To investigate the association between cytokine peripheral levels and the risk of cardiovascular disease in patients with schizophrenia and controls.A sample of 40 patients and 40 control subjects participated in the study. Psychiatric diagnosis was established following structured clinical assessment. The Framingham Score was used to assess cardiovascular risk (CVR). Serum levels of the cytokines IL-1β, IL-6, IL-8, IL-10, IL-12p70 and TNF-α were determined by cytometric bead array (CBA) technique, and the serum levels of IL-33, sST2, sTNFR1, sTNFR2, Leptin and Adiponectin by Enzyme-Linked Immunosorbent assay (ELISA).Patients with schizophrenia showed greater frequency of moderate CVR when compared with controls (p = 0.14). In addition, patients showed higher levels of sTNFR2 and Adiponectin compared to controls (p = 0.007 and p 0.001, respectively). Adiponectin and sTNFR2 were associated with CVR only in patients (p = 0.0002 and p = 0.033, respectively). In multivariate analysis controlling for socio-demographic and clinical confounders, illness duration (r = 0.492; p 0.002) and sTNFR2 (r = 0.665; p 0.004) were independent predictors of CVR.Our results reinforce the concept that patients with schizophrenia are at greater risk to develop cardiovascular diseases, and suggest that the associated chronic low-grade inflammation might play a role in this process.

Introduction Childhood trauma (CT) is known to be a vulnerability factor for schizophrenia, but the specific impacts of different trauma subtypes on the prognosis of these patients remains unclear. Objective To assess the relationships between the occurrence of overall CT and its subtypes with factors with known prognostic impact on schizophrenia, such as age at onset of symptoms, global functioning, and cognitive impairment in a sample of Brazilian patients. Methods One hundred and five stable patients diagnosed with schizophrenia according to DSM-5 criteria were evaluated using the Independent Living Skills Survey (ILSS; self-report global functioning), Schizophrenia Cognition Rating Scale (SCoRS; subjective cognitive impairment), and Childhood Trauma Questionnaire scales (CTQ; perceived overall CT, emotional neglect, physical neglect, physical abuse, and emotional and sexual abuse). Statistical analysis was performed with multivariate linear regression. Results After controlling for educational level and age, subjective cognitive impairment was directly correlated with overall perceived CT occurrence, emotional abuse, and sexual abuse. Self-report global functioning was inversely correlated with perceived overall CT occurrence, emotional abuse, and sexual abuse. Emotional abuse and physical abuse were also inversely correlated with age at onset of symptoms. Conclusions CT can be related to more severe prognoses in schizophrenia, impacting on early onset of symptoms, lower global functioning, and greater cognitive impairment. Subtypes of trauma can be associated with different prognostic risks.

Aims: Recent findings suggest that cognitive impairment can be associated with inflammation and immune changes in schizophrenia. We aimed to study possible associations between cytokine levels and cognitive performance in a sample of patients with schizophrenia.Methods: Cognition was assessed with the brief assessment of cognition in schizophrenia in 63 clinically stable outpatients with schizophrenia. Blood was collected and cytokines (IL-2, IL-4, IL-6, IL-10, IL-17A, TNF-α, IFN-γ) were measured by cytometric bead array method. Psychopathological scales were also applied.Results: IL-6 correlated negatively with general cognitive performance (rho = -0.395, p = .017) and positively with antipsychotic dose (rho = 0.412, p = .004). Multiple regression analysis showed that cognitive performance is associated with age and antipsychotic dose (p = .000 and p = .033).Conclusion: The association between IL-6 levels and cognitive performance is dependent on age and antipsychotic dose.

Objective: To test the hypothesis that genetic variations of cannabinoid receptors contribute to the pathophysiology of cognitive deficits in schizophrenia. Methods: In this genetic association case-control study, cannabinoid receptor polymorphisms CNR1 rs12720071 and CNR2 rs2229579 were tested for association with neurocognitive performance in 69 patients with schizophrenia and 45 healthy controls. Neurocognition was assessed by the Brief Assessment of Cognition in Schizophrenia (BACS). Results: We found a consistent association between CNR1 rs12720071 polymorphism and the cognitive performance of patients in several cognitive domains. Patients with C/C polymorphism presented significantly worse performance in motor speed, verbal fluency, attention/processing speed and reasoning/problem solving. Conclusion: Although limited, our data support the hypothesis that CNR1 variations may be associated with the pathogenesis of cognitive deficits of schizophrenia.

This study aimed at evaluating changes in the renin-angiotensin system (RAS) in patients with schizophrenia in comparison with controls. Plasma levels of angiotensin-converting enzyme (ACE), ACE2, angiotensin (Ang)-(1-7) and Ang II were assessed in 25 patients with schizophrenia and 20 controls. Patients with schizophrenia presented decreased levels of ACE compared to controls [median (25th-75th percentiles) = 434.79 (341.15-524.02) vs. 508.49 (396.34-608.72); p 0.05]. No significant differences were found regarding ACE2, Ang-(1-7) and Ang II levels. There were no associations between the measured molecules and clinical parameters. Our results corroborate the hypothesis that the RAS is involved in the pathophysiology of schizophrenia.

Introduction: Neurological soft signs (NSS) have been considered one of the target features and a potential endophenotype for schizophrenia. The present study aimed to characterize NSS in a sample of patients with chronic schizophrenia and to compare them with healthy control individuals. Methods: In this study, we evaluated the presence of NSS in a sample of stable patients (n = 24) diagnosed with schizophrenia according to DSM-IV criteria, recruited at the Schizophrenia Outpatient Clinic of Instituto Raul Soares, Belo Horizonte, state of Minas Gerais, southeastern Brazil. Assessment was made with the Brief Motor Scale (BMS), and extrapyramidal symptoms (EPS) were evaluated with the Simpson-Angus Scale (SAS) and the Abnormal Involuntary Movement Scale (AIMS). A control group (n = 21) was also submitted to the same battery of tests. Results: We observed a significant difference in relation to BMS and SAS scores (p < 0.0001), revealing that individuals with schizophrenia present more NSS and EPS than healthy ones. BMS total scores correlated positively with SAS scores (r = 0.495, p = 0.014), but not with AIMS scores, indicating that NSS could be influenced by the intensity of EPS. Nevertheless, we observed that this relationship remained only for motor coordination tasks (r = 0.550, p = 0.005), while motor sequencing tasks were not influenced by EPS (r = 0.313, p = 0.136). Conclusion: The results suggest that NSS are more frequent in patients with schizophrenia and that motor sequencing tasks could be more specific to the syndrome. Resumo Introdução: Sinais neurológicos sutis (SNS) têm sido considerados características básicas e potenciais endofenótipos na esquizofrenia. O presente estudo procurou caracterizar os SNS em uma amostra de pacientes com esquizofrenia crônica e compará-los com indivíduos controles saudáveis. Métodos: Neste estudo, avaliamos a presença de SNS em uma amostra de pacientes estáveis (n = 24) com o diagnóstico de esquizofrenia de acordo com os critérios do DSM-IV, recrutados no Ambulatório de Esquizofrenia do Instituto Raul Soares, Belo Horizonte, MG, Brasil. A avaliação foi realizada com a Escala Motora Breve (BMS) e sinais extrapiramidais (SEP) foram observados com a Escala de Simpson-Angus (SAS) e a Escala de Movimentos Involuntários Anormais (AIMS). Um grupo controle (n = 21) também foi submetido à mesma bateria de testes. Resultados: Observamos uma diferença significativa em relação aos escores da BMS e da SAS (p < 0,0001), revelando que indivíduos com esquizofrenia apresentam mais SNS e SEP que indivíduos saudáveis. Os escores da BMS se correlacionaram positivamente com os da SAS (r = 0,495, p = 0,014), mas não com os da AIMS, indicando que os SNS podem ser influenciados pela intensidade de SEP. No entanto, observamos que essa relação permaneceu somente para as tarefas de coordenação motora (r = 0,550, p = 0,005), enquanto as tarefas de sequenciamento motor não foram influenciadas pelos SEP (r = 0,313, p = 0,136). Conclusão: Os resultados sugerem que os SNS são mais frequentes em pacientes com esquizofrenia e que tarefas de sequenciamento motor podem ser mais específicas na síndrome.

