Background: Acute appendicitis is the most common nonobstetric cause of acute abdomen in pregnant women. In this study, we aimed to evaluate pregnant patients who were operated on for acute appendicitis in our center, which has a large pregnant population, in the light of the literature. Materials and Methods: The data of patients operated on for acute appendicitis between November 2015 and August 2022 were retrospectively examined and recorded electronically. The patients' complaints, physical examination findings, radiological data, operation type, and pathological data were recorded and examined in the light of the literature. Results: The average age of 37 patients who underwent appendectomy was 27.7 (20-42) years. In these patients, acute appendicitis was diagnosed most frequently in the 2nd trimester (48.6%). The most common presenting complaint of the patients was abdominal pain, and the most common physical examination finding was abdominal tenderness. Considering the admission time of the patients, it was observed that a delay of more than 24 hours increased the risk of perforation. Although the most common histopathological finding was acute appendicitis, malignancy was detected in one patient and enterobius vermicularis was detected in another. Conclusions: Acute appendicitis should be kept in mind in patients with new-onset right lower quadrant abdominal pain during pregnancy. Because delay in diagnosis and therefore surgery causes serious morbidity and mortality for both the fetus and the mother. [ABSTRACT FROM AUTHOR]

Background: The aim of our study is to compare laparoscopic and abdominal myomectomies performed in our clinic and to evaluate the feasibility and safety of laparoscopic myomectomies. Materials and Methods: In this study, we retrospectively evaluated 314 cases who were threated with myomectomy operation in our clinic between 2012-2022. All of the cases were divided into two groups with respect to the surgical procedure performed. 314 patients were in the abdominal myomectomy group, 47 patients were in the laparoscopic myomectomy group and the groups were compared. Results: There was no statistically significant difference between the groups in terms of demographic data (p>0.05). When the groups were evaluated in terms of postoperative white blood cell(WBC), hemoglobin, hematocrit, platelet values, operation time, hospitalisation time, postoperative blood transfusion and perioperative complications, no statistically significant difference was observed (p>0.05). There were no statistically significant difference in terms of preoperative WBC and CRP values between the groups, but postoperative WBC and CRP values were lower in the laparoscopic myomectomy group (p<0.05). Conclusions: Laparoscopic myomectomy has a short recovery time and aesthetic advantage. However, equipment and the need of experienced surgeons limit its applicability. With the development of laparoscopic technologies and increasing the experience of surgeons with laparoscopic surgery, it will provide a serious advantage over the abdominal method in terms of less blood loss, less hospital stay, shorter operation times, less trauma and better cosmetic results. Infertility, defined as failure to achieve pregnancy within 12 months of unprotected intercourse or therapeutic donor insemination in women younger than 35 years or within 6 months in women older than 35 years, affects up to 15% of couples. An infertility evaluation may be offered to any patient who by definition has infertility or is at high risk of infertility. Hysterosalpingography, hysterosalpingo-contrast sonography, saline infusion sonohysterography, hysteroscopy, laparoscopy, and bacteriological and endocrinological examinations that will be made after these studies aim to focus more on the causes of infertility. With the development of assisted reproductive technology, the need for major reproductive surgery, which may be necessary for the primary treatment of infertility, has decreased over the years. Surgical methods are mainly considered as laparoscopic and hysteroscopic techniques. However, laparotomy is also rarely required and may be needed in cases such as adhesions, mass lesions, unsuccessful surgeries, or emergency surgery. When a surgical treatment is planned for infertile patients who required surgery, it is very crucial that the procedure should be performed