1. Dynamics design of a non-natural transcription factor responding to androst-4-ene-3,17-dione.

Abstract

The production of androst-4-ene-3,17-dione (AD) by the steroidal microbial cell factory requires transcription factors (TFs) to participate in metabolic regulation. However, microbial cell factory lacks effective TFs that can respond to AD in its metabolic pathway. Additionally, finding and obtaining natural TFs that specifically respond to AD is a complex and onerous task. In this study, we devised an artificial TF that responds to AD, termed AdT, based on structure-guided molecular dynamics (MD) simulation. According to MD analysis of the conformational changes of AdT after binding to AD, an LBD in which the N- and C-termini exhibited convergence tendencies was used as a microswitch to guide the assembly of a DNA-binding domain lexA, a linker (GGGGS)2, and a transcription activation domain B42 into an artificial TF. As a proof of design, a AD biosensor was designed and constructed in yeast on the basis of the ligand-binding domain (LBD) of hormone receptor. In addition, the transcription factor activity of AdT was increased by 1.44-fold for its variant F320Y. Overall, we created non natural TF elements for AD microbial cell factory, and expected that the design TF strategy will be applied to running in parallel to the signaling machinery of the host cell. Abstract

Human pharmaceuticals have been shown to be entering the aquatic environment in quantities sufficient to produce adverse effects to aquatic organisms, particularly fish. The impacts of synthetic oestrogens have been well documented, but other groups of steroidal pharmaceuticals have not yet been studied. Hence, the present research was designed to study synthetic glucocorticoids (GCs), which are used in large amounts as immunosuppressive and anti-inflammatory drugs. This study involved different approaches, including in silico, in vitro, in vivo and genomics, to assess the effects of GCs on fish. Using reliable data on consumption of GCs in the UK and the LF2000-WQX hydrological model, mean concentrations of GCs in the river Thames were predicted to be in the range from 30 ng/L to 850 ng/L. Mammalian cell lines were transiently transfected with trout corticosteroid receptors (GR1, GR2 and MR) and the transactivation abilities of ten of the most prescribed GCs in the UK were measured in vitro. All tested GCs showed significantly higher activity with GR2 than with GR1. In order to assess the impact of low concentrations of GCs in vivo, two chronic exposure experiments were conducted with adult fathead minnows (Pimephales promelas). Both experiments showed potency-related and concentration-related impacts on various endpoints. There was a concentration-related increase in plasma glucose concentrations and a decrease in blood lymphocyte count. Induction of secondary sexual characters in females suggests a concentration-related masculinisation of fathead minnows. There was a decreasing trend in plasma vitellogenin concentrations in female fish with increasing exposure concentration of GCs. Expression profiles of selected genes (PEPCK, GR and Vtg) in liver also demonstrated concentration-related effects at all three tested concentrations. Hence, it was not possible to define a no effect concentration for the tested GCs. This study probably provides reliable estimates of the likely range of concentrations of GCs in a typical river, impacted by effluent from many sewage treatment plants. The in vitro results indicate that all tested GCs bind to fish GR in a similar manner to that reported for mammalian receptors. The in vivo results suggest that GCs could cause effects at very low (as low as 100 ng/L) concentrations that could be environmentally-relevant. The immunosuppresive effects could make fish susceptible to disease and the reproductive effects may have population-level impacts. It is very likely that the effects of different GCs will be additive, as has been shown for oestrogenic chemicals. Therefore, this study warrants further environmental risk assessment of GCs, especially in mixture scenarios.

Abstract

Background: The connection between corticotherapy and neuropsychiatric symptoms is widely known, being one of the first questions we need to assess when presenting with first episode psychiatric symptoms or confusional state. Aims: To date, data on cases related to inhaled corticotherapy and neuropsychiatric effects is scarce. In this paper we describe a rare case in a young woman. Methods: The clinical case presented led us to try to understand the data published on the subject in order to discuss it in greater length. Results and Conclusions: We present and discuss a 27-year-old patient’s case, with no previous psychiatric disease, who was admitted to our Psychiatric ward after the onset of severe acute behavioural disturbance characterized by aggressiveness, visual and auditory hallucinatory activity, misidentification and altered conscience status. It was later found that seven days earlier she had been prescribed inhaled corticotherapy for a minor respiratory infection. A few days after corticotherapy withdrawal, the clinical symptoms improved significantly.

