Purpose: Modic changes (MC) on magnetic resonance imaging (MRI) have been associated with the development and severity of low back pain (LBP). The etiology of MC remains elusive, but it has been suggested that altered metabolism may be a risk factor. As such, this study aimed to identify metabolomic biomarkers for MC phenotypes of the lumbar spine via a combined metabolomic-genomic approach. Methods: A population cohort of 3,584 southern Chinese underwent lumbar spine MRI. Blood samples were genotyped with single-nucleotide polymorphisms (SNP) arrays (n = 2,482) and serum metabolomics profiling using magnetic resonance spectroscopy (n = 757), covering 130 metabolites representing three molecular windows, were assessed. Genome-wide association studies (GWAS) were performed on each metabolite, to construct polygenic scores for predicting metabolite levels in subjects who had GWAS but not metabolomic data. Associations between predicted metabolite levels and MC phenotypes were assessed using linear/logistic regression and least absolute shrinkage and selection operator (LASSO). Two-sample Mendelian randomization analysis tested for causal relationships between metabolic biomarkers and MC. Results: 20.4% had MC (10.6% type 1, 67.2% type 2, 22.2% mixed types). Significant MC metabolomic biomarkers were mean diameter of very-low-density lipoprotein (VLDL)/low-density lipoprotein (LDL) particles and cholesterol esters/phospholipids in large LDL. Mendelian randomization indicated that decreased VLDL mean diameter may lead to MC. Conclusions: This large-scale study is the first to address metabolomics in subject with/without lumbar MC. Causality studies implicate VLDL related to MC, noting a metabolic etiology. Our study substantiates the field of “spino-metabolomics” and illustrates the power of integrating metabolomics-genomics-imaging phenotypes to discover biomarkers for spinal disorders, paving the way for more personalized spine care for patients.

Small vertebral size is a well-known risk factor for vertebral fractures. To help understanding the factors behind vertebral size, we aimed to investigate whether physical activity and participation in high-impact exercise are associated with the growth rate of the vertebral cross-sectional area (CSA) among young adults. To conduct our study, we utilized the Northern Finland Birth Cohort 1986 as our study population (n = 375). Questionnaire data about physical activity was obtained at 16, 18 and 19 years of age and lumbar magnetic resonance imaging scans at two timepoints, 20 and 30 years of age. We used generalized estimating equation (GEE) models to conduct the analyses. We did not find any statistically significant associations between vertebral CSA, physical activity, and high-impact exercise in our study sample. We conclude that neither physical activity nor high-impact sports seem to influence the change in vertebral CSA among young adults.

Background context: There is often discrepancy between clinical presentation and lumbar magnetic resonance imaging (MRI) findings. Purpose: The purpose of this study was to assess the relationship of high-intensity zones (HIZs) on MRI with low back pain (LBP), sciatica, and back-related disability. Study design: Cross-sectional, population-based Southern Chinese cohort study. Patient sample: Of 1,414 possible participants, data from 1,214 participants (453 males, 761 females; mean age of 48.1±6.3 years) were included. Outcome measures: Presence of single-level, homogeneous multilevel (same type HIZs of morphology and topography) and heterogeneous multilevel (mixed type HIZs of morphology and topography) HIZs and other MRI phenotypes were assessed at each level with T2-weighted 3T sagittal MRI of L1–S1. Low back pain (LBP) is one of the major disabling health conditions among older adults aged 60 years or older. While most causes of LBP among older adults are non-specific and self-limiting, seniors are prone to develop certain LBP pathologies and/or chronic LBP given their age-related physical and psychosocial changes. Unfortunately, no review has previously summarized/discussed various factors that may affect the effective LBP management among older adults. Accordingly, the objectives of the current narrative review were to comprehensively summarize common causes and risk factors (modifiable and non-modifiable) of developing severe/chronic LBP in older adults, to highlight specific issues in assessing and treating seniors with LBP, and to discuss future research directions. Existing evidence suggests that prevalence rates of severe and chronic LBP increase with older age. As compared to working-age adults, older adults are more likely to develop certain LBP pathologies (e.g., osteoporotic vertebral fractures, tumors, spinal infection, and lumbar spinal stenosis). Importantly, various age-related physical, psychological, and mental changes (e.g., spinal degeneration, comorbidities, physical inactivity, age-related changes in central pain processing, and dementia), as well as multiple risk factors (e.g., genetic, gender, and ethnicity), may affect the prognosis and management of LBP in older adults. Collectively, by understanding the impacts of various factors on the assessment and treatment of older adults with LBP, both clinicians and researchers can work toward the direction of more cost-effective and personalized LBP management for older people.

