Background: Providing adequate information and involving patients in treatment have become an essential component of mental health care. Despite this, research regarding the extent to which this need has been met in clinical services is still scarce. Aims: To investigate the need for information about psychiatric condition and treatment among outpatients with schizophrenia disorders and how this need is associated with service use, adjusting for sociodemographic and clinical characteristics. Methods: Need for information about condition and treatment, using the corresponding domain in the Camberwell Assessment of Need (CAN), in a representative sample of 401 schizophrenia outpatients in Santos, Brazil was assessed. Hierarchical logistic regression was used to investigate the association of information as a reported need and as an unmet need with service use variables, adjusting for sociodemographic and clinical characteristics. Results: Need for information was reported by 214 (53.4%) patients, being met in 101 (25.2%) and unmet in 113 (28.2%). Hierarchical regression indicated a significant association of a reported need with higher age of onset, family monitoring medication use last year and lower education level, which was only associated with an unmet need. Conclusion: Information was a commonly reported need and which was often unmet, showing no significant association with service use. Greater attention should be given by mental health services to information provision.

The affective burden of psychotic disorder has been increasingly recognised. However, subjective… reports of distress and its covariates, especially those related to service use, remain under-investigated in patients with psychosis. This study investigated subjective distress and its covariates in a representative sample of 401 outpatients with a confirmed diagnosis of psychotic disorders in Brazil. Distress was assessed using the corresponding domain of a standardised measure of need - the Camberwell Assessment of Need. Distress was reported as a need by 165 (41%) patients, being met in 78 (20%) and unmet in 87 (22%). Hierarchical logistic regression showed that the presence of distress as a need was predicted by attendance at psychotherapy (OR=3.49, CI=1.62-7.53), presence of suicidal ideation (OR=2.89, CI=1.75-4.79), non-attendance at psychosocial rehabilitation (OR=2.84, CI=1.31-6.19), and higher psychopathology (OR=1.09, CI=1.06-1.12). An unmet need was predicted by family not accompanying patients to treatment (OR=2.60, CI=1.05-6.44) and higher psychopathology (OR=1.05, CI=1.02-1.09). The use of a cross-sectional design and a single questionnaire domain to evaluate distress are the main limitations. Subjective distress is a common unmet need in psychosis, and can be treated. The main clinical implication is that subjective distress in psychosis may be impacted on by family engagement and psychosocial interventions

Objective The purpose of this research was to make a cross-cultural adaptation of the Medication Adherence Rating Scale (MARS) for psychiatric patients to the Brazilian context. Methods The procedure consisted of four phases: translation of the original scale, back-translation, review by an Expert Committee and Pre-test study with a patients’ sample. Results The Expert Committee corrected the items’ translation when necessary and modified the scale administration format and its instructions from self-report to face-to-face interview form in order to ensure easy understanding by the target population. During Pre-test, the instructions and most of the items were properly understood by patients, with the exception of three of them which had to be changed in order to ensure better understanding. The Pre-test sample was composed by 30 psychiatric patients, with severe and persistent disorders mainly single (46.7%), female (60.0%), with a mean age of 43.8 years old and an average of five years of education. Conclusion The Brazilian version of MARS scale is now adapted to the Brazilian Portuguese language and culture and is easily understood by the psychiatric target population. It is necessary to do further research to evaluate the scale psychometric qualities of validity and reliability in order to use it in Brazil. Objetivo Esta pesquisa teve como objetivo fazer a adaptação transcultural do instrumento Medication Adherence Rating Scale (MARS) para pacientes psiquiátricos no contexto brasileiro. Métodos O procedimento incluiu quatro fases: tradução da escala original, retradução, revisão por Comissão de Especialistas e Pré-teste com uma amostra de pacientes. Resultados A Comissão de Especialistas adequou a redação dos itens e das instruções e mudou a forma de aplicação da escala, que passou de autoaplicação para o formato de entrevista, visando à maior facilidade de compreensão pela população-alvo. No Pré-teste, as instruções e a maioria dos itens foram bem compreendidas, com exceção de três questões que apresentaram palavras que suscitaram dúvidas nos participantes, tendo sido modificadas para melhor compreensão. A amostra do pré-teste foi composta de 30 pacientes psiquiátricos, com transtornos graves e persistentes, majoritariamente solteiros (46,7%), do sexo feminino (60,0%) com idade média de 43,8 anos e cinco anos de escolaridade, em média. Conclusão A versão brasileira da escala MARS constitui um instrumento adaptado para o contexto linguístico e cultural brasileiro e de fácil compreensão pela população-alvo de pacientes psiquiátricos. Estudos futuros deverão investigar as qualidades psicométricas de validade e confiabilidade desta escala para que ela possa ser utilizada no contexto brasileiro.

