The purpose of the present study was to investigate the relevance of the external morphology of the mandibular ramus, internal bone tissue structure, and maxillofacial morphology at the site for sagittal split ramus osteotomy among different facial skeletal patterns. A total of 80 patients with jaw deformities who underwent sagittal split ramus osteotomy were included in the study. The patients were divided into two groups based on facial skeletal type (skeletal Class II or skeletal Class III). A further 7 patients with no skeletal abnormalities were established as the control group (skeletal Class I). Computed tomography images obtained from these patients were 3-dimensionally reconstructed and the morphology of the mandibular ramus determined. Thickness at the mandibular foramen in Class II was greater than that in Class III, and showed the lowest value at the midpoint of the mandibular foramen and mandibular notch in Class I. Mandibular morphology showed change according to facial skeletal type. Correlations were also observed between the cephalometric analysis values and mandibular morphology.

Purpose: To evaluate the utility of three-dimensional (3D) computed tomography (CT)-lymphography (LG) breast sentinel lymph node navigation in our institute., Patients and Methods: Between 2002 and 2013, we preoperatively identified sentinel lymph nodes (SLNs) in 576 clinically node-negative breast cancer patients with T1 and T2 breast cancer using 3D CT-LG method. SLN biopsy (SLNB) was performed in 557 of 576 patients using both the images of 3D CT-LG for guidance and the blue dye method., Results: Using 3D CT-LG, SLNs were visualized in 569 (99%) of 576 patients. Of 569 patients, both lymphatic draining ducts and SLNs from the peritumoral and periareolar areas were visualized in 549 (96%) patients. Only SLNs without lymphatic draining ducts were visualized in 20 patients. Drainage lymphatic pathways visualized with 3D CT-LG (549 cases) were classified into four patterns: single route/single SLN (355 cases, 65%), multiple routes/single SLN (59 cases, 11%) single route/multiple SLNs (62 cases, 11%) and multiple routes/multiple SLNs (73 cases, 13%). SLNs were detected in 556 (99.8%) of 557 patients during SLNB., Conclusion: CT-LG is useful for preoperative visualization of SLNs and breast lymphatic draining routes. This preoperative method should contribute greatly to the easy detection of SLNs during SLNB.

Lung motion due to respiration causes image degradation in medical imaging, especially in nuclear medicine which requires long acquisition times. We have developed a method for image correction between the respiratory-gated (RG) PET images in different respiration phases or breath-hold (BH) PET images in an inconsistent respiration phase. In the method, the RG or BH-PET images in different respiration phases are deformed under two criteria: similarity of the image intensity distribution and smoothness of the estimated motion vector field (MVF). However, only these criteria may cause unnatural motion estimation of lung. In this paper, assuming the use of a PET-CT scanner, we add another criterion that is the similarity for the motion direction estimated from inhalation and exhalation CT images. The proposed method was first applied to a numerical phantom XCAT with tumors and then applied to BH-PET image data for seven patients. The resultant tumor contrasts and the estimated motion vector fields were compared with those obtained by our previous method. Through those experiments we confirmed that the proposed method can provide an improved and more stable image quality for both RG and BH-PET images., Competing Interests: The authors declare that there is no conflict of interests regarding the publication of this paper.

Objectives: Factors affecting the improvement in the lung perfused blood volume (LPBV) were evaluated based on the presence of intrapulmonary clots (IPCs) after anti-coagulation therapy using 64-slice dual-energy CT., Materials and Methods: 96 patients exhibiting venous thromboembolism underwent initial and repeated LPBV examinations between December 2008 and July 2014. Fifteen patients were excluded due to pulmonary comorbidities, and a total of 81 patients were included in this study. Acute pulmonary embolism (PE) was diagnosed in 46 of the patients (56.7%). LPBV images were three-dimensionally reconstructed with two threshold ranges: 1-120 HU (V120) and 1-5 HU (V5), and the relative value of V5 per V120 expressed as %V5. These values were subsequently compared with indicators of the severity of PE, such as the D-dimer level, heart rate and CT measurements. This study was approved by the local ethics committee., Results: In patients with IPCs, the D-dimer, V5 and %V5values were significantly larger (p≤0.01) in the initial LPBV, although these differences disappeared in subsequent LPBV after treatment. The right ventricular (RV) diameter, RV/left ventricular (RV/LV) diameter ratio and %V5 values were also significantly reduced, whereas the V5 value did not significantly decrease (p=0.07), but V120 value significantly increased (p<0.001) after treatment. However, in patients with IPCs the change rate in %V5 [(subsequent-initial)/initial %V5] showed a better correlation with that in V5 (r=0.94, p<0.001) rate than that in V120 (r=0.19, p=0.19) after treatment., Conclusions: Increased whole lung perfusion (V120) and a decreased low perfusion volume (V5) affect the improvement in the %V5 values after treatment., (Copyright © 2015 Elsevier Ireland Ltd. Purpose: To investigate the added value of lung perfused blood volume (LPBV) using dual-energy CT for the evaluation of intrapulmonary clot (IPC) in patients suspected of having acute pulmonary embolism (PE)., Materials and Methods: Institutional review board approval was obtained for this retrospective study. Eighty-three patients suspected of having PE who underwent CT pulmonary angiography (CTPA) using a dual-energy technique were enrolled in this study. Two radiologists who were blinded retrospectively and independently reviewed CTPA images alone and the combined images with color-coded LPBV over a 4-week interval, and two separate sessions were performed with a one-month interval. Inter- and intraobserver variability and diagnostic accuracy were evaluated for each reviewer with receiver operating characteristic (ROC) curve analysis., Results: Values for inter- and intraobserver agreement, respectively, were better for CTPA combined with LPBV (ICC=0.847 and 0.937) than CTPA alone (ICC=0.748 and 0.861). For both readers, diagnostic accuracy (area under the ROC curve [Az]) were also superior, when CTPA alone (Az=0.888 [reader 1] and 0.912 [reader 2]) was compared with that after the combination with LPBV images (Az=0.966 [reader 1] and 0.959 [reader 2]) (p<0.001). However, Az values of both images might not have significant difference in statistics, because Az value of CTPA alone was high and 95% confidence intervals overlapped in both images., Conclusion: Addition of dual-energy perfusion CT to CTPA improves detection of peripheral IPCs with better interobserver agreement., (Copyright © 2014 Elsevier Ireland Ltd. Objective: The purpose is to develop and evaluate the ability of the computer-aided diagnosis (CAD) methods that apply texture analysis and pattern classification to differentiate malignant and benign bone and soft-tissue lesions on 18F-fluorodeoxy-glucose positron emission tomography/computed tomography ((18)F-FDG PET/CT) images., Methods: Subjects were 103 patients with 59 malignant and 44 benign bone and soft tissue lesions larger than 25 mm in diameter. Variable texture parameters of standardized uptake values (SUV) and CT Hounsfield unit values were three-dimensionally calculated in lesional volumes-of-interest segmented on PET/CT images. After selection of a subset of the most optimal texture parameters, a support vector machine classifier was used to automatically differentiate malignant and benign lesions. We developed three kinds of CAD method. Two of them utilized only texture parameters calculated on either CT or PET images, and the other one adopted the combined PET and CT texture parameters. Their abilities of differential diagnosis were compared with the SUV method with an optimal cut-off value of the maximum SUV., Results: The CAD methods utilizing only optimal PET (or CT) texture parameters showed sensitivity of 83.05 % (81.35 %), specificity of 63.63 % (61.36 %), and accuracy of 74.76 % (72.82 %). Although the ability of differential diagnosis by PET or CT texture analysis alone was not significantly different from the SUV method whose sensitivity, specificity, and accuracy were 64.41, 61.36, and 63.11 % (the optimal cut-off SUVmax was 5.4 ± 0.9 in the 10-fold cross-validation test), the CAD method with the combined PET and CT optimal texture parameters (PET: entropy and coarseness, CT: entropy and correlation) exhibited significantly better performance compared with the SUV method (p = 0.0008), showing a sensitivity of 86.44 %, specificity of 77.27 %, and accuracy of 82.52 %., Conclusions: The present CAD method using texture analysis to analyze the distribution/heterogeneity of SUV and CT values for malignant and benign bone and soft-tissue lesions improved the differential diagnosis on (18)F-FDG PET/CT images.

