Background: Perfluorinated compounds (PFCs) have been suspected to adversely affect human reproductive health. The aim of this study was to investigate the associations between PFC exposure and male semen quality., Methods: PFCs were measured in serum from 588 partners of pregnant women from Greenland, Poland and Ukraine who provided a semen sample, using liquid chromatography tandem mass spectrometry. Perfluorooctane sulfonate (PFOS), perfluorooctanoic acid (PFOA), perfluorohexane sulfonic acid (PFHxS) and perfluorononanoic acid (PFNA) could be detected in >97% of the samples. The associations between levels of these compounds and semen volume, sperm concentration, total sperm count, motility and morphology were assessed., Results: Across countries, sperm concentration, total sperm count and semen volume were not consistently associated with PFOS, PFOA, PFHxS or PFNA levels. The proportion of morphologically normal cells was 35% lower [95% confidence interval (CI): 4-66%) for the third tertile of PFOS exposure as compared with the first. A similar reduction was found in relation to increasing PFHxS levels. At the third PFOA exposure tertile, the percentage of motile spermatozoa was 19% (95% CI: 1 to 39%) higher than in the first., Conclusions: The most robust finding in the present study was the negative associations between PFOS exposure and sperm morphology suggesting adverse effects of PFOS on semen quality, possibly due to interference with the endocrine activity or sperm membrane function. It cannot be excluded that this association and the positive association between PFOA and semen motility, which was not consistent across countries, might represent a chance finding due to the multiple statistical tests being performed.

Background: Obesity is a strong predictor of fecundity and maternal obesity may well program semen quality during pregnancy, but to our knowledge, no published studies have evaluated this hypothesis., Methods: From a Danish pregnancy cohort established in 1984-87, 347 out of 5109 sons were selected for a follow-up study conducted from February 2005 to January 2006. Semen and blood samples were analyzed for conventional semen characteristics and reproductive hormones and related to information on maternal pre-pregnant body mass index (BMI) that was available for 328 men. Of these, 34 were sons of underweight, and 25 sons of overweight, mothers., Results: Inhibin B decreased with increasing maternal BMI (P = 0.04) and the point estimates for sperm concentration, semen volume, percent motile sperm, testosterone and FSH suggested an impaired reproductive status among sons of overweight mothers, but none of the trends were statistically significant., Conclusions: The results suggest that there may be an effect of high maternal BMI on the sons' semen quality, but the study had only enough power to justify a critical evaluation of the hypothesis in a larger study.

Background: Previous studies suggest a deleterious effect of cigarette smoking on semen quality, but their results have not been consistent. We studied the association between current smoking and semen characteristics and hormonal levels in a large group of healthy men., Methods: From 1987 to 2004, seven separate occupational or environmental semen quality studies were co-ordinated by our department. A total of 2562 men participated, each providing semen and blood sample and answering a questionnaire about lifestyle and factors related to health. Appropriate semen and smoking data were available for 2542 men., Results: Adjusting for study, age and other covariates, we observed an inverse dose-response relation between smoking and semen volume, total sperm count and percentage motile sperm. Heavy smokers had a 19% lower sperm concentration than non-smokers. We found a positive dose-response relationship between smoking and testosterone, LH and the LH/free testosterone ratios., Conclusion: Current smoking in adult life moderately impairs the semen quality. It is well known that semen quality is associated to fecundity. Therefore, it would be sensible to advise men to abstain from smoking to avoid decreased fecundity.

Objectives: This study focused on determining the testicular function of greenhouse workers exposed to pesticides., Methods: Semen was examined for 122 of 199 eligible men (61%) from 30 ornamental flower greenhouses. Sperm concentration, morphology, and viability were measured according to World Health Organization guidelines, and the curvilinear sperm velocity was determined by a computer-assisted analysis of video recordings. Three groups were formed according to expert judgment of current exposure to pesticides from cultures, pesticide formulations, and the transfer of pesticide residues from leaves to hands, and also ranked according to years of work in a greenhouse. The risk estimates were adjusted for the effects of sexual abstinence and other potentially confounding factors., Results: According to current exposure the median values of sperm concentration and the proportion of normal spermatozoa were 60% and 14% lower, respectively, in the high-level exposure group (N=13) than in the low-level group (N=44), and the values of the intermediate group fell in between. The adjusted differences between the high-level and low-level exposure groups were statistically significant, while no differences were observed for the viability and velocity of sperm and sexual hormones. The median sperm concentration was 40% lower for the men with > 10 years' experience in a greenhouse than for those with < 5 years' experience. The age-adjusted testosterone/sex-hormone-binding globulin ratio declined 1.9% (95% confidence interval 0.4-3.4%) per year of work., Conclusions: The results are compatible with the hypothesis that male fecundity may be at risk from exposure to pesticides in the manual handling of cultures in greenhouses.

