Background: As patient-initiated messaging rises, identifying variation in message volume and its relationship to clinician workload is essential. Objective: To describe the association between variation in message volume over time and time spent on the electronic health record (EHR) outside of scheduled hours. Design: Retrospective cohort study. Participants: Primary care clinicians at Cleveland Clinic Health System. Main Measures: We categorized clinicians according to their number of quarterly incoming medical advice messages (i.e., message volume) between January 2019 and December 2021 using group-based trajectory modeling. We assessed change in quarterly messages and outpatient visits between October–December 2019 (Q4) and October–December 2021 (Q12). The primary outcome was time outside of scheduled hours spent on the EHR. We used mixed effects logistic regression to describe the association between incoming portal messages and time spent on the EHR by clinician messaging group and at the clinician level. Key Results: Among the 150 clinicians, 31% were in the low-volume group (206 messages per quarter per clinician), 47% were in the moderate-volume group (505 messages), and 22% were in the high-volume group (840 messages). Mean quarterly messages increased from 340 to 695 (p < 0.001) between Q4 and Q12; mean quarterly outpatient visits fell from 711 to 575 (p = 0.005). While time spent on the EHR outside of scheduled hours increased modestly for all clinicians, this did not significantly differ by message group. Across all clinicians, each additional 10 messages was associated with an average of 12 min per quarter of additional time spent on the EHR (p < 0.001). Conclusions: Message volume increased substantially over the study period and varied by group. While messages were associated with additional time spent on the EHR outside of scheduled hours, there was no significant difference in time spent on the EHR between the high and low message volume groups. Objective: Demand for systematic linkage of patients to behavioral health care has increased because of the widespread implementation of depression screening. This study assessed the impact of deploying behavioral health social workers (BHSWs) in primary care on behavioral health visits for depression or anxiety.Methods: This quasi-experimental, stepped-wedge study included adults with a primary care visit between 2016 and 2019 at Cleveland Clinic, a large integrated health system. BHSWs were deployed in 40 practices between 2017 and 2019. Patients were allocated to a control group (diagnosed before BHSW deployment) and an intervention group (diagnosed after deployment). Data were collected on behavioral health visits (i.e., to therapists and psychiatrists) within 30 days of the diagnosis. Multilevel logistic regression models identified associations between BHSW deployment period and behavioral health visit, adjusted for demographic variables and clustering within each group.Results: Of 68,659 persons with a diagnosis, 21% had a depression diagnosis, 49% an anxiety diagnosis, and 31% both diagnoses. In the period after BHSW deployment, the proportion of patients with depression who had a behavioral health visit increased by 10 percentage points, of patients with anxiety by 9 percentage points, and of patients with both disorders by 11 percentage points. The adjusted odds of having a behavioral health visit was higher in the postdeployment period for patients with depression (adjusted odds ratio [AOR]=4.35, 95% confidence interval [CI]=3.50-5.41), anxiety (AOR=4.27, 95% CI=3.57-5.11), and both (AOR= 3.26, 95% CI=2.77-3.84).Conclusions: Integration of BHSWs in primary care was associated with increased behavioral health visits. Background: Identifying a window of opportunity when patients are motivated to lose weight might improve the effectiveness of weight loss counseling. The onset of chronic disease could create such a window.Objective: To determine whether identifying prediabetes was associated with subsequent weight loss.Methods: Our retrospective cohort study included adults with obesity and a primary care visit between 2015 and 2017. Data were collected and analysed in 2019/2020. We compared patients who developed prediabetes [haemoglobin A1c (HbA1c) ≥5.7 and <6.5] to patients with a normal HbA1c (<5.7). We ran linear regression models to identify the association between identifying prediabetes and percent body mass index (BMI) change at 6 and 12 months. The adjusted model controlled for demographic characteristics at