Objective: To identify in the literature the main contributing factors for the occurrence of Adverse Events in Primary Health Care. Methods: This is an integrative review. Based on the formation of the guiding question, a research was carried out in the databases: MEDLINE, PubMed, BDENF, SciELO and LILACS. At the end, 15 articles were included. Results: 7 of the articles included factors related to medication as main, 3 had practice and procedures, 2 organization and management, 1 communication and 2 other factors. Conclusion: Errors related to medication and diagnosis are the most common in health instances. The failure to communicate between employees and between health services, in addition to organizational factors and late diagnoses were also pointed out, being mostly avoidable. Underreporting was commonly reported in the articles, revealing that existing data may not be consistent with reality. [ABSTRACT FROM AUTHOR]

Objectives : to propose and verify the applicability of a model to measure the nutritional care of children under five years of age in the Family Health Strategy (Brazil). Methodology : this is a methodological research. The study contemplated the construction of a logical-operational model and the ascertainment of its applicability. The development of the model was based on the implementation analysis and on Donabedian's evaluative proposal for health services. The applicability was verified in 24 health teams in two municipalities in the department of Paraíba (Brazil), with the inclusion of three intertwined levels of analysis (health teams, professionals and population). The results were compared according to the municipality and type of health team by means of Student's t-test. Results : the model constructed contemplates parameters of structure (human resources and availability of materials and supplies) and process (general practices of health professionals, growth monitoring, prevention of specific nutritional deficiencies, context of the Lacta and Alimenta Brasil Strategy and management/intersectoral). The application characterized nutritional care as fragmented, noting deficiencies in the compliance with the growth curves of the child health card and in the National Iron Supplementation Program in both municipalities, and better performance in the municipality with expanded teams with a nutritionist. Conclusions : the model will be able to guide the monitoring, evaluation, management and measurement of nutritional care for children in the Family Health Strategy. The degree of implementation of food and nutrition actions was "intermediate", with a better situation for extended teams with a nutritionist. [ABSTRACT FROM AUTHOR]

To analyze areas of spatial risk for the occurrence of leprosy in a city in São Paulo. Ecological study of leprosy cases registered in a Health Information System, from 2006 to 2016. Descriptive analysis and the application of Kernel density statistics were used. 85 leprosy cases were identified, with a predominance in males; age group equal to or greater than 51 years; White breed; complete or incomplete elementary school and dimorphic clinical form. 75 cases were geocoded. In the Kernel analysis, a heterogeneous spatial distribution of the disease was observed, with concentrations of cases in the northern, eastern and small areas in the southern region of the municipality. This study showed the disease in population groups with low income, low education and in situations of poverty, thus translating the need to develop public policies that can envision new strategies for the control of leprosy. The purpose of the study is to describe the profile and performance of municipal coordinators of food and nutrition actions in Primary Health Care. This was a cross-sectional study involving 74 managers in the state of Paraíba, Brazil. Information was collected on demographic; labor and training characteristics; knowledge and use of technical documents in the area; actions developed and satisfaction with the work. Of the total, 19 coordinators were trained in nutrition, 13 were men and 46 were employed without a public competition. Nutritionists were more qualified to handle public health nutrition issues during undergraduate studies. Several technical documents of the area were referred to as not known and not used; in some of them, nutritionists presented a better situation, which was similar for the development of some actions. In addition to the importance of the nutritionist, the findings highlight problems of structure; poorly trained professionals; lack of articulation (intersectoral and between municipal and local levels); insufficient remuneration and inappropriate management mechanisms. It is concluded that coordinators lack links, training, knowledge, and adequate performance for the management of the nutrition area in Primary Health Care. [ABSTRACT FROM AUTHOR]

The article assesses the reliability of the instrument Health Care for Dementia: the Primary Care perspective, in versions for doctors and nurses. A correlational study was conducted in Basic Health Units of the Family Health Strategy belonging to the municipality of São Paulo, Brazil. The sample consisted of 87 doctors and 132 nurses working in the selected Units. Reliability was assessed by means of item stability (test-retest). Data collection was performed by the self-application of the instrument by professionals and the retesting step occurred on the 14th day after the test. The results revealed that 86.0% of the instrument's items have stability between strong and almost perfect/high; 13.3%, moderate stability and 0.7%, regular. These results allow us to indicate its use to identify the educational needs of doctors and nurses in Primary Health Care in monitoring dementias. [ABSTRACT FROM AUTHOR]