Objectives to evaluate cannabinoid receptors (CBRs) expression on peripheral immune cells, i.e., blood monocytes, neutrophils, lymphocytes, and NK cells, and their relationship to a wide range of serum cytokine levels in subjects with schizophrenia and controls. Methods A sample of 55 people with chronic schizophrenia and 48 controls were enrolled in the study. The expression of the cannabinoid receptors CB1R and CB2R was evaluated in peripheral blood leukocytes by flow cytometry. Serum levels of cytokines/chemokines were simultaneously analyzed by cytometric bead array. Results We found higher expression of cannabinoid receptors on cells of the innate immune system in subjects with schizophrenia when compared with controls. Serum levels of interleukin-4 (IL-4), IL-6, IL-10, IL-17, interferon (IFN-γ), and (C-X-C motif) ligand 10/interferon gamma-induced protein 10 (CXCL10/IP10) were decreased, while levels of the chemokine (C-C motif) ligand 2/monocyte chemoattractant protein-1 (CCL2/MCP-1) were increased in the schizophrenia group in comparison with controls. Patients with schizophrenia showed simpler correlation network between cytokines and CBRs expression than controls. Conclusion Patients with schizophrenia showed increased CBRs expression in cells of the innate immune system and simpler correlation network between cytokines and CBRs expression when compared with controls. These results suggest a defective endocannabinoid system-mediated immunomodulation in patients with schizophrenia.

Objective Changes in immune system have been reported in schizophrenia. This study aimed to evaluate the involvement of IL-33, a member of the IL-1 cytokine family, in schizophrenia and its association with cognitive performance in these patients. Methods Forty patients with chronic schizophrenia and 40 healthy subjects participated in the study. Serum levels of IL-33 and sST2 (soluble form of the IL-33 receptor) were measured using enzyme-linked immunosorbent assay (ELISA). Patients were evaluated with the Brief Assessment of Cognition in Schizophrenia (BACS) and the Schizophrenia Cognition Rating Scale (SCoRS). Results Patients with schizophrenia and controls presented similar serum levels of IL-33 and sST2. Levels of both markers were positively correlated with cognitive performance in patients with schizophrenia. Conclusion We found a significant correlation between IL-33 and sST2 levels and cognition in schizophrenia. Our results might help in the understanding of how immune markers are associated with cognitive impairment in schizophrenia. It remains to be determined whether the association between IL-33/sST2 and cognition is restricted to patients with schizophrenia.

Deficits in neurocognition and social cognition play a critical role in the functional impairment of patients with schizophrenia. Increased oxidative stress has been evidenced in schizophrenia. Increased oxidative stress can affect neuronal function and lead to impairments in neurocognitive functions (especially working memory) and social cognition.To investigate deficits in neurocognition and social cognition and their potential association with oxidative stress biomarkers in schizophrenia.Eight-five clinically stable patients with schizophrenia and 75 controls were enrolled in this study. Neurocognition was evaluated through the Brief Assessment of Cognition in Schizophrenia (BACS). Social cognition was assessed through the Hinting Task - a test of theory of mind - and an emotion processing test, Facial Emotion Recognition Test (FERT-100). Oxidative stress was assessed by measuring serum levels of glutathione (GSH) and thiobarbituric acid reactive substances (TBARS).Patients had decreased serum levels of GSH (Z=3.56; p0.001) and increased TBARS (Z=5.51; P0.001) when compared with controls. TBARS levels are higher in patients using first generation antipsychotics. Higher serum levels of TBARS in patients were associated with poor performance in working memory test (r=-0.39; p=0.002), even when controlling for age and negative symptoms (Standard Beta: -0.36; CI= -2.52 a -13.71).The association between greater lipid peroxidation, as assessed by TBARS, and worse performance in working memory corroborates theoretical models of greater vulnerability of schizophrenia to oxidative stress.

Objective: To analyze the correlation between quality of life, symptoms, and cognition assessed by the interview-based Schizophrenia Cognition Rating Scale (SCoRS). Methods: Seventy-nine outpatients diagnosed with schizophrenia were evaluated with the Quality of Life Scale – Brazilian version (QLS-BR), the SCoRS, and symptoms scales (Positive and Negative Syndrome Scale [PANSS]). After determining the potential explanatory variables using Spearman’s correlation and Student’s t test results, we ran simple, multivariate, and decision-tree regression analyses to assess the impact of SCoRS and PANSS ratings on mean overall quality of life. Results: Cognitive deficits and negative symptoms were the best predictors of quality of life. A low degree of negative symptoms (PANSS negative < 11) was a strong predictor of better quality of life (QLS ∼ 75), regardless of SCoRS rating. Among participants with more severe negative symptoms, elevated cognitive impairment (interviewer SCoRS ∼ 44) was a predictor of worse quality of life (QLS ∼ 44). Conclusions: Cognitive impairment determined by interview-based assessment seems to be a strong predictor of quality of life in subjects with severe negative symptoms. These results support the usefulness of SCoRS for cognitive assessment that is relevant to the everyday life of patients with schizophrenia.

Objective: Recent evidence has associated immune and inflammatory changes to cognitive performance in many diseases, including schizophrenia. Since this is a new research field where concepts are not yet solid and new questions and hypothesis are still arising, the present study aimed at summarizing the available clinical data associating schizophrenia, cognition and inflammation/immune function. Methods: A systematic review of the literature was made by searching the following terms in Medline: “schizophrenia or psychosis or psychotic” AND “inflamm* or immun* or cytokine or IL-* or TNF-* or kynureni* or KYNA”, AND “cognit* or attention or memory or executive function”. Results: Seventy five papers were identified using the selected terms, and seven papers were included in the review. Papers excluded focused mainly on basic research or other neuropsychiatric disorders. Conclusions: Recent findings link inflammatory markers to cognition in schizophrenia, suggesting that inflammation is associated with worst cognitive performance. Microglial activation, monoaminergic imbalance, brain abnormalities and the kynurenine pathway are possible mechanisms underlying cognitive impairment in schizophrenia. Clinical trials with addition of immunomodulatory drugs have shown promising results, opening new windows to tackle cognition in schizophrenia.