baseline, Charlson comorbidity score, and metformin, antipsychotic, antidepressant and antiobesity medication prescribed in either the first 3 months (for the 6-month outcome) or first 9 months (for 12-month outcome) and clustering within physician.Results: Of 11 290 participants, 43% developed prediabetes. At 6 months, 15% of the prediabetes group lost ≥5% of their BMI compared with 13% of the comparison group. The results were similar at 12 months with 18% of the prediabetes group losing ≥5% of their BMI compared with 17%. The prediabetes group lost a higher percentage of their BMI (β = -0.7% versus -0.3% at 6 months and β = -0.5% versus 0.01% at 12 months).Conclusions: While the percent of BMI change was small, patients with newly identified prediabetes lost more weight than a comparison group. Background: Alcohol use disorder (AUD) imposes a high mortality and economic burden. Effective treatment is available, though underutilized. Objective: Describe trends in AUD pharmacotherapy, variation in prescribing, and associated patient factors. Design: Retrospective cohort using electronic health records from 2010 to 2019. Participants: Primary care patients from 39 clinics in Ohio and Florida with diagnostic codes for alcohol dependence or abuse plus social history indicating alcohol use. PCPs in family or internal medicine with at least 20 AUD patients. Main Measures: Pharmacotherapy for AUD (naltrexone, acamprosate, and disulfiram), abstinence from alcohol, patient demographics, and comorbidities. Generalized linear mixed models were used to identify patient factors associated with prescriptions and the association of pharmacotherapy with abstinence. Key Results: We identified 13,250 patients; average age was 54 years, 66.9% were male, 75.0% were White, and median household income was $51,776 per year. Over 10 years, the prescription rate rose from 4.4 to 5.6%. Patients who were Black (aOR 0.74; 95% CI 0.58, 0.94) and insured by Medicare versus commercial insurance (aOR 0.61; 95% CI 0.48, 0.78) were less likely to be treated. Higher median household income ($10,000 increment, aOR 1.06; 95% CI 1.03, 1.10) and Medicaid versus commercial insurance (aOR 1.52; 95% CI 1.24, 1.87) were associated with treatment. Receiving pharmacotherapy was associated with subsequent documented abstinence from alcohol (aOR 1.60; 95% CI 1.33, 1.92). We identified 236 PCPs. The average prescription rate was 3.6% (range 0 to 24%). The top decile prescribed to 14.6% of their patients. The bottom 4 deciles had no prescriptions. Family physicians had higher rates of pharmacotherapy than internists (OR 1.50; 95% CI 1.21, 1.85). Conclusions: Medications for AUD are infrequently prescribed, but there is considerable variation among PCPs. Background: Over one third of American adults are at high risk for developing diabetes, which can be delayed or prevented using interventions such as medical nutrition therapy (MNT) or metformin. Physicians' self-reported rates of prediabetes treatment are improving, but patterns of actual referral, prescription, and MNT visits are unknown. Objective: To characterize treatment of prediabetes in primary care. Design: We conducted a retrospective cohort study using electronic health record data. We described patterns of treatment and used multivariable logistic regression to evaluate the association of patient factors and PCP-specific treatment rate with patient treatment. Patients: We included overweight or obese outpatients who had a first prediabetes-range hemoglobin A1c (HbA1c) during 2011–2018 and had primary care provider (PCP) follow-up within a year. Main Measures: We collected patient characteristics and the following treatments: metformin prescription; referral to MNT, diabetes education, endocrinology, or bariatric medicine; and MNT visit. We did not capture within-visit physician counseling. Key Results: Of 16,713 outpatients with prediabetes, 20.4% received treatment, including metformin prescriptions (7.8%) and MNT referrals (11.3%), but only 7.4% of referred patients completed a MNT visit. The strongest predictor of treatment was the patient's PCP's treatment rate. Some PCPs never treated prediabetes, but two treated more than half of their patients; 62% had no patients complete a MNT visit. Being younger or female and having higher body mass index or HbA1c were also positively associated with treatment. Compared to white patients, black patients were more likely to receive MNT referral and less likely to receive metformin. Conclusions: Almost 80% of patients with