by experienced surgeons. In this review, pathologies that require surgery for infertility treatment and surgical methods that can be applied to infertile patients were discussed, rather than medical treatments. Objective: To assess the effectiveness of laparoscopic transperitoneal nephrectomy in the treatment of patients with giant hydronephrosis and to compare the results with open nephrectomy. Materials and Methods: We reviewed the data of 19 patients underwent laparoscopic (laparoscopic group, n=8) and open (open group, n=11) transperitoneal nephrectomy for giant hydronephrosis between January 2008 and 2018. Demographic characteristics, clinical, laboratory and radiological findings of cases were examined. Perioperative and postoperative outcomes were reported. Mann-Whitney U and Fisher's Exact tests were used in the statistical analysis. Results: The mean operation time was 112.5±19.1 (90-140) minutes and 107.2±19.1 (80-140) minutes, respectively, for the laparoscopic and open groups (p=0.546). The mean hospitalization period was 3.5 (3-7) and 6 (5-8) days, respectively, for the laparoscopic and open approach (p=0.003). No major complications during the perioperative period were observed in any of the patients. The mean follow-up periods were 36 (6-60) and 70 (39-80) months, respectively, for the laparoscopic and open groups (p=0.000). Conclusion: Laparoscopic transperitoneal nephrectomy appears to be equally effective to the open approach with a shorter hospitalization period for the treatment of giant hydronephrosis. Objective: Short-term oncological and functional results of patients who underwent laparoscopic radical prostatectomy (LRRP) with a bladder neck and urethra preservation modified posterior approach (Demirtaş Erciyes Modification) are presented. Materials and Methods: The data of 140 patients who were operated between July 2015 and March 2020 for localized prostate cancer were analyzed retrospectively. A total of 32 patients were excluded from evaluation because a history of transurethral prostate resection or bladder neck preservation could not be applied due to the median lobe protruding into the bladder. Preoperative prostate-specific antigen (PSA), prostate biopsies, preoperative erectile function status, operation time, transfusion rate, complications, pathology results of LRRP, postoperative erectile function, and continence status were evaluated. Results: The mean age of 108 patients was 64±4.47 years, with median PSA of 9.65 ng/mL. The mean operation time was 186.96±54.1 min, and the median catheter removal time was 10 days. The median hospital stay was 4 days. The median follow-up time was 17.5 months. The prostatectomy pathology of 95% of patients was at pT2 stage. The complication rate of Clavien 3 and above was 4.6%. The surgical margin positivity rate was 10.2%. Continence rates were 88% and 92.6% at 6 and 12 months, respectively. The rate of erection with spontaneous or oral medications was 43.5%. Among 58 patients with at least 2 years follow-up, PSA recurrence was detected only in two patients. Conclusion: Laparoscopic radical prostatectomy with a bladder neck and urethra protective modified posterior approach may be an option in the selected patient group in terms of short-term oncological and functional results. Objective: With the increasing popularity of minimally-invasive surgery, laparoscopic pyeloplasty has become a staple in the armamentarium of urologists. However, the surgery has a steep learning curve and longer operative time. In this study, we aimed to evaluate the results of initial 53 cases of laparoscopic dismembered pyeloplasty in our institute. Materials and Methods: A total 52 of patients with pelvi-ureteric junction (PUJ) obstruction, 30 male and 22 female, with the mean age of 23.5 years were managed by transperitoneal laparoscopic dismembered pyeloplasty. The patients were placed in full lateral position and surgery was done using a minimum of three ports, retrograde pyelography was done in all; initial access was done by using a Veress needle. The ureter was spatulated first, first suture taken and then the PUJ was dismembered to avoid rotation of the ureter. Antegrade DJ stenting was done in all patients and one drain was left in the retroperitoneum after surgery. DJ stent was removed six weeks after surgery. Results: Fifty two patients were managed by dismembered