4. Synthesis of Uscharin Oxime Analogues and Their Biological Evaluation as HIF‐1 Inhibitors.

Abstract

Uscharin‐like non‐classical cardenolides are considered to be useful as HIF‐1 inhibitors in the treatment of cancers. To develop more potent HIF‐1 inhibitors, oximes and oxime ethers of uscharin were designed and synthesized. Oximation of uscharin exhibited unexpectedly a regioselectivity and stereoselectivity at 3′‐thiazoline but not at the 19‐aldehyde group, since either E‐ketoxime derivative 2 or its ether 5 at C‐3′ was found as the only product during oximation of uscharin using hydroxylamine or methoxyamine at room temperature. Two 19‐aldoxime derivatives 4 and 6 were also synthesized. Ketoxime analogue 2 showed more potent inhibitory effect on HIF‐1 transcriptional activity than the parental uscharin. Ketoxime ether 5 and 19‐aldoxime 6 exhibited stronger HIF‐1 suppressing effect than digoxin (the positive control drug). Moreover, ketoxime derivative 2 displayed the most potent anti‐proliferative activity against MCF‐7 cancer cells among all compounds. An analysis of structure‐activity relationship revealed that 3′‐ketoxime is a superior moiety to a thiazoline ring, while 19‐aldoxime is inferior to an aldehyde counterpart. Orbital apex syndrome (OAS) is a rare disease that presents with a complex of symptoms, including ophthalmoplegia, ptosis and visual loss. Due to the poor prognosis, making a prompt diagnosis and administering the appropriate treatment must be initiated without delay if OAS is suspected. We report here on a case of a patient with sphenoid fungal balls, and he presented with acute visual loss and ophthalmoplegia.

7. Development and evaluation of a patient-reported outcome measure (PROM) to assess the impact of steroids associated with therapy for multiple myeloma

Abstract

Multiple myeloma (MM) is a life-threatening, complex, chronic and incurable blood cancer. Glucocorticosteroids (steroids) form the backbone of all treatment regimens. Described as hard to tolerate, steroids cause a range of side effects which adversely impact health-related quality of life (HRQoL), yet have received limited attention. This thesis aims to address the issues around steroid-associated effects (StAEs) by exploring what is known about how patients experience StAEs, and to develop a PROM to be used in the clinical setting to improve patient care. A multi-phase study using international guidelines for PRO tool development was undertaken using pragmatic research methodology. The three phases were: • Phase I: Qualitative interviews • Phase II: Conceptual framework, item generation and establishing content validity • Phase III: First field test Steroid Symptom Questionnaire Multiple Myeloma (SSQ-MM) Phase one provided insight into the day to day reality for individuals with MM and their family of living with StAEs. Interviews highlighted the difficulties in distinguishing the source of symptoms, and greater difficulties in managing these toxicities. Findings from phase one and the literature review were used to inform the development of a conceptual framework that describes how MM and its treatment, including steroids, impact the individual. This conceptual framework formed the basis of a new steroid PROM (SSQ-MM) which was tested and further developed using expert patient, carer and clinician groups. In phase three the measurement properties of the SSQ-MM are demonstrated that include feasibility, acceptability, content and face validity and reliability. Information on StAEs prevalence and severity is also reported. For the first time, data supporting the significant symptom burden associated with StAEs in those with MM is presented. The researcher was approached by a colleague in Norway for use of the SSQ-MM in a Nordic MM Group clinical trial. The SSQ-MM has undergone translation into Danish, Finnish and Norwegian. A study is underway to further test the measurement properties aiming to validate the SSQ-MM for translation into clinical practice.

