Background: Unsafe injection practices pose significant risks to both patients and healthcare providers, constituting a critical issue in global healthcare ensuring the safety and appropriateness of injection practices is imperative. Aim & Objective: This study aimed to comprehensively assess the safety of injection practices across various healthcare facilities. Methodology: A Descriptive survey was conducted during March to December-2021. Non random Purposive sampling technique was adopted to select health care facilities. Total 26 healthcare facilities were selected from Government & private sectors. The study areas were OPDs and indoor wards. An observational checklist used to collect data. Data analysis was done using SPSS-28. Results: A comprehensive assessment revealed commendable adherence to injection safety practices. No deficiencies were found in the availability of resources and materials necessary for injection practices in hospitals. However, certain areas for improvement were identified. Interestingly, the practice of using a fresh pair of gloves for injections was unnoticed in 33% of government facilities and 45% of private facilities, suggesting a possible lapse in infection control protocols. Conclusion: While the healthcare facilities of Surat exhibit commendable adherences to certain aspects of injection safety, there remain opportunities for enhancement. Addressing implementing regular training programs for healthcare providers are crucial steps toward optimizing injection safety practices and ensuring the well-being of both patients and providers. Strategies to address misinformation and hesitancy about vaccines, including the fear of needles, and to overcome obstacles to access, such as the refrigeration that some vaccines demand, strongly suggest the need to develop new vaccine delivery technologies. But, given widespread distrust surrounding vaccination, these new technologies must be introduced to the public with the utmost transparency, care, and community involvement. Two emerging technologies, one a skin‐patch vaccine and the other a companion dye and detector, provide excellent examples of greatly improved delivery technologies for which such a careful approach should be developed in order to increase vaccine uptake. Aims and objectives: To determine the amount, size and characteristics of glass particulate contamination inside single‐dose ampoules. Background: Glass particulate contamination in an ampoule when opened can cause adverse effects on critically ill patients. Design: Crossectional descriptive study. Methods: Eight hundred 10‐ml sterile water ampoules were inspected for glass particulate contamination upon opening. Ampoules were opened by nurses following their usual practices. Glass particulates were inspected by stereomicroscope. Further examination was conducted using a scanning electron microscope (SEM) to determine the size and amount of the particulates in the 20 positive and 20 negative samples from stereomicroscope inspection. The Strengthening the Reporting of Observational Studies in Epidemiology checklist was used for preparing this manuscript. Results: Seven hundred and ninety‐eight ampoules were inspected, as 2 ampoules were broken. Glass particulates were detected in 65% of the ampoules (519/798). The size of glass particulates from 20 positive and 20 negative samples, ranged from 8 to 172 microns and 8 to 102 microns, respectively. The glass particulates were detected most at sized ≤ 50 microns in both positive and negative samples with the mean number of 47.8 ± 20.4 and 27.8 ± 21.8 particulates, respectively. Conclusion: Glass particulate contamination occurred while opening medication in glass ampoules. A safety tool is needed to prevent glass particulates from contaminating injection treatments. Relevance to clinical practice: Many critically ill patients and neonates need intravenous injections for treatment. Almost all medication solutions are stored in glass ampoules. Nursing personnel should be aware while drawing medication solutions from glass ampoules. Introduction: Insulin pen devices and disposable plastic insulin syringes are two common tools for insulin administration. This study aims to compare the simplicity, convenience, safety, and cost-effectiveness of insulin pens versus syringe devices in patients with type 2 diabetes mellitus (T2DM). Methods: A cross-sectional study was conducted at 14 diabetes clinics throughout Bangladesh from November 2021 to April 2022 among adults with T2DM injecting insulin by pen devices or disposable insulin syringes at least once a day for at least one year by purposive sampling. The simplicity, convenience, and safety of insulin devices were assessed using a structured questionnaire, and the study subjects were scored based on their answers; higher scores indicated a poorer response. Total scores for simplicity, convenience, and safety were obtained by adding the scores for relevant components. Their average monthly medical expense and cost of insulin therapy were recorded. The median values of the total scores and monthly expenses were compared between pen devices and disposable syringe users. Results: 737 subjects were evaluated; 406 were pen users, and 331 were vial syringe users. The pen users had lower median scores for simplicity [6.0 (5.0–8.0) vs. 7.0 (5.0–9.0), p = 0.002], convenience [4.0 (3.0–6.0) vs. 5.0 (4.0–6.0), p < 0.001], and safety [7.0 (6.0–8.0) vs. 7.0 (6.0–9.0), p = 0.008] than vial syringe users. Pen devices were more expensive than vial syringes