Objetivo Comparar os graus das sobrecargas objetiva e subjetiva sentidas por familiares cuidadores de pacientes com esquizofrenia e por familiares cuidadores de pacientes com depressão maior, bem como os fatores associados e as dimensões mais afetadas em cada grupo. Métodos Participaram desta pesquisa 50 cuidadores de pacientes com esquizofrenia e 50 cuidadores de pacientes com depressão maior. Esses familiares participaram de uma entrevista estruturada, na qual foram aplicados dois instrumentos: a escala de sobrecarga FBIS-BR e um questionário. Resultados Os resultados indicaram que os dois grupos apresentavam diferenças significativas quanto ao grau de sobrecarga, na análise detalhada dos itens da escala. Os familiares cuidadores de pacientes com esquizofrenia apresentaram sobrecarga objetiva significativamente mais elevada ao assistir o paciente na tomada de medicamentos e na administração do dinheiro e apresentaram maior sentimento de peso financeiro resultante do papel de cuidador. Os cuidadores de pacientes com depressão maior apresentaram maior frequência de supervisão de comportamentos autoagressivos, mais preocupação com a vida social dos pacientes e maior sentimento de incômodo nas tarefas de assistência na vida cotidiana. Não foram encontrados dados significativos referentes aos escores globais da escala de sobrecarga. Conclusão As diferenças encontradas nesta pesquisa apontam para a necessidade de os serviços de saúde mental planejarem intervenções específicas para cada grupo de cuidadores. Objective This study aimed to compare the degree of objective and subjective burden felt by family caregivers of patients with schizophrenia and by family caregivers of patients with major depression, as well as the associated factors and life dimensions most affected in each group. Methods Participated in this study 50 family caregivers of patients with schizophrenia and 50 family caregivers of patients with major depression. These subjects participated in a structured interview for the application of the FBIS-BR burden scale and a sociodemographic and clinical questionnaire. Results The results indicated significant differences in the degree of burden of the two family caregivers groups, in the analysis of specific scale items. The family caregivers of patients with schizophrenia showed significantly higher objective burden in the subscale “Assistance in Everyday Life”, particularly in the tasks related to medication administration and financial management, and they also had more feeling of financial burden. Caregivers of patients with major depression had a higher frequency of self-injurious behaviors supervision, more concern with the social life of patients and a greater feeling of discomfort in the service tasks in everyday life. No difference was observed between groups regarding the scale global scores. Conclusion The differences found in this study pointed out the need for mental health services to plan specific interventions for each group of family caregivers.

6. Efficacy of psychoeducation on symptomatic and functional recovery in bipolar disorder

Objective: To evaluate the efficacy of psychoeducation in the symptomatic and functional recovery, and quality of life (QoL) in a sample of patients with bipolar disorder (BD). Method: The sample comprised 55 patients with BD I and II in remission (Young Mania Rating Scale ≤6 and Hamilton Depression Rating Scale ≤7). Out-patients were matched assigned to receive 16 sessions of psychoeducation [experimental group (EG)] or 16 sessions of placebo without psychoeducation [control group (CG)]. Groups were evaluated at study baseline, midpoint, endpoint, and at 6- and 12-month follow-ups. Results: No significant differences between the groups were found for the variables evaluated (mood symptoms, functioning and QoL), except for overall clinical improvement, subjectively perceived by EG subjects. Both groups showed a trend toward improved clinical global impression and QoL (environmental). No reduction in mood symptoms or improvement in psychosocial functioning was observed. Psychosocial treatment compliance was positively correlated with global functioning, social adjustment, sociability, and global clinical impression. Conclusion: Sixteen session psychoeducation seems to be ineffective to prevent mood episodes or improve functioning in a sample of bipolar patients.

7. Escala de Satisfação dos Pacientes com os Serviços de Saúde Mental (SATIS-BR): estudo de validação

OBJETIVO: Fazer um estudo da validade de construto, validade convergente e consistência interna da Escala de Satisfação dos Pacientes com os Serviços de Saúde Mental (SATIS-BR). MÉTODO: Participaram da pesquisa 110 pacientes psiquiátricos, atendidos em cinco serviços públicos de saúde mental do interior de Minas Gerais. A escala foi aplicada em entrevistas individuais estruturadas. A escala possui 12 itens que avaliam a satisfação dos pacientes, com alternativas de resposta dispostas em escala Likert de 5 pontos. Foi também aplicada a Escala de Mudança Percebida (EMP) para a análise de validade convergente da SATIS-BR. RESULTADOS: A análise fatorial pelo método Principal Axis Factoring revelou uma estrutura fatorial de três fatores, que avaliam a satisfação dos pacientes em relação às dimensões: 1. Competência e compreensão da equipe; 2. Ajuda e acolhida; 3. Condições físicas do serviço. A análise pelo coeficiente alfa de Cronbach mostrou boa consistência interna (alfa = 0,88). A análise da validade convergente foi adequada, tendo sido obtida uma correlação de Pearson positiva significativa com a escala EMP, que avalia um construto teoricamente relacionado ao de satisfação (r = 0,41; p < 0,001). CONCLUSÃO: A escala SATIS-BR apresenta qualidades psicométricas adequadas de validade de construto, validade convergente e fidedignidade. OBJECTIVE: To investigate the construct validity, convergent validity and reliability of the Patient Satisfaction with Mental Health Services Scale (SATIS-BR). METHOD: One hundred and ten psychiatric patients attending five public mental health services located in Minas Gerais, Brazil, participated in this research. The scale was applied in individual structured interviews. The scale has 12 items evaluating patients' satisfaction, with response alternatives disposed in a five points Likert scale. In order to evaluate the convergent validity, another scale was applied, the Perceived Improvement Questionnaire (PIQ). RESULTS: Factorial analysis using the Principal Axis Factoring method revealed a 3 factors structure, evaluating satisfaction regarding the following dimensions: 1. Professional competence and understanding; 2. Help from the professional; 3. Service physical conditions. Reliability results showed adequate internal consistency evaluated by Cronbach analysis (alpha = 0,88). Convergent validity analysis showed a positive significant Pearson Correlation coefficient (r = 0,41; p < 0,001) with the PIQ scale, which evaluates the related construct of patient" perceived improvement. CONCLUSION: The SATIS-BR scale has adequate psychometric properties of construct validity, convergent validity and reliability.