Aim: To evaluate the volumetric values of intrapulmonary clots (IPCs) using 64-section dual-energy perfusion computed tomography (DEpCT)., Materials and Methods: A total of 174 patients suspected of having acute pulmonary embolism (PE) underwent DEpCT, and acute PE was diagnosed in 48 of these patients. DEpCT images were three-dimensionally reconstructed with four threshold ranges: 1-120 HU (V₁₂₀), 1-15 HU (V₁₅), 1-10 HU (V₁₀), and 1-5 HU (V₅). Each relative value per V₁₂₀ was expressed as %V₁₅, %V₁₀ and %V₅. These values were compared with the d-dimer, pulmonary arterial (PA) pressure, right ventricular (RV) diameter, RV/left ventricular diameter ratio, PA diameter, and CT angiographic obstruction index (CTOI)., Results: In patients with IPCs, PA pressure, d-dimer and volumetric values of DEpCT were significantly higher (p < 0.001). Relative volumetric values at DEpCT had better correlations with the PA pressure, PA diameter, and CTOI than absolute ones, and %V₅ especially had good correlations with PA pressure (r = 0.44, p = 0.02), PA diameter (r = 0.40, p = 0.005), and CTOI (r = 0.38, p = 0.009)., Conclusion: The relative volumetric evaluation of DEpCT images with a lower attenuation threshold range may be helpful for assessing right heart strain, because these values had good correlation with CTOI, pulmonary pressure, and diameter in suggesting right heart load., (Copyright © 2013 The Royal College of Radiologists. Background: Dual-energy perfusion CT (DEpCT) directly represents the iodine distribution in lung parenchyma and low perfusion areas caused by intrapulmonary clots (IPCs) are visualized as low attenuation areas., Purpose: To evaluate if volumetric evaluation of DEpCT can be used as a predictor of right heart strain by the presence of IPCs., Material and Methods: One hundred and ninety-six patients suspected of having acute pulmonary embolism (PE) underwent DEpCT using a 64-slice dual-source CT. DEpCT images were three-dimensionally reconstructed with four threshold ranges: 1-120 HU (V₁₂₀), 1-15 HU (V₁₅), 1-10 HU (V₁₀), and 1-5 HU (V₅). Each relative ratio per V₁₂₀ was expressed as the %V₁₅, %V₁₀, and %V₅. Volumetric data-sets were compared with D-dimer, pulmonary arterial (PA) pressure, right ventricular (RV) diameter, RV/left ventricular (RV/LV) diameter ratio, PA diameter, and PA/aorta (PA/Ao) diameter ratio. The areas under the ROC curves (AUCs) were examined for their relationship to the presence of IPCs. This study was approved by the local ethics committee., Results: PA pressure and D-dimer were significantly higher in the patients who had IPCs. In the patients with IPCs, V₁₅, V₁₀, V₅, %V₁₅, %V₁₀, and %V₅ were also significantly higher than those without IPC (P ≤ 0.001). %V₅ had a better correlation with D-dimer (r = 0.30, P < 0.001) and RV/LV diameter ratio (r = 0.27, P < 0.001), and showed a higher AUC (0.73) than the other CT measurements., Conclusion: The volumetric evaluation by DEpCT had a correlation with D-dimer and RV/LV diameter ratio, and the relative ratio of volumetric CT measurements with a lower attenuation threshold might be recommended for the analysis of acute PE.

Purpose: To determine the utility of dual-energy perfusion CT (DEpCT) of non-diseased lung segments, using dual-source CT, in comparison with perfusion single-photon emission computed tomography (SPECT)., Materials and Methods: 28 patients (18 male and 10 female; mean age 63 years; age range 18-86 years) underwent DEpCT and SPECT within a 3-day interval. The presence and location of perfusion defects in each segment of the lungs were evaluated., Results: Perfusion defects were noted in 7 of 361 segments (1.9%) by DEpCT and in 19 of 361 segments (5.3%) by perfusion SPECT. DEpCT was in good agreement with perfusion SPECT for 338 of 361 segments (93.6%). Intraobserver agreement was also good, ranging from 93.4 to 93.6% (κ = 0.64-0.75, p < 0.01)., Conclusion: For non-diseased lung segments, DEpCT correlated well with SPECT.