pyeloplasty. Six patients required preoperative urinary diversion. Intrarenal pelvis was seen in seven, crossing vessel in ten, high insertion of ureter in six and associated calculus in five patients. Conversion to open surgery was required in six patients. Initially, the operative time was more than three hours but after sufficient experience of 25 cases, it reduced drastically and in last 28 cases, the mean operative time was 123 minutes, with shortest time reported 97 minutes. Reintervention was required in eight patients and overall success rate was 87%. Conclusion: Laparoscopic pyeloplasty is a safe, minimally-invasive and viable alternative to open pyeloplasty for the management of PUJ obstruction. [ABSTRACT FROM AUTHOR]

Aim: Laparoscopic-assisted transanal pull-through (LATP) is a procedure that has gained increasing popularity in the management of Hirschsprung's disease. The purpose of this study was to compare outcomes of patients treated with LATP and transanal pull-through (TPT). Methods: Records of 45 patients with Hirschsprung's disease who underwent surgery between 2006 and 2017 were retrospectively evaluated. Results: LATP was performed in 16 patients [13 male (81%), three female (19%)]. The median age of the patients was four months (7 days-84 months). Twenty nine patients [21 male (72.4%), eight female (27.6%)] underwent TPT The median age of the patients was 11 months (10 days-90 months). The mean time to start feeding in LATP and TPT groups was two days (one-three days) and 2.6 days, respectively (p=0.074). The mean operative time was 2.6 hours in the LATP group (two-four hours) and 2.7 hours in the TPT group (p=0.971). The mean length of hospital stay in for LATP and TPT groups was 4.8 days (two-nine days) and six days (3-14 days), respectively (p=0.305). Conclusion: The advantages of LATP include multiple sample collection from several segments of the colonn at the same time as well as shorter time to frozen section diagnosis. In addition, the possibility of intraabdominal dissection allows transanal surgery to be faster and more effective. [ABSTRACT FROM AUTHOR]

Aim: To evaluate outcomes of transabdominal preperitoneal repair in patients with bilateral inguinal hernias. Methods: Sociodemographic data, and data about disease and operation with postoperative data of all patients with bilateral inguinal hernias, who were treated by transabdominal preperitoneal repair were prospectively collected. Patients were followed-up for recurrence and chronic pain in the long-term. Results: A total of 70 (67 [95.7%] males, mean age was 53.4±13.6 years) cases were included. Total 138 hernias (mostly Nyhus type 3 [n=116; 84.1%]) were repaired in 70 cases. Unilateral inguinal hernia was diagnosed in two cases during the operation. Mean operation time was 80.6±26.5 minutes. Inferior epigastric vein was injured in 2 (1.4%) cases. Parenteral analgesics were required in only 10 (14.3%) patients. Patients were discharged 1.21±0.67 days after the operation, and only 9 (12.8%) cases were hospitalized more than one day. Patients returned work or normal activity 10.5±4.7 days after the surgery. The mean follow-up period was 25.9±19.4 months. Symptomatic recurrence was observed in 2 (1.4%) patients. Six (8.7%) cases had chronic pain. Aim: To present our hospital data, comparing to laparotomic and laparoscopic hysterectomias' features and their complications in our clinic. Materials and Method: 159 patients that undergone laparotomic and laparoscopic hysterectomy between January 2010 and December 2015 in Harran University, Faculty of Medicine, Gynecology and Obstetrics Clinic, were analyzed retrospectively. The patients divided two groups: as laparotomy and laparoscopy. Demografic features, operation types, operation times,indications of hysterectomy, and postoperative complications of patients were analysed retrospectively. SPSS software was used for statistical studies, and p values < 0.05 were accepted statistically significant. Results: There was no significant difference according to age, body mass index, pre - postoperative hemoglobin values, and uterin diameters (1 and 2) in both groups. However uterin diameter 3 and total uterin volume were smaller in laparoscopy group (p < 0.001 and p = 0.018, respectively). Abnormal uterin bleeding was seen the most common indication of hysterectomy in both groups. Perioperative blood transfusion and time of hospitalisation were similarly in both groups. However the operation time was significantly longer in LSK group than laparotomy (p < 0.001). We saw that usually laparotomic approach was preferred in patients with previous surgery. Conclusion: When the data are evaluated, we saw that laparoscopic hysterectomy cases were increased with the experience of our clinic. [ABSTRACT FROM AUTHOR]

