffect on the in vitro and in vivo properties of these compounds.Of the esters prepared to date, the diethylene glycol ester is superiorin terms of in vitro stability and pharmacokinetics. These observationssuggest that the prosthetic group plays a significant role in determiningthe pharmacological properties of 18F-labeled compounds.They also support the value of continued investigation of 18F-labeled rhodamines as PET radiopharmaceuticals for myocardial perfusionimaging. Abstract: Introduction: Fluorine-18-labeled rhodamine B was developed as a potential positron emission tomography (PET) tracer for the evaluation of myocardial perfusion, but preliminary studies in mice showed no accumulation in the heart suggesting that it was rapidly hydrolyzed in vivo in mice. A study was therefore undertaken to further evaluate this hypothesis. Methods: [18F]Fluoroethylrhodamine B was equilibrated for 2 h at 37°C in human, rat and mouse serum and in phosphate-buffered saline. Samples were removed periodically and assayed by high-performance liquid chromatography. Based on the results of the stability study, microPET imaging and a biodistribution study were carried out in rats. Results: In vitro stability studies demonstrated that [18F]fluoroethylrhodamine B much more stable in rat and human sera than in mouse serum. After 2 h, the compound was >80% intact in rat serum but <30% intact in mouse serum. The microPET imaging and biodistribution studies in rats confirmed this result showing high and persistent tracer accumulation in the myocardium compared with the absence of uptake by the myocardium in mice thereby validating our original hypothesis that 18F-labeled rhodamines should accumulate in the heart. Conclusions: [18F]Fluoroethylrhodamine B is more stable in rat and human sera than it is in mouse serum. This improved stability is demonstrated by the high uptake of the tracer in the rat heart in comparison to the absence of visible uptake in the mouse heart. These observations suggest that 18F-labeled rhodamines are promising candidates for more extensive evaluation as PET tracers for the evaluation of myocardial perfusion. [Copyright &y& Elsevier]

Abstract: There is considerable interest in developing an 18F-labeled PET myocardial perfusion agent. Rhodamine dyes share several properties with 99mTc-MIBI, the most commonly used single-photon myocardial perfusion agent, suggesting that an 18F-labeled rhodamine dye might prove useful for this application. In addition to being lipophilic cations, like 99mTc-MIBI, rhodamine dyes are known to accumulate in the myocardium and are substrates for Pgp, the protein implicated in MDR1 multidrug resistance. As the first step in determining whether 18F-labeled rhodamines might be useful as myocardial perfusion agents for PET, our objective was to develop synthetic methods for preparing the 18F-labeled compounds so that they could be evaluated in vivo. Rhodamine B was chosen as the prototype compound for development of the synthesis because the ethyl substituents on the amine moieties of rhodamine B protect them from side reactions, thus eliminating the need to include (and subsequently remove) protecting groups. The 2′-[18F]fluoroethyl ester of rhodamine B was synthesized by heating rhodamine B lactone with [18F]fluoroethyltosylate in acetonitrile at 165°C for 30min using [18F]fluoroethyl tosylate, which was prepared by the reaction of ethyleneglycol ditosylate with Kryptofix 2.2.2, K2CO3, and [18F]NaF in acetonitrile for 10min at 90°C. The product was purified by semi-preparative HPLC to produce the 2′-[18F]fluoroethylester in >97% radiochemical purity with a specific activity of 1.3GBq/μmol, an isolated decay corrected yield of 35%, and a total synthesis time of 90min.