Pulmonary single photon emission computed tomography-computed tomography (SPECT-CT) fusion images provide objective and comprehensive assessment of pulmonary function and morphology relationships at cross-sectional lungs. This article reviewed the noteworthy findings of lung pathophysiology in wide-spectral lung disorders, which have been revealed on SPECT-CT fusion images in 8 years of experience. The fusion images confirmed the fundamental pathophysiologic appearance of lung low CT attenuation caused by airway obstruction-induced hypoxic vasoconstriction and that caused by direct pulmonary arterial obstruction as in acute pulmonary thromboembolism (PTE). The fusion images showed better correlation of lung perfusion distribution with lung CT attenuation changes at lung mosaic CT attenuation (MCA) compared with regional ventilation in the wide-spectral lung disorders, indicating that lung heterogeneous perfusion distribution may be a dominant mechanism of MCA on CT. SPECT-CT angiography fusion images revealed occasional dissociation between lung perfusion defects and intravascular clots in acute PTE, indicating the importance of assessment of actual effect of intravascular colts on peripheral lung perfusion. Perfusion SPECT-CT fusion images revealed the characteristic and preferential location of pulmonary infarction in acute PTE. The fusion images showed occasional unexpected perfusion defects in normal lung areas on CT in chronic obstructive pulmonary diseases and interstitial lung diseases, indicating the ability of perfusion SPECT superior to CT for detection of mild lesions in these disorders. The fusion images showed frequent "steal phenomenon"-induced perfusion defects extending to the surrounding normal lung of arteriovenous fistulas and those at normal lungs on CT in hepatopulmonary syndrome. Comprehensive assessment of lung function-CT morphology on fusion images will lead to more profound understanding of lung pathophysiology in wide-spectral lung disorders.

Purpose: The effect of computed tomography (CT) density-based, non-uniform photon attenuation correction (AC) on lung perfusion distribution and the clinical significance were evaluated., Materials and Methods: 40 patients with pulmonary emphysema, 32 with pulmonary thromboembolism, 25 with lung cancer and 8 normal controls underwent deep-inspiratory breath-hold (DIBrH) Tc-99m-MAA perfusion SPECT, using a dual-head SPECT system and a respiratory tracking device. Scatter-corrected DIBrH SPECT was automatically co-registered with DIBrH CT. AC of DIBrH SPECT was performed using an attenuation coefficient map of a variable-effective linear coefficient calculated from CT pixel density of the co-registered DIBrH CT. The effect of AC on pulmonary perfusion was evaluated by comparison with uncorrected SPECT., Results: After AC, lung perfusion in normal lungs was increased predominantly at deep lungs near the mediastinum and vertebrae and at the upper-middle lungs, with systematic increases of radioactivity (145 ± 28%) and significant enhancement of physiological gravitational ventral-dorsal gradient (P < 0.01). Throughout the lung diseases, AC significantly enhanced perfusion defect clarity and heterogeneity (P < 0.001), without noticeable artifacts. The correlation between perfusion heterogeneity and the lung diffusing capacity for carbon monoxide was significantly improved in patients with emphysema (P < 0.005)., Conclusion: CT density-based, non-uniform AC of DIBrH perfusion SPECT provides better assessment of physiologic or impaired perfusion distributions in normal and lung diseases.

Neurolymphomatosis (NL) is a rare, unique subtype of lymphomatous infiltration of peripheral nerves. Clinical/radiologic diagnosis of NL is challenging. We report F-18 FDG PET/CT findings of a case of breast diffuse large B-cell lymphoma, in which NL developed regardless of regression of systemic lesions during induction chemotherapy. FDG PET/CT showed characteristic findings of well-demarcated, linear abnormal FDG uptake along a sacral vertebral foramen, leading to diagnosis of NL, with the finding of thickened nerve roots on magnetic resonance imaging. Altered chemotherapeutic regimen resulted in disappearance of these abnormal FDG uptake, with recovery of neurologic symptoms. Peripheral nerve NL may occur during chemotherapy, and FDG PET/CT can be a useful imaging modality in diagnosis and monitoring of therapeutic response of this disease.

Purpose: Lung perfusion impairment in patients with pulmonary arteriovenous fistula (AVF) was evaluated by automated deep inspiratory breath-hold (DIBrH) perfusion single-photon emission computed tomography (SPECT)-CT fusion images., Methods: Participants were 14 patients with a single (N=6) or multiple nodular AVFs (N=8) diagnosed by contrast-enhanced CT scan and/or pulmonary angiography. After the injection of 185MBq Tc-99m-macroaggregated albumin, a whole-body scan was obtained to quantify an intrapulmonary right-to-left shunt. Subsequently, DIBrH SPECT was obtained using the continuous rotating acquisition mode of a dual-headed SPECT system, which was automatically coregistered with DIBrH CT. The anatomic relationship between AVF and perfusion defects was assessed on the fusion images., Results: The whole-body scan depicted systemic organs indicating the presence of an intrapulmonary right-to-left shunt in all the patients. DIBrH SPECT showed 34 perfusion defects in these patients, which were located at the AVF and in the surrounding lungs of the AVF on the fusion images, regardless of the absence of morphologic abnormality on CT in all the patients. These defects were considered to be caused by the 'steal phenomenon' associated with the high and fast pulmonary arterial flow to each AVF, which were more extensive and severe in the multiple AVFs compared with a single AVF (P=0.0012), occasionally extending to the entire lobe with AVF or even to the adjacent lobe. In five patients, the fusion images detected a total of six tiny AVFs with unexpectedly extensive 'steal phenomenon'-induced defects, which had been missed by other radiological imaging techniques. The summed value of the shunt index estimated by the whole-body scan and the lung perfusion defect extent estimated by DIBrH SPECT was significantly correlated with PaO2 in all the patients (P < 0.0001), with a better correlation compared with the shunt index alone., Conclusion: In addition to the right-to-left shunt, 'steal phenomenon'-induced perfusion defects are common in the surrounding lung of pulmonary nodular AVF and cause hypoxemia. DIBrH SPECT-CT fusion images contribute to the objective evaluation of 'steal phenomenon'-induced lung perfusion impairment in AVF and the detection of tiny, subtle AVFs that may be missed by other radiological imaging techniques.

Septic pulmonary embolism (SPE) is a critical disorder which requires fast diagnosis for treatment, while SPE continues to pose a diagnostic challenge in radiologic imaging. We describe detailed findings on breath-hold perfusion SPECT-CT fusion images in 2 patients with SPE. Regardless of absence or nonspecific morphologic abnormality in these patients, the fusion images showed multiple wedge-shaped segmental/subsegmental perfusion defects along specific pulmonary arteries, highly suggesting embolic event. Follow-up fusion images after treatment showed changes in lung perfusion defects and morphology. Assessment of lung morphology-perfusion correlation on reliable fusion images contributes to correct diagnosis of SPE and monitoring of treatment effect.