OBJETIVOS: O principal objetivo deste estudo foi investigar as variáveis preditoras da satisfação dos pacientes com os serviços de saúde mental. Como objetivo secundário, avaliaram-se os níveis de mudança percebida e de satisfação dos pacientes. MÉTODO: Foi realizado um estudo de corte transversal, do tipo correlacional, com 110 pacientes psiquiátricos atendidos em três serviços públicos de saúde mental. Os pacientes foram entrevistados para aplicação da Escala de Mudança Percebida (EMP) e da Escala de Satisfação dos Pacientes com os Serviços de Saúde Mental (SATIS-BR). RESULTADOS: O principal resultado encontrado foi que a percepção de mudança pelos pacientes constituiu a variável preditora mais importante da satisfação e, em segundo lugar, a maior idade. Foi detectada alta satisfação dos pacientes com os serviços e escores de percepção de mudança diferenciados em relação aos diferentes aspectos dos serviços avaliados. CONCLUSÃO: Este estudo revela a importância da percepção dos próprios pacientes sobre os resultados do tratamento como preditora da satisfação com os serviços. Os construtos de satisfação e percepção de mudança se revelaram importantes na avaliação dos serviços de saúde mental. Estudos longitudinais e com amostras maiores e aleatórias poderão fornecer dados adicionais para a reavaliação desses resultados. Objectives: The main objective of this study was to evaluate the predicting factors of patients' satisfaction with mental health services. The secondary objective was to evaluate patients' levels of perceived changes and satisfaction. METHOD: A cross-sectional correlational study was carried out with 110 patients attending three public mental health services. Patients were interviewed for the application of the Perceived Changes Scale (EMP) and the Patient's Satisfaction with Mental Health Services Scale (SATIS-BR). RESULTS: The main result was that patients' perceived change was the most important predictor of satisfaction with services and, in the second place, the greater age. It was found high levels of patients' satisfaction with mental health services and distinct levels of perceived change. CONCLUSION: This study points to the importance of patients' own perception about treatment outcome as predictor of satisfaction with mental health services. Satisfaction and perceived changes proved to be important in mental health evaluating. Longitudinal, random sampling and greater size sample studies may provide additional data for these results reevaluation.

9. Prevalência de transtornos mentais comuns e fatores associados em uma população assistida por equipes do Programa Saúde da Família

OBJETIVO: Avaliar a prevalência de casos suspeitos de transtornos mentais comuns em uma população assistida por uma equipe do Programa Saúde da Família e investigar os fatores associados à ocorrência dessa morbidade. MÉTODOS: Pesquisa de levantamento, de corte transversal. A amostra, composta por 277 sujeitos, foi obtida por meio do método de amostragem aleatória do tipo sistemática. Para a coleta de dados, foram aplicados o Self Report Questionnaire (SRQ-20) e um questionário sociodemográfico. RESULTADOS: A prevalência global de casos suspeitos de transtornos mentais comuns na população alvo foi de 43,70%. O sexo feminino apresentou uma taxa de prevalência significativamente mais elevada (48,37%), quando comparado ao sexo masculino (34,41%). A variável "renda familiar" apresentou uma relação inversa com a prevalência de transtornos mentais comuns. A análise de regressão logística indicou que apenas as variáveis "uso de medicamento" e "renda familiar" estavam associadas significativamente à suspeita de casos de transtornos mentais comuns. CONCLUSÕES: Os resultados apontaram uma elevada prevalência de transtornos mentais comuns nessa população alvo, comparativamente aos dados de outros estudos semelhantes, indicando a necessidade de um cuidado especial por parte da Atenção Primária à Saúde, em particular para as famílias caracterizadas pelos fatores associados ao transtornos mentais comuns apontados nesta investigação. OBJECTIVE: Investigate the prevalence of suspected cases of common mental disorders in a population assisted by one professional team of the Brazilian Family Health Program as well as to evaluate its associated factors. METHODS: A cross-sectional survey study was carried out using a randomized systematic sampling procedure for selecting 277 subjects from the target population. For data collection, two instruments were administered to the sample, the Self Report Questionnaire (SRQ-20) as well as a sociodemographic questionnaire. RESULTS: The global prevalence of cases suspected of presenting common mental disorders in the target population was of 43.70%. Females had a significantly higher prevalence rate (48.37%), when compared to males (34.41%). The variable "family income" had an inverse relation to the common mental disorders prevalence. The logistic regression analysis indicated that only two variables - "use of medication" and "family income" - were significantly associated to common mental disorders. CONCLUSIONS: The results indicated a higher prevalence of common mental disorders in this target-population, compared to data from other similar studies, indicating the need for a special attention to be rendered by the primary health care system to detect and treat those cases in families attended by the Brazilian Family Health Program, specially those characterized by the associated factors detected.