This article is a theoretical essay aiming to reflect on the construction of clinical performance in Primary Health Care (PHC) based on Foucault's concept. Through an explanation of a fictitious case, arguments are developed in which we defend that the main biopolitical strategy used in the clinic performed in the PHC currently; is governmentality. We point out some of Foucault's theoretical-methodological tools, inscribed in the being-with-yourself or ethics phase, as possible analysis strategies to think about possibilities for the clinic extension, from the perspective of including the subject in its construction. In addition to the governance of behaviors, which lead to subjection through the use of governance devices, the notions of self-practices, self-knowledge, and parrhesia point to the possibility of building a clinic in PHC guided by an ethical relationship between professionals and health system users. Objective: To describe the strategic actions for the coordination of care, monitoring and surveillance of COVID-19 cases in Primary Health Care. Method: An experience report type study, developed in a territory of the Family Health Strategy at the Headquarters of the municipality of Sobral - Ceará, Brazil. Results: Role of Primary Health Care in the coordination of care, surveillance and monitoring of COVID-19 cases in the health territory, and in the organization of the Health Care Network; the importance of horizontal social isolation and the isolation of positive cases at home for the control of COVID-19; the potentiality of using digital technology tools in the territory, for the dissemination of actions, dissemination of information through social networks and the practice of call centers; effectiveness of teamwork and networking in the development of pandemic control actions. Conclusion: Primary Health Care has a strategic role in actions to combat COVID-19 in the territory, especially in reducing community transmission, in responding to demands and monitoring cases and in surveillance in each phase of the pandemic. Objective: to evaluate prenatal care from the perspective of nurses within the scope of the Family Health Strategy. Method: descriptive, exploratory study with a quantitative approach, conducted with 29 nurses in 20 Family Health Strategies in 2016. Data collected through a questionnaire was analyzed with the aid of the Statistical Package for Social Sciences for Personal Computer program. Results: it was emphasized that 55.2% of nurses considered human resources insufficient. It was observed that 41.4% of the units had a rapid test for Syphilis and Human Immunodeficiency Virus; 69% material for gynecological examination, which was performed by 55.2% of professionals. Conclusion: the assessment of prenatal care, from the nurses' perspective, made contributions to the recognition of the limits and possibilities for adherence to prenatal care as recommended by the Ministry of Health, with a view to reducing risks to maternal and fetal health. The extension project "Health Education: Health promotion to improve the quality of life of the community" integrates the actions that underlie the tripod teaching-research-extension of the Medicine course at the Federal University of Maranhão-Campus Pinheiro, and takes place through educational actions in the community. Using the active methodology of the teaching-learning process, through the Arco de Maguerez, this work consists of an experience report of a descriptive character, with the objective of describing the experiences of undergraduate students of the Medicine course with the Community Agents (CHA) and users of Basic Health Units in two municipalities in Maranhão. This project aimed to disseminate health knowledge to the population, addressing Munchausen Syndrome by Proxy. 111 community agents from the municipalities covered, as well as the surrounding community, benefited from health education. Munchausen Syndrome and Munchausen Syndrome by Proxy were addressed, when CHWs had the opportunity to learn about the disorders, how to identify the syndromes, how to differentiate and how to refer the patient. Fifteen cases were diagnosed among users of health services in the municipalities involved. The exchange of experience between students and the community is essential to strengthen the citizenship process of those involved, and it is important that other training for health teams be carried out, as well as future work that addresses health education in the community be carried out. Objective: To identify the difficulties lived by the nurses during the Nursing Consultation in childcare to Haitian children in the Primary Health Care and the possibilities to overcome these challenges. Method: this is an exploratory, descriptive study with a qualitative approach, conducted with ten nurses through an interview held in July 2018. The testimonies were submitted to Content Analysis. Results: From the data analysis, two categories emerged: Difficulties in the intercommunication between nurses and families of Haitian children and Cultural difficulties for the care of Haitian children. Conclusion: the results reveal difficulties in carrying out the Nursing Consultation for children, especially related to intercommunication, due to the language spoken by Haitians, and due to the cultural issues that permeate and support the care of children by families. This paper aims to analyze the processes developed in Brazil and Portugal intended to integrate the Primary Health Care and the Specialized Health Care. Primary Health Care conceived of as the first step for accessing to the health system stands out among the most usual initiatives and thus the family doctors are attributed a prevailing role. It is also emphasized the territorialization of the health services based on the geographic location, the referral to specialized doctors and the implementation of clinic protocols agreed between the parties. The main results indicate that in Brazil they implemented decentralized regulatory systems, and, in Portugal they created local health units (structures owned by the health ministry that integrate primary health care and specialized health care provided by the same management team). In conclusion, this work underlines the need to develop a true investment policy intended to strengthen the response capacity in the specialized health care and to add more value to the work performed by the primary care professionals. This way, they will strengthen the collaborative culture that will translate into successful integration processes with results such as good health and user's satisfaction. [ABSTRACT FROM AUTHOR]

