Background: Health-care system reform in Indonesia is challenging for anesthesiologists in determining the minimum cost of anesthesia and maintaining inflammation and pain to a minimum. This study is aimed to analyze the effectiveness of general anesthesia techniques between target-controlled inhalational anesthesia (TCIA) sevoflurane and target-controlled infusion (TCI) propofol in mastectomy surgery from the perspective of cost, C-reactive protein (CRP) levels, postoperative pain level, postoperative opioid consumption, and side effects. Patients and Methods: This was a randomized controlled trial with permuted block randomization of 60 subjects allocated into TCIA sevoflurane (Group S) and TCI propofol (Group P). In Group S, we used sevoflurane 8 vol% for induction and maintained with sevoflurane 1–1.5 vol%, compressed air, and oxygen. In Group P, we used TCI propofol with target effect 4 mcg/mL and maintained with TCI propofol (target effect of 1–3 mcg/ml), compressed air, and oxygen. Results: The cost in Group S was US$ 36.33 compared to US$ 29.69 in Group P (P = 0.002). The CRP level was comparable between the two groups (38.39 [42.13] vs. 23.88 [45.26]. P = 0.487). There is neither difference in pain score, total morphine consumption, and side effects between both groups in the first 24-h postoperative period, nor morphine consumption in 24 h postoperative. Conclusion: The cost of anesthesia-related expense in TCI propofol is lower than TCIA sevoflurane in mastectomy surgery underwent general anesthesia. There is no significant difference in terms of CRP levels, postoperative pain, postoperative morphine consumption, and incidence of side effects.

Recent data suggest that the type of anesthesia used during the resection of solid tumors impacts the long-term survival of patients favoring total-intravenous-anesthesia (TIVA) over inhalative-anesthesia (INHA). Here we sought to query this impact on survival in patients undergoing resection of glioblastoma (GBM). All patients receiving elective resection of a newly diagnosed, isocitrate-dehydrogenase-1-(IDH1)-wildtype GBM under general anesthesia between January 2010 and June 2017 in the Department of Neurosurgery, Heidelberg University Hospital, were included. Patients were grouped according to the applied anesthetic technique. To adjust for potential prognostic confounders, patients were matched in a 1:2 ratio (TIVA vs. INHA), taking into account the known prognostic factors: age, extent of resection, O-6-methylguanine-DNA-methyltransferase-(MGMT)-promoter-methylation-status, pre-operative Karnofsky-performance-index and adjuvant radio- and chemotherapy. The primary endpoint was progression-free-survival (PFS) and the secondary endpoint was overall-survival (OS). In the study period, 576 patients underwent resection of a newly diagnosed, IDH-wildtype GBM. Patients with incomplete follow-up-data, on palliative treatment, having emergency or awake surgery; 54 patients remained in the TIVA-group and 417 in the INHA-group. After matching, 52 patients remained in the TIVA-group and 92 in the INHA-group. Median PFS was 6 months in both groups. The median OS was 13.5 months in the TIVA-group and 13.0 months in the INHA-group. No significant survival differences associated with the type of anesthesia were found either before or after adjustment for known prognostic factors. This retrospective study supports the notion that the current anesthetic approaches employed during the resection of IDH-wildtype GBM do not impact patient survival. We describe an unusual case of hyperacute hepatic failure following general anesthesia in a patient receiving a simultaneous kidney-pancreas transplant. Despite an aggressive evaluation of structural, immunological, viral, and toxicological causes, a definitive cause could not be elucidated. The patient required a liver transplant and suffered a protracted hospital course. We discuss the potential causes of fulminant hepatic failure and the perioperative anesthesia management of her subsequent liver transplantation. Resumo: Descrevemos um caso incomum de insuficiência hepática hiperaguda após a anestesia geral em uma paciente que recebeu um transplante simultâneo de rim-pâncreas. Apesar de uma avaliação agressiva das causas estruturais, imunológicas, virais e toxicológicas, uma causa definitiva não pôde ser identificada. A paciente precisou de um transplante de fígado que resultou em prolongamento da internação hospitalar. Discutimos as potenciais causas da insuficiência hepática fulminante e o manejo da anestesia no período perioperatório de seu subsequente transplante de fígado. OBJECTIVE To develop a technique for permanent sterilisation of female eastern grey kangaroos (Macropus giganteus) and red kangaroos (M. rufus) as part of a large-scale macropod management program on an enclosed 1545-ha site in western Sydney. METHODS Free-ranging female kangaroos (n = 1409: 1285 eastern grey kangaroos, 124 red kangaroos) were anaesthetised via remote anaesthetic drug delivery of tiletamine/zolazepam, medetomidine and acepromazine prior to inhalational anaesthesia using isoflurane-oxygen. A laparoscopic ovariectomy technique was developed using standard laparoscopic equipment to effect permanent sterilisation of the kangaroos. The technique described was also adapted for use on immature animals weighing as little as 1 kg. No direct post-surgical care was provided once the animals had recovered from the anaesthetic. RESULTS The procedure was simple to perform and had a very high success rate, with an overall project mortality rate of 2.13% (n = 30). Seven kangaroos (0.05% of all operated kangaroos) were euthanased as a direct result of the surgical procedure. Surgical complications were rare but included inadvertent gastrointestinal tract puncture with the trocar, intraoperative haemorrhage and subcutaneous emphysema leading to pouch eversion following surgery. CONCLUSION The procedure described is a rapid and effective method of permanent fertility control in macropods and carries a low mortality rate.

