Objetivo: identificar o conhecimento de gestantes e puérperas acerca da sífilis. Método: pesquisa descritiva, com abordagem qualitativa. A coleta de dados ocorreu por meio de instrumento semiestruturado, entre abril a julho de 2021, com 18 gestantes/puérperas com diagnóstico de sífilis na gestação. As respostas foram gravadas e transcritas na íntegra, sendo utilizado para análise a técnica do Discurso do Sujeito Coletivo. Resultados: identificou-se três ideias centrais: 1) Conhecimento sobre a sífilis, 2) Buscando conhecimento sobre a sífilis e 3) Falsa prevenção. O conhecimento das participantes mostrou-se conflitante, pois algumas apresentaram algum conhecimento e outras nenhum, sendo que todas deveriam ter sido orientadas sobre a doença. Considerações finais: identificou-se uma falha no atendimento ofertado nos serviços de saúde. Assim, estratégias voltadas à educação em saúde devem ser incentivadas e implementadas no acompanhamento de pré-natal, ofertando a promoção e prevenção da saúde, a fim de reduzir os casos de sífilis na gestação.

ABSTRACT Objective: to understand feelings about birth among a group of high-risk pregnant women. Method: a descriptive and qualitative study, using Alfred Schütz’s social phenomenology as a philosophical theoretical framework. The study included 25 pregnant women undergoing high-risk prenatal care. The interview had the following guiding questions: tell me about your feelings regarding the moment of birth/childbirth; How do you deal with the high-risk diagnosis? What are your expectations for birth/childbirth? Results: five categories emerged: Fear of obstetric care; Fear of complications with the baby; Fear of cesarean section; Resilience in the face of high-risk pregnancy; and Expectations for birth. Considerations: high-risk pregnant women are afraid of the care they will receive, the risks and concern about the baby’s vitality at birth. The importance of care is emphasized, with a welcoming environment, bonding and communication between health team and pregnant woman.

ABSTRACT Objectives: to assess high-risk prenatal care and identify strategies for improving care. Methods: a mixed study of a prospective cohort, with 319 mothers in a public maternity hospital, from October 2016 to August 2017, using a semi-structured instrument and interview. Analysis was performed using the chi-square test (p≤0.05). The qualitative approach was carried out through interviews with guiding questions to 13 managers, at their workplace, between January and March 2020, analyzed under social phenomenology. Results: higher rates of inadequacy were identified for all criteria. However, when care was shared, there was a higher rate for performing tests (p=0.023), consultations (p=0.002), risk stratification (p=0.013) and emergency information (≤0.000). Weaknesses in the record evidenced impairment in communication and continuity of care. Final Considerations: shared care is a strategy for improving care, however there is a need to strengthen effective referral and counter-referral to care continuity.

Objetivo: descrever o perfil epidemiológico de gestantes de alto risco, com determinantes primários para near miss materno. Método: trata-se de um estudo quantitativo, realizado em uma instituição pública. A coleta de dados ocorreu nos meses de outubro de 2016 a agosto de 2017. Os dados foram compilados no programa SPSS® versão 20.0. A pesquisa foi aprovada com parecer: 1.757.596, CAAE: 59935716700005231. Resultados: participaram do estudo 319 mulheres, 70,5 % com idade de 20 a 35 anos, 58,6% da raça branca. Em relação as características obstétricas 38,2% eram primíparas, 52,0% dos casais não tinham planejado a gestação, 99,4 % realizaram o pré-natal e realizaram mais de seis consultas (92,5%), quanto a resolução da gestação 60, 2% foram realizados cesariana. Entre os determinantes primários de near miss predominou-se: hipertensão (55,5%) e infecção (16,9%), seguidos por distúrbios respiratórios (6,9%); neurológicos (6,5%); cardíacos (6,3%) e hemorrágicos ou de coagulação (4,4%); e entre os menos expressivos estão as disfunções: renais (2,5%) e hepáticas (0,9%). Conclusão: O estudo foi importante para conhecer as gestantes que apresentam determinantes primários para near miss e assim adequar a assistência. Neste sentido, ações para o entendimento da mulher na prevenção de riscos maternos e neonatais, devem ser elaboradas e estimuladas, almejando prevenção e promoção da saúde entre as mulheres.

Objective: to unveil the guidance received on childbirth during high-risk prenatal care in health services in the light of Social Representations Theory. Method: an exploratory descriptive study with qualitative approach, with the Social Representations Theory as a methodological theoretical framework. Data collection occurred between October 2017 and January 2018, in a maternity hospital in northern Paraná, during the hospitalization of 20 puerperal women. Semi-structured interviews and thematic content analysis were used. Results: the category emerged: Knowledge developed in the face of health guidelines for pregnant women. The central nucleus was configured in care directed to women’s health and their empowerment in the pregnancy-puerperal cycle. Conclusion: health guidelines were similar at both levels of care, being directed to fetal health and well-being. Risk situations to newborns were identified in the emergency room that could result in premature cesarean delivery and complications to the conceptus.

ABSTRACT Objective: To know maternal near-miss situations during labor and delivery according to technologies in health. Method: A quantitative study conducted with puerperal women in a public institution in which the maternity department is a reference for high risk care. Data collection was performed from October 2016 to August 2017, and data were processed in SPSS 20.0. Technologies in health were used for discussing the results. Results: Participation of 319 puerperal women. Primiparous women (p=0.027), previous diagnosis of hypertension (p=0.0001), preeclampsia (p=0.0001) and urinary tract infection (p=0.002) were items with statistical significance for the maternal near-miss outcome in labor and delivery. The hard technology associated with the outcome was transportation for hospitalization (p=0.035). Among the light-hard technologies, the following were associated: characteristics of amniotic fluid (p=0.05), vital signs check (p=0.0001), and initiation of the partograph for cesarean section (p=0.026). Among the light technologies, user satisfaction was relevant (p=0.007). Conclusion: Health technologies can be used for structuring the assistance in order to prevent maternal near-miss.

ABSTRACT Objective: to analyze the prenatal follow-up of high-risk pregnancy in the public service. Method: an analytical cross-sectional study carried out in a public maternity hospital in the South of Brazil, during the hospitalization of 319 postpartum women using a semi-structured tool for transcription of the prenatal card records and interview. The data were analyzed using the Chi-Square test (p≤0.05). Results: the adequacy of prenatal care was high (74%); 22.6% intermediate; 3.4% inefficient. Prenatal care had high coverage (100%), early onset (81.5%) and six or more visits (92.4%), but (77.4%) did not receive information about gestational disease and examinations (69.3%). There was statistical significance between the quality of prenatal care and the place of prenatal care (p=0.005). Conclusion: the need to implement a specific protocol for high-risk gestation and continuous education to the teams was evidenced.
