Background: The electroencephalogram (EEG) in the new-born period is highly superior to the clinical exam in the detection and prognosis of brain dysfunctions, since it allows continuous functional documentation of the brain at the patient's bedside in a non-invasive way. However, there is still some disagreement about these findings. Objective: To describe the electroencephalographic findings in newborns with a history of prematurity. Material and methods: Cross-sectional, descriptive, retrospective study. The inclusion criteria were: newborns with a history of prematurity, regardless of gender, who underwent an EEG from June 2017 to June 2021. Patients with incomplete electroencephalographic records or clinical records without complete data were excluded; patients using sedatives (thiopental, fentanyl, midazolam, diazepam) were eliminated from the study. Results: 107 patients (37 women and 70 men) with a history of prematurity were included, with a mean gestational age at birth of 30.9 WOG ± 3.25. Electroencephalographic findings were normal in 40%, abnormal in 32%, and immature in 28%. The most frequent abnormal finding was focal paroxysmal activity in 86%. 93.4% of the participants presented comorbidities, the most frequent being neurological. Conclusion: Preterm neonates are at high risk of neurologic sequelae, and EEG is a sensitive method for assessing neuromotor and cognitive prognosis. In our study population, one-third had abnormal findings. Early postnatal screening is helpful, but additional records are usually needed to detect high-risk newborns. Introduction: Breastfeeding (BF) is an essential part of the comprehensive care nurses provide in the Neonatal Care Unit (NCU). Due to the workload and delegation of different care activities, the dynamics in these units focus on innovation using technology and postpone humanization of service, undermining BF support. Objective: To design a BF support program at NCU. Materials and methods: Qualitative participatory action research. Seven semi-structured interviews were conducted with health professionals with expertise in BF. Some of them worked in NCU of the health facility where the study was conducted; others were external. Their experience and knowledge were used to identify the key elements for designing a BF support program at the UCN. Results: They are presented in the following categories: a) breastfeeding culture, including institutional policy on breastfeeding and standardization of the process; b) quality, including registration and control of activities; and c) monitoring of indicators and continuous auditing. Discussion: A BF program has been designed and it will be implemented by the health facility to prevent the interruption of exclusive BF through support, training, standardization, and promotion. Conclusion: Nurses are interested in standardizing a program that allows them to provide patients with truthful and concise information, and they view education as a fundamental pillar in the development of a successful BF program. Background: Promoting physical contact, especially skin-to-skin contact, immediately or as soon as possible in preterm newborns is still a challenge. Objective: To determine the prevalence of physical contact between mothers and preterm newborns at birth and to analyze the association between the type of physical contact between them in the delivery room and the prevalence of exclusive breastfeeding at hospital discharge. Methodology: Cross-sectional study with a nonprobability sample of 78 mothers and 79 preterm newborns. Data were collected from July to November 2017, through interviews and medical records, and analyzed through descriptive statistics and Poisson Regression with robust variance. Results: The prevalence of any type of physical contact in the delivery room was 51.9%, without statistical difference between the type of physical contact between mothers and infants and exclusive breastfeeding at discharge. Conclusion: Physical contact by touching and kissing after birth did not influence the prevalence of exclusive breastfeeding at hospital discharge. [ABSTRACT FROM AUTHOR]