Background: The effects of intravenous and inhalation anesthesia on intraoperative and postoperative pulmonary inflammatory responses have been reported in many studies. The differences in clinical postoperative pulmonary complications (PPCs) have been also studied in cardiac and lung resection surgery. However, there are few relevant reports and the findings remain controversial. Clinical evidence for the effects of these two anesthetics on PPCs in other types of surgery is still missing. The main goal of the current study was to assess the impact of sevoflurane and propofol on the incidence of PPCs in patients undergoing oral and maxillofacial surgery. Methods: In this double-blind, randomized, controlled trial, we randomly assigned 220 adults at intermediate-to-high risk of pulmonary complications after oral and maxillofacial cancer surgery with radial forearm or fibular flap reconstruction to either propofol or sevoflurane as a general anesthetic. The occurrence of pulmonary complications according to the Clavien-Dindo score was defined as the primary (within 7 days after surgery) outcome. Results: The two intervention groups had similar characteristics at baseline. The PPCs incidence during 7 days after surgery was 32.4% and 18.2% in the propofol and sevoflurane groups, respectively (adjusted relative risk, 0.44; 95% confidence interval [CI], 0.22 to 0.91; P = 0.027). The corresponding incidence of PPCs in patients who underwent tracheotomy at the end of surgery in the two groups was 44.8% and 24.5% (adjusted relative risk, 0.39; 95% CI, 0.17 to 0.91; P = 0.030). In addition, the Clavien-Dindo classification showed significant differences between groups in minor complications (grade I and II) but not in major complications (grade III to V). Intergroup difference in the time to occurrence of the first PPC after surgery was significant (P = 0.021). There was no difference in postoperative hospital stay between the two groups. Conclusions: Compared with intravenous anesthesia, the administration of sevoflurane reduces the incidence of minor PPCs (grade I to II) in moderate- and high-risk patients who have undergone tracheotomy after oral and maxillofacial cancer surgery with radial forearm or fibular flap reconstruction.

To compare the effects on the brain metabolism of propofol-remifentanil total intravenous anesthesia and desflurane-remifentanil intravenous inhalational anesthesia in neurosurgery. Methods: Thirty-four neurosurgical patients with intracranial space-occupying surgery were randomly divided into two groups: Group A was conducted with the propofol-remifentanil total intravenous anesthesia and Group B with desflurane-remifentanil intravenous inhalational anesthesia. The patients were monitored for the brain metabolism prior to anesthesia, 1 hour after surgery, 2 hours after surgery, and half an hour after surgery. Background Propofol and remifentanil intravenous combination is one popular form of total intravenous anesthesia (TIVA) in mainstream clinical practice, but it has rarely been applied to a rat model for laryngoscopy and laryngeal electromyography (LEMG). Our objective was to establish a safe and reproducible general anesthetic protocol for laryngoscopy and endoscopic LEMG in a rat model. Our hypothesis is that TIVA allows a minimally morbid, and feasible laryngoscopy and LEMG. Methods Sprague Dawley rats were subjected to either inhalational anesthesia (IA) (isoflurane) or TIVA (propofol and remifentanil) and underwent laryngoscopy and LEMG. The primary outcome was a complete minimally interrupted rigid laryngoscopy and obtaining reproducible motor unit potentials from the posterior cricoarytenoid muscles. The secondary outcome was morbidity and mortality. Results Seventeen out of twenty-two rats underwent both TIVA and IA. Only two underwent IA only. All nineteen rats that underwent IA had a successful experiment. Seventeen rats underwent TIVA, however, only nine completed a successful experiment due to difficulty achieving a surgical plane, and respiratory events. Upon comparing the success of the two anaesthetic regimens, IA was superior to TIVA (P = 0.0008). There was no statistical difference between the amplitudes (p = 0.1985) or motor units burst duration (p = 0.82605) of both methods. Three mortalities were encountered, one of which was due to lidocaine toxicity and two were during anesthetic induction. Respiratory related morbidity was encountered in two rats, all seen with TIVA. Conclusions TIVA is not an ideal anesthetic regimen for laryngeal endoscopy and LEMG in rat models. Contrary to our hypothesis, IA did not affect the quality of the LEMG and allowed a seamless rigid endoscopy. Aims: The aims of our study were (1) to compare success of probing done blindly and probing done under endoscopic assistance (2) to correlate the findings of blind probing with endoscopic examination findings. Materials and Methods: Retrospective comparative interventional case study. Total 98 eyes of 84 children were included in the study. The probing was performed under brief inhalational anaesthesia with a laryngeal mask. All probings, secondary and in children older than 24 months, were performed under endoscopic assistance only. Results: Total 111 probings were done out of which, ninety-eight (88.29%) were primary and 13 (11.71%) secondary. Seventy-six (68.4%) probings were done without and thirty-five(31.53%) probings were done with nasal endoscopic assistance. Out of 35, 22 (19.81%) probings were primary and 13 (11.71%) secondary. The mean age for probing was 18.2 +/12.5 months.Thirty eight (45.23%) were females and 46 (54.77%) were males. Out of 76 primary probings, 13 (17.10%) probings failed. The mean age of failed primary probing was 15.2 +/6.7 months. All 22 primary probings done under endoscopic guidance had successful outcome. Out of the total 111 probings, 2 (1.80%) probings failed who underwent endonasal dacryocystorhinostomy in the same sitting. Conclusion: Use of simultaneous nasal endoscopy during probing increases the success rate, minimizes secondary procedures and isof prognostic value for children with Congenital nasolacrimal duct obstruction (CNLDO).

Appropriate and effective anesthesia is critical, because it has a strong influence on laboratory animals, and its affect greatly impacts the experimental data. Inhalational anesthesia by endotracheal intubation is currently prevailing in general anesthesia and is prefered over injection anesthesia, especially for large laboratory animals, because it is a safe and easy control agent. However, it is not common for small laboratory animals, because of the high degree of technical skills required. We assessed the capability of use for mice of the endotracheal intubation by using the endoscope system “TESALA AE-C1” and inhalational anesthesia using a ventilator. Endotracheal intubation was successfully performed on all 10 C57BL/6 mice injected with M/M/B: 0.3/4/5 comprised of medetomidine, midazoram and butorphanol, at a dose of 0.3 mg/kg + 4.0 mg/kg + 5.0 mg/kg body weight/mouse, respectively. After the intubated mice were connected with the inhalational anesthesia circuit and the ventilator, vital signs were measured until 15 min after the connection. The data with M/M/B: 0.3/4/5 showed stable and normal values, which indicated that this new endotracheal intubation method was simple, reliable and safe, which mean that this anesthesia is favorable in regard to the animal’s welfare.