It is well established that heat is associated with reduced sperm production, but the role of physiological variation in temperature has never been scrutinized in humans. We studied diurnal scrotal temperature and markers of male fertility in a population of couples planning their first pregnancy. Sixty men from a cohort of couples who were planning their first pregnancy were included and scrotal skin temperature was monitored during 3 days using a portable data recorder. Working hours and working postures were recorded daily in a questionnaire. Each man provided a fresh semen sample and the couples were followed for six menstrual cycles or until a clinical pregnancy was recognized. The median value of scrotal skin temperature was 33.3 degrees C in the daytime and 34.8 degrees C at night. In periods of sedentary work, the median temperature was on average 0.7 degrees C higher (SE=0.2 degrees C). In addition, scrotal temperature was higher in the daytime, in summer, and in leisure time compared with working hours. Median sperm concentration among men with more than 75% of their daytime readings above 35 degrees C was 33.4 x 10(6)/mL, compared with 91.8 x 10(6)/mL for men with less than half of their readings above 35 degrees C (difference 58.4; 95% CI: 25.9-77.8 x 10(6)/mL). It is concluded that a sedentary position is a significant source of increased scrotal skin temperature, and even moderate and physiological elevation in scrotal skin temperature is associated with a substantially reduced sperm concentration. Sedentary work should be considered as an important potential confounder for reduced sperm count in epidemiological research.

A correlation between birthweight and sperm counts in adult life was anticipated because impaired fetal growth could impair replication of Sertoli cells produced in fetal life. Furthermore, it was expected that males born with a high birthweight might have impaired sperm production as they are expected to have been exposed to higher levels of oestrogen in fetal life. We examined these predictions by including midwives records for 296 Danish males that had taken part in studies on male fecundity for other reasons. The study provided no support for these expectations. If anything, males born with a birthweight of 3000-3999 g had slightly lower sperm counts and more abnormal spermatozoa.

The predictive value of sperm motility parameters obtained by computer-assisted semen analysis (CASA) was evaluated for the fertility of men from general population. In a prospective study with couples stopping use of contraception in order to try to conceive, CASA was performed on semen samples from 358 men. A recently developed CASA system, Copenhagen Rigshospitalet Image house sperm Motility Analysis System (CRISMAS) was used for assessment of motility parameters. This system has an editing function which allows correction of tracks made by the computer. Probably due to this function, the concentration assessment made by CRISMAS was very close to that made by the technician (median difference <5%) in all concentration ranges. Correlation between CASA parameters and fertility of normal couples (measured as probability of achieving pregnancy) was examined by the Cox regression model. In univariate models ln(sperm concentration) [beta = 0.331, risk ratio (RR) = 1.392, P = 0.0001], ln(total sperm count) (beta = 0.252, RR = 1.286, P = 0.0007) and percentage motile spermatozoa (beta = 0.014, RR = 1.014, P = 0.0004) were most significant predictors for fertility. In a multivariate analysis ln(sperm concentration) (beta = 0.268, RR = 1.307, P = 0.0016) and percentage motile spermatozoa (beta = 0.010, RR = 1.010, P = 0.011) but even more significantly the combined parameter, ln(concentration of motile spermatozoa) (beta = 0.329, RR = 1.389, P = 0.0001), were the only parameters of predictive value for fertility of men in the general population. In conclusion, these parameters obtained by CASA measurements can be used for prediction of fertility potential in normal men. This appears to be the first study showing the value of CASA in prediction of fertility in the general male population.