10. Estudo de validade da escala de sobrecarga de familiares cuidadores de pacientes psiquiátricos

A sobrecarga de familiares cuidadores de pacientes psiquiátricos tem sido amplamente estudada por pesquisas internacionais, usando escalas validadas, mas raramente no Brasil, talvez devido à carência de escalas validadas deste construto. OBJETIVO: Esta pesquisa avaliou a validade da versão brasileira da escala Family Burden Interview Schedule (FBIS-BR). MÉTODO: Participaram cem familiares de pacientes psiquiátricos de três instituições psiquiátricas de Minas Gerais, entrevistados com aplicação de um questionário sociodemográfico e três escalas de medida: FBIS-BR, BI e SRQ-20. RESULTADOS: A validade de critério da escala FBIS-BR foi analisada pela sua correlação com a escala BI, já validada para o Brasil e que avalia o mesmo construto, obtendo-se correlações significativas, entre 0,23 e 0,69 (p < 0,01). A validade de construto da escala FBIS-BR foi analisada pela sua correlação com a SRQ-20, que avalia um construto diferente, de transtornos psicológicos, porém teoricamente relacionado ao de sobrecarga, tendo-se obtido correlações significativas (p < 0,01) entre 0,31 e 0,49. CONCLUSÃO: A escala FBIS-BR constitui um instrumento de medida válido para avaliar a sobrecarga dos familiares cuidadores de pacientes psiquiátricos, podendo ser utilizada em serviços de saúde mental para identificar as dificuldades e as necessidades destes familiares e para direcionar novas intervenções de orientação e suporte.

OBJECTIVE: To investigate the satisfaction of health-care professionals in inpatient and outpatient psychiatric hospitals of a Brazilian medium-sized city. METHOD: The study evaluated 136 health-care professionals from six hospitals; of which two were outpatient hospitals, two general hospitals, and two psychiatric hospitals. All professionals answered the Brazilian Mental Health Services' Staff Satisfaction Scale. RESULTS: An average satisfaction score of 3.26 was observed, which is situated between indifference (level 3) and satisfaction (level 4). Factors "service quality" (3.48) and "relationships at work" (3.48) showed higher scores compared to "service participation" (3.20) and "work conditions" (2.97) (p < 0.001). The female patient unit in the psychiatric hospital presented lower satisfaction scores (p < 0.001). Satisfaction was higher in the category "technicians" compared to "physicians" and "nurses" (p = 0.004). Moreover, day workers reported higher satisfaction compared to night workers regarding "service quality" and "service participation" (Student's t, p = 0.01 and p = 0.007). DISCUSSION AND CONCLUSION: Results show an intermediate level between indifference and satisfaction with services, with higher scores regarding care provided to the patients. Comparisons among the studied facilities revealed the numerous factors involved in determining one's satisfaction. They suggest advancements and reform measures likely to occur in the region's psychiatric health-care services. Monitoring satisfaction proved useful in predicting service quality improvements. OBJETIVO: Investigar a satisfação de profissionais pertencentes às instituições psiquiátricas de internação integral e parcial em uma cidade de porte médio. MÉTODO: Foram avaliados 136 profissionais de seis serviços, sendo dois de hospitalização parcial, duas enfermarias em hospital geral e duas unidades em hospital psiquiátrico. Os profissionais responderam à Escala de Avaliação da Satisfação da Equipe. RESULTADOS: Observou-se um escore global médio de satisfação de 3,26, que se situa entre indiferença (nível 3) e satisfação (nível 4), sendo maior nos fatores "Qualidade do Serviço" (3,48) e "Relacionamentos no Trabalho" (3,48) em relação à "Participação no Serviço" (3,20) e "Condições de Trabalho" (2,97) (p < 0,001). Observou-se satisfação inferior na unidade feminina do hospital psiquiátrico (p < 0,001). A satisfação foi maior na categoria "técnicos" em relação a médicos e enfermeiros (p = 0,004) e maior entre profissionais do período diurno do que noturno, nos fatores "Qualidade do Serviço" e "Participação no Serviço" (t de Student, p = 0,01 e p = 0,007). DISCUSSÃO E CONCLUSÃO: Os resultados mostraram níveis intermediários de satisfação com os serviços, com maiores escores de satisfação relacionados à assistência aos pacientes. A comparação dos serviços aponta a diversidade de fatores envolvidos na determinação da satisfação e sugere avanços no processo de reforma da assistência psiquiátrica na região. O monitoramento da satisfação mostra utilidade na perspectiva de desenvolvimento da qualidade dos serviços.

Cette recherche évalue l'impact d'un programme de réinsertion sociale développé à l'unité 304 de l'hôpital Louis-H. Lafontaine pour préparer et suivre les patients psychiatriques de longue durée dans leur réinsertion dans la communauté. Trois ans après leur sortie de l'hôpital, les patients ayant participé au programme ont été comparés à des patients-contrôle appariés sortant d'autres unités. Les résultats indiquent que la participation au programme, couplée à un milieu résidentiel supervisé par du personnel professionnel, favorise une meilleure réinsertion des patients de longue durée., This study evaluates the impact of a rehabilitation program developed by a specific unit (304) of Louis H. Lafontaine Hospital to prepare severely disabled long-term psychiatric inpatients for discharge. Patients who participated in the program were evaluated three years after their discharge and compared to control patients paired to them by age, sex and years of hospitalization and discharged from other units of the hospital. The results suggest that the rehabilitation program, if coupled with an adequately staffed residential environment giving patients continuous care, offers a better outcome, at least in the case of the severely disabled long-term psychiatric inpatients.