Even shallow residual neuromuscular block [i.e. train-of-four (TOF) ratio around 0.6] is harmful. It can be effectively antagonised by small doses of neostigmine, but reports are limited to intravenous anaesthesia. Inhalational anaesthesia may enhance neuromuscular block and delay recovery. It is not known whether low doses of neostigmine are still effective in the context of inhalational anaesthesia.To assess the effectiveness of low doses of neostigmine to antagonise shallow atracurium block during desflurane anaesthesia.Randomised controlled trial, four groups.Single centre, University Hospital, May 2010 to March 2011.Forty-eight American Society of Anesthesiologists I-III patients undergoing desflurane anaesthesia.At TOF ratio 0.6, patients were randomised to one of four treatments (physiological saline, 10, 20 or 30 µg kg(-1) neostigmine, n = 12 for each).Primary efficacy endpoint: time interval between study drug injection and a TOF ratio more than 0.9 using acceleromyography. Secondary efficacy endpoint: neuromuscular recovery after 5 and 10 min.After physiological saline, the time interval [median (range)] between a TOF ratio of 0.6 and 0.9 was 14 (7 to 18) min. After 10, 20 and 30 µg kg(-1) neostigmine, it was reduced to 5 (3 to 8) min, 5 (3 to 10) and 4 (2 to 6) min, respectively (P0.001 compared to physiological saline). At 5 min after physiological saline, the TOF ratio [mean (SD)] was 0.73 (0.05) and 0.91 (0.06), 0.90 (0.10), 0.96 (0.02) after neostigmine 10, 20 or 30 µg kg(-1), respectively (P0.01 compared to physiological saline). At 10 min after physiological saline, the TOF ratio was 0.86 (0.08) and 1.0 (0), 0.98 (0.03), 1.0 (0) after neostigmine 10, 20 or 30 µg kg(-1), respectively (P0.01 compared to physiological saline).Under desflurane anaesthesia, neostigmine 10 µg kg(-1) is effective in antagonising shallow atracurium block. Introduction: Paediatric cochlear implantation has become an established procedure for treating irreversible hearing loss. The aim of this survey was to review the peri-operative anaesthetic management of this programme since its inception. Methods: We conducted a retrospective clinical audit of 184 paediatric (age Results: A total of 184 procedures (including five bilateral cases) were performed on 173 patients. The average age (mean [standard deviation, range]) at the time of surgery was 3.7 (±2.6, 0.7–11) years. The mean duration of the unilateral and bilateral implantation procedures were 3.2 (±1.0, 1.2–6.5) hours and 4.7 (±0.8, 4–6) hours respectively. Inhalational anaesthesia (66.3%) was the most popular choice for induction. 15 (8.1%) patients required additional analgesia in the post-anaesthetic care unit. In the ward, 14 patients (7.6%) required stronger analgesia in addition to paracetamol. Peak paracetamol requirement (22.9 [±16.6] mg/kg) was on the first postoperative day. Nausea and vomiting was reported in 19 (10.3%) procedures. The correlation between increasing age and post-operative nausea and vomiting was statistically significant (pConclusion: Cochlear implantation in paediatric patients is a relatively safe procedure involving few complications.

A Lacaune ewe-lamb was referred with bilateral pinnal swelling and intense head-shaking. Soft, fluid-filled, painless swellings were present on the concave surface of both ear pinnae. Needle aspiration revealed sero-sanguineous fluid, confirming the clinical suspicion of auricular haematomas. Cutaneous lesions characteristic of sarcoptic mange were present on the nasal planum; diagnosis was confirmed based on the results of microscopic examination of skin scrapings (i.e., presence of Sarcoptes scabiei ); for treatment of the disorder, moxidectin was administered twice, 11 days apart. The animal was operated under injectable anaesthesia. Auricular haematomas were drained by using Penrose tubes. However, and despite repeated post-surgical otic cleansing and administration of broad-spectrum antibiotics, local infection developed post-operatively. Subsequently, a second operation was performed, under inhalational anaesthesia, where the incisional technique was applied. Post-operative care included otic cleansing and broad-spectrum antibiotic administration, coupled with light bandaging of the ears. After 40 days, the animal recovered completely; sarcoptic mange was successfully treated and no relapse of the haematomas was seen. During the subsequent breeding period, the ewe was mated for the first time and became pregnant. The findings support the hypothesis that the highly pruritic sarcoptic mange induced head-shaking and self-inflicted traumas of the pinnae, resulting in the development of auricular haematomas. Treatment required extensive surgical and long post-operative care. From a health management viewpoint, the above treatment schedule can only be justified in young, high-value animals.

Background Adequate surgical field visualization is imperative for successful outcomes in endoscopic sinus surgery (ESS). The type of anesthetic administered can alter a patient's hemodynamics and impact endoscopic visualization during surgery. We review the current evidence regarding the effect of total intravenous anesthesia (TIVA) compared to inhalational anesthesia (INA) on visualization of the surgical field during ESS. Methods A systematic review of the literature was performed. Ovid MEDLINE, Scopus, and Cochrane databases were searched from 1946 to January 2012. Citations from the primary search were reviewed and filtered to identify all relevant abstracts in English. Articles meriting full review included prospective controlled trials enrolling adult patients undergoing ESS that were randomized to a group receiving INA or TIVA with outcome measures focused on surgical field visualization. Results Seven eligible trials fulfilled inclusion criteria. Four of the 7 demonstrated a statistically significant improvement in surgical field grade during ESS when receiving TIVA compared with INA. However, detailed INA concentrations were often not provided. High levels of INA may have been administered; therefore, side effects of INA rather than effects of an ideal INA administration were possibly represented. Analgesic administration also varied widely among the anesthetic groups, further complicating interpretation of study results. The lack of power and the heterogeneity of the studies precluded a formal meta-analysis. Conclusion Although several studies reported that TIVA improves surgical conditions in ESS, there are significant limitations. These findings prevent any definite recommendation at this point, emphasizing the need for further high-quality studies.