Background Cognition is a major determinant of functioning in patients with schizophrenia. There is evidence that the endocannabinoid system influences cognition in human subjects, and participates in the pathophysiology of schizophrenia. In a previous study, we have shown that the expression of cannabinoid receptors (CB1R and CB2R) on peripheral lymphocytes is inversely correlated with performance in the Brief Assessment of Cognition in Schizophrenia (BACS), in patients with schizophrenia. Recently, CBRs polymorphisms have been associated with an increased risk for schizophrenia, structural changes in the central nervous systems and in cognitive performance of the patients. The aim of the present study was to investigate the association between CBRs polymorphisms and cognitive performance as assessed by the BACS. Methods A sample of 85 stable medicated patients (61% men; age = 41.6 ± 12.2 years; illness duration = 12.8 ± 10.7 years) was enrolled in this study. Two CB1R polymorphisms (rs1049353; rs12720071) and one CB2R polymorphism (rs2229579) were tested. Results We did not find any difference in general cognitive performance (BACS total score) regarding the three polymorphisms tested. However, when we analysed specific cognitive domains we have found a significant difference (p=0.002) regarding working memory (assessed by the Digit Span test) in patients with the rs12720071 polymorphism, where those with allele C performed better than those with T/T genotype. Since about a third of the patients (34%) had a history of past use of cannabis and 2.5% reported current use, we performed the rs12720071 polymorphism analysis excluding these patients. In this subgroup of patients, those with allele C also performed significantly better on Digit Span test (p=0.037). Discussion In this sample, the rs12720071 polymorphism of CB1R appears to influence performance on a working memory task that is sensitive to prefrontal cortex function.

Objectives: To investigate the determinants of family burden in a sample of patients with schizophrenia and their caregivers. Methods: Thirty-one stable patients with schizophrenia and their main caregivers were recruited. Sociodemographic variables were assessed in a semi-structured interview, and positive and negative symptoms were assessed with the Positive and Negative Syndrome Scale (PANSS). Cognitive performance was assessed with the Schizophrenia Cognition Rating Scale (SCoRS). Levels of burden on caregivers were assessed with the Family Burden Interview Schedule (FBIS). Interactions among variables were analyzed using Pearson correlations and linear regression analysis. Results: Objective and subjective FBIS scores were 1.9 (standard deviation [SD] = 0.5) and 2.4 (SD = 0.6) respectively. Objective burden correlated positively with positive and negative symptoms, and cognitive impairment. Subjective burden correlated positively with positive symptoms and negatively with mean age of disease onset. Positive, negative and cognitive symptoms accounted for 47.6% of the variance of objective burden, with negative symptoms accounting independently for 30.3%. Age of onset, parents as caregivers and positive symptoms accounted for 28% of the variance of subjective burden, with age of onset independently explaining 20.3%. Conclusion: Patients' clinical and sociodemographic variables are important determinants of family burden in schizophrenia. Objective burden is predicted by symptoms, particularly negative ones. Subjective burden is predicted by symptoms and sociodemographic variables, particularly age of disease onset. Resumo Objetivo: Investigar os determinantes de sobrecarga familiar numa amostra de pacientes com esquizofrenia e seus cuidadores. Métodos: Trinta e um pacientes com esquizofrenia e seus principais cuidadores participaram do estudo. Variáveis sociodemográficas foram analisadas por meio de uma entrevista semiestruturada, e sintomas positivos e negativos foram quantificados através da Positive and Negative Syndrome Scale (PANSS). O desempenho cognitivo foi mensurado pela Schizophrenia Cognition Rating Scale (SCoRS). A Escala de Avaliação da Sobrecarga dos Familiares (FBIS) avaliou os níveis de sobrecarga entre os cuidadores. As interações entre as variáveis foram verificadas por meio do coeficiente de correlação de Pearson e de análise de regressão linear. Resultados: Os escores objetivos e subjetivos da FBIS foram 1,9 [desvio padrão (DP) 0,5] e 2,4 (DP 0,6), respectivamente. Sobrecarga objetiva se correlacionou positivamente com sintomas positivos e negativos, e com prejuízos cognitivos. A sobrecarga subjetiva se correlacionou positivamente com sintomas positivos, e negativamente com a idade de início da doença. Os sintomas positivos e negativos e os prejuízos cognitivos contribuíram com 47,6% da variância da sobrecarga objetiva, sendo que sintomas negativos foram responsáveis independentemente por 30,3%. A idade de início da doença, os pais na posição de cuidadores e os sintomas positivos contribuíram com 28% da variância da sobrecarga subjetiva, sendo que a idade de início da doença contribuiu independentemente com 20,3%. Conclusões: Variáveis sociodemográficas e clínicas dos pacientes são importantes determinantes de sobrecarga familiar na esquizofrenia. A sobrecarga objetiva pode ser predita por sintomas, especialmente os negativos. A sobrecarga subjetiva pode ser predita por sintomas e características sociodemográficas, particularmente a idade de início da doença.

BACKGROUND: Despite the old Kraepelinean concept that bipolar disorder (BD) does not evolve with cognitive decline, the presence of cognitive impairment, especially executive dysfunction has been recognized in BD patients. Brain-derived neurotrophic factor (BDNF) and pro-inflammatory molecules are important contributors to the pathophysiology of BD, and imbalance in peripheral levels of these molecules may be implicated in the cognitive decline observed in BD patients. We aimed to investigate the executive performance of BD type I euthymic patients and its relation with the plasma levels of BDNF, TNF-α and its related soluble receptors (sTNFR1 and sTNFR2). METHODS: We evaluated executive functioning through the Frontal Assessment Battery (FAB). Plasma levels of BDNF, TNF-α, sTNFR1 and sTNFR2 were measured using enzyme-linked immunosorbent assay (ELISA) in 25 euthymic type I BD patients and 25 age and gender matched healthy controls. OBJECTIVE: The Rey Auditory-Verbal Learning Test, which is used to evaluate learning and memory, is a widely recognized tool in the general literature on neuropsychology. This paper aims at presenting the performance of Brazilian adult subjects on the Rey Auditory-Verbal Learning Test, and was written after we published a previous study on the performance of Brazilian elderly subjects on this same test. METHOD: A version of the test, featuring a list of high-frequency one-syllable and two-syllable concrete Portuguese substantives, was developed. Two hundred and forty-three (243) subjects from both genders were allocated to 6 different age groups (20-24; 25-29; 30-34; 35-44; 45-54 and 55-60 years old). They were then tested using the Rey Auditory-Verbal Learning Test. RESULTS: Performance on the Rey Auditory-Verbal Learning Test showed a positive correlation with educational level and a negative correlation with age. Women performed significantly better than men. When applied across similar age ranges, our results were similar to those recorded for the English version of the Rey Auditory-Verbal Learning Test. CONCLUSION: Our results suggest that the adaptation of the Rey Auditory-Verbal Learning Test to Brazilian Portuguese is appropriate and that it is applicable to Brazilian subjects for memory capacity evaluation purposes and across similar age groups and educational levels.

