Abstract

INTRODUCTION: Older patients with multimorbidity, polypharmacy and related complex care needs represent a growing proportion of the population and a challenge for healthcare systems. Particularly in transitional care (hospital admission and hospital discharge), medical errors, inappropriate treatment, patient concerns and lack of confidence in healthcare are major problems that may arise from a lack of information continuity. The aim of this study is to develop an intervention to improve informational continuity of care at the interface between general practice and hospital care. METHODS AND ANALYSIS: A qualitative approach will be used to develop our participatory intervention. Overall, 32 semistructured interviews with relevant stakeholders will be conducted and analysed. The stakeholders will include healthcare professionals from the outpatient setting (general practitioners, healthcare assistants, ambulatory care nurses) and the inpatient setting (clinical doctors, nurses, pharmacists, clinical information scientists) as well as patients and informal caregivers. At a series of workshops based on the results of the stakeholder analyses, we aim to develop a participatory intervention that will then be implemented in a subsequent pilot study. The same stakeholder groups will be invited for participation in the workshops. ETHICS AND DISSEMINATION: Ethical approval for this study was waived by the Ethics Committee of Goethe University Frankfurt because of the nature of the proposed study. Written informed consent will be obtained from all study participants prior to participation. Results will be tested in a pilot study and disseminated at (inter)national conferences and via publication in peer-reviewed journals. Abstract

Background: Practice guidelines are an important instrument for quality improvement – also in primary care. For years, members of the German College of General Practice and Family Medicine (DEGAM) have developed methodologically ambitious guidelines for family physicians or have been involved in the development of interdisciplinary guidelines mostly on a voluntary basis. Since 2020, the Innovation Committee of the German Federal Joint Committee (G-BA) has announced funding programs for the (further) development of guidelines. Objectives: This publication intends to present DEGAM guideline projects currently funded by the Innovation Committee of the G‑BA. Materials and methods: The concepts of the funded guidelines are briefly described by the authors. Results: In the second round of the G‑BA's call for proposals for the funding of practice guidelines (in 2021), five author teams of DEGAM guidelines applied and received funding. The funded DEGAM guideline projects include (a) the development of a new guideline, (b) the update of two existing guidelines, and (c) the update and upgrading of an existing DEGAM guideline from S1 to S3 level, as well as the update of an S3 guideline as a living guideline. Conclusions: Guideline development is resource-intensive and requires voluntary commitment. The G‑BA funding opens up opportunities for DEGAM to (further) develop guidelines that would otherwise not have been developed at all or not as upgrades. In order to advance guideline programs, additional funding is needed, since the resources of voluntary guideline authors are limited. The G‑BA funding also supports the recruitment of young researchers for guideline development in the medium term. The extent to which this G‑BA support is sustainable will be proven in the future. Abstract

Ziel der Studie: Die Wirksamkeit von Screening und Brief Intervention (SBI) zur Reduktion des Alkoholkonsums konnte in einigen Studien gezeigt werden. Trotz vieler Anstrengungen zur Implementierung von SBI in die Regelversorgung wird jedoch nur ein kleiner Teil von Patienten entsprechend versorgt. Deshalb wurde in der Studie untersucht, inwieweit sich Hausarzte durch Barrieren beeintrachtigt sehen und ob bestimmte Barrieren im Sinne sozialer Erwunschtheit verzerrt beurteilt werden. Methodik: Der Fragebogen wurde an 700 Hausarzte in Sachsen-Anhalt verschickt. Ergebnisse: 178 Fragebogen wurden ausgewertet. Neben zu geringer Vergutung wurde zu geringe Adherence als entscheidende Barriere angegeben. Hausarzte mit geringer Tendenz zu sozialer Erwunschtheit finden SBI unangenehmer und geben SBI eine geringere Bedeutung als Hausarzte mit hoher Tendenz. Schlussfolgerung: Masnahmen zur hausarztlichen Suchtpravention sollten auf Basis einer differenzierteren Kenntnis motivationaler Effekte einzelner Barrieren weiterentwickelt werden.