Background and purpose Drooling is a challenging entity to manage. Botulinum toxin A (BOTOX-A) infiltration of salivary glands is a promising alternative to surgical treatment. This study aims to assess the outcome of BOTOX-A salivary glands infiltration in children with drooling. Methods Patients treated between January 2012 and March 2015 were enrolled. BOTOX-A was injected in the parotid and submandibular glands under ultrasound control and general inhalational anesthesia. The outcome was evaluated through the DSFS: Drooling Severity (1-best to 5-worst) and Frequency (1 to 4) Scale, that was applied before treatment, and 1-, 3-, and 6-month after injection. The inclusion criteria were a DSS ≥4 and/or DFS ≥3. Statistical significance was set at 5%. Results There were 17 patients aged 12.1±5.1 [4–19]years, all of them with neurologic impairment. After the first injection, 13 (76.5%) patients had reduction of the severity (S) and 12 (70.6%) of the frequency (F) scale; in 6 (35.5%) patients drooling resolved completely. Pre-treatment S+F score was 8.59±0.71 [7–9]; it decreased significantly to 4.65±2.32 ( p =0.001) at 1-month post-injection evaluation. At 3-month and 6-month the scores were also significantly lower than the pre-treatment one (4.00±1.96, p =0.002; 5.36±2.20, p =0.005; respectively), but there was a significant increase between the 3-month and 6-month evaluations ( p =0.01). With a follow-up of 20.1±9.2 [4–38] months, 4 out of the 13 successful injections needed a second one after 7.5±3.1 [3–10] months. The patient with the longest time not requiring re-injection had 28months of follow-up. One (6%) patient presented mild dysphagia that regressed spontaneously. All but two (88%) parents/caregivers would repeat the treatment. Conclusions BOTOX-A seems to be an effective minimal invasive treatment for drooling with few complications. After 6months the need for re-injection becomes substantial but it may not be necessary for several months. Further studies are needed to establish the most effective dosage and frequency of injections. Level of evidence IV

Resumo Os rebanhos de ovinos do Centro-Oeste do Brasil, principalmente da raça Santa Inês, aumentaram muito nos últimos anos. Dessa forma, a casuística clínica também cresceu e a realização de cirurgias de grande porte sem dor e com segurança se faz presente, associada ao uso da anestesia geral inalatória. Este trabalho objetivou avaliar os parâmetros cardiopulmonares e hemogasométricos em ovinos sob anestesia inalatória com ventilação controlada, bem como avaliar a influência de alternâncias de decúbito nos mesmos parâmetros. Os animais foram sedados com acepromazina, induzidos com propofol, intubados e colocados sob anestesia inalatória com isoflurano durante manutenção anestésica. As frequências cardíaca e respiratória, pressão arterial e a concentração de gases sanguíneos foram aferidas a cada cinco minutos por um total de 45 minutos, durante os quais os animais foram alternados entre decúbito lateral direito, dorsal e esquerdo. Os parâmetros aferidos não apresentaram diferenças significativas entre os tempos e decúbitos, concluindo-se que a anestesia geral inalatória com isoflurano mostra-se viável e segura na espécie ovina, pois mantém os parâmetros cardiopulmonares e hemogasométricos em níveis seguros. Abstract Sheep herds, especially Santa Ines breed, have grown in the Brazilian Midwest in recent years. Therefore, clinical cases have also grown, and along came the need of conducting major surgeries without pain associated with the use of inhalational anesthesia. This study evaluated the cardiopulmonary and blood gas parameters in sheep under inhalational anesthesia with controlled ventilation, and assessed the effect of decubitus alternations on the parameters. The animals were anesthetized with acepromazine and propofol, intubated and placed on inhalational anesthesia with isoflurane. The respiratory rates, blood pressure and blood gas concentrations were measured every for minutes for a total of 45 minutes during which the animals were alternated right, dorsal and left decubitus. The measured parameters showed no significant differences among the times and positions, concluding that inhalational anesthesia with isoflurane is valid and safe for sheep, as it keeps the cardiopulmonary parameters and arterial blood gases at safe levels.

Survival in warm renal ischemia models is not only dependent on the treatment or surgical technique being evaluated, but also on factors inherent to the model itself. Use of rats of various strains in previous studies makes interstudy comparison difficult when trying to design an appropriate model control that would yield intermediate survival. In this study, impact of rat strain on survival after prolonged warm renal ischemia in the setting of delivery-controlled inhalational anesthesia was evaluated.Under general delivery-controlled inhalation anesthesia with isoflurane, Dahl salt-sensitive, Wistar-Furth, Sprague-Dawley, and spontaneously hypertensive rats (n = 66 rats) were subjected to 150 minutes of unilateral renal warm ischemia time, subsequent reperfusion, and contralateral nephrectomy. Animals were followed up for 1 month, after which survivors were euthanized and morphologic changes in kidneys were scored.Thirty-day survival was: Dahl salt sensitive, 78%; Wistar-Furth, 67%; Sprague-Dawley, 55%; and spontaneously hypertensive rats, 0% (P.0001). Histologic acute injury scores were higher for non-survivors versus 30-day survivors (P.0001).Our data strongly suggest that rat strain is a major factor influencing survival and that strain and warm ischemia time selections must be considered together when designing a model control yielding intermediate survival. Further study is warranted in order to compare the effect of delivery-controlled inhalational versus historical anesthesia methods on animal survival.

The laryngeal mask airway (LMA) has been used in various animal species anesthetized for the purpose of device evaluation, but the device has not been evaluated in rabbits during surgery. The authors tested the feasibility and potential advantages of using the LMA in 50 rabbits undergoing surgery under spontaneous-breathing inhalational anesthesia, focusing mainly on the technique of insertion and its efficacy. The LMA was easily inserted and no air leakage at the larynx was detected. Although four rabbits developed lingual cyanosis, this was reversible and most likely due to lingual vascular compression by the LMA. The authors conclude that the LMA is an attractive alternative to endotracheal intubation, as the mask can be inserted easily and rapidly and its correct placement is easily confirmed.