in terms of average medical expense per month [BDT 5000 (3500–7000) vs. 3000 (2000–5000), p < 0.001], the total cost of insulin therapy per month [BDT 2000 (1500–3000) vs. 1200 (800–1700), p < 0.001] and cost per unit of insulin used [BDT 2.08 (1.39–2.78) vs. 0.96 (0.64–1.39), p < 0.001]. Non-significant differences in favor of pens were observed in HbA1c levels [8.7 (7.8–10) vs. 8.9 (7.9–10)%, p = 0.607] and proportions of subjects having HbA1c < 7% (6.9 vs. 6.3%, p = 0.991). Conclusion: Insulin pens are simpler, more convenient, and safe but more expensive than vial syringes. Glycemic control is comparable between pen and syringe users. Background: In nursing practice, flushing the catheters pre and post-drug administration is considered an important clinical procedure to prevent complications, and requires the use of several syringes to comply with international standards of care. We envisioned an innovative double-chamber syringe that enables the filling and administration of both solutions. Following current international recommendations, the development of new medical devices should integrate Health Technology Assessment. The Human-centred design is usually used for that assessment purposes, as a method that actively include end-users in the devices development process. Method: Application of the Human-Centred Design through the involvement of nurses in the initial stages of the device development in order to accomplish the initial stages of Technology Readiness Level. A multi-method approach was used, including literature/guidelines review, focus groups with end-users and expert panels. Results: The involvement of nurses enabled the definition of user requirements and contexts of use, as well as the evaluation of design solutions and prototypes in order to accomplish with usability and ergonomic features of the medical device. Over the course of the twentieth century, as hospitals cleaned up, they came to produce more and more rubbish. Beginning in the 1970s and gaining pace in the 1980s and 1990s, single-use plastic items (syringes, blood bags, tubing) saturated everyday medical practice across the globe. This essay brings the question of plastic to bear upon the longer history of twentieth century sanitary science. The widespread adoption of single-use disposable medical plastics consolidated a century's worth of changes in medical hygiene. As strange as it may seem today, the initial uptake of medical plastics was not driven primarily by concerns about hygiene. Plastic began as a mid-century technology of convenience and durability. It was not until the end of the twentieth century that it morphed into a powerful symbol and instrument of medical hygiene. Today, both patients and practitioners have embraced plastic as an indispensable technology of clean medicine. The procession of single-use medical plastics through everyday medicine now comprises a constant, if disposable, infrastructure of medical hygiene. This new processional infrastructure of disposable hygiene has produced another, albeit unintended, consequence. This new regime has exponentially increased hospitals’ material outputs. Background: The purpose of this study is to report experience with a disposable plastic syringe mold with Interceed™ for vaginoplasty in patients with vaginal aplasia. Case: The current study reports six patients diagnosed as having the Mayer-Rokitansky-Kuster-Hauser syndrome. The mold was prepared with a disposable plastic syringe (20 mL) with Interceed wrapped over it. After creation of neovagina, the mold wrapped with Interceed was secured in the neovagina. On the seventh day after operation, the originally placed mold (mold wrapped with Interceed) was replaced with mold without Interceed. Since this mold is lighter and easier to adjust, it was less stressful for the patients to master the procedure. Average operation time was 20 minutes with average blood loss being 10 mL. Average hospital stay was 10 days. The mean length of the neovagina one week postoperation was 8 cm with two fingers in width in all patients. No remarkable postoperative complications were noted. At 2 months after surgery, the neovagina was confirmed to be completely epithelialized in all patients assessed by Schiller's test. Conclusions: This innovative surgical procedure using a disposable syringe plastic mold wrapped with Interceed seems a more useful and patient compliant approach for the treatment of vaginal agenesis. The value of compliance of disposable elements such as infusion lines and syringes can significantly affect the performance characteristics of infusion devices. In a technique used to determine the compliance of infusion lines, the item under test is placed in a semi sealed water filled enclosure and the volume of fluid displaced is measured on an analytical weighing balance as set pressures are applied to the test item. Thermal drift of the measurement chamber was minimised by inclusion of discs of Invar alloy. Measurements were also made to determine the compliance of syringes used with syringe drivers where the volume of fluid displaced upon release of established pressure levels within a syringe was measured using a separate measurement configuration using again an analytical weighing balance. These techniques were used to determine the relative contributions to compliance of infusion lines and syringes to the dynamics of operation of syringe drivers. This indicated that contributions to compliance from syringes were significantly greater than that of connected infusion