OBJETIVO: Determinar os pontos de melhor sensibilidade e especificidade do Inventário de Depressão de Beck (BDI) e da Escala de Avaliação de Depressão de Hamilton (HAM-D) no diagnóstico de depressão associada à epilepsia. MÉTODOS: Setenta e três pacientes de um centro de referência no tratamento da epilepsia foram submetidos à avaliação neuropsiquiátrica. Foram colhidos dados clínicos e sociodemográficos, sendo utilizados os seguintes instrumentos: entrevista clínica estruturada (MINI-PLUS) para diagnóstico psiquiátrico conforme o DSM-IV, HAM-D e BDI. RESULTADOS: No momento da entrevista, 27,4% dos pacientes estavam deprimidos e 37% preenchiam critérios para diagnóstico de depressão maior ao longo da vida. A análise da curva ROC indicou que o ponto de corte em 16 (> 16) para o BDI (sensibilidade de 94,4%, especificidade de 90,6%) e em 16 (> 16) para a HAM-D (sensibilidade de 95%, especificidade de 75,5%) representou dicotomização ótima entre deprimidos e não deprimidos. Ambos os instrumentos apresentaram um valor preditivo negativo superior a 95%. CONCLUSÃO: A frequência de depressão maior é elevada em pacientes com epilepsia. BDI e a HAM-D podem auxiliar o clínico na identificação da depressão associada à epilepsia, diminuindo seu subdiagnóstico. OBJECTIVE: To determine cutoff points of highest sensitivity and specificity on the Beck Depression Inventory (BDI) and Hamilton Depression Rating Scale (HAM-D) for depression diagnosis in epilepsy. METHODS: Seventy-three patients from a referral center for the treatment of epilepsy underwent neuropsychiatric evaluation. We collected clinical and socio-demographic data, and applied the following instruments: Structured Clinical Interview (MINI-PLUS) for psychiatric diagnosis according to DSM-IV, HAM-D and BDI. RESULTS: At assessment, 27.4% of the patients were depressed and 37% met diagnostic criteria for lifetime major depression. The ROC curve analysis indicated that a score > 16 on the BDI (94.4% sensitivity, 90.6% specificity) and > 16 on the HAM-D (95% sensitivity, 75.5% specificity) revealed great dichotomy between depressed and nondepressed patients. Both instruments showed a negative predictive value exceeding 95%. CONCLUSION: The frequency of major depression is elevated in patients with epilepsy. BDI and HAM-D can help physicians in the identification of depression in epilepsy, reducing its underdiagnosis.

Contexto: O transtorno bipolar tipo I está comumente associado a comorbidades clínicas e psiquiátricas, mas ainda há poucos dados disponíveis sobre pacientes brasileiros. Objetivos: O objetivo do presente estudo foi avaliar a prevalência de comorbidades clínicas e psiquiátricas em uma amostra brasileira de pacientes bipolares tipo I. O objetivo secundário foi investigar as associações de características clínico-demográficas e comorbidades com tentativas de suicídio. Métodos: Foram incluídos neste estudo 94 pacientes bipolares tipo I. O diagnóstico psiquiátrico foi determinado utilizando-se a avaliação Mini International Neuropsychiatric Interview (MINI-Plus). O diagnóstico de comorbidades clínicas foi baseado na história clínica e no acompanhamento de clínicos gerais. Resultados: As comorbidades mais prevalentes nos pacientes bipolares foram: transtorno de ansiedade generalizada (19,20%), dependência de substâncias (43,60%), hipertensão arterial (29,80%), diabetes mellitus (17,00%), dislipidemia (22,30%) e hipotireoidismo (19,10%). Não foram encontradas diferenças estatísticas em relação às características demográficas ou à prevalência de comorbidades nos grupos com e sem tentativa de suicídio. Conclusão: Pacientes bipolares atendidos em serviço psiquiátrico apresentam elevada prevalência de comorbidades psiquiátricas e clínicas. Nessa população, tentativas de suicídio não se associam com a presença de comorbidades ou características demográficas.

INTRODUÇÃO: A preservação das habilidades de compreensão da fala nos idosos está correlacionada com a preservação das funções cognitivas. Essa habilidade é característica fundamental para a comunicação e sua avaliação possibilita a identificação de déficits cognitivos mais específicos, além de facilitar o desenvolvimento de estratégias de intervenção mais precoces e eficientes. OBJETIVO: Disponibilizar normas contemporâneas para a utilização de um instrumento de avaliação da compreensão verbal, o Token Test, para idosos brasileiros saudáveis. MÉTODO: Uma amostra com 120 idosos (76 mulheres), com médias de 71,1 anos para idade e de 6,9 anos de educação formal para escolaridade, foi avaliada utilizando o Miniexame do Estado Mental, para identificação da existência de comprometimento cognitivo, e o Token Test, para avaliação da compreensão da fala. RESULTADOS: Foram encontradas correlações significativas (p < 0,01) entre o escore do Token Test e a educação formal e a idade, os quais corresponderam respectivamente a 5% e 21% da variância compartilhada. Tais variáveis foram consideradas nas tabelas normativas. CONCLUSÃO:Nossos dados indicam que as normas disponibilizadas aqui são suficientemente representativas. Este trabalho abre caminho para estudos futuros de comparação entre o desempenho no Token Test de idosos saudáveis e daqueles em processos de declínio cognitivo. INTRODUCTION: The preservation of oral comprehension in the elderly is correlated with the preservation of their cognitive functions. Oral comprehension is a key feature for communication and its evaluation allows for the identification of more specific cognitive deficits, in addition to facilitating the development of more effective, early intervention strategies. OBJECTIVE: Provide contemporary standards for the use of an instrument to assess oral comprehension, the Token Test, in a sample of healthy seniors. METHOD: A sample of 120 patients (76 women) with mean age of 71.1 years and 6.9 years of formal education, was assessed using the Mini Mental State Examination to identify the existence of cognitive impairment, and the Token Test to assess oral comprehension. RESULTS: There were significant correlations (p < 0.01) between the token test scores for education and age, which accounted for 5% and 21% of shared variance, respectively. These two variables were considered on the normative data tables. DISCUSSION: The data obtained indicate that the standards provided here are sufficiently representative. This study identifies the need for future studies comparing the Token Test performance in elderly people either healthy or in the process of cognitive decline.

CONTEXTO: A Qualidade de Vida (QV) constitui valiosa forma de avaliação do impacto de doenças na vida diária e favorece intervenções mais efetivas sob uma perspectiva biopsicossocial. OBJETIVOS: Este artigo procura analisar criticamente os dados disponíveis sobre as relações entre QV e cognição na esquizofrenia. MÉTODOS: Foi realizada uma pesquisa na base de dados PubMed e Lilacs com os descritores de assunto "schizophrenia" e "cognition disorder" ou "cognition" e "quality of life" ou "outcome assessment". RESULTADOS: Dos 27 artigos selecionados que estudavam a relação entre cognição e QV medida por meio de escalas, 20 mostraram associações. Essas foram mais importantes nos estudos que utilizaram escalas de QV administrada por pesquisador, em comparação com escalas de autoavaliação. CONCLUSÃO: A associação entre cognição e QV é mais evidente nos estudos que utilizaram medidas de QV feitas por pesquisadores do que naqueles que utilizaram autoavaliações. A avaliação cognitiva é importante não somente como parâmetro determinante de QV dos pacientes, mas também auxilia na interpretação das escalas de QV, sendo as autoavaliações mais adequadas em pacientes com menor prejuízo cognitivo. BACKGROUND: The Quality of Life (QoL) is a valuable way of assessing the impact of diseases in daily life and favors more effective interventions under a biopsychosocial perspective. OBJECTIVES: This article aimed to critically analyze the available data on the relationship between QoL and cognitive function in schizophrenia. METHODS: We performed a search in PubMed and Lilacs databases with the terms "schizophrenia" and "cognition disorder" or "cognition" and "quality of life" or "outcome assessment". RESULTS: From the 27 selected articles that studied the relationship between cognition and QoL measured by scales, 20 articles showed associations. The association was more robust in studies using scales administered by a researcher, compared to self-report scales. DISCUSSION: The cognitive assessment is important not only as a parameter for determining QoL of patients, but also as a factor to be taken into account when analyzing the data obtained by self-report QoL scales.

