Using an amorphous silicon (a-Si) EPID for transit dosimetry requires detailed characterization of its dosimetric response in a variety of conditions. In this study, a measurement-based model was developed to calibrate an a-Si EPID response to dose for transit dosimetry by comparison with a reference ionization chamber. The ionization chamber reference depth and the required additional buildup thickness for electronic portal imaging devices (EPID) transit dosimetry were determined. The combined effects of changes in radiation field size, phantom thickness, and the off-axis distance on EPID transit dosimetry were characterized. The effect of scattered radiation on out-of-field response was investigated for different field sizes and phantom thicknesses by evaluation of the differences in image profiles and in-water measured profiles. An algorithm was developed to automatically apply these corrections to EPID images based on the user-specified field size and phantom thickness. The average phantom thickness and an effective field size were used for IMRT fields, and images were acquired in cine mode in the presence of an anthropomorphic phantom. The effective field size was defined as the percentage of the jaw-defined field that was involved during the delivery. Nine head and neck dynamic IMRT fields were tested by comparison with a MatriXX two-dimensional array dosimeter using the Gamma (3%, 3 mm) evaluation. A depth of 1.5 cm was selected as the ionization chamber reference depth. An additional 2.2 mm of copper buildup was added to the EPID. Comparison of EPID and MatriXX dose images for the tested fields showed that using a 10% threshold, the average number of points with Gamma index <1 was 96.5%. The agreement in the out-of field area was shown by selection of a 2% threshold which on average resulted in 94.8% of points with a Gamma index <1. The suggested method is less complicated than previously reported techniques and can be used for all a-Si EPIDs regardless of the manufacturer.

In this study an amorphous silicon electronic portal imaging device (a-Si EPID) converted to direct detection configuration was investigated as a transit dosimeter for intensity modulated radiation therapy (IMRT). After calibration to dose and correction for a background offset signal, the EPID-measured absolute IMRT transit doses for 29 fields were compared to a MatriXX two-dimensional array of ionization chambers (as reference) using Gamma evaluation (3%, 3 mm). The MatriXX was first evaluated as reference for transit dosimetry. The accuracy of EPID measurements was also investigated by comparison of point dose measurements by an ionization chamber on the central axis with slab and anthropomorphic phantoms in a range of simple to complex fields. The uncertainty in ionization chamber measurements in IMRT fields was also investigated by its displacement from the central axis and comparison with the central axis measurements. Comparison of the absolute doses measured by the EPID and MatriXX with slab phantoms in IMRT fields showed that on average 96.4% and 97.5% of points had a Gamma index<1 in head and neck and prostate fields, respectively. For absolute dose comparisons with anthropomorphic phantoms, the values changed to an average of 93.6%, 93.7% and 94.4% of points with Gamma index<1 in head and neck, brain and prostate fields, respectively. Point doses measured by the EPID and ionization chamber were within 3% difference for all conditions. The deviations introduced in the response of the ionization chamber in IMRT fields were<1%. The direct EPID performance for transit dosimetry showed that it has the potential to perform accurate, efficient and comprehensive in vivo dosimetry for IMRT.