The objective of the study was to corroborate or refute the hypothesis that farmers having a high intake of organic grown commodities have a high semen quality due to their expected lower level of dietary pesticides intake. Food frequency data and semen were collected from 256 farmers (171 traditional farmers and 85 organic farmers, overall participation rate: 32%) who were selected from central registers. Each farmer delivered one semen sample before the spraying season started. The farmers were divided into three groups where the commodities from organic production contributed no (N, 0%), medium (M, 1-49%), or a high (H, 50-100%) proportion of the fruit and vegetables consumed. Farmers having a high relative intake of organically grown fruit and vegetables also had a high relative consumption of organically produced meat, milk, and bread, and differences were observed comparing the actual mean intake of single commodities, such as rice, potato, and pork meat. The current individual dietary intake of 40 pesticides was estimated using food frequencies and generalized serving size data in combination with data on pesticide concentrations in food commodities as obtained from the National Danish Food Monitoring Program. The estimated pesticide intake was significantly lower among farmers of group H, but for all three groups of farmers the average dietary intake of 40 pesticides was at or below 1% of the acceptable daily intake (ADI) except for the dithiocarbamates (max = 0.21 microg/kg day = 2.2% ADI), methidathion, (max = 0.01 microg/kg day = 1.4% ADI), and 2-phenylphenol (max = 0.21 microg/kg day = 1.1% ADI). The median sperm concentration for the three groups of farmers was not significantly different (p = 0.40, median sperm concentration was N = 62, M = 44, and H = 75 million/ml). The group of men without organic food intake had a significant lower proportion of morphologically normal spermatozoa, but in relation to 14 other semen parameters no significant differences were found between the groups. Intake of 40 individual pesticides was correlated with four semen parameters (concentration, percentage dead spermatozoa, percentage normal sperm heads, and motility [VCL]). Five significant correlations (p value 0.01) were found among the 160 comparisons in relation to percentage dead spermatozoa: azinphos-methyl, carbaryl, chlorfenson, fenitrothion, and tetradifon. For all of them a lower percentage of dead spermatozoa were found in the groups with a high dietary intake of the specific pesticide. In contrast, for all pesticides evaluated only minor differences were found between the groups when considering spermatozoa concentration, morphology, and motility. In conclusion, the estimated dietary intake of 40 pesticides did not entail a risk of impaired semen quality, but precautions should be taken when generalizing this negative result to populations with a higher dietary exposure level or an intake of other groups of pesticides.

Objectives: Organic solvents have been suspected to exert detrimental effects on human spermiogenesis. Styrene, which is both mutagenic and neurotoxic, was selected as a suitable organic solvent for further assessment of a possible effect on semen quality and sperm DNA damage., Subjects and Methods: Semen samples were collected from 23 reinforced plastics workers at the time of employment and after 6 months of styrene exposure and from 21 nonexposed farmers. Intra-individual changes in conventional semen parameters and sperm-DNA denaturation patterns were related to the internal dose of styrene exposure as measured by postshift urinary mandelic acid., Results: A statistically significant decline in sperm density was seen during styrene exposure from 63.5 to 46.0 million sperm/ml, whereas no decline was seen in the nonexposed subjects. The total sperm count was almost halved from an initial value of 175 million sperm/ejaculate. However, no relationship was apparent when the sperm parameters were related to internal levels of exposure. However, an exposure-response relationship was shown for DNA-denaturation patterns, but the numbers were small., Conclusion: A declining sperm count following styrene exposure is suggested. However, the findings of the internal and external comparisons are inconsistent, and this may be due to the high intraindividual variability of semen parameters and the limited study size but may also be attributable to a weak internal exposure gradient. Spermatogenesis may be vulnerable to styrene exposure. However, due to the small numbers these findings are only preliminary.

The effects of zinc, magnesium and calcium in seminal plasma on time-to-pregnancy (TTP) in healthy couples, on conventional semen parameters and computer-assisted semen analysis (CASA) parameters were evaluated. The localization of chelatable zinc ions in seminal plasma and spermatozoa were assessed by autometallography (AMG). Differences in chelatable zinc localization in samples with high and low total zinc were evaluated. Semen samples from 25 couples with short TTP and 25 couples with long TTP were subjected to conventional semen analysis, CASA, zinc and magnesium measurements by inductively coupled plasma mass spectrometry, and calcium by flame atomic absorption spectrometry. The cations were strongly inter-correlated, but no correlation with TTP or conventional semen parameters was found. Semen samples with high zinc concentrations exhibited statistically significant poorer motility assessed by the CASA parameters straight line velocity and linearity than samples with low zinc content. Calcium concentration also showed statistically significant differences for the same parameters, but the effect was removed by entering zinc concentration into a multiple regression model. Semen samples with high total zinc exhibited stronger staining of the seminal plasma at AMG. It is suggested that high seminal zinc concentrations have a suppressing effect on progressive motility of the spermatozoa ('quality of movement'), but not on percentage of motile spermatozoa ('quantity of movement').