To discuss paroxysmal events that mimic epileptic seizures with emphasis on their semiologic differences and therapeutic perspectives. We did a narrative review of the literature based on selected papers in Medline and Bireme after searching for the uniterms "non-epileptic seizures" and "psychogenic seizures". Nonepileptic seizures (NES) can be classified in physiological (syncope, migraine, transitory ischemic attack) and psychogenic (voluntary or involuntary). The gold-standard in the differentiation of epileptic and nonepileptic seizures is the video-electroencefalogram, but many semiologic features can contribute to this process. The treatment of NES is based mainly on psychotherapy and pharmacotherapy directed to comorbid psychiatric disorders. Despite the high prevalence and elevated morbidity of the NES, there are just few studies on this subject in the Brazilian literature. There are many diagnostic and therapeutic challenges related to NES. Hence, psychiatrists should be aware of the occurrence of NES, avoiding iatrogenic interventions such as the unnecessary administration of anti-epileptic drugs.

CONTEXTO: Uma síndrome amnésica clássica caracteriza-se por evidente prejuízo da memória anterógrada, variável e temporária amnésia retrógrada, sendo as formas não-declarativas da memória poupadas. Entretanto, publicações recentes relataram casos de prejuízo desproporcional da memória retrógrada em relação à anterógrada. OBJETIVOS: Relatar o caso de um paciente de 26 anos de idade com um quadro grave de amnésia retrógrada, aparentemente sem fatores desencadeantes. MÉTODOS: Entrevista psiquiátrica e avaliação neuropsicológica. RESULTADOS: A perda de memória do paciente se estendia por toda sua vida, mas ele era capaz de adquirir e reter novas informações. Ele também apresentava prejuízos na produção e na compreensão de palavras, assim como no reconhecimento e no uso de objetos. CONCLUSÃO: A formulação diagnóstica final do caso é difícil, apontando possivelmente o contínuo existente entre a amnésia retrógrada psicogênica e a orgânica. BACKGROUND: A classic amnestic syndrome is characterized by a significant impairment of the anterograde memory, a variable and transitory retrograde amnesia with preserved non-declarative memory. However, case reports of patients with disproportionate compromise of the retrograde memory have been described in the recent literature. OBJECTIVES: To report a 26-year-old patient with a severe global retrograde amnesia with no evident triggering factor. METHODS: Psychiatric interview and neuropsychological evaluation. RESULTS: His memory loss compromised all domains of his life, although he could acquire and retain new information. He also exhibited prominent deficits in production and comprehension of common words as well as in recognition and use of objects. DISCUSSION: The final diagnostic formulation of the present case is difficult possibly indicating a continuum between psychogenic and organic retrograde amnesia.

Historically, measures of everyday functioning have focused exclusively on real-world performance. Despite the unquestionable value of “real-world functioning”, it has become clear that instruments for its assessment might not be as accurate as desirable. Functional capacity is a domain of everyday functioning that can be assessed through performance-based measures. In the last decade, functional capacity has become a cornerstone for the assessment of everyday functioning, since, alongside measures of real-world functioning, it provides a much more comprehensive picture of functional outcomes than any measurement alone. Functional capacity is more stable and less vulnerable to influence from environmental factors than other domains, and its correlation with cognitive functions has encouraged the Measurement and Treatment Research to Improve Cognition in Schizophrenia (MATRICS) project to suggest that a performance-based measure of functional capacity be included as a co-primary assessment of cognition in clinical trials. Functional capacity assessment instruments may be also useful in the evaluation of remission in schizophrenia. Validation of these instruments in different countries is desirable, and should always include cross-cultural adaptation; within large countries, adjustment for regional variations should be considered.

OBJETIVO: O uso de modelos experimentais tem permitido importantes avanços no diagnóstico e na terapêutica de doenças somáticas, tais como diabetes e hipertensão. No caso da esquizofrenia, entretanto, as tentativas de modelos experimentais causaram, historicamente, pouco impacto e algum ceticismo. Estudos mais recentes, contudo, indicam que a Ciência Cognitiva aplicada ao uso de modelos pode nos ajudar na compreensão da fisiopatologia da esquizofrenia. O estudo objetivou realizar uma revisão crítica dos modelos experimentais propostos para a esquizofrenia. RESULTADOS E DISCUSSÃO: As dificuldades próprias dos modelos de esquizofrenia são a subjetividade dos sintomas, a dificuldade em reproduzi-los em animais e a complexidade clínica a ser totalizada. Fenótipo tão complexo só pode ser abordado pela separação de seus componentes (endofenótipos) e pela respectiva manipulação de seus correlatos experimentais, feita por intervenções específicas (e.g. farmacológicas, cirúrgicas, genéticas) na busca de um mecanismo comum para estes endofenótipos. A correlação entre resultados e sintomas deve apoiar-se em hipótese explanatória abrangente. Até o presente, a doença parece envolver desconexão neuronal difusa decorrente de anormalidades cerebrais sutis, de causa genética e/ou ambiental. CONCLUSÕES: A integração da informação dos modelos atualmente em uso pode contribuir de modo significativo para a compreensão da esquizofrenia. OBJECTIVE: Diagnostic and therapy of somatic diseases like diabetes and hypertension have improved notably with the use of experimental models. For schizophrenia the proposal of a model has made little impact and even scepticism. Nevertheless the most recent studies indicate that "Cognitive Sciences" applied to specific models may help us to find out mechanisms of the disease. This article reviews the models presently under investigation for schizophrenia. RESULTS AND DISCUSSION: The difficulty to model schizophrenia results from the subjectivity of its symptoms, the difficult to reproduce them in animals and the disease complexity. Research on such a complex phenotype can only proceed by separating its components (endophenotypes) from each other and by the respective manipulation of its experimental counterparts, made by specific interventions (e.g. pharmacological, surgical, genetic), in the search of a common mechanism leading to these endophenotypes. For integrating these findings with symptoms a global explanatory theory is required. So far, the disease seems to result from a diffuse neuronal disconnection as a consequence of minor brain abnormalities with a genetic and/or environmental cause. CONCLUSIONS: An integrative approach of the diversity of models presently used may improve our understanding of schizophrenia.

Relatamos o caso de uma paciente com diagnóstico de esquizofrenia paranóide que desenvolveu dois quadros de hepatotoxicidade atribuídos a efeitos colaterais do ácido valpróico e da clozapina. Essa relação foi estabelecida pela proximidade temporal entre o início da medicação e a instalação do quadro, pela resolução deste após a suspensão da medicação e pela exclusão de outras causas de doença hepática. No seguimento, o controle clínico da paciente foi realizado com eletroconvulsoterapia (ECT). Salientamos a importância da vigilância cuidadosa dos efeitos adversos de psicofármacos, especialmente sobre a função hepática, e da ECT como possibilidade terapêutica diante de limitações farmacológicas.

O termo ecstasy é usado para descrever diversas substâncias que compartilham estruturas químicas e efeitos semelhantes, referindo-se mais comumente a 3,4-metilenodioximetanfetamina (3,4-MDMA). Os efeitos psíquicos da MDMA são, sobretudo, alucinógenos e estimulantes. A tendência atual considera o delírio de Cotard como sendo a crença delirante de estar morto ou de que seus órgãos estejam paralisados ou podres, independentemente do diagnóstico do paciente. Neste artigo, relatamos o caso clínico de um paciente que apresentou quadro psicótico com delírios hipocondríacos e alucinações olfativas com características de síndrome de Cotard associado ao uso crônico de ecstasy. Foi medicado com olanzapina e obteve remissão completa dos sintomas.