Objective: to understand how mothers experienced the care of their preterm infants at home in the midst of the COVID-19 pandemic. Method: qualitative, descriptive study, developed between January and December 2021, through remote interviews with 25 mothers of preterm infants, one month after hospital discharge, with thematic Content Analysis. Results: maternal reoccupations related to child care in the pandemic were revealed, as well as influences of the pandemic period on family routine; monitoring of children in discontinued health services; overload and fear. These aspects may expose the baby to a greater risk of developmental alterations, as well as affect maternal self-efficacy, since the fear of complications resulting from preterm birth was associated with the anguish of possible contamination by the coronavirus. Conclusion: the prematurity experienced in the midst of the COVID-19 pandemic had a negative impact both on the daily life of the family and on the health monitoring of preterm infants. Introduction: physiotherapy intervention consisting of oral, perioral, and non-nutritive suction stimulation is a tool applied in preterm new-borns to accelerate the achievement of full oral feeding (tube removal). Objectives: to describe the postmenstrual age of acquisition of full oral feeding through orofacial intervention in preterm neonates and to associate it to their degree of prematurity and early treatment initiation. Material and methods: prospective, longitudinal, descriptive and correlational study carried out in the Neonatal Unit of Hospital Universitario Miguel Servet in Zaragoza, in premature infants with tube. The inclusion criterion was being premature and using nasogastric tube feeding. The exclusion criteria were the presence of serious associated pathology and the existence of different syndromes. Results: the sample included 145 preterm infants, of which 20 % were extremely premature (n = 29), 51.70 % very premature (n = 75) and 28.30 % moderately premature (n = 41). The mean number of weeks of postmenstrual age at which they achieved full oral feeding was 35.90 ± 2.03 (n =145). A very strong positive correlation (rho = 0.774) was observed between the age of initiation of treatment and the age at which full oral feeding was achieved, especially in the moderately preterm group (rho = 0.841). Conclusion: the application of the suction and swallow protocol allows preterm infants to achieve full oral feeding before term birth. This intervention, if carried out early shortens the times for its achievement, especially in the moderately premature group. [ABSTRACT FROM AUTHOR]

Introduction: Premature newborns have an immature cardiorespiratory system that hinders adaptation to the extrauterine environment. This leads to multiple complications which require interventions to improve ventilation and gas exchange, like oxygenating using therapeutic support devices such as continuous positive airway pressure or CPAP. Objective: To determine the effect of therapeutic touch on preterm newborns' adaptation to CPAP. Materials and methods: This is a quasi-experimental study, with a sample of 13 PTNB who underwent therapeutic touch for 15 minutes in 2 daily sessions (6 am and 8 pm), with pre- and postassessment of NOC "Preterm Infant Organization." Descriptive analysis was performed. Results: The participants were allocated to the control group and the intervention group in a similar way. Pre- and post-intervention changes were observed, but especially in the hand posture indicator, with a difference between both groups of 0.74 with a p-value of 0.006. Conclusions: The practice of therapeutic touch with RNPT receiving nasal CPAP allows for improving the comfort of the RNPT through physiological and neurobehavioral indicators. [ABSTRACT FROM AUTHOR]

Introduction. This article presents a reflection on the need for a holistic view of the problem of premature births, so that healthcare teams can understand the multiplicity of factors present in premature births, and its consequences for the minor, the mother, the family members, and the healthcare staff involved. Topic of reflection. The topic is based on two investigations carried out with mothers, fathers, and children's beneficiaries of the Mother Kangaroo Program in a State hospital, at two moments, 2012 and 2014, with some of the authors of this article. Conclusions. In addition to the review of the scientific literature related to the topic and the problem, it is concluded that, under a holistic view, the problem is understood in a comprehensive way and better communication with the parents is encouraged. With the healthcare team, cross-disciplinary handling is achieved, overcoming the objects of isolated studies. Objective: determine the content validity of the Hospital-Home mobile application for home monitoring by parents of the motor development of premature children. The active participation of parents is fundamental for the acquisition of motor skills demonstrating better results. Methodology: quantitative study, a validation process is carried out for the content of an application with the expert judgment technique. Results: a general kappa index of 0.739 was obtained for the content of the application, which indicates a substantial concordance between evaluators. Conclusions: home follow-up processes require constant feedback for parents, so the Baby motor skills can be a useful tool based on scientific evidence for parents by providing reliable information