The "normal-sized ovary carcinoma syndrome (NOCS)" is defined as a clinical situation, in which diffuse metastatic malignant disease of the abdominal cavity of the female with normal-sized ovaries is noted, but no origin is assigned by preoperative or intraoperative evaluation. We report a case of ovarian serous surface papillary carcinoma with manifestation of NOCS, where F-18 FDG PET/CT scan correctly detected the site of origin, although computed tomography and magnetic resonance imaging failed to detect it. This technique may play a role to discern the site of origin of some malignancies.

Background: The functional mechanism of lung mosaic computed tomography attenuation (MCA) in pulmonary vascular disease (PVD) and obstructive airway disease (OAD) has not yet been fully clarified., Purpose: To clarify the mechanism of MCA in these diseases by assessing the relationship between regional lung function and CT attenuation change at MCA sites with the use of automated deep-inspiratory breath-hold (DIBrH) perfusion single-photon emission computed tomography (SPECT)-CT fusion images and non-breath-hold Technegas SPECT., Material and Methods: Subjects were 42 PVD patients (31 pulmonary thromboembolism, four primary/two secondary pulmonary hypertension, and five Takayasu arteritis), 12 OAD patients (five acute asthma, four obliterative bronchiolitis, and three bronchiectasis), and 12 normal controls, all of whom had MCA on DIBrH CT. The relationship between regional lung function and CT attenuation change at the lung slices with MCA was assessed using DIBrH perfusion SPECT-CT fusion images and non-breath-hold Technegas SPECT. The severity of perfusion defects with or without MCA was quantified by regions-of-interest analysis., Results: On DIBrH CT and perfusion SPECT, in contrast to no noticeable CT attenuation abnormality and fairly uniform perfusion in controls, 60 MCA and 274 perfusion defects in PVD patients, and 18 MCA and 61 defects in OAD patients were identified, with a total of 77 ventilation defects on Technegas SPECT in all patients. SPECT-CT correlation showed that, throughout the 78 MCA sites of all patients, lung perfusion was persistently decreased at low CT attenuation and preserved at intervening high CT attenuation, while lung ventilation was poorly correlated with CT attenuation change. The radioactivity ratios of reduced perfusion and the intervening preserved perfusion at the 78 perfusion defects with MCA were significantly lower than those at the remaining 257 defects without MCA (P<0.0001)., Conclusion: Although further validation is required, our results indicate that heterogeneous pulmonary arterial perfusion may be a dominant mechanism of MCA in PVD and OAD.

Purpose: The relation between lung perfusion defects and intravascular clots in acute pulmonary thromboembolism (PTE) was comprehensively assessed on deep-inspiratory breath-hold (DIBrH) perfusion SPECT-computed tomographic pulmonary angiography (CTPA) fusion images., Materials and Methods: Subjects were 34 acute PTE patients, who had successfully performed DIBrH perfusion SPECT using a dual-headed SPECT and a respiratory tracking system. Automated DIBrH SPECT-CTPA fusion images were used to assess the relation between lung perfusion defects and intravascular clots detected by CTPA., Results: DIBrH SPECT visualized 175 lobar/segmental or subsegmental defects in 34 patients, and CTPA visualized 61 intravascular clots at variable locations in 30 (88%) patients, but no clots in four (12%) patients. In 30 patients with clots, the fusion images confirmed that 69 (41%) perfusion defects (20 segmental, 45 subsegmental and 4 lobar defects) of total 166 defects were located in lung territories without clots, although the remaining 97 (58%) defects were located in lung territories with clots. Perfusion defect was absent in lung territories with clots (one lobar branch and three segmental branches) in four (12%) of these patients. In four patients without clots, nine perfusion defects including four segmental ones were present., Conclusion: Because of unexpected dissociation between intravascular clots and lung perfusion defects, the present fusion images will be a useful adjunct to CTPA in the diagnosis of acute PTE.

Objective: Anatomic relation between pulmonary perfusion and morphology in pulmonary emphysema was assessed on deep-inspiratory breath-hold (DIBrH) perfusion single-photon emission computed tomography (SPECT)-CT fusion images., Methods: Subjects were 38 patients with pulmonary emphysema and 11 non-smoker controls, who successfully underwent DIBrH and non-BrH perfusion SPECT using a dual-headed SPECT system during the period between January 2004 and June 2006. DIBrH SPECT was three-dimensionally co-registered with DIBrH CT to comprehend the relationship between lung perfusion defects and CT low attenuation areas (LAA). By comparing the appearance of lung perfusion on DIBrH with non-BrH SPECT, the correlation with the rate constant for the alveolar-capillary transfer of carbon monoxide (DLCO/VA) was compared between perfusion abnormalities on these SPECTs and LAA on CT., Results: DIBrH SPECT provided fairly uniform perfusion in controls, but significantly enhanced perfusion heterogeneity when compared with non-BrH SPECT in pulmonary emphysema patients (P < 0.001). The reliable DIBrH SPECT-CT fusion images confirmed more extended perfusion defects than LAA on CT in majority (73%) of patients. Perfusion abnormalities on DIBrH SPECT were more closely correlated with DLCO/VA than LAA on CT (P < 0.05)., Conclusions: DIBrH SPECT identifies affected lungs with perfusion abnormality better than does non-BrH SPECT in pulmonary emphysema. DIBrH SPECT-CT fusion images are useful for more accurately localizing affected lungs than morphologic CT alone in this disease.

Objective: The purpose of this study was to evaluate the clinical applicability and feasibility of deep-inspiratory breath-hold (DIBrH) perfusion SPECT for improving adverse respiratory motion effects and for accuracy of SPECT/CT image fusion., Materials and Methods: Eighty-seven consecutive patients with chronic obstructive pulmonary disease (COPD) (n = 43), acute pulmonary thromboembolism (PTE) (n = 26), and interstitial lung disease (ILD), (n = 18), underwent respiratory-monitored DIBrH SPECT with a dual-headed SPECT system. Two COPD and four acute PTE patients were excluded because of inappropriate scanning due to DIBrH difficulty. DIBrH SPECT was automatically fused with DIBrH CT. Perfusion defect clarity and heterogeneity and SPECT/CT matching were compared between DIBrH SPECT and non-breath-hold SPECT., Results: Compared with non-breath-hold SPECT, DIBrH SPECT significantly enhanced defect clarity in acute PTE (p < 0.0001) and perfusion heterogeneity (coefficient of variations [CV] of pixel counts) in COPD and ILD (p < 0.0001). CV in COPD was also better correlated with lung diffusing capacity for carbon monoxide (p < 0.05). DIBrH SPECT also significantly improved SPECT/CT matching (p < 0.0001), with excellent matching of CT lung internal landmarks and pathology with corresponding defects. Fusion images confirmed wedge-shaped defects extending along specific pulmonary arterial branches in acute PTE and heterogeneous defects associated with airway or lung parenchymal abnormalities in COPD and ILD, with perfusion distribution consistent with lung CT attenuation changes., Conclusion: DIBrH SPECT is acceptable for routine application to improve respiratory motion effects and accuracy of SPECT/CT image fusion. Confirmative perfusion-morphologic correlation with reliable fusion images appears useful for clarifying the cause of perfusion defects and abnormal lung CT attenuation.