OBJETIVO: Os níveis de satisfação e sobrecarga de profissionais em serviços de saúde mental afetam tanto a sua saúde e o bem-estar quanto a qualidade do cuidado prestado aos pacientes. Para avaliar estas dimensões, é necessário ter acesso a escalas de medida válidas e fidedignas. Este artigo visou descrever um estudo da validade de construto de duas escalas de avaliação da satisfação (SATIS-BR) e da sobrecarga (IMPACTO-BR) de profissionais em serviços de saúde mental. MÉTODOS: As escalas foram aplicadas a uma amostra de 172 profissionais, que trabalhavam em seis serviços de psiquiatria de uma cidade de porte médio de São Paulo. A validade de construto foi avaliada a partir do teste da hipótese de que deve haver uma correlação significativa entre dois construtos teoricamente relacionados. Para isso, foram feitas análises estatísticas correlacionais de Spearman entre as escalas de satisfação e sobrecarga, assim como uma comparação de dois subgrupos de sujeitos, com e sem indicativo de estresse, com relação aos seus graus de satisfação e sobrecarga no trabalho. RESULTADOS: Obteve-se correlação significativa negativa esperada entre os escores de satisfação e sobrecarga (r = -0,38). Obteve-se ainda a diferença significativa entre dois grupos de sujeitos, com e sem estresse, com relação ao grau de satisfação e sobrecarga no trabalho, sujeitos com estresse apresentando níveis mais baixos de satisfação no trabalho e grau mais elevado de sobrecarga. CONCLUSÕES: Conclui-se que as escalas analisadas apresentam validade de construto e são adequadas para monitorar a qualidade de serviços de saúde mental. A escala de sobrecarga constituiu um preditor do estresse apresentado pelos trabalhadores em saúde mental. OBJECTIVE: Satisfaction and burden of professionals in mental health services can affect their well being and also the quality of the care they provide to the patients. In order to evaluate those dimensions, it is necessary to have access to valid and reliable measure scales. This study describes the evaluation of the construct validity of two scales measuring satisfaction (SATIS-BR) and burden (IMPACTO-BR) of professionals in mental health services. METHODS: The scales were applied to a sample of 172 professionals who worked in six mental health services in a medium city of the state of São Paulo. Construct validity was evaluated by testing the hypotheses that theoretically related constructs should present statistical significant correlation. A Spearman correlation analysis was performed between those two scales and a comparison was made between two groups of subjects, with and without indicative of stress, regarding their levels of satisfaction and burden. RESULTS: A significant negative correlation was observed between the satisfaction and burden scores (r= -0,38). It was also observed a significant difference in satisfaction and burden degree between the two groups of subjects, with and without indicative of stress, subjects with stress presenting lower degree of job satisfaction and higher level of burden in work. CONCLUSIONS: It was concluded that the satisfaction and burden scales analyzed present construct validity and are adequate to evaluate the quality of mental health services. Burden scores were predictive of stress in mental health professionals.

OBJECTIVE: This study aimed at evaluating the characteristics of mental disturbances in a low socio-economic level sample of persons attended by a community family health program called Programa de Saude da Familia (PSF), in order to identify their need for care. METHOD: Trained research assistants interviewed 400 residents, indicated by the PSF team. Subjects were of a low economic level catchment area of a town in the interior of the state of Minas Gerais, in Brazil. Interviewers applied the Questionnaire of General Health (QSG), which evaluates the characteristics of mental health non-psychotic problems. RESULTS: A high percentage of persons (37.8%) had symptomatic global scores in the scale, which indicates the presence of problems in mental health, especially in Sub-scales Regarding Sleep Disturbances (41%) and Desire of Death (38.25%), and less in the Sub-scales of Distrust in Performance (27.75%), Psychosomatic Disturbances (25.5%) and Psychological Stress (22%). A significantly higher percentage of men had symptomatic global scores (45.1%) compared to women (32.6%), but a greater percentage of women (44.1%) had symptomatic scores in the sub-scale regarding desire of death compared to men (29.9%). CONCLUSION: The results obtained described important problems in mental health, differentiated by sex, and pointed to the need for mental health care of those persons.

OBJECTIVE: Psychiatric deinstitutionalization resulted in greater involvement of families in daily care of patients. Family caregivers burden has been described by international researches. In Brazil very few studies investigated family burden with validated scales, perhaps due to the restrict availability of scales to measure this construct. The present study examined the reliability of the Brazilian version of the Family Burden Interview Schedule. METHOD: Scale reliability was assessed through its internal consistency and temporal stability. A sample of 243 family caregivers of psychiatric patients attending public outpatient services were interviewed with the scale. Socio-demographic data were collected with a standardized form. A sub-sample of 42 relatives participated in a re-test, after 3 weeks, for assessment of the scale temporal stability. RESULTS: Cronbach alpha coefficients ranged from 0.58 to 0.90 for global and domains scores. Pearson correlation coefficients and intra-class correlation coefficients for test and retest ranged from 0.54 to 0.90. DISCUSSION: Results for internal consistency were similar to the original scale. Four subscales presented alpha coefficients superior to the original ones. The temporal stability cannot be compared because this analysis was not performed for the original scale. CONCLUSION: The Brazilian version of the scale had good psychometric properties of temporal stability and internal consistency. OBJETIVO: A desinstitucionalização psiquiátrica resultou em maior envolvimento das famílias nos cuidados diários dos pacientes. Pesquisas internacionais constataram a sobrecarga dos familiares, mas, no Brasil, poucas pesquisas a investigaram utilizando escalas validadas, talvez devido à carência de instrumentos de medida deste construto. Este estudo avaliou a confiabilidade da versão brasileira da escala Family Burden Interview Schedule. MÉTODO: Avaliou-se a confiabilidade, em termos da consistência interna e estabilidade temporal. Foram entrevistados 243 familiares cuidadores de pacientes psiquiátricos de serviços públicos de saúde mental. Os dados sociodemográficos foram obtidos por questionário padronizado. Uma subamostra de 42 familiares foi entrevistada novamente, após três semanas, para avaliação da estabilidade temporal teste-reteste. RESULTADOS: Os coeficientes alfa de Cronbach para os escores globais e domínios variaram de 0,58 a 0,90. Os coeficientes de correlação de Pearson e de correlação intraclasse para o teste-reteste foram positivos e significativos, variando de 0,54 a 0,90. DISCUSSÃO: Os resultados da consistência interna foram semelhantes aos da escala original, mas quatro subescalas apresentaram valores de alfa superiores. A estabilidade não pode ser comparada, pois esta não foi avaliada para a escala original. CONCLUSÃO: A versão brasileira da escala apresentou boas propriedades psicométricas de estabilidade temporal e consistência interna.