and practices about their children's motor development. Kangaroo-Mother Care (KMC) is a follow-up program for premature infants with comorbidities such as bronchopulmonary dysplasia (BPD). The objective was to characterize patients with BPD and describe their nutritional, neurological, and respiratory outcomes in a KMC. A retrospective cohort study of the KMC in a University Hospital between 2015-2018. Included patients with BPD and excluded major malformations. Nutritional, neurological, and respiratory outcomes were evaluated at 40 weeks, 6, and 12 months of corrected-age using scales recommended in Colombia. A descriptive analysis was carried out, the variables were compared according to the severity of BPD. The changes in the outcomes in patients with complete follow-up were evaluated. 490 patients were admitted in four years, 50 with BPD (10,2%); 58% with moderate-BPD, none with severe BPD. 46% were born at <29 weeks of gestationalage, 96% had low socioeconomic status and 90% and 90% of these submitted to mechanical ventilation. In the follow-up, height-for-age remained at risk at 6 and 12 months. 98% had abnormal Amiel-Tisson test. Around 75% had respiratory symptoms between 6 and 12 months and 33% had at least one hospital readmission. Patients with BPD in KMC have alterations in height-for-age, neurodevelopment, and respiratory system when complete the follow-up. Strategies for integrating KMC into the health system should be strengthened. [ABSTRACT FROM AUTHOR]

Introduction: Birth, hospitalization, transition from hospital to home and care for preterm infants are critical events for parents. Materials and Methods: A descriptive qualitative study was conducted with four fathers and 12 mothers who had experienced preterm infant care. Semi-structured interviews were conducted between March and October 2020 and grounded theory tools were used for analysis. Results: The following four categories were identified: coping with preterm birth, experiencing day-to-day prematurity in neonatal units, empowering parents to care for preterm infants and caring at home. The category for empowering parents to care for preterm infants was identified as the central category in the experience. Discussion: Experiencing preterm birth is complex and dynamic as it involves a high emotional burden for parents. Caring for preterm infants requires developing specific skills before discharge in which empowerment plays a central role. Parental empowerment facilitates this transition supported by nursing accompaniment, information provided, involvement in the care of their child in the neonatal unit, family support and individual conditions of parents. Conclusions: Experiencing parental empowerment in the care of their preterm infants is a key element that facilitates the transition from hospital to home and encourages the development of home care skills. Introduction: To understand premature birth through perceptions of families with premature babies admitted to neonatal intensive care unit from a cross-cultural perspective. Materials and methods: An exploratory-descriptive qualitative study was conducted through participant observation and semi-structured interviews with 16 relatives of premature babies admitted to the neonatal intensive care unit of a maternity hospital in the state of Bahia. R Software was used for multidimensional analysis of texts and questionnaires and content analysis. Results: Four categories were established: premature babies with their particularities in family's perceptions; family's feelings caused by premature birth; spirituality as a strategy to give meaning to premature birth; and premature birth from professional healthcare. Discussion: Understanding the experiences of families in the neonatal environment can contribute to healthcare professionals and institutions reorienting their practices to target integrality of care by identifying the particularities of each family and recognizing their cultural diversity. Conclusions: Understanding the needs and culture of families contributes to the provision of more humane and effective care in which the family plays a part in planning and decision making on integral care of their premature baby, not only highlighting biological factors but also social and psychological factors that comprehensively involve the premature baby and their family. Objective: To evaluate the beliefs that guide the care of the premature newborn at home from the perspective of the caregiving mother. Methods: This is a descriptive study with a qualitative approach. 