We performed preliminary tests of the feasibility of multi-detector computed tomographic lymphography (MDCT-LG) with interstitial injection of iopamidol for mapping cutaneous lymphatic drainage pathways. MDCT-LG images were obtained following cutaneous injection of a total of 1ml iopamidol bilaterally into hind legs of 10 dogs. The locations of the first draining lymph nodes were marked on the skin under MDCT-LG guidance. Five dogs served for postmortem examination of lymphatic anatomy, and the remaining 5 underwent MDCT-LG after ligation of the afferent lymphatic vessels of the first draining popliteal nodes. Clinically, MDCT-LG was attempted in 6 patients with cutaneous malignant melanoma and compared with Tc-99m-human serum albumin lymphoscintigraphy. MDCT-LG clearly visualized the first draining lymph nodes and their afferent lymphatic vessels draining from the contrast injection sites with detailed underlying anatomy in all dogs. At surgery, all these first draining nodes could be found at predicted locations under MDCT-LG guidance. MDCT-LG showed rerouting of lymphatic vessels after ligation of the afferent lymph vessels of the popliteal nodes in the second 5 dogs. Clinically, MDCT-LG also allowed accurate mapping and biopsy of the first draining nodes from primary tumors at predicted locations, with minimal skin incision. Lymphoscintigraphy failed to identify these nodes due to overlapping radioactivity of clustered nodes or transport of the radiotracer to subsequent distant nodes in 4 patients. Although a more extensive study is warranted for further validation, preoperative interstitial MDCT-LG appears to have the potential feasibility for accurate sentinel lymph node mapping and biopsy in patients with cutaneous melanoma.

Objective: We sought to evaluate the utility of integrated breath-hold single-photon emission tomography and computed tomography imaging compared with that of simple calculation with the lung segment-counting technique for predicting residual pulmonary function in patients undergoing surgical intervention for lung cancer., Methods: A prospective series of 22 patients undergoing anatomic lung resection for cancer were enrolled in this study. Postoperative residual forced expiratory volume in 1 second was predicted by measuring the radioactivity counts of the affected lobes or segments to be resected within the entire lungs by placement of regions of interest on single-photon emission tomography and computed tomography images. Residual forced expiratory volume in 1 second was also estimated by using the segment-counting technique., Results: Both predicted values agreed well with postoperative forced expiratory volume in 1 second. Although the residual forced expiratory volume in 1 second predicted by means of single-photon emission tomography and computed tomography correlated well with that predicted by using segment counting, the values were significantly underestimated by the segment-counting technique in 4 outliers with severe emphysema. There were 2 patients with borderline pulmonary functional reserve whose residual forced expiratory volume in 1 second values were predicted more accurately by means of single-photon emission tomography and computed tomography than by using segment counting., Conclusion: Integrated breath-hold single-photon emission tomography and computed tomography images allow the accurate prediction of postoperative pulmonary function but without statistical superiority over the simple segment-counting technique. Further study of the usefulness of single-photon emission tomography and computed tomography in patients with severe emphysema and borderline lung function should prove valuable because the segment-counting technique underestimates pulmonary functional reserve in these patients.

Background: To evaluate experimentally and clinically the feasibility of a newly developed technique of endoscopic computed tomography (CT) lymphography with endoscopic submucosal injection of iopamidol for esophageal sentinel lymph node (SLN) mapping and biopsy examination., Methods: Nine anesthetized dogs underwent CT after endoscopic submucosal injection of 2 mL iopamidol; 1.25-mm thick CT images were obtained before and at 1, 3, 5, 7, and 10 minutes after contrast injection. Clinically, 12 patients with superficial esophageal cancer (preoperative imaging stage: cT1, cN0) underwent CT lymphography in a similar fashion at 1, 5, and 10 minutes after peritumoral injection, followed by radical esophagectomy and regional lymph node dissection under CT lymphography guidance., Results: CT lymphography visualized the draining lymphatic vessels and SLNs within 5 minutes after contrast injection. All 14 SLNs in dogs (average, 1.5 nodes per animal; range, 1-2) and 28 SLNs in patients (average, 2.3 nodes per patient; range, 1-4) were found intraoperatively at the correct location under CT lymphography guidance. Lymph node metastasis could be detected with excellent sensitivity and accuracy in this small number of patients with no false-negative findings; metastasis was positive only in the preoperatively identified SLNs in 4 patients and in both SLNs and distant nodes in 1 patient, and was negative in all resected nodes in the remaining 7 patients., Conclusions: Endoscopic CT lymphography appears to allow accurate identification of direction and locations of lymph flow and SLNs, and has the potential clinical applicability for esophageal SLN mapping and biopsy examination, but will require a large study to determine its accuracy and usefulness.

Regional computed tomography attenuation (CTA) alteration at perfusion defects in acute pulmonary thromboembolism (PTE) was comprehensively assessed using deep-inspiratory breath-hold SPECT-CT fusion images. Subjects were 14 acute and 9 chronic PTE patients and 13 control subjects. Regional perfusion, CTA, and intravascular clots were correlated on deep-inspiratory breath-hold SPECT-unenhanced/angiographic CT fusion images. Fusion images visualized hypo-CTA in 57% of the acute PTE patients, which preferentially occurred at extensively and severely decreased perfusion areas caused by central clots. CTA at 35 defects of acute PTE was significantly decreased compared with that of normal lungs (P<0.001), but the degree was less compared with chronic PTE (P<0.0001). Fusion images also revealed variable relationships of clots and regional perfusion/CTA in the distal lungs of each central clot. Fusion images provide important information about the actual effects of intravascular clots on peripheral perfusion/CTA and indicate that lung CTA can be decreased at perfusion defects in acute PTE.