Introduction: Orthognathic treatment has assumed an important role in orthodontics and maxillofacial surgery in the last years; however, little has been investigated about this type of treatment. Objectives: The main purpose of this study was to identify major factors and/or predictors associated with postoperative pain (PP) and hospital length of stay (LOS) after orthognathic surgery. Materials and methods: 52 patients who underwent orthognathic surgery from 2008 to 2010 at the University Hospital of São Paulo University were investigated. Study variables such as patient characteristics, preoperative, intraoperative and postoperative data were collected. The outcome variables were PP and LOS. Descriptive and analytical statistics was computed for all variables. Results: 27.6% of patients had pain in the postoperative period. Lidocaine used in general anesthesia was an associated factor of PP. The overall mean LOS was 2 days; gender, location of procedure, type and duration of anesthesia were identified as probable predisposing factors. There was a significant correlation between anesthesia time and discharge. Anesthesia variables were more predictably related with postoperative pain and hospitalization time. Location of orthognathic procedure, however, was an important surgical variable that influenced in LOS. Conclusions: Intravenous lidocaine boluses used during general anesthesia were associated with PP. Male patients, single-jaw surgery, inhalational anesthesia and duration of anesthesia were predisposing factors that improve LOS. Introducción: la cirurgía ortognática ha asumido un papel importante en la ortodoncia y en la cirugía maxilofacial en los últimos años. Sin embargo, se ha investigado poco sobre este tipo de tratamiento. Objetivos: el objetivo principal de este estudio fue identificar los principales factores o predictores asociados con el dolor posoperatorio (DP) y el tiempo de internación (TI) después de la cirugía ortognática. Materiales y métodos: fueron estudiados 52 pacientes que se sometieron a cirugía ortognática (2008 - 2010) en el Hospital Universitario de la Universidad de São Paulo. Se recolectaron variables de estudio tales como características de los pacientes y datos perioperatorios. Estadística descriptiva y analítica se calculó para todas las variables. Resultados: el 27,6% de los pacientes presentaron dolor en el posoperatorio. La lidocaína utilizada en la anestesia general fue un factor asociado del DP. El TI medio global fue de 2 días. El género, la localización del procedimiento, el tipo y la duración de la anestesia se identificaron como factores predisponentes probables. Hubo una correlación significativa entre el tiempo de anestesia y de descarga. Las variables de anestesia podían predecirse más cuando estaban relacionadas con el DP y el TI. La localización de la cirurgía ortognática, sin embargo, fue una variable quirúrgico importante que influyó en lo TI. Conclusiones: la lidocaína intravenosa en bolo utilizada durante la anestesia general se asoció con el DP. Los pacientes varones, la cirugía sobre una sola mandíbula, la anestesia inhalatoria y la duración de la anestesia fueron factores predisponentes que prolongaron el TI.

Paediatric airway management is a great challenge even for an experienced anaesthesiologist. Difficult airway in huge cervical teratoma further exaggerates the complexity. This case report is intended at describing the intubation difficulties that were confronted during the airway management of a three year old girl presenting with huge neck teratoma and respiratory distress. This patient was successfully intubated with uncuffed endotracheal tubes in second attempt under inhalational anaesthesia with halothane and spontaneous ventilation. This case exemplifies the importance of careful preoperative workup of an anticipated difficult airway in paediatric patients with neck swelling to minimize any perioperative complications.

Purpose To compare operating room and hospital discharge times between adult strabismus surgeries using intravenous propofol sedation with local anesthesia versus those using general anesthesia. Methods Thirty adult patients underwent uncomplicated strabismus procedures performed by one surgeon using propofol sedation and local subtenon's anesthesia. These were retrospectively matched with adult patients undergoing similar procedures by another surgeon using general inhalational anesthesia. Only one muscle had undergone previous surgery, and no adjustable sutures were used. Times from incision closure to leaving the operating room (Out OR time), and to hospital discharge (DC time) are compared. Results Propofol/local Out OR times ranged 2 to 8 minutes (mean, 4.8). General anesthesia Out OR times ranged from 3 to 28 minutes (mean, 8.8) (means differ at P P P P Conclusions Many adult strabismus surgeries may be performed more efficiently with intravenous propofol sedation and local subtenon's anesthesia than with general anesthesia. Times from the end of surgery to leaving both the OR and the hospital are decreased compared with those of general anesthesia. Extreme delays are rare with propofol/local, but they occurred with general anesthesia.

Whether opioids administered before skin incision, under inhalational anesthesia, improve cardiovascular and plasma catecholamine responses to surgical stimulation compared with those administered after skin incision remains unclear. We compared the effects of fentanyl injected before and after skin incision on these responses.We studied 50 healthy female patients [American Society of Anesthesiologist (ASA) physical status 1] who underwent elective total abdominal hysterectomy through an infraumbilical incision (midline incision) under nitrous oxide (60%)-oxygen-isoflurane (1.2%) anesthesia. Fentanyl (2.0 or 4.0 microg.kg(-1)) was administered i.v. 5 min before (pretreatment group) or 5 min after (posttreatment group) skin incision. Control patients received a saline injection. Heart rate (HR) and mean arterial blood pressure (MAP) were recorded 1 min before incision and serially for 30 min afterward. Plasma levels of norepinephrine (Nor) and epinephrine (Epi) were determined 1 min before incision and serially up to 20 min after skin incision.The MAP response to incision had decreased after 10 min in posttreatment fentanyl (2 microg.kg(-1)) (P0.05) and after 8, 10, 15, and 20 min in posttreatment fentanyl (4 microg.kg(-1)) ( P0.05). At the same doses, fentanyl administered before skin incision attenuated MAP response to incision after 1 min with the smaller dose (P0.05) and after 1, 3, 5, 6, 8, 10, 15, and 20 min with the higher dose (P0.05). Fentanyl suppressed Epi response to surgery 8 and 20 min after skin incision (P0.05) at both doses, except for 8 min after incision in pretreatment fentanyl (2 microg.kg(-1)). Overall, the hemodynamic and sympathoadrenergic responses after skin incision were attenuated, with the exception of plasma Nor after fentanyl irrespective of time and dose.Our results indicated that fentanyl depressed cardiovascular and plasma catecholamine responses irrespective of the time of administration, and that the higher dose of fentanyl produced a greater suppression of MAP and HR responses. In addition, the depressant effects on MAP of high-dose fentanyl administered 5 min before skin incision lasted longer than when injected 5 min after incision. At both doses, the opioid attenuated the rise in plasma Epi, but not Nor.