Abstract: This study was aimed to improve knowledge about swine ovarian follicular function, paying attention to angiogenesis, since new vessel growth is a fundamental event in ovarian function. In particular, we investigated a potential involvement of netrin-1, a protein known as a guidance axon factor. Firstly, we studied the expression and immunolocalization of netrin-1 in swine ovarian follicle and its effect on cultured swine granulosa cell viability and steroidogenesis. Furthermore, aortic endothelial cells were employed to verify a possible netrin-1 effect on angiogenesis. Our data demonstrate the expression and the presence of netrin-1 in swine follicular fluid; in addition, it was shown that netrin-1 inhibits granulosa cell viability and estradiol 17β levels while it stimulates progesterone production. Netrin-1 also inhibits aortic endothelial cell growth in the angiogenesis bioassay. This effect appears to be mediated by inhibiting vascular endothelial growth factor and stimulating nitric oxide. Abstract: The applicability of comprehensive two-dimensional gas chromatography (GC×GC) for sterol analysis was investigated by separation and identification of endogenous sterols in standards, and spiked in human urine. The modulation temperature was optimized to achieve the best separation and signal enhancement. The separation pattern of trimethylsilyl (TMS) derivatives of sterols was compared on two complementary column sets. Whilst the BPX5/BPX50 column set offers better overall separation, BPX50/BPX5 provides better peak shape and sensitivity. Comparison of the identification power of GC×GC–TOFMS against both the NIST05 MS library and a laboratory created (in-house) TOFMS library was carried out on a free sterols extract of urine, derivatised and spiked at the World Anti-Doping Agency (WADA) limit of 2ngmL−1. The average match quality for 19 analysed sterols on the BPX50/BPX5 column set was 950/1000 when searched against the in-house library; only four were identified against the NIST05 library, at a match threshold of 800. The match quality of GC×GC–TOFMS spectra was superior to that for analysis using 1D GC–TOFMS for sterols spiked in urine at 10ngmL−1. An r 2 >0.997 was obtained for the concentration range between 0.25ngmL−1 and 10ngmL−1 for three selected sterols. The lowest limit of detection (LOD) was obtained for estrone (0.1ngmL−1) and the highest LOD was for 5α-androstan-3α,11β-diol-17-one, epitestosterone and cholesteryl butyrate (1ngmL−1), using a match threshold of at least 800 and signal-to-noise ratio of at least 10. TOFMS coupled to GC×GC enabled satisfactory identification of sterols in urine at their LOD. A minimum acceptable match (MAM) criterion for urinary sterols using 2D retention times and TOF mass spectra is introduced. This study shows that GC×GC–TOFMS yields high specificity for steroids derived from urine, with detection limits appropriate for use in doping control. [Copyright &y& Elsevier]

Liquid chromatography coupled to tandem mass spectrometry has been used for the detection and the structural characterization of T-rich model oligonucleotides covalently modified by estradiol-2,3-quinone. After separation by gradient elution, adducts were analyzed by negative electrospray mass spectrometry, enabling to evidence and localize the modifications in the oligonucleotide sequence. Modifications by one molecule of estrogen were evidenced on purines (A, G) whereas no reaction was observed on pyrimidic bases (T). Isomeric adducts were differentiated using tandem mass spectrometry, and energy resolved mass spectrometry allowed to underline differences in the behavior of the adducts towards collisional excitation into an ion trap device. Abstract