Purpose: To evaluate the feasibility of an endoscopic computed tomographic (CT) lymphography technique with submucosal injection of iopamidol for direct visualization of lymphatic drainage pathways in dogs and in patients with operable esophageal cancer., Materials and Methods: With institutional animal committee approval, a total of 2 mL of undiluted iopamidol was injected into the esophageal (n = 6) or gastric (n = 3) submucosa in nine dogs by using a flexible endoscope. Multi-detector row CT images (section thickness, 1.25 mm) were obtained before contrast material injection and during the 10 minutes after injection. The animals were euthanized so that their lymphatic anatomy could be examined. With ethical committee approval and patient informed consent, nine patients with esophageal cancer also underwent CT lymphography with peritumoral injection of 2 mL of iopamidol, followed by esophagectomy and regional lymph node dissection with CT lymphographic guidance. The histopathologic features of dissected nodes, including sentinel lymph nodes (SLNs), were examined., Results: CT lymphography depicted the direct connection of lymphatic drainage vessels with enhanced and/or unenhanced nodes (ie, SLNs) as early as within 5 minutes after contrast material injection in all subjects. All 13 SLNs in dogs (1.4 nodes per animal) and 18 SLNs in patients (two nodes per patient) were found and dissected at the correct location by using CT lymphographic guidance. In patients, histopathologic examination revealed the high predictive value of CT lymphographic-guided SLN biopsy: Only one of the preoperatively identified SLNs in three patients and both SLNs and adjacent nodes in two patients were positive for metastasis; all resected nodes in the remaining four patients were negative., Conclusion: Endoscopic CT lymphography is a feasible method for visualizing the direct connection between and the accurate anatomic location of SLNs and lymphatic drainage vessels., (RSNA, 2005)

Study Objectives: To determine the ability of quantitative CT, with special reference to area of emphysema, to predict early postoperative oxygenation capacity and outcome after lung lobectomy for cancer., Methods: Sixty-two consecutive patients scheduled to undergo lung lobectomy for cancer were enrolled in this study. The area of emphysema (< - 910 Hounsfield units) was measured on a three-dimensional CT lung model. Arterial oxygen saturation (Sao(2)) was calculated from Pao(2) measured 1 day before and 1 day after surgery with patients at rest breathing room air. A patient was considered to have recovered at the completion of a standardized management regimen., Results: Postoperative Sao(2) (postSao(2)) was predicted by the baseline value and the area of emphysema with the use of a regression equation. Ten of the 62 patients (16%) had postoperative cardiopulmonary complications (CPCs). The median time to postoperative recovery was 3 days (range, 1 to 17 days). Predicted postSao(2) and predicted postoperative FEV(1) were shown to be significant independent predictors of postoperative CPCs as well as postoperative recovery time., Conclusion: Determining the area of emphysema by quantitative CT is useful in predicting early postoperative oxygenation capacity. Predicted oxygenation capacity and predicted ventilatory capacity independently affect perioperative outcomes. Therefore, using quantitative CT in combination with spirometry may improve risk prediction in patients undergoing lung lobectomy for cancer. However, the role of quantitative CT in grading nonemphysematous lung diseases, such as interstitial lung diseases, must be investigated.

Objective: Three-dimensional multidetector-row computed tomographic lymphography (3D MDCT-LG) with interstitial injection of a widely available nonionic monometric contrast medium iopamidol was used for navigation of breast sentinel lymph node (SLN) biopsy., Methods: 3D MDCT-LG was obtained after massage of the interstitially injection sites of a total of 4-5 mL undiluted iopamidol at periareolar and peritumoral areas in 68 consecutive patients with early-stage breast cancer, using a 4 detector-row CT scanner. Drainage lymphatic patterns and SLN anatomy were assessed on 3D MDCT-LG images. 3D MDCT-LG-navigated SLN biopsy with combined use of blue dye was followed by backup axillary lymph node dissection to evaluate accuracy of SLN biopsy., Results: The 3D MDCT-LG images clearly localized primary SLNs by visualizing the direct connection between these nodes and their afferent lymphatic vessels on detailed anatomy of the surrounding structures in all patients. Drainage lymphatic pathways on these images were classified into 4 patterns: single route/single SLN (39 cases, 57%), multiple routes/multiple SLNs (10 cases, 15%), single route/multiple SLNs (9 cases, 13%), and multiple routes/single SLN (10 cases, 15%). Under 3D MDCT-LG navigation, SLNs was found at the accurate location in all patients. With backup axillary lymph node dissection, metastasis was found in 14 (20%) patients, and 8 of these patients had metastasis only in the preoperatively identified SLNs. In other 5 positive patients, metastasis was found both in the SLN and non-SLNs. However, micrometastasis eventually was found only in non-SLN in an elderly patient. Overall, the sensitivity, false-negative rate, and accuracy of 3D CT-L-navigated SLN biopsy were 92% (13/14 patients), 7% (1/14 patients), and 98% (67/68 patients), respectively., Conclusions: Topographic 3D interstitial MDCT-LG can be a widely available and reliable navigator for breast SLN biopsy.

Purpose: Preferential location of acute pulmonary thromboembolism (PTE) induced consolidative opacities (infarction/atelectasis) was determined on respiratory gated perfusion SPECT-CT fusion images., Method: Gated end-inspiratory perfusion SPECT images were obtained in 21 patients with acute PTE and 17 patients with inflammatory diseases, using a triple-headed SPECT system and a respiratory tracking device. Anatomical relationships of consolidative opacities and perfusion defects were assessed on gated SPECT-rest inspiratory CT fusion images. The size and radioactivity of perfusion defects with acute PTE consolidative opacities were compared with those of defects without these opacities. The contribution of fusion images for differential diagnosis of acute PTE induced and inflammatory disease induced lesions was evaluated by receiver operating characteristic (ROC) curve analysis., Results: Of the total 56 acute PTE induced consolidative opacities, 42 (75%) were located at the peripheral interface between the severely decreased and adjacent relatively preserved perfusion areas within wedge shaped perfusion defects on fusion images. These defects with consolidative opacities were significantly larger and had taken up less radioactivity compared with those in the 86 defects without these lesions (P<0.0001). In contrast, of the 29 inflammatory disease induced opacities, 14 (48.2%) had the matched defects and 13 (44.8%) were located at the proximal portion of defects. These preferential locations of acute PTE induced and inflammation induced lesions were significantly different (P<0.01). In ROC curves, the combined reading of fusion images showed a significantly higher differential diagnostic accuracy compared with the reading of CT and SPECT images alone (P<0.01)., Conclusions: Acute PTE induced consolidative opacities preferentially occur at the peripheral lung interface between severely decreased and adjacent relatively preserved perfusion areas within relatively large and severely decreased perfusion defects. The fusion images, which provide an accurate assessment of the morphological-perfusion defect relationship could, potentially, provide a differential diagnosis between acute PTE induced and inflammatory disease induced lesions.

