Purpose: This study aimed to evaluate the impact of body habitus factors on image quality of low-tube voltage computed tomography (CT) during the hepatic arterial phase., Materials and Methods: Ninety-seven patients (66 men, 31 women; age range, 26-78 years) who underwent clinically indicated liver dynamic CT examination were enrolled in the study. Analysis with 80-kVp CT and intermediate tube current (277-337 mA) was performed in the late hepatic arterial phase using a 320-detector row scanner with adaptive iterative dose reduction 3-dimensional reconstruction. Patient body habitus was measured using body weight (BW), body mass index (BMI), lateral width (LW) of the abdomen, and muscle volume (MV) of the abdominal wall. On hepatic arterial phase, the mean image noise and contrast-to-noise ratio (CNR) for the aorta and liver were assessed. The correlations between body habitus factors and image quality parameters were evaluated., Results: In all patients, MV showed the strongest correlation with image noise, followed by BW and LW (r = 0.684, 0.570, and 0.555, respectively). The BMI showed the fourth strongest correlation with image noise among all body habitus factors (r = 0.377). With respect to CNR of the aorta, MV and BW showed the strongest inverse correlation (r = -0.590 and -0.600, respectively), followed by LW and BMI (r = -0.557 and -0.423, respectively). Regarding the CNR of the liver, MV showed the strongest inverse correlation (r = -0.279), although the correlation efficiency was weak compared with other correlations., Conclusions: Among various body habitus factors, MV showed the strongest association with image noise and CNR in the hepatic arterial phase using 80-kVp CT.

Objectives: To compare diagnostic accuracy of gadoxetic acid-enhanced magnetic resonance imaging (MRI) (original set) and original MRI combined with the arterial phase of low-tube-voltage computed tomography (CT) (hybrid set)., Methods: In hybrid set, we substituted the CT arterial phase for MRI arterial phase. Three observers independently interpreted. The accuracy of each image set was evaluated using the alternative-free response receiver operating characteristic method., Results: The mean Az values for original set (0.96±0.01) was higher than that for hybrid set (0.94±0.01), but the difference was not significant (P=.10)., Conclusions: In cases with degradation of magnetic resonance arterial phase, reuse of CT arterial phase might be helpful instead of repeating MRI., (Copyright © 2015 Elsevier Inc. Purpose: To validate the diagnostic performance of quadruple phase low tube voltage liver CT through the comparison with Gd-EOB-DTPA enhanced liver MRI for the detection of HCC., Materials and Methods: Non-obese patients (38 men, eight women) with 68 HCCs underwent quadruple-phase CT at 16 MDCT (using low tube voltage, 80kVp; moderately high tube current, 280mAs) and Gd-EOB-DTPA-enhanced 3T MRI. Three observers independently and randomly reviewed the CT and MR images on a tumor-by-tumor basis. The diagnostic accuracy of these techniques for detecting HCC was assessed using alternative free-response receiver operating characteristic analysis. Sensitivity and positive predictive values were evaluated. The mean effective doses for the low dose CT were also evaluated., Results: The areas under the ROC curves were 0.963, 0.959, and 0.941 for low dose CT and 0.981, 0.982, and 0.976 for MRI. Differences in Az of the two techniques for each observer were not statistically significant (P>.05). Differences in sensitivity and positive predictive values between the two techniques for each observer were not also statistically significant: sensitivity (86.8%, 82.4%, 85.3% for CT and 95.6%, 94.1%, 91.2% for MRI) and positive predictive values (92.2%, 90.3%, 89.2% for CT and 92.9%, 92.8%, 92.5% for MRI). Six HCCs (8.8%) in five patients were observed only on hepatobiliary phase of MRI, and all were smaller than 1.5cm. The mean effective dose for CT was approximately 10.2mSv., Conclusions: Quadruple-phase low-dose liver CT (80kVp, 280mAs) had relatively good diagnostic performance for detecting HCC in non-obese patients. Because no significant difference was observed between low-dose CT and MRI, the use of low-dose liver CT can be justified based on its reduced radiation effects., (Copyright © 2012 Elsevier Ireland Ltd. Aim: To evaluate the relationship between hepatic fat infiltration and abdominal fat volume by using computed tomography (CT)., Methods: Three hundred and six patients who visited our obesity clinic between November 2007 and April 2008 underwent fat protocol CT scans. The age range of the patients was 19 to 79 years and the mean age was 49 years. The male to female ratio was 116:190. Liver and spleen attenuation measurements were taken with three regions of interests (ROIs) from the liver and two ROIs from the spleen. Hepatic attenuation indices (HAIs) were measured as follows: (1) hepatic parenchymal attenuation (CT(LP)); (2) liver to spleen attenuation ratio (LS ratio); and (3) difference between hepatic and splenic attenuation (LS(dif)). Abdominal fat volume was measured using a 3 mm slice CT scan starting at the level of the umbilicus and was automatically calculated by a workstation. Abdominal fat was classified into total fat (TF), visceral fat (VF), and subcutaneous fat (SF). We used a bivariate correlation method to assess the relationship between the three HAIs and TF, VF, and SF., Results: There were significant negative correlations between CT(LP), LS ratio, and LS(dif) with TF, VF, and SF, respectively. The CT(LP) showed a strong negative correlation with TF and VF (r = -0.415 and -0.434, respectively, P < 0.001). The correlation between CT(LP) and SF was less significant (r = -0.313, P < 0.001)., Conclusion: Fatty infiltration of the liver was correlated with amount of abdominal fat and VF was more strongly associated with fatty liver than SF.

