Background: To prevent or delay the onset of psychotic disorders or ameliorate their course, prodromal research has strived to identify and treat individuals at risk of developing psychosis. While this approach is laudable, it is, however, not entirely unproblematic from clinical and conceptual perspectives. For example, it remains unclear how we are to understand the development from a nonpsychotic, distressing condition such as schizotypal disorder to a psychotic disorder such as schizophrenia? The current terminology on the subject implies either a nonlinear jump ("conversion") or a more linear progression ("transition") from one disorder to another. To enrich our understanding of such diagnostic shifts, we examined the psychopathological pictures of patients who "transitioned" from schizotypal disorder to schizophrenia. Methods: From a larger study on psychopathology, we examined 40 patients who were diagnosed with schizotypal disorder at baseline. At 5-year follow-up, 30 patients maintained the diagnosis of schizotypal disorder, while 10 were re-diagnosed with schizophrenia. We examined detailed descriptions of the 10 patients who progressed to schizophrenia, comparing psychopathology and level of functioning. Results: The level of functioning decreased slightly from baseline to follow-up in 9 out of 10 patients. Eight patients had previously had micro-psychotic or psychotic experiences. All patients had self-disorders at baseline, and several patients had perceptual disorders. Nine patients had formal thought disorders at baseline. The progression is illustrated by 2 cases. Conclusion: In this small study, we did not find any striking changes in any of the patients, neither in terms of psychopathological manifestations nor in terms of their level of functioning. Mental illness is one of the main reasons for homelessness in Denmark. Yet, some patients do not accept psychiatric treatment despite need for treatment. This can lead to involuntary admission. In this study, we examined changes after involuntary admissions of mentally ill homeless people. We conducted a longitudinal, naturalistic follow-up study of patients who had been involuntarily admitted by the Homeless Outreach Psychiatric Service through 1 year. We examined changes in mental condition, social situation, and substance use of mentally ill homeless people one year after involuntary admission. At baseline, 28 patients were included: 86% of them had been admitted before, 46% had a substance use, and 25% had prior been convicted of a violent crime. 89% of the patients were discharged to the streets. At 12 months, 50% of the patients had symptom reduction, 36% were in psychiatric treatment, and only 14% had acquired permanent housing. No improvements in substance use were observed and 11% of the patients had new convictions after 12 months. During the study period, a total of 24 readmissions distributed on 14 patients was found. Involuntary admissions can be an effective treatment of psychiatric aspects of mentally ill homeless patients. Unfortunately, a substantial part of these patients was quickly discharged before sufficient treatment, leaving room for improvement. Furthermore, few improvements were seen concerning the patients' social situation. More focus on the coherence between psychiatric and municipality services, targeting the social issues and substance use of homeless patients, is strongly needed. [ABSTRACT FROM AUTHOR]

Little is known about which factors actually motivate individuals with psychosis to seek help or how psychosis may complicate the help-seeking process. The aim of this article is to examine the steps of this process and how psychopathological experiences might affect and interfere with it. In this qualitative study we interviewed nine patients with a first episode of psychosis. The interviews were transcribed and analysed according to the principles of thematic analysis using inductive as well as deductive methods. The crucial step in help-seeking behaviour seemed to be for the patients to identify the kind of problem they were facing. None of them clearly recognized their psychotic or otherwise anomalous experiences as symptoms of a mental disorder, and most of them did not seriously question the reality status of these experiences. For most of the patients it was an untenable social situation that caused them to seek help. When they did seek help the majority did not initially contact the psychiatric services. It seems paradoxical to expect patients who experience symptoms of psychosis for the first time to be able to unambiguously identify them as being exactly that and accordingly seek out psychiatric help, as diminished insight into illness is an inherent feature of psychosis. However, the phenomenon of 'double bookkeeping' seemed to provide an opening for seeking help from psychiatry in spite of compromised insight. The differential diagnosis of obsessive–compulsive disorder (OCD) and schizophrenia-spectrum disorders can be difficult. In the current diagnostic criteria, basic concepts such as obsession and delusion overlap. This study examined lifetime schizophrenia-spectrum psychopathology, including subtle schizotypal symptomatology and subjective anomalies such as self-disorders, in a sample diagnosed with OCD in a specialized setting. The study also examined the differential diagnostic potential of the classic psychopathological notions of true obsession ('with resistance') and pseudo-obsession. The study involved 42 outpatients diagnosed with OCD at two clinics specialized in the treatment of OCD. The patients underwent semi-structured, narrative interviews assessing a comprehensive battery of psychopathological instruments. The final lifetime research-diagnosis was based on a consensus between a senior clinical psychiatrist and an experienced research clinician. The study found that 29% of the patients fulfilled criteria of schizophrenia or another non-affective psychosis as main, lifetime DSM-5 research-diagnosis. Another 33% received a research-diagnosis of schizotypal personality disorder, 10% a research-diagnosis of major depression and 29% a main research-diagnosis of OCD. Self-disorders aggregated in the schizophrenia-spectrum