new prediabetes never received treatment, and those who did receive referrals had very poor visit completion. To our knowledge, our study is the first to examine differences in APC and physician practice content. Background: Successful implementation of new care models within a health system is likely dependent on contextual factors at the individual sites of care.Objective: To identify practice setting components contributing to uptake of new team-based care models.Design: Convergent mixed-methods design.Participants: Employees and patients of primary care practices implementing two team-based models in a large, integrated health system.Main Measures: Field observations of 9 practices and 75 interviews, provider and staff surveys to assess adaptive reserve and burnout, analysis of quality metrics, and patient panel comorbidity scores. The data were collected simultaneously, then merged, thematically analyzed, and interpreted by a multidisciplinary team.Key Results: Based on analysis of observations and interviews, the 9 practices were categorized into 3 groups-high, partial, and low uptake of new team-based models. Uptake was related to (1) practices' responsiveness to change and (2) flexible workflow as related to team roles. Strength of local leadership and stable staffing mediated practices' ability to achieve high performance in these two domains. Higher performance on several quality metrics was associated with high uptake practices compared to the lower uptake groups. Mean Adaptive Reserve Measure and Maslach Burnout Inventory scores did not differ significantly between higher and lower uptake practices.Conclusion: Uptake of new team-based care delivery models is related to practices' ability to respond to change and to adapt team roles in workflow, influenced by both local leadership and stable staffing. Better performance on quality metrics may identify high uptake practices. This qualitative study examines to what extent and why physicans still prescribe self-monitoring of blood glucose (SMBG) in patients with non-insulin-treated type 2 diabetes (NITT2D) when the evidence shows it increases cost without improving hemoglobin A1c (HbA1c), general well being, or health-related quality of life. Semistructured phone interviews with 17 primary care physicians indicated that the majority continue to recommend routine self-monitoring of blood glucose due to a compelling belief in its ability to promote the lifestyle changes needed for glycemic control. Targeting physician beliefs about the effectiveness of self-monitoring of blood glucose, and designing robust interventions accordingly, may help reduce this practice. [ABSTRACT FROM AUTHOR]

Objectives To assess prostate cancer screening practices in primary care since the initial United States Preventive Services Task Force ( USPSTF) recommendation against prostate-specific antigen ( PSA) testing for older men, and to assess primary provider variation associated with prostate cancer screening. Patients and Methods Our study population included 160 211 men aged ≥40 years with at least one visit to a primary care clinic in any of the study years in a large, integrated health system. We conducted a retrospective cohort study using electronic medical record data from January 2007 to December 2014. Yearly rates of screening PSA testing by primary care providers ( PCPs), rates of re-screening, and rates of prostate biopsies were assessed. Results Annual PSA-screening testing declined from 2007 to 2014 in all age groups, as did biennial and quadrennial screening. Yearly rates declined for men aged ≥70 years, from 22.8% to 8.9%; ages 50-69 years, from 39.2% to 20%; and ages 40-49 years, from 11% to 4.6%. Overall rates were lower for African-American (A-A) men vs non-A-A men; for men with a family history of prostate cancer, rates were similar or slightly higher than for those without a family history. PCP variation associated with ordering of PSA testing did not substantially change after the USPSTF recommendations. While the number of men screened and rates of follow-up prostate cancer screening declined in 2011-2014 compared to 2007-2010, similar re-screening rates were noted for men aged 45-75 years with initial PSA levels of <1 ng/mL or 1-3 ng/mL in both the earlier and later cohorts. For men aged >75 years with initial PSA levels of <3 ng/ mL screened in both cohorts, follow-up screening rates were similar. Rates of prostate biopsy declined for men aged ≥70 years in 2014 compared to 2007. For men who had PSA screening, rates of first