Leptin is a hormone secreted mainly by the adipose cells with a primary role in the regulation of body weight by establishing a feedback loop between the energy reserves and the hypothalamic centers that control food intake. Recent data suggest that, in addition, leptin interacts with other endocrine systems to provide critical information about the size of the fat stores, acting as a permissive factor that allows the triggering of energy-demanding situations, as the onset of puberty and the reproduction, only when the size of the fuel reserve is large enough to guarantee its success. In addition, leptin appears to play a role during pregnancy and lactation, as it is produced by the placenta and is present in maternal milk. The fact that leptin levels are always higher in females, even after correcting for body fat content, suggests that the interaction between the adipose tissue and the reproductive system is modulated in a different way in males and females by androgenic and estrogenic hormones. In fact, adipose tissue samples taken from male donors are completely refractory in vitro to the action of both estrogens and androgens. On the contrary, dihydrotestosterone, androstenedione and dehydroepiandrosterone-S are potent inhibitors of leptin secretion, while estradiol induces a strong stimulation in adipose tissue taken from women. The article presents a case study of a 19-month-old toddler with perinatally acquired hepatitis C infection. The child is positive with hepatitis C virus (HCV) RNA and HCV antibody and got her infection from her mother who had a history of intravenous drug use. The article discusses the use of steroids as a treatment for type 2 autoimmune hepatitis which may exacerbate the HCV infection.

Seven undescribed C21 steroids, namely cynanchin A – G , together with thirteen known analogues, were isolated from the roots of cynanchum otophyllum. Their structures were elucidated by 1D, 2D NMR and MS spectra, as well as chemical methods. Meanwhile, all of isolates were tested for their anti-hepatic fibrosis activity. Among them, compounds 4–6 , 10–12 and 14–17 showed moderate or significant inhibitory effects for the proliferation of hepatic stellate cells (HSCs) induced by transforming growth factor- β 1 (TGF- β 1) in vitro. Seven undescribed steriods together with thirteen known compounds were isolated from the roots of Cynanchum otophyllum. Some of them showed moderate or marked anti-hepatic fibrosis activity. The present study aims to evaluate the effectiveness of different treatment regimens on BCT-induced lung injury. Methods: A total of 35 Sprague Dawley rats were divided into five experimental groups (n = 7): sham, control; BCT; BCT + MP, BCT group treated with methylprednisolone (MP; 30 mg/kg on first day and 3 mg/kg/d on the following days); BCT + Q, BCT group treated with quercetin (Q; 50 mg/kg/d for seven days); and BCT + MP + Q, BCT group treated with the same doses of MP and Q. Serum Clara Cell Protein-16 (CC-16), thiobarbituric acid reactive substances (TBARS), and superoxide dismutase (SOD) levels were analyzed to determine histopathological changes in the lung tissues. Results: Elevated serum CC-16 and TBARS levels and reduced serum SOD levels were found in the BCT group compared to the Sham group. There was a significant change in the serum CC-16 levels in the BCT + MP group compared to the Sham group. Serum TBARS levels were significantly lower in the BCT + MP and BCT + Q group compared to the BCT group. The combined therapy regimen yielded significantly decreased CC-16 levels and increased serum SOD levels compared to the individual treatment groups. Serum TBARS levels did not significantly differ between the BCT + MP + Q group and the other treatment groups. Compared to the BCT + MP + Q group, the BCT + MP group showed significantly lower alveolar edema (AED) and alveolar exudate (AEX) scores, while the BCT + Q group showed significantly lower peribronchial inflammatory cell infiltration (PICI) and AED scores. Conclusions: The combined usage of quercetin and low dose MP treatment after initial high dose MP at the early stage of lung injury after BCT is more effective.

Abstract

El objetivo de la investigacion fue analizar morfologicamente los sitios de implantes endometriales sobre la region externa de la musculatura de la pared abdominal anterior de ratones, tratados con dexametasona. Para ello, fueron utilizados 15 ratones albinos (Rattus norvergicus albinus), del linaje Wistar, de 90 dias de edad, los cuales fueron sometidos a induccion de la endometriosis y divididos en los siguientes grupos: Grupo I ­ ratones inducidos a la endometriosis y evaluados tras 21 dias (control); Grupo II ­ ratones inducidos a la endometriosis y tras 21 dias, tratados con dexametasona durante 10 dias; Grupo III ­ ratones inducidos a la endometriosis, y tras 21 dias, tratados con dexametasona durante 15 dias. La dexametasona fue administrada en dosis de 0,8mg /dia/animal. Nuestros resultados muestran que el proceso inflamatorio cronico en la endometriosis no baja despues del tratamiento con dexametasona durante 10 dias, mientras que el tratamiento con dexametasona durante 15 dias cambio del proceso inflamatorio cronico en la endometriosis y estimulo la proliferacion de glandulas en los implantes endometriales