groups. True obsessions had a specificity of 93% and a sensitivity of 58% for a main diagnosis of OCD. In conclusion, a high proportion of clinically diagnosed OCD patients fulfilled diagnostic criteria of a schizophrenia-spectrum disorder. The conspicuous obsessive–compulsive symptomatology may have resulted in a disregard of psychotic symptoms and other psychopathology. Furthermore, the differentiation of obsessions from related psychopathological phenomena is insufficient and a conceptual and empirical effort in this domain is required in the future. • The MMPI-2 classified first episode psychosis patients more accurately than the SCID. • The results point to increased diagnostic accuracy if the MMPI-2 is added to the SCID. • The SCID demonstrated high specificity but low sensitivity for identifying non-affective psychosis in a first-admission sample. • The Goldberg Index, revised for the MMPI-2, was validated as a measure distinguishing psychotic from non-psychotic psychiatric patients. We aimed to evaluate the Minnesota Multiphasic Personality Inventory-2 (MMPI-2) as a potential means of improving on the SCID's diagnostic efficacy. 76 first-admission patients were assigned DSM-IV consensus diagnoses by two experienced psychiatrists using all available information, then dichotomized into non-affective psychosis and other mental illness groups. The patients were also given the SCID and the MMPI-2. The diagnostic performance of the MMPI-2 was compared to that of the SCID to assess both diagnostic accuracy and incremental validity. MMPI-2 scales 8 (Schizophrenia) and BIZ (Bizarre Mentations) correctly identified 58% and 56% respectively of non-affective psychotic patients. The Goldberg Index had an overall correct classification rate of 70%, but only identified 49% of the psychosis group. The SCID had a correct classification rate of 66% but correctly identified only 25% of the non-affective psychosis patients. Three MMPI-2 scales combined with the SCID resulted in an overall correct classification rate of 73%, and identification of 66% of the non-affective psychosis patients. The results suggest that the MMPI-2 may identify early psychosis at least as well as the SCID. Introduction: The notion of a full psychosis continuum in which psychosis is seen on a continuum with normal experience rests heavily on studies showing that psychotic symptoms are common in the general population. However, the methods of assessment in the studies have been criticized. Objectives: (1) to investigate how many from the general population who answered positively on a psychotic-like symptoms questionnaire and (2) to clinically assess a subset of those with psychotic-like experiences (PLE). Methods: A self-rating questionnaire concerning PLEs was given to individuals from the general population. A subsample of those, who scored positively for PLEs, was clinically assessed for psychosis. Results: Totally, 5.7% of the participants rated positive for PLEs according to the self-rating questionnaire. Forty of these were clinically assessed and 7 (17.5%) of them were found to have had a psychotic experience, 4 of them were already in psychiatric treatment. Conclusion: The self-reported psychotic-like phenomena were more common in the general population than psychotic disorders. However, when assessed clinically, the experienced phenomena did correspond to psychotic phenomena except in a few patients, who were found to suffer from a psychotic disorder. Self-disorders have been hypothesized to be an underlying and trait-like core feature of schizophrenia-spectrum disorders and a certain degree of temporal stability of self-disorders would therefore be expected. The aim of the study was to examine the persistence of self-disorders measured by the Examination of Anomalous Self Experiences over a time span of 5 years. 48 patients with schizophrenia-spectrum disorders were thoroughly assessed for psychopathology at baseline and 5 years later. Self-disorders were assessed by the Examination of Anomalous Self Experiences. The level of self-disorders was same at the two occasions for the full Examination of Anomalous Self Disorders and for four out of the five domains. For one domain, the level of self-disorders increased slightly from baseline to follow-up. The correlations between baseline and follow-up were moderate. 9 out of the 13 most-frequently rated items at baseline showed equal frequencies at follow-up. The baseline level of self-disorders predicted global symptomatic, but not functional outcome. Self-disorders measured by the Examination of Anomalous Self Experiences show a high level of temporal persistence over 5 years and predict symptomatic outcome. [ABSTRACT FROM AUTHOR]

Aim The notion of a disordered self as a core disturbance of schizophrenia was proposed in many foundational texts. Recent studies, spurred by the development of the Examination of Anomalous Self- Experience ( EASE), seem to indicate that self-disorders are a specific manifestation of schizophrenia vulnerability. Follow-up studies of help-seeking, prodromal and first-admission patients have demonstrated the utility of self-disorders for predicting later schizophrenia-spectrum disturbance. We wished to extend these findings by gauging the predictive value of self-disorders in a premorbid, non-clinical population at high risk for schizophrenia. Methods Children from the Copenhagen High- Risk Project with high-genetic risk for schizophrenia ( N = 212) were assessed premorbidly (average age = 15), and diagnostically re-evaluated after 10 and 25 years. Since the EASE was not available at the time of premorbid assessment, we hypothesized that a proxy scale drawn from the Minnesota Multiphasic Personality Inventory ( MMPI) could distinguish those who later developed a schizophrenia-spectrum disorder ( N = 68) from those who remained healthy ( N = 64). The Self- Disorder Scale comprised 32 items whose content suggested an aspect of self-disorder as measured by the EASE. Results Premorbid Self-Disorder Scale scores significantly predicted lifetime schizophrenia-spectrum diagnosis in the high-risk cohort. Although there was considerable item overlap between the new scale and an existing