Objective: To present our experience with laparoscopic donor nephrectomy (LDN), our complications and management modalities. Material and methods: Fifty-one transperitoneal LDNs performed in our clinic between the years 2011, and 2015, were evaluated retrospectively. Demographic characteristics of the patients, operative and postoperative data and complications were evaluated. Results: Nineteen female and 32 male patients with ages ranging from 24 to 65 years underwent left-(n=44), and right-sided (n=7) LDNs. Six patients had two, and one patient three renal arteries. Mean operation time was 115±11 (min-max: 90-150) minutes, and mean warm ischemia time 111±9 (min- max: 90-140 sec) seconds. Mean hospital stay was found to be 2.5±0.5 days. No patient needed to switch to open surgery. In one patient, lumbar vein was ruptured, and hemostatic control was achieved laparoscopically. Postoperative paralytic ileus developed in two patients. Three patients had postoperative atelectasis, and a febrile (38.1°C) episode. Conclusion: LDN is a minimally invasive method with advantages of short hospital stay, less analgesic requirement, and better cosmetic results. However it should be performed by surgeons with advanced laparoscopic experience. [ABSTRACT FROM AUTHOR]

Objective: Laparoscopic wedge resection is commonly used for gastric submucosal lesions. Early surgical and late oncologic results are similar to those of open surgery, although there are debates on limitations. In this study, we evaluated patients undergoing laparoscopic surgery for submucosal tumors. Method: Twenty-five patients, who had laparoscopic surgical resection for submucosal gastric lesions in Bakırköy Sadi Konuk hospital between 2012-2018, were retrospectively analyzed. Demographic characteristics, tumor size, location, and surgical treatment were recorded. The performed laparoscopic surgery technic is described. Results: Fourteen patients were female and 11 patients were male. The average age was 62.4. The mean diameter of the lesion was 46.7 mm. The largest tumor diameter was 80 mm. Three tumors were observed in the gastroesophageal junction, 1 in the cardia, 5 in the fundus, 10 in the corpus and 6 in the antrum. The mean operation duration was 46.7 minutes. Wound infections observed in 2 patients, and systemic infection in 1 patient. No mortality was recorded. Conclusion: Laparoscopic wedge resection for the submucosal gastric tumors were safe. Surgical methods that we use, large lesions, the lesions close to antrum or gastroesophageal junction can remove safely. [ABSTRACT FROM AUTHOR]

Objective: This was a prospective study to compare the outcome of laparoscopic transperitoneal ureterolithotomy (LTU) with laparoscopic retroperitoneal ureterolithotomy (LRU) as a primary treatment for a large stone in the proximal ureter. Material and methods: A total of 24 patients with a solitary, large (>1.5 cm), and impacted stone in the proximal ureter was selected and randomly divided into two groups. The first group included 13 patients who were treated by LTU, and the second group included 11 patients who were treated by LRU. Patient demographics and stone characteristics as well as the operative and postoperative data of both groups were compared and statistically analyzed. Results: There was no significant difference between the two groups regarding patient demographics and stone characteristics. The mean operative time was significantly shorter in the LTU group than in the LRU group [116.2±21.8 min vs 137.3±17.9 min, respectively (p=0.02)]. The mean time to oral intake was significantly longer in the LTU group than in the LRU group [21.2±4.9 h vs 15.5±2.8 h, respectively (p=0.002)]. There was significant higher rate (27.3%) of changing to open surgery in LRU (p=0.04). The stone-free rate was significantly higher in the LTU group than in the LRU group [100% vs. 72.8%, respectively (p=0.03)]. There was no statistically significant difference between the two groups regarding the mean blood loss, mean hospital stay, mean analgesia dose, blood transfusion rate, postoperative fever, and stone migration during surgery. Conclusion: Both approaches of laparoscopic ureterolithotomy are effective in treating large impacted stones in the proximal ureter. LTU has significantly shorter operative time and lower rate of open conversion but has a significantly longer time to oral intake. [ABSTRACT FROM AUTHOR]