Purpose: A major problem associated with amorphous silicon (a-Si) electronic portal imaging devices (EPIDs) for transit dosimetry is the presence of a phosphor layer, which can introduce large deviations from water-equivalent behavior due to energy-dependent response and visible light scattering. In this study, an amorphous silicon EPID was modified to a direct detection configuration by removing the phosphor layer, and the accuracy of using it for transit dosimetry measurements was investigated for 6 and 18 MV treatment beams by comparison to ion-chamber in water measurements., Methods: Solid water and copper were both evaluated as buildup materials. Using the optimum buildup thickness in each case, effects of changes in radiation field size, source to detector distance, and patient/phantom thickness were investigated by comparison to reference measurements made by an ionization chamber on the central axis. The off-axis response of the imager was also investigated by comparison of EPID image profiles to dose profiles obtained by a scanning ionization chamber in a water tank with various thicknesses of slab phantoms, and an anthropomorphic phantom in the beam using Gamma evaluation (3%, 3 mm criteria). The imaging characteristics of the direct EPID were investigated by comparison to a commercial EPID using QC3V phantom, and by taking images of an anthropomorphic pelvic phantom containing fiducial gold markers., Results: Either 30 mm of solid water or 3.3 mm of copper were found to be the most suitable buildup thicknesses with solid water providing more accurate results. Using solid water buildup, the EPID response compared to the reference dosimeter within 2% for all conditions except phantom thicknesses larger than 25 cm in 6 MV beams, which was up to 6.5%. Gamma evaluation results comparing EPID profiles and reference ionization chamber profiles showed that for 6 and 18 MV beams, at least 91.8% and 90.9% of points had a Gamma <1 for all phantoms, respectively. But using copper buildup, the EPID response had more discrepancies from the ionization chamber reference measurements, including: More than 2% difference for small air gaps using 6 MV beams, up to 8% difference for phantom thicknesses larger than 25 cm in 6 MV beams, and large differences (up to 9.3%) for increasing phantom thicknesses in 18 MV beams. The percentage of points with Gamma <1 with copper buildup were at least 96.6% and 99.8% in 6 and 18 MV beams, respectively., Conclusions: The direct EPID performs as an ion-chamber detector for transit dosimetry applications in all geometries studied except for small discrepancies at 6 MV for thick phantoms. This can be ameliorated by the calibration of the EPID to dose at an intermediate phantom thickness. The major current limitation of the direct EPID is poor quality of images compared with the clinical configuration, which could be overcome by a method to interchange between imaging and dosimetry setups.

In this work, an amorphous silicon electronic portal imaging device (a-Si EPID) dose prediction model based on the energy fluence model of the Pinnacle treatment planning system Version 7 (Philips Medical Systems, Madison, WI) is developed. An energy fluence matrix at very high resolution (< 1 mm) is used to incorporate multileaf collimator (MLC) leaf effects in the predicted EPID images. The primary dose deposited in the EPID is calculated from the energy fluence using experimentally derived radially dependent EPID interaction coefficients. Separate coefficients are used for the open beam energy fluence component and the component of the energy fluence transmitted through closed MLC leaves to each EPID pixel. A spatially invariant EPID dose deposition kernel that describes both radiative dose deposition, central axis EPID backscatter, and optical glare is convolved with the primary dose. The kernel is further optimized to give accurate EPID penumbra prediction and EPID scatter factor with changing MLC field size. An EPID calibration method was developed to reduce the effect of nonuniform backscatter from the support arm (E-arm) in a calibrated EPID image. This method removes the backscatter component from the pixel sensitivity (flood field) correction matrix retaining only field-specific backscatter in the images. The model was compared to EPID images for jaw and MLC defined open fields and eight head and neck intensity modulated radiotherapy (IMRT) fields. For the head and neck IMRT fields with 2%, 2 mm criteria 97.6 +/- 0.6% (mean +/- 1 standard deviation) of points passed with a gamma index less than 1, and for 3%, 3 mm 99.4 +/- 0.4% of points were within the criteria. For these fields, the 2%, 2 mm pass score reduced to 96.0 +/- 1.5% when backscatter was present in the pixel sensitivity correction matrix. The model incorporates the effect of MLC leaf transmission, EPID response to open and MLC leakage dose components, and accurately predicts EPID images of IMRT fields. Removing the backscatter component of the pixel sensitivity matrix correction reduces the effect of nonuniform E-arm backscatter.

The aim of this work was to experimentally determine the difference in response of an amorphous silicon (a-Si) electronic portal imaging device (EPID) to the open and multileaf collimator (MLC) transmitted beam components of intensity modulated radiation therapy (IMRT) beams. EPID dose response curves were measured for open and MLC transmitted (MLCtr) 10 x 10 cm2 beams at central axis and with off axis distance using a shifting field technique. The EPID signal was obtained by replacing the flood-field correction with a pixel sensitivity variation matrix correction. This signal, which includes energy-dependent response, was then compared to ion-chamber measurements. An EPID calibration method to remove the effect of beam energy variations on EPID response was developed for IMRT beams. This method uses the component of open and MLCtr fluence to an EPID pixel calculated from the MLC delivery file and applies separate radially dependent calibration factors for each component. The calibration procedure does not correct for scatter differences between ion chamber in water measurements and EPID response; these must be accounted for separately with a kernel-based approach or similar method. The EPID response at central axis for the open beam was found to be 1.28 +/- 0.03 of the response for the MLCtr beam, with the ratio increasing to 1.39 at 12.5 cm off axis. The EPID response to MLCtr radiation did not change with off-axis distance. Filtering the beam with copper plates to reduce the beam energy difference between open and MLCtr beams was investigated; however, these were not effective at reducing EPID response differences. The change in EPID response for uniform sliding window IMRT beams with MLCtr dose components from 0.3% to 69% was predicted to within 2.3% using the separate EPID response calibration factors for each dose component. A clinical IMRT image calibrated with this method differed by nearly 30% in high MLCtr regions from an image calibrated with an open beam calibration factor only. Accounting for the difference in EPID response to open and MLCtr radiation should improve IMRT dosimetry with a-Si EPIDs.