lines. Purpose: To demonstrate pharmacokinetic (PK) comparability for a single dose of 600 mg subcutaneous (SC) trastuzumab, administered via a novel single-use injection device (SID) or handheld syringe in 119 randomized healthy male subjects. Methods: The co-primary PK endpoints area under the time-concentration curve from the start of dosing to day 22 (AUC) and maximum observed trastuzumab serum concentration ( C) were dose-normalized and body-weight-adjusted, and compared using geometric mean ratios (GMRs). SID performance, injection site pain, adverse events, and antidrug antibodies (ADAs) were assessed. Results: GMRs and 90 % confidence intervals (CIs) were 1.01 (0.96-1.07) for AUC and 1.02 (0.96-1.10) for C, which fell within the prespecified bioequivalence range (0.80-1.25). No SID quality issues or failures occurred. Adverse events were mostly mild, with no deaths, adverse event-related withdrawals, or life-threatening, cardiac, or serious events reported. The ADA rate was low, and no neutralizing antibodies were detected. Conclusions: Trastuzumab SC via SID demonstrated comparable PK and safety to handheld syringe administration. Abstract: Background: Jet injectors (JIs) avoid safety drawbacks of needle–syringe (N–S) while generating similar immune responses. A new generation of disposable-syringe jet injectors (DSJIs) overcomes the cross-contamination risk of multi-use-nozzle devices used in 20th-century campaigns. In the first study in humans, the newly-US-licensed LectraJet® model M3 RA DSJI was compared to N–S. Methods: Sixty healthy adults received one 0.5mL intramuscular dose of the 2009–2010 seasonal, trivalent, inactivated influenza vaccine (TIV) in randomized, double-masked fashion by either DSJI (n =30) or N–S (n =30). Adverse reactions were monitored for 90 days after injection, and serologic responses assayed by hemagglutination inhibition (HI) at days 28 and 90. Results: There were no related serious adverse events (SAEs), nor differing rates of unsolicited AEs between DSJI and N–S. Solicited erythema and induration occurred more often after DSJI, but were transient and well-tolerated; a trend was noted for fewer systemic reactions by DSJI. Pre-vaccination HI geometric mean titers (GMT) increased by 28 days for H1N1, H3N2, and B antigens by 13-, 14-, and 8-fold via DSJI, and by 7-, 10-, and 7-fold for N–S, respectively. No trending differences in GMT, seroconversion, or seroprotection were noted; sample sizes precluded non-inferiority assessment. Conclusions: DSJI delivery of TIV is well-tolerated and immunogenic. Abstract: Background: Disposable-syringe jet injectors (DSJIs) have the potential to deliver vaccines safely and affordably to millions of children around the world. We estimated the incremental costs of transitioning from needles and syringes to delivering childhood vaccines with DSJIs in Brazil, India, and South Africa. Methods: Two scenarios were assessed: (1) DSJI delivery of all vaccines at current dose and depth; (2) a change to intradermal (ID) delivery with DSJIs for hepatitis B and yellow fever vaccines, while the other vaccines are delivered by DSJIs at current dose and depth. The main advantage of ID delivery is that only a small fraction of the standard dose may be needed to obtain an immune response similar to that of subcutaneous or intramuscular injection. Cost categories included were vaccines, injection equipment, waste management, and vaccine transport. Some delivery cost items, such as training and personnel were excluded as were treatment cost savings caused by a reduction in diseases transmitted due to unsafe injections. Results: In the standard dose and depth scenario, the incremental costs of introducing DSJIs per fully vaccinated child amount to US$ 0.57 in Brazil, US$ 0.65 in India and US$ 1.24 in South Africa. In the ID scenario, there are cost savings of US$ 0.11 per child in Brazil, and added costs of US$ 0.45 and US$ 0.76 per child in India and South Africa, respectively. The most important incremental cost item is jet injector disposable syringes. Conclusion: The incremental costs should be evaluated against other vaccine delivery technologies that can deliver the same benefits to patients, health care workers, and the community. AbstractObjective:Several different durable or disposable insulin pen delivery devices are currently available, and newer, improved devices are being introduced. One prefilled insulin device, FlexPen (FP), has recently been improved (known as the Next Generation FlexPen (NGFP) in Europe or the improved FlexPen in the United States). The aim of this review is to summarize the clinical and health economic data of FP and its modified version.Methods:Relevant clinical and health economic terms relating to insulin pens were used to search Medline for studies and other publications involving FP and NGFP.Results:Sixteen publications investigating FP and/or the NGFP were identified. Patients prefer FP and are more confident with its use in comparison to vial/syringe insulin administration: in a study of 105 patients with type 1 or type 2 diabetes, 85 of patients found FP to be more discreet for use in public than a syringe, 74 of patients found FP to be easier to use overall and 82 of patients had more confidence with setting the correct dose with FP. Four publications investigated the dosing accuracy of FP or NGFP: all studies found the study doses for both were within ISO-specified limits. Pharmacoeconomic issues with insulin pen devices were identified in four papers, and switching to