Background: The use of intraoperative neuromonitoring is a well-established method of detecting neurologic injuries during spine surgery. Anaesthesia, especially inhalational agents, influence motor evoked potential (MEP) monitoring. The aim of our study was to compare the effect of balanced anaesthesia (BA) (intravenous plus inhalational anaesthesia) and total intravenous anaesthesia (TIVA) on the incidence of intraoperative neuromonitoring changes, interventions performed and neurological outcomes of patients following high-risk spinal surgery. Methods: After Research and Ethics Board approval, a retrospective review of 155 patients who underwent spinal surgery with MEP was performed. Data were collected on changes in MEP and/or somatosensory evoked potential, interventions performed and neurological outcomes. Patients were divided into BA and TIVA groups and data were analysed. Background and Aims: Increasing number of pediatric ambulatory surgeries are being carried out in general anesthesia using supraglottic airways (SGAs). Literature comparing sevoflurane and desflurane for the maintenance of SGA-based anesthesia is limited. Hence, we planned this prospective randomized study to compare the maintenance, emergence and recovery characteristics of sevoflurane and desflurane for pediatric ambulatory surgery. Material and Methods: Sixty children aged 6 months to 6 years posted for short surgical procedures were enrolled into the study. Anesthesia was induced with intravenous propofol (maximum 4 mg/kg), SGA was inserted, and children were randomized to receive sevoflurane or desflurane for the maintenance of anesthesia. No muscle relaxants were administered, and all children received caudal block and rectal paracetamol suppository. Demographic data, perioperative hemodynamics and adverse events, emergence and recovery characteristics, postoperative pain, and emergence agitation (EA) were recorded. Data were analyzed using SPSS (version 16.0, IBM Corporation, Armonk, New York, USA). P < 0.05 was considered statistically significant. Results: Demography, perioperative hemodynamics, and duration of inhalational anesthesia were comparable between two groups. There were no respiratory adverse events in either group during maintenance. Time to awakening and time to removal of SGA were shorter with desflurane (5.3 ± 1.4 and 5.8 ± 1.3 min) than sevoflurane (9.1 ± 2.4 and 10.0 ± 1.6 min) (P < 0.0001). Recovery (steward recovery score = 6) was faster with desflurane (18 ± 8.4 min) than sevoflurane (45.3 ± 9.7 min) (P < 0.001). The incidence of EA was 16.7% with desflurane and 10% with sevoflurane (P = 0.226). Conclusion: Desflurane provides faster emergence and recovery in comparison to sevoflurane when used for the maintenance of anesthesia through SGA in children. Both sevoflurane and desflurane can be safely used in children for lower abdominal surgeries.

Background and Objectives: Functional endoscopic sinus surgery (FESS) being a delicate technique, intraoperative bleeding is one of the major challenges. Even a little bleeding can adversely affect the surgeon's ability to visualize the region to be operated. General anesthesia is preferred over topical anesthesia in FESS. This study was conducted to compare the surgical field using total intravenous anesthesia (TIVA) with propofol and inhalational anesthesia with isoflurane for FESS. Secondary outcomes such as intraoperative blood loss and the incidence of perioperative complications were also recorded. Subjects and Methods: A total of sixty patients in the age group of 16–60 years with physical status American Society of Anesthesiologists Classes I and II, undergoing FESS were randomly divided into two groups of thirty each after taking informed consent and approval from the Hospital Ethics Committee. Thirty patients in Group I: received isoflurane-based inhalational anesthesia and other Thirty patients in Group II: were administered TIVA with propofol. Various parameters were recorded and statistically analyzed. Results: There was improved quality of surgical field at the end of surgery in the Group II as compared to Group I. Total blood loss during surgery and incidence of intraoperative complications were less in Group II as compared to Group I. Conclusion: This study concludes that in FESS, using TIVA with propofol decreases blood loss and the incidence of complications during surgery in addition to providing good quality of surgical field.

Inhalational anesthesia has dominated the practice of pediatric anesthesia. However, as the introduction of agents such as propofol, short-acting opioids, midazolam, and dexmedetomidine a monumental change has occurred. With increasing use, the overwhelming advantages of total intravenous anesthesia (TIVA) have emerged and driven change in practice. These advantages, outlined in this review, will justify why TIVA will supercede inhalational anesthesia in future pediatric anesthetic practice.

Background. Inhalational agents have been widely used for anesthesia in laboratory animals. However, the safe use of inhalational agents in small laboratory animals has been limited by the lack of a suitable and effective scavenging system for the removal of waste anesthetic gases. The aim of the present study is to develop an anesthetic system that can be manufactured using common household and laboratory items. Materials and methods. An anesthetic system was designed for rats weighing from 300 to 350 g. A face mask for the rat was made by cutting off the distal part of a 50-ml centrifuge tube. A scavenging hood was made from a transparent plastic food storage box. Exhaust of anesthetic gases from the scavenger hood was facilitated by fitting an outlet connected to a pump. Four experienced researchers or technicians tested the scavenger hood. Results. In 79.2% of the trials the participants could smell halothane when the pump from the scavenger system was not operational. However, when the pump was switched on, halothane was detected only 16.7% of the time (P Conclusion. We have developed a simple and effective method of delivering inhalational anesthesia to small laboratory animals and of removing waste anesthetic gases.