Amorphous silicon (a-Si) electronic portal imaging devices (EPIDs) have typically been calibrated to dose at central axis (CAX). Division of acquired images by the flood-field (FF) image that corrects for pixel sensitivity variation as well as open field energy-dependent off-axis response variation should result in a flat EPID response over the entire matrix for the same field size. While the beam profile can be reintroduced to the image by an additional correction matrix, the CAX EPID response to dose calibration factor is assumed to apply to all pixels in the detector. The aim of this work was to investigate the dose response of the Varian aS500 amorphous silicon detector across the entire detector area. First it was established that the EPID response across the panel became stable (within approximately 0.2%) for MU settings greater than approximately 200 MU. The EPID was then FF calibrated with a high MU setting of approximately 400 for all subsequent experiments. Whole detector images with varying MU settings from 2-500 were then acquired for two dose rates (300 and 600 MU/min) for 6 MV photons for two EPIDs. The FF corrected EPID response was approximately flat or uniform across the detector for greater than 100 MU delivered (within 0.5%). However, the off-axis EPID response was greater than the CAX response for small MU irradiations, giving a raised EPID profile. Up to 5% increase in response at 20 cm off-axis compared to CAX was found for very small MU settings for one EPID, while it was within 2% for the second (newer) EPID. Off-axis response nonuniformities attributed to detector damage were also found for the older EPID. Similar results were obtained with the EPID at 18 MV energy and operating in asynchronous mode (acquisition not synchronized with beam pulses), however the profiles were flatter and more irregular for the small MU irradiations. By moving the detector laterally and repeating the experiments, the increase in response off-axis was found to depend on the pixel position relative to the beam CAX. When the beam was heavily filtered by a phantom the off-axis response variation was reduced markedly to within 0.5% for all MU settings. Independent measurements of off-axis point doses with ion chamber did not show any change in off-axis factor with MUs. Measurements of beam quality (TMR20-10) for MU settings of 2, 5, and 100 at central axis and at 15 cm off-axis could not explain the effect. The response change is unlikely to be significant for clinical IMRT verification with this imaging/acclerator system where MUs are of the order of 100-300, provided the detector does not exhibit radiation damage artifacts.

This work assessed the dosimetric performance of an amorphous silicon electronic portal imaging device (EPID) for measurement and quality assurance of enhanced dynamic wedge (EDW) profile and wedge factor. EPID measurements of EDW profiles were corrected for pixel sensitivity variation and spectral over-response relative to ion chamber and compared to ion-chamber and diode-array measurements. The dependence of EPID measurements on wedge direction and source to EPID distance was assessed. The long-term stability was investigated by weekly measurement of EDW profiles and wedge factors over a seven month interval. An empirical correction method was developed to improve EPID profile agreement with diode-array measurements. The EPID profiles differed from conventional measurements by up to 5%. Backscatter from the EPID housing was also found to affect measurements by up to 4%, resulting in changes in EPID measured profile with the direction of the moving jaw and source to EPID distance. EPID profile measurements varied by a maximum of 0.3% (1 SD) within the umbra, and wedge factors varied by 0.3% (1 SD) over the seven month interval. The correction function improved agreement between EPID and diode array to within 2% for all wedge angles and energies. Due to the ease of use and reproducibility of the EPID-measured EDW profiles the device is highly suited to regular measurement of EDW.