30 semi-structured interviewees were carried out with mothers who had premature children. The Grounded Theory was used to analyze the speeches seized in the interviews. Results: It was evidenced that the mothers of premature newborns had low income, little access to the health system and lived in areas considered geographically and economically less favored. It was evident that mothers were afraid and insecure when taking care of their children and did not receive support from the Family Health Strategy. Conclusion: The mothers of the study reported great interference in the adoption of parenting practices at home, especially the grandmother and they do not feel prepared to take care of the premature child at home, because they are afraid to perform the care. Objective: To analyze obstetric variables associated with the birth of moderate and late premature babies in a city in the southern region of Brazil. Method: Cross-sectional study that analyzed the obstetric variables of 151 moderate and late preterm infants from May 2016 to May 2017 resident in Santa Maria/RS. Data were collected by means of their own instrument daily, and subsequently analyzed descriptively and by comparison of frequency and correlation. Results: Twinning (14.6%) was the most frequent feature among late twins when compared to the moderate group (p = 0.019). The other factors showed no significant difference. The higher number of previous premature births has a significant correlation with the lower gestational age at birth (CI = - 0.522, p = 0.008). Conclusion: For the two groups of premature infants studied, the reasons did not present significant difference, which suggests homogeneity regarding the obstetric reasons for early birth. Objective: to analyze in the literature the scientific evidence available on the position of the premature newborn (PTNB) in a neonatal intensive care unit. Method: integrative literature review carried out in the MEDLINE / PubMed, CINAHL, LILACS AND BDENF databases in February 2020 without time delimitation. Results: 27 articles were included, with a predominance of experimental studies. It was found that positioning can improve the physiological parameters of PTNB such as breathing, heart rate, brain flow, nutrition. In addition to benefits in behavioral responses with emphasis on decreasing pain, comfort, posture and sleep. Conclusion: proper positioning can contribute to improving the physiological and behavioral responses of PTNB and should be a practice incorporated by the health team. [ABSTRACT FROM AUTHOR]

- Abstract
Background: Nutritive suction is an important milestone for correct newborn feeding. Premature infants do not develop this ability therefore, several proposals for their appearance have being designed. Objective: Assess whether the stimulation of the innate oral reflexes using neuromotor reactions reduces the time to acquire sucking behavior. Material and methods: Newborns before 34 weeks of gestation, fed by tube feeding due to an ineffective suction, were studied. Control group consisted of newborns with the same characteristics born during 2016. Experimental group was stimulated by neuromotor reactions until the tube feeding was retired. Observed variables included transition time to complete oral feeding, weight, height and cranial perimeter at discharge, peripheral oxygen saturation and heart rate before and after intervention, as well as quality of suction behavior using ECLES scale. Results: A total of 23 babies from intervention group completed the study, and 25 from historical control group. Children of intervention group showed an accelerated transition to independent oral feeding and higher values of height (p = 0.01), weight (p < 0.001) and cranial perimeter (p = 0.04), at the time of hospital discharge. Gestational age at discharge and total duration of hospitalization did not show differences between groups (p > 0.05). Conclusion: Oral stimuli intervention by means of neuromotor reactions reduced the time necessary to reach suction behavior in preterm babies. [ABSTRACT FROM AUTHOR]

- Abstract
The objective was to describe maternal experiences related to breastfeeding of premature infants in a neonatal unit of a baby-friendly hospital. This is a qualitative exploratory-descriptive study conducted in 2015. Six mothers of premature infants hospitalized in a baby-friendly hospital participated in the research. Information was collected through semi-structured interviews. Thematic content analysis was used. Three thematic categories emerged: Difficulties faced during the breastfeeding process; Human milk bank milking routine and Feelings related to breastfeeding of a premature child. It is concluded that, in this scenario, mothers experienced difficulties, such as the baby's clinical conditions and feelings of stress and anxiety. However, they were pleased to be able to breastfeed their premature children and assist in recovery. It was also possible to identify the importance of health team performance in this process. 46. FACTORES FETALES ASOCIADOS A PREMATURIDAD.