Purpose: To determine occupational exposure of the anesthesiologist and surgeon to nitrous oxide and desflurane during general anesthesia for ear-nose-throat (ENT) surgery in children and adults. Methods: An observational clinical trial was performed in ten children (C) and ten adults (A). Tracheas were intubated, in adults, with cuffed tubes and in children with uncuffed tubes. The operating room was equipped with modern air conditioning and waste anesthetic gas scavengers. Gas samples were obtained during the operative procedure every 90 sec from the breathing zone of subjects. Time-weighted averages (TWA) over the time of exposure were calculated for nitrous oxide and desflurane. Background: Nasolacrimal duct obstruction occurs in 5% to 6% of neonates. Many studies advocate the probing of nasolacrimal duct obstruction under general inhalational anesthesia in patients at a late age (12 to 13 months) because a high percentage will resolve spontaneously. Others support early surgical intervention in patients aged younger than 6 to 9 months without anesthesia. We present late nasolacrimal duct probing under intravenous propofol sedation as an alternative approach to the treatment of nasolacrimal duct obstruction with a decrease in cost and time compared with probing under general inhalational anesthesia. Methods: We made a retrospective review of patient charts of children who underwent nasolacrimal duct probing with intravenous propofol sedation from April 1996 to September 1997. Procedure time and cost of procedure were compared for patients who had probings under propofol sedation to patients who had probings under general anesthesia. Results: A total of 22 patients (31 eyes) underwent nasolacrimal duct probing with propofol sedation; the patients' ages ranged from 11.5 to 39 months (average age, 17.8 months). Twenty-six (84%) of 31 eyes had resolution of the symptoms. The average total time for procedure under propofol sedation was 10.5 minutes, compared with 43.6 minutes under general inhalational anesthesia. The average total recovery time under propofol sedation was 13.6 minutes, compared with 121.1 minutes with general inhalational anesthesia. The cost of probing under propofol sedation was one third less than the cost of probing under general inhalational anesthesia. Discussion: Late probing for nasolacrimal duct obstruction under intravenous propofol sedation is comparable in efficacy to late probing under general inhalational anesthesia with a shorter time for the procedure and decreased expense. Children swallow coins more than any other object. Many methods for removal of impacted coins at the upper end of the oesophagus are enumerated in literature, each having it's own merits and demerits. This retrospective study in 100 consecutive paediatric patients presents a magill forcep technique for removal of coins under inhalational anaesthesia using Mc-Intosch laryngoscope. Intubation and rigid endoscopy both were avoided, thereby minimizing hospital stay and cost of treatment. This also decreases the chances of complications. Moreover the technique was found to be at par with rigid endoscopy in term of efficacy and safety. However interestingly statistical analyses of the data by 'Chi SquareTest' (ξ(2)) revealed the technique to be more applicable in younger age group i.e. 0-6 years.

OBJECTIVE Patients will be discharged from the postoperative recovery room mostly on subjective clinical assessment. In this study an approach to a more quantitative judgment of postoperative vigilance is made by recording the P300-latency and neuropsychological tests. METHODS 22 adult patients undergoing a disc operation were examined. For induction of anesthesia thiopental (4-5 mg/kg), fentanyl (0.1 mg) and a musclerelaxant (atracurium 0.4 mg/kg or succinylcholine 1-2 mg/kg after precurarisation with atracurium 5 mg) were given. Anesthesia was then continued with enflurance (1.0-1.2 MAC) in a mixture of 67% nitrous oxide in oxygen. If postoperative analgesia was needed, piritramid was injected in boli a 3-6 mg. The P300 was acoustically stimulated with an oddball-paradigm and recorded at Fz and Cz. Afterwards the latencies were measured and compared with a vigilance score composed of clinical parameters and neuropsychological tests. Recordings were done preoperatively and every 30 minutes up to 2 hours postoperatively. A correlation between P300-latencies and vigilance score was made with the coefficient of Spearman. Comparison of pre- and postoperative values was managed by using Wilcoxon test for matched pairs with Bonferroni-correction. RESULTS Immediately after operation, P300 was obtained only in 8 patients (36%). The latencies were delayed (394 +/- 35 ms versus 326 +/- 12 ms preoperatively). During follow-up patients recovered and 2 hours postoperatively only one patient had no P300. At the end of the examination period P300-latencies of most patients had not yet reached the preoperative levels. The vigilance score in parallel showed decreases immediately after the operation and increases later on. However there were discrepancies between P300 latencies and neuropsychological findings, in some cases possibly due to the sedative effects of postoperative analgetics. CONCLUSION Recording of P300-latencies showed remarkable cognitive deficits because of subclinical anesthesia hangover even 2 hours after a routine inhalational anesthesia. It is a good quantifiable method for assessment of postoperative vigilance. In some cases P300-latency is a more sensitive parameter for vigilance phenomena than clinical and neuropsychological scores.

Summary Internal cardioverter‐defibrillator implantation can be performed under local or general anaesthesia. Whether the technique of general anaesthesia influences the defibrillation threshold remains a matter of debate. We therefore compared, in a prospective, randomised clinical study, the effect of intravenous anaesthesia using propofol with inhalational anaesthesia using isoflurane on the defibrillation threshold in 68 patients scheduled for transvenous single-lead internal cardioverter‐defibrillator implantation. Defibrillation threshold was measured at implantation and at device testing 1 week and 1 month after implantation. Patients acted as their own controls. Neither the anaesthetic technique nor the duration of anaesthesia was associated with significant changes in the defibrillation threshold. We conclude that in this group of high-risk patients, both types of anaesthesia are acceptable techniques for internal cardioverter‐defibrillator implantation and testing.

A study was undertaken to assess the performance of the Komesaroff vaporizer, placed within the circuit, in ventilated patients during maintenance of closed circuit anaesthesia with halothane or isoflurane. Following intravenous induction, anaesthesia was maintained by inhalation. This was achieved using a conventional vaporizer outside the circle for the first 10 minutes to manage the fast uptake phase. The fresh gas flow was then reduced to the basal oxygen requirement with the Komesaroff vaporizer within the circle maintaining inhalational anaesthesia. Complete isolation of the circuit was achieved by returning all anaesthetic gases to the circuit following analysis and using a bag-in-bottle ventilator. The Komesaroff vaporizer dial was positioned at between the first and second division and end-tidal volatile anaesthetic agent levels were measured. This study demonstrated that at dial positions I or 1.5 with either agent, the end-tidal volatile concentration plateaued at clinically acceptable levels. The Komesaroff vaporizer can therefore be used safely in ventilated patients to maintain closed circuit anaesthesia provided clinical observation and monitoring are meticulous.