It was hypothesized that occupational exposure to pesticides during a spraying season causes changes in semen quality that might be detected in a longitudinal study. We analyzed the within-person changes in semen quality and reproductive hormones across a spraying season in groups of farmers using and not using pesticides. A total of 248 men collected two semen samples (participation rate: 32%). The median sperm concentration declined significantly from the first to the second sample in both groups, but there was no statistical difference in the decline between the two groups, unadjusted or adjusted. Only minor changes were found in sperm morphology, vitality, motility, sperm chromatin denaturation (SCSA), and reproductive hormones, and the differences in changes between the two groups were nonsignificant, or, in the opposite direction to the expected. There was no relation between the changes in sperm parameters in relation to pesticide exposure variables. In conclusion, use of pesticides by Danish farmers is not a likely cause of short-term effects on semen quality and reproductive hormones.

Background: Semen analysis is part of the routine assessment of infertile couples. WHO defines a sperm concentration above 20x10(6) per mL seminal fluid as normal. We studied the association between semen quality and the probability of conception in a single menstrual cycle in Danish couples with no previous reproductive experience., Methods: In 1992-94, we invited 52,255 trades-union members aged 20-35 years, who lived with a partner and had no children to take part in the study; 430 couples agreed. The couples discontinued use of contraception, and were followed up for six menstrual cycles or until a pregnancy was verified within this period. Each man was asked to provide a semen sample at enrolment (which was analysed without freezing). Women kept a daily record of vaginal bleeding and sexual activity. The association between semen quality and likelihood of pregnancy was assessed by logistic regression, adjusted for sexual activity and female factors associated with low fertility., Results: There were 256 (59.5%) pregnancies among the 430 couples: 165 (65.0%) among those with a sperm concentration of 40x10(6)/mL or more and 84 (51.2%) among those with lower sperm concentrations. The probability of conception increased with increasing sperm concentration up to 40x10(6)/mL, but any higher sperm density was not associated with additional likelihood of pregnancy. The proportion of sperm with normal morphology was strongly related to likelihood of pregnancy independently of sperm concentration. Semen volume and motility were of limited value in pregnancy prediction., Interpretation: Our study suggests that the current WHO guidelines for normal semen quality should be used with caution. Some men with sperm counts above the lower limit of the normal range defined by WHO may in fact be subfertile.

Welding may involve hazards to the male reproductive system, but previous studies of semen quality have produced inconsistent results. We studied the effects of welding on markers of semen quality in a Danish nationwide sample of 430 first-time pregnancy planners without earlier reproductive experience. Couples were recruited among members of the union of metal workers and three other trade unions and were followed from termination of birth control until pregnancy for a maximum of six menstrual cycles. The males provided semen samples in each cycle. Median sperm density for welders was 56 x 10(6)/mL (52.5 x 10(6)/mL and 50.0 x 10(6)/mL in two reference groups). No statistically significant differences attributable to welding were found in proportions of morphologically normal sperm, sperm motility assessed by computer-aided sperm analysis, or sex hormones (testosterone, follicle-stimulating hormone, and luteinizing hormone). These negative findings may not apply to populations with high-level exposure to welding fume or to welders exposed to other putative hazards, e.g., heat.