prostate biopsy increased in later years for A-A men and men with a family history of prostate cancer. Conclusions Prostate cancer screening declined from 2007 to 2014 even in higher-risk groups and follow-up screening rates were not related to previous PSA level. However, rates of first prostate biopsy in men who were screened with a PSA test were higher for men with an increased risk of prostate cancer in later years. Variation in PSA testing was noted among PCPs. Background: The hospitalist model is rapidly altering the landscape for inpatient care in the United States, yet evidence about the clinical and economic outcomes of care by hospitalists is derived from a small number of single-hospital studies examining the practices of a few physicians. Methods: We conducted a retrospective cohort study of 76,926 patients 18 years of age or older who were hospitalized between September 2002 and June 2005 for pneumonia, heart failure, chest pain, ischemic stroke, urinary tract infection, acute exacerbation of chronic obstructive pulmonary disease, or acute myocardial infarction at 45 hospitals throughout the United States. We used multivariable models to compare the outcomes of care by 284 hospitalists, 993 general internists, and 971 family physicians. Results: As compared with patients cared for by general internists, patients cared for by hospitalists had a modestly shorter hospital stay (adjusted difference, 0.4 day; P<0.001) and lower costs (adjusted difference, $268; P=0.02) but a similar inpatient rate of death (odds ratio, 0.95; 95% confidence interval [CI], 0.85 to 1.05) and 14-day readmission rate (odds ratio, 0.98; 95% CI, 0.91 to 1.05). As compared with patients cared for by family physicians, patients cared for by hospitalists had a shorter length of stay (adjusted difference, 0.4 day; P<0.001), and the costs (adjusted difference, $125; P=0.33), rate of death (odds ratio, 0.95; 95% CI, 0.83 to 1.07), and 14-day readmission rate (odds ratio, 0.95; 95% CI, 0.87 to 1.04) were similar. Conclusions: For common inpatient diagnoses, the hospitalist model is associated with a small reduction in the length of stay without an adverse effect on rates of death or readmission. Although vaginitis is a common outpatient problem, only 60% of patients can be diagnosed at the initial office visit of a primary care provider using the office procedures of pH testing, whiff tests, normal saline, and potassium hydroxide preps. To determine the most cost-effective diagnostic and treatment approach for the medical management of vaginitis. Decision and cost-effectiveness analyses. Healthy women with symptoms of vaginitis undiagnosed after an initial pelvic exam, wet mount preparations, pH, and the four criteria to diagnose bacterial vaginosis. General office practice. We evaluated 28 diagnostic strategies comprised of combinations of pH testing, vaginal cultures for yeast and Trichomonas vaginalis, Gram's stain for bacterial vaginosis, and DNA probes for Neisseria gonorrhoeae and Chlamydia. Data sources for the study were confined to English language literature. The outcome measures were symptom-days and costs. The least expensive strategy was to perform yeast culture, gonorrhoeae and Chlamydia probes at the initial visit, and Gram's stain and Trichomonas culture only when the vaginal pH exceeded 4.9 ($330, 7.30 symptom days). Other strategies cost $8 to $76 more and increased duration of symptoms by up to 1.3 days. In probabilistic sensitivity analysis, this strategy was always the most effective strategy and was also least expensive 58% of the time. For patients with vaginitis symptoms undiagnosed by pelvic examination, wet mount preparations and related office tests, a comprehensive, pH-guided testing strategy at the initial office visit is less expensive and more effective than ordering tests sequentially. Background Interprofessional team-based models of primary care delivery may improve efficiency and increase patient-centered time with the physician. The purpose of our study is to evaluate the effect of a new team-based primary care delivery model implemented in our health system on physician-patient face-to-face time and patient experience of the visit compared with usual care. Methods We conducted time-motion analyses of ambulatory visits with 20 primary care physicians across 8 practice sites in our health system from June through August 2014. Ten physicians practicing in the team-based model and 10 physicians practicing usual care at the same practice sites were included. The time that the