Abstract

Introduction: Salbutamol and terbutaline are short-acting β2 adrenergic agonists that produce bronchial smooth muscle relaxation and are widely used in obstructive pulmonary diseases. Nevertheless, their use has been the cause of a paradoxical bronchoconstriction, which is a rare and potentially serious adverse reaction. The aim of this study is to report a case of paradoxical bronchoconstriction caused by β2 adrenergic agonists. Methods: This case is about a 50-year-old asthmatic patient who describes a history of repeated acute asthma attacks after salbutamol inhalation or terbutaline nebulization. A double-blind crossover study was performed over 3 days, in order to compare the effects of each bronchodilator. Forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC), and maximal expiratory flow 25-75 (MEF25-75) were measured. Results: On the first day, a bronchoconstriction caused by deep and repeated inhalations was eliminated. On the second day, an airway obstruction was confirmed by a decrease in FEV1 at 40% from baseline values after nebulization of a standard dose of terbutaline. On the third day, a spirometry was performed before and after nebulization of a standard dose of ipratropium bromide, and there were no significant changes in the spirometric parameters. Finally the patient was discharged with a written warning mentioning the danger of salbutamol and terbutaline use. Conclusion: Salbutamol and terbutaline are generally well-tolerated β2 adrenergic agonists. Nevertheless, in rare cases, these substances can cause a paradoxical bronchoconstriction. Doctors must therefore remain vigilant about its side effect and possibly investigate each case.

Abstract

Introduction: Chronic Obstructive Pulmonary Disease (COPD) can lead, during its evolution, to muscle mass loss or obesity. The aim of the study was to assess the impact of the body mass index on the functional severity of COPD. Methods: In this study 197 patients were included. Body mass index (BMI) was calculated. COPD was confirmed by spirometry and reversibility test and classified according to GOLD 2010. Results: In the study population, the median age was 64 years (32-89) with a sex-ratio at 0,68.The median BMI was 26,4 kg/m2 (15,4-52,2). Underweight was present in 8,6%, overweight in 24,9% and obesity in 28,4% of patients. COPD was classified in: stage 1 (n=21), stage 2 (n=86), stage 3 (n=67) and stage 4 (n=23). Abstract

Introduction: ACOS shares clinical and spirometric features of asthma and COPD. Several ACOS definitions exist. FeNO is a marker of eosinophilic airway inflammation commonly used to diagnose and monitor allergic asthma. Recently, it was proposed to set apart COPD and ACOS. Aim of the study: Evaluate FeNO contribution to distinguish COPD from ACOS. Methods: This study included spirometry files with forced expiratory volume in one second (FEV 1 ) over forced vital capacity (FVC) after bronchodilator 1 and/or FVC. Subjects were interviewed and underwent FeNO measurement. Among the population, a group underwent a blood test for eosinophils counting. Results: Thirty-two patients were enrolled (28 males). Mean age was 57.93 [36-79]. All subjects were active or past cigarette smokers or exposed to other toxics or wood smoke. Twenty subjects were suspected to have asthma, twelve to have COPD. Only 4 subjects had an increased FeNO (10.93 parts per billion [1 – 48]). Thus, the other 28 subjects were classified as COPD. There was a negative correlation between FeNO level and smoking (p=0.001). There was a positive correlation between FeNO and FEV 1 improvement after bronchodilator in percentage (21.31% [0-52]) (p=0.001) and in milliliter (345.31 ml [0-880]) (p=0.0003). Eosinophils were counted in 17 cases (219.79/mm3 [8-616]) and were positively correlated to FeNO (p=0.032). Conclusion: Reversibility in FEV 1 and/or FVC associated to FeNO and eosinophils counting seem to be more reliable to differenciate COPD and ACOS. More studies are needed to help for diagnosis in daily practice.