Animal models of psychiatric disorders are a challenging but highly relevant issue. Most psychiatric disorders are very heterogeneous syndromes, resulting from multiple and varied causal factors and characterized by symptoms that can only be inferred with significant limitations in non-human models. As constructing a model that reproduces a whole psychiatric syndrome seems virtually impossible, researchers have tried to focus on endophenotypes, i.e., discrete traits that are more proximal to predisposing genes than the whole syndrome. These can be explored in a wide range of approaches, such as in pharmacological, lesion, and environmental models. Another challenge is to understand how genes interact with environmental factors over time to result in the syndromic phenotype. A better understanding of the subcellular mechanisms that enhance or allow brain resistance to environmental influences is required, as is a global thesis compatible with the diversity of diseases sharing similar behavioral and biological traits. With an experimental inventory of the possible causes of minor developmental failures, we may systematically explore their consequences in the adult animal and be able to decide if this will enlighten the understanding of one or another psychiatric disease.

Schizophrenia is a chronic psychiatric syndrome characterized by generalized cognitive deficits that are associated with functional impairment. The endocannabinoid system (ECS) modulates neurotransmission and neuronal plasticity and is important for cognitive functioning. Evidence points to the involvement of this neuromodulatory system in the pathophysiology of schizophrenia and that alteration of the ECS on peripheral lymphocytes could reflect central changes. The objective of this study was to compare levels of peripheral endocannabinoid receptor expression in patients with schizophrenia and healthy subjects and find evidence of association between peripheral expression of those receptors and cognitive performance. Patients with stabilized schizophrenia (N=53) and controls (N=22) underwent clinical and cognitive evaluation, and assessment of cannabinoid receptor expression on the surface of peripheral immune cells (lymphocytes, natural killer cells and monocytes) by flow cytometry. Patients with schizophrenia had lower levels of cannabinoid receptor expression on total T lymphocytes, but after controlling for possible confounders this difference did not remain significant. In patients, increased cannabinoid receptor expression on lymphocytes and monocytes was significantly correlated with worst cognitive performance. These data provide additional evidence of the involvement of the ECS in the pathophysiology of cognitive deficits in schizophrenia.

OBJECTIVE: Schizophrenia is a psychiatric disorder whose mechanisms have remained only partially elucidated. The current proposals regarding its biological basis, such as the dopaminergic hypothesis, do not fully explain the diversity of its symptoms, indicating that other processes may be involved. This paper aims to review evidence supporting the involvement of the endocannabinoid system (ECS), a neurotransmitter group that is the target of Cannabis sativa compounds, in this disorder. METHODS: A systematic review of original papers, published in English, indexed in PubMed up to April, 2012. RESULTS: Most studies employed genetics and histological, neuroimaging or neurochemical methods - either in vivo or post-mortem - to investigate whether components of the ECS are compromised in patients. Overall, the data show changes in cannabinoid receptors in certain brain regions as well as altered levels in endocannabinoid levels in cerebrospinal fluid and/or blood. CONCLUSIONS: Although a dysfunction of the ECS has been described, results are not entirely consistent across studies. Further data are warrant to better define a role of this system in schizophrenia. OBJETIVO: A esquizofrenia é um transtorno psiquiátrico cujos mecanismos permanecem apenas parcialmente elucidados. As atuais propostas relativas à base biológica, tais como a hipótese dopaminérgica, não explicam por completo a diversidade de seus sintomas, o que indica que outros processos podem estar envolvidos. Este artigo tem como objetivo revisar indícios que sustentem o envolvimento do sistema endocanabinoide (SECB), um grupo de neurotransmissoresalvo dos compostos da Cannabis sativa, nesse transtorno. MÉTODOS: Revisão sistemática dos artigos originais, publicados em inglês e indexados no PubMed até abril de 2012. RESULTADOS: A maioria dos estudos empregou métodos neuroquímicos ou de neuroimagem genéticos e histológicos - tanto in vivo quanto post-mortem - para investigar se os componentes do SECB estão comprometidos nos pacientes. De modo geral, os dados mostram mudanças nos receptores canabinoides em determinadas regiões cerebrais, bem como a alteração dos níveis de endocanabinoides no líquido cefalorraquidiano e/ou no sangue. CONCLUSÕES: Ainda que a disfunção do SECB tenha sido descrita, os resultados dos estudos não são totalmente consistentes. São necessários mais dados para definir melhor o papel desse sistema na esquizofrenia.

Interview-based scales can be used as coprimary measures to complement the assessment of cognitive impairment in schizophrenia. One major question that arises from the use of such tools is how specific they are in relation to other psychopathological domains. We analyse the specificity of the Positive and Negative Syndrome Scale (PANSS) negative subscale and the Schizophrenia Cognition Rating Scale (SCoRS).We performed a principal component analysis (PCA) of PANSS negative subscale, rated by the interviewer, and SCoRS ratings from three different sources (patient, informant, and interviewer) in 101 patients with schizophrenia. Additionally, we correlated mean SCoRS ratings to PANSS negative subscale items to determine whether any PANSS item is particularly related to cognition.The PCA showed that the two first components, which explained approximately 40% of the total variance of the scales, represent the SCoRS ratings and the PANSS negative subscale ratings, respectively. The mean interviewer SCoRS was significantly correlated with the PANSS negative Item 5 (difficulty in abstract thinking) and with the mean PANSS negative subscale. The latter correlation was no longer significant when "difficulty in abstract thinking" was eliminated from PANSS negative subscale.In general, SCoRS and PANSS negative subscale scores address different constructs; however, the PANSS negative item "difficulty in abstract thinking" seems to address a cognitive dimension.

O objetivo deste trabalho é discutir o significado dos sinais neurológicos sutis e a relevância para a pesquisa em psiquiatria, com ênfase na esquizofrenia e no transtorno bipolar (TB). Realizou-se uma revisão da literatura nas bases de dados Medline e Bireme. Sinais neurológicos sutis são alterações no exame neurológico que compreendem funções diversas como integração sensorial, coordenação motora, sequenciamento motor e presença de reflexos primitivos. Esses sinais indicam disfunção cerebral não focal, podendo se apresentar como fatores de risco para transtornos psiquiátricos. Podem indicar endofenótipos relacionados a disfunções em circuitos neurais específicos, fornecendo informações relevantes para fisiopatologia desses transtornos. Apesar disso, há poucos trabalhos sobre o tema na literatura nacional. A observação de sinais neurológicos sutis aponta para o potencial de o exame neurológico preencher uma lacuna entre a pesquisa neurobiológica e a prática clínica. This work aims to discuss the meaning of neurological soft signs and their relevance for research in psychiatry, with emphasis in schizophrenia and bipolar disorder. A narrative review of the literature was performed based on the databases Medline and Bireme. Neurological soft signs are abnormalities on the neurological exam affecting several functions, such as sensory integration, motor coordination, motor sequencing and presence of primitive reflexes. These signs indicate non-focal brain dysfunction and can be regarded as risk factors for psychiatric disorders. They can also suggest endophenotypes related with specific neural circuits, providing relevant information on the pathofisiology of these disorders. There are only a few studies on this subject in the Brazilian literature. The observation of neurological soft signs points to the potential of the neurological exam to link the neurobiological knowledge and the clinical practice.