Osteoporotic vertebral fractures are common among ageing populations worldwide. Although small vertebral size has been established as an independent risk factor for vertebral fracturing, relatively few determinants of vertebral size are currently known. The present study aimed to reveal lifetime factors that associate with midlife vertebral size. Overall physical activity across the lifespan, sports participation in adulthood, occupational physical activities in adulthood, and physical growth in early life were investigated. A subsample of the Northern Finland Birth Cohort Study 1966 was used, with a 46-year follow-up (n = 1,540). Vertebral dimensions were obtained by magnetic resonance imaging of the lumbar spine at the age of 46. The present data showed that high lifelong leisure-time physical activity and active participation in high-impact sports in adulthood were associated with large midlife vertebral size among women, but no such association was detected among men. Occupational physical activities were not associated with vertebral size among either sex. Early-life weight gain predicted large midlife vertebrae among both sexes, and the effect of height gain on vertebral size seemed to be mediated by adult height. The present findings show that the female vertebra in particular seems to benefit from high leisure-time physical activity and active participation in high-impact sports. Regardless of sex, early development also seems to play a role in determining later-life vertebral size. Prospective studies should confirm the causality of the present findings, and further research is needed to shed light on other lifetime factors as determinants of vertebral size. Pienen nikamakoon tiedetään lisäävän nikamamurtuman riskiä, mutta nikamakokoon vaikuttavia tekijöitä tunnetaan toistaiseksi varsin vähän. Tämän tutkimuksen tavoitteena on selvittää keski-iän nikamakokoon vaikuttavia elinaikaisia tekijöitä. Erityisesti tutkitaan vapaa-ajan liikunnallisuuden, lajikohtaisen harrastusaktiivisuuden, työn liikunnallisten piirteiden sekä lapsuuden ja nuoruuden fyysisen kehityksen yhteyttä nikamakokoon. Tutkimusjoukkona käytetään osaotosta Pohjois-Suomen vuoden 1966 syntymäkohortista (n = 1 540). Seuranta-aika on ollut 46 vuotta, ja selkänikaman koko on määritetty tutkittavista 46 vuoden iässä otetuista lannerangan magneettikuvista. Tutkimusaineistossa naisten elinaikainen vapaa-ajan liikunnallisuus sekä aktiivinen luustoa kuormittavien liikuntalajien harrastaminen ovat yhteydessä suureen nikamakokoon keski-iässä. Miesten liikunnallisuuden ja nikamakoon välillä ei sen sijaan havaittu vastaavaa yhteyttä, eivätkä työn liikunnalliset piirteet olleet yhteydessä nikamakokoon kummallakaan sukupuolella. Varhainen painon kasvu ennusti suurta keski-iän nikamakokoa sukupuolesta riippumatta, ja varhaisen pituuskasvun vaikutus nikamakokoon näytti välittyvän aikuispituuden kautta. Tutkimuksen tulosten perusteella siis erityisesti naiset näyttäisivät hyötyvän vapaa-ajan liikunnallisesta aktiivisuudesta ja luuta kuormittavien lajien harrastamisesta aikuisiällä. Sukupuolesta riippumatta myös lapsuuden ja nuoruuden fyysisellä kehityksellä näyttäisi olevan merkitystä keski-iän nikamakokoon. Tulevaisuudessa prospektiivisten tutkimusasetelmien tulisi vahvistaa tässä tutkimuksessa havaittujen yhteyksien kausaliteetti. Lisäksi tutkimusta tarvittaisiin jatkossa muiden nikamakokoon vaikuttavien elinaikaisten tekijöiden selvittämiseksi.