The aim of this research was to verify the readiness for the treatment in alcoholics who were taking part of a therapeutical intervention, following the Minnesota Model. A sample of 25 moderate and severe level alcoholists took part on this research. They ranged from 23 to 60 years of age, both male and female. The scale SADD and SOCRATES were applied during the first interview, in the beginning of the treatment. Three additional interviews were performed in the middle, at the end and one month after the treatment, to appraise the readiness for the treatment through SOCRATES scale. The results showed that, although there had been 64% of adhesion to treatment, there was not reduction of ambivalence of the alcoholics. There has been significant correlation between the SOCRATES scale factors. This research points to the need of using motivating techniques to reduce the ambivalence, in order to increase the adhesion and to prevent relapse.

Perceived improvement has been proposed as an important patient-reported outcome measure in mental health services evaluation. One advantage of this measure is that it can indicate whether other outcome measures, as pre-post differences in symptoms, correspond to a noticeable impact in patients' lives, as assessed by themselves. This study investigated the association between observer and self-reported symptomatic changes and perceived improvement by patients treated in two Brazilian outpatient mental health services. Significant and positive correlations were found between perceived improvement scores and both pre-post differences, obtained in observer-reported and patient-reported symptom scores. Nevertheless, scores of perceived improvement showed to be more correlated to patient-reported than to the observer-rated symptomatic change score. In addition, a greater correlation was found between perceived improvement scores and post-treatment symptom scores, compared to pre-treatment. These results suggest that an improvement in symptom severity, measured by pre-post differences scores, corresponds to the patient perception that he is actually better than before. However, the correlations found were moderate, suggesting that other factors may also be related to perceived improvement and must be investigated in further studies.

The authors compared the levels of job burden and stress in psychiatry residents with those of other healthcare professionals at inpatient and outpatient psychiatric hospitals in a medium-sized Brazilian city.In this study, the levels of job burden and stress of 136 healthcare workers and 36 psychiatry residents from six various psychiatric facilities (two day-hospitals, two inpatient units of psychiatric hospitals, and two general hospitals) were evaluated. All participants completed two rating scales for job burden and stress.Psychiatry residents showed higher job burden and stress than other healthcare workers. There was a negative correlation between the burden scores and age in the sample of employees, but no correlation of burden and age in residents.Psychiatric residents in this study suffered higher levels of job burden and stress than other healthcare professionals, suggesting the need for changes in residency programs to deal with this issue.

Objetivo: Avaliar a satisfação de familiares cuidadores de pacientes psiquiátricos com os serviços de saúde mental e seus fatores associados. Método: Realizou-se uma pesquisa avaliativa de serviços, do tipo correlacional e de corte transversal, com uma amostra de 85 familiares cuidadores de pacientes psiquiátricos, atendidos em três serviços de saúde mental públicos, situados em três cidades do interior de Minas Gerais. Utilizaram-se a Escala de Avaliação da Satisfação dos Familiares com os Serviços de Saúde Mental (SATIS-BR) e um questionário de variáveis sociodemográficas e clínicas. Resultados: A maioria dos familiares estava satisfeita ou muito satisfeita em relação aos aspectos avaliados dos serviços. A satisfação estava significativa e positivamente associada à idade dos pacientes e ao número de meses sem internação psiquiátrica. Não foi detectada influência do tipo de serviço no grau de satisfação. Conclusão: As variáveis dos pacientes foram os principais fatores associados à satisfação dos familiares. A satisfação dos familiares foi elevada, tendo-se inferido a influência parcial do contraste com outros serviços de saúde. Foi apontada a necessidade de maior diferenciação entre os serviços. Estudos futuros, com amostras maiores e aleatórias, poderão reavaliar esses resultados.

OBJETIVO: Investigar os fatores associados à qualidade de vida (QV) em pacientes com esquizofrenia, em particular a percepção de mudanças pelo próprio paciente, em função do tratamento recebido em serviços de saúde mental. MÉTODO: O estudo foi conduzido em Divinópolis (MG), com pacientes atendidos no Serviço de Referência em Saúde Mental. Foram realizadas entrevistas individuais estruturadas com a aplicação da Escala de Qualidade de Vida (QLS-BR), Escala de Mudança Percebida (EMP) e questionário sociodemográfico e clínico. Foram conduzidas três análises de regressão linear múltipla, para determinar a importância relativa dos fatores preditores da QV. RESULTADOS: Participaram deste estudo 72 pacientes, sendo a maioria do sexo masculino (59,7%), com diagnóstico de esquizofrenia paranoide (87,5%). A QV dos pacientes enquadrou-se na categoria de considerável prejuízo, com um escore médio global de 3,64. A média global das mudanças percebidas pelos pacientes foi de 2,46. Os principais preditores de melhor QV foram: em primeiro lugar, o escore global de mudança percebida e os escores das subescalas "Aspectos psicológicos e sono" e "Ocupação e saúde física". Outros três preditores foram: estar trabalhando, tomar a medicação sozinho e fazer uso de medicação apenas do tipo oral. CONCLUSÃO: A percepção de mudanças pelo próprio paciente, em função do tratamento, é um fator preditivo importante da QV.