Purpose: Experimental and clinical evaluation of the potential utility of indirect computed tomographic lymphography (CT-LG) with intrapulmonary injection of iopamidol for preoperative localization of sentinel lymph node station in non-small cell lung cancer., Methods: CT-LG with intrapulmonary injection of 0.5 mL of undiluted iopamidol was performed in 10 dogs using a multidetector-row CT unit, followed by postmortem examination of enhanced lymph nodes in 5 of these dogs. The CT-LG with peritumoral injection of 1 mL of the contrast agent was also performed in 9 patients with non-small cell lung cancer without lymphadenopathy. At surgery, enhanced lymph nodes were resected under CT-LG guide, followed by standard lymph node dissection with macroscopic and histologic examination. A significant enhancement of lymph nodes was determined when CT attenuation value was increased with 30 Hounsfield units (HU) compared with precontrast images., Results: CT-LG visualized a total of 15 enhanced lymph nodes (on average, 1.5 nodes per animal) within 2 minutes after contrast injection in the 10 dogs, with average size of 6.7+/- 1.9 mm and average maximum CT attenuation of 149 +/- 41 HU. All the 8 enhanced nodes in 5 dogs were found in the appropriate anatomic locations in postmortem examinations. Without noticeable complications, CT-LG visualized 30 ipsilateral intrathoracic lymph nodes including 19 hilar/pulmonary and 11 mediastinal nodes in the 9 patients (on average, 2.2 hilar/pulmonary and 1.1 mediastinal nodes per patient) within 2 minutes after contrast injection, with average size of 4.7+/- 0.4 mm and average maximum CT attenuation of 134 +/- 52 HU. At surgery, all these enhanced nodes could be accurately found and resected under CT-LG guidance. Metastasis was not evident in either of these enhanced lymph nodes or the remaining distant nodes in all patients., Conclusion: Quick and accurate localization of sentinel lymph node station on detailed underlying lung anatomy by using indirect CT-LG may be of value to guide selective lymph node dissection for minimally invasive surgery in non-small cell lung cancer.

Background: Preoperative localization of the sentinel node basin would guide selective lymph node dissection. We tried to identify these nodal stations with indirect computed tomographic lymphography using a conventional extracellular contrast agent, iopamidol., Methods: Eleven consecutive patients scheduled to undergo anatomic resection of suspected lung cancer, without lymphadenopathy, were given a peritumoral injection of undiluted iopamidol under computed tomography guidance, and lymphatic migration was assessed by multidetector-row helical computed tomography., Results: There were no complications such as bleeding, pneumothorax, or allergic reactions. Enhanced nodes were detected in all but 1 patient who had diffuse lymph nodal calcification. Enhanced nodes were identified at 32 ipsilateral intrathoracic nodal stations (20 hilar stations and 12 mediastinal stations). The average length of the longer axis of the enhanced nodes was 4.8 mm (range, 3 to 8 mm), and the average attenuation of the enhanced nodes was 132 (range, 46 to 261) Hounsfield units. In 9 patients with confirmed lung cancer, enhanced nodes appeared at 26 nodal stations, and all apparent enhanced nodes were identified as actual lymph nodes at appropriate position during lymphadenectomy. None of the resected lymph nodes had metastatic involvement., Conclusions: Indirect computed tomographic lymphography with the peritumoral injection of iopamidol effectively depicts the drainage nodes unless they are diffusely calcified. Although further study is required, this method could guide selective lymph node dissection.

Background: The sentinel lymph node biopsy (SLNB) technique is established in the treatment of breast cancer. The current technique of mapping the SLN with blue dye or radiotracers requires a learning period. Tracer and injection site selection and intraoperative pathologic examination have been discussed., Methods: We developed a three-dimensional computed tomography lymphography (3D CT-LG) technique with commercially available iopamidol. SLNB and backup dissection were performed in 40 patients with T1 and T2 breast cancer. Feasibility and efficacy of CT-LG were examined., Results: In all patients, lymph flow and the surrounding anatomical environment were visualized with 3D CT-LG. SLNB was successful because of accurate navigation by 3D CT-LG. SLN was detected in all patients, whereas dye navigation failed in seven fatty axilla and two patients with prior excisional biopsy. Backup dissection confirmed the accuracy of CT-LG-guided SLNB. A false negative result was found in only one patient. Preoperative prediction was feasible in cases of SLN metastasis., Conclusions: CT-LG allowed accurate SLN localization by quickly and adequately visualizing the direct connection between the SLN and its afferent lymphatic vessels. Detailed cross-sectional images of lymphatic anatomy during CT resulted in successful SLNB with shortening of the presurgical examination schedule.

In this study, respiratory-gated ventilation and perfusion single-photon emission tomography (SPET) were used to define regional functional impairment and to obtain reliable co-registration with computed tomography (CT) images in various lung diseases. Using a triple-headed SPET unit and a physiological synchroniser, gated perfusion SPET was performed in a total of 78 patients with different pulmonary diseases, including metastatic nodules (n = 15); in 34 of these patients, it was performed in combination with gated technetium-99m Technegas SPET. Projection data were acquired using 60 stops over 120 degrees for each detector. Gated end-inspiration and ungated images were reconstructed from 1/8 data centered at peak inspiration for each regular respiratory cycle and full respiratory cycle data, respectively. Gated images were registered with tidal inspiration CT images using automated three-dimensional (3D) registration software. Registration mismatch was assessed by measuring 3D distance of the centroid of the nine selected round perfusion-defective nodules. Gated SPET images were completed within 29 min, and increased the number of visible ventilation and perfusion defects by 9.7% and 17.2%, respectively, as compared with ungated images; furthermore, lesion-to-normal lung contrast was significantly higher on gated SPET images. In the nine round perfusion-defective nodules, gated images yielded a significantly better SPET-CT match compared with ungated images (4.9 +/- 3.1 mm vs 19.0 +/- 9.1 mm, P<0.001). The co-registered SPET-CT images allowed accurate perception of the location and extent of each ventilation/perfusion defect on the underlying CT anatomy, and characterised the pathophysiology of the various diseases. By reducing respiratory motion effects and enhancing perfusion/ventilation defect clarity, gated SPET can provide reliable co-registered images with CT images to accurately characterise regional functional impairment in various lung diseases.