- Abstract
Objective: to identify fetal factors associated with prematurity in the Vicente Corral Hospital of Cuenca-Ecuador in the 2016-2017 period. Materials and methods: a case-control study was designed, with double contrasts, using two controls of equal postnatal age and sex per each case. From a homogeneous population of infants less than one week of age born at the Vicente Corral Hospital in Cuenca-Ecuador in the period 2016-2017, a sample of 160 cases and 320 controls was obtained. A descriptive and inferential statistical analysis was performed, to establish the statistical association, OR, 95% CI and statistical significance with p-value <0,05 were used. Results: in the case group, they were 75,0% late premature and in the control group, they were 62,5% born full term. The prematurity was associated with: twinning OR=7,36; 95% CI=2,61- 20,74; p=0,000. History of premature bother OR=2,96; 95% CI=1,61-5,44; p=0,000. Birth weight lower than the 10th percentile OR=1,66; 95% CI=1,01-2,75; p=0,048. The factors that were not associated with prematurity were: sex χ²=0,100; p-value=0,752. Major congenital malformations OR=2,02; 95% CI=0,40-10,12; p=0,405. Conclusions: In the present study, late preterm predominated in the case group and full term born in the control group. Fetal factors truly associated with prematurity were: twinness, the history of preterm born sibling and birth weight less than the 10th percentile; neonate sex and major congenital malformations were not associated with prematurity. Objective: to produce and validate a booklet, based on Jean Watson’s Theory, on home care for premature newborns, based on caregivers’ experiences. Method: a methodological study, developed in the following stages: diagnosis of knowledge needs about home care; survey of scientific content; educational material production; and validity by judges/experts. Results: the literature review resulted in 19 articles. The main themes (breastfeeding, bath care, bond building, infection prevention and support network) were listed for producing the booklet “Booklet for Premature Newborns: Demystifying Home Care”. The booklet content and appearance received the overall Content Validity Index of 0.85, considered suitable within the scientific rigor of validity. Final considerations: the booklet produced and validated is an educational material whose main role is to provide knowledge and awaken caregivers’ autonomy in providing home care to newborns. RESUMEN Objetivo: producir y validar una cartilla, basada en la Teoría de Jean Watson, sobre el cuidado domiciliario del recién nacido prematuro, a partir de las experiencias de los cuidadores. Método: estudio metodológico, desarrollado en las siguientes etapas: diagnóstico de las necesidades de conocimiento sobre el cuidado domiciliario; encuesta de contenido científico; producción de material educativo; y validación por jueces/expertos. Resultados: la revisión de la literatura resultó en 19 artículos. Se enumeraron los temas principales (lactancia materna, cuidado del baño, creación de vínculos, prevención de infecciones y red de apoyo) para la producción del folleto “Cuaderno para Recién Nacidos Prematuros: Desmitificando el Cuidado Domiciliario”. El contenido y apariencia del cuadernillo recibió el Índice de Validez de Contenido Global de 0,85, considerado adecuado dentro del rigor científico de la validez. Consideraciones finales: la cartilla producida y validada es un material educativo que tiene como función principal proporcionar conocimientos y despertar la autonomía de los cuidadores en la atención domiciliaria al recién nacido. RESUMO Objetivo: produzir e validar cartilha, baseado na Teoria de Jean Watson, sobre os cuidados domiciliares ao recém-nascido prematuro, a partir de vivências de cuidadores. Método: estudo metodológico, desenvolvido nas etapas: diagnóstico das necessidades de conhecimento sobre os cuidados domiciliares; levantamento dos conteúdos científicos; produção de material educativo; e validação por juízas/experts. Resultados: a revisão da literatura resultou em 19 artigos. Elencaram-se os principais temas (amamentação, cuidados com o banho, construção de vínculo, prevenção de infecções e rede de apoio), para produção da cartilha intitulada “Cartilha de Cuidados com o Recém-Nascido Prematuro: Desmistificando o Cuidar no Domicílio”. O conteúdo e aparência da cartilha recebeu o Índice de Validade de Conteúdo Global de 0,85, considerado adequado dentro do rigor científico de validade. Considerações finais: a cartilha produzida e validada é um material educativo cujo papel principal é proporcionar conhecimento e despertar a autonomia dos cuidadores na prestação dos cuidados domiciliares ao recém-nascido.