Abstract

The present research had the purpose to analyze morphologically the sites of endometrial implants on the external area of the musculature of the anterior abdominal wall of female rats, treated with dexamethasone. For so, these 15 albino female rats were used (Rattus norvegicus albinus), of the lineage Wistar with 90 days of age, that were submitted to the induction of the endometriosis, and divided in the following groups: Group I induced female rats to endometriosis and evaluated after 21 days (control); Group II induced female rats to endometriosis and after 21 days, treated with dexamethasone for 10 days; Group III - induced female rats to endometriosis, and after 21 days, treated with dexamethasone for 15 days. The dexamethasone was administered in the dosage of 0,8 mg/day/animal. Our results showed that the chronic inflammatory process in the endometriosis doesn't decrease after treatment with dexamethasone for 10 days, while the treatment with dexamethasone for 15 days reverted the chronic inflammatory process in the endometriosis, besides stimulating the proliferation of glands in endometrial implants El objetivo de la investigación fue analizar morfológicamente los sitios de implantes endometriales sobre la región externa de la musculatura de la pared abdominal anterior de ratones, tratados con dexametasona. 21. Persistent cough in children as a presenting feature of asthma

Abstract

Introducción: Los estudios epidemiológicos sugieren que la presencia de tos es un mal indicador de asma. Objetivo: Revisión en torno a la presencia de tos persistente como único síntoma respiratorio y su eventual relación con asma en niños. Material y Métodos: Se realizó una búsqueda bibliográfica considerando revisiones o artículos originales que evalúen la presencia de tos persistente en niños, brindando un modelo de manejo para ésta condición. Resultados: Las vías de la tos y de la broncoconstricción no son las mismas, por lo que no es un síntoma confiable para establecer el diagnóstico de asma en niños. La mayoría de veces obedece a un problema no específico que se resuelve de manera espontánea. Debe eliminarse la exposición a alergenos como tabaco. En ocasiones se emplean descongestionantes o antihistamínicos por periodos cortos y eventualmente antibióticos orales. Se recomienda medir la función pulmonar en aquellos casos, que se pueda. No existe evidencia que guíe el uso de esteroides inhalados, sin embargo, en ocasiones se recomiendan iniciar dosis moderadamente altas por 8-12 semanas; si no hay mejoría, el diagnóstico de asma es razonablemente improbable. Conclusiones: La mayoría de niños con tos persistente no desarrollarán asma por lo que no se beneficiarán de la administración de corticoides inhalados. El término asma necesita ser interpretado con cuidado a la luz de los distintos fenotipos de asma en la infancia Introduction: Epidemiological studies suggest that the presence of cough indicates a poorer prognosis in children. Objective: To review the presence of cough as the only respiratory symptom and its eventual relationship with asthma. Methods and patients: We realized a bibliographic search of reviews and original articles evaluating the presence of persistent cough in children and those suggesting a treatment model for this condition. Results: Mechanisms of cough and bronchoconstriction are not the same, consequently persistent cough alone is not sufficiently reliable to make the diagnosis of asthma in children. In the majority of cases it is due to a non-specific problem that spontaneously resolves. The elimination of alergens or contamination such as tabacco smoke should be avised, as well as sometimes the use of decongestants or antihistamines for short periods and eventually oral antibiotics. It is recommended that pulmonary function should be measured when possible. There is no evidence to guide the use of inhaled steroids, but on occasions the use of moderately high doses for 8-12 weeks should be advised, if there is no improvement the diagnosis of asthma is improbable. Conclusions: The majority of children with a persistent cough do not develop asthma, for this reason they do not benefit from the use of inhaled steroids. The use of the word asthma should be used with caution in light of the different phenotypes of asthma in childhood