OBJECTIVE: Poor impulse control is thought to be one of the characteristics of alcohol addiction. The capacity to remain abstinent may be linked to cognitive bias related to three dimensions of impulsivity: motor, non-planning, and attentional impulsivity. The aim of this study was to evaluate the neuropsychological profile related to these impulsivity dimensions in alcohol-dependent patients within 15 -120 days of abstinence. METHOD: We compared 31 alcohol-dependent patients to 30 matched healthy controls regarding their performances on the Continuous Performance Task, the Iowa Gambling Test, and the Wisconsin Card Sorting Test, each of which is thought to tax primarily one of the three dimensions of impulsivity just outlined. RESULTS: When compared to controls, alcohol-dependent patients presented more commission errors on the Continuous Performance Task; made more disadvantageous choices on the Iowa Gambling Test; and made more perseverative errors on the Wisconsin Card Sorting Test. There was no significant correlation between performance on these tests and the length of abstinence. CONCLUSIONS: These results suggest that deficits related to motor, non-planning and attentional components of impulsivity exist in alcohol-dependent patients, in the period immediately after acute alcohol withdrawal. These results may help guide interventions designed to prevent the risk of relapse in alcohol-abstinent patients. OBJETIVO: O controle deficiente dos impulsos é considerado uma das características da dependência do álcool. A capacidade de permanecer abstinente pode estar ligada a viés cognitivo relacionado a três dimensões da impulsividade: motora, de atenção e por falta de planejamento. O presente estudo objetivou avaliar o perfil neuropsicológico relacionado a estas dimensões da impulsividade em pacientes dependentes de álcool em 15 a 120 dias de abstinência. MÉTODO: Nós comparamos o desempenho de 31 pacientes dependentes de álcool a 30 controles saudáveis na Continuous Performance Task, no Iowa Gambling Test e no Wisconsin Card Sorting Test, que são considerados testes capazes de avaliar primariamente as citadas dimensões de impulsividade. RESULTADOS: Em relação aos controles, o grupo pacientes dependentes de álcool cometeu mais erros de comissão na Continuous Performance Task; fez escolhas menos vantajosas no Iowa Gambling Test e mais erros perseverativos no Wisconsin Card Sorting Test. Não houve correlação significativa entre o desempenho nestes testes e o tempo de abstinência. CONCLUSÕES: Estes resultados sugerem que há déficits relacionados aos componentes da impulsividade (motores, atencionais e por não-planejamento) em pacientes dependentes de álcool, no período imediatamente após a fase aguda de retirada do álcool. Estes resultados podem ajudar a guiar intervenções para impedir o risco do recaída em pacientes em curto período de abstinência de álcool.

Na classificação de Gralnick, de 1942, a folie à deux é dividida em quatro subtipos básicos: folie imposée, folie communiquée, folie simultannée e folie induite. O último termo se refere à adição de novas idéias delirantes por paciente previamente psicótico sob influência de outro paciente. Relatamos um caso de folie induite ocorrido entre mãe e filho primariamente psicóticos (transtorno delirante e esquizofrenia, respectivamente), que passaram a compartilhar delírios de natureza persecutória e sexual. Casos de folie à deux geralmente correspondem ao transtorno psicótico induzido (DSM-IV-TR) ou transtorno delirante induzido (CID-10). No entanto, o paciente deste relato não pode receber tais diagnósticos, já que nos critérios validados atualmente percebe-se a exigência de que o parceiro induzido não possua um transtorno psicótico anterior ao início do compartilhamento do delírio. Consideramos que o presente relato exemplifica a insuficiência das modernas classificações nesta área. Casos de folie induite podem necessitar de manejo distinto dos demais casos de psicose compartilhada ou não compartilhada. Sugerimos que os critérios diagnósticos correntes poderiam ser revisados para incluir condições psiquiátricas como essa. In the Gralnick classification of 1942 the term folie à deux is classically divided into four basic subtypes: folie imposée, folie communiquée, folie simultannée and folie induite. This last one refers to the addiction of new delusions in a previous psychotic patient, under the influence of another patient. We report a case of folie induite that occurred between mother and son, both diagnosed previously with psychotic disorders (delusional disorder and schizophrenia, respectively), who started to share persecutory and sexual delusions. Cases of folie à deux generally correspond to the induced psychotic disorder in the DSM-IV-TR or the induced delusional disorder in the ICD-10. However, our patient do not meet neither diagnosis, since, in both, it is necessary that the induced delusional patient do not have any previous psychotic disorder. We think that our report underscores the insufficiency of the modern classifications in this area. Cases of folie induite may require a management different from those used in other cases of shared or non-shared psychosis. We suggest that modern criteria should be revised to include cases such as the present one.

OBJECTIVE: The Rey Auditory-Verbal Learning Test is a widely recognized test in neuropsychological literature to evaluate learning and memory. This paper presents the performance of six age groups of Brazilian elderly on the Rey Auditory-Verbal Learning Test. METHOD: A version of the test was developed with a list of high-frequency one-syllable and two-syllable concrete Portuguese substantives. Two hundred and twenty-three subjects of both genders were allocated to 6 age groups (60-64, 65-69; 70-74; 75-79; 80-84 and 85-89 years old) and tested with the Rey Auditory-Verbal Learning Test. RESULTS: Educational level and age had a positive and a negative correlation, respectively, with performance on the Rey Auditory-Verbal Learning Test. Women performed significantly better than men. Our results were similar to those found for the Rey Auditory-Verbal Learning Test English version, across similar age ranges. CONCLUSION: Our results suggest that the Brazilian Portuguese Rey Auditory-Verbal Learning Test adaptation was adequate and applicable for evaluating the memory capacity of Brazilian subjects, across similar age and educational levels. OBJETIVO: O teste de aprendizagem auditivo-verbal de Rey é um teste mundialmente reconhecido na literatura neuropsicológica que avalia aprendizagem e memória. Este trabalho apresenta a performance de seis grupos de idosos brasileiros (agrupados em faixas etárias distintas) no teste de aprendizagem auditivo-verbal de Rey. MÉTODO: A versão utilizada do teste foi desenvolvida com uma lista de substantivos concretos com uma ou duas sílabas muito freqüentes na língua portuguesa falada no Brasil. Duzentos e vinte e três sujeitos de ambos os sexos foram alocados em seis grupos de acordo com a idade (60-64, 65-69; 70-74; 75-79; 80-84 e 85-89 anos) e submetidos ao teste de aprendizagem auditivo-verbal de Rey. RESULTADOS: O nível educacional e a idade tiveram correlação positiva e negativa, respectivamente, com a performance no teste de aprendizagem auditivo-verbal de Rey. Mulheres desempenharam o teste significativamente melhor que os homens. Nossos resultados são semelhantes aos encontrados na versão inglesa do teste de aprendizagem auditivo-verbal de Rey para sujeitos com idade semelhante. CONCLUSÃO: Nossos resultados sugerem que a adaptação brasileira do teste de aprendizagem auditivo-verbal de Rey é adequada e aplicável para avaliação da memória em sujeitos brasileiros de mesma idade e nível educacional.