physician was in the room and the duration of physician-patient face-to-face interaction were measured. After each visit, patients answered questions regarding satisfaction with their visit. Results A total of 98 patient visits were observed (44 in the team-based model and 54 in usual care). Total time that the physician and patient spent together at the visit did not differ significantly between the models. However, total duration of face-to-face interaction and proportion of the visit spent in face-to-face interaction were significantly greater in the team-based model. Patient satisfaction did not differ between the 2 models. Background The US Preventive Services Task Force (USPSTF) guidelines recommend one-time abdominal aortic aneurysm ultrasound screening for men aged 65 to 75 years who ever smoked. Reported screening rates have been 13% to 26% but did not include computed tomography, magnetic resonance imaging, and nonaortic abdominal ultrasound, which provide adequate visualization of the aorta. The objective of this study was to evaluate rates of screening performed intentionally with ultrasound and incidentally with other abdominal imaging, determine rates of redundant screening, and evaluate patient and physician characteristics associated with screening. Methods Cross-sectional study of patient encounters in 2007 and 2012 to determine abdominal aortic aneurysm screening trends in primary care practices. Participants included all patients who were seen in a primary care office and were eligible for screening by USPSTF guidelines. The primary outcome was percentage of eligible patients screened for abdominal aortic aneurysm by ultrasound or other abdominal imaging. Results There were 15,120 patients eligible for screening in 2007, and 22,355 in 2012. Screening with ultrasounds increased from 3.6% in 2007 to 9.2% in 2012. Screening with any imaging that included the aorta increased from 31% in 2007 to 41% in 2012. Of 2595 screening ultrasounds performed in either cohort, 800 (31%) were performed on patients who had already undergone another imaging modality. Of 153 physicians who had >50 eligible patients, rates of abdominal aortic aneurysm screening ranged from 7.5% to 79% (median 39%, interquartile range 31%-47%), and rates of ultrasound screening ranged from 0% to 47% (median 6.3%, interquartile range 3.6%-11.4%). Physician characteristics positively associated with screened patients included female sex (odds ratio [OR] 1.32; 95% confidence interval [CI], 1.12-1.54), specialty (Internal Medicine vs Family Medicine: OR 1.32; 95% CI, 1.14-1.54), and location (academic medical center vs family health center: OR 1.30; 95% CI, 1.04-1.62). Conclusions Abdominal aortic aneurysm screening rates remain below 50%, but are improving over time. Screening by individual physicians varied widely, indicating substantial opportunity for educational interventions. Most abdominal aortic aneurysm screening is completed incidentally, and some patients later undergo unnecessary ultrasound screening. Objectives: In 2010, the American Heart Association unveiled a strategic plan to reduce cardiovascular deaths by targeting seven components of ideal cardiovascular health. Although education is a sensible first step, it is not known whether awareness correlates with healthy behavior. The objective of the study was to examine the association between awareness of risk factors and ideal cardiovascular health behavior. Methods: We surveyed patients 40 years and older at five ambulatory clinics. The survey measured demographics, health management behaviors, comorbidities, and awareness of five modifiable cardiac risk factors (smoking, obesity, high cholesterol, hypertension and diabetes mellitus) and one protective factor (exercise). Healthy behavior was defined as follows: diabetes, hemoglobin Ale <8.0%; hypertension, systolic blood pressure <140 mm Hg), high cholesterol, medication adherence; obesity, attempting to lose weight; smoking, abstinence; and exercise, ≥30 minutes/day, ≥3 times per week. Results: For five modifiable risk factors, awareness was positively associated with healthy behavior in multivariable models: obesity, hypertension, exercise, cholesterol, and diabetes. Awareness was inversely associated with smoking abstention. Conclusions: Awareness that a specific factor increases the risk for cardiovascular disease was positively associated with healthy behavior regarding most risk factors; however, the association was modest, suggesting that awareness alone does not motivate behavior.