Background: There is circumstantial evidence that human sperm count may have declined during past decades. The purpose of this study was to identify the association between semen quality and year of birth., Methods: The study comprised 8608 men consulting four Danish medical centres from 1968 to 1992 because of infertility. Data on semen quality and urogenital disorders were obtained from medical records while lifestyle data were collected from a subset of the population by a postal questionnaire (response 80%). Semen characteristics were analysed as a linear function of year of birth, centre, season and calendar year at time of semen examination, sexual abstinence and lifestyle factors. Effects of age were accounted for by restriction and stratified analysis., Results: The sperm count declined with increasing year of birth at two of the four centres, but this association disappeared when confounders were adjusted for. Within the subset of men born 1950-1970 we revealed a decrease in the average sperm count by 1.9 mill/ml (95% confidence interval [CI]: 1.45, 2.27) per one advancing year of birth. This finding was consistent across centres even after adjustment for effects of covariates. The proportion of morphologically normal sperm cells changed in parallel with the sperm count, while semen volume did not decline in any time periods., Conclusions: We found a birth cohort effect on sperm count and morphology among Danish infertile men born after 1950 but not in men born in the first part of the century. The findings are compatible with an environmental impact during prenatal life but the evidence is far from unequivocal.

Recent experimental studies in rodents document the spermatotoxic effects of water-soluble hexavalent chromium. Welders comprise, worldwide, a major occupational group with acknowledged exposure to chromium. This study examines the relationship between semen quality and chromium in the urine and blood of a population of 30 tungsten inert gas (TIG) stainless steel welders, 30 mild steel welders and 47 non-welding workers. Each subject provided two to three semen samples. The chromium concentration ranged from 0.17 to 4.74 nmol mmol1 creatinine (median 1.08) in post-shift spot urine and from 6.0 to 46.4 nmol l-1 in blood. None of several semen parameters deteriorated with increasing level of internal exposure to chromium. Low-level exposure to hexavalent chromium associated with TIG stainless steel and mild steel welding do not appear to be a major hazard for human spermatogenesis.

Several studies suggest that welding is detrimental to the male reproductive system. Welding fume and radiant heat are of interest as possible causal factors. This study investigates semen quality and sex hormone concentrations among 17 manual metal arc alloyed steel welders with a moderate exposure to radiant heat (globe temperature ranging from 31.1 degrees to 44.8 degrees C), but without substantial exposure to welding fume toxicants. During exposure to heat the skin temperature in the groin increased on average by 1.4 degrees C (SE +/- 0.72 degrees C). Sperm count and motile sperm count were non-significantly reduced among welders in comparison with two different reference groups. Within the group of welders the proportion of sperm with normal shape declined significantly after six weeks of exposure to heat and increased after a break in exposure. Sperm count and sperm concentration had the same pattern of intraindividual change in relation to exposure to radiant heat, but the changes were not statistically significant. No consistent changes in concentrations of sex hormones were found. The welders investigated were more exposed to radiant heat than welders in general. The results suggest that the study group of welders experienced a reversible decrease in semen quality, most likely caused by a moderate exposure to radiant heat (about five hours a day through several weeks). It remains to be established if even lower levels of exposure to radiant heat in the general population of welders has any impact on semen quality and fertility.

Occupational exposure to hexavalent chromium is of concern because of the carcinogenic action of this metal. The purpose of this study was to evaluate internal exposure to chromium in welders who were exposed to low levels of chromium. Chromium in urine, blood, and seminal fluid was determined among 60 welders and 45 referents. The concentration of chromium in urine and blood did not change across a workshift or across a 3-wk break in exposure. However, stainless-steel and mild-steel welders who were exposed to low levels of chromium and steel welders who were mildly exposed had significantly increased levels of chromium in post-shift urine (mean 2.1 nmol/mmol creatinine [standard deviation (SD) = 1.0] and 1.3 nmol/mmol creatinine [SD = 0.5], respectively) compared with referents (mean 0.7 nmol/mmol creatinine [SD = 0.3]). Pre-shift blood chromium concentrations showed a similar variation between exposed workers and referents. Subgroups of stainless-steel welders had very high levels of chromium in seminal fluid. This finding may, however, be explained by nonoccupational factors and, therefore, warrants further study. Attention should focus on the potential risk of delayed health effects among stainless-steel and mild-steel welders who heretofore were not thought to be at risk from chromium exposure.