Dosimetric properties of an amorphous silicon electronic portal imaging device (EPID) for verification of dynamic intensity modulated radiation therapy (IMRT) delivery were investigated. The EPID was utilized with continuous frame-averaging during the beam delivery. Properties studied included effect of buildup, dose linearity, field size response, sampling of rapid multileaf collimator (MLC) leaf speeds, response to dose-rate fluctuations, memory effect, and reproducibility. The dependence of response on EPID calibration and a dead time in image frame acquisition occurring every 64 frames were measured. EPID measurements were also compared to ion chamber and film for open and wedged static fields and IMRT fields. The EPID was linear with dose and dose rate, and response to MLC leaf speeds up to 2.5 cm s(-1) was found to be linear. A field size dependent response of up to 5% relative to dmax ion-chamber measurement was found. Reproducibility was within 0.8% (1 standard deviation) for an IMRT delivery recorded at intervals over a period of one month. The dead time in frame acquisition resulted in errors in the EPID that increased with leaf speed and were over 20% for a 1 cm leaf gap moving at 1.0 cm s(-1). The EPID measurements were also found to depend on the input beam profile utilized for EPID flood-field calibration. The EPID shows promise as a device for verification of IMRT, the major limitation currently being due to dead-time in frame acquisition.

Purpose: To develop a frame-by-frame correction for the energy response of amorphous silicon electronic portal imaging devices (a-Si EPIDs) to radiation that has transmitted through the multileaf collimator (MLC) and to integrate this correction into the backscatter shielded EPID (BSS-EPID) dose-to-water conversion model. Methods: Individual EPID frames were acquired using a Varian frame grabber and iTools acquisition software then processed using in-house software developed in MATLAB. For each EPID image frame, the region below the MLC leaves was identified and all pixels in this region were multiplied by a factor of 1.3 to correct for the under-response of the imager to MLC transmitted radiation. The corrected frames were then summed to form a corrected integrated EPID image. This correction was implemented as an initial step in the BSS-EPID dose-to-water conversion model which was then used to compute dose planes in a water phantom for 35 IMRT fields. The calculated dose planes, with and without the proposed MLC transmission correction, were compared to measurements in solid water using a two-dimensional diode array. Results: It was observed that the integration of the MLC transmission correction into the BSS-EPID dose model improved agreement between modeled and measured dose planes. In particular, the MLC correction produced higher pass rates for almost all Head and Neck fields tested, yielding an average pass rate of 99.8% for 2%/2 mm criteria. A two-sample independent t-test and fisher F-test were used to show that the MLC transmission correction resulted in a statistically significant reduction in the mean and the standard deviation of the gamma values, respectively, to give a more accurate and consistent dose-to-water conversion. Conclusions: The frame-by-frame MLC transmission response correction was shown to improve the accuracy and reduce the variability of the BSS-EPID dose-to-water conversion model. The correction may be applied as a preprocessing step in any pretreatment portal dosimetry calculation and has been shown to be beneficial for highly modulated IMRT fields. [ABSTRACT FROM AUTHOR]