Purpose: To evaluate multidetector CT features of right intrahepatic portosystemic venous shunt (IPSVS)., Materials and Methods: Pathways of right IPSVS were evaluated from 20 patients. Diameters of right portal veins were measured in IPSVS patients, 30 cirrhotic and 30 healthy patients., Result: Among 22 IPSVSs, shunt between posterior branch and inferior phrenic vein was most common. Diameters of the posterior branch were larger in IPSVS patients than in other groups., Conclusion: Most right IPSVSs drain to inferior phrenic vein through dilated posterior branch., (Copyright © 2010 Elsevier Inc. Objective: To evaluate the outcomes of small (5-10 mm), arterially enhancing nodules (SAENs) shown exclusively at the hepatic arterial phase of CT in a hepatocellular carcinoma (HCC) surveillance population and to determine risk factors for developing HCC., Methods: The study population included 112 patients (male:female = 100:12; aged 36-92 years) with 175 SAENs who were at risk of HCC. We evaluated serial changes during follow-up (1.4-41.8 months, mean 35.7 months) and analysed the initial CT findings of SAENs and the accompanying lesions to elucidate the risk factors for HCC development., Results: Of 175 SAENs, 101(57.7%) disappeared and 34(19.4%) persisted. Forty SAENs (22.9%) became HCC in 33 patients (29.5%). Presence of HCC treatment history (p = 0.005, risk ratio = 7.429), a larger size of SAEN (p = 0.003, risk ratio = 1.630), presence of coexistent HCC (p = 0.021, risk ratio = 3.777) and absence of coexistent typical arterioportal shunts (p = 0.003, risk ratio = 4.459) turned out to be independently significant risk factors for future development of HCC., Conclusion: SAENs were frequently seen in an HCC surveillance population and have a 22.9% probability of becoming HCC on a per-lesion basis. Risk increased particularly when the lesion was associated with a previous or concurrent HCC, a large size or found without a coexistent typical arterioportal shunt.

Objective: We compared the diagnostic performance of gadoxetic acid-enhanced MRI with that of triple-phase 16-, 40-, and 64-MDCT in the preoperative detection of hepatocellular carcinoma (HCC)., Subjects and Methods: Sixty-two consecutively registered patients (54 men, eight women; age range, 31-67 years) with 83 HCCs underwent triple-phase (arterial, portal venous, equilibrium) CT at 16-, 40-, or 64-MDCT and gadoxetic acid-enhanced 3-T MRI. The diagnosis of HCC was established after surgical resection. Three observers independently and randomly reviewed the MR and CT images on a tumor-by-tumor basis. The diagnostic accuracy of these techniques in the detection of HCC was assessed with alternative free response receiver operating characteristic (ROC) analysis. Sensitivity, positive and negative predictive values, and sensitivity according to tumor size were evaluated., Results: For each observer, the areas under the ROC curve were 0.971, 0.959, and 0.967 for MRI and 0.947, 0.950, and 0.943 for CT. The differences were not statistically significant between the two techniques for each observer (p > 0.05). The differences in sensitivity and positive and negative predictive values between the two techniques for each observer were not statistically significant (p > 0.05). Among 10 HCCs 1 cm in diameter or smaller, each of the observers detected seven tumors with MRI. With CT, one observer detected five, one observer detected four, and one observer detected three HCCs with no statistically significant difference (p > 0.05)., Conclusion: Gadoxetic acid-enhanced MRI and triple-phase MDCT have similar diagnostic performance in the preoperative detection of HCC, but MRI may be better than MDCT in the detection of HCC 1 cm in diameter or smaller.