Summary It has been suggested that large doses of opioids may suppress the interleukin-6 response to surgery. We examined the effects of the supplementation of inhalational anaesthesia with either 3 or 15 mg.kg π1 fentanyl on the circulating interleukin-6, interleukin-8, C-reactive protein, cortisol and glucose concentrations in 16 patients undergoing pelvic surgery. In both groups, surgery evoked the expected glucose, cortisol and interleukin-6 response but no increase in interleukin-8 was detected. There were no significant differences between the two groups. We conclude that the supplementation of inhalational anaesthesia with conventional doses of opioids does not modify the cytokine response to surgery.

The effects of minimally invasive surgery (MIS) on postoperative nutrition and food intake have not been reported. We compare the effects of MIS access techniques on postoperative status by analyzing the changes in body weight and food intake in an immature mouse model. Immature 5- to 7-week-old male A/J mice underwent laparotomy under general inhalational anesthesia (n = 20/group). In addition, two other groups of mice underwent MIS using either pneumoperitoneum with carbon dioxide (CO2) or gasless suspension (n = 20/group). Operative and anesthetic times were equal for all groups. Body weight and the amount of food and water intake were recorded over a 2-week postoperative period. Data were analyzed statistically by ANOVA and p < 0.05 was considered to be significant. The pain of childbirth is arguably one of the most severe types of pain a woman will endure in her lifetime. Relief of the pain of childbirth has always been controversial. Misinterpretations of biblical scripture (“In sorrow thou shalt bring forth children”) resulted in centuries of denial of pain relief, as clergy insisted that suffering in labor was consistent with divine intent.1 Fifteenth-century midwives were burned at the stake for offering pain relief during labor.2 The modern era of anesthetics began with the first demonstration of inhalational anesthesia during surgery at the Massachusetts General Hospital in 1846; a few months . . .

Guidelines for paediatric practice suggest that the laryngeal mask airway (LMA) is best inserted after a period of inhalational anaesthesia. This study compared the success rate and incidence of complications for LMA placement using two different techniques of general anaesthesia. Sixty ASA I or II patients between the ages of 19 months and 14 years of age were studied. In Group 1, anaesthesia was induced with propofol, 4 mg·kg−1, and placement of the LMA was then attempted. In Group 2, anaesthesia was similarly induced with propofol, but the lungs were then ventilated with halothane in oxygen for placement of the LMA. Although limb movement occurred more often in Group 1 (P < 0.001), there were no differences between the groups for other complications, nor for success in placing the LMA. We conclude that in paediatric patients, the LMA may be successfully placed using propofol, 4 mg·kg−1 alone.

Objective: Compare anesthesia with sevoflurane/remifentanyl (SR) to total intravenous anesthesia with propofol/remifentanyl (PR) in patients undergoing endoscopic sinus surgery for chronic rhinosinusitis in the following parameters: 1) effects on blood flow to the mucosa; 2) amount of blood loss; and 3) surgical field visualization score (SFS).Method: A total of 23 adults undergoing endoscopic sinus surgery for chronic rhinosinusitis within a 6-month period were randomized to receive SR or PR. The SFS was based on the Boezaart scale. Sinonasal mucosa blood flow was measured by optical rhinometry (OR). Blood loss was recovered with the Neptune Waste Management System.Results: The groups were evenly distributed, with similar clinical characteristics and Lund-Mackay score. Summary The effects of pollution from waste anaesthetic gases are still unclear, but they may cause adverse effects in both exposed operating-room personnel or have a more global impact on the environment. A standardization of measuring operating-room pollution has been developed utilizing a time averaged value. The amount that occurs varies greatly, although is drastically reduced by scavenging. There appears to be little evidence that it increases the incidence of tumours in exposed workers. However, there is some evidence suggesting that it may have unfavorable effects on pregnant women and, recently, it has been suggested that it may reduce conception rates. There is evidence of a small effect on the immune system, although this is probably of no clinical relevance. Its effects on the environment are mostly benign as the volatile anaesthetics are ozone friendly. However, N 2 O does increase the ‘greenhouse' effect, although anaesthetic use, when compared with other methods of N 2 O production, is small. Overall, the evidence for a severe problem with pollution from anaesthetic waste gases is small.

The French health policy PMSI project (Program for a Medical Information System) is mainly aimed at development of cost analysis based on diagnosis related groups. An indicator of the relative costs of anaesthetics, the relative complexity index (ICR beta), was defined as the result of a computation of different indexes of surgical and anaesthetic procedures such as duration, patients ASA status, degree of emergency, stay in the recovery room. The aims of this study were 1) to analyze relative part of each index in ICR beta value and in ICR beta variance and 2) to assess the value of ICR beta as a cost or a complexity index. The study included 14,435 anaesthetics analyzed with the Dunn and Clarks multiple linear regression. Mean ICR beta was 267.28. The duration of the procedure accounted for 46% of the ICR beta value and surgery for 25%. The anaesthesia and the recovery accounted for 15% of the ICR beta value, but only for 0.0341 and 0.0347 respectively of the ICR beta variance. The product of the surgical procedure index by the ASA status index accounted for 0.259 of the ICR beta variance and the duration index for 0.650. The comparison of homogeneous groups with regard to the surgical procedure shows that intravenous anaesthesia accounts for 12.4% of the ICR beta value, inhalational anaesthesia for 13.84%, but exceeds 15% for regional anaesthesia (44.5% for caudal anaesthesia). For appendectomies (n = 114) duration index is still an important parameter in ICR beta variance (delta R2 = 0.354) but equivalent to the emergency index (delta R2 = 0.363).(ABSTRACT TRUNCATED AT 250 WORDS)