OBJECTIVE: This study aimed to validate the Perception of Change Scale - Family Version, which evaluates the perception of family caregivers in regard to the treatment outcomes of psychiatric patients in mental health services. METHOD: Family caregivers (N = 300) of psychiatric patients attending mental health services completed the Perception of Change Scale - Family Version. The scale has 19 items rated in a three-point Likert scale that evaluate changes perceived in the patient's life as a result of treatment. RESULTS: The factorial analysis revealed a four-factor structure, with the following dimensions: 1) occupation, 2) psychological factors, 3) relationships, and 4) physical health. In the internal consistency analysis, Cronbach's alpha coefficient was 0.85. The test-retest temporal stability analysis yielded a significant intraclass correlation coefficient (r = 0.96; p < 0.005). The convergent validity analysis revealed a positive significant correlation with another scale evaluating a distinct but theoretically related construct of family satisfaction with services (r = 0.41; p < 0.05). CONCLUSION: The Perception of Change Scale - Family Version has adequate reliability and construct and convergent validity. It can be used to evaluate treatment outcome in mental health services from the perspective of family caregivers, indicating targets to improve treatment. OBJETIVO: O objetivo desta pesquisa foi validar a Escala de Mudança Percebida pelos familiares, que avalia o resultado do tratamento recebido por pacientes psiquiátricos em serviços de saúde mental, baseado na escala canadense Questionnaire of Perceived Changes. MÉTODOS: Participaram da pesquisa 300 familiares cuidadores de pacientes psiquiátricos, atendidos em serviços de saúde mental. A escala foi aplicada em entrevistas individuais estruturadas. A escala possui 19 itens que avaliam mudanças percebidas na vida dos pacientes em função do tratamento, com alternativas de resposta em escala Likert de três pontos. RESULTADOS: A escala apresentou estrutura fatorial de quatro fatores, avaliando mudanças nas seguintes dimensões: 1) Ocupação, 2) Dimensão psicológica, 3) Relacionamentos e 4) Saúde física. Possui adequadas consistência interna (alfa = 0,85) e estabilidade temporal teste-reteste (r = 0,96; p < 0,05). A análise da validade convergente indicou correlação positiva significativa com a escala de construto relacionado de satisfação dos familiares com os serviços (r = 0,41; p < 0,05). CONCLUSÃO: A Escala de Mudança Percebida apresenta qualidades psicométricas adequadas de fidedignidade, validade de construto e validade convergente. Poderá ser usada para avaliar resultados do tratamento, na percepção dos familiares, apontando mudanças positivas na vida dos pacientes e aspectos a serem melhorados no tratamento.

INTRODUÇÃO: Para avaliar a qualidade dos serviços de saúde mental, medidas de resultados relatados pelos próprios usuários têm sido destacadas e, mais recentemente, a percepção de mudanças. O objetivo desta pesquisa foi realizar a adaptação transcultural para o Brasil da escala canadense Questionnaire of Perceived Change, obtendo-se a Escala de Mudança Percebida (EMP), em duas versões: a dos pacientes e a dos familiares. MÉTODO: Participaram da pesquisa 20 familiares e 23 pacientes psiquiátricos de um serviço de saúde mental de uma cidade de porte médio de Minas Gerais. A escala foi submetida aos procedimentos de adaptação transcultural: tradução, retradução, análise por comissão de especialistas, estudo piloto. A escala original possui 20 itens que avaliam a mudança percebida em quatro dimensões da vida dos pacientes. As alternativas de resposta estão dispostas em escala tipo Likert de 4 pontos. RESULTADOS: Foram feitas modificações, incluindo: uma nova forma de aplicação por um entrevistador, número balanceado de alternativas de resposta resultando em três opções, redação dos itens em linguagem simples, eliminação de um item, uso de exemplos e inclusão de duas perguntas abertas. A escala mostrou-se de fácil compreensão pelos usuários. CONCLUSÃO: As duas versões da escala EMP estão adaptadas ao contexto brasileiro e apresentam equivalência semântica com a escala original. Elas servirão para avaliar os resultados do tratamento, na percepção dos seus usuários. INTRODUCTION: To evaluate the quality of mental health services, patient-reported outcome measures have been used and, more recently, a measure of perception of change by users. This research aimed to make the transcultural adaptation to Brazil of the canadian Questionnaire of Perceived Changes, in order to obtain the Escala de Mudança Percebida (EMP), in two forms, pa-tient-scale and family-scale. METHOD: Twenty family members and 23 psychiatric patients from a mental health services in a median size town of Minas Gerais, Brazil, participated in this research. The scale was submitted to transcultural adaptation procedures: translation, backtranslation, analysis by a specialists committee and pilot study. The original scale is composed of 20 items which evaluate the users´ perceived change in four domains of patient's life. Response alternatives are disposed in a four point Likert scale. RESULTS: The scale was modified, including: a new application procedure by an interviewer, a balanced number of response alternatives with three options, the elimination of one item, a more simple elaboration of items, the use of examples and the inclusion of two open questions. The scales were easily understood by users. CONCLUSION: The two EMP scale forms are adapted to Brazilian context and have semantic equivalence to the original scale. They will be useful to evaluate the treatment results according to the users´ perception.