In a cross sectional field study concerning the male reproductive system in metalworkers, the major findings were a moderate deterioration of semen quality in mild steel welders and less reliable changes in semen quality in low exposed stainless steel welders. In the present study, a longitudinal design was adopted to deal with methodological drawbacks inherent in the cross sectional approach. The study relies on the assumption that the effect of welding is causal and reversible. The semen quality of 19 mild steel welders, 18 stainless steel welders and 16 non-welding metal-workers, was examined before and three, five, and eight weeks after a three week break in exposure (summer vacation). No consistent improvement in any semen parameter in the follow up period relative to the prevacation period was found in either mild steel or stainless steel welders. The results indicate either a non-causal nature of reported associations between welding exposure and poor semen quality, or that the effect of welding is non-reversible within the rather short non-exposure period.

Welding may be detrimental to the male reproductive system. To test this hypothesis, semen quality was examined in 35 stainless steel welders, 46 mild steel welders, and 54 non-welding metal workers and electricians. These figures represent a participation rate of 37.1% in welders and 36.7% in non-welding subjects. The mean exposure to welding fume particulates was 1.3 mg/m3 (SD 0.8) in stainless steel welders using tungsten inert gas, 3.2 mg/m3 (SD 1.0) in low exposed mild steel welders using manual metal arc or metal active gas (n = 31), and 4.7 mg/m3 (SD 2.1) in high exposed mild steel welders (n = 15). The semen quality of each participant was defined in terms of the mean values of the particular semen parameters in three semen samples delivered at monthly intervals in a period with occupational exposure in a steady state. The sperm concentration was not reduced in either mild steel or stainless steel welders. The sperm count per ejaculate, the proportion of normal sperm forms, the degree of sperm motility, and the linear penetration rate of the sperm were significantly decreased and the sperm concentration of follicle stimulating hormone (FSH) was non-significantly increased in mild steel welders. A dose response relation between exposure to welding fumes and these semen parameters (sperm count excepted) was found. Semen quality decreased and FSH concentrations increased with increasing exposure. Significant deteriorations in some semen parameters were also observed in stainless steel welders. An analysis of information from questionnaires obtained from the whole population including subjects who declined to participate indicated an underestimation of effects due to selection bias. Potential confounding was treated by restriction and statistical analysis. The results support the hypothesis that mild steel welding and to a lesser extent stainless steel welding with tungsten inert gas is associated with reduced semen quality at exposure in the range of the Danish process specific threshold limit values of welding.

Study Question: Is fetal exposure to lower-chlorinated polychlorinated biphenyls (LC-PCBs) in indoor air of private homes built with PCB-containing materials associated with semen characteristics and testicular volume in adult men?Summary Answer: We observed only marginal and inconsistent associations between maternal exposure to PCBs in indoor air and semen quality, testicular size and reproductive hormones in the adult offspring.What Is Known Already: Recent studies have shown LC-PCBs to exhibit endocrine-disrupting properties and increase the risk of cryptorchidism. Although exposure to LC-PCBs in indoor air is relatively common, the long-term impact of prenatal exposure on male reproductive health has not yet been investigated.Study Design, Size, Duration: In this cohort study, participants were men (18+ years) whose mothers carried them while living in one of two residential areas where indoor air had been contaminated by LC-PCB evaporating from building materials in subsets of the apartments. Men were considered prenatally exposed if their mother had lived in a PCB-contaminated apartment and unexposed if their mother had lived in an uncontaminated apartment for a minimum of 1 year during the 3.6 years before conception or during the first trimester. Mothers of prenatally unexposed men could not have lived in a contaminated apartment at any point. Recruitment lasted from 2017 to 2019. In total, 73 exposed and 111 unexposed men gave a blood and semen sample.Participants/materials, Setting, Methods: Percentage differences in semen volume, sperm concentration, total sperm count, morphologically normal spermatozoa, progressively motile spermatozoa and DNA fragmentation index (DFI) between prenatally exposed and unexposed men were estimated using negative binomial regression. Associations with total and calculated free testosterone (CFT), LH and FSH were modeled using the linear regression. Odds of small testicular volume was estimated with logistic regression.Main Results and the Role Of Chance: Overall, the results of this study were conflicting. No differences in semen volume, sperm concentration, testosterone and CFT were observed between the groups, but there were slight indications of lower total sperm count, increased FSH and risk of small testicles, alongside lower sperm DFI and a higher proportion of normal spermatozoa in men exposed to LCB-PCBs from indoor air during fetal life.