A simplified method of verifying intensity modulated radiation therapy (IMRT) fields using a Varian aS500 amorphous silicon electronic portal imaging device (EPID) is demonstrated. Unlike previous approaches, it does not involve time consuming or complicated analytical processing of the data. The central axis pixel response of the EPID, as well as the profile characteristics obtained from images acquired with a 6 MV photon beam, was examined as a function of field size. Ion chamber measurements at various depths in a water phantom were then collected and it was found that at a specific depth dref, the dose response and profile characteristics closely matched the results of the EPID analysis. The only manipulation required to be performed on the EPID images was the multiplication of a matrix of off axis ratio values to remove the effect of the flood field calibration. Similarly, dref was found for 18 MV. Planar dose maps at dref in a water phantom for a bar pattern, a strip pattern, and 14 clinical IMRT fields from two patient cases each being from a separate anatomical region, i.e., head and neck as well as the pelvis, for both energies were generated by the Pinnacle planning system (V7.4). EPID images of these fields were acquired and converted to planar dose maps and compared directly with the Pinnacle planar dose maps. Radiographic film dosimetry and a MapCHECK dosimetry device (Sun Nuclear Corporation, Melbourne, FL) were used as an independent verification of the dose distribution. Gamma analysis of the EPID, film, and Pinnacle planar dose maps generated for the clinical IMRT fields showed that approximately 97% of all points passed using a 3% dose/3mm DTA tolerance test. Based on the range of fields studied, the author’s results appear to justify using this approach as a method to verify dose distributions calculated on a treatment planning system, including complex intensity modulated fields. The purpose of this study was to experimentally quantify the change in response of an amorphous silicon (a-Si) electronic portal imaging device (EPID) to dynamic multileaf collimator (dMLC) beams with varying MLC-transmitted dose components and incorporate the response into a commercial treatment planning system (TPS) EPID prediction model. A combination of uniform intensity dMLC beams and static beams were designed to quantify the effect of MLC transmission on EPID response at the central axis of 10×10 cm2 beams, at off-axis positions using wide dMLC beam profiles, and at different field sizes. The EPID response to MLC transmitted radiation was 0.79±0.02 of the response to open beam radiation at the central axis of a 10×10 cm2 field. The EPID response to MLC transmitted radiation was further reduced relative to the open beam response with off-axis distance. The EPID response was more sensitive to field size changes for MLC transmitted radiation compared to open beam radiation by a factor of up to 1.17 at large field sizes. The results were used to create EPID response correction factors as a function of the fraction of MLC transmitted radiation, off-axis distance, and field size. Software was developed to apply the correction factors to each pixel in the TPS predicted EPID image. The corrected images agreed more closely with the measured EPID images in areas of intensity modulated fields with a large fraction of MLC transmission and, as a result the accuracy of portal dosimetry with a-Si EPIDs can be improved. Purpose: Current amorphous silicon electronic portal imaging devices (a-Si EPIDs) that are frequently used in radiotherapy applications employ a metal plate/phosphor screen configuration to optimize x-ray detection efficiency. The phosphor acts to convert x rays into an optical signal that is detected by an underlying photodiode array. The dosimetric response of EPIDs has been well characterized, in part through the development of computational models. Such models, however, have generally made simplifying assumptions with regards to the transport of optical photons within these detectors. The goal of this work was to develop and experimentally validate a new Monte Carlo (MC) model of an a-Si EPID that simulates both x-ray and optical photon transport in a self-contained manner. Using this model the authors establish a definitive characterization of the effects of optical transport on the dosimetric response of a-Si EPIDs employing gadolinium oxysulfide phosphor screens. Methods: The Geant4 MC toolkit was used to develop a model of an a-Si EPID that employs standard electromagnetic and optical physics classes. The sensitivity of EPID response to uncertainties in optical transport parameters was evaluated by investigating their effects on the EPID point spread function (PSF). An optical blur kernel was also calculated to isolate the component of the PSF resulting purely from optical transport. A 6 MV photon source model was developed and integrated into the MC model to investigate EPID dosimetric response. Field size output factors and relative dose profiles were calculated for a set of open fields by separately scoring energy deposited in the phosphor and optical absorption events in the photodiode. These were then compared to quantify effects resulting from optical photon transport. The EPID model was validated against experimental measurements taken using a research EPID. Results: Optical photon scatter within the phosphor screen noticeably broadened the PSF. Variations in optical transport parameters reported in the literature caused fluctuations in the PSF full width at half maximum (FWHM) and full width at tenth maximum (FWTM) of less than 3% and 5%, respectively, confirming model robustness. Greater deviations (up to 9.5% and 36% for FWHM and FWTM, respectively) were observed when optical parameters were largely different from reference values. When scoring energy deposition in the phosphor, measured and calculated output factors agreed within statistical uncertainties and at least 94% of the MC simulated profile data points passed 3%/3 mm γ-index criterion for all field sizes considered. Despite statistical uncertainties in optical simulations arising from computational limitations, no differences were observed between optical and energy deposition profiles. Conclusions: Simulations demonstrated noticeable blurring of the EPID PSF when scoring optical absorption events in the photodiode relative to energy deposition in the phosphor. However, modeling the standard electromagnetic transport alone should suffice when using MC methods to predict EPID dose-response to static, open 6 MV fields with a standard a-Si photodiode array. Therefore, using energy deposition in the phosphor as a surrogate for EPID dose-response is a valid approach that should not require additional corrections for optical transport effects in current a-Si EPIDs employing phosphor screens.