FP from vial/syringe was found to increase treatment adherence from 59 to 68 (p < 0.01), as measured by medication possession ratio. Switching to FP is also a cost-effective option for patients. Mean all-cause annual treatment (−1748/patient, p < 0.01), hypoglycaemia-attributable costs (−908/patient, p < 0.01), and other diabetes-attributable costs (−643/patient, p < 0.01) were reduced following the switch from vial/syringe.Conclusions:Some limitations of traditional insulin administration devices can be overcome with insulin pen devices. AbstractBackground:To determine the preferable method for self-injecting insulin, we compared the handling, safety, and dose accuracy of a conventional disposable syringe and vial with FlexPen®(Novo Nordisk A/S, Bagsvaerd, Denmark), a prefilled pen.Methods:Insulin therapy-naive healthcare professionals (HCPs) (n= 30), unfamiliar with insulin delivery, injected 10 U of insulin into a sponge pad using either a syringe and vial or the FlexPen, both with 30-gauge 8-mm needles, on day 1. The following day, they used the alternative method. They evaluated the handling of the two methods on device-specific questionnaires and compared overall preference on a third questionnaire. To evaluate dose accuracy, 30 insulin therapy-experienced HCPs and 20 insulin therapy-naive HCPs were asked to deliver 10 U of insulin using each method, and the amount discharged was weighed.Results:FlexPen was rated easier to use and overall more preferable than the syringe and vial by insulin therapy-naive HCPs (P< 0.001). The pen device was more accurate than the syringe and vial when used by experienced HCPs (mean ± SD dose delivered, 9.91 ± 0.11 U vs. 9.82 ± 0.25 U, respectively; P< 0.001) and by insulin therapy-naive HCPs (9.91 ± 0.12 U vs. 9.74 ± 0.85 U; P< 0.001).Conclusions:Insulin therapy-naive HCPs found FlexPen easier to handle and preferable to use compared to a conventional syringe and vial. BACKGROUND. Foam sclerotherapy is an increasingly popular modality in the treatment of varicose veins. Worldwide, the most popular agents used are sodium tetradecyl sulfate (STS) and polidocanol (POL). The double-syringe system technique to make foam out of a sclerosing solution and air has received wide attention for its ease and reproducibility. This study examined the possibility that the relative silicone content of various disposable connectors may affect overall foam stability. We also evaluated the differences in the stability of foam between STS and POL. MATERIALS AND METHODS. In the first part of the study, one nondispos able stainless steel connector and five disposable plastic connectors were used to create foam from STS 0.50% and air. The procedure was then repeated to produce foam from POL 0.50% and air from each of the six different connector types. As a measure of foam stability, once foam was created with each type of connector, the time required for half of the original volume of sclerosing solution to settle was recorded. In the second part of the study, foam was created with a nondispo sable stainless steel connector only and various concentrations of STS and POL. Foam stability was then measured for these different concentrations of sclerosants. RESULTS. The time for sclerosing solution to settle to half of its initial volume was found to vary according to the specific sclerosant and concentration used, with no statistically significant variation based on connector type. CONCLUSIONS. The type of connector used in the double- syringe system technique to produce foam for sclerotherapy is not a factor in foam stability. Intravitreal injections have become the most commonly performed intraocular treatments worldwide. Because intravitreal injections may induce severe adverse events, such as infectious and noninfectious endophthalmitis, cataract, ocular hypertension, vitreous hemorrhage, or retinal detachment, appropriate awareness of the materials and techniques used are essential to reduce these sight-threatening complications. This review provides insights into the needles, syringes, silicone oil coating, sterilization methods, devices to assist intravitreal injections, scleral piercing techniques using needles, syringe handling, anesthesia, and safety issues related to materials and techniques. It is paramount that physicians be aware of every step involved in intravitreal injections and consider the roles and implications of all materials and techniques used. The ability to understand the theoretical and practical circumstances may definitely lead to state-of-the-art treatments delivered to patients. The most important practical recommendations are: choosing syringes with as little silicone oil as possible, or, preferably, none; avoiding agitation of syringes; awareness that most biologics (e.g., antiangiogenic proteins) are susceptible to changes in molecular properties under some conditions, such as agitation and temperature variation; understanding that improper materials and techniques may lead to complications after intravitreal injections, e.g., inflammation; and recognizing that some devices may contribute to an enhanced, safer, and faster intravitreal injection technique. On-table colonic lavage for preparing obstructed bowel prior to anastomosis or stoma formation is now routine in adults, but it is seldom used in paediatric practice. We describe a simple technique for intraoperative colonic lavage using inexpensive medical devices that are also readily available and disposable, a Replogle tube, a fine-bore sucker, and a 20-ml syringe.