MMPI scale (psychoticism), the overlap did not account for the Self-Disorder Scale's predictive efficacy. Conclusion The results support the notion of self-disorders as a core vulnerability feature in schizophrenia, detectable premorbidly in those developing later schizophrenia-spectrum disorders. Background: Psychosis and signs of severe mental disorders such as schizophrenia often emerge in adolescence. Much attention has been devoted to identifying the individuals experiencing psychosis at an early stage. Implementation of early detection services is widely acknowledged as effective in reducing treatment delays. However, little is known about the diagnostic patterns of individuals entering an early detection service regardless of the initial psychopathological evaluation. The aim of the study is to investigate the diagnostic trajectories of individuals evaluated by an early detection of psychosis unit. Methods: A real-life cross-sectional study coupling register data with information of diagnoses allocated by clinical evaluation in an early detection unit. All individuals clinically evaluated in the early detection unit from 2012 to 2015 were included. A Central Psychiatric Research Register was searched for lifetime allocated diagnoses. Register search allowed a minimum of one-year follow-up after evaluation. To determine the predictive ability of the early detection unit, diagnoses allocated by the initial clinical evaluation were compared to later diagnoses in psychiatric services. Results: In total 450 individuals were clinically evaluated in the early detection unit during a period of 3 years. Previously 174 (39 %) patients had been in contact with mental healthcare services, and 28 (6.2%) had previously been diagnosed with psychosis. During follow-up, a diagnosis of psychosis was allocated in 146 (32.4%) of all evaluated individuals. In the clinical assessment by the early detection unit 107 (73.3 %) were diagnosed with psychosis. In total 39 (14.7%) were assessed false negative for psychosis by the early detection unit, they were later diagnosed with psychosis in mental healthcare services. The majority of psychosis diagnoses was allocated within one year after assessment in the early detection unit and half of individuals who were diagnosed with psychosis was re-diagnosed with schizophrenia (n=73). The hazard ratio of receiving a diagnosis of psychosis subsequently in mental healthcare services in individuals diagnosed with psychosis by the early detection unit was 4.73 (95 % CI: 3.01 – 7.44, p < 0.0001) compared to individuals not found psychotic by the early detection unit. Discussion: That more than a third of the clinical evaluated has previously been help-seeking in mental healthcare system demonstrates a source of delay in treatment, suggesting that contact to other parts of mental healthcare services can delay detection of psychosis. Of the individuals not evaluated cases of psychosis 15 % later received a diagnose of psychosis. This suggests that an important subgroup of the help-seeking individuals undergoes transition to psychosis after contact or that detection of psychosis has not been possible by evaluation in the early detection unit. In conclusion, attention should be made to all individuals self-referred to an early detection unit, as diagnoses of psychosis is seen in the majority within the first year after contact. The presence of delusions is considered a key feature of psychosis, but despite the psychopathological centrality of the concept of delusion, its definition and comprehension is a matter of continuing debate. In recent years studies showing that delusions are common in the general population have accumulated and challenged the way we perceive psychotic illness. In this systematic review, we examine the basis of the psychosis continuum-hypothesis, by reviewing a representative section of the original literature that report measures of delusional ideation in the general population, focusing specifically on methodology. Three online databases were systematically searched for relevant studies. After applying criteria of inclusion and exclusion, 17 articles were included for comprehensive review. Estimates of the distribution of delusions in the general population vary substantially, as does the mode of assessment. The methodology relies with few exceptions exclusively on self-report and fully structured interview by lay person. We conclude that measures of delusions in the general population should be interpreted cautiously due to inherent difficulties in methodology. Hypothesizing a continuum of delusion between normality and full-blown psychosis is deemed premature based on the reviewed studies. [ABSTRACT FROM AUTHOR]

Background: In an effort to make people with signs of psychosis seek help as early as possible, Region Zealand launched in 2012 an early detection team project as the first and only in Denmark. The project consisted of a combination of easy access and an information campaign targeting the public. This nation-wide study examined characteristics and help-seeking behavior of patients with first-episode schizophrenia (FES) in the early detection region in comparison with other Danish regions.Method: Data from the Danish National Schizophrenia register on all Danish patients diagnosed with first-episode schizophrenia during 2012 to 2015 were linked to demographic and health care data drawn from official national registers. Binary logistic regression analyses examined the difference between the early detection region and other regions controlling for demographic characteristics and utilization of mental health care services and contacts to general practitioner (GP).Results: Patients in the early detection region were younger (OR = 0.51; CI: 0.42-0.62; p < 0.000) than in regions without early detection teams. Furthermore, they were more likely to be of Danish origin, and less likely to have contact with mental health services and GPs prior to FES.Conclusion: The study suggests that implementing an early detection team in combination with an information campaign contributed to detecting patients with first-episode schizophrenia earlier than in regions without the early detection team.