A demência fronto-temporal é uma importante causa de demência no período pré-senil. Caracteriza-se por significativas modificações do comportamento e da personalidade, enquanto o funcionamento cognitivo avaliado por testes psicométricos tradicionais encontra-se relativamente preservado. Muitos pacientes buscam o psiquiatra em virtude dos sintomas comportamentais proeminentes, como apatia, desinibição e comportamentos perseverativos ou estereotipados. O tratamento racional da demência fronto-temporal é atualmente limitado. Os sintomas comportamentais são controlados principalmente por inibidores seletivos da recaptação de serotonina. Frontotemporal dementia is a major cause of dementia in the presenium. It is characterized by significant changes in behavior and personality, while cognitive functioning as assessed by traditional psychometric tests is relatively preserved. Thus, many patients present to the psychiatrist because of the prominence of behavioral symptoms, such as apathy, disinhibition, perseverative or stereotyped behaviors. Rational treatment for frontotemporal dementia is currently limited. The behavioral symptoms are controlled mainly with selective serotonin reuptake inhibitors. La demencia frontotemporal es una importante causa de demencia en el período presenil de la vida. Se caracteriza por alteraciones significativas en el comportamiento y en la personalidad, mientras la función cognitiva evaluada por pruebas psicométricas convencionales resulta relativamente preservada. Muchos pacientes recurren al psiquiatra en función de síntomas comportamentales sobresalientes como apatía, desinhibición y comportamientos perseverantes o estereotipados. El tratamiento racional de la demencia frontotemporal aún se encuentra bastante limitado. Los síntomas comportamentales se controlan principalmente con inhibidores selectivos de la recaptación de serotonina.

RESUMO Objetivo Realizar a tradução e a adaptação cultural da Escala de Despersonalização de Cambridge (EDC) para o português brasileiro. Métodos Foi realizada a tradução da escala em duas versões, seguida por retrotradução por nativo de língua inglesa. Os itens da escala foram comparados e adaptados para o contexto brasileiro e, então, transformados em uma versão-teste, que foi aplicada em 30 participantes sem diagnóstico de transtorno psiquiátrico. Resultados A versão em português da EDC mostrou boa compreensibilidade. A maior parte dos participantes relatou experiências transitórias ligadas à despersonalização nos últimos seis meses (53,3% dos casos), incluindo a sensação de ser um observador externo de si mesmo (35%) e sensação de estar fora do corpo (10%). Conclusões O presente estudo confirma a alta frequência de experiências de despersonalização. Serão necessários estudos para verificação das propriedades psicométricas da escala.

The UCSD Performance-based Skills Assessment (UPSA) is a measure of Functional Capacity and assesses skills involved in community tasks. It has good psychometrics properties, and is currently recommended as a co-primary assessment of cognition in the MATRICS Project. To our knowledge so far, there are no studies in western developing countries concerning Functional Capacity in Schizophrenia. The aims of this study were to translate, culturally adapt and validate the UPSA to assess Functional Capacity in community-dwelling patients with Schizophrenia living in Brazil. Eighty-two subjects (52 patients, 30 controls) were evaluated using: the Brazilian version of the UPSA (UPSA-1-BR), PANSS, Personal and Social Performance (PSP) and Global Assessment of Functioning (GAF). In the reliability test, UPSA-1-BR showed good Internal Consistency (Cronbach’s alpha of 0.88) and strong correlation between test and retest (4-month gap; r = 0.91; p < 0.01). Spearman’s rho values showed a moderate correlation between UPSA-1-BR and both PSP (0.50; p < 0.01) and GAF (0.46; p < 0.01) scores. UPSA-1-BR is capable of differentiating people with and without Schizophrenia. Patients scored lower than controls (58.9 versus 79.1), with an AUC of 0.79 (95%IC: 0.69–0.89). Sensitivity and specificity values of 0.71 and 0.70, respectively, were found in the cut-off point of 73.5, for separation of patients and controls, with predictive values of 80% (positive) and 58% (negative). UPSA-B-BR was also evaluated. UPSA-1-BR and its brief version presented adequate psychometric properties and proved to be valid and reliable instruments in the assessment of Functional Capacity in subjects with Schizophrenia.

Background: Toxoplasma gondii (T. gondii) infection has been identified as a risk factor for schizophrenia. Objectives: Herein, we sought to evaluate the association between T. gondii infection and clinical symptoms and quality of life in patients with schizophrenia. Methods: We conducted a cross-sectional study with 48 patients with chronic schizophrenia and 40 controls. Peripheral blood was drawn, and IgM and IgG anti-T. gondii antibodies were evaluated by Enzyme-Linked Immunosorbent Assay (ELISA). Depressive, positive and negative symptoms were assessed, respectively, by the Calgary Depression Scale (CDS) and the Positive and Negative Syndrome Scale (PANSS). Cognitive performance was assessed in patients by the Brazilian version of the Schizophrenia Cognition Rating Scale (SCoRS-BR). Quality of life was assessed by the Brazilian version of the Quality of Life in Schizophrenia scale (QLS-BR). Results: The prevalence and titers of T. gondii IgM and IgG antibodies did not differ between patients and controls. The positive serology for T. gondii IgG antibodies was not associated with illness symptoms, cognitive performance, depressive symptoms or quality of life. Discussion: Our findings suggest that toxoplasmosis infection is not associated with severity of symptoms, quality of life, cognitive or depressive symptoms in schizophrenia patients.

We describe a case that illustrates the particularities of DPD and proposals for treatment.M., 27, single, with a university degree, sought psychi -atric treatment with complaints that she "did not feel her body". She reported that she felt strange and empty, that her body seemed to be somewhere else and hollow, with nothing but the skin, and it seemed to be someone else's body. She had come to the point of wearing numerous brace -lets to mark the boundaries of her own limbs. She also suf -fered from affective detachment, frequently stating “I feel like I was dead” or “I feel nothing”, but complained of in -tense anxiety in social situations. Reality testing was intact. Slightly depressed mood and mitigated panic-like symp-toms were also identified; however, she did not fulfill the criteria for any other DSM axis I disorder, as confirmed by the MINI-plus. There were no comorbid conditions or history of drug abuse. She had a normal neurological examination, and an EEG showed no abnormalities.As the patient did not respond to risperidone 2 mg/day, motrigine may also be beneficial, albeit partially, in female it was replaced with a selective serotonin reuptake inhibitor (SSRI), which led to anxiety improvement, but the specific symptoms of DPD grew worse. A subsequent change to ven -lafaxine 225 mg/day led to a significant mood improvement and a reduction in panic-like episodes; however, deperson -alization and derealization remained unchanged. Lamotrigine was then added at an initial dose of 25 mg/day, with a gradual increase to up to 200 mg/day. The patient had significant improvement in different aspects, such as affect, interpersonal contact and social interac-tion. The depersonalization symptoms gradually decreased, which made it possible for the patient to go back to work. During that period, a Portuguese version of the Cambridge Depersonalization Scale (CDS) was used.

Introduction Cognitive impairment is a core feature of schizophrenia, associated with significant social and functional impairment. Schizophrenia has also been associated with inflammation and immune changes. Recently it has been showed that IL-6, a pro-inflammatory cytokine, was associated with cognitive impairment. Objectives The aim of this study was to assess the associations between a pro-inflammatory cytokines (IL-6) and an anti-inflammatory cytokine (IL-4) with social cognition in a sample of stable patients with schizophrenia. Methods Social cognition was assessed using the Face Emotion Identification Task (FEIT) and the Hinting Task in 29 stable outpatients with schizophrenia. Blood was collected and cytokines (IL-4, IL-6) assays were realized (CBA method). Results IL-6 levels showed a significant inverse correlation with FEIT global score (p = 0.019, rho = -0.475) and a trend of significant inverse correlation with the Hinting Task (p = 0.079, rho = -0.332). However, IL-4 levels showed no significant association with the FEIT or the Hinting task (rho = 0.079 and 0,121; respectively). Conclusions Elevated IL-6 levels seem to play a role in social cognitive impairment in stable patients with schizophrenia. These results should be interpreted with caution due to the limited number of participants in the study.