Background: Accurate localization of the breast sentinel lymph node (SLN) can be challenging as a minimally invasive approach to the treatment of early-stage breast cancer. We tested the potential capability of interstitial computed tomographic lymphography (CT-LG) using a conventional contrast agent (iopamidol) for SLN mapping., Methods: In 14 female dogs, 0.5 and 1 mL of undiluted iopamidol was injected subcutaneously into the 2 skin areas overlying the mammary gland. Contiguous, 2 mm-thick multidetector helical CT images were obtained through the upper breast and axilla before, and for 60 minutes after, gentle massage at the injection site. Three-dimensional (3D) CT images were obtained from the postcontrast images showing the greatest SLN enhancement. This CT-LG with 2 mL of iopamidol was also evaluated in 5 human female volunteers., Results: The direct connection of SLN and lymphatic vessels draining from the injection sites in the animal models was clearly visualized, even with 0.5 mL of iopamidol. With this dose, the SLN attenuation was maximally enhanced, with a mean of 274 Hounsfield units (HU) on the first postcontrast images. The topographic 3D images provided the comprehensive anatomy of these lymphatic pathways. Of the 28 SLNs and 184 distant nodes visualized on CT images, all of the SLNs (100%) and 161 (87.5%) of the distant nodes could be resected at premortem and/or postmortem, with a good correlation in the locations and sizes with those on the CT images. The CT-LG effectively localized 5 SLNs with averaged maximum attenuation of 223 HU in the human volunteers, without any significant adverse effects., Conclusion: Interstitial CT-LG using small volumes of iopamidol can sufficiently visualize breast lymphatic drainage and may have potential utility for breast SLN mapping.

Rationale and Objectives: The capability of an indirect computed tomographic lymphography (CT-LG) using a nonionic monometric contrast medium iopamidol for visualizing breast lymphatic pathways was preliminarily tested., Materials and Methods: In 10 female dogs, a total of 0.5 and 1 mL of undiluted iopamidol was injected subcutaneously into the skin areas overlying the both caudal mammary glands. Contiguous 2-mm-thick multidetector raw helical CT images were obtained through the upper thorax and axilla before and during 60 minutes after gentle massage at the injection sites, with reconstruction into three-dimensional (3D) postcontrast CT images. The first lymph node (1st LN) directly draining from the injection sites was marked under CT guidance, followed by pre- and postmortem examinations. This CT-LG with 2-mL iopamidol was also attempted in five human female volunteers., Results: Even with 0.5-mL iopamidol, the CT-LG clearly visualized the direct connection of the 1st LN and lymphatic vessels draining from the injection sites throughout the examination time in all the animals, with the maximum CT attenuation of 269 Hounsfield units (HU) +/- 137 in the 1st LN on the first postcontrast images. The topographic 3D images provided comprehensive anatomic outlines of these lymphatic pathways. Of the total of 20 opacified 1st LN and 110 distant nodes, all the 1st LN (100%) and 92 (83.6%) distant nodes could be resected at pre- or postmortem, with a good correlation with the CT images. The CT-LG also effectively localized the 1st LN with the maximum attenuation of 223 HU +/- 63 in the human volunteers, without any significant late adverse effects., Conclusion: Indirect CT-LG with iopamidol may have excellent potential for visualizing breast lymphatic drainage and for preoperative localization of breast sentinel lymph nodes.

Purpose: A phantom study was conducted to evaluate the feasibility of body contour definition with Compton scatter photons from external sources of technetium-99m pertechnetate (Tc-99m) to create a fusion image of CT and SPECT images., Methods: External sources of 1 mCi (37 MBq) Tc-99m were placed on each collimator, and body-contour SPECT images were obtained with an energy window of 100 keV +/- 25% for detecting 90 degrees and 180 degrees Compton scatter photons of Tc-99m from the body surface in water-filled cylindrical and hexagonal phantoms, and in a chest phantom with a Tc-99m-avid simulated lung nodule and multimethod surface markers. In the chest phantom, each transaxial SPECT slices was registered with the corresponding CT slice by using image-matching soft ware. A summation of the registered images yielded a three-dimensional (3-D) fusion image of this phantom., Results: This method clearly visualized the body contour on all the SPECT slices in all the phantoms except for the complex hexagonal phantom. There was no significant difference between the known and SPECT-measured diameters of the cylindrical phantom. The fit of CT and SPECT images of the chest phantom was achieved with a mean alignment error of 5% in visual inspection, which was improved to 0.2% after correction of the magnification of the SPECT images according to the resultant dimensional differences. The 3-D fusion image of this phantom effectively visualized the anatomic location of the lung nodule and surface markers., Conclusion: This simple method effectively provided boundary information on the cold phantoms. Although further improvements in the registration technique with CT images are desirable, the body-contour SPECT image obtained by this method has the potential for accurately creating a 3-D fusion image with CT images, and is a feasible way of anticipating the anatomical localization of a target tissue.

Subcutaneous and tissue space emphysema following surgical extraction of mandibular third molars is a rare and serious complication of dental surgery. Recognition of mediastinal emphysema following surgical extraction is difficult because there are no absolute clinical symptoms and signs. We present two cases of emphysema following extraction of a lower third molar and discuss the contribution of CT to the early recognition of the presence and spatial migration of air and to clinical management.

An initial attempt to superimpose in-111-labeled platelet SPECT over CT/MRI in two patients with intracardiac thrombus is presented. One patient was a 65-year-old woman with a history of mitral and tricuspid valve replacement. This patient had a large thrombus measuring 74 x 43 x 34 mm in the right atrium. Fusion imaging with CT showed increased in-111 activity on the surface of the thrombus, which was visualized as circular activity on a planar image. The other patient was a 72-year-old woman who had a thrombus measuring 17 x 14 x 12 mm at the cardiac apex. Planar imaging showed two discrete areas of abnormal in-111 activity. Superimposition of the SPECT image over MR revealed that one of these areas corresponded to the small thrombus, whereas the other related to a previous subendomyocardial biopsy. In both patients, the activity ratio of the abnormal in-111 accumulation versus the cardiac chamber on SPECT was greater than that on planar images. The correlation of in-111 platelet SPECT and CT/MRI was useful in accurately localizing the intracardiac thrombus.