OBJETIVO: Apresentar os principais aspectos da reforma psiquiátrica ocorrida em diversos países e no Brasil, destacar a política de reinserção social dos pacientes e descrever os procedimentos da desospitalização ocorridos em Barbacena, Minas Gerais, com ênfase nas residências terapêuticas e nas intervenções de reabilitação psicossocial. MÉTODO: Pesquisa bibliográfica realizada nas fontes eletrônicas Medline e Lilacs (1990 a 2006), com os seguintes descritores: deinstitutionalization, psychiatric reform, community-based treatment, psychosocial rehabilitation. Descrição do processo de implantação das residências terapêuticas (RT) por meio de consulta aos documentos da Prefeitura de Barbacena e de observação direta das residências. RESULTADOS: A maioria dos trabalhos selecionados indicou que pacientes desospitalizados apresentam melhora na autonomia, na interação social, no nível global de funcionamento e na qualidade de vida. A desospitalização implica a existência de serviços comunitários com diversos níveis de atenção e requer a presença de profissionais qualificados e programas de reabilitação efetivos. Em Barbacena, o processo de desospitalização teve início em 2000. Atualmente existem 24 RTs no município. CONCLUSÃO: Apesar das dificuldades na sua implantação, os tratamentos de base comunitária tornaram-se o modelo dominante de cuidados psiquiátricos. Para os pacientes desospitalizados, as RTs têm importante papel no processo de reinserção social. OBJECTIVE: The aim of this article is to show the more relevant aspects of psychiatric reform and the community insert of psychiatric patients around the world and in Brazil. It will be detached the procedures of discharge occurred in the city of Barbacena, emphasizing the residential services and the psychosocial approaches. METHODS: Databases Medline and Lilacs were searched between 1990 and 2006 using the following keywords: deinstitutionalization, psychiatric reform, community-based treatment, psychosocial rehabilitation. For the implementation of the residential services, official documents were used. RESULTS: Review and follow-up studies were selected. The most of the studies indicate that the patients have better autonomy, social interaction, global behavior and life quality when they live in community settings. Nevertheless, the authors emphasize the importance of community support, professional staff and rehabilitation programs as a condition for good outcomes. In Barbacena, the procedures of deinstitutionalization began in 2000. Nowadays there are twenty four residential services in this city. DISCUSSION: In despite of difficulties in the psychiatric reform process, the community-based treatment and psychosocial rehabilitation approach are the principal models of psychiatric care presently, and the residential services play an important role in this process.

25. Unmet Needs in Outpatients with Psychotic Disorders in Brazil

Introduction The assessment of needs among patients with severe mental disorders is one of the most studied measures in treatment planning and monitoring of mental health services. Nevertheless, studies on this subject are scarce in Brazil, even following the recent expansion of community services. Objectives To investigate the prevalence and correlates of unmet needs among outpatients with psychotic disorders in Brazilian community mental health services. Methods This is a cross-sectional study with a random sample of 401 outpatients with psychotic disorders in the city of Santos, Brazil. Needs were assessed through interviews with patients using the Camberwell Assessment of Needs (CAN), which investigates 22 clinical, social and basic needs. The Positive and Negative Symptom Scale (PANSS) was used to assess symptoms. Data were analyzed with multiple linear regression, using p Results The most reported needs were psychotic symptoms (67.0%), information needs (55.0%), psychological distress (41.8%) and money (41.5%). The mean number of unmet needs was 2.91 (+-2.56). A greater number of unmet needs was significantly associated to lower education, more negative symptoms and general psychopathology, all with p = 0.001. Conclusions The most reported needs were clinical and social. The number of unmet needs was consistent with the results of international studies. Furthermore, the association of greater unmet needs with lower education and higher symptoms severity may indicate the need for further and comprehensive psychosocial interventions.

Esta pesquisa teve como objetivo fazer a análise psicométrica de fidedignidade do teste e do reteste da versão brasileira da escala Independent Living Skills Survey (ILSS-BR), que avalia a autonomia de pacientes crônicos em diversas áreas do funcionamento social. Um estudo prévio havia mostrado que esta escala apresentava qualidades psicométricas adequadas de consistência interna das suas subescalas, assim como validade discriminante e validade de construto, sem ter avaliado, no entanto, sua estabilidade temporal. Os resultados da presente pesquisa mostraram que a escala ILSS-BR apresentou coeficientes de correlação significativos entre os escores do teste e do reteste, para todas as suas subescalas e para o escore global. Esses resultados indicam que o ILSS-BR é um instrumento de medida que apresenta estabilidade temporal, mostrando-se, portanto, um instrumento fidedigno para ser utilizado no planejamento e na avaliação de programas relacionados à reabilitação psicossocial de pacientes psiquiátricos, no contexto brasileiro. This research analyzed the psychometric property of test-retest reliability of the Brazilian version of the Independent Living Skills Survey scale (ILSS-BR), developed to provide global assessments of functional living skills of chronically mentally ill individuals. Previous research have demonstrated that the ILSS-BR had good psychometric properties of reliability and validity, in terms of the internal consistency of its sub-scales, construct validity of the scale and discriminant validity, but have not analyzed its temporal stability. The results of the present research showed significant correlation coefficients between the test and the retest scores of the ILSS-BR, for each of its sub-scales and for the global score. This result demonstrated that the ILSS-BR has temporal stability and is a reliable scale to be used in the planning and evaluation of psychosocial rehabilitation program for psychiatric patients, in brazilian context.