Objective: To compare diagnostic performance of ferucarbotran-enhanced 3.0-T magnetic resonance (MR) imaging using parallel imaging technique with that of triple-phase multidetector row computed tomography (MDCT) for the preoperative detection of hepatocellular carcinoma (HCC)., Methods: Eighty-six consecutive patients with a total of 128 surgically proven HCCs were enrolled in this study. All patients underwent ferucarbotran-enhanced 3.0-T MR imaging using parallel imaging technique and triple-phase MDCT before hepatic resection. Three experienced radiologists independently analyzed each images on a segment-by-segment basis. The accuracy of these techniques for the detection of HCC was assessed by performing a receiver operating characteristic (ROC) analysis of 104 resected hepatic segments with at least 1 HCC and 113 resected hepatic segments without HCC., Results: The mean value of the area under the ROC curve (Az) of the ferucarbotran-enhanced 3.0-T MR imaging (0.990) was significantly higher than that of the triple-phase MDCT (0.964) (P = 0.00). The mean sensitivity of the ferucarbotran-enhanced 3.0-T MR imaging (98.1%) was significantly higher than that of the triple-phase MDCT (92.9%) (P = 0.00). The higher sensitivity was largely attributable to a greater ability of the 3.0-T MR imaging to detect small HCC (< or =1 cm) (92.6% in 3.0-T MR imaging and 37.0% in MDCT; P = 0.00). No significant difference was found for their mean specificities (98.2% in 3.0-T MR imaging and 97.6% in MDCT; P = 0.86)., Conclusions: Ferucarbotran-enhanced 3.0-T MR imaging using parallel imaging technique is a more accurate diagnostic tool than triple-phase MDCT for the preoperative detection of HCC. Ferucarbotran-enhanced 3.0-T MR imaging has a higher sensitivity than triple-phase MDCT, especially for small HCCs (< or =1 cm).

Objective: To describe the clinical presentation and computed tomographic (CT) features of ascending retrocecal appendicitis., Methods: During the past 8 years, 1670 patients with surgically proven appendicitis were identified by a retrospective investigation. Thirty-three patients who underwent contrast-enhanced abdominal CT before surgery and had ascending retrocecal appendicitis at surgery were included in the patient cohort. The clinical and CT findings of these 33 patients were analyzed., Results: Patients presented with right lower abdominal pain (49%, 16/33), right flank pain (24%, 8/33), right upper abdominal pain (18%, 6/33), and periumbilical pain (15%, 5/33). Inflamed ascending retrocecal appendices were visualized completely in 70% (23/33), partially in 21% (7 of 33), and not detected in the remaining 9% (3/33). Periappendiceal inflammatory changes were most commonly observed in the retrocolic space in 88% (29/33), followed by paracolic gutter (30%, 10/33), pararenal space (27%, 9/33), mesentery (24%, 8/33), perirenal space (18%, 6/33), and subhepatic space (3%, 1/33). Inflammatory thickening of Gerota fascia (70%, 23/33) and the lateroconal fascia (64%, 21/33) was observed as well. Perforation of the appendix with the formation of an abscess was present in 49% (16/33). The abscesses were most commonly located in the retrocolic space (88%, 14/16). Diffuse wall thickenings of the cecum (67%, 22/33) and the ascending colon (64%, 21/33) were often observed also. Appendicoliths were found in 33% (11/33)., Conclusions: More than half of patients with ascending retrocecal appendicitis presented with atypical clinical presentation. At CT, ascending retrocecal appendicitis was associated with a high incidence of retroperitoneal inflammatory changes and appendiceal perforation.