Introduction: Studies on patient safety in Primary Health Care (PHC) are up to date; however, there are no details on professionals' actual performance in relation to this matter. Thus, it is important to evaluate the safety culture characterization among nursing professionals in this scenario, since it will promote better care for users. Objective: To evaluate the patient safety culture from the perspective of nurses in Primary Health Care. Method: Descriptive study of quantitative approach carried out in fourteen health Units in Curitiba City, Paraná State (PR), Brazil from July to December 2017. There were forty-three nurses participating in this study. Medical Office Survey on Patient Safety Culture (MOSPSC) was the tool used, comprising fifty-one questions that evaluate twelve dimensions of patient safety. This tool, Portuguese version, showed a Cronbach alpha coefficient of 0.95, which expresses high reliability. All data were analyzed by Descriptive Statistics using Likert scale. Results: Inconsistencies were found related to the information exchange with other institutions, work process, fear of guilt, punishment for errors, and management. In the overall evaluation, 50% of the Family Health Strategy (FHS) professionals rated patient safety "very good", and 73.9% of Basic Health Units (BHU) professionals rated patient safety "good". Conclusion: Results show a safety culture with potentialities and weaknesses that need to be developed, considering the nurse the cornerstone to the success of quality and safety enhancement of patient in Primary Health Care. Objective: To describe the health needs of men and the factors that motivate the practice of self-medication in a city in northeastern Brazil. Method: This is a descriptive, exploratory study with a qualitative approach, whose research field was a family health unit in a city in the interior of Bahia, Brazil. The study included men who performed the practice of selfmedication, from November 2015 to May 2016. For data collection, we used the semi-structured interview, organized and presented by the Collective Subject Discourse (CSD) method. Results: Considering the key expressions and the central ideas of the discourses, two distinct but complementary aspects emerged, the health needs and the motivations for self-medication that pervade issues involving men's conception of public health services. Dissatisfaction with the provision of care, lack of specific programs and easy access to medicines are the main factors that facilitate the practice of selfmedication. Conclusions: It is necessary to create mechanisms for strengthening and qualifying Primary Care regarding the importance of quality and comprehensiveness of care, so that men abandon the practice of self-medication and value health prevention. - Abstract
This paper aims to describe the scenario and to identify factors associated with non-use of teleconsultation by primary care physicians in the macrorregião Norte de Minas Gerais (Northern macroregion of Minas Gerais), Brazil. This study is a cross-sectional one. For data collection, a self-administered questionnaire was fill in by the health managers working in all the municipalities of that macroregion; it was previously tested through some of those professionals. The Poisson regression with robust variance was used after bivariate analyzes to identify factors associated with non-use of teleconsultation.To participate in the study 86 municipal health managers were invited. Important structural barriers to teleconsultation implementation were identified: lack of computers with access to internet in basic health units (76.7%) and poor internet connection quality (16.3%). After multiple analysis, only the lack of motivation strategies by the health managers was associated (p<0.007; PR=2.19; IC95%:1.24-3.86) with non-use of teleconsultation by physicians. The results reinforce the role played by the health manager in promoting the use of teleconsultation service in primary care. [ABSTRACT FROM AUTHOR]

Objective: To assess the quality when filling out a health follow-up instrument for children in a town in northeastern Brazil. Method: It is a cross-sectional study involving 204 children who received care at 16 family health units. This study analyzes how they filled out the child's health card (CS) and the Child's Health Record (CSI), the differences between both and the associated factors. Results: the filling out was unsatisfactory in 37.2% of the CS and 50.5% of the CSI. Regarding the CSIs, the correct filling out was observed less often in the following cases: supply of iron supplements (0%), development checkups (1.1%), prenatal care (9.9%), feeding after stopping the breastfeeding (17.6%), and growth curves (0%-27.5%). There was a lower percentage of correct filling out for the CSIs regarding the baby's name, type of labor and weight indication in the graph. Newborns from married mothers (p = 0.045) and female babies (p = 0.017) showed lower probability of inappropriate filling out of both CSs and CSIs, respectively.