Objective: To compare the results of transperitoneal laparoscopic nephrectomy in patients with atrophic and hydronephrotic kidneys. Material and methods: Clinical data were collected from 35 patients who had undergone laparoscopic nephrectomies for atrophic or hydronephrotic non-functioning kidneys between January 2010 and March 2014. Comparative analysis was carried out between the two groups examining demographic characteristics, imaging modalities, etiology, operative times, port numbers, conversion to open surgery, complications, pre- and post-operative hemoglobin and creatinine values, transfussion rates and length of hospital stays. Results: Laparoscopic nephrectomy was performed for atrophic kidneys in 20 (57%) patients and for hydronephrotic kidneys in 15 (42%) patients. In the atrophic group, 3 patients (15%) required transfusion because of bleeding but none of the patients required conversion to open surgery. In the hydronephrotic group one patient (6.6%) required transfusion and conversion to open surgery because of bleeding. Both of the groups were similar in terms of postoperative hospital stay but compared to the atrophic kidneys, hydronephrotic ones were associated with a longer total operative times (90.1 min vs. 73.6 min, p=0.03). Any serious complication (except for bleeding) and mortality were not encountered in both groups. The horseshoe kidney is the most frequent renal anomaly, with a prevalence of 0.25% and a male to female ratio of 2:1. In this article we aimed to report a 50-year-old man who had left kidney stones accompanied with a horseshoe kidney. In this case percutaneous nephrolithotomy was deemed to be a risky procedure due to malrotation of the pelviocalyceal system and possible interposition of bowel loops between kidney and the abdominal wall. Therefore, we preferred laparoscopic pyelolithotomy. At the end of the procedure, the patient was stone-free. We observed no complication. The patient was discharged after 72 hours. We assume that laparoscopic pyelolithotomy is a safe and effective approach for renal pelvic stone in case of horseshoe kidney. [ABSTRACT FROM AUTHOR]

Objective: To compare the surgical and functional outcomes of open, laparoscopic and robotic dismembered pyeloplasty for the treatment of patients with ureteropelvic junction obstruction (UPJO). Material and methods: Between 2007 and 2012, a total of 56 patients underwent conventional open (Group 1; n=25), laparoscopic (Group 2; n=16), and robotic (Group 3; n=15) dismembered pyeloplasty operations. Preoperative evaluation was performed using urinalysis, urine culture, blood biochemistry, urinary ultrasound, intravenous pyelogram (IVP) (optional) and Mercaptoacetyltriglycine (MAG-3) renal scan. The mean operation time, estimated blood loss, drain removal time, narcotic analgesic requirements, length of hospital stay and functional outcomes were compared among groups. Statistical analyses were performed using SPSS Statistics v. 20 (IBM, Armonk, NY, USA) software, and statistically significant differences were determined using a p value <0.05. Results: The mean age of the patients was 30 years in Group 1, 34.3 years in Group 2 and 32.9 years in Group 3. The mean operation time was 127, 130 and 114 min (p=0.32), and the estimated blood loss was 105, 31 and 28 mL, respectively (p=0.001). The drain was removed after 4.36 (±1.3), 2.33 (±0.6) and 1.8 (±0.6) days after surgery (p<0.001), and the mean hospital stay was 4.14 (±1.8), 2.8 (±0.75) and 2 (±1) days, respectively (p<0.001). Narcotic analgesic requirement was significantly higher in Group 1 compared with Groups 2 and 3 (p=0.02). The radiographic and symptomatic success rates were 96% in Group 1, 93.75% in Group 2 and 93.3% in Group 3. Conclusion: Laparoscopic and robotic pyeloplasty are feasible, effective, reliable and minimally invasive treatment approaches for the treatment of UPJO as compared with open dismembered pyeloplasty.
